Introduction: Inguinal hernia is one of the most common urgent problems encountered by surgeons. Material-Methods: Patients, who underwent surgery due to inguinal hernia diagnosis between February 1999 and February 2016, were investigated retrospectively. There were 1345 patients treated due to inguinal hernia diagnosis. Out of 250 patients who were operated under emergency conditions, 171 patients were operated within the first nine years and 79 patients were operated within the last eight years. 131 patients (106 male and 25 female) were treated with unilateral open preperitoneal mesh repair. Patients treated with unilateral preperitoneal mesh repair were assessed in two groups according to surgery conditions such as those with preperitoneal hernia repair under emergency conditions (group 1) and those with preperitoneal hernia repair under elective conditions (group 2). Results: There were total of 124 patients included in the study; in which 54 of them (37 male and 17 female, range of age: 43-92) (group 1) were operated under emergency conditions and 70 of them (61 male and 9 female, range of age: 21-88) (group 2) were operated under elective conditions. The mean age was 67 in group 1 and 58 in group 2. There was comorbidity in 28 patients of group 1 and 16 patients of group 2. 27.7% (n=15) of patients who underwent emergent surgery due to incarceration, while 38.5% (n=27) of patients who were operated under elective conditions had been operated due to hernia on the same side before. There were 10 femoral hernias in group 1 and 2 femoral hernia in group 2. Conclusion: There is no difference between postoperative complication and recurrence rates in inguinal hernia patients operated under elective conditions and inguinal hernia patients operated under emergency conditions with open preperitoneal mesh repair technique. Successful results were obtained even in the presence of surgical site contamination. It is noteworthy for open anterior preperitoneal repair that surgery can be finished with a single incision especially in cases accompanying strangulation. It is cheaper than laparoscopic repair, leads pain control and lack of neuropathic pain, has less testicular complications, and it is more advantageous than other open techniques (such as Lichtenstein).

Aim: The aim of the present study was to examine the factors associated with mortality in patients who underwent splenectomy for treatment of hematological diseases. Materials and Methods: This descriptive study consisted of a retrospective evaluation of the hospital records of 48 patients who underwent splenectomy for treatment of hematological disease between January 2005 - November 2008 in the General Surgery Department of Dicle University, Faculty of Medicine. Results: Of the 48 patients included in the study, 56.2% (27 patients) were female and the mean age was 36.65 ± 15.58 years. 60.4% of the patients (N=29) had Chronic ITP and the mean hospitalization time was 7.31 ± 3.45 days. There was a statistically significant difference between the pre- and post-operative leukocyte and thrombocyte counts (p<0.001). 22.9% (N=11) of the patients had been given blood transfusions before the operation and 29.1% of the patients (N=14) had been given blood transfu-sions after the operation. Complications arose in 62.5% of the patients and mortality was not observed in any of the patients during hospitalization. Conclusion: Similar to other reports in the literature, splenectomy was determined to be safe in terms of mortality.

Introduction: In the past, hyperthyroidism was thought to be protective against thyroid cancer. However, the relationship be-tween thyroid stimulating hormone (TSH) and cancer has become more prominent within the last 15 years. In addition, surgical techniques related to thyroid cancer have also seen many changes over the past several years. In the current study, we review these changes in surgical techniques, and examine the relationship between thyroid functions and cancer development. Materials and methods: We retrospectively evaluated the records of 239 patients who underwent thyroidectomy in the General Surgery Department of Istanbul Medeniyet University Faculty of Medicine between January 1999 and January 2014. Patients who underwent thyroidectomy between 1999 and 2006 were categorized in subgroup I, and those who underwent the surgery between 2006 and 2014 were categorized in subgroup II. Data from 2006 were included in both groups. There were 157 patients who had preoperative serum TSH levels that were either normal (0.27-4.20 μIU/mL) or high (>4.20 μIU/mL); these patients were categorized in "subgroup I for TSH". There were 82 patients who had low preoperative serum TSH levels; these were categorized in "subgroup II for TSH". Results: Of the cases with thyroid cancer, 84.9% were women, 15.1% were men, and their mean age was 46.62±13.65 (16-80) years. Of the cases with thyroid cancer, 54.4% had papillary carcinoma, 37.2% had micropapillary carcinoma, 4.6% had follicular carcinoma, 3% had medullary carcinoma, and 0.8% had anaplastic carcinoma. When operation types were evaluated according to the years in which they were performed (1999-2014), the total thyroidectomy rate was significantly higher in Group II (2006-2014) than in Group I (1999-2006, p<0.05). The bilateral subtotal thyroidectomy rate was significantly higher in Group I (1999-2-6, p<0.05). When we examined pathology results with respect to TSH levels, there was no significant association between TSH levels and papillary carcinoma or other carcinoma types (p>0.05). On the other hand, micropapillary carcinoma was signifi-cantly more frequent among patients with low TSH levels (p=0.003). Conclusion: The gold standard for the treatment of thyroid cancer is surgical resection, and currently, the most preferred opera-tion type is total or near-total thyroidectomy; this is due to its advantages related to recurrence and survival, as well as low complication rates.
