Should the visceral peritoneum be closed over mesh in abdominal sacrocolpopexy?

    Kulhan, M.; Kulhan, N.G.; Ata, N.; Nayki, U.A.; Nayki, C.; Ulug, P.; Yilmaz, N.

Introduction and hypothesis: Peritonisation of mesh during Abdominal sacrocolpopexy is generally advocated to prevent adhesions to the viscera; however, randomized clinical trials are lacking. In this study; we aimed to investigate whether the mesh peritonisation is clinically significant or not. Material method: Thirty-four patients who were operated for the reason of pelvic organ prolapse were included in the study. Patients were divided into two groups by retrospective scanning from the files and surgical reports. Group 1 patients consisted of those who underwent peritonisation and group 2 patients consisted of those who did not in abdominal sacrocolpopexy. Results: Operative time and the amount of blood lost were statistically less in the group 2. Postoperative pain and analgesic drug requirements were obviously higher in the group 1. Postoperative De novo dyspareunia and urinary urgency were higher in the group 1. There were no statistical differences between the groups in terms of other complications. Conclusion: We noticed that there was no difference between the patients who were peritonized and those who were not in terms of postoperative complications.

Does synchronous endometrioid endometrial cancer have any prognostic effect on Stage i endometrioid ovarian cancer?

    Narin, M.A.; Karalok, A.; Basaran, D.; Ureyen, I.; Turkmen, O.; Turan, T.; Tulunay, G.

Objective To determine the effect of synchronous endometrial endometrioid cancer (SEEC) on the prognosis of patients with Stage 1 endometrioid ovarian cancer (EOC). Study design Clinicopathological data of cases with Stage 1 EOC from January 2000 to November 2013 were retrieved from the computerized database of Etlik Zubeyde Hanim Women's Health and Research Hospital. Of the 31 patients included in the study, 15 patients had primary synchronous endometrial and ovarian cancer (SEOC) (Group 1) and 16 patients had EOC alone (Group 2). Results Ovarian cancer substage and grade were compared between the two groups, and no significant differences were found. Most of the patients with SEEC had Grade 1 tumours (n = 13, 86.7%). In Group 1, nine (60.0%) patients had endometrial tumours with superficial myometrial invasion, and six (40.0%) patients had deep myometrial invasion. Median follow-up was 94 months. Ten-year disease-free survival rates were 92.9% for Group 1 and 84.6% for Group 2 (p = 0.565). Conclusion Patients with Stage 1 EOC have excellent long-term survival. The presence of SEEC does not influence the prognosis of patients with Stage 1 EOC, even in the presence of deep myometrial invasion.

Evaluation of the effects of single or multiple dose methotrexate administration, salpingectomy on ovarian reserve of rat with the measurement of anti-Müllerian hormone (AMH) levels and histological analysis

    Ulug, Pasa; Oner, Gokalp

Study design: In this prospective controlled experimental study, forty female Wistar-albino rats were randomly divided into four groups; healthy control group (group 1), one month after single dose (50 mg/ m2) MTX administration group (group 2), multiple (1 mg/kg, 1, 3, 5, 7 days) dose MTX administration group (group 3), and unilateral salpingectomy applied group (group 4). Ovarian reserves were assessed with anti-Mu llerian hormone (AMH) and ovarian follicles including primordial, primary, secondary, tertiary and total follicle counts.; Setting: Kobay and Duzen experimental research laboratory.; Results: Serum AMH levels decreased significantly in the single and multiple dose MTX administration group and salpingectomy group compared with control group. Primordial follicle counts were comparable between single dose MTX group and control group whereas all type of follicle counts in single and multiple dose MTX and salpingectomy group were significantly reduced compared with control. The least affected ovarian reserve in the treatment groups was single dose MTX group.; Conclusions: Ovarian reserve markers including follicle counts and AMH levels were decreased after single, multiple dose MTX administration, and salpingectomy. Single dose MTX was the least detrimental method on ovarian reserve for the treatment of ectopic pregnancy.; Objective: To evaluate the effects of single, multiple dose methotrexate (MTX) administration and salpingectomy on ovarian reserve in a rat model.
