A possible interaction o TIMP-1 and TSP-1 with familial mediterranean fever

Yildirim, Malik Ejder; Kurtulgan, Hande Kucuk; Kiliçgün, Hasan; Bakir, Deniz; Ersan, Sepil

Aim: Matrix metalloproteinases (MMPs) may influence many biological and pathological processes including inflammatory responses. Thrombospondins (TSP) are a glycoprotein group of the extracellular matrix. In this study, we aimed to investigate a possible association of TIMP-1 (an inhibitor of metalloproteinases) and TSP-1 (an adhesive protein involved in cell-matrix interactions) with familial Mediterranean fever. Material and Method: Thirty FMF patients who had compound heterozygous or homozygous MEFV mutations and thirty healthy controls were included in this study. The patients were selected after screening by reverse hybridization procedure. TIMP-1 and TSP-1 levels of the patients and controls were measured by ELIZA. Results: TIMP-1 and TSP-1 levels of the patients who had homozygous or compound heterozygous MEFV mutation were compared with 30 healthy controls. The levels of TIMP-1 in the patients were statistically higher than those in the control group (p < 0.001). There was no significant difference between the patients and controls in terms of TSP-1 levels (p = 0.84). Discussion: IL-1 beta has an important role in FMF disease and it may stimulate expression of TIMP-1. TIMP-1 levels are increased in FMF patients on the basis of inflammation and higher TIMP-1 levels in patients may be associated with the self-limited nature of FMF. TSP-1 level can be modulated by proinflammatory cytokines but there was no any significant difference between TSP-1 levels of FMF patients and the control group in our study. It can be thought that there is no interaction between TSP-1 and the pathogenesis of FMF.

Aim: The aim of this study was to Investigate whether the endoscopic findings, corona! computed tomography data and the level of histopathological changes observed in tissue samples had any effects on the quality of life in chronic rhinosinusitis cases. Material and Method: In this prospective study, 34 patients diagnosed with chronic rhinosinusitis (Group 1) and 34 volunteer individuals having no chronic rhinosinusitis (Group 2) were included. Detailed anamnesis, endoscopic examination findings, quality of life in patients with chronic rhinosinusitis questionnaire scores and coronal paranasal computed tomography scores were recorded in the files of all the cases. Endoscopic evaluation findings were evaluated using the Lund-Kennedy scoring system, all the cases were given a chronic rhinosinusitis evaluation questionnaire, coronal paranasal computed tomography findings were scored using the Lund-MacKay system. Results: The average quality-of-life score, the average radiological score, the average endoscopic examination score, the histopathological evaluation of the surgical specimens, the average inflammatory score, the average fibrosis scores and the average osteoblastic-osteoclastic score was statistically different between two groups. When a correlation analysis was performed, the quality-of-life scores were found to have a negative correlation with the radiological scores, endoscopic scores. fibrosis scores and osteoblastic-osteoclastic activity scores in Group 1. And also radiological scores were positively correlated both with the endoscopic scores and the osteoblastic-osteoclastic activity scores and a positive correlation was found between the endoscopic scoring and the mucosal inflammation in Group 1. Discussion: The results of our study showed that the histopathological changes affected the quality of life negatively.

Comparison of the effects of propofol plus fentanyl with midazolam plus fentanyl on pr, qtc interval and qt dispersion in patients undergoing coronary artery surgery; [Koroner arter cerrahisi planlanan hastalarda propofol-fentanil ile midazolam-fentanil’in pr, qtc aralığı ve qt dispersiyonu üzerindeki etkilerinin karşılaştırılması]

Onk, Didem; Onk, Oruç Alper; Değirmenci, Hüsnü; Küçüksu, Zafer; Bedir, Zehra; Kuyrukluyıldız, Ufuk; Ayazoğlu, Tülin Akarsu

Aim: To compare the effects of propofol and midazolam on markers of myocardial homogeneity when used during anesthesia induction in patients undergoing coronary artery bypass grafting. Material and Method: A total of 50 patients with normal left ventricular systolic function and normal electrocardiogram were randomized into two groups as to receive propofol Group 1 (n=25, mean age: 68.28±10.44) or midazolam (n=25, mean age: 67.88±8.90) during induction of anesthesia. Standard 12-lead electrocardiograms were obtained just before induction of anesthesia to ensure baseline compara-bility of two groups. Patients were monitored with an ambulatory electrocardiogram before and after surgery, and individual average QT duration, QTc duration, P dispersion and QT dispersion were calculated. Results: Dis-tribution of baseline characteristics were similar between two groups. Two groups were also similar regarding preoperative QT duration, QTc duration, QT dispersion and p-wave dispersion. Mean of individual average QT duration, QTc duration, and P dispersion did not differ significantly between two groups whereas mean of individual average QT dispersion was significantly higher in propofol group than that in midazolam group in the postoperative period (32.80±13.07 vs. 24.60±9.34, in Group 1 and 2, respectively, p=0.01). Discussion: Midazolam seems to have minimal effect on the electrical stabil-ity of the myocardium, whereas propofol is likely to limit recovery of myocardial homogeneity after CABG to some extent when used during induction of anesthesia.

    Tubal Reanastamoz Sonrası Fertilite Sonuçları

Arlier, Sefa; Işıl Adıgüzel, Fikriye; Seyfettinoglu, Sevtap; Narin, Mehmet Ali; Nazik, Hakan; Adıgüzel, Cevdet

Aim: The aim of this study was to reveal the reasons why women request reversal of tubal sterilisation and to compare surgical techniques in terms of fertility outcomes. Material and Method: This retrospective study included 36 patients, who underwent tubal reanastomosis surgery in our clinic between January 2008- June 2015. Analysis was made of laparoscopic tubal reanastomosis applied to 7 patients and of reanastomosis applied with laparotomy to 29 patients. For each patient, data were recorded on age at the time of sterilization, age at the time of reversal, body mass index, smoking use, alcohol use, pelvic surgery history, subfertility, additional diseases, known male infertility, demographics, hormone profiles, hysterosalpingograhy (HSG) results, applied sterilization methods, reversal methods, and the reasons for requesting tubal reversal. The fertility outcomes were evaluated. Results: At the end of mean 2 years follow-up of patients, in the laparotomy group, 20 (69%) patients had not achieved pregnancy, 4 (13.8%) had abortus, 1 (3.4%) had an ectopic pregnancy and 4 (13.8%) patients had live births. In the laparoscopic group, pregnancy was not achieved in 5 patients (71.4%), 1 (%) was complicated with ectopic pregnancy, and 1 14.3%) had a live birth. Discussion: Before reversal of tubal sterilisation, a detailed patient history should be taken, the data of the sterilization operation should be accessed, and laparoscopic surgery must be presented to the patient as an option. When considering the probability of pregnancy, the patients's age, BMI, additional diseases, and other factors that affect fertility must be taken into consideration. Although the number of cases was limited, the results of this study support the view that laparoscopic tubal reanastomosis is a safe and effective procedure.

Doz Bağımlı Olarak C Vitamininin Doku Koruyucu Etkilerinin Araştırılması

    Kılıçgün, Hasan

Aim: Vitamin C is an excellent antioxidant in biological systems. However, It is also widely used as a pro-oxidant. This paradoxical behavior results because it is an excellent reducing agent. As for carbon tetrachloride, the incidence of carbon tetrachloride-induced hepatotoxicity is reported to be on the increase. In this study, it is aimed to test, whether additional intake of different concentrations of vitamin C improves antioxidative protection on lipid peroxidation, alanine transaminase, aspartate transaminase, glutathione and protein oxidation levels in single dose carbon tetrachloride-treated male Wistar rats. Material and Method: Two control groups and four treatment groups totaly sixty Wistar albino rats were used in this study. The control groups were fed via oral route a placebo (4 ml of distilled water), while test groups 100 mg /kg, 200 mg/kg, 400 mg/kg and 800 mg/kg body weight of vitamin C in 4ml of distilled water was given via gastric intubation. Results: The administration of vitamin C for 40 days produced a significant decrease with increasing concentrations from 100 to 400 mg/kg of vitamin C in differnt parameters carbon tetrachloride-treated rats. But, the same protection was not observed at higher concentration of vitamin C. Moreover, it has showed hazardous activity at 800 mg/kg concentration as much as carbon tetrachloride-induced toxicity. Discussion: These results showed that vitamin C had not the potential to be used as tissue protective agent at higher doses. Also, results suggest that the same substannces that optimizes antioxidant capacity may also act as prooxidant dependent on its concentration.
