Emir, İzzet; Coşgun, Muharrem Said

MINOCA syndrome is a background myocardial infarction with non‐obstructive coronary artery. Epicardial coronary artery disease, epi‐\rcardial spasm, microvascular spasm, microvascular thrombosis, microvascular embolism or spontaneous coronary artery dissection in\rthe etiopathogenesis. Diagnosis is made by the presence of findings consistent with acute myocardial infarction, detection of non‐ob‐\rstructive coronary arteries, and exclusion of specific alternative diagnoses. Treatment is done according to etiological reasons. In this\rsyndrome, the mortality rate is higher at 1‐year follow‐up. In this review, attention was drawn to the MINOCA syndrome.
RDL Kalıcı Kaydıİncele - ArticleYalnız künye2022Effect of Anxiety on Blood Pressure in Hypertensive COVID-19 Patients
Coşgun, Muharrem Said
Objective: Hypertension is the most common comorbidity accompanying COVID‐19. In addition, a history of hypertension has been cor‐\rrelated with a poor prognosis. This study aimed to investigate the effect of significant anxiety on blood pressure regulation in hyper‐\rtensive patients during the COVID‐19 outbreak.\rMaterial and Method: A total of 218 patients with a confirmed diagnosis of COVID‐19 were included in the study. The mean age of the\rpatients was 63±9 years, and 53.2% were female. Patients with a history of uncontrolled hypertension or irregular drug use during hos‐\rpitalization were excluded from the study. Anxiety levels of all patients were determined using the Beck Anxiety Scale (cut‐off point\r≥10). During the isolated ward follow‐up of the patients, blood pressure values were monitored. Additional antihypertensive drug re‐\rquirements were recorded.\rResults: Anxiety is a prevalent symptom in hypertensive patients followed in isolated wards due to COVID‐19 (39.4%). In addition, anx‐\riety is more common in the female gender (61.6% vs. 47.7%; p = 0.044). The need for additional antihypertensive medication increases\rapproximately twofold in patients with anxiety (15.9% vs. 29.1%; p = 0.020). The length of hospital stay was also meaningfully longer\rin the group with significant anxiety (6.0 ± 2.9 days vs. 7.1 ± 3.2 days; p = 0.034). However, anxiety did not affect prognostic conditions\rsuch as intensive care unit referral and mortality.\rConclusion: Anxiety is a common psychological disorder in patients treated for COVID‐19. In addition, anxiety is directly related to blood\rpressure regulation. In the management of the COVID‐19 outbreak, psychological support treatment in addition to medical treatment\rmay be beneficial, especially in patients with comorbidities such as hypertension.

RDL Kalıcı Kaydıİncele - Review ArticleYalnız künye2019Approach to A to Z Radial Artery Intervention
Buyuklu, Mutlu; Değirmenci, Hüsnü
Transradial access was first used in 1989 for diagnostic coronary angiography. With further improvements in the technique, it has gained\rwide popularity in percutaneous transluminal coronary angioplasty and percutaneous coronary interventions including stenting. When\rperformed with appropriate indications, the transradial approach is a preferable technique for coronary interventions due to lower rate\rof bleeding complications, increased patient comfort, shorter hospital stay and follow-up period, and decreased workload.

Değirmenci, Hüsnü; Bakirci, Eftal Murat
Stent stripping is a potentially serious complication of percutaneous vascular interventions. Various devices and techniques can be used for the retrieval procedure. There is no proven standard method for the retrieval of embolized stent. Different techniques and devices can be used depending on the creativity of the operator as well as the individual situation of the case. The goal of this review is to pro- vide information on the mechanisms and risk factors of stent stripping and to describe some simple and some more advanced retrieval equipment and techniques.
RDL Kalıcı Kaydıİncele - ArticleYalnız künye2018A Dramatic Complication: Iatrogenic Radial Artery Perforation
Buyuklu, Mutlu; Değirmenci, Hüsnü; Bakirci, Eftal Murat; Hamur, Hikmet
A 75 years old woman patient was admitted with a non-ST-segment elevation myocardial infarction. After insertion of a 6F radial sheath into the right radial artery and intra-arterial administration of heparin plus verapamil, 0.035 inch guidewire and catheter could not be advanced under fluoroscopic guidance. Immediately afterwards, radial angiography was performed, which displayed a radial artery per- foration with significant contrast extravasation. Held in place for 20 minutes with application of external compression with a sphygmo- manometer cuff at the level of systolic blood pressure. We used balloon assisted tracking and guide catheter tamponade tecnique to complete the angioplasty procedure via the radial artery and successfully seal the radial artery perforation. We describe a case where this technique was used to both treat radial artery perforation and to complete the angioplasty via the radial approach.
