Importance of parental anxiety in management of developmental breast conditions in children A study with a prospective hierarchical regression model

    Özdamar, Mustafa Yaşar; Biçer, Şenol

The parent is the most critical link and decision-maker between the patient and the healthcare provider in treating many pediatric diseases. This entity is essential for the management of pediatric breast diseases for which the rate of surgical intervention is known to be very low. Although previous publications have emphasized that pediatric breast diseases may cause alarming anxiety in parents, the demographic factors that influence this anxiety have not been investigated. Even if practitioners complete patient management with appropriate procedures, treatment is incomplete if the questions remain unanswered. In this observational prospective study, we investigated the demographic factors that affect parental anxiety, which should be prioritized to prevent incomplete management. The Beck Anxiety Inventory score (BAS) created by the parents of 409 boys and girls aged 0 to 17 with breast conditions was recorded at the diagnosis, termination of treatment, and final control stages. A 2-stage hierarchical logistic regression model was applied to show how strongly the demographic characteristics of parents and their children predicted the parental BAS. Of the demographic characteristics, there was a significant correlation (P < .05) between the patient’s sex, age, developmental period, Tanner stage, referral status, management method, family’s place of residence, economic distress, and BAS. However, according to the 2-stage hierarchical regression model, only 3 demographic characteristics, the patient’s gender, place of residence, and method used in patient management, significantly predicted BAS (P < .05, ΔR2 = .35). Among the many factors that affect anxiety experienced by parents whose son or daughter has breast problems, the gender of the child, place of residence of the family, and management methods used by the practitioner are demographic characteristics that should be taken into consideration.

Effect of anakinra, tocilizumab, and the combination thereof on bladder ischemia-reperfusion damage in albino Wistar-type rats; [Efecto de la anakinra, el tocilizumab y la combinación de ambos sobre el daño por isquemia-reperfusión vesical en ratas albinas tipo Wistar]

Bicer, Senol; Suleyman, Bahadir; Mammadov, Renad; Yavuzer, Bulent; Cicek, Betul; Altuner, Durdu; Coban, Taha A.; Suleyman, Halis

Several studies have reported that oxidative stress, and proinflam-matory cytokines such as tumor necrosis factor-alpha (TNF-α), interleukin-one beta (IL-1β), and interleukin-six (IL-6) are the main factors underlying bladder ischemia-reperfusion (I/R) damage. Anakinra and tocilizumab are known to be antioxidants and proinflammatory cytokine inhibitors. Our study aims to investigate if anakinra, tocilizumab, and the combination (ATC) thereof have a protective effect against oxidative and inflammatory bladder damage induced through the I/R procedure in rats, and evaluate by comparing these compounds. Male rats were divided into five groups: bladder sham-operation applied group (SG); bladder only I/R applied group (IRG); anakinra+bladder I/R applied group (AIR); tocilizumab+bladder I/R applied group (TIR); and ATC+bladder I/R applied group (ATIR). An atraumatic clamp was placed on the abdominal aorta of animals in all groups (except SG), and one hour of ischemia followed by two hours of reperfusion was performed. Our biochemical find-ings showed that anakinra and tocilizumab significantly inhibited the increase of oxidant malondialdehyde (MDA) and the decrease of antioxidants such as total glu-tathione (tGSH), superoxide dismutase (SOD), and catalase (CAT) in bladder tissue by I/R, both at the same levels. Furthermore, anakinra and tocilizumab significantly suppressed the I/R-associated increase of TNF-α, IL-1β, and IL-6 in bladder tissue. ATC was the one that best prevented the I/R-related increase in MDA, TNF-α, IL-1β, and IL-6 and the decrease in tGSH, SOD, and CAT in the bladder tissue. ATC was more beneficial than anakinra or tocilizumab alone in treating bladder I/R damage.

Topal, Ismail; Özdamar, Mustafa Yaşar; Catakli, Tulin; Malkoc, İsmail; Hacimuftuoglu, Ahmet; Mamoulakis, Charalampos; Tsatsakis, Aristidis; Tsarouhas, Konstantinos; Tsitsimpikou, Christina; Taghizadehghalehjoughi, Ali

Background: Taxifolin (TXF) is a flavonoid found abundantly in citrus/onion. Encouraging results on its renoprotective effect have been reported in a limited number of drug-induced nephrotoxicity animal models. The present study aimed to evaluate for the first time the potential renoprotective effects of TXF in a paracetamol (PAR)-induced nephrotoxicity rat model. Methods: Rats were divided into three equal groups (n = 6 animals per group). Group 1 (PAR group, PARG) received PAR diluted in normal saline by gavage (1000 mg/kg). Group 2 (TXF group, TXFG) received TXF diluted in normal saline by gavage (50 mg/kg) one hour after PAR administration. Group 3 (control group, CG) received normal saline. Twenty-four hours after PAR administration, all animals were sacrificed using high-dose anesthesia. Blood samples were collected and kidneys were removed. Results: The serum blood urea nitrogen, creatinine levels and serum malondialdehyde levels were significantly increased in the PARG. The serum glutathione peroxidase, glutathione reductase and total glutathione levels were significantly higher in the TXFG. At the same time, the kidneys of the PARG animals demonstrated tubular epithelium swelling, distension and severe vacuolar degeneration. The kidneys of the TXFG animals showed mildly dilated/congested blood vessels. Conclusions: The TXF renoprotective effects are promising in preventing PAR-induced nephrotoxicity, mainly through antioxidant activity, and warrant further testing in future studies.

The preventive role of medical ozone therapy on remote organ injury induced by hepatic ischemia reperfusion

Akbas, Emin Murat; Gürsul, Cebrail; Sayar, Ilyas; Gürbüzel, Mehmet; Biçer, Şenol; Demirtas, Levent; Gurbuzel, Ahmet

Aim: This study explores the effects of ozone therapy on the prevention of remote organ injury in rats with induced hepatic ischemiareperfusion injury. Materials and Methods: Twenty-one male Sprague Dawley rats were sorted randomly into three groups: (a) A control group in which the rats’ abdomens were operated on by laparotomy, and their livers then viewed and closed with a suture; (b) Rats with induced hepatic ischemia-reperfusion injuries; and (c) Rats induced with the same injuries that were administered ozone therapy before reperfusion. The rats’ hepatic arteries, portal veins, and bile ducts were clamped, and ischemia was induced for 45 min. Reperfusion was provided for 90 min in the second and third groups. The third group was administered 0.5 mg/kg ozone intraperitoneally before reperfusion. Liver, lung, and kidney tissues were taken for biochemical and histopathological examinations. Results: In the second group, malondialdehyde levels in the lung and kidney tissues were higher than in the control group, while catalase and superoxide dismutase enzyme levels were lower. The third group had lower malondialdehyde levels and higher catalase and superoxide dismutase enzyme levels than the group that did not receive the therapy. In histopathological evaluations, the damage to both kidney and lung tissues in the third group was higher than in the control group; however, this damage was significantly lower than in the second group. Conclusions: The findings indicated that the administration of ozone therapy attenuated lung and kidney injuries caused by hepatic ischemia, since partial protection was demonstrated in many instances.

Acute mesenteric lymphadenitis in children: Findings related to differential diagnosis and hospitalization

    Ozdamar, Mustafa Yasar; Karavas, Erdal

Introduction: Diagnostic discrimination between acute appendicitis (AA) and mesenteric lymphadenitis (AML) may require more diagnostic tests or great skill after excluding other diagnoses. This study aimed to make a differential diagnosis between AA and AML patients with previous and new parameters and to examine which parameters should be taken into account regarding whether the AML patient should be hospitalized or discharged. Material and methods: One hundred and twenty-three AML and 134 AA patients, randomly selected, were included in the study. Demographic, clinical, and laboratory data of all subjects were analyzed. Ultrasonographic and rarely computed tomography examinations evaluating for the enlarged lymph nodes with the shortest diameter in the right lower quadrant of the AML patients were performed. Also, the erect abdominal radiographs (EAR) of AML and AA patients were evaluated. Results: While there was no statistically significant difference in age or gender, C-reactive protein, white blood cell count, monocyte percentage as well as symptoms between the AA and AML groups (p > 0.05), neutrophil and lymphocyte percentage, appearance of EAR and L/M ratio were significantly different between the two groups (p < 0.05). There was no correlation between the short-axis diameter of the mesenteric lymph node and clinical and laboratory findings in the AML group (p > 0.05). Conclusions: Based on the results with ROC curve analysis, we propose to take into account the L/M ratio and the EAR from the parameters in the differential diagnosis between AA and AML in addition to previously reported parameters. Also, we recommend that the same features may be used to decide whether AML patients under observation are hospitalized or not.

Detection of insulin-like growth factor receptor-1 in the human cremaster muscle and its role in the etiology of the undescended testis

Özdamar, Mustafa Yaşar; Şahin, Sevinç; Zengin, Kürşad; Seçkin, Selda; Gürdal, Mesut

Purpose: Previous studies of undescended testis (UT) has focused on insulin-like hormone 3 (INSL3), the genitofemoral nerve, and androgens in the testicular descent. Leydig cells, which are under the control of insulin-like growth factor-1 (IGF1), produce both androgens and INSL3. We aimed to investigate whether insulin-like growth factor receptor-1(IGFR1) exists in the cremaster muscle (CM) complex and is associated with normally descended testis as well as UT cases in humans. Methods: We studied 30 CM from 15 patients who comprised the UT group (UTG), and 15 patients with unilateral testicular torsion (Control group; CG). Muscles, nerves, and vessels within the CM specimen were examined to determine the presence of IGFR1. Results: The mean staining score (MSS) of IGFR1 in CM and its nerves were higher in the CG than in the UTG. These results were statistically significant (p = 0.01 and p = 0.02). Although the MSS of IGF1R was higher in the vessels of CM in the CG than the UTG, this was not statistically significant (p = 0.48). Conclusions: IGFR1 with heterotetrameric receptor via IGF1, IGF2, insulin, and probably androgen, contribute to the remodeling and development of CM as well as the testis descent. In the current study, the presence of the IGFR1 in the CM was shown. Additionally, the IGFR1 density of the CM was lower in the UT cases than in the CG cases. Further evaluation of IGFR1 and other etiological factors can elucidate how they interact.

Role of ozone therapy in preventing testicular damage in an experimental cryptorchid rat model

Biçer, Şenol; Gürsul, Cebrail; Sayar, İlyas; Akman, Orhan; Çakarlı, Seçil; Aydın, Merve

Background: Cryptorchidism is the most common developmental abnormality of the male reproductive system. If left untreated, it results with infertility and testicular cancer. According to current evidence, surgery is the mainstay of treatment, and hormonal therapy approaches are still under investigation. For the protection of testicular functions, antioxidants have emerged as novel options. This study aimed to evaluate the protective properties of ozone, a strong antioxidant, on testicular tissue. Material/Methods: Thirty-five male Wistar-albino rats, 1-month-old, were used for the study. Groups were formed as follows: 1) control, 2) sham surgery (cryptorchidism), 3) cryptorchidism plus ozone, 4) cryptorchidism plus human chorionic gonadotropin (hCG), and 5) ozone plus hCG. Surgical procedures were performed on all rats except the control group. All rats except the control group were used to create an experimental cryptorchidism model, and left testes of animals were surgically placed into the abdomen. After 1 month of surgery, groups 3, 4, and 5 were given corresponding treatments intraperitoneally for 4 weeks. At the end of the study period, testicular atrophy index (TAI) and testicular sperm motility (TSM) were assessed and biochemical, histopathological, and immunohistochemical tests were performed. Results: TAI and TSM were higher in the ozone, hCG, and ozone plus hCG groups than in the sham surgery group (p=0.001). TSM in the ozone group was significantly higher than in the hCG and ozone plus hCG groups. In biochemical analyses, the parameters of oxidative stress (GPx1, MDA, CAT, GSH, SOD) indicated increased oxidative activity in cryptorchidism, which was resolved by applying ozone and hCG (p=0.001). In addition, apoptotic markers, Caspase 3 and bcl-2 were significantly decreased by applying ozone and hCG (p=0.001). Conclusions: Results of this study suggest that ozone therapy, either as a single agent or in combination with hCG, is a promising approach for protection of testicular functions.

Contribution of ultrasound-guided ilioinguinal/iliohypogastric block to intraoperative anesthesia in children - Randomized controlled study

    Kupeli, Ilke; Bicer, Senol

Objectives: The aim of this study was to investigate the effect of ilioinguinal/iliohypogastric nerve block (II/IH) only supported by ketamine sedation on intraoperative anesthesia and analgesia. Methods: A total of 60 patients aged between 2 and 6 years, were included in the study. The patients were divided into three groups: 1) the II/IH block and ketamine group, 2) the II/IH block by inserting the laryngeal mask (LMA) and sevoflurane group, and 3) the LMA and sevoflurane group. Peroperative heart rate (HR), mean arterial pressure (MAP), amount of sevoflurane used, postoperative pain scores, number of children needing rescue analgesia, and time required for the first analgesia were recorded. Results: The patients, consisting of 34 males and 26 females had an average age of 4 ± 1.3 years. There was no difference in terms of HR and MAP between the ketamine + block applied group and LMA + block applied group; however, HRs were higher and MAPs were lower in the group in which no block was added but only sevoflurane maintenance was provided and the amount of sevoflurane used significantly decreased in the group supported by (II/IH) block (P < 005). In the postoperative period, the HR, MAP and pain scores were higher in the group with no added block but this difference was significant in the first 6 hours (P < 0.05). Rescue analgesia was applied to a fewer of patients in the groups with added blocks and nausea-vomiting occurred only in the group without block. It was observed that analgesia was needed in average for 6.1 hours in the block group to which ketamine was applied; 4.7 hours in average in the block group to which LMA was applied; and for 2.1 hours in average in the group not supported by block (P < 0.05). Conclusion: In this study, it was determined that II/IH block added to general anesthesia or sedation in pediatric lower abdominal surgery reduced the need for intraoperative anesthesia, provided a more stable intraoperative hemodynamics and analgesia, and provided less pain scores, longer analgesia duration, and shorter analgesia need in the postoperative period.

Testicular ischemia caused by incarcerated inguinal hernia in infants: Incidence, conservative treatment procedure, and follow-up

Ozdamar, Mustafa Yasar; Karakus, Osman Zeki

Purpose: Testicular ischemia and necrosis, especially in the infant age, may result from incarcerated inguinal hernia. Duration of ischemia is a significant factor for the affected testicle. We aimed to present a case series on the conservative management in the testicular ischemia caused by incarcerated inguinal hernia. Materials and Methods: Inguinal hernia repairs performed in between March 2009 and December 2014 were investigated retrospectively. Patients' characteristics, hernia side, incarceration, testicular ischemia and complications were recorded. Color Doppler ultrasonography was performed in the incarcerated inguinal hernia patients preoperatively and was repeated on 3 and 7 days and then at 1, 3 and 6 months postoperatively. The testicle sizes, volumes, and arterial flow patterns of them were recorded at the same time. Results: Total 785 inguinal hernias were treated in 738 male patients, ranging from 18 days to 16 years. From all male patients, 44 (5.9%) had the IIH. There were 16 (36.3%) irreducible hernias in 44 incarcerated hernia patients. Of these 16, testicular ischemia was determined in 9 (56.2%) infants with the irreducible incarcerated hernia. Orchidopexy procedure was performed in these patients. Testicular atrophy was occurred in two patients (22.2%). In the others, testicular volumes and perfusions were normal during follow-up (mean 8.3 ± 2.2 months). Conclusion: Testicular ischemia resulting from incarcerated inguinal hernia may be treated conservatively without orchiectomy for the ischemic testicle and testicular ischemia may be followed with color Doppler ultrasound for at least 6 months. The inguinal hernia repair in infants should be subject to urgent surgery rather than elective surgery. So, the testicular ischemia in infants with the inguinal hernia will be an avoidable complication.

Use of ozone to treat ileostomy dermatitis in an experimental rat model

Biçer, Şenol; Sayar, İlyas; Gürsul, Cebrail; Işık, Arda; Aydın, Merve; Peker, Kemal; Demiryilmaz, İsmail

Background: Dermatitis associated with ileostomy is an important problem that affects many people, especially children. The aim of this study was to investigate the therapeutic effects of ozone on dermatitis due to ileostomy, and to develop an alternative treatment option. Material/Methods: A total of 28 rats were divided into 4 groups: control, ileostomy, ozone, and zinc oxide. Ileostomy was performed in all rats except the control group. After a 1-week waiting time, the ozone group was administered ozone therapy and the zinc oxide group was administered zinc oxide cream locally once a day for a total of 7 days. All rats were sacrificed at the end of this period. The efficacy of treatment was examined by biochemical, histopathological, and immunohistochemical parameters. The levels of malondialdehyde (MDA), total glutathione (tGSH), total antioxidant capacity (TAC), and total oxidant status (TOS) were measured from tissue. Vascular endothelial growth factor (VEGF) and proliferating cell nuclear antigen (PCNA) were examined immunohistochemically. Results: Dermatitis occurred pathologically in all rats that underwent ileostomy surgery. The lowest dermatitis score was in the ozone treatment group (p<0.05). Ileostomy dermatitis caused increased levels of MDA and TOS. Ozone treatment resulted in reduced MDA and TOS levels, while the levels of tGSH and TAC were increased (p<0.05). Both VEGF and PCNA immunostaining were augmented in the ozone treatment group (p<0.05). Conclusions: Local ozone application may be a good alternative compared to the conventional treatment methods for the prevention of skin lesions that develop after ileostomy.
