Background Right-ventricular dysfunction (RVD) in acute pulmonary embolism (PE) carries excess short-term mortality; fast, transparent risk stratification is needed. Objective To develop and evaluate an interpretable Binary Rule Search (BRS) framework for RVD detection on a fully binarized clinical dataset, emphasizing conservative robustness. Methods We analyzed a single-center cohort ( N = 363; development = 250, external validation = 113) recoded into 0/1 predictors. For subset sizes k = 1–5, a Rust BRS searched bit-mask rules maximizing Matthews correlation coefficient (MCC) on development data and was assessed over 500 stratified 250/113 hold-out repeats. We report MCC with bootstrap 95 % CIs plus sensitivity, specificity, precision, F1, and AUC-ROC. Robustness was summarized by the Stability-Bound Rule Score (SBRS). Results Performance increased with k; triplets offered the best parsimony–robustness balance, with modest conservative gains for quintets. The strongest quintet by point estimate was S{Age65_79; ThromMain; ThromBilat; DVTuni; Malig} (MCC = 0.326; 95 % CI 0.152–0.498). A balanced k = 4 rule, S{SexMale; ThromMain; Hypertens; HeartFail}, achieved Sensitivity 0.791, Specificity 0.557, F1 0.630, AUC-ROC 0.674 on external validation. Decision-tree checks yielded high lower-bound performance (e.g., L95 = 0.302 for S{ThromMain; DVTdist; COPD}), whereas LogNNet often matched mean MCC but showed consistently lower L95, indicating greater dispersion. PFI highlighted a thrombus-centric signal (ThromMain/ThromBilat/DVT) with meaningful secondary contributions from heart failure and hypertension. Conclusions Interpretable BRS ensembles deliver clinically acceptable, conservatively bounded performance using few binary predictors and transparent logic, supporting clinician-facing decision support in resource-constrained settings. We provide rule masks, uncertainty summaries, and an offline CDSS prototype; prospective multicenter validation is warranted.

Lung cancer ranks high among the many cancers that claim to live in the USA. More women have died from lung cancer than breast cancer since 1987. Approximately 160,000 Americans lose their lives to lung cancer each year, and the disease is predicted to produce 225,000 new cases each year. Curiously, at the turn of the twentieth century, lung cancer was a rare occurrence. The growth in smoking among both sexes is mainly responsible for its meteoric ascent in the subsequent decades. The two most common types of lung cancer are small cell lung cancer (SCLC) and non-small cell lung cancer (NSCLC). About 85% of lung cancers are non-small-cell lung tumors. More specific subtypes of non-small cell lung cancer (NSCLC) exist according to histology, including adenocarcinoma, SCC, and large cell carcinoma.

Objectives: In this study, the dimensions of the soft palate, the position of the hyoid bone in relation to the mandibular plane and C3 vertebrae, and the width of the upper airway were measured on lateral cephalograms of patients with obstructive sleep apnea syndrome of different severities diagnosed by polysomnography. This study aimed to investigate the association between increased apnea severity and anatomical formations relevant to dental practice. Materials and Methods: Our study was designed as a retrospective data analysis of 182 patients, 78 females and 104 males. In our study, patients were divided into four groups: patients with simple snoring with an Apnea-Hipopnea index (AHI) value below 5, patients with mild OSAS with an AHI value of 5–15, patients with moderate OSAS with an AHI value of 15–30 and patients with severe OSAS with an AHI value above 30. Measurements obtained from lateral cephalograms were compared between these groups. Results: Statistically significant narrowing was observed in all other groups compared to the control group in oropharyngeal width values in upper airway measurements (P<0.05). The distance between the hyoid and mandibular plane was significantly lower in the control group than in the severe OSAS group (P=0.001). A statistically significant elongation of the soft palate was also observed in patients with OSAS compared to the control group (P<0.05). Conclusions: Our findings support that the position of the hyoid bone is important in the diagnosis of OSAS. Furthermore, a significant correlation was found between soft palate length and OSAS. These results suggest that surgical techniques such as maxillary advancement or mandibular advancement, or both, to increase airway width may be effective in the treatment of OSAS.

Background and Objective: Everolimus is an anticancer agent that inhibits the Mammalian Target of Rapamycin (mTOR). It causes side effect such as pneumonia. This study aimed to evaluate rutin's effect on potential pulmonary injury in everolimus-administered rats biochemically and histopathologically. Materials and Methods: All rats were split into: Healthy (HG), everolimus-treated (EVR) (2 mg/kg) and rutin (50 mg/kg)+everolimus-treated (REV). Rutin was administered to the REV group and physiological saline was given orally tothe EVR and HG groups. As 1 hr later, everolimus 2 mg/kg was administered to REV and EVR groups. The process was repeated daily, 4 times weekly. Then, high dose anaesthesia was used to kill the rats and lung tissue samples were extracted. Oxidative stress and pro-inflammatory cytokine markers were examined and ANOVA test was applied. Results: Oxidative and inflammatory lung damage developed in the everolimus-treated rats. Rutin attenuated this oxidative and pro-inflammatory lung damage induced by everolimus. Rats administered with everolimus developed severe interstitial pneumonia, lymphoid hyperplasia and desquamation in lung tissues. This severe damage was reduced to mild level by administering rutin. Conclusion: The adding rutin to the treatment regimen for pulmonary damage associated with everolimus may have a preventive impact.

Background and Objective: Acrolein-induced tissue damage has been attributed to oxidative stress and inflammation. This study aimed to determine the effects of adenosine triphosphate (ATP) and agomelatine, two substances known to have antioxidant and antiinflammatory effects, separately and in combination, on possible acrolein-induced lung damage in rats. Materials and Methods: There were 30 albino Wistar rats divided into five groups of six: Healthy (HG), acrolein (AC), ATP+acrolein (ATAC), agomelatine+acrolein (AGAC) and ATP+agomelatine+acrolein (AAAC). The dose of ATP administered intraperitoneally was 4 mg/kg and the dose of agomelatine administered orally was 25 mg/kg. An oral dose of acrolein 3 mg/kg was given 1 hr following the administration of ATP and agomelatine. For 30 days, the treatment protocol was followed once a day. Afterwards, the rats were euthanized and their lungs were removed. Oxidants, antioxidants and pro-inflammatory cytokines were measured in lung tissues. Results: In lung tissues, acrolein increased malondialdehyde, nuclear factor kappa, tumor necrosis factor-alpha, interleukin-1beta and interleukin-6 levels and decreased total glutathione, superoxide dismutase and catalase levels (p<0.001). The ATP+agomelatine, ATP and agomelatine treatments were the most effective at preventing these biochemical changes, respectively (p<0.001). As a result, the biochemical data of the HG and AAAC groups were similar (p>0.05). Conclusion: It could be suggested that both ATP and agomelatine might be effective against the pneumotoxic effects of acrolein, though a combination therapy would be more effective.

Objectives: This study aimed to evaluate how Rhinapi nasal spray affects symptoms of allergic rhinitis. Methods: In this prospective, multicenter, observational study, 10,000 patients (comprising 5028 males and 4972 females) exhibiting symptoms of allergic rhinitis (namely, nasal discharge, sneezing, nasal itching, and nasal obstruction) from different centers in different regions of Turkey were enrolled in the study between March 2022 and March 2023. All the patients wanted to participate in the study and were administered Rhinapi one puff to each nostril three times a day, for a period of 3 weeks. Total symptom scores, quality of life (QoL) scores, and otolaryngological examination scores were evaluated before and 3 weeks after treatment. Results: The scores for discharge from the nose, sneezing, nasal pruritus, and blockage of the nose all indicated improvement when compared to pre-medication and post-medication. This difference achieved statistical significance (P <.001). The mean total symptom score fell following treatment (P <.001): whilst the score was 11.09 ± 3.41 before administering Rhinapi; after administration, the average score was 6.23 ± 2.41. The mean QoL scores also altered after medication (P <.001), improving from a mean value of 6.44 ± 1.55 to a mean of 7.31 ± 1.24. Significant improvement was also noted in the scores for conchal color and degree of edema after the treatment had been administered (P <.001). Conclusion: The study demonstrates that Rhinapi nasal spray decreases total symptom scores, and results in improved QoL and otolaryngological examination scores. Propolis spray may be recommended for patients with allergic rhinitis alongside other treatments.

Favipiravir is a broad-spectrum antiviral drug that is a viral RNA-dependent RNA polymerase inhibitor. Favipiravir is used in high doses to treat COVID-19 but has a side effect on humans at high doses. The side effects of favipiravir have been associated with oxidative stress in the literature. In this trial, we investigated the biochemical and histopathological effects of lacidipine, thiamine pyrophosphate (TTP), and adenosine triphosphate (ATP), drugs with antioxidant properties, on the lung toxicity caused by high-dose favipiravir in rats. The rats were classified into five groups: healthy (HG), favipiravir alone (Fav), lacidipine+favipiravir (LFav), TPP+favipiravir (TFav), and ATP+favipiravir (AFav). Favipiravir (800 mg/kg) was administered twice daily for seven days. Laci-dipine (4 mg/kg), TPP (20 mg/kg), and ATP (25 mg/kg) were administered once daily for seven days. Oxidant (malondialdehyde), non-enzymatic (total glutathi-one), and enzymatic (superoxide dismutase and catalase) antioxidant levels were measured in the excised lung tissues. Furthermore, the tissues were histopatho-logically examined. The systemic administration of high doses of favipiravir in-creased oxidant levels and decreased antioxidant levels in the lung tissue of rats. In parallel, the histopathological examination of the lung tissue revealed the presence of severe mononuclear cell infiltrations in interstitial areas and pronounced lymphoid hyperplasia. Lacidipine exhibited superior efficacy in mit-igating oxidative stress and preventing the decline of antioxidants induced by favipiravir compared with TPP and ATP. Histopathologically, the lacidipine administration significantly reduced lung oxidative damage. TTP moderately reduced severe favipiravir-associated lung injury. However, ATP was ineffective against favipiravir-associated lung injury. Lacidipine offers more therapeutic benefits than TPP in treating oxidative lung injury caused by high doses of favipiravir.

Pulmonology is one of the branches that frequently receive consultation requests from the emergency department. Pulmonology consultation (PC) is requested from almost all clinical branches due to the diagnosis and treatment of any respiratory condition, preoperative evaluation, or postoperative pulmonary problems. The aim of our study was to describe the profile of the pulmonology consultations received from emergency departments in Turkiye. A total of 32 centers from Turkiye (the PuPCEST Study Group) were included to the study. The demographic, clinical, laboratory and radiological data of the consulted cases were examined. The final result of the consultation and the justification of the consultation by the consulting pulmonologist were recorded. We identified 1712 patients, 64% of which applied to the emergency department by themselves and 41.4% were women. Eighty-five percent of the patients had a previously diagnosed disease. Dyspnea was the reason for consultation in 34.7% of the cases. The leading radiological finding was consolidation (13%). Exacerbation of preexisting lung disease was present in 39% of patients. The most commonly established diagnoses by pulmonologists were chronic obstructive pulmonary disease (19%) and pneumonia (12%). While 35% of the patients were discharged, 35% were interned into the chest diseases ward. The majority of patients were hospitalized and treated conservatively. It may be suggested that most of the applications would be evaluated in the pulmonology outpatient clinic which may result in a decrease in emergency department visits/consultations. Thus, improvements in the reorganization of the pulmonology outpatient clinics and follow-up visits may positively contribute emergency admission rates.

Gür, Volkan; Yapici, Furkan; Subasi, Izzet Ozay; Gökgöz, Mehmet Burak; Tosun, Mustafa; Tardus, Ismail; Koçkara, Nizamettin
Introduction Venous thromboembolism (VTE), particularly pulmonary embolism (PE), is the third highest cause of death in trauma patients who survive beyond the first day. Musculoskeletal surgery is associated with several complications, some of which may be life-threatening, including deep vein thrombosis (DVT) and PE. Objective This research aims to describe risk variables for VTE after upper extremity (UE) fracture at a single institution and estimate the incidence of PE following UE fracture. Methods The writers accessed the database via their respective universities using the International Standard Classification (ICD) codes. The medical files of patients aged 18 and older who sought treatment at our emergency department for an injury to their UE and also sought treatment at the orthopedics and traumatology clinic between the years 2013 and 2021 were manually scanned. The patients who applied to the Chest Diseases Clinic within 30 days after the trauma and were diagnosed with PE in the ICD code scan were included in the study. Results UE trauma was the cause of admission to the emergency department for 3,265 patients, and 21 of those patients (0.64%) were found to have PE. Fifteen of the patients were male, and six were female. The median age was 59 years (IQR 17). There were no deaths associated with PE. One of the patients had a scaphoid fracture, seven patients had a humerus fracture, five patients had a distal radius fracture, two patients had an acromioclavicular joint injury, one patient had a shoulder dislocation, one patient had a finger fracture, four patients had wrist crush injury. Three patients had diabetes mellitus. Five patients were active smokers. JAK-2 gene V617F mutation was detected in one patient. One patient was diagnosed with prostate cancer, and one had gastric cancer. One patient had a central venous catheter. Two patients were being treated for hypothyroidism. Two patients had hypertension. Conclusion According to the findings of our research, the probability of developing PE in the days following of an injury to the UE was found to be 0.64%. Patients with UE injuries who are active smokers and who also have diabetes, hypertension, hypothyroidism, cancer, coagulation disorder (JAK2 gene V617F mutation), or a central venous catheter may benefit from anticoagulant prophylaxis. This is because these patients are at a higher risk of developing dangerous blood clots.
