Objective: To investigate whether morphologic changes in the cranial component of the temporomandibular joint are associated with aging or tooth loss and, if so, whether articular eminence flattening is more associated with age or edentulism. Methods: Ortopantomographs of patients admitted to the hospital for routine dental treatment were retrospectively analyzed. Patients were divided into 4 groups according to their age and edentulous status. Group1: 49 patients aged 18-35 years with occlusal support, Group2: 46 patients aged 36-59 years with occlusal support, Group3: 43 patients over 60 years with occlusal support, and Group4: 42 patients over 60 years with lost occlusal support. The Frankfort horizontal plane and the articular eminence slope were drawn on the radiographs and the angle between them was measured and the differences between the groups were analyzed. Results: There was no significant correlation between age and flattening of the articular eminence angle (P=.645). A statistically highly significant difference was observed between Group 4 and all other groups in terms of articular eminence angles (P=.01). Conclusion: Since there was no significant correlation between age and articular eminence angle, and since the articular eminence angle was significantly different in Group 4, which consisted of completely edentulous patients, compared to all other groups, it was concluded that AEA may be related to edentulism rather than age. Study emphasizes the importance of occlusion by identifying tooth loss as a possible cause of the decrease in AE slope.
RDL Kalıcı Kaydıİncele - ArticleAçık Erişim2026Three-dimensional finite element analysis of temporomandibular joint stresses following botulinum toxin injection and occlusal splint therapy in bruxism
Haşimoğlu, Elif; Balaban, Emre; Halat, İsmail Burak; Akın, Muhammet
Background: Bruxism-related clenching can impose excessive mechanical loads on the temporomandibular joint (TMJ), particularly in the presence of anterior disc displacement (ADD). Understanding how different therapeutic strategies influence joint stress under these conditions can support exploratory, hypothesis-generating assessment of biomechanical unloading within patient-specific models. Therefore, this study aimed to evaluate TMJ stress distributions under normal and displaced disc conditions and to assess the biomechanical effects of simulated treatment strategies using a patient-specific finite element framework. Materials and methods: A patient-specific three-dimensional finite element model derived from clinically acquired computed tomography (CT) data was constructed to simulate TMJ biomechanics during clenching. Stress patterns were analyzed under three disc conditions: normal disc position, unilateral ADD, and bilateral ADD. Intervention scenarios included simulated reductions in masticatory muscle forces representing botulinum toxin Type-A (BoNT-A) therapy and occlusal splint therapy, applied individually and in combination. Peak von Mises stresses within the articular disc and joint structures were compared across all conditions. Results: ADD increased peak stresses within the articular disc and adjacent joint surfaces and shifted stress concentration toward the posterior region of the disc. Simulated reductions in muscle force resulted in progressive decreases in peak joint stresses as additional muscle groups were targeted, whereas splint therapy alone provided moderate stress reduction. The greatest biomechanical unloading was observed when occlusal splint therapy was combined with BoNT-A–related muscle force reduction. Notably, a strategy targeting the masseter and temporalis muscles combined with splint therapy produced lower peak disc stresses than a three-muscle reduction strategy without splint across all disc conditions. Conclusion: In this exploratory single-patient finite element model, ADD increased predicted TMJ stresses during clenching, whereas simulated muscle-force reduction and splint-related changes in occlusal load transmission reduced peak stress metrics, with the largest reductions observed in combined scenarios. These results are hypothesis-generating and intended for relative, within-model comparisons under stated assumptions rather than clinical treatment recommendations.

Taşkesen, Fatih; Cezairli, Burak
Objective: To compare the comfort and anxiety levels of patients undergoing two different temporomandibular joint (TMJ) arthrocentesis techniques. Methods: Fifty female patients were randomly assigned into two groups of 25 based on the treatment modality: Group 1, single-puncture Type-1 arthrocentesis (SPA); Group 2, conventional double-puncture arthrocentesis (DPA). Preoperative and postoperative anxiety was scored with the State-Trait Anxiety Inventory-S (STAI-S) questionnaire. Blood pressure and heart rate were assessed preoperatively during the application of local anesthesia and at needle insertion into the joint cavity, the 5th and 10th minute of the procedure, and end of the procedure. Results: STAI-S scores were lower postoperative than preoperative in both groups. Heart rate and mean arterial pressure were highest during anesthesia and needle insertion (p < 0.005). Changes in heart rate, mean arterial pressure, and STAI-S scores were statistically similar between the groups. Conclusion: DPA and SPA were tolerated similarly by the patients.
RDL Kalıcı Kaydıİncele - ArticleYalnız künye2023Efficacy of prolotherapy and arthrocentesis in management of temporomandibular joint hypermobility
Taşkesen, Fatih; Cezairli, Burak
Objective: The aim of this study was to compare the efficacy of dextrose prolotherapy in conjunction with arthrocentesis and dextrose prolotherapy alone in the management of symptomatic TMJ hypermobility. Methods: Twenty-four patients suffering from TMJ symptomatic hypermobility received 10% dextrose injections for three sessions at four-week intervals. Twelve patients (P group) received prolotherapy alone; the other 12 (PA group) also underwent a single arthrocentesis session. Patients were evaluated for maximal incisal opening, maximal incisal opening without pain, pain at rest, pain during chewing function, TMJ sound, and locking episode frequency. Results: Pain scores were significantly reduced in both groups, and the locking episode frequency was decreased to a greater extent in the PA than the P group, at both the short and long terms. Conclusion: Prolotherapy is effective in the management of TMJ hypermobility. However, prolotherapy with arthrocentesis may be superior to prolotherapy alone in the management of TMJ hypermobility.

Gülnahar, Yakup; Alpan, Aysan Lektemur; Gülnahar, Evrem
Background: The aim of this study is to compare the effects of %4 articaine and %2 lidocaine on inferior alveolar nerve block (IANB) for implant surgery in the posterior mandible. Material and Methods: The patients who have inserted implants in the posterior mandible were divided into 2 groups for IANB: lidocaine and articaine. VAS = visual analog scale, pain during surgery and injection, lip numbness time, mandibular canal-implant apex distance, age, gender, bone density, implant number, release incision, adjacent teeth, and duration of surgery were analyzed using t-test, Mann-Whitney U test, Spearman's coefficient, and, Pearson's chi-squared test. This trial followed the recommendations of the Consort Statement for reporting randomized controlled trials. Results: 577 patients were included and 1185 dental implants were analyzed. There was no significant difference between the two groups in terms of injection and surgery VAS values (p>0.05). The lip numbness time of lido-caine was 3.06±3.22min while articaine was found to be 2.96±3.09min (p>0.05). Mandibular canal-implant apex distance was found to be 2.28±0.75mm in the articaine and 2.45±0.86mm in the lidocaine group (p<0.05). Release incision was made more in the articaine group (51/252) than in the lidocaine group (40/325) (p<0.05). Conclusions: There was no difference between the %4 articaine and %2 lidocaine in terms of pain perception in posterior mandible implant applications. Both anesthetics provided adequate anesthesia for implant application.
RDL Kalıcı Kaydıİncele - ArticleYalnız künye2023The effectiveness of the masseteric nerve block compared with trigger point injections and dry needling in myofascial pain
Taşkesen, Fatih; Cezairli, Burak
Objective: To compare the efficacy of three different treatment methods in the management of myofascial pain: masseteric nerve block (MNB), trigger point injection with local anesthetic (LA), and dry needling (DN). Methods: Forty-five patients diagnosed with myofascial pain and trigger points in masseter muscles were treated with MNB (n = 15), DN (n = 15), and trigger point injection with LA (n = 15). Pain on palpation (PoP), pain on function (PoF), and maximum mouth opening (MMO) scores were measured and compared before the injections and all follow-ups after the injections. Results: MMO values were significantly increased in each group. The decrease in PoF values was statistically significant between the groups at 12 weeks (baseline time period). Discussion: Results of the present study indicate that MNB was not as effective as trigger point injection with local anesthetic or dry-needling in the management of masticatory myofascial pain.

Cezairli, Burak; Taskesen, Fatih
