Karaca, Serhat; Şirinoğlu Capan, Belen
Objective: The aim of this study was to evaluate the effect of parental presence on dental anxiety in children during dental treatments. Materials and methods: The study was conducted with 194 children between January-April, 2020. The children were randomly divided into two subgroups. Children in group-I were treated in parent’s presence (pp), and in group-II in parent’s absence (pa). The Wong-Baker FACES Pain Rating Scale (WBFPS) and The Modified Dental Anxiety Scale (MDAS) were used for subjective measurements, whereas the objective measurement was performed by measuring the heart rate. Results: The mean age of 194 children was 6.26 ± 1.15 years, ranging from 5-8 years of age. The mean MDAS score of all children was 15.1 ± 4.72. No significant correlations were found in terms of dental anxiety between the children’s gender and age with heart rate, WBFPS and MDAS scores. Preoperative WBFPS scores (6.83 ± 1.04 pp and 7.01 ± 0.93 pa) were higher than postoperative scores (5.34 ± 2.11 pp and 5.74 ± 2.04 pa), with no statistically significant difference. Although there was no statistically significant results, the paediatric dentist observed a deterioration in the children’s behavior throughout the sessions in group-II compared to children in group-I. Conclusions: Parental presence has no statistically significant effect on dental anxiety in children during dental treatments.

Karaca, S.; Capan, B. Sirinoglu
Aim: The aim of the present study is to evaluate the relationship between sequential visits in dental caries treatments and dental anxiety levels of paediatric patients through the use of anxiety scales and physiological measurements. Study Design: A total of 224 children aged 5–8 years who needed at least two bilateral restorative dental caries treatments of the mandibular first primary molar was included in the study. The treatment duration was approximately 20 minutes and time span between the two appointments was a maximum of two weeks. The Wong-Baker FACES Pain Rating Scale (WBFPS) and The Modified Dental Anxiety Scale (MDAS) were used for subjective measurements and the objective measurement of dental anxiety was performed by measuring the heart rate using a portable pulse oximeter. Statistical analysis was carried out using the Statistical Package for the Social Sciences version 22 (IBM corp. Armonk, NY, USA). Results: Out of the 224 children, 117 were girls (mean age 6.4 ± 1.1 years) and 107 were boys (mean age 6.1 ± 1.2 years). The mean age of the patients was 6.26 ± 1.147 years, ranging from 5 to 8. There were no statistical significant differences between children’s age and genders with dental anxiety. However, there were statistical significant differences for the variables regarding the sequential visits. When comparing the MDAS scores statistically significant differences were observed between the first and second visits (p<0.001). Both preoperative and postoperative WBFPS scores were significantly higher at the first visit than at the second visit. Statistically significant differences were observed in the heart rate scores between the treatment sessions (p<0.001). Conclusion: The results of this study show a significant reduction in dental anxiety after sequental dental visits in children aged 5–8 years, which highlights the importance of the sequental visits in paediatric dentistry.

Karaca, Nurcan Kutluer; Karaca, Serhat; Karakurt, Tülay Ceren Ölmeztürk
Introduction: This study aims to compare the incidence of emergency delirium in children who underwent dental rehabilitation under either sevoflurane anesthesia or remifentanil infusion-added sevoflurane anesthesia. Methods: This retrospective study evaluated patients scheduled for dental treatment under general anesthesia between April to September 2019 in a tertiary hospital. Patients were categorized into two groups receiving sevoflurane (group S) or remifentanil infusion and sevoflurane (group R). Emergence delirium and postoperative pain were evaluated by the Paediatric Anesthesia Emergence Delirium scale and The Face, Legs, Activity, Cry, Controllability scale every 10 min. The extubation time, postanesthetic care unit discharge time, duration of surgery, and anesthesia were also recorded. Results: Data from 80 subjects were analyzed. The incidence of emergency delirium was higher in group S than in Group R (67.5%, 27/40; P < 0.001). Greater Paediatric Anesthesia Emergence Delirium (10.2 ± 1.1 / 8.25 ± 1.4; P < 0.001) and The Face, Legs, Activity, Cry, Controllability scale (3.10 ± 0.9 / 2, 03 ± 1.2; P < 0.001) scores were observed in group S than group R. A longer extubation time and shorter postanesthetic care unit discharge time were observed in group R than in group S (P < 0.001). Conclusions: A lower incidence of emergency delirium was observed after remifentanil infusion was added to sevoflurane in children after dental treatment under general anesthesia. Moreover, remifentanil resulted in a more comfortable postoperative period due to reduced postoperative pain, extubation, and recovery time.

Background: In this study, it was planned to retrospectively determine the congenital agenesis of the third molars in 1460 panoramic films of pediatric patients in the Erzincan region and to examine the distribution according to gender and location in the jaws. Materials and Methods: This retrospective study was conducted at Erzincan Binali Yıldırım University, Faculty of Dentistry from June 2018-January 2020. 1460 patients(752 females and 708 males) between the ages of 10-14 were studied with panoramic radiograph to assess for agenesis of mandibular third molars. Statistical data were analyzed in SPSS 22 program. The Pearson chi-square test was performed for determining the prevalence of congenitally agenesis of the third molar teeth. Results: The prevalence of third molar teeth agenesis was 35.6 %. Total number of patients with single missing third molar was 222(15.5 %). The ratio of the third molar agenesis for females(38 %) was higher than that for males(33%). Significantly higher number of third-molar teeth were found to be missing from the maxilla(57%) than the mandible(43%). 529 patients(54%) had agenesis only on the right side and 469 patients(46%) had agenesis only on the left. Conclusion: The agenesis of the third molars was investigated in pediatric patients living in Erzincan province and its prevalence was found 35.6%. Congenital deficiency of the third molar teeth is more common in women, while it has been observed more in the maxilla than in the mandible. More comprehensive studies can be done by increasing the number of individuals examined.

BIS-GMA is frequently is involved in the structure of composite materials and may remain unpolymerized after polymerization. Residual Bis-GMA has harmful effects, it is crucial to determine the amounts released monomer from dental materials. The aim of this study is to compare residual BIS-GMA amounts in three types of composite. A total of 45 (n=15) composite (Filtek Ultimate Universal Restorative,3M ESPE,Germany), flowable composite (Nextcomp Flow,Meta Biomed,Kore), bulk fill composite(Tetric N Ceram, Ivoclar,Lichtenstein) cylindrical samples were polymerized in Teflon molds in accordance with the manufacturer's instructions. The samples were placed in 1.5ml ethanol (Carlo Erba, Pharm. Grade, 99.9%) and kept at 37oC. Monomer release of the samples was evaluated on the first, third and seventh days using the High Performance Liquid Chromatography technique. BIS-GMA release was shown with the highest amounts within all groups on the first day. The elution in composite fillings was significantly lower than the other materials on the first day(p<0.05). The highest total emission rates were observed in the flowable composite. The material selected in clinical practice should be considered because the monomer release may vary depending on the composite type.
