Objective: The aim of this study is to investigate the impact of different catheter types used during right transradial coronary angiography on clinical outcomes in patients with acute coronary syndrome (ACS). The study seeks to evaluate the differences between the single guiding catheter technique and standard catheter approaches in terms of procedural efficiency and patient safety. Materials and Methods: A total of 63 patients diagnosed with acute coronary syndrome (ACS) who underwent right transradial coronary angiography and percutaneous coronary intervention (PCI) were included in the study. Based on the type of catheter used, patients were divided into two groups: the single-catheter group and the standard-catheter group. In the standard group, diagnostic imaging of the non-culprit artery was performed using a diagnostic catheter, followed by engagement of the culprit lesion with a guiding catheter to proceed with the intervention. In contrast, in the single-catheter group, both diagnostic and interventional procedures were performed using a 3.5 Judkins Left (JL) guiding catheter (Boston Scientific Mach 1™). The two groups were compared in terms of total procedure time, fluoroscopy time, radiation dose, and the amount of contrast medium used. Results: The fluoroscopy time was significantly shorter in the single-catheter group, with a median of 11.1 minutes (range: 3.8–18.8), compared to 12.9 minutes (range: 5.2–45.2) in the standard-catheter group (p = 0.003). Radiation dose was also significantly lower in the single-catheter group (76.1 Gy∙cm²) than in the standard-catheter group (125.4 Gy∙cm²; p = 0.001). The total procedure time was 27.0 minutes in the single-catheter group and 36.0 minutes in the standard-catheter group, showing a significant difference (p = 0.0001). However, no statistically significant difference was observed between the groups in terms of the amount of contrast agent used (104.1 ± 25.5 ml vs. 112.1 ± 19.2 ml; p = 0.16). Conclusion: The efficacy and safety of the single-catheter technique using a JL 3.5 guiding catheter (Boston Scientific Mach 1™) in percutaneous coronary interventions were evaluated. The findings demonstrated that the single-catheter strategy significantly reduced fluoroscopy time, radiation dose, and total procedure duration. Eliminating the need for catheter exchanges not only improved procedural efficiency but also contributed to a reduction in the risk of vascular complications.
