Objective: Cardiovascular disease is prevalent among individuals diagnosed with non-alcoholic fatty liver disease (NAFLD). Epicardial adipose tissue (EAT), carotid intima-media thickness (CIMT), and aortic propagation velocity (APV) are associated with atherosclerosis and cardiovascular disease (CVD). The aim of this study is to determine whether NAFLD is associated with EAT, CIMT, and APV. Methods: Individuals aged ≥18 and <65 years who presented for routine follow-up were included. The study population comprised 75 healthy controls (40 females, 35 males) and 141 patients with NAFLD (68 females, 73 males). Epicardial adipose tissue thickness, carotid intima-media thickness, and aortic propagation velocity were evaluated using transthoracic echocardiography in patients who were diagnosed with hepatosteatosis by ultrasound during routine check-ups at the internal medicine clinic. Results: NAFLD was positively associated with epicardial adipose tissue thickness and carotid intima–media thickness, and negatively associated with aortic propagation velocity. Patients with NAFLD had significantly higher EAT and CIMT values and lower APV values than controls. Logistic regression analysis identified body mass index (BMI) and EAT as independent predictors of hepatosteatosis. Each unit increase in BMI was associated with a 1.29-fold increase in the risk of hepatosteatosis, whereas each unit increase in EAT was associated with a 5.49-fold increase. Conclusion: Given the established associations of CIMT, APV, and EAT with atherosclerosis and cardiovascular disease, assessment of patients with NAFLD using these noninvasive parameters may help identify subclinical cardiovascular risk.
RDL Kalıcı Kaydıİncele - ArticleAçık Erişim2026Effects of early shoulder exercise on functional recovery after cardiac implantable electronic device implantation: A 3-year randomized cohort follow-up
Cosgun, Muharrem Said; Cosgun, Cansu
Background. Shoulder dysfunction after cardiac implantable electronic device (CIED) implantation may impair upper-extremity function and quality of life (QoL). Although a prior randomized trial demonstrated short-term benefits of early postoperative exercises, long-term outcomes remain unclear. Aim. To evaluate the 3-year efficacy and safety of an early postoperative shoulder exercise protocol after CIED implantation. Methods. Of 92 patients randomized in the original trial, 82 completed the 3-year follow-up. Participants had completed a 6-week intervention consisting of pendulum exercises (PE), stretching & strengthening exercises (SSE) or standard care, without further structured exercise. Outcomes included Visual Analog Scale (VAS) pain scores, grip strength, shoulder flexion and abduction range of motion (ROM), Quick Disabilities of the Arm, Shoulder, and Hand (QuickDASH), and the Physical and Mental Component Summary scores (PCS and MCS) of the 36-Item Short-Form Survey (SF-36). Lead-related complications were also assessed. Results. VAS and SF-36 MCS scores were comparable across groups. However, compared with standard care, both exercise groups maintained superior outcomes at 3 years for grip strength (PE: +9.0 kgf; SSE: +9.0 kgf), shoulder flexion ROM (PE: +10.0 degrees; SSE: +11.0 degrees), shoulder abduction ROM (PE: +7.0 degrees; SSE: +7.0 degrees), QuickDASH (PE: -5.0 points; SSE: -6.0 points), and SF-36 PCS (PE: +9.0 points; SSE: +10.0 points) (all P < 0.05). No lead-related complications occurred during follow-up. Conclusion. Early postoperative shoulder exercise after CIED implantation provides sustained improvements in shoulder function and physical QoL without compromising lead safety.

Background. Cardiovascular diseases (CVDs) are highly prevalent among patients with rheumatoid arthritis (RA). Epicardial adipose tissue, serum betatrophin, chemerin, and adropin levels are factors associated with atherosclerosis and cardiovascular involvement. Objectives. This study aimed to investigate the relationship between RA and epicardial fat thickness (EFT), as well as serum betatrophin, chemerin and adropin levels. Materials and methods. This cross-sectional study included 80 patients (62 women and 18 men) diagnosed with RA according to the American College of Rheumatology/The European Alliance of Associations for Rheumatology (ACR/EULAR) 2010 RA classification criteria and 80 healthy controls (64 women and 16 men). Exclusion criteria comprised other autoimmune diseases, CVDs, diabetes mellitus, other endocrine disorders, acute or chronic pancreatic disorders, malignancy, pregnancy, breastfeeding, or antihyperlipidemic drug usage. Serum betatrophin, chemerin and adropin concentrations were measured. Epicardial fat thickness was evaluated with transthoracic echocardiography. Results. Adropin levels were significantly lower in the RA group compared to the control group (p < 0.001). Chemerin levels and EFT were significantly higher in the RA group than in the control group (p = 0.016, p < 0.001, respectively). When assessing the relationship between biomarkers and EFT in RA patients, a strong positive correlation was observed between chemerin and EFT (r = 0.73, p = 0.046) in patients with high disease activity. Conclusions. Epicardial fat thickness, as an indicator of cardiovascular involvement, is higher in patients with RA. Moreover, high chemerin levels and low adropin levels in these patients may be indicative of cardiovascular involvement. Copyright by Author(s)

Background: We aimed to examine whether serum magnesium level is related to cardiovascular events in hemodialysis patients by investigating the relationship between serum magnesium (Mg) level and carotid intima media thickness (CIMT). Methods: In this cross-sectional study, we evaluated 88 hemodialysis patients. The mean CIMT of these patients was measured and recorded by doppler ultrasonography. Patients were divided into two groups according to their serum magnesium level. Correlation analysis and linear regression analysis were used to define the relationship between study parameters. We divided our patients into 2 groups according to Mg value as group 1 (Mg ≥ 2.3 mg/dl) and group 2 (Mg < 2.3 mg/dl). Results: The mean CIMT of patients with group 1 and group 2 were respectively 0.93 ± 0.22 mm and 1.03 ± 0.22 mm. There was statistically significant difference between two groups in terms of the mean CIMT value (p = 0.045). We found negative, low-level, statistically significant correlations between Mg and mean CIMT (r=-0.272; p = 0.010), calcium and mean CIMT (r: -0.221, p: 0.039) and hemoglobin and mean CIMT (r: -0.215, p: 0.044). According to regression analysis, serum magnesium level and age were found independent risk factors for mean CIMT value (p: 0.004 and p: 0.045). Conclusions: We found a significant relationship between serum Mg level and CIMT, which indicates carotid atherosclerosis.

Objective: Nonalcoholic fatty liver disease has been connected to coronary artery disease, which is the main cause of mortality. The connection between the degree of nonalcoholic fatty liver disease-induced liver fibrosis and the severity of coronary artery disease is unclear, and it was aimed to be evaluated. Methods: 200 nonalcoholic fatty liver disease patients over the age of 18 who underwent emergency or elective coronary angiography were included in this study. Both nonalcoholic fatty liver disease fibrosis and fibrosis-4 scores were calculated to determine liver fibrosis stages. Carotid intima-media thickness and SYNergy between Percutaneous Coronary Intervention (PCI) with TAXus and cardiac surgery score were calculated to determine the severity of coronary artery disease. Results: There was no difference in carotid intima-media thickness between the groups in the fatty liver stages, but when the patients were staged according to nonalcoholic fatty liver disease fibrosis and fibrosis-4 scores, the mean of carotid intima-media thickness increased as the risk of fibrosis increased. When patients were classified into 2 categories based on their median carotid intima-media thickness value, a significant difference was realized in terms of nonalcoholic fatty liver disease fibrosis and fibrosis-4 scores. There was no significant difference in SYNergy between PCI with TAXus and cardiac surgery score when the patients were evaluated according to the fatty liver stages, nonalcoholic fatty liver disease fibrosis and fibrosis-4 scores. Considering severe coronary artery disease (SYNergy between PCI with TAXus and cardiac surgery score > 0 or not) when patients were classified into 2 categories, nonalcoholic fatty liver disease fibrosis was found to be higher in the group with severe coronary artery disease, however, there was no difference in fibrosis-4 scores between the groups. Conclusion: This study suggests that the frequency and severity of coronary artery disease increase in the progression from simple fatty liver to the fibrotic liver. Our data, supporting the literature, reveal that nonalcoholic fatty liver disease fibrosis is more closely related to the prevalence and severity of coronary artery disease than the fibrosis-4 score.

Objective: In this study, we aimed to evaluate the in-hospital mortality indicators in patients presenting with ST-elevation myocar- dial infarction (STEMI) to our clinic and undergoing primary angioplasty. Methods: Four hundred patients presenting to our clinic with a diagnosis of STEMI and undergoing primary angioplasty were included in the study. Patient files were retrospectively reviewed. The relationship between in-hospital mortality and the development of complications after primary angioplasty and patients’ demographic, clinical, and laboratory characteristics were evaluated. Results: In the patient group included in the study, advanced age (≥65), diabetes mellitus, hypertension, and female gender were found to increase in-hospital mortality significantly. A significant increase in in-hospital mortality was observed in patients with left ventricular ejection fraction (LV EF) below 40%. Elevated baseline creatinine and troponin levels, high heart rate, presence of shock, and hypotension significantly increased in-hospital mortality in patients. Conclusion: In our study, we found that the risk factors, clinical and laboratory characteristics, and complications developed in patients presenting with acute STEMI are directly related to patient mortality. Identifying these mortality predictors is essential in determining patient risk, guiding treatment, and follow-up.
RDL Kalıcı Kaydıİncele - ArticleAçık Erişim2024EFFECT OF TAXIFOLIN ON DOXORUBICIN-INDUCED OXIDATIVE CARDIAC DAMAGE IN RATS: A BIOCHEMICAL AND HISTOPATHOLOGICAL EVALUATION

Doxorubicin is a widely used anthracycline-derived broad-spectrum antitumoral antibiotic drug. However, cardiotoxicity due to doxorubicin treatment has warranted dose reduction or complete discontinuation in certain cases. The role of oxidative stress in the pathogenesis of doxorubicin-induced cardiotoxicity has been previously demonstrated. Against this background, this study aimed to investigate the protective effect of the potent antioxidant flavone taxifolin against possible oxidative heart damage biochemically and histopathologically induced by doxorubicin. Albino Wistar male rats were divided into three groups: healthy controls (HG), a group given doxorubicin alone (DG), and a group given taxifolin + doxorubicin (TDG). Taxifolin was administered orally at a dose of 50 mg/kg via gavage. Doxorubicin was injected intraperitoneally at a dose of 5 mg/kg. This procedure was repeated for 7 days. The results of the biochemical experiment showed that taxifolin significantly inhibited doxorubicin-induced malondialdehyde increases and glutathion decreases in heart tissues. In addition, taxifolin significantly suppressed the increases in cardiac damage markers, such as serum troponin I, creatine kinase, and creatine kinase-MB, induced by doxorubicin. Taxifolin treatment has also been histopathologically shown to alleviate doxorubicin-induced heart tissue damage. Accordingly, the results of the present study suggest that taxifolin may be useful in the treatment of doxorubicin-induced oxidative heart damage.

The aim of the study was to investigate the effects of lacidipine, amlodipine, and benidipine on the analgesic activity and toxicity of tramadol. Rats (n = 6/each group) were divided into five groups as control (CG), tramadol (TRD), lacidipine+tramadol (LTRD), amlodipine+tramadol (ATRD), and benidipine+tramadol (BTRD). Tramadol was administered once at a dose of 50 mg/kg and lacidipine, amlodipin, benidipin were administered at a dose of 4 mg/kg orally, once a day for 10 days. After the animals were sacrificed, malondialdehyde (MDA) and total glutathione (tGSH) levels were measured in brain, heart, liver, and kidney tissues. Troponin I (Tp I), alanine aminotransferase (ALT), aspartate aminotransferase (AST), blood urea nitrogen (BUN), and creatinine levels were determined in serum. Paw pain thresholds were measured one hour before and after drug administration. The analgesic effect of tramadol was increased the most by benidipine, while amlodipine and lacidipine increased it equally. Lacidipine failed to suppress MDA increase and tGSH decrease in brain tissue. Benidipine suppressed MDA increase and tGSH decrease in brain tissue better than amlodipine. All three drugs suppressed cardiac tissue oxidative stress and the release of TP I into the circulation. Lacidipine and amlodipine could not prevent tramadol-induced oxidative stress in liver and kidney tissues, whereas benidipine prevented excessive MDA increase and tGSH decrease. Benidipine significantly suppressed the increase of ALT, AST, BUN, and creatinine. Analgesic activity was 35.1% in TRD, 43.7% in LTRD, 50.4% in ATRD, and 67.5% in BTRD. Study results show that benidipine and tramadol co-treatment is better than co-treatment with lacidipine or amlodipine in achieving minimal toxic effects and maximum analgesia.

Background: Atrial fibrillation (AF) is the most prevalent cardiac arrhythmia worldwide and is associated with an increased risk of thromboembolism, ischemic stroke, impaired quality of life, and mortality. The latest research that shows the prevalence and incidence of AF patients in Türkiye was the Turkish Adults’ Heart Disease and Risk Factors study, which included 3,450 patients and collected data until 2006/07.The Turkish Real Life Atrial Fibrillation in Clinical Practice (TRAFFIC) study is planned to present current prevalence data, reveal the reflection of new treatment and risk approaches in our country, and develop new prediction models in terms of outcomes. Methods: The TRAFFIC study is a national, prospective, multicenter, observational registry. The study aims to collect data from at least 1900 patients diagnosed with atrial fibrillation, with the participation of 40 centers from Türkiye. The following data will be collected from patients: baseline demographic characteristics, medical history, vital signs, symptoms of AF, ECG and echocardiographic findings, CHADS2-VASC2 and HAS-BLED (1-year risk of major bleeding) risk scores, interventional treatments, antithrombotic and antiarrhythmic medications, or other medications used by the patients. For patients who use warfarin, international normalized ratio levels will be monitored. Follow-up data will be collected at 6, 12, 18, and 24 months. Primary endpoints are defined as systemic embolism or major safety endpoints (major bleeding, clinically relevant nonmajor bleeding, and minor bleeding as defined by the International Society on Thrombosis and Hemostasis). The main secondary endpoints include major adverse cardiovascular events (systemic embolism, myocardial infarction, and cardiovascular death), all-cause mortality, and hospitalizations due to all causes or specific reasons. Results: The results of the 12-month follow-up of the study are planned to be shared by the end of 2023. Conclusion: The TRAFFIC study will reveal the prevalence and incidence, demographic characteristics, and risk profiles of AF patients in Türkiye. Additionally, it will provide insights into how current treatments are reflected in this population. Furthermore, risk prediction modeling and risk scoring can be conducted for patients with AF.

Duman, Hakan; Ipek, Emrah; Durak, Hüseyin; Şahin, Müjgan Ayşenur; Ergül, Elif; Yilmaz, Ahmet Seyda; Bakirci, Eftal Murat
Background: This study assessed the association between a novel inflammatory marker, uric acid (UA)-to-albumin ratio (UAR), and preprocedural intracoronary artery thrombus (ICAT) in ST elevation myocardial infarction (STEMI). Material/Methods: A total of 171 STEMI patients treated by primary percutaneous coronary intervention between February and December 2023 were evaluated prospectively in this cross-sectional study. The patients were stratified into 2 groups as low (grades 1 to 3) and high-(ICAT) groups (grades 4 and 5). To determine the independent predictors of lower and higher ICAT, multivariate regression analysis was performed. Results: C-reactive protein (CRP), UA, and UAR were significantly higher in the high ICAT group (1.11 (0.3-2.8) vs 0.80 (0.10-2.8), P=0.037; 5.4 (3.5-7.2) vs 4.9 (3.4-5.6), P<0.001; 1.78 (0.82-3) vs 1.48 (0.77-2.57), P<0.001, respectively). However, albumin levels were similar between groups (3.1 (2.1-4.4) vs 3.3 (2.1-4.4), P=0.243). Higher UAR (OR: 3.95% CI: 1.23-12.7, P=0.021), lower left ventricular ejection fraction (LVEF) (OR=0.802; 95% CI 0.7537-0.872; P<0.001), longer pain-wire crossing time (OR=1; 95% CI: 1-1.02; P<0.001), and diabetes mellitus (OR=0.181; 95% CI 0.46-0.7; P<0.001) were independent predictors of ICAT. Conclusions: UAR, a marker of inflammation, is an independent predictor of ICAT in patients with STEMI.
