Comparative Study on the Protective Effect of Thiamine and Thiamine Pyrophosphate Against Hydroxychloroquine-Induced Cardiomyopathy in Rats

Emir, Izzet; Yavuzer, Bulent; Suleyman, Bahadir; Sarigul, Cengiz; Mendil, Ali Sefa; Sezgin, Esra Tuba; Altuner, Durdu; Gursul, Cebrail; Suleyman, Halis

Background: Hydroxychloroquine (HCQ), widely used in autoimmune and inflammatory diseases, has been associated with cardiotoxicity driven by oxidative, mitochondrial, and metabolic disturbances. However, no comparative evidence exists regarding whether thiamine, thiamine pyrophosphate (TPP), or their combination (TTPC) can mitigate HCQ-induced myocardial injury. Objective: This study examined the biochemical and histopathological effects of thiamine, TPP, and their combination in a rat model of HCQ-induced cardiomyopathy. Methods: Thirty male Wistar rats were assigned to five groups: healthy control, HCQ, thiamine + HCQ, TPP + HCQ, and TTPC + HCQ. Thiamine (20 mg/kg, intraperitoneal), TPP (20 mg/kg, intraperitoneal), or TTPC (20 mg/kg each, intraperitoneal) was administered once daily, followed by HCQ (120 mg/kg, oral, twice daily). After seven days, cardiac tissue was analyzed for MDA, tGSH, SOD, and CAT, while serum TnI, lactate, and LDH were measured from tail-vein blood samples. Cardiac samples underwent histopathological examination. Results: HCQ exposure markedly increased MDA, TnI, LDH, and lactate levels while reducing tGSH, SOD, and CAT, indicating severe oxidative and metabolic insult. Thiamine co-treatment failed to ameliorate these disturbances. Conversely, TPP restored redox balance, attenuated biomarker elevations, and improved cardiac biochemical profiles. TTPC produced comparable improvements but did not exceed those of TPP alone. Histopathologically, HCQ caused pronounced myocyte degeneration and mononuclear infiltration, whereas TPP and TTPC groups showed only mild inflammatory changes with preserved myocardial architecture. Conclusions: HCQ induces a marked redox imbalance accompanied by well-defined histopathological myocardial degeneration. TPP afforded robust cardio-protection, whereas thiamine offered no meaningful benefit. Collectively, these findings position TPP as a biologically plausible, clinically relevant candidate for mitigating HCQ-induced cardiomyopathy.

    Emir, İzzet; Aydın, Yıldız

This study aims to analyze the effects of features and classifiers in detecting cardiovascular diseases (CVD), which remain the leading cause of morbidity and mortality worldwide. Early and accurate detection of CVD significantly affects treatment outcomes. Therefore, the proposed method aims to automatically detect cardiovascular diseases via artificial intelligence. In this research, the performances of artificial intelligence methods for the cardiovascular disease detection problem are analyzed. The dataset used in this study was sourced from the publicly available Kaggle platform. It used for performance analysis in the developed application includes the features of 70000 patients such as age, gender, height, weight, blood pressure, cholesterol, glucose, smoking and alcohol use. These features were classified with Gradient Boosting, XGBoost, SVM, Random Forest, Logistic Regression, Convolutional Neural Network (CNN), Recurrent Neural Network (RNN), Long Short-Term Memory (LSTM) methods and performance comparison was performed. In the experimental results, the highest accuracy rate of 72.55% was obtained using the Gradient Boosting method, demonstrating its superior performance in cardiovascular disease detection. In addition, it was observed that the classification performance decreased when the high blood pressure attribute was removed from the dataset, while the removal of other features did not significantly affect the performance.

Comparison of Postoperative Anxiety, Depression, Somatization, and Somatosensory Perception Levels of Patients After Off-Pump and On-Pump Coronary Artery Bypass Graft Surgery

Emir, Izzet; Simsek, Meltem Hazel; Isik, Nurten Arslan; Onk, Oruc Alper

Introduction: This study investigated the effect of coronary artery bypass grafting surgery performed using either the on-pump or off-pump technique on the levels of postoperative anxiety, depression, somatization, and somatosensory amplification in patients. Methods: There were 100 participants in this cross-sectional study who completed the Sociodemographic Data Form, the Hospital Anxiety and Depression Scale, the Somatization Scale (SS), and the Somatosensory Amplification Scale. Results: Compared with the off-pump group, the on-pump group had a higher mean SS score. Conclusion: Because the mean SS score was lower in patients after off-pump surgery, it may be more advantageous to perform off-pump surgeries in the future. Before the superiority of the off-pump over the on-pump can be definitively established, however, data from large, randomized trials are required.

Effect of thiamine pyrophosphate on oxidative damage in the brain and heart of rats with experimentally induced occlusion of the common carotid artery.; [Efecto del pirofosfato de tiamina sobre el daño oxidativo en el cerebro y el corazón de ratas con oclusión inducida experimentalmente de la arteria carótida común.]

Emir, Izzet; Suleyman, Zeynep; Suleyman, Halis

It is known that a sudden increase in cerebral blood flow (hyperper-fusion) with carotid revascularisation may disrupt and damage the blood-brain barrier. This study aimed to explore thiamine pyrophosphate’s (TPP) protective effects against potential brain and heart damage resulting from carotid cross-clamping and unclamping in rats. The animals were divided into common carotid cross-clamping and unclamping (CCU), TPP+common carotid cross-clamping and unclamping (TCCU), and sham operation (SG) groups. The TCCU group received an intraperitoneal injection (IP) of 20 mg/kg TPP one hour before anesthesia. The CCU and SG groups received distilled water as a solvent. Ischemia was induced by maintaining the clips closed for 10 min. For the SG group, only a subcutaneous incision was made. Afterward, the clips were removed, the incisions were stitched, and reperfusion was continued for six hours. Subsequently, the rats were euthanized with high-dosage general anesthesia, and heart and brain tissues were removed. TPP significantly suppressed the I/R-induced malondialdehyde (MDA) increase and decreased total glutathione (tGSH) levels in brain and heart tissues. TPP prevented the increase of tumor necrosis factor-alpha (TNF-α), interleukin-1 β (IL-1β), and interleukin-6 (IL-6) levels in both brain and heart tissues. In blood serum, TPP suppressed I/R-induced increase in troponin I (TP I) and creatine kinase-MB (CK-MB) in the blood. TPP was shown to protect the brain and distant cardiac tissues against oxidative and inflammatory damage induced by cerebral I/R.

Ölmeztürk Karakurt, Tülay Ceren; Emir, İzzet; Bedir, Zehra; Ozkaloglu Erdem, Kezban Tuna; Süleyman, Halis; Sarıgül, Cengiz; Mendil, Ali Sefa

Aim: We aimed to investigate the preventive effects of carvacrol against ketamine-induced cardiotoxicity biochemically and histopathologically in an experimental model. Material and method: The rats were divided into three groups; healthy control (HC), ketamine alone (KG), and ketamine + carvacrol (KCG) groups. Serum Creatine Kinase Myocardial Band (CK-MB) and Troponin I (TP I) levels were determined. Malondialdehyde (MDA), Glutathione (GSH), Superoxide Dismutase (SOD), Tumor Necrosis Factor α (TNF-α), Interleukin 1 beta (IL-1beta), and Interleukin 6 (IL-6) levels were measured in the heart tissues of the rats. Heart tissues were also evaluated histopathologically. Results: In the ketamine-treated group, tissue MDA, TNF-α, IL-1beta, and IL-6 levels increased while tissue GSH and SOD levels decreased significantly compared with the control group. However, in the ketamine plus carvacrol applied group, all those alterations were significantly less pronounced, close to the healthy controls. Severe mononuclear cell infiltrations, degenerated myocytes and hemorrhage were determined in the ketamine alone administered group, and these alterations were at a mild level in the carvacrol + ketamine administered group. Conclusion: Prolonged exposure to ketamine resulted in induced oxidative stress in rat heart tissue; concomitant carvacrol application could counteract the negative effects of ketamine by protecting tissues from lipid peroxidation and decreasing the inflammatory response.

Emir, Izzet; Bulut, Seval; Suleyman, Bahadır; Mammadov, Renad; Yucel, Nurinisa; Cicek, Betul; Yazici, Gulce Naz; Altuner, Durdu; Gunay, Murat; Suleyman, Halis

Aim: The current study aimed to investigate the protective effects of adenosine triphosphate (ATP), metyrosine, and melatonin on possible methylphenidate cardiotoxicity in rats using biochemical and histopathological methods. Methods: Thirty rats were separated into five groups: healthy (HG), methylphenidate (MP), ATP + methylphenidate (ATMP), metyrosine + methylphenidate (MSMP), and melatonin + methylphenidate (MLMP). ATP (5 mg/kg) was given intraperitoneally once daily, metyrosine (50 mg/kg) orally twice daily, and melatonin (10 mg/kg) orally once daily. Methylphenidate (10 mg/kg) was administered orally once daily for 1 h after ATP, metyrosine and melatonin. The protocol was repeated for 30 days. Subsequently, blood samples were taken from the tail veins of the animals to measure adrenaline, noradrenaline, dopamine, troponin I (TP I) and creatine kinase MB (CK-MB) levels; the animals were then euthanized and the heart tissues were extracted. Tissues were analyzed for malondialdehyde (MDA), total glutathione (tGSH), superoxide dismutase (SOD), and catalase (CAT) and histopathologically. Results: In MP group, MDA, adrenaline, noradrenaline, dopamine, TP I, and CK-MB levels increased (p < 0.001) and tGSH, SOD, and CAT levels decreased (p < 0.001) compared to HG, and histopathologic damage developed. Oxidant levels were lower and antioxidant levels were higher in ATMP, MSMP, and MLMP groups compared to MP group (p < 0.001). Catecholamine levels were measured lower in the MSMP group compared to the MP group (p < 0.001). TP I and CK-MB levels were lower in ATMP, MSMP and MLMP groups compared to MP (p < 0.05), with the lowest being in rats given ATP (p < 0.001). ATP, melatonin, and metirozin applications were effective to different degrees in preventing histopathological changes. Conclusion: This study may guide clinical trials using ATP and melatonin to prevent methylphenidate-induced myocardial injury.

Effect of Lercanidipine, Carvacrol and Their Combination on Ischemia and Reperfusion-induced Distant Organ (Heart) Damage in Rats

Emir, Izzet; Altuner, Durdu; Suleyman, Bahadir; Mammadov, Renad; Suleyman, Zeynep; Coskun, Resit; Coban, Taha Abdulkadir; Suleyman, Halis

Background and Objective: Ischemia/reperfusion (I/R) damage has still been a reason for high morbidity and mortality for patients. Systemic damage is also observed in addition to local damage in lower extremity I/R event. It was purposed to research the protective effect of lercanidipine, carvacrol and lercanidipine, carvacrol their combination (LCC) against cardiac damage possible to develop after the I/R procedure administered to the lower extremities in rats. Materials and Methods: All rats were assigned into five groups, as sham operation (SG), only performed ischemia/reperfusion (I/R) to the lower extremity (IRG), lercanidipine+I/R (LIR), carvacrol+I/R (CIR) and lercanidipine, carvacrol combination+I/R (LCIR), randomly. One hour prior to anesthesia, 10 mg kg(-1) of lercanidipine was given to the LIR group. Carvacrol (50 mg kg(-1)) was injected to the CIR group. Lercanidipine (10 mg kg(-1)) was administered and carvacrol (50 mg kg(-1)) to the LCIR group. In IRG, LIR, CIR and LCIR groups, the ischemia was induced-occluding the femoral-artery for 4 hrs, reperfusion was for 24 hrs. Results: It was observed that malondialdehyde (MDA), troponin I (TPI), Creatine Kinase (CK-MB) and Interleukin 6 (IL-6) levels were lower in the heart tissue applied to LCC and total glutathione, superoxide dismutase and catalase levels were higher than the ones in the groups treated with lercanidipine and carvacrol. Conclusion: Current study suggested that lercanidipine and carvacrol were efficient in the treatment of heart injury, but LCC provided a more efficient treatment. These findings indicated that LCC alone was possible to be better than lercanidipine and carvacrol in reducing the severity of I/R-induced heart damage that occurred in any part of the body.

Evaluation of aortic stiffness by echocardiography in tympanosclerosis patients

Hamur, Hikmet; Onk, Oruc Alper; Coskun, Resit; Cosgun, Muharrem Said; Salcan, Ismail; Emır, Izzet

Background and Aims. Tympanosclerosis (TS) is a scarring process that may occur during otitis media. Aortic stiffness (AS) is a significant predictor for the development of heart diseases due to its close relationship with atherosclerosis. Similar pathophysiological processes based on inflammation may explain both TS and AS formation. This study aimed to determine echocardiographically whether aortic elasticity is impaired in TS-detected patients and to correlate blood inflammatory parameters with TS and aortic elasticity. Methods. Ninety-eight participants diagnosed with chronic otitis media were enrolled in the study. TS-detected 42 participants were assigned to the study group, while 56 without TS constituted the control group. The two groups' demographic, clinical, echocardiographic, and laboratory characteristics were comparable. Results. Demographic, clinical, and laboratory parameter differences were insignificant. Hs-CRP, neutrophil-to-lympho-cyte ratio, and systemic immune-inflammation index were significantly higher in the study group than in the control group (P=0.018, P=0.003, P=0.019, respectively). The study group had significantly lower aortic strain (11.80 ± 4.84 vs. 16.30 ± 3.91; P<0.001) and distensibility (5.23 ± 2.68 vs. 7.24 ± 2.89; P=0.001) values than the control group. The AS index was significantly higher in the study group than in the control group (4.81 ± 2.41 vs. 3.12 ± 1.02; P<0.001). Conclusion. In TS-detected patients, AS parameters were found to be impaired. Aortic elasticity parameters measured by echocardiography, a non-invasive and easily accessible method, may signify early cardiovascular involvement in TS-developed patients.

The Effect of Preoperative Anxiety on Postoperative Symptoms in Patients Without a History of Anxiety Scheduled for Coronary Artery Bypass Grafting

    Isik, Nurten Arslan; Emir, Izzet

The aim of this study was to measure the level of preoperative anxiety in patients scheduled for coronary artery bypass grafting (CABG) to assess the relationship between anxiety and postoperative symptoms. Materials and Methods. This descriptive study was conducted at a single university hospital from February to November 2021. Seventy-four CABG patients who completed the Anxiety Specific to Surgery Questionnaire, the Visual Analogue Scale, the Modified Borg Scale, the Rhodes Index of Nausea, Vomiting and Retching were included in the study. The data were analyzed by the mean, standard deviation, frequency distribution, Mann-Whitney U Test, and Spearman's correlation analysis. Results. The average age of participants was 65.55 +/- 8.35; 66.2% of them were males. The ASSQ total scores of female participants were significantly higher as compared to male participants (p < 0.05). There was a statistically significant positive relationship between the levels of anxiety in patients before CABG and the levels of postoperative pain and dyspnea severity (p < 0.05). Conclusions. Patients were found to experience predominantly moderate to severe anxiety before CABG, and increased dyspnea complaints and pain in the postoperative period. A coordinated, multidisciplinary approach to preoperative training of healthcare professionals may offer a promising way to provide more efficient and productive services.
