OBJECTIVE: Benign paroxysmal positional vertigo is the most common peripheral vestibular disorder. It can recur despite successful maneuvers. While various risk factors for recurrence have been investigated, these remain controversial. The aim of this study was to identify potential risk factors for benign paroxysmal positional vertigo recurrence and contribute to the development of preventive treatment strategies. METHODS: The study involved 301 patients who presented to our clinic with dizziness and were diagnosed with benign paroxysmal positional vertigo between December 2019 and May 2022. Two groups, with and without benign paroxysmal positional vertigo recurrence, were compared in terms of age, gender, hypertension, diabetes mellitus, hyperlipidemia, cardiovascular disease, asthma, hypothyroidism, magnesium deficiency, vitamin D deficiency, vitamin B12 deficiency, and iron deficiency to investigate the effects of these factors on recurrence. RESULTS: Recurrence was observed in 38 (12.6%) of the 301 patients. No age or gender difference was determined between the groups. Univariate analysis showed that hypertension (55.3 vs. 29.3%; p=0.003), vitamin B12 deficiency (21.1 vs. 6.5%; p=0.007), hyperlipidemia (26.3 vs. 5.7%; p<0.001), and asthma (15.8 vs. 5.7%; p=0.035) were more common in patients with recurrence. Multivariate logistic regression analysis identified only hyperlipidemia as a risk factor, raising the risk of recurrence approximately 3.7-fold (OR 3.71; 95%CI 1.36–10.13; p=0.011). CONCLUSION: Consideration of underlying comorbidities and systemic factors, effective management of existing comorbidities, and individualized treatment strategies are crucial in order to reduce benign paroxysmal positional vertigo recurrence rates and improve patients’ quality of life.

Frontal recess is an important region in endoscopic surgery in terms of its complex anatomic structure and excessive pneumatized cell variations. With the International Frontal Sinus Anatomy Classification (IFAC), classification and nomenclature of cellular variations in this region have been facilitated. The aim of the present study was to determine the incidence of frontal recess cell variations in computed tomography (CT) according to IFAC and to investigate the possible effect of these cells on the development of frontal sinusitis. In this observational, retrospective, and cross-sectional study, the frontal recess cell variations on both sides of the patients who underwent paranasal sinus CT examination were analyzed according to IFAC, and the possible relationship between these cellular variations and the presence of frontal sinusitis was evaluated by statistical methods. A total of 428 sides of 214 paranasal sinus computed tomography were examined. Agger nasi cell (ANC) was observed in 90.8%, while supra agger cell (SAC) were detected in 57.9% and supra agger frontal cell (SAFC) in 20.5%. Supra bulla cell (SBC) were observed in 80.1% of the sides and supra bulla frontal cell (SBFC) in 19.3%. Supraorbital ethmoid cell (SOEC) were detected on the 41.8% of sides, while frontal septal cells (FSCs) were observed in the 28.9%. The results of the present study showed that the prevalence of cells, especially other than ANC cells varies between populations. In addition, the presence of SAC, SAFC, SBC, SBFC, and SOEC appeared to be associated with a tendency to the development of frontal sinusitis.

Altıparmak, Serkan; Kars, Ayhan
BACKGROUND: The pan-immune-inflammation value (PIV) is a recently described novel biomarker obtained from complete blood count parameters. Systemic biomarkers have been investigated in a number of studies aimed at confirming the role of the inflammatory process in the etiology of Bell’s palsy (BP) and elucidating the association with prognosis. This study investigated the relationship between PIV and BP. METHODS: Sixty patients aged 18-75 years presenting with BP and 51 healthy controls with similar age and sex profiles were included in this retrospective study. Pan-immune-inflammation value and systemic immune-inflammation index (SII) values were calculated and compared between the 2 groups. The severity of paralysis in the BP group was recorded according to the House–Brackmann (HB) grading system and compared between the 2 groups. RESULTS: Both PIV and SII values were significantly higher in the BP group (P < .001). Intra-group comparisons based on paralysis grading revealed a significant difference in SII values in grades IV and V (P = .04). CONCLUSION: The findings suggest that PIV may represent a useful and readily available biomarker reflecting systemic inflammation in patients with BP. However, larger prospective studies are now required to confirm its diagnostic and prognostic values.
RDL Kalıcı Kaydıİncele - ArticleYalnız künye2026Modification for the Repair of Nasal Septal Perforation: A Bipediculated Crescent-Shaped Septal Flap
Sahin, Abdulkadir; Sakat, Muhammed Sedat; Kilic, Korhan; Kars, Ayhan
Nasal septal perforation (NSP) is an anatomic defect located in the nasal septal cartilage/bone and mucosa. The most common cause of NSPs is septoplasty. Repair of NSPs is difficult regardless of the surgical technique due to their location and the health of the surrounding tissues. Currently, no technique has been defined to be used for the closure of all NSPs. We aimed to present our bipediculated crescent-shaped unilateral slide flap technique, which we designed and used as a modification for closure of NSPs, and its surgical results. A retrospective medical record review of patients who underwent NSP repair in our clinic was performed. All patients underwent a surgical procedure with the same technique by the same surgeon. Thirty-six patients were included in the study. The mean age of the patients was 35.58±9.6. The most common cause of NSP was previous septal surgeries. The NSP dimensions of the patients were between 5 and 23 mm. The NSP closure rate was determined as 94.4% in the 6-month follow-up results after surgery. A bipediculated crescent-shaped unilateral sliding flap is an effective, easy, and modification in the repair of NSPs.

Background: Emergence delirium (ED) is a common postoperative complication in the pediatric population. Although the pathophysiology of ED has not been fully elucidated, an association has been suggested between inflammation and the development of the condition. Aims: The aim of this study was to investigate the associations between the development of ED and preoperative inflammatory indices, primarily the systemic immune inflammation index (SII) and the pan-immune inflammation value (PIV) calculated from complete blood count parameters in pediatric patients undergoing adenoidectomy or adenotonsillectomy. Methods: This prospective study was conducted with children aged 3–12 years who underwent adenoidectomy or adenotonsillectomy at our hospital. Preoperative inflammatory indices were calculated from complete blood count parameters, including the SII (platelet × neutrophil/lymphocyte), PIV (platelet × neutrophil × monocyte/lymphocyte), neutrophil-to-lymphocyte ratio (NLR), neutrophil-to-monocyte ratio (NMR), monocyte-to-lymphocyte ratio (MLR), platelet-to-lymphocyte ratio (PLR), and platelet-to-monocyte ratio (PMR). ED was assessed using the Pediatric Anesthesia Emergence Delirium (PAED) scale, while the Face, Legs, Activity, Cry, Consolability (FLACC) scale was used to minimize confounding by postoperative pain. Statistical analyses included receiver operating characteristic (ROC) curve analysis and multivariable logistic regression to assess the association between inflammatory indices and emergence delirium. Results: ED occurred in 32 out of 99 patients (32.3%). SII exhibited an AUC of 0.850 with a cut-off value of 412 (sensitivity 81.3%; specificity 86.6%). PIV exhibited an area under the curve of 0.883, with a cut-off value of 289 (sensitivity 75.0%; specificity 92.5%). In the univariable analysis, operative time, SII, and PIV were significantly associated with ED. In the multivariable analysis, operative time, SII and PIV were significantly associated with ED. Conclusions: SII and PIV values derived from routine complete blood count parameters were observed to be associated with the development of ED in pediatric patients undergoing adenotonsillectomy and may provide useful insights into the potential role of systemic inflammation in the pathophysiology of such delirium.
RDL Kalıcı Kaydıİncele - ArticleYalnız künye2026A Comparison of the Functional Efficacy of the Spreader Graft and Spreader Flap Techniques Used in Open Rhinoplasty Based on Nasal Obstruction Symptoms Evaluation (NOSE) Scores
Aksakal, Ceyhun; Kars, Ayhan; Altiparmak, Serkan
The aim of this study was to measure postoperative functional satisfaction in patients who underwent spreader graft or spreader flap procedures using the standardized Nasal Obstruction Symptoms Evaluation Score (NOSE) and to compare the 2 methods. Preoperative and early (1–3 mo), middle (4–6 mo), and late (≥9 mo) postoperative NOSE scores were compared in patients who underwent open rhinoplasty with the spreader graft (group 1) or spreader flap (group 2) techniques at our hospital. The patients’ demographic and surgical data were also analyzed. The study involved 63 patients who met the inclusion criteria. The spreader flap technique was used in 34 cases (29% female, mean age: 27.0±4.7 y) and the spreader graft in 29 (31% female, mean age: 26.2±5.4 y). The mean NOSE scores improved significantly in both groups at postoperative early evaluations (21.4±9.5 in the spreader flap and 18.7±14.9 in the spreader graft groups) compared with preoperative values (77.6±13.7 in the spreader flap and 76.9±15.4 in the spreader graft groups) (P<0.001). However, there were no significant differences in evaluations among the early, middle, and late postoperative periods. The study results revealed no significant difference between spreader graft and flap use in terms of improvement in functional outcomes. Both techniques are therefore functionally effective in rhinoplasty.

RDL Kalıcı Kaydıİncele - ArticleYayıncıda açık erişim2026Vestibular Dysfunction and Disease Activity in Rheumatoid Arthritis: A Controlled cVEMP Study
Kars, Ayhan; Atalay, Fatma; Kocaer, Sinem Burcu; Anbar, Sezai Sacid; Altıparmak, Serkan; Kılıc, Korhan; Topal, Kübra
Objective: This study aimed to evaluate vestibular function and the relationship between vestibular dysfunction, if present, and disease activity in rheumatoid arthritis (RA) patients without vestibular symptoms, using the cervical vestibular evoked myogenic potentials (cVEMP) test. Materials and methods: This prospective study was conducted with 44 RA patients without vestibular symptoms and 26 healthy volunteers. All participants underwent a detailed ENT examination and the cVEMP test. Erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) values were recorded on the date of the test, and disease activity score (DAS)28-CRP values were calculated. cVEMP recordings were then compared with ESR, CRP, and DAS28-CRP values. Results: A cVEMP response was observed in 29.6% of the RA group, a unilateral cVEMP loss being determined in 22.7% and bilateral loss in 47.7%. All members of the control group achieved a response. This difference was statistically significant (p < 0.001). When disease activity was compared with cVEMP response status, no statistically significant differences were observed in terms of remission, or of low and moderate activity rates (p > 0.05). Conclusion: This study showed that vestibular dysfunction is widespread among RA patients, but no statistically significant relationship was determined between cVEMP findings and DAS28-CRP, ESR, or CRP values reflecting disease activity. These findings suggest that vestibular system involvement in RA may be independent of acute disease activity and more related to chronic inflammatory and immune-mediated processes.

Objective: The aim of this retrospective study is to describe the clinical features and management of patients diagnosed with deep neck infection (DNI) and hospitalized in our clinic, and to share our experience, together with a review of the current literature. Methods: Patients who were diagnosed with DNI and treated in our clinic between January 2020 and June 2025 were retrospectively analyzed. Inclusion criteria: age > 18 years, complete medical records, diagnosis confirmed by clinical, laboratory, and imaging findings. Exclusion criteria: immunosuppressed patients, missing data, and pediatric cases. Diagnosis was based on clinical examination, laboratory tests, and imaging. Intravenous antibiotics were administered to all patients: incision and drainage or repeated aspiration was performed when abscesses were detected. Patients were followed for at least 3 months after discharge. Results: Sixty-two patients, 36 male and 26 female, were included in the study. Analysis of the etiological factors causing DNI revealed that 25 (40%) cases were odontogenic in origin and 19 (30.6%) derived from tonsillitis. Surgical drainage was required in 50% of patients. The overall complication rate was 9.7%, and no mortality occurred. Conclusion: Odontogenic infections were the leading cause of DNIs, followed by tonsillitis. Complete recovery was achieved with antibiotics alone in 50% of patients, and surgical intervention was successful in all remaining cases, resulting in 100% overall cure without recurrence. Early diagnosis and prompt combined management are key to excellent outcomes.

Kars, Ayhan; Atalay, Fatma; Topal, Kubra; Anbar, Sezai Sacid; Akyel, Fatma; Kiliç, Korhan
Purpose: This research was conducted to assess cervical vestibular evoked myogenic potential (cVEMP) test responses in patients with primary hypothyroidism without vestibular symptoms. Materials and Methods: Thirty-one patients diagnosed with primary hypothyroidism and who had not yet started on hormone replacement therapy, together with a 26-member control group, were enrolled in the research. All participants underwent detailed otorhinolaryngological examinations and thyroid function, cVEMP, and audiological tests. cVEMP records were compared between the groups. Results: Thirty-one patients aged between 21 and 57 years (38.5 +/- 9.7) and a control group of 26 healthy volunteers aged between 20 and 52 years (36.1 +/- 8.8) were included in the study. Absent VEMP responses were observed in 12 members of the patient group and in three of the control group. Interpeak amplitudes (IPAs) in both ears were significantly lower in the patient group than in the controls. While no significant difference was found between the groups' VEMP threshold values, asymmetry rates in both ears were significantly higher in the patient group. Conclusion: cVEMP can be usefully employed in the detection of subclinical vestibular dysfunction in hypothyroid patients.
