Non-Motor Symptoms in Gabapentin Users: A Clinical Evaluation

    Tanoglu, Ceyda; Ersoy, Alevtina

Objective: Gabapentin is widely used for neuropathic pain, epilepsy, and other indications; however, its non-motor side effects remain understudied. This prospective, cross-sectional observational study aimed to evaluate non-motor symptoms in gabapentin users, assessed with the Non-Motor Symptom Scale (NMSS). Methods: The Turkish version of the NMSS was administered once to 105 patients who had been receiving gabapentin for at least 120 days and to 107 demographically matched healthy controls at our Non-Motor Symptom Scale (NMSS) between January and December 2022. Exclusion criteria included neurodegenerative diseases, diabetes, psychiatric disorders, and thyroid dysfunction. Group comparisons were performed using independent samples t-tests for continuous variables and chi-square or Fisher’s exact tests for categorical data. Results: Gabapentin users had significantly higher total NMSS scores than controls (11.6 ± 5.56 vs. 6.03 ± 4.28, P < 0.001). Symptoms such as salivation, constipation, urgency, nocturia, pain, memory impairment, depression, anxiety, sexual dysfunction, orthostatic hypotension, daytime sleepiness, and leg edema were reported more frequently in the gabapentin group (P < 0.05). Conclusion: Gabapentin use was associated with a higher frequency of non-motor symptoms, as assessed by the NMSS. This study suggests that the NMSS may serve as a structured tool for the systematic screening of non-motor symptoms in patients receiving gabapentin. Prospective studies with indication-matched controls and validated assessment tools are warranted to confirm these associations.

Is “normal” ferritin truly enough? Strong correlation between iron stores and RLS severity in patients with normal ferritin levels

    Tanoğlu, Ceyda; Ersoy, Alevtina

Objective We aimed to investigate the relationship between peripheral iron stores and symptom severity in drug-naive patients with idiopathic Restless Legs Syndrome (RLS) whose serum ferritin levels were within the laboratory reference range. Additionally, we sought to determine the independent effects of age, sex, comorbidities, and biochemical parameters in predicting RLS severity. Materials and methods This cross-sectional study was conducted at the neurology outpatient clinic of a single university hospital. A total of 113 drug-naive patients with idiopathic RLS were prospectively enrolled during clinical visits. Patients with ferritin levels outside the laboratory reference range (22–322 μg/L) or with major confounding comorbidities (e.g., peripheral neuropathy, renal failure, pregnancy) were excluded to ensure sample homogeneity Demographic data and comorbidities were recorded through face-to-face interviews. Disease severity was assessed by a neurologist using the International Restless Legs Syndrome Rating Scale. Ferritin, magnesium, vitamin B12, vitamin D, folate; and hemoglobin were analyzed in fasting venous blood samples. Multiple linear regression analysis was used to identify independent predictors of symptom severity. Results The mean age of the participants was 52 ± 13.8 years, and their median ferritin level was 51.4 μg/L. Multiple regression analysis identified serum ferritin level (p < 0.001) and age (p = 0.001) as independent and significant predictors of RLS severity. No independent effects of sex, hemoglobin, vitamin D, or other comorbidities on severity were detected. Conclusion Even when serum ferritin levels are within the so-called “normal” limits, a very strong negative correlation exists between ferritin level and symptom severity. Our findings highlight the importance of evaluating ferritin not merely as a threshold value in RLS management but as a continuous parameter that can be associated with symptom severity even within normal ranges.

Cerebellar cavernous angioma associated with developmental venous anomaly presenting as a cerebellopontine angle neoplasm in CT scan

Ogul, Hayri; Gozgec, Elif; Demirdogen, Filiz; Kantarci, Mecit

Association of developmental venous anomaly (DVA) and cavernous malformation (CM) of the posterior fossa is rare. If bleeding occurs, it must be assigned to the CM. Developmental venous anomalies, considered as normal vascular variation, are rarely symptomatic. Indeed, the CMs are true malformations and have a high bleeding potential. © The Author(s) 2023. Effects of Short- and Long-Term Vortioxetine Administration on Reproductive Function in Female Rats

Bingul, Eda; Hizal, Elif; Keles, Ozgecan; Yucel, Nurinisa; Suleyman, Zeynep; Gunay, Murat; Yasar, Hasan; Suleyman, Halis

Background/Objectives: Vortioxetine is a multimodal antidepressant that modulates serotonergic, noradrenergic, and dopaminergic systems, yet its effects on female reproductive physiology remain incompletely defined. This study investigated how short- and long-term vortioxetine exposure influences reproductive function in female rats, integrating measurements of oxidative-stress biomarkers, catecholamines, and endocrine endpoints (prolactin and corticosterone). Methods: Forty-two female Wistar albino rats were randomized into seven groups (n = 6): healthy control; short-term vortioxetine; long-term vortioxetine; and the same two regimens each combined with metyrosine or metyrapone. Metyrosine and metyrapone (50 mg/kg, oral gavage) were given 1 h before vortioxetine (10 mg/kg). Treatments lasted one week (short-term) or four weeks (long-term). Serum MDA, tGSH, SOD, CAT, adrenaline, noradrenaline, dopamine, serotonin, prolactin, and corticosterone were measured. Fertility outcomes were assessed by co-housing females with males for one month. Results: Vortioxetine increased MDA, catecholamines, and serotonin and decreased antioxidant markers and prolactin versus controls (p < 0.001). Long-term vortioxetine produced a marked rise in corticosterone that coincided with infertility and delayed parturition. Metyrosine lowered oxidative stress and catecholamines but did not reverse infertility. In contrast, metyrapone blunted corticosterone elevation and preserved reproductive capacity. Conclusions: Long-term vortioxetine induced infertility in female rats, likely mediated by corticosterone elevation consistent with hypothalamic–pituitary–adrenal axis dysregulation. These findings suggest the need to monitor reproductive risk when considering vortioxetine in women of reproductive age and warrant further mechanistic and translational studies.

Safety and effectiveness of Rivaroxaban, Dabigatran and Apixaban in patients with non-valvular atrial fibrillation for stroke prophylaxis

    Tanoğlu, Ceyda; Ersoy, Alevtina

Background: Atrial fibrillation is the most common arrhythmia that causes an increased risk of thromboembolism. We aimed to evaluate stroke and major bleeding risk in patients with atrial fibrillation using rivaroxaban, apixaban, dabigatran and the effects of using antiplatelet, atorvastatin and proton pump inhibitor (PPI) on development of stroke. Methods: Patients who were administered rivaroxaban, dabigatran or apixaban for atrial fibrillation between June 2014 and December 2020 were retrospectively analysed. Demographic data, CHADS2 and CHA2DS2-VASc scores, HAS-BLED scores, antiplatelet, proton pump inhibitor, atorvastatin medications were evaluated. Furthermore, we evaluated the risk of major bleeding and stroke during treatment. Results: We investigated 162 patients using dabigatran, 255 patients using rivaroxaban and 104 patients using apixaban. No significant difference was observed between the groups in terms of CHA2DS2-VASc scores and the use of atorvastatin, proton pump inhibitor and antiplatelet. HAS-BLED scores before DOACs treatment were statistically significantly higher in the apixaban group compared to rivaroxaban and dabigatran groups (p = 0.038); we found no difference between the study groups in terms of major bleeding (p = 0.528) and stroke risk (p = 0.498). The use of antiplatelet, proton pump inhibitor and atorvastatin did not have a significant effect on stroke risk (p = 0.533, p = 0.169 and p = 0.949). Conclusion: Rivaroxaban, dabigatran and apixaban have similar safety and efficacy for stroke prophylaxis. The use of antiplatelet, proton pump inhibitor and atorvastatin did not have a significant effect on stroke risk.

Time Between Symptom Onset and Emergency Department Arrival and Thrombolytic Treatment Rates in Patients with Acute Ischemic Stroke

    Ersoy, Alevtina; Işık, Bahar

Objective: The objective of this research was to reveal the interval from the start of an acute ischemic stroke (AIS) to the patient’s arrival at the reference center, defined as time to treatment, as well as to identify the reasons for delayed presentation when appli- cable. We also wanted to elucidate whether there were changes in the time to treatment in individuals with a prior diagnosis of AIS compared to those who presented with the diagnosis of AIS for the first time. Additionally, we aimed to assess the rate of thrombo- lytic treatment administered to the patients. Methods: This prospective study was conducted with patients aged 18 and above diagnosed with ischemic AIS in the emergency department (ED) during a 3-month period. In total, 150 patients were included in the study. After obtaining written consent, responses of patients were recorded on the data collection form through direct communication or, in the case of those unable to respond, from their close relatives. Radiological data were obtained from hospital records. Statistical Package for the Social Sciences (SPSS) version 17.0 program was used for data analysis. Results: Among the patients, there were 88 males and 62 females, with an average age of 71.97 ± 13.53 years. From the total group of 150 people, 105 (70%) were experiencing an acute ischemic stroke (AIS) for the first time, while 45 individuals (30%) had a his- tory of prior AIS diagnoses. The mean time to apply to the hospital for treatment was 11.34 ± 19.63 hours. In patients with a history of prior AIS, the mean time to apply for treatment was 11.31 ± 20.2 hours. The number of patients admitted to the ED within the first 1 hour was 30 (20%), while the count of patients who were admitted to the ED within the initial 3 hours was 72 (48%). The rate of those admitted within the first 4.5 hours was 54.7%. The proportion of individuals with a university education among those who presented within the first 3 hours was higher compared to those who did not seek medical attention within the first 3 hours. No significant difference was observed in other parameters examined. The number of patients who received thrombolytic treatment was 49 (32.67%). As a reason for late admission, patients (27%) mostly stated that they waited with the idea that their symptoms would go away on their own. Those who were conscious of early application comprised only 3% of the patients. Conclusion: The findings showed that patients were not consciously aware of the need for early presentation in the case of AIS. Furthermore, onset-to-door time remained almost the same in patients who had previously presented with AIS, and this naturally reflected on the rate of thrombolytic treatment administration. There is a need to raise awareness about this issue within the com- munity, utilizing media outlets.

Yasar, Hasan; Cimen, Ferda Keskin; Suleyman, Halis; Kurt, Nezahat; Arslan, Yusuf Kemal

Background and Objective: Cisplatin is a drug used to treat cancer, but it causes oxidative damage to muscle tissue. Rutin has an antioxidant effect. The study aims to investigate the biochemical and histopathological effects of rutin on the oxidative damage of cisplatin in skeletal muscle tissue. Materials and Methods: Rats were divided into four groups: A control group (CG), an only cisplatin-administered group (CIS), a cisplatin and rutin 50 mg kg(-1)-administered group (R-50) and a cisplatin and rutin 100 mg kg(-1)-administered group (R-100). Analyses were performed from the hindlimb muscles of experimental animals. Analyses of malondialdehyde (MDA), Total Glutathione (tGSH), glutathione reductase (GSHRd), glutathione S-transferase (GST) and superoxide dismutase (SOD) were performed. Histopathologic examination was performed in all groups. The CG and the other groups were compared. Results: The CG and R-100 groups were similar in all biochemical examinations. A statistically significant difference was detected between the control group and the CIS and R-50 groups in all examinations. Histopathological examinations revealed oxidative damage of cisplatin in the CIS group and R-50 group. The findings in the R-100 group were similar to the control group. Conclusion: Cisplatin causes oxidative damage to skeletal muscle tissue. Rutin has the potential to prevent the formation of such oxidative damage.

Ozdemir, Gokhan; Eren, Fettah; Aygul, Recep; Kizildag, Nazım; Kocaturk, Idris; Mammadi, Azer; Ersoy, Ayse Nur; Ildiz, Omer Faruk; Gunduz, Zehra Betul; Korez, Muslu Kazım

Background: We sought to examine the feasibility, safety and preliminary efficacy of anterior cerebral artery (ACA) occlusions in patients undergoing endovascular treatment. Methods: Four hundred five consecutive patients with large-vessel occlusion treated with endovascular treatment were analysed to identify all patients with acute ACA occlusion who underwent endovascular treatment. Results: Twenty had ACA occlusion (primary ACA occlusion: 9, rescue ACA occlusion: 11), 395 patients had other occlusions (internal carotid artery and MCA). The median [IQR] mRS score in the third month was significantly higher in the ACA-rescue occlusion group versus the ACA-primary occlusion group. The rate of haematoma in patients with ACA-occlusions was significantly higher compared with the ACA-primary occlusion group. Moreover, the three-month mortality rate was higher in patients with ACA-rescue than the patients with ACA-primary. Conclusions: Although endovascular treatment can be considered in patients with primary ACA occlusions, our data suggest that future clinical trials are needed to determine the efficacy of endovascular treatment for ACA occlusions. Unfavourable outcomes in our study were considered to occur in the rescue ACA occlusions.

    Endovascular treatment for M3 occlusions

Ozdemir, Gokhan; Eren, Alper; Aygul, Recep; Eren, Fettah; Kizildag, Nazım; Kocaturk, Idris; Mammadi, Azer; Ersoy, Ayse Nur; Korez, Muslu Kazım

Introduction: Endovascular treatment (ET) is a beneficial treatment for M1–2 occlusions of the middle cerebral artery. Mortality and disability rates are high if large vessel occlusions are not treated. While these rates are lower in M3 occlusions, important branch blockages can lead to disability. Endovascular treatment of small vessel occlusions is difficult, and there are no studies on the effectiveness of endovascular treatment for M3 occlusions. Accordingly, in this study, our aim was to assess the feasibility, safety, and preliminary efficacy of endovascular therapy for M3 occlusions. Methods: This study involved a retrospective analysis of a prospectively collected from two centres for acute ischemic stroke of the anterior system between July 2015 and April 2020. Demographic, radiologic, procedural and outcome variables were collected for patients who underwent endovascular therapy for acute ischemic stroke of the anterior system. Results: Complete or near complete reperfusion (mTICI 2b-3) of the M3 occlusion was achieved in 15 cases (38.5%). Complete (mTICI 3) reperfusion was achieved in 24 cases (61.5%). Twenty-six patients were treated for primary M3 occlusion, while 13 patients with M3 occlusion were treated as a rescue strategy after successful treatment of a proximal greater vascular occlusion. Complete or near complete reperfusion (mTICI 2b-3) of primary occlusion was achieved in eight cases (30.8%), and complete (mTICI 3) reperfusion was achieved in 18 cases (69.2%). In addition, complete or near complete reperfusion (mTICI 2b-3) of rescue M3 occlusion was achieved in seven cases (%53.8), while complete (mTICI 3) reperfusion was achieved in six cases (46.2%). Only one patient with primary M3 occlusion had ICH due to extravasation. The patient's neurological examination one month later was normal. Conclusions: This retrospective study demonstrates that endovascular treatment of M3 occlusions is safe, effective and reliable.
