The Effects of Vaccines on the Sequelae Rates of Recurrent Infections and the Severity of Pulmonary COVID-19 Infection by Imaging

Bahadir, Suzan; Kabacaoglu, Ebru; Memis, Kemal Bugra; Hasan, Hasan Ilksen; Aydin, Sonay

Although vaccines have been shown to reduce the number of COVID-19 infection cases significantly, vaccine-related reactions, long COVID-19 syndrome, and COVID-19 infection following vaccination continue to be a burden on healthcare services and warrant further scientific research. The purpose of this study was to research the severity of pulmonary COVID-19 infection following vaccination and the sequelae rates of recurrent infections in vaccinated cases by imaging. Patients who underwent follow-up CTs at 1 month, 3 months, and 6 months in our hospital with a diagnosis of COVID-19 were scanned retrospectively. Furthermore, all essential information was gathered from patients’ immunization records. The major findings of our study were: (1) sequelae were frequently observed in unvaccinated cases; (2) the correlation between vaccination status and the severity of sequelae was significant; (3) there was not any significant relationship between the vaccine type and the severity of sequelae; and (4) hematocrit, hemoglobin, and lymphocyte parameters may be used as predictors of sequelae rates. COVID-19 infection, although reduced in prevalence following the development of vaccines, still remains a public health concern because of reinfection. Vaccination not only appears to protect against primary infection, but also seems to reduce reinfection and sequalae rates following reinfection.

Normal umbilical artery doppler values in 18–22 week old fetuses with single umbilical artery

Kazci, Omer; Aydin, Sonay; Fatihoglu, Erdem; Tokur, Oğuzhan; Bahadir, Suzan; Karavas, Erdal; Kantarci, Mecit

Umbilical cord with a single umbilical artery (SUA) can carry twice the blood volume of a three-vessel cord (TVC). So, the normal hemodynamics of the fetuses with SUA was different from those with TVC. Furthermore, structural abnormalities, fetal aneuploidy, and intrinsic growth retardation may be associated with the presence of a SUA. In order to evaluate these patients, intermittent doppler measurements have been suggested. From this point, we aimed to determine the CDUS flow parameters in SUA cases and to demonstrate that these flow parameters are different from the TVC parameters. Ultrasound (US) examinations were performed in the 18–22 weeks of gestation during routine fetal anatomy screening. Resistance index (RI), Pulsatility index (PI), and S/D: systole to diastole ratio values were measured. The samples were taken from the proximal, mid-portion, and distal of the umbilical cord. In addition to Doppler Ultrasound values, AC and estimated fetal weight (EFW) values were also recorded. The study included 167 pregnant women, 86 of whom were study group with SUA and 81 were control group with TVC. The measurements of RI, PI, and S/D at all three levels were significantly lower in the SUA group compared to the TVC group. The resistance in the UA of fetuses with SUA is lower than in fetuses with TVC. The resistance in the UA of fetuses with SUA decreases from the fetal end to the placental end. Knowing the normal values for fetuses with SUA might provide a better and more reliable Doppler Ultrasoundassessment.

Gokharman, Fatma Dilek; Kadirhan, Ozlem; Celik Aydin, Ozlem; Yalcin, Arzu Gulsah; Kosar, Pınar; Aydin, Sonay

Introduction: Orbital wall fractures that may develop in maxillofacial traumas (MFTs) may cause ophthalmic complications (OCs). The aim of this study is to determine the frequency of orbital fractures (OFs) accompanying MFTs and findings suspicious for orbital traumatic involvement. Materials and Methods: Computed tomography (CT) images of 887 patients who presented to the emergency department within a 1-year period with a history of MFT were retrospectively scanned. During the examination, patients with orbital wall fractures, craniofacial bone fractures, and posttraumatic soft tissue changes were recorded. Results: OF was observed in 47 (5.3%) of the patients admitted for MFT. In cases with OFs, accompanying nasal (25.5%), ethmoid (2.1%), frontal (19.1%), maxillary (38%), and zygomatic bone fracture (10.6%), sphenoid (4.3%), and soft tissue damage (55.3%) were observed. It was observed that the pathologies mentioned at these levels were significantly higher than in patients without orbital involvement (p < 0.05). In our study, mild (48.9%) and moderate-severe (2.12–4.25%) OCs accompanying OFs were observed after MFT. Conclusions: The frequency of MFT varies depending on various factors, and such studies are needed to take preventive measures. Knowing the risk and frequency of orbital damage accompanying MFTs may help reduce complications by allowing rapid and accurate diagnosis.

Computed Tomography-Guided Percutaneous Thoracic Biopsy: A Single-Center Experience

Karavaş, Erdal; Aydin, Sonay; Taydas, Onur; Ünver, Edhem; Aydemir, Hüseyin; Köroğlu, Mert

Objective: Imaging-guided percutaneous thoracic biopsy is the preferred method for the diagnosis of lesions that cannot be achieved with bronchoscopy. The main reason for this is that it is less invasive and risky than surgical procedures. It is most com monly done with the help of computed tomography. The biopsy can be taken as a true-cut or fine-needle aspiration biopsy for lung, mediastinum, and pleural lesions. The aim of this study was to share our experience of imaging-guided percutaneous tho racic biopsy in a single center. Methods: A total of 78 patients applied with biopsy in our cen ter between 2009 and 2014 were retrospectively screened. Computed tomography was used as the imaging method in percutaneous thorax biopsies. The selected needle was either a fine-needle or true-cut biopsy needle. Results: The patients comprised 67 males and 11 females with a mean age of 63.8 ± 11.5 years. There was no mortality due to treatment in any of the patients. Procedural complications were determined as lung contusion in 1 patient (1.3%) and pneumo thorax requiring aspiration in 3 patients (3.8%). According to the biopsy results, the most frequent pathological diagnosis was non-small-cell lung cancer (26.9%, n=21). In 3 patients (3.8%), the biopsy result was non-diagnostic. Conclusion: The positive diagnosis rate of percutaneous tho rax biopsy under imaging guidance has been reported to be 85-95%. Therefore, an imaging-guided percutaneous thoracic biopsy is a safe and reliable method with low complication rates in pulmonary, hilar, and mediastinal masses.

Thoracic computed tomography measures have predictive value in the diagnosis of chronic obstructive pulmonary disease

Karavaş, Erdal; Aydin, Sonay; Ünver, Edhem; Kupeli, Ali; Tosun, Mustafa

Chronic Obstructive Pulmonary Disease (COPD) is diagnosed with pulmonary function tests (PFTs). However, since not all patients can be diagnosed with PFTs, some are diagnosed with radiological or clinical findings. The purpose of this study was to define the properties of obstructive airway disease through thoracic computed tomography (CT) and to identify the diagnostic efficacy of CT findings. A total of 160 patients who underwent PFT and thoracic CT assessment July 2018 - January 2019, were retrospectively analyzed. Based on PFT findings, patients were categorized into three groups as having normal, restrictive or obstructive airways. Age, height, weight, and body mass indexes of the groups were recorded. Pulmonary height, width, right-left hemi-diaphragm height, sterno-diaphragmatic angle and retrosternal transparent area length in axial sections were also recorded. Diagnostic efficacies of these parameters in the detection of obstructive airway disease were measured. Of the 160 patients (109 males, 51 females; mean age = 59.5), 91 (56.9) had normal PFT, 58 (36.2%) had obstructive and 11 (6.9%) had restrictive airway disease. Pulmonary height, width, sterno-diaphragmatic angle, and retrosternal transparent area length were significantly higher in patients with obstructive airway disease while the right-left hemi-diaphragm height was significantly lower (p<0.001). About 60-75% sensitivity and specificity were obtained when identifying the obstructive airway disease with these parameters. These values obtained from the axial and sagittal sections could contribute to the diagnosis of obstructive airway disease.

Triple rule-out computed tomography angiography: Evaluation of acute chest pain in COVID-19 patients in the emergency department

Bahadir, Suzan; Aydin, Sonay; Kantarci, Mecit; Unver, Edhem; Karavas, Erdal; Senbil, Duezguen Can

BACKGROUND The aim of this study was to define clinical evidence supporting that triple rule-out computed tomography angiography (TRO CTA) is a comprehensive and feasible diagnostic tool in patients with novel coronavirus disease 2019 (COVID-19) who were admitted to the emergency department (ED) for acute chest pain. Optimizing diagnostic imaging strategies in COVID-19 related thromboembolic events, will help for rapid and noninvasive diagnoses and results will be effective for patients and healthcare systems in all aspects. AIM To define clinical evidence supporting that TRO CTA is a comprehensive and feasible diagnostic tool in COVID-19 patients who were admitted to the ED for acute chest pain, and to assess outcomes of optimizing diagnostic imaging strategies, particularly TRO CTA use, in COVID-19 related thromboembolic events. METHODS TRO CTA images were evaluated for the presence of coronary artery disease, pulmonary thromboembolism (PTE), or acute aortic syndromes. Statistical analyses were used for evaluation of significant association between the variables. A two tailed P-value < 0.05 was considered statistically significant. RESULTS Fifty-three patients were included into the study. In 31 patients (65.9%), there was not any pathology, while PTE was diagnosed in 11 patients. There was no significant relationship between the rates of pathology on CTA and history of hypertension. On the other hand, the diabetes mellitus rate was much higher in the acute coronary syndrome group, particularly in the PTE group (8/31 = 25.8% vs 6/16 = 37.5%, P = 0.001). The rate of dyslipidemia was significantly higher in the group with pathology on CTA while compared to those without pathology apart from imaging findings of the pneumonia group (62.5% vs 38.7%, P < 0.001). Smoking history rates were similar in the groups. Platelets, D-dimer, fibrinogen, C-reactive protein, and erythrocyte sedimentation rate values were higher in COVID-19 cases with additional pathologies. CONCLUSION TRO CTA is an effective imaging method in evaluation of all thoracic vascular systems at once and gives accurate results in COVID-19 patients.

Type 2 dynamic curves: A diagnostic dilemma

Karavas, Erdal; Ece, Bunyamin; Aydin, Sonay

BACKGROUND Magnetic resonance imaging (MRI) with multiparametric dynamic contrast plays a critical role in the assessment of breast lesions. Dynamic curves are a critical parameter in determining the benign or malignant nature of lesions. Dynamic curves of type 1 are known to represent benign masses, while dynamic curves of type 3 are known to identify malignant masses. Type 2 dynamic curves have a sensitivity of 42.6% and specificity of 75% for malignancy detection. AIM To investigate the pathological diagnosis of lesions with type 2 dynamic curves. METHODS We evaluated breast MRI examinations performed between 2020 and 2021 retrospectively and included lesions with type 2 dynamic curves. We included 38 lesions from 33 patients. The lesions were evaluated for their pathological diagnosis and morphological characteristics. RESULTS Twenty-six lesions were malignant, while twelve were benign. The most frequently encountered benign lesion (7/12, 58.3%) was sclerosing adenosis, while the most frequently encountered malignant diagnosis was invasive ductal cancer. The presence of a type 2 dynamic curve had a sensitivity of 40.2% and specificity of 73.4% for predicting malignancy. By combining type 2 curves and morphological features, the sensitivity and specificity were increased. CONCLUSION The high rates of malignancy detected histopathologically among patients with type 2 dynamic curves in our study are remarkable. Type 2 dynamic curves can be detected in benign breast masses, especially in sclerosing adenosis cases. Considering morphological features can increase the diagnostic accuracy in cases with type 2 dynamic curves.

    Imaging of acute appendicitis: Advances

Aydin, Sonay; Karavas, Erdal; Senbil, Duezguen Can

We read with interest the review by Teng et al, who summarized the current approach to the diagnosis and treatment of acute appendicitis (AA). Also, the article summarizes the clinical scoring systems very effectively. In one of the previous studies conducted by our research group, we showed that the use of the Alvarado score, ultrasound and C-reactive protein values in combination provides a safe confirmation or exclusion of the diagnosis of AA. Computed tomography is particularly sensitive in detecting periappendiceal abscess, peritonitis and gangrenous changes. Computed tomography is not a good diagnostic tool in pediatric patients because of the ionizing radiation it produces. Ultrasound is a valuable diagnostic tool to differentiate AA from lymphoid hyperplasia. Presence of fluid collection in the periappendiceal and lamina propria thickness less than 1 mm are the most effective parameters in differentiating appendicitis from lymphoid hyperplasia. Although AA is the most common cause of surgical acute abdomen, it remains an important diagnostic and clinical challenge. By combining clinical scoring systems, laboratory data and appropriate imaging methods, diagnostic accuracy and adherence to treatment can be increased. Lymphoid hyperplasia and perforated appendicitis present significant diagnostic challenges in children. Additional ultrasound findings are increasingly defined to differentiate AA from these conditions.

Imaging Aspects of Hepatic Alveolar Echinococcosis: Retrospective Findings of a Surgical Center in Turkey

Kantarci, Mecit; Aydin, Sonay; Eren, Suat; Ogul, Hayri; Akhan, Okan

Radiologists should be aware of the findings of alveolar echinococcosis (AE) due to the diagnostic and management value of imaging. We are attempting to define the most common diagnostic imaging findings of liver AE, along with the prevalence and distribution of those findings. The patients’ US, CT, and MRI images were reviewed retrospectively. CT images were acquired with and without the administration of contrast medium. The MRI protocol includes T2-weighted images (WI), diffusion (WI), apparent diffusion coefficient (ADC) maps, and pre-and post-contrast T1WIs. The current study included 61 patients. The mean age of the population was 58.2 ± 9.6 years According to Kratzer’s categorization (US), 139 lesions (73.1%) were categorized as hailstorm. According to Graeter’s classification (CT), 139 (73.1%) lesions were type 1-diffuse infiltrating. The most frequent types were Kodama type 2 and 3 lesions (MRI) (42.6% and 48.7%, accordingly). P2N0M0 was the most frequent subtype. The current study defines the major, characteristic imaging findings of liver AE using US, CT, and MRI. Since US, CT, and MRI have all been utilized to diagnose AE, we believe that a multi-modality classification system is needed. The study’s findings may aid radiologists in accurately and timely diagnosing liver AE.

Sonographic findings of COVID-19 related acute scrotal infection and associations with clinical-laboratory data

Aydin, Sonay; Tokur, Oğuzhan; Kazci, Omer; Ece, Bunyamin; Karavas, Erdal; Kantarci, Mecit

Background: During the Coronavirus Disease 2019 (COVID-19) pandemic, patients present to hospitals with a wide range of symptoms. Some of these symptoms include acute orchitis and epididymitis. The goal of this research is to see if COVID-19 infection and scrotal infection are associated. Materials and methods: Patients with a COVID-19 (+) who agreed to a scrotal ultrasound (US) examination were studied prospectively in a tertiary care center between October 2021 and February 2022. The severity of the disease was used to divide patients into groups. Patients diagnosed with acute scrotal infection based on scrotal ultrasonography findings were compared in these groups, as were their age, comorbidities, and laboratory data. Results: The median age of the 213 participants was 61.7 ± 8.3. During the ultrasonographic examination of the patients, 15 (7%), 8 (3.7%), 17 (7.9%), and 40 (18.7%) were diagnosed with acute orchitis, acute epididymitis, acute epididymo-orchitis, and scrotal infection, respectively. Acute scrotal infection was far more common in patients with a higher clinical severity of the disease. The patients' comorbidities were also assessed, and it was discovered that they were statistically more common in the same group. Conclusions: Even if there are no clinical symptoms, ultrasonography can help detect acute scrotal infection in COVID-19 patients. Furthermore, in groups with higher clinical severity, this association is more likely to be seen. It is critical to understand this in order to avoid complications.

Computed tomography at every step: Long coronavirus disease

Aydin, Sonay; Unver, Edhem; Karavas, Erdal; Yalcin, Seven; Kantarci, Mecit

Background: Some patients continue to experience symptoms related to Coronavirus disease (COVID-19) after the acute phase of infection. Imaging studies, especially computed tomography (CT) of the chest, have gained importance since the beginning of the pandemic. CT can help diagnose COVID-19, assess the extent of pulmonary involvement, and predict the disease severity. We aimed to define the frequency of persistent symptoms and correlate their presence with the results of laboratory findings and the severity of the disease based on the findings of chest CT. Methods: We examined patients discharged from the hospital after treatment for COVID-19 and whose nasopharyngeal swab sample tested positive for severe acute respiratory syndrome coronavirus 2 after at least 4 weeks from the initial diagnosis. The patients were asked about the presence of persisting symptoms. In addition to the demographic data, laboratory results and severity levels seen on the chest CT were recorded. Results: In all, 116 patients were included in the study, of which 61 reported at least one persisting symptom (52.5%). The mean age of the study population was 48.90 ± 17.74 years. The persistent symptoms included shortness of breath, chest pain, cough, muscle weakness, dizziness, headache, fatigue, and palpitations. The mean CT severity score was 3.80 ± 0.38 and it was lower in patients without any persistent symptoms. Conclusions: Levels of C-reactive protein and fibrinogen, anemia, and female sex were associated with some of the persistent symptoms., the severity of the disease seen on CT was a successful predictor for the disease severity/prognosis and was also correlated with prolonged COVID infection and more persistent symptoms.

Gestational diabetes mellitus and early hemodynamic changes in fetus

Fatihoglu, Erdem; Aydin, Sonay; Karavas, Erdal; Kantarci, Mecit

Background: Gestational diabetes mellitus (GDM) incidence can increase up to 14%. GDM creates a risk for developing type 2 diabetes mellitus after pregnancy. Umbilical artery (UA) and fetal middle cerebral artery (MCA) changes in GDM have been studied before. The previous studies have contradictory results. In the current study, we aim to detect and define the impairment of color Doppler ultrasound (CDUS) characteristics in UA and MCA for the pregnant with GDM. Methods: US examinations were all performed at 18-22 weeks of gestation with a 3.5 MHz convex transducer. We recorded peak systolic velocity (PSV), end diastolic velocity, pulsatility index, resistive index, and systole/diastole ratio values of both UA and MCA at 18-22 weeks of gestation. GDM diagnosis was created according to the American Diabetes Association guidelines. Results: Sixty GDM patients and 61 healthy controls were included into the study. Median MCA PSV value was lower in GDM group (28 cm/s vs. 32 cm/s, P = 0.37). Among UA CDUS parameters, we cannot find any significant difference. In GDM group, we could not detect any significant correlation between CDUS parameters and HbA1C values. Conclusion: GDM changes fetal brain hemodynamics and the change can be detected at 18-22 weeks of gestation. Decreased fetal MCA PSV values can serve as an early warning for GDM.

The pneumatization of the sphenoid sinus, its variations and relations with surrounding neurovascular anatomic structures: A computerized tomography study

Fatihoglu, Erdem; Aydin, Sonay; Karavas, Erdal; Kantarci, Mecit

Introduction and objectives: The wide range of variability of SS pneumatization and relation with surrounding structures can result in serious complications; seeing that, the assessment of regional anatomy is essential for both surgeons and radiologists. We mainly aim to reveal the possible correlation between the SS pneumatization types and protrusion/dehiscence of the adjacent neurovascular structures in a larger population by using computerized tomography (CT) images. Methods: The type of SS pneumatization (I–IV), pneumatization of anterior clinoid process (ACP), greater wing of sphenoid (GWS), and pterygoid process (PP) was evaluated. Protrusion and dehiscence of ICA, ON, MN, and VN was noted. Results: 1003 patients were included into the study. ICA, ON, and MN protrusions were not seen in patients with the type I or II SS on both sides. These protrusions were most frequently seen along with the type IV SS on both sides (p < 0.05). ICA, ON, MN, and VN dehiscence was not found in any patients with the type I SS. The rate of ICA protrusion increased with presence of GWS and PP; ICA dehiscence was found to be positively correlated with ACP, GWS, and PP pneumatization. Conclusions: Variations, either amount or the extent, of the pneumatization of the SS are related with the presence of protrusion/dehiscence of ICA, ON, MN and VN. Knowing and reporting these relations can decrease the rate of complications during skull base surgery.

Normal pancreatic thickness values in healthy children: an MRI study

Aydin, Sonay; Fatihoglu, Erdem; Karavas, Erdal; Kantarci, Mecit

Introduction: Normal pancreatic thickness values on ultrasound (US) have been defined in literature. However, there is insufficient information about normal pancreatic measurements acquired from computed tomography (CT) or magnetic resonance imaging (MRI). To define normal pancreatic thickness measurements acquired from different localizations in order to provide reference values for more objectively identified parenchymal thickness changes. Materials and methods: A retrospective evaluation was made of the abdominal MRI examinations of 162 pediatric patients. Patients with any pancreatic disease, or chronic gastrointestinal inflammatory disease were excluded from the study. Measurements were taken from T2-weighted images. Results: Evaluation was made of 162 children, comprising 82 (50.6%) males and 80 (49.3%) females with a mean age of 9.8 ± 2.4 years. Mean pancreatic thickness was 18.3 ± 3.1 mm, 10.2 ± 2.9 mm, 14.9 ± 3 mm, 14.9 ± 3.3 mm in head, neck, body and tail localizations, accordingly. A positive correlation was determined between age, height, weight, body mass index (BMI) and pancreatic thickness in all the anatomic localizations (r > 0.55, p < 0.05). No significant difference was determined with gender. Interobserver agreement between two researchers was moderate and strong according to the different anatomic localizations. Conclusions: The defined normal ranges are mostly consistent with previously published US and CT based values. Pancreatic thickness values were positively correlated with age, height, weight and BMI for all four anatomical regions of the pancreas. Knowledge of normal pancreatic thickness values will increase the diagnostic accuracy of radiologists in the assessment of pancreatic diseases and may aid in interpreting atrophy in the setting of chronic pancreatitis.
