Tokur, Oguzhan; Aydin, Sonay; Kilinc, Funda; Degirmenci, Tulin; Oztekin, Pelin Seher

Background: The evaluation of axillary lymph nodes (ALNs) using ultrasound (US) plays a crucial role in the diagnosis and management of breast cancer. This study aims to assess the diagnostic performance of both quantitative and qualitative features of suspicious ALNs using superb microvascular imaging (SMI). To the best of our knowledge, this is the first prospective study in the literature to evaluate the largest number of suspicious ALNs using SMI and to analyze the combined diagnostic value of these findings. Methods: Our prospective study involved 102 patients with suspicious lymph nodes in the axilla. SMI method was employed individually to assess distribution, appearance, and number of vascular structures. After histopathological or follow-up confimation, images were re-evaluated. We also examined the diagnostic value of various combinations of these vascular features. Lastly, two radiologists, each with 22 and 4 years of experience, independently reviewed the images, blinded to clinical and pathological data, to assess interobserver agreement. Results: A total of 108 lymph nodes were evaluated in 102 patients, including 67 benign and 41 malignant cases. The diagnostic performance of various combinations in receiver operating characteristic (ROC) analysis revealed that the highest accuracy (78.70%) and area under the curve (AUC) value in ROC analysis were achieved by distribution-appearance combinations (AUC: 0.776, p < 0.001). Interobserver agreement was significant for both the SMI and PDUS methods. In patients diagnosed with breast cancer, the diagnostic accuracy for suspicious axillary lymph nodes reached 92.76%, with an AUC value of 0.855. Conclusion: In conclusion, our findings suggest that SMI facilitates more accurate identification of suspicious axillary lymph nodes prior to surgical intervention.

Aydin, Sonay; Aydin, Ozlem Celik; Yazar, Mesut Furkan; Aydemir, Huseyin; Kantarci, Mecit; Barun, Sureyya

BACKGROUND In cases of coronavirus disease 2019 ( COVID- 19), favipiravir is commonly included to the therapy regimen. Drug interactions between favipiravir and other COVID-19 therapy drugs are frequently researched. However, no research on possible drug interactions between Favipiravir and radiocontrast agents, which have become almost crucial in diagnostic processes while not being part of the treatment, has been found. AIM To determine potential medication interactions between Favipiravir and radiocontrast agents. METHODS The study comprised patients who were taking Favipiravir for COVID-19 therapy and underwent a contrast-enhanced computed tomography (CT) or magnetic resonance imaging (MRI) test while taking the medicine. The computerized patient files of the cases included in the study, as well as the pharmacovigilance forms in the designated hospital, were evaluated for this purpose. RESULTS The study included the evaluation of data from 1046 patients. The study sample's mean age was 47.23 +/- 9.48 years. The mean age of cases with drug interactions was statistically significant greater than that of cases with no drug interactions (P = 0.003). When evaluated with logistic regression analysis, a 1-year raises in age increases the risk of developing drug interactions by 1.63 times (P = 0.023). There was no statistically significant difference in the occurrence of medication interactions between the sexes (P = 0.090). Possible medication interactions were discovered in 42 cases (4%). CONCLUSION The findings of this study revealed that the most notable findings as a result of the combined use of contrast agents and favipiravir were increased creatinine and transaminase values, as well as an increase in the frequency of nausea and vomiting. The majority of drug interactions discovered were modest enough that they were not reflected in the clinic. Drug interactions become more common as people get older.

Use of the vertebrae and iliac bone as references for localizing the appendix vermiformis in computed tomography

Ozturk, Muhsin Ozgun; Resorlu, Mustafa; Aydin, Sonay; Memis, Kemal Bugra

BACKGROUND The appendix vermiformis is a part of the gastrointestinal tract, situated in the lower right quadrant of the abdomen. Acute appendicitis, acute inflammation of the appendix vermiformis, is the most common cause of acute abdomen requiring surgical intervention. Although computed tomography (CT) offers high diagnostic efficacy in assessing the appendix across various anatomical positions, it also involves radiation exposure. Reducing exposure factors and narrowing the field of view (FOV) are ways to decrease the radiation dose to the patient. To narrow the FOV, appendix locations within the population must be defined using metric markers. AIM To determine the location of the appendix vermiformis on CT using the vertebrae and the right iliac bone as anatomical landmarks. METHODS This retrospective study examined 470 patients presenting with abdominal pain who underwent abdominal CT scans between January 01, 2015 and January 01, 2018. Forty-three patients were excluded due to various reasons. The most superior and inferior points and the origin of the appendix were measured separately in relation to the vertebrae and right iliac bone for localization. The population was divided into normal and acute appendicitis groups, and the relationship between appendix location and anthropometric parameters relationship was examined. P values below 0.05 were considered statistically significant. RESULTS The final analysis included 427 adult patients (206 females and 221 males) with a mean age of 42.1 +/- 19.5 years. An ascending appendix course was the most common (90.4%). The appendix ranged from the L2 vertebral body level to the coccygeal vertebral level relative to the vertebrae. The appendix ranged between (-) 140.5 mm and (+) 87.4 mm relative to the right iliac bone. A negative correlation was found between patient age, height, body mass index, and the highest and lowest points of the appendix in regard to the vertebrae. CONCLUSION The study's findings unveiled the locations of the appendix in the population in relation to the bony anatomical landmarks. These data can be used as the basis for future research aimed at reducing patient exposure to ionizing radiation.

Characteristics of the imaging diagnosis of alveolar echinococcosis

Aydin, Sonay; Irgul, Baris; Memis, Kemal Bugra; Kizilgoz, Volkan; Kantarci, Mecit

Alveolar echinococcosis (AE) primarily manifests in the liver and exhibits characteristics resembling those of slow-growing malignant tumours. Untreated Echinococcus multilocularis infection can be lethal. By infiltrating the vascular systems, biliary tracts, and the hilum of the liver, it might lead to various problems. Due to its ability to infiltrate neighbouring tissues or metastasize to distant organs, AE can often be mistaken for malignancies. We present a concise overview of the epidemiological and pathophysiological characteristics of AE, as well as the clinical manifestations of the disease. This article primarily examines the imaging characteristics of AE using various imaging techniques such as ultrasonography, computed tomography (CT), magnetic resonance imaging, diffusion-weighted imaging, and virtual non-enhanced dual-energy CT. We additionally examined the contribution of radiography in the diagnosis, treatment, and monitoring of the condition.

Memis, Kemal Bugra; Bulut, Hasan; Cinar, Hasibe Gokce; Ucan, Berna; Uner, Cigdem; Kaya, Ozkan; Aydin, Sonay

Introduction: Understanding the variations of abdominal vascular structures is important for preventing complications of abdominal surgical procedures for gastrointestinal disease such as necrotizing enterocolitis or others that may arise in patients with congenital cardiac disease. We analysed the coeliac trunk and its branches in children with congenital heart disease to determine whether there is a greater prevalence of associated vascular abnormalities. Methods: We retrospectively analysed thoracic computed tomography (CT) angiograms performed in our hospital in paediatric patients with congenital heart disease. We documented the anatomical variations observed in abdominal sections in which the coeliac trunk and hepatic arteries were included in the field of view. We used the Uflacker classification to describe anatomical variants of the coeliac trunk, and the Michels classification and its modified version (Hiatt classification) to describe the anatomy of the hepatic artery system. Results: Our study included 178 patients with congenital heart disease. We identified coeliac trunk variants in 10.7% of the patients. Gastrosplenic trunk was to the most prevalent variant, amounting to 5.6% of total cases. We found hepatic artery variations in 19.1% of the patients. According to the Michels classification, the prevalence of accessory left hepatic artery arising from the left gastric artery as 4.5%, compared to 6.7% based on the Hiatt classification. Conclusion: The prevalence of coeliac trunk and hepatic artery variations in patients with congenital heart disease was not greater in our study compared to other series in the literature. Clinicians must be vigilant about the variations detected in multislice CT scans to avoid complications resulting from vascular abnormalities, especially in patients who undergo abdominal surgery. Taskent, Ismail; Ece, Bunyamin; Aydin, Sonay

Objective: Sarcopenia, characterized by progressive skeletal muscle loss, poses significant health risks, including physical impairment and mortality. The relationship between sarcopenia and insulin resistance suggests insulin therapy’s potential in preserving muscle mass, particularly in Type 2 diabetes mellitus (T2DM) patients. This study aims to evaluate the Psoas Muscle Index (PMI) via computed tomography (CT) in middle-aged T2DM patients on insulin therapy versus oral antidiabetic drugs (OAD) and controls. Methods: This retrospective study included 107 middle-aged T2DM patients undergoing non-contrast CT scans and 58 age-matched controls. CT images were analyzed to calculate PMI. Statistical analysis included Chi-square tests, independent samples t-tests, Mann–Whitney U tests, and correlation analyses. Results: Insulin-treated patients exhibited higher PMI than OAD users (p < 0.001), while OAD users had lower PMI than controls (p < 0.001). No significant difference was found between insulin-treated patients and controls (p = 0.616). Negative correlations were observed between T2DM duration/age and PMI across all groups, with a positive correlation between T2DM duration and BMI observed in the OAD group. Conclusions: Insulin therapy in T2DM patients, regardless of age or disease duration, positively impacts muscle mass, highlighting its potential in preserving muscular health and advocating for tailored treatment strategies in T2DM management.

Cetin, Turkhun; Tokur, Oguzhan; Bozkurt, Hayrunnisa Bekis; Aydin, Sonay; Memis, Kemal Bugra; Kantarci, Mecit

Shear wave elastography (SWE) has become popular in clinical practice for many diseases. However, there is not adequate research on spleen-related diseases. This study aimed to investigate the potential of quantitative values obtained through SWE in evaluating spleen pathologies in the pediatric population and to demonstrate its performance to differentiate splenomegaly-related diseases. The research group retrospectively included children with pathological diagnoses related to the spleen from November 2016 to April 2021, and they were categorized into three groups, including portal hypertension (PH), benign lymphoid hyperplasia (BLH), and malignant infiltration (MI). Spleen sizes and parenchymal stiffness were also calculated for each group. Subsequently, mean spleen stiffness in each group was compared with normal values within the same age group. In total, 2781 children (1379 children for the study group; 1402 children for the control group) were enrolled in the study. The highest stiffness was observed in the PH group, which is statistically higher than others (p < 0.05). Although the mean spleen stiffness in the group with BLH was higher than the control and MI group, the difference was not statistically significant (p = 0.08). The mean stiffness in the group with MI was significantly lower than both the control group (p = 0.005) and PH (p = 0.01). In conclusion, using SWE in the differential diagnosis of etiologies causing splenomegaly could make an important contribution.

Kazci, Omer; Kadirhan, Ozlem; Uner, Cigdem; Karavas, Erdal; Ucan, Berna; Aydin, Sonay

Introduction: Liver biopsies are the main method in the diagnosis and treatment of paediatric liver pathologies. Major complication rates of paediatric liver biopsies range from 0% to 6.6% in the literature and minor complication rates range from 0% to 25%. In this study, we aimed to review the complications, indications and results of percutaneous core liver biopsies with paediatric sonography in a tertiary care centre by an interventional radiologist. Methods: We retrospectively evaluated the results, indications and complications of paediatric liver biopsies performed in our tertiary health centre between January 2017 and December 2020. Biopsies were performed with a 16G semi-automatic needle in 17 patients (29.8%) and with an 18G semi-automatic needle in 40 patients (70.2%). Biopsies were performed only with local anaesthesia in patients older than 12 years; in younger patients, it was performed under general anaesthesia. Results: Fifty-eight liver biopsies were obtained from 57 children (34 males, 23 females). The most common indications were elevated liver enzymes (33 patients), cholestasis (14 patients), and adiposity and metabolic problems (6 patents). The most common pathological diagnoses were chronic hepatitis (33 patients) and steatosis (10 patients). Major complication in the form of symptomatic subcapsular haematoma developed after liver biopsy performed with 18G needle in only one patient (1.8%). Conclusions: As previously stated in the literature, percutaneous biopsies performed by interventional radiologists in paediatric patients under the guidance of sonography can be used in diagnosis and treatment; the complication rate is low and it is a safe method.

A novel artificial intelligence-based hybrid system to improve breast cancer detection using DCE-MRI

Akgül, Ismail; Kaya, Volkan; Karavaş, Erdal; Aydin, Sonay; Baran, Ahmet

The interpretation of breast magnetic resonance imaging (MRI) in the healthcare field depends on the good knowledge and experience of radiologists. Recent developments in artificial intelligence (AI) have shown advances in the field of radiology. However, the desired levels have not been reached in the field of radiology yet. In this study, a novel model structure is proposed to characterize the diagnostic performance of AI technology for individual breast dynamic contrast material–enhanced (DCE) MRI sequences. In the proposed model structure, Inception-v3, EfficientNet-B3, and DenseNet-201 models were used as hybrids together with the Yolo-v3 algorithm to detect breast and cancer regions. In the proposed model, DCE-MRI sequences (T2, ADC, Diffusion, Non-Contrast Fat Non-Suppressed T1, Non-Contrast Fat Suppressed T1, Contrast Fat Suppressed T1, and Subtraction T1) were evaluated separately and validation was made, thus providing a unique perspective. According to the validation results, the model structure with the best performance was determined as Yolo-v3 + DenseNet-201. With this model structure, 92.41% accuracy, 0.5936 loss, 92.44% sensitivity, and 92.44% specificity rates were obtained. In addition, it was determined that the results obtained without using contrast material in the best model were 91.53% accuracy, 0.9646 loss, 92.19% sensitivity, and 92.19% specificity. Therefore, it is predicted that the need for contrast material use can be reduced with the help of this model structure.

Complementary comments on diagnosis, severity and prognosis prediction of acute pancreatitis

    Ozturk, Muhsin Ozgun; Aydin, Sonay

The radiological differential diagnosis of acute pancreatitis includes diffuse pancreatic lymphoma, diffuse autoimmune pancreatitis and groove located mass lesions that may mimic groove pancreatitis. Dual energy computed tomography and diffusion weighted magnetic resonance imaging are useful in the early diagnosis of acute pancreatitis, and dual energy computed tomography is also useful in severity assessment and prognosis prediction. Walled off necrosis is an important complication in terms of prognosis, and it is important to know its radiological findings and distinguish it from pseudocyst.

A detailed look in radioanatomical aspects of ligamentum arteriosum calcification in pediatric population

Aydemir, Huseyin; Fatihoglu, Erdem; Kizilgoz, Volkan; Kahraman, Sevket; Yazar, Mesut F.; Aydin, Sonay; Kantarci, Mecit

After birth, ductus arteriosus forms ligamentum arteriosum and sometimes can be calcified. Ligamentum arteriosum calcification can be seen as subtle-punctate or coarse-linear between pulmonary artery and the aorta in the non-contrast computed tomography (CT) images, and between the pulmonary conus and the aortic knob in the chest radiography. In this study, we aim to estimate the prevalence of ligamentum arteriosum calcification on non-contrast CT and chest radiography in a pediatric population. Patients aged 0-18 years, who underwent non- contrast CT and chest radiography at Department of Radiology between March 1, 2020 and October 31, 2021 were evaluated retrospectively. It was examined whether there was ligamentum arteriosum calcification in non-contrast CT and chest radiography. If there is calcification, it was recommended as subtle-punctate or coarse-linear, and information was documented by grouping the patients’ age. If ligamentum arteriosum calcification was present, it was divided morphologically into two groups (subtle-punctate and coarse-linear) and classified according to the age groups. The research population consisted of 1003 patients. Non-contrast CT revealed calcification in 25.5% of patients (n=256). 11.3% of patients had subtle-punctate calcification, and 14.3% had coarse-linear calcification. Radiography had a 1.7% (n:17) detection rate for calcification. Ligamentum arteriosum calcification is a common finding in all age groups in the pediatric population. When reporting pediatric CT studies, radiologists should be able to distinguish ligamentum arteriosum calcification from other pathological and anatomical conditions and accept it as a normal finding in order to avoid unnecessary further investigations.

Evaluation of the Effect of Topical Prostaglandin Analog Treatment on Orbital Structures in Open-Angle Glaucoma with Computed Tomography

Durmuş Ece, Berire Şeyma; Yozgat, Zübeyir; Bayramlı, Hüseyin; Ece, Bunyamin; Aydin, Sonay

Background/Objectives: This study aims to evaluate the computed tomography (CT) scans of glaucoma patients using prostaglandin analogs (PGA) in one eye, investigate findings associated with prostaglandin-associated periorbitopathy (PAP), and compare these findings with those of the contralateral eyes. Methods: Patients with open-angle glaucoma who had CT images of the orbital region taken for another reason at least one month after starting PGA treatment in one eye were included in the study. Enophthalmos measurements from thin-slice CT images, along with 3D volume measurements of orbital fat tissue, periorbital muscles, and the optic nerve, were performed. Ophthalmological examination findings and treatment information were collected. The values were compared with those of the contralateral eyes of the same patients not using PGA. Intraclass correlation coefficients (ICCs) were computed to evaluate measurement repeatability. Results: Forty patients were included in the study. Among them, 29 (72.5%) used latanoprost, 9 (22.5%) used bimatoprost, and 2 (5%) used travoprost. The mean enophthalmos values on the treated side (15.5 ± 2.0 mm) were lower than on the untreated side (16.1 ± 1.4 mm), but this difference was not statistically significant (p = 0.07). In 29 patients (72.5%), enophthalmos measurements were smaller on the treated side, with 7 patients (17.5%) showing a difference of 2 mm or more. No significant correlation was found between the duration of PGA use and enophthalmos measurements (p = 0.768 r = −0.048). Additionally, no significant differences were found in orbital fat volume, total extraocular muscle volume, and optic nerve volume (p > 0.05). ICC values demonstrated excellent reliability (ICC > 0.75) for all measurements. Conclusions: We did not find significant differences in enophthalmos measurements, orbital fat volume, total muscle volume, and optic nerve volume between the PGA-treated and untreated eyes.

When Undergoing Thoracic CT (Computerized Tomography) Angiographies for Congenital Heart Diseases, Is It Possible to Identify Coronary Artery Anomalies?

Uner, Cigdem; Gulmez, Ali Osman; Cinar, Hasibe Gokce; Bulut, Hasan; Kaya, Ozkan; Gokharman, Fatma Dilek; Aydin, Sonay

Introduction and Objective: The aim of this study was to evaluate the coronary arteries in patients undergoing thoracic CT angiography for congenital heart disease, to determine the frequency of detection of coronary artery anomalies in congenital heart diseases, and to determine which type of anomaly is more common in which disease. Materials and Methods: In our investigation, a 128-detector multidetector computed tomography machine was used to perform thorax CT angiography. The acquisition parameters were set to 80–100 kVp based on the patient’s age and mAs that the device automatically determined based on the patient’s weight. During the examination, an intravenous (IV) nonionic contrast material dose of 1–1.5 mL/kg was employed. An automated injector was used to inject contrast material at a rate of 1.5–2 mL/s. In the axial plane, 2.5 mm sections were extracted, and they were rebuilt with 0.625 mm section thickness. Results: Between October 2022 and May 2024, 132 patients who were diagnosed with congenital heart disease by echocardiography and underwent Thorax CT angiography in our department were retrospectively evaluated. Of the evaluated patients, 32 were excluded with exclusion criteria such as patients being younger than 3 months, older than 18 years, insufficient contrast enhancement in imaging and contrast-enhanced imaging, thin vascular structure, and motion and contrast artifacts; the remaining 100 patients were included in this study. The age range of these patients was 3 months to 18 years (mean age 4.4 years). Conclusion: In congenital heart diseases, attention to the coronary arteries on thoracic CT angiography examination in the presence of possible coronary anomalies may provide useful information.
