Assessment of Potentially Unnecessary Breast Magnetic Resonance Imaging Utilization and Its Economic Impact: A Retrospective Study

Senbil, Duzgun Can; Kiliçparlar, Muslum Burak; Kantarci, Mecit; Aydin, Sonay

Background: In this study, breast magnetic resonance imaging (MRI) examinations were retrospectively reviewed. This study evaluated whether the indications for breast MRI were consistent with the recommendations of the Breast Imaging Reporting and Data System (BI-RADS)atlas. Additionally, the study assessed whether prior ultrasound and mammography findings were sufficient for diagnosis. The aim was to identify potentially unnecessary breast MRI examinations and their associated financial burden. Methods: Breast MRI reports were compared with previous mammography and ultrasound reports. Patients for whom breast MRI provided no additional diagnostic benefit were identified, and the indications for MRI examination were analyzed. Results: Comparison of BI-RADS categories assigned on prior ultrasound and mammography with those assigned after breast MRI revealed that 321 (64.3%) of the 499 patients could have been adequately managed without breast MRI. In these patients, MRI findings did not alter clinical management or treatment decisions. Conclusion: Breast MRI is a costly imaging modality with limited accessibility in many healthcare settings. Therefore, its economic impact should be carefully considered when determining appropriate indications for its use. These findings suggest that the absence of restrictive criteria for breast MRI requests may contribute to substantial financial losses due to unnecessary examinations.

Cardiac Magnetic Resonance Findings and NT-proBNP in Heart Failure with Reduced Ejection Fraction

Memis, Kemal Bugra; Doğan, İffet; Şahin, Defne; Gulmez, Ali Osman; Özer, Ece; Kantarci, Mecit

Background/Objectives: N-terminal pro-B-type natriuretic peptide (NT-proBNP) is an established heart failure biomarker, but its associations with cardiac magnetic resonance (CMR) parameters in patients with reduced ejection fraction remain unclear. This study examined the associations between serum NT-proBNP concentrations and CMR-derived functional, structural, and tissue characterization parameters in patients with HFrEF and evaluated which imaging parameters remained associated with NT-proBNP after multivariable adjustment. Methods: This retrospective single-center study included 92 consecutive patients with HFrEF who underwent CMR and NT-proBNP measurement within a 24 h window, with a median absolute interval of 6 h (range, 1–15 h) between the two assessments. CMR analysis included left ventricular ejection fraction (LVEF), left ventricular and left atrial (LA) cavity diameters, myocardial mass, extracellular volume fraction (ECV), the late gadolinium enhancement (LGE) burden, and the T2 signal intensity (T2-SI) ratio. Spearman correlation, univariate linear regression, and multivariable linear regression analyses were performed using log-transformed NT-proBNP values. Results: Log NT-proBNP demonstrated significant correlations with LVEF (rs = −0.359, p < 0.001), T2-SI ratio (rs = −0.332, p = 0.001), ECV (rs = 0.383, p < 0.001), and LA diameter (rs = 0.347, p < 0.001). No statistically significant correlations were observed between log-transformed NT-proBNP and LGE burden, LV cavity diameter, myocardial mass, LV wall thickness, age, or the recorded comorbidities. In multivariable analysis, LVEF, T2-SI ratio, and LA diameter remained associated with NT-proBNP after adjustment for the available covariates. Conclusions: Lower LVEF, a lower T2-SI ratio, and a larger LA diameter were associated with higher NT-proBNP levels in patients with HFrEF. The T2-SI ratio and LA diameter should be considered exploratory imaging markers requiring prospective validation.

Ertogrul, Rodi; Ogul, Hayri; Yahsi, Yusuf; Sakci, Zakir; Ozdemir, Mustafa; Kantarci, Mecit; Levent, Akin

Background Superior capsular folds and fold-like structures in the glenohumeral joint are potential sources of superior shoulder POS=12ptpain. Purpose To obtain detailed information about the arthrographic anatomy and morphology of superior capsular folds and fold-like structures. Material and Methods This study followed a retrospective design and involved 300 consecutive shoulder magnetic resonance (MR) images and MR arthrograms in two centers. The origins and insertions of the superior capsular folds and fold-like structures in the glenohumeral joints were carefully examined and documented. Four subtypes of extensions in the sagittal-oblique plane of the posterior synovial fold were identified from the MR arthrograms, according to their associations with the biceps tendon long head, superior glenoid notch, and supraspinatus tendon. Further, two morphological subtypes of superior synovial folds were identified: cord-shaped and fan-shaped. Results Superior capsular folds and fold-like structures in the shoulder joint were identified in 11 (4%) patients. Of these 11 superior capsular folds, 6 (54.5%) were then confirmed upon arthroscopy. The cord-shaped type was the most common MR arthrographic superior synovial fold and was detected in 8/11 (72.7%) patients. Type B was the most common form according to the course and insertion of the superior capsular fold (4/11 [37%] patients). Conclusion Superior capsular folds, probably normal capsular anatomical variants, tend to be present in the posterior portions of the superior shoulder joint capsule and are rarely seen in MR arthrograms. These anatomical variations tend to be symptomatic, especially in young individuals.

Normative Values of Femoral Inclination Angle and Medial Proximal Femoral Angle: An MRI-Based Study

Memis, Kemal Bugra; Kızılgöz, Volkan; Aydın, Sonay; Sunar, Mukadder; Gökgöz, Mehmet Burak; Durmaz, Önder; Gülmez, Ali Osman; Özkan, Berna Hatun; Kantarci, Mecit

BACKGROUND This study aimed to establish normative values for the femoral inclination angle (FIA) and medial proximal femoral angle (MPFA) using magnetic resonance imaging (MRI), which minimizes projectional and rotational errors, in a large cohort. Sex-based and side-to-side comparisons, as well as correlations with age, were also investigated for these proximal femoral angle measurements. MATERIAL AND METHODS After exclusion of patients with trauma, infection, prior surgery, advanced osteoarthritis, femoroacetabular impingement, avascular necrosis, coxa vara, coxa valga, and low-quality magnetic resonance images, 631 pairs of proximal femurs were retrospectively evaluated for FIA and MPFA measurements. All measurements were performed by consensus between 2 radiologists, then analyzed regarding sex and side differences and correlations with age. RESULTS The mean FIA was 128.53±4.62°, whereas the mean MPFA was 86.62±3.40°. No significant differences were observed between men and women (FIA: P=0.057, MPFA: P=0.050) or between right and left sides (FIA: P=0.178, MPFA: P=0.853). Age was not significantly correlated with either proximal femoral angle measurement. No significant association was found between age and FIA (right: P=0.554, left: P=0.321) or MPFA (right: P=0.954, left: P=0.935). CONCLUSIONS This large MRI-based study provides normative reference values for FIA and MPFA. Neither angle was significantly influenced by sex, side, or age, suggesting that these parameters remain relatively stable across adult populations. These findings may serve as useful reference values for assessment of proximal femoral morphology and preoperative orthopedic planning.

The Role of Imaging Methods in the Diagnosis and Treatment of Idiopathic Granulomatous Mastitis

Aydin, Sonay; Kantarci, Mecit; Aydemir, Hüseyin

Idiopathic granulomatous mastitis (IGM) is an uncommon chronic inflammatory disease of the breast with an unclear etiology that mostly impacts young women. Its incidence is higher in the Middle East and Asian countries, particularly in Türkiye. Clinical signs such as mass, fistula formation, swelling, and radiographic abnormalities such as lymph node enlargement, abscess, calcifications, localized or widespread asymmetric density, and hypoechoic lesions may result in a misdiagnosis of malignancy or infection. There are no imaging findings that distinguish between IGM and breast cancer in any imaging method; in most cases, imaging tests like ultrasonography, magnetic resonance imaging, or mammography reveal non-specific characteristics, leading to biopsy. Histological assessment is used to reach a conclusive diagnosis of IGM. Common medical treatments include systemic steroid treatment (oral corticosteroid), local steroid treatment (local steroid injection in breast,), antibiotics, immunosuppressive drugs (methotrexate, etc.), and monoclonal antibodies. Abscess drainage and surgical excision are further alternatives. Although the conclusive diagnosis of IGM is made histologically, imaging methods have a significant impact in evaluating the number, location, their size, abscess development, and the likelihood of intervention, stability or change in lesions, and response to treatment in patients with confirmed diagnosis. In addition, radiology and interventional radiology units guide clinicians in the timely diagnosis of the disease and play an important role in local steroid injection, which is used effectively in its treatment.

Association between insall-salvati index and anterior cruciate ligament tears: A cross-sectional study

Kizilgöz, Volkan; Aydin, Sonay; Kantarci, Mecit; Kockara, Nizamettin; Gülmez, Ali Osman; Gür, Volkan

Aim: There are many studies in the literature presenting morphological predisposing factors of anterior cruciate ligament (ACL) tears. Insall-Salvati index (ISI), which is widely used to measure patellar height, was investigated as a predisposing risk factor for the ruptures of ACL. Material and Methods: Patellar tendon length (PTL), Patellar length (PL), and anterior tibial translation (ATT) were measured, and ISI values were calculated for the study and control groups. Knee magnetic resonance (MR) images of 33 patients who had undergone ACL reconstruction due to ruptured ACL, which is surgically confirmed (study group), and 90 patients who had undergone meniscus operation with surgically confirmed intact ACL (control group) were reinterpreted by two reviewers in terms of ISI and ATT values. Independent samples T-test was used to compare the results of the study and control groups. Additionally, intraclass correlation coefficient (ICC) values were presented to verify the interobserver difference between reviewers. Results: PTL, ATT, and ISI values showed a statistically significant difference, and higher values were obtained in the study group than in the control group (p<0,001). There was excellent agreement between reviewers regarding PTL, PL, ISI, and ATT measurements. Discussion: ISI had been used as an indicator of patellar dislocation. In this research, ISI was investigated to be in a relationship with ACL injuries in operated patients. The results of this study indicated higher ISI values in patients with ACL rupture. High ISI values might be a predisposing factor for ACL rupture and may be used in preventive medicine in terms of ACL injuries.

Rectal ameboma: A new entity in the differential diagnosis of rectal cancer

Memis, Kemal Bugra; Celik, Ayse Sena; Aydin, Sonay; Kantarci, Mecit

We examined the case report written by Ke et al, describing a rare clinical case. In this editorial, we would like to emphasize the differential diagnosis of rectal masses through a rare case. We describe a case of ameboma, which manifested itself as a mass in the rectum in terms of imaging and rectoscopic features, in an immunocompetent patient who had complaints of constipation and rectal bleeding for weeks. The initial diagnosis suggested malignancy due to imaging and rectoscopic features, but the pathology report reported it as amoebiasis. After ten days of metronidazole and oral amebicide (diloxanide furoate) treatment, the patient's symptoms and radiological findings were successfully regressed.

Magnetic resonance arthrography with positional manoeuvre for the diagnosis of synovial fold of posterior shoulder joint capsule

Keles, Papatya; Ogul, Hayri; Tuncer, Kutsi; Sakci, Zakir; Ay, Mutlu; Kantarci, Mecit

Objectives: The objective of this study is to prospectively assess the effectiveness of shoulder magnetic resonance (MR) arthrograms with positional manoeuvres in detecting posterior synovial folds. Methods: Two radiologists independently assessed all axial MR arthrograms in internal rotation, neutral position, and external rotation for the presence of a posterior synovial fold. The diagnostic performances of the MR arthrograms were then compared, with results validated through arthroscopy. Results: Arthroscopy was performed on 81 of the 150 patients included in the study. A posterior synovial fold was identified arthroscopically in eleven of these patients. Measurements of the posterior synovial fold obtained in external rotation and the neutral position of the arm showed a significant correlation with arthroscopic results (p < 0.05). For detecting the posterior synovial fold with arthroscopic correlation, the sensitivity and specificity values for observer 1 and observer 2 were 100–81.4% and 100–88.6%, respectively, for MR arthrograms in the neutral position; 100–52.9% and 100–62.9% for MR arthrograms in external rotation; and 100–95.7% and 81.8–98.6% for MR arthrograms in internal rotation. There was a fair agreement for MR arthrography in external rotation for detecting posterior synovial folds, while MR arthrograms in internal rotation and neutral position showed near-perfect and significant interobserver agreement. Conclusion: The rotational positions of the humeral neck during MR arthrographic examination can influence the diagnostic specificity and sensitivity of axial MR arthrograms in detecting the posterior synovial fold. Clinical relevance statement: The posterior synovial fold can mimic a posterior labral detachment. Therefore, its correct identification is crucial in order to avoid unnecessary surgical procedures. Key Points: Movement of the shoulder may introduce variability in MR arthrography appearance. Rotation of the humeral neck during MR arthrography can affect diagnoses in posterior synovial fold detection. Given that posterior synovial folds can imitate posterior labral detachment, their correct identification is crucial to avoid unnecessary surgical procedures.

Evaluation of Femoral Head Avascular Necrosis with Virtual Noncalcium Dual-Energy Computed Tomography

Ozturk, Muhsin Ozgun; Kantarci, Mecit; Aydin, Sonay; Kizilgöz, Volkan; Kockara, Nizamettin; Gur, Volkan

Objective Our aim was to investigate the effectiveness of the dual-energy computed tomography (DECT) virtual noncalcium (VNCa) technique in avascular necrosis (AVN) for detecting bone marrow edema (BME) and staging. Methods This prospective study included adult patients diagnosed with unilateral or bilateral femoral head AVN between January 2023 and December 2023, who had magnetic resonance imaging (MRI) and DECT. Two participants were excluded from the study due to undergoing surgical procedures during the period between the scans. Two reviewers, blinded to MRI images and clinical data, visually examined color-coded VNCa pictures to assess BME using a binary classification (0 = normal bone marrow, 1 = BME). Same 2 reviewers also used color-coded and nonmapped images to stage AVN in accordance to the "Association for Research on Osseous Circulation"(ARCO) staging system. Interobserver agreements for the visual evaluation and staging were calculated with κ coefficient. Following a visual assessment of BME and the staging of AVN, same 2 reviewers conducted CT density measurements on regions of BME regions utilizing DECT noncalcium images. An independent third investigator (reference standard) utilized MRI, x-ray, and clinical data to confirm the definitive diagnosis and staging of AVN. A P value less than 0.05 was considered statistically significant. Results Fifty patients (28 men, 22 women, mean age: 44.2 ± 13.1 years, range: 25-75 years) were included in the final analysis. The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of the VNCa technique in detecting BME were 96.0%, 94.4%, 97.9%, 89.4%, and 95.6%, respectively, for reviewer 1; and 96.0%, 88.9%, 96.0%, 88.9%, and 94.1%, respectively, for reviewer 2. Interobserver agreement was almost perfect (κ = 0.84). Both reviewer 1 and reviewer 2 accurately classified 92.7% of the AVNs. The density measurements showed a statistically significant difference (P = 0.001) between the edema regions and the normal marrow regions. No statistically significant difference was observed in the density measurements of edema regions at different stages (P = 0.13). Conclusions DECT VNCa technique exhibits excellent performance in detecting BME in hip AVN cases, as well as accurately determining the stage of AVN.

Cerebellar cavernous angioma associated with developmental venous anomaly presenting as a cerebellopontine angle neoplasm in CT scan

Ogul, Hayri; Gozgec, Elif; Demirdogen, Filiz; Kantarci, Mecit

Association of developmental venous anomaly (DVA) and cavernous malformation (CM) of the posterior fossa is rare. If bleeding occurs, it must be assigned to the CM. Developmental venous anomalies, considered as normal vascular variation, are rarely symptomatic. Indeed, the CMs are true malformations and have a high bleeding potential. © The Author(s) 2023. Assessment of the potential interactions between favipiravir and radiocontrast agents

Aydin, Sonay; Aydin, Ozlem Celik; Yazar, Mesut Furkan; Aydemir, Huseyin; Kantarci, Mecit; Barun, Sureyya

BACKGROUND In cases of coronavirus disease 2019 ( COVID- 19), favipiravir is commonly included to the therapy regimen. Drug interactions between favipiravir and other COVID-19 therapy drugs are frequently researched. However, no research on possible drug interactions between Favipiravir and radiocontrast agents, which have become almost crucial in diagnostic processes while not being part of the treatment, has been found. AIM To determine potential medication interactions between Favipiravir and radiocontrast agents. METHODS The study comprised patients who were taking Favipiravir for COVID-19 therapy and underwent a contrast-enhanced computed tomography (CT) or magnetic resonance imaging (MRI) test while taking the medicine. The computerized patient files of the cases included in the study, as well as the pharmacovigilance forms in the designated hospital, were evaluated for this purpose. RESULTS The study included the evaluation of data from 1046 patients. The study sample's mean age was 47.23 +/- 9.48 years. The mean age of cases with drug interactions was statistically significant greater than that of cases with no drug interactions (P = 0.003). When evaluated with logistic regression analysis, a 1-year raises in age increases the risk of developing drug interactions by 1.63 times (P = 0.023). There was no statistically significant difference in the occurrence of medication interactions between the sexes (P = 0.090). Possible medication interactions were discovered in 42 cases (4%). CONCLUSION The findings of this study revealed that the most notable findings as a result of the combined use of contrast agents and favipiravir were increased creatinine and transaminase values, as well as an increase in the frequency of nausea and vomiting. The majority of drug interactions discovered were modest enough that they were not reflected in the clinic. Drug interactions become more common as people get older.

Incidental findings of cervical magnetic resonance imaging: A retrospective reinterpretation of a large adult population

Kizilgöz, Volkan; Kantarci, Mecit; Aydemir, Hueseyin

BackgroundMagnetic resonance imaging (MRI) of the cervical spine is one of the routine MRI scans of the cervical region in investigating spinal disc pathologies, spinal stenosis, and the detection of spinal lesions, which are the major parameters to be evaluated in this examination.PurposeThe authors of this study are focused on a different aspect of cervical MRI, revealing the incidences and reporting rates of extraspinal incidental findings.MethodsA total of 1000 patients (324 males, 676 females, mean age 47 +/- 14) who had undergone an MRI of the cervical spine were enrolled in this study. The magnetic resonance (MR) images of these patients were re-interpreted with respect to the incidental extraspinal imaging findings. The incidence and reporting rate of each incidental finding encountered during the evaluation were presented in percentages.Results726 patients in this study had at least one incidental lesion. The results of this study revealed that the most common incidental lesions encountered during the reinterpretation of cervical MRI were nasopharyngeal mucosal thickening (n = 442) and thyroid hypertrophy (n = 231). The total reporting rate of incidental findings was 5.29%.ConclusionThere are many data to be reported and evaluated by MR imaging of the cervical spine, not only the main parameters of MRI scanning in the routine daily practice of radiologists. All our colleagues should be aware and careful of these incidental findings, which may be the initial medical data of the patients' diagnoses, or to avoid undesirable medicolegal problems.

Characteristics of the imaging diagnosis of alveolar echinococcosis

Aydin, Sonay; Irgul, Baris; Memis, Kemal Bugra; Kizilgoz, Volkan; Kantarci, Mecit

Alveolar echinococcosis (AE) primarily manifests in the liver and exhibits characteristics resembling those of slow-growing malignant tumours. Untreated Echinococcus multilocularis infection can be lethal. By infiltrating the vascular systems, biliary tracts, and the hilum of the liver, it might lead to various problems. Due to its ability to infiltrate neighbouring tissues or metastasize to distant organs, AE can often be mistaken for malignancies. We present a concise overview of the epidemiological and pathophysiological characteristics of AE, as well as the clinical manifestations of the disease. This article primarily examines the imaging characteristics of AE using various imaging techniques such as ultrasonography, computed tomography (CT), magnetic resonance imaging, diffusion-weighted imaging, and virtual non-enhanced dual-energy CT. We additionally examined the contribution of radiography in the diagnosis, treatment, and monitoring of the condition.

Shear Wave Ultrasonographic Elastography in Pediatric Spleens and Its Role in Differential Diagnosis

Cetin, Turkhun; Tokur, Oguzhan; Bozkurt, Hayrunnisa Bekis; Aydin, Sonay; Memis, Kemal Bugra; Kantarci, Mecit

Shear wave elastography (SWE) has become popular in clinical practice for many diseases. However, there is not adequate research on spleen-related diseases. This study aimed to investigate the potential of quantitative values obtained through SWE in evaluating spleen pathologies in the pediatric population and to demonstrate its performance to differentiate splenomegaly-related diseases. The research group retrospectively included children with pathological diagnoses related to the spleen from November 2016 to April 2021, and they were categorized into three groups, including portal hypertension (PH), benign lymphoid hyperplasia (BLH), and malignant infiltration (MI). Spleen sizes and parenchymal stiffness were also calculated for each group. Subsequently, mean spleen stiffness in each group was compared with normal values within the same age group. In total, 2781 children (1379 children for the study group; 1402 children for the control group) were enrolled in the study. The highest stiffness was observed in the PH group, which is statistically higher than others (p < 0.05). Although the mean spleen stiffness in the group with BLH was higher than the control and MI group, the difference was not statistically significant (p = 0.08). The mean stiffness in the group with MI was significantly lower than both the control group (p = 0.005) and PH (p = 0.01). In conclusion, using SWE in the differential diagnosis of etiologies causing splenomegaly could make an important contribution.

Examination of ethmoidal roof regarding Keros and Yenigun classifications in a Turkish population: a computerized tomography study

Kolak, Merve; Kızılgöz, Volkan; Kantarci, Mecit

Purpose: Understanding ethmoid roof morphology is crucial to prevent complications in endoscopic sinus surgery. This study aimed to evaluate the morphological properties of the ethmoidal roof regarding gender and age differences using Keros and Yenigun classifications on high-resolution computed tomography images. Methods: We retrospectively analyzed 891 high-resolution computed tomography paranasal sinus study images and measured the depth of the cribriform plate in coronal sections and the anterior–posterior length in axial planes. The study retrospectively examined CT images of paranasal sinuses of patients living in the eastern Anatolian region of Turkey. Results: In both Keros and Yenigun Classifications, the most common class was type 2, and the least common class was type 3. According to Keros et al.'s method, no significant difference was observed between men and women (p = 0.698). However, according to Yenigun et al., the average values of women in terms of the anterior–posterior distance of the ethmoid roof were significantly higher than men (p = 0.001). When examined according to age, a very low, negative correlation was revealed regarding Keros and Yenigun classifications (p = 0.047 and p < 0.001 retrospectively). According to Keros and Yenigun's classification, there was no significant difference between the left and right sides (p = 0.488 and p = 0.919, respectively). Conclusion: The morphological properties of the ethmoidal roof have importance to be considered for preoperative planning. Studying larger patient groups and meta-analyses that gather various research results about this subject might help better understand the ethmoidal roof morphology among populations.

A detailed look in radioanatomical aspects of ligamentum arteriosum calcification in pediatric population

Aydemir, Huseyin; Fatihoglu, Erdem; Kizilgoz, Volkan; Kahraman, Sevket; Yazar, Mesut F.; Aydin, Sonay; Kantarci, Mecit

After birth, ductus arteriosus forms ligamentum arteriosum and sometimes can be calcified. Ligamentum arteriosum calcification can be seen as subtle-punctate or coarse-linear between pulmonary artery and the aorta in the non-contrast computed tomography (CT) images, and between the pulmonary conus and the aortic knob in the chest radiography. In this study, we aim to estimate the prevalence of ligamentum arteriosum calcification on non-contrast CT and chest radiography in a pediatric population. Patients aged 0-18 years, who underwent non- contrast CT and chest radiography at Department of Radiology between March 1, 2020 and October 31, 2021 were evaluated retrospectively. It was examined whether there was ligamentum arteriosum calcification in non-contrast CT and chest radiography. If there is calcification, it was recommended as subtle-punctate or coarse-linear, and information was documented by grouping the patients’ age. If ligamentum arteriosum calcification was present, it was divided morphologically into two groups (subtle-punctate and coarse-linear) and classified according to the age groups. The research population consisted of 1003 patients. Non-contrast CT revealed calcification in 25.5% of patients (n=256). 11.3% of patients had subtle-punctate calcification, and 14.3% had coarse-linear calcification. Radiography had a 1.7% (n:17) detection rate for calcification. Ligamentum arteriosum calcification is a common finding in all age groups in the pediatric population.
