Objective: This study aimed to examine internalized stigma, perceived social support, and the quality of life and its associations among patients admitted to a forensic psychiatry unit. Material and Method: A total of 97 patients treated at a High Security Forensic Psychiatry Unit were included. A Sociodemographic Data Form, the Multi-Dimensional Scale of Perceived Social Support (MSPSS), the Internalized Stigma of Mental Illness (ISMI), and the World Health Organization Quality of Life Short Form (WHOQOL) were administered to all patients. Results: The mean ISMI, MSPSS, and WHOQL scores were 74.3±8.4, 38.3±8.4, and 75.9±10.5, respectively. A negative correlation between the total MSPSS and total ISMI scores as well as between total the ISMI and total WHOQOL scores was found; on the other hand, a positive and significant association was determined between the total MSPSS and total WHOQOL scores (p<0.001, r=-0.367; p<0.001, r=-0.550; p<0.001, and r=0.496, respectively). The MSPSS total and sub-scale scores, age, alienation, stereotype endorsement, and perceived discrimination predicted the total WHOQOL score (R2=0.613; Model F=12.242; p<0.001). Conclusion: Forensic psychiatry patients experience internalized stigma and reduced social support. Increasing levels of internalized stigma were associated with progressively lower levels of perceived social support and life quality. Predictors of the quality of life include the perceived social support, stereotype endorsement, discrimination, and alienation. This study sheds some light on factors affecting the quality of life in this relatively under examined group of forensic psychiatry patients.

Yildiz, Sevler; Atmaca, Murad; Kılıçaslan, Aslı Kazğan; Tabara, Muhammed Fatih

Aim: Gambling disorder (GD) is a mental illness with an increasing frequency in society that characterized by the urge to gamble despite negative consequences and desire to quit. In this study, we aimed to examine the mean platelet volume (MPV), neutrophil/lymphocyte ratio (NLR), platelet/lymphocyte ratio (PLR) and the relationship between these values and impulsivity in patients with GD.Material and Method: 42 patients diagnosed with gambling disorder and 42 healthy controls were included in this study. Sociodemographic data form, Barratt Impulsivity Scale Short Form (BIS-11-SF) were administered to all participants, and then a complete blood count was requested from these individuals. NLR, PLR and MPV were measured according to complete blood count. Results: The mean BSI-11-SF total score of the patient group was found to be statistically significantly higher than the control group (p <0.05). It was found that while the motor impulsivity scores of GD patients increased, the MPV values significantly decreased (r=-0.33, p = 0.03). There was no significant difference between the patient and control groups in terms of NLR (p = 0.288), PLR (p = 0.377) and MPV (p = 0.883) values. In addition, in the patient group, there was a weak negative correlation between age and motor impulsivity score and a weak negative correlation between motor impulsivity score and MPV (r=-0.33, p=0.03), (r=-0.31, p=0.04).Conclusion: It was concluded that an individual’s diagnosis with gambling disorder had a high level of impulsivity, but there was no relationship between gambling and complete blood count parameters. Further studies are needed to explain the etiopathogenesis of gambling disorder.

Yildiz, Sevler; Kurt, Osman; Korkmaz, Sevda; Kılıçaslan, Aslı Kazğan

Aim: In the present study, we aimed to investigate the differences in type D personality and self-esteem between patients with conversion disorder (CD) and the control group. Material and Method: We carried out the study with 100 patients diagnosed with CD and 100 matching healthy individuals. We used a sociodemographic information form, the Type D Personality Scale (DS-14), and the Rosenberg Self-Esteem Scale (RSES) as the data collection tools. Considering that the scales should have high discriminative powers, we calculated the cut- off points on each scale performing the ROC analysis. Results: The results showed that the patients with CD had significantly higher negative affectivity (NA), social inhibition (SI), and DS-14 total scores than the control group (p<0.001). Also, the patient group obtained significantly higher scores on the RSES than the control group, indicating lower self-esteem among the patients (p<0.001). Finally, we concluded significant positive correlations between the RSES scores and NA (r=0.549 p<0.001), SI (r=0.410 p<0.001) and DS-14 total scores (r=0.521 p<0.001). Conclusion: Overall, we found that CD patients had type D personality traits and low self-esteem. As the patients had decreased self-esteem, their DS-14 scores increased. Uncovering SI and NA to be high in these patients may drive physicians to take measures to mitigate these situations and adopt a different perspective on CD. In addition, the results may contribute to better addressing patients’ conversion symptoms and personality traits compatible with CD.

Yildiz, Sevler; Kurt, Osman; Korkmaz, Sevda; Kılıçaslan, Aslı Kazğan

The present study aimed to determine the possible correlation between medical treatment [oral antipsychotics (OAP), long-acting injectable antipsychotics (LAI), oral + long-acting injectable antipsychotics (OAP + LAI)] and symptom severity, and the compliance of schizophrenia patients with the medication during the COVID-19 pan- demic. The study was conducted with 100 schizophrenia patients who applied to the psychiatry outpatient clinic during the pandemic and met the study criteria. Sociode- mographic data form, Positive and Negative Symptom Scale (PANSS), and Functional Recovery Scale in Schizophrenia (FRSS) were applied to each participant. The rate of patients who quit current treatment was 19%. A statistically significant difference was determined between the patients on OAP and those on LAI, and between those on OAP and those on OAP + LAI based on the overall scale scores (p <0.001). There were significant differences between the FRSS scores of the patients who discontinued treatment, those who continued medication, and those who switched to depot antipsychotic medication (p <0.001). There was a significant difference between those who discontinued medication and those who continued medication based on the overall scale scores (p <0.001). In the present study, it was determined that the symptoms of patients on only LAI medication and those in OAP + LAI medication were lower, and their functionality scores were higher when compared to the patients on only OAP medication. It was also determined that among the schizophrenia patients who applied to psychiatry clinic during pandemic, those who were on LAI exhibited better func- tionality and lower symptom burden when compared to other treatment modalities. During the pandemic, it was observed that LAI medication protected the patients better.

Evaluation of depression, self-esteem and hopelessness in patients admitted to psychiatry outpatient clinic in geriatric population during COVID-19 outbreak

Yildiz, Sevler; Uğur, Kerim; Kurt, Osman; Kılıçaslan, Aslı Kazğan

Infection control measures taken to ensure social isolation during the COVID-19 pandemic process seem to negatively affect the mental health of elderly individuals. In this study, it is aimed to examine the effects of pandemic on the mental health of the elderly. 150 patients over the age of 65 who were admitted to the psychiatry outpatient clinic for the first time during the pandemic period were included in the study. Socio demographic data form, Geriatric Depression Scale (GDS), Beck Hopelessness Scale (BHS), Rosenberg Self Esteem Scale (RSE) were applied to all cases after the psychiatric interview conducted by us. In the geriatric population, it was observed that there was a significant negative correlation between BHS and RSE (r = -0.241 p = 0.003), and a positive significant relationship between BHS and GDS (r = 0.478 p <0.001). There was no significant correlation between RSE and GHS (r = -0.042 p = 0.614). During the COVID-19 pandemic, it is understood that the hope level and self-esteem of the geriatric population are low, and their mood is depressive. Among the measures to be taken during infection control, we think that elderly people should be given psychosocial support, and thus a healthier environment can be offered to these individuals in terms of physical and mental aspects.

Molecular Mechanism of the Protective Effect of Tianeptine Against Ketamine-Induced Cardiac Injury in Rats

Yildiz, Sevler; Ugur, Kerim; Tas, Hakan G.; Ozcicek, Fatih; Kuyrukluyildiz, Ufuk; Mendil, Ali S.; Cankaya, Murat; Mamedov, Renad; Suleyman, Halis

Ketamine is a short-acting anesthetic drug that is derived from phencyclidine. Ketamine is used to treat depression and chronic pain disorders, as well as for anesthesia, analgesia, and sedation. Ketamine’s sympathomimetic characteristic causes cardiotoxicity. The pathophysiology of ketamine’s harmful impact has been linked to reactive oxygen species (ROS) and proinflammatory cytokines such as interleukin 1 beta (IL-1), interleukin 6 (IL-6) and tumor necrosis factor alpha (TNF-α). Tianeptine is an antidepressant that works similarly to tricyclic antidepressants. According to studies, tianeptine reduces the production of proinflammatory cytokines such as ROS, IL-1, IL-6, and TNF-α. Tianeptine has a sympatholytic action as well. All of this evidence suggests that tianeptine might help to reduce ketamine cardiotoxicity. The goal of our research is to use biochemical and histological techniques to see how tianeptine affects ketamine-induced cardiotoxicity in rats.

Kazğan, Aslı; Korkmaz, Sevda; Yildiz, Sevler; Korkmaz, Hasan; Telo, Selda; Atmaca, Murad

Objective: In hypertensive people, some biochemical parameters that are associated with oxidative stress and endothelial functions may play a role in the manifestation of anxiety symptoms, and even in the intensification of clinical symptoms of the disease. The purpose of our correlative study is to examine the association between anxiety severity and levels of oxidative stress parameters of Urotensin-II and S100B protein in patients with essential hypertension. Method: A total of 153 patients, who have applied Sociodemographic and Clinical Data Form, Beck Anxiety Inventory (BAI), Beck Depression Inventory and venous blood samples were taken after blood pressure measurement. S100B and Urotensin-II levels were measured with Enzyme-Linked Immunosorbent Assay method. Results: A positive correlation was determined between depression and anxiety scores, and systolic blood pressure (p=0.049, r= 0.160; p<0.01, r=0.292). There was a positive relation between BAI scores and Urotensin-II levels (p= 0.043, r= 0.164). A positive correlation was determined between Urotensin-II levels and S100B levels (p<0.01, r= 0.711). A statistically significant difference was observed in serum Urotensin-II levels between depression group and no depression group (p=0.017). Discussion: It was determined in our study that Urotensin-II level increased with increasing anxiety level in essential hypertension patients. It was also determined that Urotensin-II level was higher in hypertensive patients with depression compared to patients who don't have depression. Our study will shed light on new studies that will investigate the contribution of Urotensin- II and S100B in the etiology of patients diagnosed with essential hypertension, which is very commonly accompanied by anxiety and depressive symptoms.
