Diagnostic Comparison of MRI/Ultrasound Fusion Biopsy and Systematic Biopsy in PI-RADS 4-5 Prostate Lesions

    Levent, Akın; Durmaz, Önder

Objective: Multiparametric magnetic resonance imaging (mpMRI) has transformed prostate cancer (PCa) diagnosis by enabling lesion-targeted biopsies. The optimal biopsy approach—systematic biopsy (SB), MRI-ultrasound fusion targeted biopsy (TB), or their combination (CB)—remains debated, especially in patients with high-risk Prostate Imaging-Reporting and Data System (PI-RADS) 4-5 lesions. This study compared these strategies for detecting overall and csPCa. Methods: This retrospective study included 330 patients who underwent MRI-ultrasound fusion TB with 12-core SB between July 2022 and May 2025. Only lesions classified as PI-RADS v2.1 category 4 or 5 were analyzed. Detection rates of overall PCa and csPCa (Gleason score ≥ 7) were compared among SB, TB, and CB. Subgroup analyses were performed by PI-RADS category and lesion location (anterior, middle, posterior). Logistic regression identified independent predictors of csPCa. Results: A total of 429 lesions were evaluated. Overall, PCa detection rates were 62% for SB, 70% for TB, and 77% for CB (P < 0.001); the corresponding csPCa rates were 47%, 56%, and 63% (P < 0.001). TB detected csPCa more effectively than SB (56% vs. 47%, P < 0.01). PI-RADS 5 lesions had higher detection rates than PI-RADS 4 lesions across all methods. CB achieved the highest rates in all locations, especially in anterior and middle lesions. Age, positive digital rectal examination, prostate-specific antigen density, prior biopsy, and PI-RADS category 5 were independent predictors of csPCa. Conclusion: In PI-RADS 4–5 lesions, combined SB and TB provide the highest detection rate for csPCa. While TB improves selective detection, SB remains essential to avoid missing significant disease.

The Relationship Between Mammographic Features of BI-RADS 4 and 5 Calcifications and Pathological Diagnoses

Babatürk, Andelib; Kaymaz, Esin; Seyfettin, Ayça; Eskalen, Zeynep; Büyükceran, Emre Utkan; Memis, Kemal Bugra; Gültekin, Serap

Objective: To evaluate the relationship between mammographic features of BI-RADS category 4 and 5 breast calcifications and their corresponding pathological diagnoses, and to determine whether specific morphological or distribution patterns are predictive of malignancy. Methods: This retrospective study included 54 patients who underwent biopsy for suspicious breast calcifications between August 2021 and March 2024. Among these patients, 30 underwent stereotactic vacuum-assisted biopsy, and 24 underwent wire-guided localization followed by excisional biopsy. The mammographic morphology and distribution patterns of the calcifications were assessed using the BI-RADS lexicon, and the corresponding histopathological diagnoses were recorded. The association between mammographic features and pathological outcomes was subsequently analyzed. Results: Fine pleomorphic calcifications were the most common morphology (40.7%). A grouped distribution was the predominant pattern (61.1%). Benign lesions were mostly categorized as BI-RADS 4A, B3 lesions as BI-RADS 4B, and DCIS cases as BI-RADS 4C (P = 0.03). The Grouped distribution was more frequent among benign and B3 lesions, whereas DCIS exhibited more frequent linear and segmental patterns (P = 0.01). Malignancy rates were highest for fine linear/branching and fine pleomorphic morphologies. Conclusion: A significant association exists between the mammographic characteristics of BI-RADS 4 and 5 calcifications and their pathological outcomes. In our study, fine pleomorphic morphology and linear or segmental distribution patterns consistently indicated a increased likelihood of malignancy, particularly ductal carcinoma in situ, whereas benign and B3 lesions were more commonly associated with grouped distribution patterns. These results highlight that detailed evaluation of calcification morphology and distribution on mammography can improve diagnostic confidence, aid in distinguishing higher-risk lesions, and contribute to more informed biopsy decision-making.

Evaluation of Aortic and Pulmonary Artery Diameters Using Computed Tomography Angiography

    Kabalci, Halime; Gülmez, Ali Osman

Objective: The truncus pulmonalis, the main pulmonary artery (MPA), originates from the right ventricle at the level of the upper edge of the left third rib cartilage and carries blood to both lungs. After a short course, it branches into two arteries beneath the aortic arch: the right pulmonary artery (RPA) and the left pulmonary artery (LPA). The aorta, the main artery that carries blood to the body, originates from the left ventricle. It is divided into three sections: the ascending aorta (AA), the aortic arch, and the descending aorta. Methods: Between January and December 2023, the files of adult patients who were suspected of having pulmonary embolism and who underwent thoracic CT angiography (CTA) at the Erzincan Mengücek Gazi Training and Research Hospital Emergency Department were reviewed. The patients’ DICOM-format images were exported. These images were uploaded to the 3D-Slicer (version 5.10.0 for Windows) and the MPA, RPA, LPA, and AA diameters were measured in the axial plane. The MPA-to-AA diameter ratio was calculated for each patient and designated PA:A. Results: This study included 52 men and 50 women, for a total of 102 patients. The patients’ average age was 67. No significant difference in vessel diameters was observed between genders. AA, MPA, RPA, and LPA diameters were higher in the >67 age group than in the ≤67 age group. There was no significant difference in the PA:A ratio between age groups. A positive, moderate, and statistically significant correlation was found between age and the diameters of AA, MPA, RPA, and LPA. No significant correlation was found between age and the pulmonary artery-to-aorta (PA:A) ratio. A strong, positive, and statistically significant correlation was found between MPA diameter and the diameters of RPA and LPA. A positive, moderately significant correlation was found between MPA diameter and AA diameter. A strong, significant positive correlation was found between RPA diameter and LPA diameter. Conclusion: Aortic and pulmonary artery diameters have been evaluated using various methods in previous studies. To better understand the clinical importance of aortic and pulmonary artery diameters in cardiopulmonary diseases, it is necessary to know their normal reference values. The Framingham Heart Study reported an average MPA diameter of 25.1 mm and an average PA:A ratio of 0.77. In our study of the Turkish population, the mean AA, MPA, RPA, and LPA diameters were 34.1 mm, 25.9 mm, 21.1 mm, and 20.2 mm, respectively, and the PA:A ratio was 0.78. Further studies in larger populations are needed to establish reference values for aortic and pulmonary artery diameters in healthy individuals.

Diagnostic Effectiveness of Ischiofemoral Space and Quadratus Femoris Distance Measurements in Ischiofemoral Impingement Syndrome

Kızılgöz, Volkan; Sunar, Mukadder; Gökgöz, Mehmet Burak; Kolak, Merve

Objective: To ascertain whether measurements of the quadratus femoris and ischiofemoral intervals obtained on pelvic magnetic resonance imaging (MRI) examinations are useful for diagnosing ischiofemoral impingement (IFI) syndrome. Methods: MRIs of 178 patients (104 females and 74 males) were reinterpreted bilaterally for signal changes on T2-weighted sequences in the ischiofemoral space (IFS) and quadratus femoris space (QFS), indicating IFI. Receiver operating characteristic (ROC) curves were plotted for each measurement parameter, and the corresponding area under the curve (AUC) values were reported. A P value of less than 0.05 was considered to indicate a significant difference, and the results were assessed at the 95% confidence level. Results: There was no significant difference in age between females and males (P = 0.064). Radiologically, 17.7% of all participants were diagnosed with IFI. Significant differences were observed between IFS and obturator femoris space values by gender, with both values being significantly lower in women. A higher percentage of patients testing positive for IFI was observed in women compared with men. In the ROC analysis conducted according to IFS, the AUC was 0.840. The highest value of sensitivity × specificity was 21.29 mm. The sensitivity at this value was 0.676. The specificity at this value was 0.857. In the ROC analysis using QFS, the AUC was 0.856. The highest value of (sensitivity × specificity) was 16.17 mm. The sensitivity at this value was 0.765. The specificity at this value was 0.794. Conclusion: The results of the current study indicated a significant relationship between the IFS and QFS measurements and IFI. These intervals showed a significant negative correlation with age. This study also identified important cut-off values for IFS and QFS intervals that indicate IFI, with relatively high sensitivity and specificity.

Predicting Breast Cancer Biomarker Expression with Diffusion-Weighted Magnetic Resonance Imaging

Oztepe, Muhammet Fırat; Memis, Kemal Bugra; Çelik, Ayşe Sena

Objective: To evaluate the diagnostic performance of diffusion-weighted magnetic resonance imaging (DWI) and apparent diffusion coefficient (ADC) values in differentiating benign and malignant breast lesions, and to investigate correlations between ADC values and immunohistochemical (IHC) biomarkers (estrogen receptor, progesterone receptor, human epidermal growth factor receptor 2, Ki-67) as well as axillary lymph node status. Methods: This retrospective study included 148 female patients (159 breast lesions) who underwent preoperative breast magnetic resonance imaging (MRI) between January 2022 and December 2024. DWI was performed using b values of 0 and 1000 s/mm2, and mean ADC values were calculated from regions of interest placed within solid tumor areas. Histopathological and IHC analyses were used to classify lesions and molecular subtypes. Results: Among the 159 lesions, 56 (35.2%) were malignant and 103 (64.8%) were benign. Malignant lesions exhibited significantly lower ADC values than benign ones (0.92 × 10-3 mm2/s vs. 1.66 × 10-3 mm2/s). The optimal ADC cut-off for malignancy was 1.24 × 10-3 mm2/s, yielding 95.2% sensitivity and 89.1% specificity. Hormone receptor-positive tumors and lesions with high Ki-67 index showed lower ADC values. No significant correlation was found between ADC and human epidermal growth factor receptor 2 status. Triple-negative breast cancers demonstrated the highest ADC values among subtypes. ADC values of metastatic axillary lymph nodes were significantly lower than those of contralateral benign nodes (0.78 × 10-3 vs. 1.82 × 10-3 mm2/s). Conclusion: ADC values are effective in distinguishing malignant from benign breast lesions and provide non-invasive insights into tumor biology. Lower ADC values correlate with malignancy, hormone receptor positivity, proliferative activity, and metastatic nodal involvement. DWI is a reliable, non-contrast imaging biomarker that may enhance personalized evaluation of breast cancer.

The Relationship Between Non-alcoholic Fatty Liver Disease and Epicardial Adipose Tissue Thickness, Carotid Intima Media Thickness, and Aortic Propagation Velocity

Aydin, Sonay; Ozcicek, Fatih; Hayme, Serhat; Coşgun, Muharrem Said; Ozturk, Ersin; Öztürk, Gülay Kızılgün

Objective: Cardiovascular disease is prevalent among individuals diagnosed with non-alcoholic fatty liver disease (NAFLD). Epicardial adipose tissue (EAT), carotid intima-media thickness (CIMT), and aortic propagation velocity (APV) are associated with atherosclerosis and cardiovascular disease (CVD). The aim of this study is to determine whether NAFLD is associated with EAT, CIMT, and APV. Methods: Individuals aged ≥18 and <65 years who presented for routine follow-up were included. The study population comprised 75 healthy controls (40 females, 35 males) and 141 patients with NAFLD (68 females, 73 males). Epicardial adipose tissue thickness, carotid intima-media thickness, and aortic propagation velocity were evaluated using transthoracic echocardiography in patients who were diagnosed with hepatosteatosis by ultrasound during routine check-ups at the internal medicine clinic. Results: NAFLD was positively associated with epicardial adipose tissue thickness and carotid intima–media thickness, and negatively associated with aortic propagation velocity. Patients with NAFLD had significantly higher EAT and CIMT values and lower APV values than controls. Logistic regression analysis identified body mass index (BMI) and EAT as independent predictors of hepatosteatosis. Each unit increase in BMI was associated with a 1.29-fold increase in the risk of hepatosteatosis, whereas each unit increase in EAT was associated with a 5.49-fold increase. Conclusion: Given the established associations of CIMT, APV, and EAT with atherosclerosis and cardiovascular disease, assessment of patients with NAFLD using these noninvasive parameters may help identify subclinical cardiovascular risk.

Assessment of Potentially Unnecessary Breast Magnetic Resonance Imaging Utilization and Its Economic Impact: A Retrospective Study

Senbil, Duzgun Can; Kiliçparlar, Muslum Burak; Kantarci, Mecit; Aydin, Sonay

Background: In this study, breast magnetic resonance imaging (MRI) examinations were retrospectively reviewed. This study evaluated whether the indications for breast MRI were consistent with the recommendations of the Breast Imaging Reporting and Data System (BI-RADS)atlas. Additionally, the study assessed whether prior ultrasound and mammography findings were sufficient for diagnosis. The aim was to identify potentially unnecessary breast MRI examinations and their associated financial burden. Methods: Breast MRI reports were compared with previous mammography and ultrasound reports. Patients for whom breast MRI provided no additional diagnostic benefit were identified, and the indications for MRI examination were analyzed. Results: Comparison of BI-RADS categories assigned on prior ultrasound and mammography with those assigned after breast MRI revealed that 321 (64.3%) of the 499 patients could have been adequately managed without breast MRI. In these patients, MRI findings did not alter clinical management or treatment decisions. Conclusion: Breast MRI is a costly imaging modality with limited accessibility in many healthcare settings. Therefore, its economic impact should be carefully considered when determining appropriate indications for its use. These findings suggest that the absence of restrictive criteria for breast MRI requests may contribute to substantial financial losses due to unnecessary examinations.

Emergency radiology in displaced populations: imaging and practical challenges

Çakmak, Vefa; Çakmak, Pınar; Herek, Duygu; Aydın, Sonay

Migrants and refugees tend to use emergency departments as their primary source of care, leading to a substantial increase in emergency radiological imaging. Migrants and refugees have risk factors, such as low vaccination rates, poor hygiene, malnutrition, and inadequate self-care, which increase their risk of contracting infectious diseases. For example, the prevalence of tuberculosis among refugee populations in host countries is increasing, and imaging findings related to tuberculosis are frequently observed by radiologists. Strengthening screening programs in host countries for tuberculosis and other infectious diseases among migrant populations can help mitigate the risk of transmission within migrant communities. Another condition, cystic echinococcosis, is more common among refugees and migrants from the Middle East and Afghanistan. For radiologists working in host countries, echinococcosis involving the liver and lungs should be considered in the differential diagnosis. Both intentional (e.g., violence, assault) and unintentional (e.g., workplace injuries, accidents) traumas are frequently encountered in emergency radiology, particularly among refugees and immigrants. Workplace injuries are four times more common among migrants and refugees than among the local population due to their work in high-risk industries, such as construction and heavy industry, and emergency radiology frequently encounters radiological findings of organ injuries due to falls from height. In addition, healthcare professionals in emergency radiology face various challenges when dealing with migrant and refugee patients, such as communication barriers, social security problems, and psychological distress. Cardiac Magnetic Resonance Findings and NT-proBNP in Heart Failure with Reduced Ejection Fraction

Memis, Kemal Bugra; Doğan, İffet; Şahin, Defne; Gulmez, Ali Osman; Özer, Ece; Kantarci, Mecit

Background/Objectives: N-terminal pro-B-type natriuretic peptide (NT-proBNP) is an established heart failure biomarker, but its associations with cardiac magnetic resonance (CMR) parameters in patients with reduced ejection fraction remain unclear. This study examined the associations between serum NT-proBNP concentrations and CMR-derived functional, structural, and tissue characterization parameters in patients with HFrEF and evaluated which imaging parameters remained associated with NT-proBNP after multivariable adjustment. Methods: This retrospective single-center study included 92 consecutive patients with HFrEF who underwent CMR and NT-proBNP measurement within a 24 h window, with a median absolute interval of 6 h (range, 1–15 h) between the two assessments. CMR analysis included left ventricular ejection fraction (LVEF), left ventricular and left atrial (LA) cavity diameters, myocardial mass, extracellular volume fraction (ECV), the late gadolinium enhancement (LGE) burden, and the T2 signal intensity (T2-SI) ratio. Spearman correlation, univariate linear regression, and multivariable linear regression analyses were performed using log-transformed NT-proBNP values. Results: Log NT-proBNP demonstrated significant correlations with LVEF (rs = −0.359, p < 0.001), T2-SI ratio (rs = −0.332, p = 0.001), ECV (rs = 0.383, p < 0.001), and LA diameter (rs = 0.347, p < 0.001). No statistically significant correlations were observed between log-transformed NT-proBNP and LGE burden, LV cavity diameter, myocardial mass, LV wall thickness, age, or the recorded comorbidities. In multivariable analysis, LVEF, T2-SI ratio, and LA diameter remained associated with NT-proBNP after adjustment for the available covariates. Conclusions: Lower LVEF, a lower T2-SI ratio, and a larger LA diameter were associated with higher NT-proBNP levels in patients with HFrEF. The T2-SI ratio and LA diameter should be considered exploratory imaging markers requiring prospective validation.

MRI-Derived Scapular Superomedial Angle and Acromial Morphology: A Retrospective Cross-Sectional Reliability and Morphometric Association Study

Kandefer, Sami; Gür, Volkan; Gökgöz, Mehmet Burak; Can, Muhammet Ali; Taş, Metin; Memiş, Kemal Buğra; Kantarcı, Mecit; Koçkara, Nizamettin; Yapıcı, Furkan

Background/Objectives: The magnetic resonance imaging (MRI)-derived scapular superomedial angle (SMA) may serve as a continuous descriptor of scapular geometry. We evaluated whether SMA is associated with acromial morphology, examined SMA and subacromial space (SAS) across acromial categories, and quantified measurement reliability. Methods: In this retrospective cross-sectional study, 306 shoulders from 279 patients (27 bilateral) were analyzed after screening 353 examinations. SMA and SAS were measured independently by two orthopedic observers, and acromial morphology was classified independently by two radiologists, all in blinded sessions. Reliability, group comparisons, receiver operating characteristic (ROC) analyses, and patient-level clustered generalized estimating equations were performed. Results: Interobserver reliability was excellent for SMA (intraclass correlation coefficient [ICC], 0.960) and SAS (ICC, 0.926), and intraobserver reliability was excellent for SMA and good for SAS (ICC, 0.944 and 0.848); acromial classification agreement was almost perfect (linear-weighted kappa, 0.980). SMA increased stepwise across Type I to III acromia (128.7°, 133.9°, 137.2°; p < 0.001), whereas SAS decreased (7.5, 6.8, 6.3 mm; p < 0.001). Each 1° SMA increase was independently associated with Type III morphology (adjusted odds ratio, 1.18; p < 0.001). SMA was not independently associated with rotator-cuff severity (p = 0.842). Because adjacent Type II–III differences approached the minimum detectable change, individual-level discrimination was limited. Conclusions: MRI-derived SMA is a reproducible descriptor associated with acromial morphology at the group level, but adjacent-category differences approached measurement error. SMA should not substitute for direct acromial classification, and external validation is required before clinical thresholds are considered.
