OBJECTIVE: Our aim in the study is to investigate circadian rhythm, season, age, gender, and hemogram parameters in children diagnosed with febrile convulsions. METHODS: The data of a total of 478 children, consisting of 160 patients with febrile convulsions (90 male/70 female) and 318 control groups (healthy, convulsiyon free-febrile), were compared. RESULTS: The average age of all patients was 25.7±14.7 (minimum 6-maximum 60) months, and the most common convulsion age was 13-18 months. Circadian rhythm was similar, application seasons were similar. The male/female ratio was 1.3. MPV in winter was lower than other seasons (p=0.002). At younger ages, lymphocyte count was higher (p=0.048) and NLR was lower (p=0.036). NLR was lower in patients with febrile convulsions with anemia than in patients with febrile convulsions without anemia (p=0.029). NLR was lower in patients with febrile convulsions with anemia than in the convulsion-free febrile control group with anemia (p=0.001). In patients with febrile convulsions, MPV was lower (p=0.033) and NLR was higher (p=0.001) than in the convulsion-free febrile control group. One third of the patients had hypocapnia. CONCLUSION: Young age, anemia, inflammation, and hypocapnia may facilitate the occurrence of febrile convulsions. MPV and NLO, which are hemogram parameters, can guide us about inflammation. Blood gases may provide information regarding hypocapnia. These rapid examinations may help elucidate the etiology. Prospective studies are needed for circadian rhythm in febrile convulsions.

Background. Cardiovascular diseases (CVDs) are highly prevalent among patients with rheumatoid arthritis (RA). Epicardial adipose tissue, serum betatrophin, chemerin, and adropin levels are factors associated with atherosclerosis and cardiovascular involvement. Objectives. This study aimed to investigate the relationship between RA and epicardial fat thickness (EFT), as well as serum betatrophin, chemerin and adropin levels. Materials and methods. This cross-sectional study included 80 patients (62 women and 18 men) diagnosed with RA according to the American College of Rheumatology/The European Alliance of Associations for Rheumatology (ACR/EULAR) 2010 RA classification criteria and 80 healthy controls (64 women and 16 men). Exclusion criteria comprised other autoimmune diseases, CVDs, diabetes mellitus, other endocrine disorders, acute or chronic pancreatic disorders, malignancy, pregnancy, breastfeeding, or antihyperlipidemic drug usage. Serum betatrophin, chemerin and adropin concentrations were measured. Epicardial fat thickness was evaluated with transthoracic echocardiography. Results. Adropin levels were significantly lower in the RA group compared to the control group (p < 0.001). Chemerin levels and EFT were significantly higher in the RA group than in the control group (p = 0.016, p < 0.001, respectively). When assessing the relationship between biomarkers and EFT in RA patients, a strong positive correlation was observed between chemerin and EFT (r = 0.73, p = 0.046) in patients with high disease activity. Conclusions. Epicardial fat thickness, as an indicator of cardiovascular involvement, is higher in patients with RA. Moreover, high chemerin levels and low adropin levels in these patients may be indicative of cardiovascular involvement. Copyright by Author(s)

Erectile dysfunction (ED) is associated with endothelial damage, especially atheroscle-rosis. The search for biomarkers that can predict ED still continues. Adropin is known to affect nitric oxide (NO) bioavailability and energy homeostasis. In our study, we have aimed to investigate the relationship between serum adropin levels and ED. Male patients with and without ED between 40–60 years of age, who presented to the outpatient clinics of urology between November 2019–February 2020, were prospectively included in the study. Biochemical values measured at the time of admission to the outpatient clinic. According to the International Index of Erectile Function-5 (IIEF-5) scores which range between 5 and 25 points, patients with a score ≤21 were considered to have ED. The patients were divided into two groups: as ED and non-ED control groups. Laboratory values obtained at admission to outpatient clinics of urology were compared between groups. Patients with (n: 40), and without (n: 40) ED were included in the study. The mean age (50.2 ± 5.7 years), average body mass index (BMI) (29.7 ± 2.5 kg/m2 ), IIEF score (15.8 ± 6 pts), serum adropin (584.8 ± 172 pg/mL), and total testosterone (396.4 ± 91.7 ng/dL) levels were recorded. Serum adropin and testosterone levels were statistically significantly higher in the non-ED group than in the ED group (712.3 ± 222 pg/mL vs. 511.1 ± 145 pg/mL, p < 0.001 and 420.5 ± 56 ng/dL vs. 374.3 ± 98 ng/dL, respectively p = 0.032). Whereas fasting blood glucose (FBG) values were found to be statistically significantly higher in the ED group (100.2 ± 14 mg/dL vs. 143.8 ± 78 mg/dL, p = 0.001). According to the results of our study, serum levels of adropin which improves endothelial functions were comparatively lower in ED patients, as expected.
RDL Kalıcı Kaydıİncele - ArticleAçık Erişim2024Analysis of Isotretinoin-Induced Alterations in the Levels of Plasma Trace Elements: Investigation of the Relationship Between Magnesium, Phosphorus, Potassium, Zinc, and Treatment-Related Side Effects
Akman, Tugrul Cagri; Yazici, Mustafa; Atila, Alptug; Mertoglu, Cuma
Isotretinoin is an effective treatment against acne vulgaris, but it also causes many side effects during and after the treatment. The relationship between the changes in the levels of plasma trace elements of patients with AV after 3 months of isotretinoin treatment and the side effects was investigated in the study. Plasma samples of 35 patients were collected before and after isotretinoin treatment. Samples were analyzed by Inductively Coupled Plasma–Mass Spectrometer. After treatment, the levels of phosphorus, magnesium, and zinc in plasma increased statistically, while the level of potassium decreased (p < 0.05). The treatment had differing effects on zinc levels based on the gender of the individuals. With treatment, the levels of zinc in the plasma of men showed a greater increase compared to women (p = 7.3e-04). Additionally, the correlation matrix analysis revealed a strong correlation (R > 0.8) between magnesium and calcium. According to the study results, the change in phosphorus and potassium levels shows that isotretinoin affects kidney functions. The results suggest that phosphorus, potassium, magnesium, and zinc are associated with fatigue, dry skin and chapped lips, hair loss, and sebum secretion, respectively. Consequently, the study emphasizes the need for a comprehensive pre-treatment assessment, including monitoring of liver and kidney function as well as the levels of phosphorus and potassium in patients.

Smoking is the leading cause of preventable death. Epicardial adipose tissue (EAT) surrounds the heart surface and creates local and systemic effects secreting the hormones, cytokines, inflammatory mediators. Previous studies demonstrated that both smoking and EAT have a strong association with inflammation and atherosclerosis. Our study aimed to determine the relationship of smoking with EAT thickness and inflammation by evaluating smokers and non-smokers. A total of 259 healthy male and female participants between the ages of 18-65, without a history of chronic disease and with a body mass index within normal limits, were included in a study. EAT thickness measurements were made by transthoracic echocardiography and EAT thicknesses of smokers and non-smokers were compared. In addition, the effects of smoking and EAT thickness on different inflammatory parameters were evaluated. When the EAT thicknesses were compared between smokers and non-smokers (2.60±1.2), a statistically significant difference was found in favor of smokers (3.84±1.84) (p<0.001). A moderate positive correlation was found among age, body mass index, smoking duration in years, pack/year and EAT thickness. The difference among waist circumference, the number of cigarettes smoked daily, diastolic blood pressure, fasting blood glucose, ALT, AST, total cholesterol, triglyceride, LDL-cholesterol, CRP, uric acid, platelet count, MPV values, platelet/lymphocyte ratio and EAT thickness were found meaningful, but the weak correlation in different ratios were determined. Smoking was found to be the most important determinant of EAT thickness. Other determinants of EAT thickness were age, body mass index, CRP and female gender. The significant statistical relationship between smoking and EAT thickness suggests that smoking increases EAT thickness and inflammatory parameters. Co-assessment of the EAT and blood inflammatory parameters in smokers may guide the initiation of medical treatment in primary prevention or other therapies.

It remains important to investigate the changing and impact of routine blood values (RBVs) in order to predict mortality and follow an appropriate treatment in COVID-19 patients. In the study, the importance of RBVs in the mortality of patients with COVID-19 was investigated. The changes in the biochemical, hematological, and immunological parameters of patients who recovered (n = 4364) and died (n = 233) from COVID-19 over time and their relationship with the mortality of the disease were evaluated retrospectively. Odds ratios of the parameters affecting one-month mortality were calculated by running multiple-logistic-regression analysis. The cut off values and diagnostic efficiencies of the parameters that posed a risk for mortality were obtained via receiver operating curve analysis. It was determined that the C-reactive protein (CRP), D-dimer, procalcitonin, erythrocyte-sedimentation-rate (ESR), troponin values were at abnormal levels until death occurred in the patients who died. In addition, the procalcitonin levels were consistently high in patients who died. The patients who died generally had a sustained increase in their leukocyte and neutrophil levels and biochemical variables, and an ongoing decrease in lymphopenia and eosinopenia levels. Although significant changes were observed in liver function tests, cardiac troponin, hemogram values, kidney function tests and parameters related to inflammation in deceased patients, high ESR, international-normalized-ratio (INR), prothrombin-time (PT), CRP, D-dimer, ferritin and red-cell-distribution width (RDW) values, respectively, were the most effective predictive mortality risk biomarkers of COVID-19. In addition, neutrophilia, leukocytosis, thrombocytopenia, erythrocytopenia were other risk predictors of mortality. Indicators was found in this study can be successfully used to predict mortality from COVID-19.

Purpose: The aim of this study was to evaluate the traditional and novel obesity-related indices in the determination of metabolic syndrome in adults and to determine which marker is the better predictor. Materials and Methods: A total of 419 adults between the ages of 18-65 were included in this study. Body weight, height, waist, hip and waist circumference, and blood pressure were measured; fasting blood glucose, total cholesterol, triglyceride, low-density lipoprotein cholesterol and high-density lipoprotein cholesterol values were analyzed. Metabolic syndrome (MetS) was defined using the International Diabetes Federation criteria. The values of 23 obesity-related indices were calculated. Results: The prevalence of metabolic syndrome is 58.7% (male 41.2%; female 67.7%). The triglyceride glucose (TyG) index has the largest area under the curve (AUC) in both men (AUC = 0.894, cutoff = 3.9) and women (AUC = 0.901, cutoff = 3.9). In men, lipid accumulation product (LAP) had the second highest determination for MetS (AUC = 0.880, cut-off = 51.1), followed by TyG-waist/hip (AUC = 0.876, cut-off = 3.7). Cardiometabolic index (CMI) (AUC = 0.872, cut-off value = 1.3) and visceral adiposity index VAI (AUC = 0.868, cut-off value = 4.1) had the second and third largest AUCs, respectively, in women. Conclusion: TyG index is the best predictor of MetS. Waist circumference could be an alternative index in large epidemiology survey due to its convenient and cost-efective characteristics.

RESULTS: Native thiol, total thiol, and native thiol/total thiol were lower in Group 4 than Groups 1, 3, and 5 (p<0.001). However, disulfide/native thiol and disulfide/total thiol were higher in Group 4 than all other groups (p<0.001). IMA was higher in Groups 3 and 4 than all other groups (p<0.001). Ferroxidase was lower in Groups 3 and 4 than Group 2 (p<0.001). CONCLUSION: Thiol-disulfide homeostasis impairment in favor of disulfide may have a function in the progress of diabetic retinopathy. Furthermore, the disruptions of IMA and ferroxidase levels involve in the development of diabetic retinopathy and neuropathy.

Aim: This study is aimed to evaluate the relationship between serum vitamin D levels and metabolic syndrome in hemodialysis patients. Materials and Methods: This study was conducted with participation of 80 patients undergoing hemodialysis for more than 6 months three times a week, aged > 18 years. Their height, dry weight, waist circumference were measured. Biochemical parameters such as serum 25-hydroxyvitamin D, lipid profile, and fasting blood glucose were analyzed. Metabolic syndrome was defined using the National Cholesterol Education Program Adult Treatment Panel III criteria. Severe vitamin D deficiency, 25 (OH) vitamin D < 5 ng/ml; mild vitamin D deficiency, 25 (OH) vitamin D 5-15 ng/ml; vitamin D insufficiency, 25 (OH) vitamin D 16-30 ng/ml, and vitamin D sufficiency, 25 (OH) vitamin D > 30 ng/ml were categorized. Statistical analysis of the data was performed with the use of SPSS version 21.0. Results: 48.4% of hemodialysis patients were identified to have metabolic syndrome. According to the serum levels of vitamin D; 35.0% of patients had severe vitamin D deficiency, 37.4% of patients had mild vitamin D deficiency, 18.8% of patients had vitamin D insufficiency and 8.8% of patients had vitamin D sufficiency (> 30 ng/ml). Vitamin D insufficiency was significantly associated with metabolic syndrome and central obesity. Conclusions: Deficiency/insufficiency is observed in serum 25-hydroxyvitamin D levels in hemodialysis patients. Vitamin D insufficiency is associated with metabolic syndrome.

Background: Reducing and maintaining body weight has become more important than ever as obesity is becoming increasingly common worldwide. This study was aimed to investigate the effects of diets with different protein contents administered to obese women on anthropometric measurements, inflammatory markers, and cardiometabolic risk factors. Materials and Methods: This randomized controlled trial was conducted with sixty volunteering obese women aged between 20 and 45 years. The subjects were divided into two groups in equal numbers. The high-protein (HP) group (n = 30) was administered an iso-caloric HP diet (25% protein, 30% fat, and 45% carbohydrate), and the control group (n = 30) an isocaloric low-protein diet (15% protein, 30% fat, and 55% carbohydrate), and both groups were followed up for 8 weeks. The subjects' descriptive data, anthropometric measurements, homeostatic model assessment-insulin resistance (HOMA-IR), lipid profiles, and high-sensitive C-reactive protein (hs-CRP), tumor necrosis factor-alpha (TNF-), and interleukin-6 (IL-6) levels were analyzed. Results: There was no significant difference at baseline (except for low-density lipoprotein cholesterol [LDL-C]), and end-of-study (except for IL-6, systolic blood pressure [SBP], and diastolic blood pressure) values of parameters between the two groups; after adjusted for baseline measurements, a significant difference was observed between the groups for body weight, body mass index, waist circumference, HOMA-IR, LDL-C, hs-CRP, TNF-, IL-6, and SBP (P = 0.004, P = 0.001, P = 0.003, P = 0.029, P = 0.004, P = 0.016, P = 0.004, P = 0.010, and P = 0.000, respectively) and were greater in the HP group than in the control group (P < 0.05). Conclusion: The HP diet was effective on improvement in HOMA-IR, SBP, LDL-C, hs-CRP, TNF-, IL-6, and resulted in body weight loss.

Mertoglu, Cuma; Huyut, Mehmet Tahir; Arslan, Yusuf; Ceylan, Yasar; Coban, Taha Abdulkadir
How the routine laboratory tests change in terms of coronavirus disease 2019 (COVID-19) was retrospectively analyzed in a large group of patients. Biochemical, hematological and inflammatory variables of a totaly 555 (n = 532 in non-intensive care unit (non-ICU), n = 23 in ICU) patients diagnosed with COVID-19 were analyzed accessing them through the laboratory information system. White blood cell (WBC), neutrophil (NEU), platelet large cell ratio, neutrophil to lymphocyte ratio (NLR), derived NLR (d-NLR), aspartate aminotransferase, urea, creatine kinase (CK) myocardial band (CK-MB), procalcitonin (PCT) values were high whereas lymphocyte (LYM), eosinophil, red blood cells (RBC), hemoglobin, lymphocyte to monocyte ratio, estimated glomerular filtration rate values were low in the ICU group when compared with non-ICU. WBC, NEU, red cell distribution width, alanine transaminase, creatinine, urea, CK-MB, CK, direct bilirubin, lactate dehydrogenase, glucose, C-reactive protein, erythrocyte sedimentation rate, ferritin, D-dimer, PCT and international normalized ratio values increased while RBC, hemoglobin, hematocrit, mean corpuscular volume and total bilirubin values decreased in a significant proportion of patients in both groups based on the normal reference ranges. LYM count was found to be low in a significant number of patients (57.5%) especially in the ICU group and as an important risk factor and diagnostic parameter on admission to ICU (OR: 125, AUC: 0.74). Routine laboratory tests provide important information in terms of both diagnosis and severity of COVID-19. Lymphopenia is a condition that should be monitored which indicates the severity of the disease.
