The Effect of Acute Psychotic Period on Physiological Values Measured in Blood in Male Patients With Schizophrenia

Celikel, Guler; Yalcin, Suat; Kalelioglu, Tevfik; Karamustafalioglu, Nesrin

Background: There is increasing evidence that plasma osmolarity is an independent prognostic factor associated with mortality in patients with cardiovascular disease. Whole blood viscosity (WBV) is also associated with established cardiometabolic risk factors for cardiovascular disease. The present study aimed to investigate the effects of the acute phase of schizophrenia on physiological parameters, including WBV and plasma osmolarity, in male patients with schizophrenia. Methods: A total of 121 male patients with schizophrenia were included in this study. Routine blood test results were obtained from hospital records, and the laboratory values required for the calculation of viscosity and osmolarity were recorded. Laboratory data from the patients' most recent acute exacerbation were retrieved from hospital records and used as the acute-phase data. Results: There was no statistically significant difference in serum osmolarity between the acute and remission phases (p = 0.360). Blood urea nitrogen (BUN) and glucose levels were significantly higher during the remission phase, whereas sodium levels were significantly higher during the acute phase. No significant differences were observed in WBV values between the acute and remission phases (p > 0.05). Furthermore, there were no significant differences in WBV-related parameters, including hematocrit (HCT) and total protein, between the two phases. Low-density lipoprotein (LDL), high-density lipoprotein (HDL), total cholesterol, and triglyceride levels were significantly higher during the remission phase. Conclusions: Although differences were observed in osmolarity-related parameters and viscosity-associated metabolic variables between the acute and remission phases in male patients with schizophrenia, no statistically significant differences were found in serum osmolarity or WBV between the two phases.

    Ozdemir, Sevki; Sari, Haticetul Kubra

Objective: Evaluating the microvascular structure in rheumatological diseases provides important information for understanding the pathogenesis and clinical management of these diseases. Nail capillaroscopy is a reliable method that allows non-invasive examination of these changes. This study aimed to evaluate nail capillary findings in rheumatological diseases using digital dermoscopy and to compare capillary patterns in different disease groups. Methods: The study included 200 patients diagnosed with rheumatological diseases and 50 healthy volunteers. The nail bed capillaries of all participants were evaluated using a digital dermatoscope. Capillary examination assessed the presence of edema, tortuous, elongated, dilated, and brush-like capillaries, microhemorrhages, and avascular areas. Statistical analyses were performed using appropriate parametric and non-parametric tests, and a P value < 0.05 was considered statistically significant. Results: The frequency of capillary abnormalities was significantly higher in the patient group than in the control group. The most frequently observed findings were edema and tortuous, elongated capillaries. Capillary patterns differed by disease group, with more specific findings particularly prominent in patients with systemic sclerosis. Conclusion: Nail capillaroscopy is an effective, easily applicable, and non-invasive method for evaluating microvascular changes in rheumatological diseases. Capillary patterns observed in different disease groups can provide clinically valuable information for diagnosis and disease monitoring.

    Koç, Alparslan; Polat, Eyup Can

Objective: Clinical pharmacy is a health field that is spreading all over the world. Numerous studies have demonstrated that clinical pharmacy services offered to healthcare providers lead to improved therapeutic outcomes. Intensive care units are the primary clinics where clinical pharmacists work. Due to factors such as variable clinical conditions of patients and the use of multiple medications, drug-related problems are frequently encountered in intensive care units. This study aims to conduct a comprehensive bibliometric analysis of research on clinical pharmacy in intensive care units by examining productivity trends, citation impact, collaboration networks, and thematic evolution using VOSviewer software. Methods: We used all relevant studies from the Web of Science database published between 1981 and 2024 that included the keywords "intensive care unit" and "clinical pharmacy" for this analysis. The analyses were performed using the VOSviewer (v1.6.20) software. Results: A total of 866 publications were analyzed. The country with the most publications was the United States. Original research articles were the most common publication type, and the most cited article was by Tsuji et al. (n=535). The most frequently used keywords were "critical care", "intensive care unit", "clinical pharmacy", and "clinical pharmacist". Conclusion: Our results show that research on clinical pharmacy in intensive care units is increasing steadily every year and that the services provided in these settings have a beneficial effect on treatment outcomes. Therefore, our study will be an important guide for researchers investigating clinical pharmacy services in intensive care units.

Diagnostic Effectiveness of Ischiofemoral Space and Quadratus Femoris Distance Measurements in Ischiofemoral Impingement Syndrome

Kızılgöz, Volkan; Sunar, Mukadder; Gökgöz, Mehmet Burak; Kolak, Merve

Objective: To ascertain whether measurements of the quadratus femoris and ischiofemoral intervals obtained on pelvic magnetic resonance imaging (MRI) examinations are useful for diagnosing ischiofemoral impingement (IFI) syndrome. Methods: MRIs of 178 patients (104 females and 74 males) were reinterpreted bilaterally for signal changes on T2-weighted sequences in the ischiofemoral space (IFS) and quadratus femoris space (QFS), indicating IFI. Receiver operating characteristic (ROC) curves were plotted for each measurement parameter, and the corresponding area under the curve (AUC) values were reported. A P value of less than 0.05 was considered to indicate a significant difference, and the results were assessed at the 95% confidence level. Results: There was no significant difference in age between females and males (P = 0.064). Radiologically, 17.7% of all participants were diagnosed with IFI. Significant differences were observed between IFS and obturator femoris space values by gender, with both values being significantly lower in women. A higher percentage of patients testing positive for IFI was observed in women compared with men. In the ROC analysis conducted according to IFS, the AUC was 0.840. The highest value of sensitivity × specificity was 21.29 mm. The sensitivity at this value was 0.676. The specificity at this value was 0.857. In the ROC analysis using QFS, the AUC was 0.856. The highest value of (sensitivity × specificity) was 16.17 mm. The sensitivity at this value was 0.765. The specificity at this value was 0.794. Conclusion: The results of the current study indicated a significant relationship between the IFS and QFS measurements and IFI. These intervals showed a significant negative correlation with age. This study also identified important cut-off values for IFS and QFS intervals that indicate IFI, with relatively high sensitivity and specificity.

Nutritional and Inflammatory Indicators for Identifying Sarcopenia in Hemodialysis Patients

Kara, Ali Veysel; Şahin, Hilal; Doğan Akagündüz, Diğdem

Objective: Sarcopenia represents a frequent clinical condition in patients receiving maintenance hemodialysis and is linked to adverse functional and health-related outcomes. Owing to its multifactorial nature, early identification of patients at risk remains challenging in routine practice. This study evaluated nutritional and inflammatory indicators associated with sarcopenia and their ability to identify patients with sarcopenia. Methods: This cross-sectional study included 72 clinically stable adults receiving maintenance hemodialysis between August and September 2024. Sarcopenia was defined according to European Working Group on Sarcopenia in Older People 2 (EWGSOP2) criteria, requiring low muscle strength and reduced muscle mass assessed by handgrip dynamometry and bioelectrical impedance analysis. Nutritional status was evaluated using the Subjective Global Assessment (SGA), and dietary energy and protein intake were assessed using 2 non-consecutive 24-hour recalls analyzed with BEBİS software. Serum albumin, C-reactive protein (CRP), and neutrophil-to-lymphocyte ratio were measured. Multivariable logistic regression and receiver-operating characteristic analyses were performed. Results: Sarcopenia was identified in 18 patients (25%). Sarcopenic individuals had significantly lower protein and energy intake, lower serum albumin levels, and higher CRP concentrations than non-sarcopenic patients. In multivariable analyses, low protein intake (<1.0 g/kg/day), inadequate energy intake (<25 kcal/kg/day), hypoalbuminemia (<3.5 g/dL), elevated CRP (>5 mg/L), and SGA category B/C were independently associated with sarcopenia, whereas adequate energy intake was inversely associated. Receiver-operating characteristic analysis showed acceptable discrimination for individual indicators, with the highest accuracy for serum albumin (area under the curve [AUC] = 0.77). A combined nutritional–inflammatory model integrating dietary intake, albumin, and CRP demonstrated the greatest discriminatory ability (AUC = 0.83). Conclusion: Routinely available nutritional and inflammatory indicators are closely associated with sarcopenia in hemodialysis patients. Their combined assessment may facilitate early identification of individuals with sarcopenia and support timely nutritional evaluation and intervention.

Impact of Intraperitoneal Drain Placement on Postoperative Pain After Total Laparoscopic Hysterectomy

Küçükatalay, Özge; Kantarcı, Sercan; Dağlı, Uğurcan; Kanmaz, Ahkam Göksel; Töz, Emrah; Ileri, Alper; Inan, Abdurrahman Hamdi

Introduction: Total laparoscopic hysterectomy (TLH) is widely performed for benign gynecologic conditions; however, postoperative pain particularly shoulder pain related to residual intraperitoneal CO2 remains a clinical concern. This study aimed to evaluate whether drain placement reduces postoperative pain and analgesic requirements after benign TLH. Methods: In this prospective observational study, 129 women undergoing TLH were included; 86 received an intraperitoneal drain and 43 did not. Drain placement was based on the surgeon’s intraoperative judgment. To ensure a homogeneous low-complexity cohort, cases with complications or deviations from standard TLH were excluded. Postoperative shoulder, chest, and abdominal pain were assessed using a 10-point visual analog scale at 6, 12, and 24 hours. Analgesic use and early postoperative outcomes were also recorded. Results: Baseline demographic and perioperative characteristics were comparable between groups. Multivariable regression analysis showed that drain placement was independently associated with higher shoulder pain at 6 and 12 hours (p = 0.015 and p < 0.001) and higher chest pain at 6 hours (p = 0.039). Abdominal pain was higher at 6 hours but lower at 24 hours in the drain group (p = 0.039 and p = 0.032). Despite these time-dependent differences, overall postoperative analgesic consumption during the first 2 days was significantly higher in patients with drains (p < 0.05). Conclusion: Intraperitoneal drain placement after benign TLH was associated with increased early postoperative pain and did not reduce overall analgesic consumption. Routine drain use cannot be recommended, and selective placement should be reserved for specific clinical situations.

Altıparmak, Serkan; Özcan, İbrahim; Kaya, Altan; Aytekin, Aynur; Karaman, Hatice

Background: Radiotherapy-induced ototoxicity remains a significant clinical problem, and potential protective agents such as rosmarinic acid have gained increasing research interest. Objectives: This study investigated the effectiveness of systemic and local rosmarinic acid administration in radiotherapy-induced ototoxicity using the auditory brainstem response (ABR), distortion product otoacoustic emission (DPOAE) test, and histopathological methods. Material and methods: Sixty rats were included in the study. Seven groups were created, including control, radiotherapy-only, and combinations of radiotherapy with intratympanic or intraperitoneal rosmarinic acid, as well as a radiotherapy plus intratympanic saline group. ABR and DPOAE measurements were performed with all animals initially and at the end of the third week. The temporal bones of the decapitated rats were also fixed in formalin for histopathological measurements. Results: ABR thresholds increased significantly in the radiotherapy–only group (from 22.8 ± 3.1 to 45.4 ± 3.6 dB, p = 0.004) and the radiotherapy plus intratympanic saline group (from 24.6 ± 3.1 to 47.1 ± 3.6 dB, p = 0.005), while remaining stable in the other groups (p > 0.05). Correspondingly, DPOAE values decreased and histopathological damage scores were markedly higher in these two groups. Conclusions and significance: Rosmarinic acid protects against radiotherapy-induced ototoxicity when applied both systemically and locally, as confirmed by histopathological examination and objective audiological methods.

The Relationship Between Death Anxiety, Hope, and Psychological Well-Being in Individuals Diagnosed With Cancer

    Korkmaz, Elif Mirac; Lok, Neslihan

This study employed a descriptive and correlational research design to examine the relationship between death anxiety, hope, and psychological well-being in individuals diagnosed with cancer. Conducted in 2024 with 188 patients receiving treatment at an education and research hospital in Turkey, the study collected data using the Personal Information Form, the Death Anxiety Scale, the Dispositional Hope Scale, and the Psychological Well-Being Scale. Findings indicated that the mean total score of the Death Anxiety Scale was 7.32 ± 2.53. Death anxiety scores differed significantly by gender, marital status, educational level, cancer type, cancer stage, treatment type, hope, and psychological well-being (p < 0.05), but not by employment status, income level, or social security (p > 0.05). No significant association was found between age and death anxiety (p > 0.05). Moderate negative correlations were observed between death anxiety and hope, as well as between death anxiety and psychological well-being (p < 0.05). These results suggest that increases in death anxiety are associated with decreases in both hope and psychological well-being. Future research is recommended to further explore the influence of death anxiety on psychological health outcomes and to identify additional contributing factors.

Protective effects of adenosine triphosphate, thiamin, thiamin pyrophosphate, and their combination against linezolid-induced renal injury and lactic acidosis in rats

Admis, Ozlem; Yavuzer, Bulent; Mammadov, Renad; Suleyman, Bahadir; Altuner, Durdu; Mendil, Ali Sefa; Yucel, Nurinisa; Suleyman, Halis

Linezolid, an oxazolidinone antibiotic effective against Gram-positive pathogens, may cause nephrotoxicity and lactic acidosis during prolonged therapy. This experimental study investigated the protective effects of adenosine triphosphate (ATP), thiamin, thiamin pyrophosphate (TPP), and their combination (ATTP) on linezolid-induced renal injury and lactic acidosis in rats. Thirty-six male Wistar rats were randomly divided into six groups (n = 6): healthy control (HG), linezolid only (LZD), ATP+linezolid (ATLZD), thiamin + linezolid (TLZD), TPP+linezolid (TPLZD), and ATP+thiamin + TPP+linezolid (ATTPL). Linezolid (125 mg/kg, orally) was administered twice daily, while ATP (4 mg/kg), thiamin (20 mg/kg), and TPP (20 mg/kg) were given intraperitoneally once daily for 28 days. At the end of treatment, kidney tissues were examined for oxidative stress markers [malondialdehyde (MDA), total glutathione (tGSH), superoxide dismutase (SOD), catalase (CAT)] and histopathology, and blood samples were analyzed for blood urea nitrogen (BUN), creatinine, and lactate. Linezolid increased oxidative stress, suppressed antioxidants, and elevated BUN, creatinine, and lactate levels. ATP partially improved the oxidative balance in renal tissue but failed to prevent hyperlactatemia and impaired renal function. Thiamin did not produce significant changes. TPP markedly improved oxidative stress markers and reduced renal dysfunction. The triple combination provided the most pronounced protection, restoring antioxidant defenses, kidney function, and lactate levels to near-control values. Histopathological evaluation revealed marked tubular degeneration, interstitial hemorrhage, and mononuclear cell infiltration in the linezolid group, which were markedly improved by TPP and combination therapy. These findings indicate that TPP protects against linezolid-induced nephrotoxicity and lactic acidosis, with its efficacy further enhanced by ATP.
