A novel biomarker renalase and its relationship with its substrates in schizophrenia

Catak, Zekiye; Kocdemir, Esra; Ugur, Kader; Yardim, Meltem; Sahin, Ibrahim; Kaya, Hilal; Aydin, Suleyman

Schizophrenia, particularly the form related to excessive dopamine (DA), is a chronic psychotic disorder affecting millions of people worldwide. Renalase metabolizes its catecholamine (CA) substrates, including DA, suggesting that there might be an association between renalase levels and schizophrenia occurrence. Therefore, the current study aimed to evaluate the renalase and CA levels in the serum of patients with schizophrenia. The study was conducted with thirty-three schizophrenia patients and an age-and gender-matched group of thirty-one controls. Renalase and CA levels were measured by using an enzyme-linked immunosorbent assay (ELISA). Renalase levels were significantly lower in the schizophrenia patients than in the control group (p<0.05), whereas DA levels were significantly higher (p<0.05). The epinephrine (Epi) levels of both groups were similar (p=0.186), while the norepinephrine levels in patients with schizophrenia were significantly lower than those in the control group (p<0.05). The areas under the curves for the renalase-dopamine, renalase-norepinephrine and renalase-epinephrine ratios were 0.805, 95% confidence interval (CI): 0.699-0.912 (p<0.001); 0.726, 95% CI: 0.594-0.859 (p=0.032); and 0.656, 95% CI: 0.520-0.791 (p=0.02). The high DA levels in patients with schizophrenia might be due to low renalase levels. Renalase enzyme levels may play a substantial role in the pathophysiology of schizophrenia. Thus, this enzyme might be a new future target for the treatment and diagnosis of schizophrenia after intrabrain renalase and DA dynamics have been further evaluated.

Immunostaining characteristics of irisin in benign and malignant renal cancers

    Kuloğlu, T.; Artaş, G.; Yardim, M.; Sahin, I.; Aydin, Y.; Beyoğlu, N.; Özercan, İ. H.; Yalcin, M.H.; Ugur, K.; Aydin, S.

We investigated the expression of irisin in renal cancers using immunocytochemistry. Irisin has been reported to exhibit anticancer properties. The study groups consisted of 22 cases each of control renal tissue, oncocytoma, chromophobe renal cell carcinoma (RCC), clear cell RCC (Fuhrman nuclear grades 1, 2, 3 and 4) and papillary RCC. We evaluated 10 slides for each of 176 cases. Slides were immunostained for irisin and histoscores were calculated for the prevalence and strength of immunostaining. Fuhrman nuclear grade 1, 2, 3 clear cell RCC and papillary RCC exhibited no irisin immunoreactivity. Irisin immunoreactivity was observed in some Fuhrman nuclear grade 4 RCCs. We found a significant decrease in irisin staining in chromophobe RCC compared to the control. Immunoreactivity in the oncocytoma tissue was comparable to the control group. Irisin immunoreactivity in chromophobe RCC decreased and no immunoreactivity was observed in Fuhrman nuclear grade 1, 2, 3 clear cell RCC and papillary RCC. Immunistochemical screening of irisin in renal oncocytomas and renal cancers may be useful for differential diagnosis.

Can the negative effects of ketamine abuse on female genital organbe prevented by nimesulide? An experimental study

Turkler, Can; Onat, Taylan; Yildirim, Engin; Kaplan, Selcuk; Yazici, Gulce N.; Mammadov, Renad; Sunar, Mukadder

The objective of this study is to investigate the effects of nimesulide on ketamine-induced ovarian and uterine toxicity by biochemical and histopathological examinations. Ketamine is an anesthetic agent whose use leads to overproduction of catecholamines. Nimesulide is a cyclooxy-genase-2 inhibitor, which has also been reported to exert a significant antioxidant effect. Wistar albino female rats were randomly divided into three groups as follows: ketamine group (60 mg/ kg), ketamine (60 mg/kg)+nimesulide (50 mg/kg) group, and a healthy control group. Then, the biochemical levels and histopathological findings in the ovaries and uteri of the rats were examined for malondialdehyde, myeloperoxidase, total glutathione and superoxide dismutase. The study demonstrated that, in the uterine and ovarian tissues of rats that have been administered ketamine, there was a decrease in the levels of total glutathione and superoxide dismutase, while malondialdehyde and myeloperoxidase was increased: however it was observed that these ratios were reversed in the ketamine+nimesulide group. It was also proved that the negative effects of ketamine can be corrected with nimesulide when the myometrial and endometrial thicknesses are compared. Antioxidants such as nimesulide may protect against the damage caused by ketamine to the genital organs in young women.

Comparison of freeze-dried tibialis anterior allograft and four-strand hamstring autograft in anterior cruciate ligament reconstruction

Issın, Ahmet; Öner, Ali; Sofu, Hakan; Yurten, Hakan

Purpose: The aim of this retrospective study was to evaluate the clinical outcomes of the patients who underwent primary anterior cruciate ligament (ACL) reconstruction surgery with either hamstring autograft or freeze-dried tibialis anterior allograft, which performed by the same surgeon using the same fixation technique. Methods: In this retrospective study, patients who had primary ACL reconstruction using either four-strand hamstring autograft (FSH) or freeze-dried irradiated tibialis anterior allograft (FDT) between 2012 and 2015 were evaluated. Patients who were skeletally mature with a minimum follow-up of 24 months and who had no previous surgery from the affected knee were included; patients who had multiple ligament injuries or chondral lesions over Outerbridge grade 2 were excluded from the study. Patients were grouped according to the graft type used in ACL reconstruction. Tegner activity scale and Lysholm knee scoring scale were used to assess patients' activity levels and functional status preoperatively and at the final follow-up. KT-2000 arthrometer measurements were done at the final follow-up to evaluate anterior laxity. Results: There were 27 patients (mean age 27 ± 8.9 years) in the FSH group and 36 patients (mean age 27.1 ± 6.7 years) in the FDT group. The mean follow-up time was 38.2 ± 3.5 months for the FSH group and 41 ± 6.1 months for the FDT group. There were no statistically significant differences between the groups when preoperative and postoperative Tegner-Lysholm scores were compared (Tegner P = 0.583, 0.742; Lysholm P = 0.592, 0.249). The mean anteroposterior laxity and side-to-side differences measured by KT-2000 were 4.1 mm and 2.1 mm for the FSH group, respectively; 4.2 mm and 2.2 mm for the FDT group, respectively. There was not a statistically significant difference (P = 0.745, 0.562 respectively). Conclusions: Primary ACL reconstruction with a single loop freeze-dried irradiated tibialis anterior allograft revealed comparable results with four-strand hamstring autograft in non-athlete patients. Level of evidence: Level III, Therapeutic study.

Unstable Reverse Total Shoulder Arthroplasty: How to Avoid and Manage

Özer, Mustafa; Çetinkaya, Mehmet; Kanatlı, Ulunay

Although the implementation of reverse total shoulder arthroplasty (RTSA) is more common today, the complication rates in the literature still reach as high as 68%. The instability following the RTSA is the most frequent reason for revision surgery and the most difficult problem to fix. Moreover, this complication has a very high recurrence rate. The factors affecting the stability of the RTSA are compressive forces, humerosocket depth, and glenosphere dimensions. The risk factors for instability are the component malposition, insufficient soft tissue tension and soft tissue coverage, choice of deltopectoral approach, subscapularis deficiency, body mass index over 30 kg/m2, male gender, any previous history of surgery performed on the affected shoulder, impingement of the components, heterotopic ossification, axillary nerve palsy, and asymmetrical polyethylene wear. Following the development of the instability, deltoid muscle and axillary nerve integrity, glenoid and humeral component position, and bone pathologies and defects should be evaluated. If these are normal, close reduction can be considered as the initial intervention in these patients. Open reduction is the second option if the close reduction fails. In these patients, glenoid and humeral components, polyethylene insert wear, bone pathologies and defects, and soft tissue tension should be evaluated. Appropriate soft tissue tension can be restored by increasing the humeral length or lateralization of the joint center of rotation offset in patients with instability due to soft tissue laxity. Humeral shortening and excessive glenoid medialization are the main problems in recurrence. In patients with humeral shortening without excessive glenoid medialization, the stability is ensured by one or more of the following: implementing a higher sized insert, metal heightener (spacer), longer cemented humeral stem, and a structural humeral bone graft. If a longer humeral stem comes up short, the stability is reexamined following changing the glenosphere with a higher sized or eccentric one. The glenoid offset is enhanced in patients with excessive glenoid medialization by lateralization or increasing the size of glenosphere and in those with glenoid defect by using bone grafts. The instability risk can be kept to minimum by choosing the most appropriate technique considering the implant design, placement of the components in anatomical position (height and version), avoiding any potential impingement, and adjusting the soft tissue tension by minimal damage.

The effects of lycopene on alloxan induced diabetic optic neuropathy

Icel, Erel; Icel, Aykut; Uçak, Turgay; Karakurt, Yücel; Elpeze, Burcu; Keskin Çimen, Ferda; Süleyman, Halis

Aim: To determine the effects of lycopene treatment in prevention of diabetes associated inflammatory response and oxidative stress in an experimental model. With this aim we investigated the levels of oxidative stress markers including Malondialdehyde (MDA), and total oxidative status (TOS)together with inflammatory markers including nuclear factor- kappa B (NFKB) and tumor necrosis factor α (TNF-α) and antioxidants including total glutathione (TGSH), total oxidative status (TOS) and total anti-oxidative status (TAS) levels on eye tissue. Material and methods: Totally 18 albino Wistar male rats (250–280 grams) assigned into three groups, with six rats in each group as follows: healthy group (HG), control group (CG), and lycopene group (LG). The diabetes was induced with alloxan administration in rats of CG and LG. Lycopene (4 mg/kg) was administered to the rats in LG once a day for 3 months. At the end of this period, the animals were sacrificed and their eyes were enucleated for histopathological evaluations. From the tissues, MDA, GSH, TOS, TAS, TNF-α and NF-κB levels were analyzed. Results: MDA, TOS, OSI, NFKB and TNF-α levels were significantly higher, while TGSH and TAS levels were significantly lower in CG compared with HG (p < 0.001). On the other hand in LG; MDA, TOS, OSI, NFKB and TNF-α levels were significantly lower, while TGSH and TAS levels were significantly higher compared with CG (p < 0.001). Regarding histopathological findings, although there was severe damage on optic nerve of rats in CG; there was only a slight damage in lycopene administered group. Conclusion: For the first time in literature we determined that, lycopene was significantly effective in prevention of augmented inflammation and oxidative stress on eye tissue associated with diabetes, as well as the tissue damage on optic nerve. However, studies investigating the long-term clinical effects of lycopene on diabetic individuals are warranted.

Comparison of Tuberculin Skin Test (TST) and T-SPOT.TB Tests for Diagnosis of Latent Tuberculosis Infection (LTBI) in HIV-infected Patients; [HIV ile Enfekte Hastalarda Latent Tüberküloz Enfeksiyonunu (LTBE) Belirlemede Tüberkülin Deri Testi (TDT) ve T-SPOT.TB Testlerinin Karşılaştırılması]

Binay, Umut Devrim; Fincanci, Muzaffer; Fersan, Esra; Karakeçili, Faruk

Tuberculosis (TB) is the most common opportunistic infection in human immunodeficiency virus (HIV)-infected patients. Diagnosis and treatment of latent tuberculosis infection (LTBI) is the most important step in preventing the development of active TB. In our country where TB is moderately endemic, HIV-infected patients should be investigated for LTBI. Tuberculin skin test (TST) and interferon-gamma release assays (IGRA) are used in the diagnosis of LTBI but there isn't a standard practice. The aim of this study is to compare the TST and T-SPOT.TB test efficiency in the diagnosis of LTBI in HIV-infected patients. Patients who had no previous active TB infection, who were not treated for LTBI and who had no active tuberculosis infection at the time of admission were included in the study. A total of 100 HIV-infected patients who were admitted to the Infectious Diseases and Clinical Microbiology outpatient clinic between June 2015 and March 2016 were evaluated cross-sectionally. CD4+ T lymphocyte counts in the last one month were detected. All patients underwent chest radiography at the time of admission. Patients who are not considered as active TB infection with clinical and laboratory findings and who had no TST within the last one month were included in the study. TST was performed after the blood samples were taken for T-SPOT.TB test. In our study, 87% of the patients were male and the mean age was 40.2. The mean CD4+ T lymphocyte count was 605 cells/mm3 (26-1313). 16% of the patients had a history of encountring a person with tuberculosis and 81% had BCG vaccination scar. TST positivity and T-SPOT.TB positivity were 22.9% and 22%, respectively. The concordance between the two tests was found to be moderate (Kappa= 0.491). It was determined that BCG vaccination and the presence of a contact with a patient with TB did not affect TST and T-SPOT.TB test positivity (p> 0.05). There was a positive correlation between CD4+ T lymphocyte count and TST measurement values (r= 0.3, p= 0.003). Accordingly, as the number of CD4+ T lymphocytes increased, TST positivity increased (p= 0.007). T-SPOT.TB test was not affected by CD4+ T lymphocyte count (p= 0.289). Our study showed that TST was affected by CD4+ T lymphocyte count and patients' compliance with this test was also low. On the contrary T-SPOT.TB test was not affected by CD4+ T lymphocyte count. There was no statistically significant difference between T-SPOT.TB test positivity and CD4+ T lymphocyte count (p= 0.289). The concordance between the two tests was found to be moderate. It is thought that the main reason for the discordance between the tests is due to false negative or false positive results of TST. In conclusion, T-SPOT.TB was found more reliable in the diagnosis of LTBI in HIV-infected individuals. In the light of these findings, especially in HIV-infected patients with low CD4+ T lymphocyte counts, T-SPOT.TB test can be considered for LTBI diagnosis.

Dursunoğlu, Benan; Yuca, Hafize; Gözcü, Sefa; Yilmaz, Bilal; Güvenalp, Zühal

Obesity is a serious disease that can be due to genetic and environmental reasons and is defined by the World Health Organization as abnormal or excessive fat accumulation that may impair health. There is an increasing trend towards herbal slimming products in obesity treatment. The quality control analysis of these products is important for public health. In this study, new, simple, and sensitive high performance liquid chromatography methods were developed for qualitative and quantitative determination of arbutin, hydroquinone, ursolic acid, chlorogenic acid, epigallocatechin gallate, epigallocatechin, epicatechin gallate, and catechin in eleven herbal slimming products in Turkey containing Camellia sinensis, Ilex paraguariensis, and Calluna vulgaris. The intra- and interday precisions, stated as the relative standard deviation, were less than 2% and accuracy, based on relative error, was less than 10%. The limits of detection and quantification for arbutin and hydroquinone were 0.80 and 2.40 µg/mL and for the other compounds were 0.30 and 0.90 µg/mL, respectively. The methods developed can be used for routine quality control analysis of these effective compounds in different herbal slimming products.

The forelock sign: A new arthroscopic finding in partial subscapularis tears

Çetinkaya, Mehmet; Kanatli, Ulunay; Ataoğlu, Muhammet Baybars; Özer, Mustafa; Ayanoğlu, Tacettin; Bölükbaşi, Selçuk

Objectives: This study aims to identify the forelock sign of partial tears of the subscapularis and to compare the incidences of this sign and air bag sign. Patients and methods: One hundred and twenty patients (39 males, 81 females; mean age 51.3 years; range, 25 to 79) diagnosed as isolated subscapularis tear or isolated supraspinatus tear from among patients who underwent shoulder arthroscopy between January 2013 and January 2016 were divided into four groups of 30 patients each as the subscapularis tear (group 1), fullthickness supraspinatus tear (group 2), bursal-side supraspinatus tear (group 3), and articular-side supraspinatus tear (group 4) groups. All patients had video records of their operation. The integrity of the long head of biceps tendon (LHBT), Lafosse classification of the subscapularis tear, and the incidence of the forelock and air bag signs were evaluated. Results: The incidence of the air bag sign in group 1 was 10% and that of the forelock sign was 60%. The forelock sign was significantly more frequent in group 1 than in the other groups (odds ratio 10.46: 3.9-27.8 with 95% confidence interval) and the air bag sign (p < 0.001). The LHBT was mostly pathologic in this group, which was also statistically significant (p < 0.01). The incidence of the forelock sign in group 1 was similar in each LHBT pathology and Lafosse subgroups. Conclusion: The forelock sign identified in this study was significantly frequent (60%) in partial subscapularis tendon tears with strong inter- and intra-observer consistency while the air bag sign was rare (10%). The LHBT disorders were strongly associated with subscapularis tears; however, the forelock sign was not an indicator of the condition of the LHBT and type of partial subscapularis tendon tear according to the Lafosse classification.

Purpose: The purpose of this study was to comparatively evaluate the effectiveness of intra-articular PRP and HA injections applied as the treatment of Outerbridge grade 2 chondral lesions in patellofemoral joint during arthroscopic ACL reconstruction. Methods: The clinical and radiographic data of 61 patients between 18 and 45 years of age were evaluated. The patients were separated into three groups. Hyaluronic acid injection was applied in 22 knees (Group 1), PRP injection was applied in 18 knees (Group 2), and 21 knees did not have any specific treatment except ACL reconstruction (Group 3). All patients were followed clinically at least for 12 months. Clinical examination of the operated knee, visual analogue scale (VAS) score, Lysholm knee score, and Tegner activity scale were the outcome measures. Routine X-ray and MRI were also performed for all patients at 12-month postoperative follow-up visit. Results: Although the mean VAS and Lysholm scores at 3-month follow-up were better in Group 1 and 2 than Group 3, the efficacy of intra-articular PRP on healing process regarding progression of the mean VAS and Lysholm scores through 6- and 12-month follow-ups was significantly better and longer than HA. No statistically significant differences were detected according to Tegner activity scale between the groups at 3 and 6 months; however, Group 2 had better activity level than both Group 1 (p < 0.001) and 3 (p < 0.001) at the end of 12 months after surgery. Conclusion: Intra-articular PRP injection applied as the treatment of concomitant Outerbridge grade 2 chondral lesion in patellofemoral joint during ACL reconstruction revealed better and durable clinical outcomes via decreasing the potentially negative effects of chondral pathology on postoperative healing with respect to HA injection. Level of evidence: III—retrospective comparative study.

Keeled or Pegged Polyethylene Glenoid Components

Özer, Mustafa; Çetinkaya, Mehmet; Kanatlı, Ulunay

The glenoid baseplate survival is of paramount importance for the outcome of anatomical and reverse total shoulder arthroplasty procedures. Various prosthetic designs have been described and experienced to decrease the failure rates and improve patients’ satisfaction. Most of the orthopaedic surgeons have a great eagerness to implant and advocate the success of uncemented prosthetic components in arthroplasty procedures to avoid complications of cementing and troubles during the revision procedures. However, higher rates of failure were almost always reported for metal-backed glenoid components in the literature. Studies in the literature have consistently reported unsatisfactory results following implantation of uncemented metal-backed glenoid components. The most common complication of total shoulder arthroplasty (TSA) is the failure of the polyethylene glenoid component which accounts for the majority of the deleterious outcomes and manifests clinically by pain, loss of function, and presence of a clunking noise. High rates of radiolucency at the bone-cement interface were previously reported numbers of times in the literature. However, this fact does not always show up clinically as symptomatic loosening during the postoperative follow-ups. This chapter aimed to compare the mostly implanted materials named pegged and keeled glenoid components to provide a guide to the shoulder surgeons in decision-making for TSA procedures.

Antibiotics used in patients after surgery and effects of human serum paraoxonase-i (PON1) enzyme activity

    Yilmaz, Aycan; Dilek, Esra

Background: Paraoxonase (PON; arilesterase, [EC 3.1.8.1]) is an enzyme from the group arilesterases (ARE). This enzyme is capable of hydrolyzing paraoxone which is the active metabolite of parathion, an organic phosphorus insecticide. PON activity was found to be low in individuals prone to development of atherosclerosis such as diabetes, familial hypercholesterolemia and kidney disorders. It was noted that PON enzyme activity decreases in relation to age increase in adults. PON enzyme activity is approximately half of that in newborns and premature babies. Approximately one year after birth, it reaches the adult level. It can be said that PON1 has significant role on living organisms. For this reason, many studies on interactions of PON-drugs are needed. Objective: In this article, our aim is to investigate in vitro effects of four pharmaceutically active agents (fosfomycin, cefuroxime axetil, cefaclor monohydrate, and cefixime) which are often used in patients after surgery on human serum paraoxanase-I (PON1) enzyme activity. Methods: In this article, we purify paraoxonase-I enzyme from human serum by using ammonium sulfate precipitation (in the range of 60-80%), ion exchange and gel filtration chromatography. We use electrophoresis to check the purity of the enzyme. We investigate the paraoxonase activity of the enzyme at 412 nm the inhibition effects of the active substances. Paraoxone is used as the substrate. Activity measurements arw made at different inhibitor concentrations related to inhibitor studies and % Activity- [I] graphs are drawn for drug active substances. Lineweaver-Burk graphics are used to determine the Ki constants. Finally, to determine the types of inhibition we interpret these graphs. Results: The active agents used after surgery decreased the PON1 enzyme activity. They showed different inhibition mechanism. The inhibition mechanism of fosfomycin and cefaclor monohydrate was noncompetitive, cefixime was uncompetitive and cefuroxime axetil was a competitive inhibitor. The IC50 values for fosfomycin, cefuroxime axetil, cefaclor monohydrate, and cefixime were calculated to be 31.5 mM, 1.03 mM, 4.18 mM and 0.781 mM, respectively, and the Ki constants were determined to be 27.98 ± 12.25 mM, 2.20 ± 0.22 mM, 4.81 ± 2.25 mM and 1.12 ± 0.32 mM, respectively. The IC50 and Ki values showed that cefixime active agent has the maximum inhibition. Conclusion: In this study, we have detected that cefuroxime axetil inhibited competitively in vitro paraoxonase activity of this enzyme. According to this information, we thought that cefuroxime axetil linked to the active site of the enzyme. Fosfomycin and cefaclor monohydrate can be attached with amino acids out of the active site of the enzyme because they inhibit enzyme noncompetitively. Cefixime can be attached only to the enzyme-substrate complex because it inhibits enzyme uncompetitively.

Oxidative stress parameters, selenium levels, dna damage, and phthalate levels in plastic workers

Gurdemir, Gulru; Erkekoglu, Pinar; Balci, Aylin; Sur, Unzile; Ozkemahlı, Gizem; Tutkun, Engin; Yılmaz, Hinc; Asci, Ali; Kocer-Gumusel, Belma

Di(2-ethylhexyl)phthalate (DEHP) is the most widely used phthalate. DEHP is highly used in PVC floor-ings and PVC windows and carpeting. The objective of this study was to determine sex hormone levels, oxidative stress parameters, selenium levels, DNA damage, and phthalate levels in plastics workers (n = 24, age = 20–58 years) working in the production of rubber mechanical goods and exposed to DEHP in workplace. The control group (n = 29, age = 25–54, all male) was selected from age-matched healthy adults. Antioxidant parameters and DNA damage were determined by spectrophotometry. Selenium levels were determined by atomic absorption spectroscopy. Plasma hormone levels were measured by chemiluminescence microparticle immunoassay. Plasma phthalate levels were determined by high-pressure liquid chromatography. Plastic workers had lower serum testosterone and free T4 levels and higher follicle-stimulating hormone levels vs. controls. Liver enzyme activities were markedly higher in workers vs. controls. There were also increases in plasma glutathione peroxidase levels and marked decreases in plasma selenium and erythrocyte total glutathione levels in plastics workers (P < 0.05 vs. control). Plasma 8-hydroxy-2′-deoxyguanosine levels were 14-fold higher in plastics workers than in controls. Plasma DEHP and mono(2-ethylhexyl)phthalate were also markedly higher in workers vs. controls. The results of this study show that occupational exposure to DEHP may lead to disturbances in sex hormones, increased liver problems, higher oxidative stress and DNA damage levels, and lower trace element concentrations in workers. More comprehensive and mechanistic studies with higher numbers of subjects are needed to show the unwanted effects of occupational exposure to DEHP.

Background/aim: The aim of this study was to investigate postoperative audiometric threshold shifts in patients who underwent primary total knee arthroplasty (TKA) using gentamicin-loaded bone cement (GLBC) in comparison with the ones who underwent TKA without GLBC. Materials and methods: Forty patients (gentamicin group) who underwent primary TKA using GLBC and 29 patients (control group) who underwent primary TKA using standard bone cement were included in this prospective case-control study. Baseline pure-tone audiometric evaluation was performed preoperatively and repeated at the postoperative third day for all patients. Control audiometric evaluation was performed weekly for patients who were diagnosed with ototoxicity according to audiometric threshold shifts. Results: Ototoxicity was diagnosed in 8 of 40 patients (20%) in the gentamicin group according to postoperative audiometric threshold shifts, whereas no ototoxicity was observed in the control group. Patients who were diagnosed with ototoxicity had no permanent audiometric threshold shifts in follow-up audiometric evaluation and these patients had no clinical complaints of difference in hearing. Conclusion: According to our results, audiometric threshold shifts can be detected in patients who undergo primary TKA usingentamicin loaded bone cement. However, no permanent shifts were observed during close follow-up.

The Effect of Taxifolin on Cisplatin-Induced Pulmonary Damage in Rats: A Biochemical and Histopathological Evaluation

Unver, Edhem; Tosun, Mustafa; Olmez, Hasan; Kuzucu, Mehmet; Cimen, Ferda Keskin; Suleyman, Zeynep; Xiao, Cheng

The effect of taxifolin on cisplatin-induced oxidative pulmonary damage was investigated biochemically and histopathologically in male albino Wistar rats. There were four groups, with six animals in each group: 50 mg/kg of taxifolin plus 2.5 mg/kg of cisplatin (TC) group, 2.5 mg/kg of cisplatin only (CIS) group, 50 mg/kg of taxifolin only (TG) group, and a healthy control group (HG). In terms of the experimental procedure, the animals in the TC and TG groups were first treated via oral gavage. The CIS and HG groups received distilled water as solvent, respectively. One hour later, the TC and CIS groups received cisplatin at a dose of 2.5 mg/kg (injected intraperitoneally). Taxifolin, cisplatin, and the distilled water were administered at the indicated dose and volume, using the same method daily for 14 d. At the end of this period, the animals were killed with a high dosage of thiopental anaesthesia (50 mg/kg). Blood and lung tissue samples were taken for biochemical (malondialdehyde (MDA), myeloperoxidase (MPO), total glutathione (tGSH), and 8-hydroxy-2 deoxyguanosine (8-OHdG)) analyses and histopathological examinations. The biochemical and histopathological results in the TC and HG groups were then compared with those in the CIS group. Cisplatin increased the levels of MDA, myeloperoxidase, and 8-OHdG, a marker of oxidative DNA damage, and reduced the amount of tGSH in the lung tissue. Moreover, severe alveolar damage, including oedema and extensive alveolar septal fibrosis, in addition to infiltration of polymorphic nuclear leucocytes and haemorrhagic foci, was observed in the CIS group. These histopathological findings demonstrate that taxifolin provides protection against pulmonary oxidative stress by preventing increases in oxidant parameters and decreases in antioxidants.

Introduction: This study aimed to compare the indications and outcomes of penetrating keratoplasty in geriatric patients and to compare the younger geriatric patients with older geriatric patients. Materials and Method: We evaluated the data of 489 eyes of 427 patients who underwent corneal transplantation surgery between August 2010 and July 2015 at a tertiary referral center in Turkey. We included 90 eyes of 81 patients aged ≥ 65 years with at least 1-year follow-up data were. The patients were divided into Groups 1 and 2 comprising patients aged 65–79 and ≥80 years, respectively. Results: Groups 1 and 2 included 66 and 24 eyes, respectively. Bullous keratopathy was the most common indication for penetrating keratoplasty surgery in both groups (Group 1=34.9%, Group 2=66.7%, p=0.036). The incidence of secondary glaucoma after surgery (Group 1=18.2%, Group 2=15.6%, p=0.254) and graft survival period (Group 1=29.9±15.8 months, Group 2=26.7±13.2 months, p=0.374) were not significantly different between the two groups. The graft rejection and failure rates between the groups were analyzed, and no difference was observed between them (p=0.531, chi-squared=0.392 for graft rejection; p=0.699, chi-squared=0.150 for graft failure). Conclusion: Bullous keratopathy as an indication for penetrating keratoplasty is significantly more common in older geriatric patients. The outcomes of penetrating keratoplasty in older geriatric patients are as good as those in younger geriatric patients.

Ototoxicity of cisplatin is one of the major dose-limiting side effects. Cisplatin-dependent ototoxic-ity is known to be associated with reactive oxygen species. Protective effect of taxifolin, being a flavanone found in onions, milk thistle, French maritime, and Douglas fir bark, against cisplatin-associated ototoxicity will be examined in this study. There are no studies in the literature examining the protective effect of taxifolin against cisplatin-induced ototoxicity. 50 mg/kg taxifolin was orally administered to TXC group rats (n-6), and distilled water was orally administered as a solvent to CG (n-6) and HG (n-6) groups. One hour later, 5 mg/kg cisplatin was administered intraperitoneally (i.p) to TXC and CG groups. This procedure was repeated once a day for 7 days. At the end of this period, all animals were sacrificed by high-dose anesthesia (50 mg/kg thiopen-tal sodium) and biochemical and histopathological examinations were performed on the dissected cochlear tissue. Our biochemical test results showed that oxidant parameters were significantly increased whereas antiox-idant parameters were significantly decreased in the cisplatin group (CG) compared to taxifolin (TXC) and healthy (HG) groups (p < 0.0001 for both parameters). Histopathological results showed that severe cochlear vestibular membrane degeneration, dilated conjunctival blood vessels, edema and destruction developed in the cisplatin group. However, no pathological findings were found in the taxifolin-treated group except for mild degeneration and edema. This information suggests that taxifolin may be useful in the treatment of cisplatin-associated oxidative cochlear damage.
