Relationship Between Erectile Dysfunction, the Neutrophil-to-Lymphocyte Ratio, and the Platelet-to-Lymphocyte Ratio

    Karabakan, Mehmet; Bozkurt, Aliseydi

Objective: Various chronic diseases, including hypertension (HT), diabetes mellitus (DM), and coronary artery disease (CAD), together with the medications used in the treatment of these conditions, are considered a part of the erectile dysfunction (ED) etiology. The neutrophil-to-lymphocyte ratio (NLR) and the platelet-to-lymphocyte ratio (PLR) provide information about many diseases. The NLR and PLR increase in systemic inflammation, certain gynecological and gastrointestinal cancers, CAD, various oncological diseases, and especially in acute coronary syndromes. In this study, the relationship between PLR, NLR, and ED severity were examined in patients with ED. Methods: All data from patients were screened retrospectively. Body mass index, the International Index of Erectile Function-5 (IIEF-5) scores, age, fasting blood glucose, whole blood count, lipid, and hormone profile values were measured. The PLR and NLR values from both the patient and control groups were evaluated. The IIEF-5 questionnaire was used to measure the quality of erection in all the groups. A total of 131 patients with ED were divided into four groups: severe (34 patients), moderate (32 patients), mild-to-moderate (36 patients), and mild ED (29 patients). In addition, a control group was formed with 26 healthy men. Results: No statistically significant difference was observed between the groups in terms of the mean age, hypertension, smoking status, alcohol use, DM, CAD, and cholesterol and triglycerides levels. The mean PLR values were 125.3 +/- 41.4, 120.6 +/- 36.1, 118.2 +/- 50.4, 104.9 +/- 3.2, and 107.5 +/- 37.4 in the severe, moderate, mild-to-moderate, mild ED, and control groups, respectively. There was no significant difference in the PLR ratio among the ED groups and controls (p>0.05). The mean NLR values were 2.40 +/- 1.22, 2.34 +/- 0.88, 2.26 +/- 1.22, 2.1 +/- 0.87, and 1.76 +/- 0.7 in the severe, moderate, mild-to-moderate, mild ED, and control groups, respectively. Compared with the control group, this value was statistically significant for patients with all ED groups (p<0.05). Conclusion: The NLR value was found to be higher in patients with ED. The NLR value may be related to ED, and it can give valuable information in patients with ED.

Salcan, Ismail; Salcan, Sara; Erdem, Haktan Bagis; Müderris, Suphi

Objective: Allergic rhinitis (AR) is one of the most frequently encountered allergic disorders and has a typical prevalence of between 18 and 40%. The factors which underlie allergic disorders may vary since they are linked to climate, geography, different social conditions and genetic makeup. The objective of our research was to investigate any connections between polymorphism in the serotonin transporter gene and allergic rhinitis. Methods: This study involved the participation of two groups aged between 15 and 60 years of age who attended the ENT clinic of the Medical Faculty at Erzincan University: 100 individuals with allergic rhinitis plus a control group of 200 individuals who lacked the condition. The AR cases were established on the basis of history, physical examination and skin prick testing. Peripheral blood samples were taken from both groups and subjected to genetic analyses. The analysis utilised a 5-HTT Taqman (R) primer as genetic probe. The Chi-squared method was utilised for statistical analysis. Results: 129 of the participants were female (64.5%) and 71 (35.5%) male. The average age of those participating was 34.5 +/- 14.6 (range: 20-65) years. In reference to the sex distribution of the patients, 79% of the patients were female. There was a significant difference between the groups according to sex (p <0.01). No significant differences were noted between the patient and control groups when compared according to genetic polymorphism or place of residence. In 61% of the allergic rhinitis sufferers, their symptoms were seasonal. The incidence of AR was higher in both females and in those living in an urban rather than rural setting. When the polymorphism is examined in both the patient and control groups, the most common type is the mutant type. In both the cases and controls, when the polymorphism was investigated, the most frequently seen type was the Mutant variety and no significant difference in frequency was noted according to sex or place of residence. Conclusion: In this study, when the cases and controls were compared in terms of the polymorphism in the rs35521 serotonin transporter gene, there was no difference at the level of statistical significance between the frequency of occurrence of the mutant or heterozygote form of the gene.

A possible interaction o TIMP-1 and TSP-1 with familial mediterranean fever

Yildirim, Malik Ejder; Kurtulgan, Hande Kucuk; Kiliçgün, Hasan; Bakir, Deniz; Ersan, Sepil

Aim: Matrix metalloproteinases (MMPs) may influence many biological and pathological processes including inflammatory responses. Thrombospondins (TSP) are a glycoprotein group of the extracellular matrix. In this study, we aimed to investigate a possible association of TIMP-1 (an inhibitor of metalloproteinases) and TSP-1 (an adhesive protein involved in cell-matrix interactions) with familial Mediterranean fever. Material and Method: Thirty FMF patients who had compound heterozygous or homozygous MEFV mutations and thirty healthy controls were included in this study. The patients were selected after screening by reverse hybridization procedure. TIMP-1 and TSP-1 levels of the patients and controls were measured by ELIZA. Results: TIMP-1 and TSP-1 levels of the patients who had homozygous or compound heterozygous MEFV mutation were compared with 30 healthy controls. The levels of TIMP-1 in the patients were statistically higher than those in the control group (p < 0.001). There was no significant difference between the patients and controls in terms of TSP-1 levels (p = 0.84). Discussion: IL-1 beta has an important role in FMF disease and it may stimulate expression of TIMP-1. TIMP-1 levels are increased in FMF patients on the basis of inflammation and higher TIMP-1 levels in patients may be associated with the self-limited nature of FMF. TSP-1 level can be modulated by proinflammatory cytokines but there was no any significant difference between TSP-1 levels of FMF patients and the control group in our study. It can be thought that there is no interaction between TSP-1 and the pathogenesis of FMF.

Karavas, Erdal; Karakecili, Faruk; Balci, Mecdi Gurhan

Objectives: To investigate the effect of the biopsy technique used in percutaneous liver biopsies applied with semi-automatic 16 gauge (G) and 18G Tru-cut and Menghini aspiration needles in patients with chronic hepatitis B (CHB) on pathological evaluation. Materials and Methods: The study included 104 cases diagnosed with CHB who underwent liver biopsy between 2013 and 2018. The pathology results of biopsies were evaluated with the Menghini technique under ultrasound (USG) guidance (n=26), and with 16G (n=54) and 18G (n=24) semi-automatic Tru-cut needles under USG. Results: The fibrosis score in 5 (9.3%) of the 16G cases and in 3 (12.5%) of the 18G cases, and the ISHAK score in 3 (5.6%) of the 16G and in 1 (4.2%) of the 18G cases could not be determined. There was no significant difference between the methods and needle types in terms of the number of pieces, number of portal sites, fibrosis and ISHAK score (p>0.05). There was a statistically significant difference between the biopsies performed with Menghini method and Tru-cut method with 16G and 18G (p<0.0001) in terms of material length. The diagnosis rates for the Menghini technique, and 16G and 18G Tru-cut needles were 100%, 90.7% and 83.3%, respectively, with no statistically significant difference determined (p>0.05). Conclusion: Although a larger tissue piece is obtained with the Menghini technique, allowing evaluation of a larger portal area, no significant difference was determined between the techniques in the pathological evaluation. Taking patient safety and comfort into consideration, Tru-cut needle biopsy under USG guidance is recommended rather than the Menghini technique as less trauma is created.

    Inan, Hakki Caner; Kiraç, Merve

Objective: Benign paroxysmal positional vertigo (BPPV) is the most common peripheral vestibular system disease causing dizziness. It occurs more in the 5th decade of life and affects the posterior canal in 90% of the patients. The most effective treatment method is canalith repositioning (CRP) maneuver. The aim of this study is to evaluate the effects of betahistine and dimenhydrinate therapies in addition to CRP maneuver on BPPV patients. Methods: The study included 64 patients who had complaints of dizziness and were diagnosed with BPPV by their history and provocation maneuvers. The patients were divided into two groups. In Group 1, only repositioning maneuver was performed. Group 2 was divided into two subgroups. In Group 2a, repositioning maneuver was performed and betahistine 24 mg twice daily was given for 10 days. In Group 2b, repositioning maneuver was performed and dimenhydrinate 50 mg once daily was given for five days. On the 10th day, all patients were reexamined, and provocation maneuver was performed. Dizziness handicap inventory (DHI) was completed and outcomes were reviewed for therapeutic efficacy. Results: Mean DHI scores in all patient groups statistically significantly decreased from a pre-treatment level of 52.16 (range, 20-100) to a post-treatment level of 17.84 (range, 0-78) (p<0.001). No statistically significant differences were found in terms of DHI scores between Group 1 (repositioning maneuver only) and Group 2 (repositioning maneuver plus betahistine or dimenhydrinate). Conclusion: The most effective treatment method of BPPV is repositioning maneuver. Addition of betahistine or dimenhydrinate pharmacotherapy to repositioning maneuver did not show superiority to treatment with repositioning maneuvers alone.

Introduction: No definitive data describing associations between cases of Open Abdomen (OA) and Entero-atmospheric fistulae (EAF) exist. The World Society of Emergency Surgery (WSES) and the Panamerican Trauma Society (PTS) thus analyzed the International Register of Open Abdomen (IROA) to assess this question. Material and methods: A prospective analysis of adult patients enrolled in the IROA. Results: Among 649 adult patients with OA 58 (8.9%) developed EAF. Indications for OA were peritonitis (51.2%) and traumatic-injury (16.8%). The most frequently utilized temporary abdominal closure techniques were Commercial-NPWT (46.8%) and Bogotà-bag (21.9%). Mean OA days were 7.9 ± 18.22. Overall mortality rate was 29.7%, with EAF having no impact on mortality. Multivariate analysis associated cancer (p = 0.018), days of OA (p = 0.003) and time to provision-of-nutrition (p = 0.016) with EAF occurrence. Conclusion: Entero-atmospheric fistulas are influenced by the duration of open abdomen treatment and by the nutritional status of the patient. Peritonitis, intestinal anastomosis, negative pressure and oral or enteral nutrition were not risk factors for EAF during OA treatment.

Ethanolic (EEC) and aqueous (WEC) extracts of cinnamon (Cinnamomum verum) were evaluated for their antioxidant profiles by eight distinguished bioanalytical antioxidant methods. Their inhibitory effects were tested against some enzymes including acetylcholinesterase, butyrylcholinesterase, α-glycosidase and α-amylase, which linked to different diseases. Additionally, the antioxidant properties were determined and polyphenolic compositions of the both extracts were evaluated by LC-MS/MS analysis. According to the LC-MS/MS experiments, thirteen compounds were found in WEC and EEC. Also, p-hydroxybenzoic acid (321.1 mg/kg extract), p-coumaric acid (291.4 mg/kg extract), and pyrogallol (142.4 mg/kg extract) were found to be the most abundant ingredients in the WEC. On the other hand, pyrogallol (264.3 mg/kg extract), ferulic acid (224.7 mg/kg extract) and p-coumaric acid (170.2 mg/kg extract) were found as the most plentiful chemicals in the EEC. For the estimation of the antioxidant capacities of the both extracts (WEC and EEC), DPPH· and ABTS•+ scavenging activities, as well as Fe3+-Fe2+, and Cu2+-Cu+ reducing assays were studied. The IC50 values of the WEC and EEC indicated that they were potent effective DPPH· (21. 25 and 15.71 μg/mL) and ABTS•+ (6.52 and 5.79 μg/mL) scavengers, as well as AChE (221.33 and 110.26 μg/mL), BChE (461.69 and 94.93 μg/mL), α-glycosidase (206.86 and 220.00 μg/mL) and α-amylase (189.86 and 200.86 μg/mL) inhibitors. As a conclusion, both EEC and WEC had rich phenolic contents and demonstrated effective anticholinergic, antidiabetic and antioxidant effects.

Hackney, Anthony C.; Kallman, Ashley L.; Aǧgön, Eser

Study aim: Evidence supports female sex hormones have an influencing effect on amultitude of physiological and psychological systems related to exercise. Little is known, however, whether is effect persist into the recovery from exercise. Our objective was to examine aspects of muscle damage/inflammation process during recovery in healthy, exercise-trained women following endurance activity at the mid-follicular (MF; low sex hormone level) and mid-luteal (ML; elevated sex hormone levels) phases of their menstrual cycle. Material and methods: The MF and ML exercise sessions consisted of running for 90 minutes at 70% VO2max on atreadmill in a controlled laboratory environment. Menstrual cycle phase was hormonally confirmed, diet and physical activity was control throughout the study. Outcome measures were: Blood creatine kinase (CK) and interleukin-6 (IL-6) assessed at immediate-post exercise (IP), 24-hour and 72-hour into recovery. Statistics involved ANOVA procedures. Results: At 24-hours and 72-hour into recovery CK activity was greater in MF than ML (p < 0.05) while for IL-6 at IP, 24-hour and 72-hour responses were significantly greater at MF than at ML (p < 0.05). Conclusions: Amore robust recovery CK and IL-6 response occur in the MF of the menstrual cycle when female sex hormones are reduced. This finding suggests female sex hormone changes due to menstrual cycle phase affect the physiologic responses during the extended recovery period from intensive exercise in eumenorrheic women.

Background/aim: This study aimed to investigate the relationship between the iodine concentration (IC) of perigastric fat tissue as assessed by dual-energy computed tomography (DECT) and serosal invasion of gastric cancer. Materials and methods: A total of 41 patients underwent preoperative staging evaluation for gastric cancer using DECT between July 2015 and March 2018. Patients were divided into 2 groups based on pathology results: Serosal invasion (stage T4a) and intact serosa (stages T1–T3). Cutoff values, the diagnostic efficacy of IC in the perigastric fat tissue, and the perigastric fat tissue/tumor (P/T) ratio were determined. Results: Among the 41 patients, 22 had stage T4a gastric cancer and 19 patients had gastric cancer with a stage lower than T4a. The mean IC of perigastric fat tissue and the P/T ratio were significantly higher in patients with serosal invasion than in those with intact serosa (P < 0.001). During the arterial phase, the area under the curve (AUC) was 0.915 and 0.854 for the IC of perigastric fat tissue and the P/T ratio, respectively. During the venous phase, the AUC was 0.890 and 0.876 for the IC of perigastric fat tissue and the P/T ratio, respectively. Conclusion: The IC in the perigastric fat tissue seems to be a reliable indicator for serosal invasion of gastric cancer.

The preventive effects of rutin on immobilization stress-induced cardiac damage

Coskun, Resit; Celik, Aziz Inan; Suleyman, Zeynep; Cimen, Ferda Keskin; Cankaya, Murat

The previous studies formed the role of oxidants and proinflammatory cytokines in the pathogenesis of acute psychological stress-related cardiac damage leading to mortality and other complications. The aim of the study is to examine the protective effect of rutin against stress-induced cardiac damage. In the literature, no studies have been found analyzing the effects of rutin in acute stress-related oxidative damage induced by immobilization method in rats. Rutin was administered orally to rutin + stress applied (RSG) group albino rats at a dose of 50 mg/kg. For healthy control (HG) and stress applied to control (SAG) groups, distilled water as a solvent was orally administered at the same volume (0.5 mL). One hour after rutin and distilled water applications, all animals except for HG brought into a supine position and their legs and arms were ligated and kept in the same position for 24 h. The oxidant, cytokine and cardiac biomarker levels in blood serum and heart tissues of SAG animals were found to be significantly higher and total glutathione was lower than RSG and HG groups. Histopathologically dilated congested blood vessels and myocardial destruction, hemorrhage, edema, and polymorphonuclear leukocyte infiltration were observed in the SAG heart tissues. On the other hand, the histological heart tissue results of the RSG group was found to be similar compared to the healthy tissue except for the slightly dilated congested blood vessel. These results indicate that the rutin may be useful in the treatment of stress-related oxidative cardiac damage.

Benidipine can prevent liver ischemia reperfusion injury in rats: a biochemical and histopathological evaluation

Cimen, Orhan; Eken, Hüseyin; Keskin Cimen, Ferda; Ozbek Bilgin, Asli; Pehlivanoglu, Kamil; Kurnaz, Eray; Gülaboglu, Mine; Suleyman, Bahadir; Suleyman, Halis

Ischemia reperfusion (IR) injury is defined as a pathological process which initiates with tissue deoxygenation, continues with oxidative stress, and spreads with inflammation. Increased intracellular calcium is responsible for oxidative stress and inflammation during this process. Benidipine is an antihypertensive drug acting as an T/L-type calcium channel blocker. The effect of benidipine on cyclooxygenase-2 (COX-2) activity is not known. Benidipine being a calcium channel blocker suggests that it may suppress oxidant and proinflammatory COX-2 production. The aim of this study was to examine the effect of benidipine on IR induced liver injury in albino Wistar male rats. Animals were divided into three groups: liver ischemia reperfusion (LIR), 2 mg/kg benidipine + liver ischemia reperfusion (BLIR), and sham surgery (SHAM). Hepatic artery, portal vein and bile duct were clamped to obtain ischemia for one hour and reperfusion for six hours. Malondialdehyde (MDA), alanine aminotransferase (ALT), aspartate aminotransferase (AST) and COX-2 levels were higher, whereas total glutathione (tGSH) and cyclooxygenase-1 (COX-1) levels were lower in the LIR group. Liver tissue of the LIR group showed marked dilated congested blood vessels and increased bile duct proliferation, polymorphonuclear leukocyte infiltration, edema, hemorrhage and destruction. MDA, ALT, AST and COX-2 levels were lower, and tGSH and COX-1 levels were higher in the BLIR group treated with benidipine. Liver tissue of the BLIR group was found to be normal except for mild dilated blood vessels. These experimental findings indicate that benidipine may be useful in the treatment of liver IR injury.

Prevalence and Load of Demodex folliculorum and Demodex brevis (Acari: Demodicidae) in Patients with Chronic Blepharitis in the Province of Erzincan, Turkey

    Zeytun, Erhan; Karakurt, Yücel

Chronic blepharitis is an ocular disease frequently encountered by ophthalmologists. Demodex mites can play a role in the pathogenesis of blepharitis along with bacterial agents, especially in treatment-resistant cases or recurrent cases after treatment. This study was performed to determine the prevalence and load of Demodex folliculorum (Simon) and Demodex brevis Akbulutova in chronic blepharitis patients and to assess the relationship between the prevalence and load of Demodex species and ocular symptoms. The study included 365 patients diagnosed with chronic blepharitis in clinical examination, and 175 controls without any chronic or ocular disease. In the study, two eyelashes were sampled from the lower and upper lids of the right and left eyes (a total of eight samples) of the participants. Eyelash samples were examined under a light microscope, and Demodex species were identified and counted. Demodex were detected in 79.2% (95% CI: 75-83%) of patients and 31.4% (95% CI: 24-38%) of controls in this study. D. folliculorum alone (mean: 4.96; min: 1; max: 17; P < 0.001) was detected in 72.3% of patients, in 0.7% D. brevis alone (mean: 1.00, P > 0.05), and in 27% both D. folliculorum and D. brevis (mean: 21.65; min: 2; max: 79; P < 0.001). In Demodex positive controls, only D. folliculorum (mean: 2.38; min: 1; max: 6) was detected while there was no D. brevis. Nevertheless, mean ocular symptom scores were significantly higher in Demodex positive patients than in Demodex negative patients (P < 0.001). Itching, foreign body sensation, and redness were the most common complaints in Demodex positive patients. As a result, Demodex mites were high in numbers in patients with chronic blepharitis in Erzincan. There was a positive correlation between Demodex mites and chronic blepharitis and ocular symptoms. It may be helpful to consider these findings in clinical assessment of blepharitis patients.

Do intra-articular pathologies accompanying symptomatic acromioclavicular joint degeneration vary across age groups?

Özer, Mustafa; Ataoğlu, Muhammet Baybars; Çetinkaya, Mehmet; Ayanoğlu, Tacettin; Kaptan, Ahmet Yiğit; Kanatli, Ulunay

Objectives: This study aims to evaluate the accompanying intraarticular pathologies in patients who underwent arthroscopic distal clavicle resection (DCR) for symptomatic acromioclavicular (AC) joint degeneration based on age groups and to reveal which additional pathologies should be considered across different age groups during physical examination of patients suspected of AC joint degeneration. Patients and methods: The study included 156 patients (55 males, 101 females; mean age 57.2±10.0 years; range, 35 to 80 years) who underwent arthroscopic DCR between January 2006 and December 2017 and had at least one clinical positive test for AC joint degeneration during the preoperative physical examination. The patients were divided into three groups as those aged < 50 years (group 1), between 50-65 years (group 2), and > 65 years (group 3). The concomitant intra-articular pathologies were evaluated across different age groups and compared between the groups. Results: Concomitant intra-articular pathologies were detected in 117 (75%) of a total of 156 patients. Additional pathology rate increased with increasing age (p=0.002). More than one concomitant intraarticular pathologies were detected in 37 patients (23.7%). This rate increased with increasing age (p=0.002). The number of patients with superior labrum anterior posterior (SLAP) lesion as the only additional pathology was 33 (21.2%). This rate decreased with increasing age (p=0.015). In group 1, the rate of concomitant SLAP lesion was 44.1%. Conclusion: The high incidence of intra-articular pathologies accompanying symptomatic AC joint degeneration raises the importance of careful physical examination, detailed imaging, and arthroscopic surgery to obtain good results in patients scheduled for DCR. The frequency of AC joint degeneration and concomitant SLAP lesions, particularly in younger patients, should be considered during clinical examinations.
