Burden of cardiovascular disease across 29 countries and GPs’ decision to treat hypertension in oldest-old

Streit, Sven; Gussekloo, Jacobijn; Burman, Robert A.; Collins, Claire; Kitanovska, Biljana Gerasimovska; Gintere, Sandra; Gómez Bravo, Raquel; Hoffmann, Kathryn; Iftode, Claudia; Johansen, Kasper L.

Objectives: We previously found large variations in general practitioner (GP) hypertension treatment probability in oldest-old (>80 years) between countries. We wanted to explore whether differences in country-specific cardiovascular disease (CVD) burden and life expectancy could explain the differences. Design: This is a survey study using case-vignettes of oldest-old patients with different comorbidities and blood pressure levels. An ecological multilevel model analysis was performed. Setting: GP respondents from European General Practice Research Network (EGPRN) countries, Brazil and New Zeeland. Subjects: This study included 2543 GPs from 29 countries. Main outcome measures: GP treatment probability to start or not start antihypertensive treatment based on responses to case-vignettes; either low (<50% started treatment) or high (≥50% started treatment). CVD burden is defined as ratio of disability-adjusted life years (DALYs) lost due to ischemic heart disease and/or stroke and total DALYs lost per country; life expectancy at age 60 and prevalence of oldest-old per country. Results: Of 1947 GPs (76%) responding to all vignettes, 787 (40%) scored high treatment probability and 1160 (60%) scored low. GPs in high CVD burden countries had higher odds of treatment probability (OR 3.70; 95% confidence interval (CI) 3.00–4.57); in countries with low life expectancy at 60, CVD was associated with high treatment probability (OR 2.18, 95% CI 1.12–4.25); but not in countries with high life expectancy (OR 1.06, 95% CI 0.56–1.98). Conclusions: GPs’ choice to treat/not treat hypertension in oldest-old was explained by differences in country-specific health characteristics. GPs in countries with high CVD burden and low life expectancy at age 60 were most likely to treat hypertension in oldest-old.Key Points • General practitioners (GPs) are in a clinical dilemma when deciding whether (or not) to treat hypertension in the oldest-old (>80 years of age). • In this study including 1947 GPs from 29 countries, we found that a high country-specific cardiovascular disease (CVD) burden (i.e. myocardial infarction and/or stroke) was associated with a higher GP treatment probability in patients aged >80 years. • However, the association was modified by country-specific life expectancy at age 60. While there was a positive association for GPs in countries with a low life expectancy at age 60, there was no association in countries with a high life expectancy at age 60. • These findings help explaining some of the large variation seen in the decision as to whether or not to treat hypertension in the oldest-old.

Pharmacokinetics of cefquinome in red-eared slider turtles (Trachemys scripta elegans) after single intravenous and intramuscular injections

    Uney, K.; Altan, F.; Cetin, G.; Aboubakr, M.; Dik, B.; Sayın, Z.; Er, A.; Elmas, M.

The purpose of this study was to evaluate the pharmacokinetics of cefquinome (CFQ) following single intravenous (IV) or intramuscular (IM) injections of 2 mg/kg body weight in red-eared slider turtles. Plasma concentrations of CFQ were determined by high-performance liquid chromatography and analyzed using noncompartmental methods. The pharmacokinetic parameters following IV injection were as follows: elimination half-life (t1/2λz) 21.73 ± 4.95 hr, volume of distribution at steady-state (Vdss) 0.37 ± 0.11 L/kg, area under the plasma concentration–time curve (AUC0–∞) 163 ± 32 μg hr−1 ml−1, and total body clearance (ClT) 12.66 ± 2.51 ml hr−1 kg−1. The pharmacokinetic parameters after IM injection were as follows: peak plasma concentration (Cmax) 3.94 ± 0.84 μg/ml, time to peak concentration (Tmax) 3 hr, t1/2λz 26.90 ± 4.33 hr, and AUC0–∞ 145 ± 48 μg hr−1 ml−1. The bioavailability after IM injection was 88%. Data suggest that CFQ has a favorable pharmacokinetic profile with a long half-life and a high bioavailability in red-eared slider turtles. Further studies are needed to establish a multiple dosage regimen and evaluate clinical efficacy.

Dilek, Esra; Caglar, Sema; Erdogan, Kübra; Caglar, Bülent; Sahin, Onur

In this study, the effects of four novel mononuclear palladium(II) and platinum(II) complexes on the activity of human serum paraoxanase1 were examined. First, four novel mononuclear palladium(II) and platinum(II) complexes were synthesized with a nitrogen donor ligand 1-(2-aminoethyl)pyrrolidine and nonsteroidal anti-inflammatory drugs diclofenac, mefenamic acid. These complexes were characterized by spectroscopic, thermal, and elemental analyses. The crystal structures of complex [Pd(2-amepyr)2](dicl)2 1 and [Pd(2-amepyr)2](mef)2 3 were determined by X-ray crystallography. Then, paraoxonase1 enzyme was purified from human serum. The effects of these complexes on enzyme were evaluated in vitro. The complexes consist of the cationic unit and the counterions. The diclofenac and mefenamic acid acted as a counterion in the complexes. It was observed that all the complexes were stable up to high temperatures. These complexes, even at low doses, inhibited the activity of the enzyme with different inhibition mechanisms.

Effect of anakinra, an interleukin one beta antagonist, on oxidative testicular damage induced in rats with ischemia reperfusion; [Efecto de la anakinra, que es un antagonista de la interleucina 1 beta en oxidativo daño testicular, inducida en ratas con isquemia reperfusión]

Hirik, Erkan; Suleyman, Bahadir; Mammadov, Renad; Yapanoglu, Turgut; Cimen, Ferda Keskin; Cetin, Nihal; Kurt, Nezahat

Background: It has been reported in the literature that proinflammatory interleukin-1 beta (IL-1β) is increased in cases of testicular ischemia reperfusion (I/R) damage. This information suggests that anakinra, an IL-1β antagonist, may be effective in testicular I/R damage. Objective: In our study, we investigated the effect of anakinra on testicular I/R damage induced in rats with torsion/detorsion. Methods: The 50 mg/kg anakinra + testicular torsion/detorsion (KTD-50) and 100 mg/kg anakinra + testicular torsion/detorsion (KTD-100) groups received an intraperitoneal (i.p.) injection of 50 mg/kg and 100 mg/kg of anakinra, respectively. In turn, the testicular torsion/detorsion (TTD) and sham operation (SOG) groups received a single dose of distilled water as a solvent 1 h before ketamine anaesthesia. After the testes of the TTD, KTD-50 and KTD-100 groups were subjected to torsion and detorsion for 4 h each, the rats were killed with a high-dose anaesthesia, and their testicles were removed and evaluated through biochemical, gene expression and histopathological examinations. The results were evaluated in comparison with those of the SOG group. Results: The levels of malondialdehyde (MDA), myeloperoxidase (MPO) and IL-1β showed significant increases in the TTD group, which underwent torsion/detorsion, compared to the KTD-50, KTD-100 and SOG groups. Conversely, the levels of glutathione (tGSH), glutathione peroxidase (GPO) and glutathione s-transferase (GST) were found to be significantly higher in the KTD-50, KTD-100 and SOG groups than in the TTD group. Conclusion: Anakinra at a 100 mg/kg dose histologically suppressed better oxidative stress and tunica albuginea, germ cell, seminiferous tubule and interstitial damage in the testicular tissue compared to a 50 mg/kg dose. Experimental results indicate that anakinra might be beneficial in the attenuation of testicular I/R damage.

Karabulut, Ibrahim; Koc, Erdem; Yilmaz, Ali Haydar; Ahiskali, Elif Oral; Keskin, Ercument; Adanur, Senol; Resorlu, Berkan

OBJECTIVE:: In this study, we aimed to compare cases of retrograde intrarenal surgery performed under spinal or general anesthesia through investigating relevant parameters for the first time in the literature. MATERIAL AND METHOD:: In total, 86 patients with diagnosis of kidney stone who were treated by retrograde intrarenal surgery were included in this randomized controlled prospective study. In total, 43 of these operations were performed under spinal anesthesia (group I) and 43 were performed under general anesthesia (group II). The groups were compared in terms of demographic features, American Society of Anesthesiologists score, duration of operation, complication rates, postoperative visual analogue scale, postoperative hospitalization period, stone-free rates, and cost value ratios. RESULTS:: There were no significant differences between the two groups in terms of demographic findings, preoperative stone loads, postoperative stone-free rates, complication rates and postoperative hospitalization periods (p > 0.05). Postoperative visual analogue scale scores and cost value ratios were found statistically significantly lower in the spinal anesthesia group (group I) when compared with the general anesthesia group (group II; p < 0.001). CONCLUSION:: Performing retrograde intrarenal surgery in the presence of spinal anesthesia is equally effective with general anesthesia. Spinal anesthesia also appears to be a more advantageous method due to statistically significantly lower mean postoperative pain scores and treatment cost value ratios.

Kockara, Nizamettin; Sofu, Hakan; Issin, Ahmet; Çamurcu, Yalkın; Bursali, Aysegul

Background The main purposes of the present study were to evaluate the functional improvement, to identify the predictors of the clinical outcome after open surgical management of femoroacetabular impingement (FAI) in mid-term, and to determine 5-year survival rate. Methods In this retrospective study, the clinical data of 33 patients (34 hips) were evaluated. Sex, age, body mass index (BMI), type of FAI, duration of symptoms up to surgical treatment, internal rotation of the affected hip, improvement of the Harris hip score (HHS) in the first 2 years, joint space narrowing evaluated radiographically and classified according to Kellgren–Lawrence classification, and any labral tear were the independent variables. Harris hip score and degenerative arthritic changes at the latest follow-up were identified as outcome measures. Univariate and multivariate analyses were performed. The mean post-operative follow-up was 6 years. Results Harris hip scores at the latest follow-up were significantly worse in patients with a pre-operative age >35 years, BMI ≥25 kg/m2, internal rotation of the hip ≤10° grade 1 or 2 joint space narrowing, labral tear, and <15 points improvement of the HHS at the first 2 years follow-up. According to multivariate analysis; major predictors of the outcome were BMI, labral tear, and improvement of the HHS at the first 2 years. Conclusions A pre-operative BMI ≥25 kg/m2, labral tear, and <20 points improvement of the HHS in the first 2 years of surgery should be considered as the major predictors of the clinical outcome after open surgical management of idiopathic FAI.

Effect of Prone and Supine Positioning on Computed Tomography Urography Examination

Karavaş, Erdal; Gürel, Safiye; Klyan, Aysu; Halicioǧlu, Slddlka; Daǧistan, Emine

Objectives: The aim of this study is to evaluate the effects of performing computed tomography (CT) urogram in the prone position in terms of diagnosis. Methods: CT urograms of 208 patients imaged randomly in the prone and supine positions were included in this study. A total of 199 patients and 370 collecting systems were examined in total. Axial raw data and reconstructed coronal thin and thick MIP images with a slice thickness of 5 mm were evaluated by 2 independent radiologists blinded to the initial diagnosis. Renal collecting system, ureters, and bladder were included in radiological evaluation. Anatomically, the renal collecting system was separated into 7 regions. Filling and dilatation of collecting systems were evaluated via images at urogram phase by scoring. Results: Filling in lower pole infundibulum (p = 0.006), distal ureter (p = 0.006); and highly dilated lower pole calyx (p = 0.020), pelvis (p = 0.006), and bladder (p < 0.001) were determined to be better in images in the prone position compared to the supine position. There were no statistical differences in other regions. Conclusion: Better contrast material filling is achieved in dilated or non-dilated lower pole collecting system, dilated renal pelvis, non-dilated distal ureter of kidney, and in the bladder only by imaging the urogram phase in the prone position compared to the supine position. Additionally, presence of dilatation is a factor that could adversely affect filling. Studies in the future may investigate the contribution of prone positioning to CT urogram with larger series comparing it with other methods and modalities.

Çöbden, Adem; Çamurcu, İsmet Yalkın; Üçpunar, Hanifi; Sofu, Hakan; Duman, Serda

Introduction: Osteoporosis in men is often underdiagnosed and neglected compared to postmenopausal women. Major risk factors should be determined to prevent this disease, which has potential risk of mortality and morbidity for elder men. This study’s purpose was to determine the risk factors for osteoporosis and osteopenia in male patients aged >60 years. Materials and Method: Between January 2015 and June 2017, male patients aged >60 years admitted to our centers who underwent dual energy x-ray absorptiometry (DXA) were included in this retrospective study. Patients’ age, body mass index, site of residence (village/ city), smoking status, and systemic diseases were examined through our records. Patients were divided into control, osteopenia, and osteoporosis groups according to DXA measurements. Serum vitamin D measurements also were evaluated. Multivariate analysis was performed to determine major risk factors for osteoporosis and osteopenia. Results: Compared with the control group, patients in the group categorized as having osteoporosis or osteopenia had a significantly lower body mass index and more commonly resided in the city (p<0.001 and p<0.005, respectively). Serum vitamin D levels were significantly lower in the osteoporosis and osteopenia groups than in the control group (p=0.001). According to multivariate analysis, insufficient or deficient serum vitamin D levels were determined to be a major risk factor for osteoporosis and osteopenia (p=0.001). Conclusion: According to our results, men aged >60 years with deficient serum vitamin D levels (<20 ng/mL) were six times more likely to have osteopenia and 80 times more likely to have osteoporosis.

Gadolinium-Based Contrast Agent Extravasation Mimicking Subarachnoid Hemorrhage After Electroconvulsive Therapy

Taydas, Onur; Ogul, Hayri; Ozcan, Halil; Kantarci, Mecit

Background: Electroconvulsive therapy (ECT) is a safe method that has been applied for many years in medical treatment−resistant depression treatment. In this case report, contrast extravasation due to deterioration of the blood-brain barrier after ECT mimicking subarachnoid hemorrhage (SAH) on magnetic resonance imaging (MRI) is discussed. Case Description: A 70-year-old male patient who underwent ECT presented with sulcal hyperintensities suggesting subarachnoid hemorrhage on fluid-attenuated inversion recovery sequence of MRI obtained after ECT. However, there was no evidence to suggest SAH on nonenhanced brain computed tomography. Conclusion: It should be kept in mind that patients may have contrast extravasation due to deterioration of the blood-brain barrier after ECT, and other alternative methods should be used for the diagnosis of SAH in these patients, not MRI.

Evaluation of the Efficacy of Tea Tree Oil on the Density of Demodex Mites (Acari: Demodicidae) and Ocular Symptoms in Patients with Demodectic Blepharitis

    Karakurt, Yücel; Zeytun, Erhan

The objective of this study was to examine the efficacy of 7.5% tea tree oil (TTO) eyelash shampoo on the density of Demodex (Acari: Demodicidae) and ocular symptoms in patients with demodectic blepharitis. A total of 135 patients diagnosed with demodectic blepharitis, following clinical inspection and parasitological examinations, were included in the study. Patients were treated with eyelash shampoo, with or without TTO. Full Demodex reduction was attained in 36% of the patients who used eyelash shampoo with TTO, and the average Demodex count was reduced from 6.33/eyelash to zero (P < 0.001). In contrast, the average Demodex count in patients who did not achieve full reduction (64%) was reduced from 12.46/eyelash to 4.15/eyelash (P < 0.001). Full reduction was observed in 11.7% of patients who used TTO-free eyelash shampoo, and average Demodex count reduced from 2.00/eyelash to zero (P = 0.017). The average Demodex count was reduced from 11.98/eyelash to 7.91/eyelash in patients (88.3%) who did not achieve full reduction (P = 0.024). Additionally, ocular symptom scores (which are generated based on subjective experiences of itching, burning, the feeling of a foreign body in the eye, eye redness, and cylindrical dandruff) were reduced significantly (P < 0.001) in patients who used the eyelash shampoo with TTO. The symptom scores remained almost the same in patients who used TTO-free eyelash shampoo (P > 0.05). In conclusion, it was determined that eyelash shampoo with TTO is 3 times more effective at achieving full Demodex reduction, significantly reducing the Demodex count, and relieving ocular symptoms in patients where full reduction cannot be attained, without adverse side effects.

Predictor Role of Pretreatment Resistive and Pulsatile Indexes in the Success of Medical Expulsive Therapy of Ureteral Stones

Erbay, Güven; Yalcın, Ahmet; Gultekin, Mehmet Hamza

Objective: To investigate the role of renal resistive index (RI) and pulsatile index levels measured with renal Doppler ultrasonography in predicting the success of medical expulsive treatment in patients with ureteral stones. Materials and Methods: Patients admitted to our clinic between January and December 2017 with a ureteral stone of less than 1 cm in diameter were evaluated in terms of their RI and pulsatile index values obtained using Doppler ultrasonography, and the localization and diameter of their stone and grade of hydronephrosis using computed tomography at the time of admission. After 4 weeks of medical expulsive treatment with tamsulosin, spontaneous stone passage was assessed to investigate the relationship between the data obtained before treatment and the success of medical expulsive treatment. Results: Forty-eight of 71 patients passed the stone spontaneously after treatment (67.6%). Among the related variables, the largest diameter of stone and ipsilateral RI levels were independently associated with the success of medical expulsive treatment (P <.001). In the receiver operating characteristic curve analysis, a RI value of less than 0.72 was associated with spontaneous stone expulsion with a sensitivity of 89.6%, a specificity of 69.6%, and a positive predictive value of 97%. Conclusion: It is possible to predict the success of medical expulsive treatment using RI levels. We anticipate that in patients who are not likely to pass the stones spontaneously with medical treatment, surgery should not be postponed to avoid irreversible kidney damage.

The crystal structures of 1, 3, 4 and 5 were determined by X-ray diffraction analysis, whereas 2 and 6 were obtained as microcrystalline powders. X-ray diffraction analysis demonstrated that 1 occurs as a tetranuclear complex, while 3, 4 and 5 have binuclear structures. A short Ag⋯Ag distances of 2.8446 and 2.8823 Å for 1, 2.8409 Å for 3, 2.8942 Å for 4 and 2.8423 Å for 5 were found in the complexes, indicating that argentophilic interactions exist between the silver ions. In 1, the dicl ligands act as μ3-O,O′,O′ bridging ligands between three Ag(I) ions. In 3, 4 and 5, the dicl and mef ligands behave as bridging ligands and bind two Ag(I) ions together with the carboxylato oxygen atoms. The simultaneous TG/DTG and DTA techniques were applied to interpret the mass losses, decomposition temperatures and corresponding processes of the thermal behaviours of the complexes. The experimental and calculated mass losses of the proposed structures for complexes 2 and 6 are quite compatible with each other and these complexes have very similar FT-IR spectra to the other complexes, which reflect their same structural geometries and characteristics. The cytotoxic activities of the complexes (1–6) were tested against three different cancer cell lines, MCF-7, HT-29 and HepG2, and one normal cell line, 3 T3-L1. XTT assay results demonstrated that although all the silver(I) complexes showed good cytotoxic activity, depending on the cell types tested, the complexes with mefanamic acid (2, 4 and 6) were found to predominate over those with diclofenac (1, 3 and 5), as well as the superiority of 3-picoline to 4-picoline and 2-picoline. Moreover, compared with the cytotoxic potential of carboplatin, 4 especially exhibited a significant cancer cell inhibitory rate and lower cytotoxicity toward the normal cell line, with much higher selectivity indexes.

OBJECTIVE: Recent studies have shown that atherosclerosis is associated with erectile dysfunction and the serum bilirubin level. In this study, the serum total bilirubin levels of patients with erectile dysfunction were measured to investigate the relationship between the levels of erectile dysfunction and total bilirubin. METHODS: A total of 94 patients with erectile dysfunction were divided into three groups; severe erectile dysfunction (33 patients), moderate erectile dysfunction (31 patients), and mild erectile dysfunction (30 patients). In addition, a control group was formed with 31 healthy men. The International Index of Erectile Function-5 Questionnaire was used to measure the quality of erection in all the groups. The body mass index was calculated for all the participants. The serum glucose, low-density lipoprotein and high-density lipoprotein, cholesterol, triglyceride, total bilirubin, and total testosterone levels were also determined. RESULTS: No statistically significant difference was observed between the groups in terms of the mean age, hypertension, smoking status, alcohol use, cardiovascular diseases, hepatobiliary disease, diabetes mellitus, and levels of total testosterone, low-density lipoprotein-cholesterol, and triglyceride. However, high-density lipoprotein, body mass index, and total bilirubin were significantly lower compared to the control group (p < 0.001). The serum total bilirubin level was found to be 0.41 ± 0.21 ng/dL in the severe erectile dysfunction, 0.43 ± 0.19 ng/dL in the moderate erectile dysfunction, and 0.48 ± 0.11 ng/dL in the mild erectile dysfunction groups (p < 0.001). CONCLUSION: Considering the significant differences between the erectile dysfunction and control groups in terms of serum total bilirubin levels, a low level of bilirubin may have a role in the etiology of erectile dysfunction.

In polycystic ovary syndrome (PCOS), myo-inositol (MI) supplements have shown many beneficial effects. In this study, therefore, we aimed to investigate the serum level of myo-inositol oxygenase (MIOX), which is the only enzyme catalyzing MI in vivo, in patients with PCOS. Serum MIOX enzyme levels and other laboratory parameters were compared between sixty patients, who were diagnosed with PCOS for the first time, and sixty healthy individuals at similar age and sex. MIOX serum levels were not different between two groups (p = 0.7428). MIOX median and 95% CI were 19.4 and 10.6–39.1 in the control group and 16.4 and 7.6–46.2 in the patient group respectively. Demographic data, biochemical and hematological parameters, hormone parameters were not different except from the lymphocyte count between the two groups. Lymphocyte count was higher in the patient group. Although the ratio of LH/FSH was higher in the patient group, it was not statistically significant. Our results suggest that serum MIOX levels do not change in PCOS. It was, therefore, concluded that MI deficiency observed in PCOS was not related to the level of MIOX enzyme which cleaves MI.

Aims: Turkey has the highest prevalence of diabetes in Europe. It is therefore essential to know the overall cardiovascular risk and reveal the predictors of metabolic control in Turkish adults with diabetes mellitus. Methods: A nationwide, multicenter survey consecutively enrolled patients who were under follow up for at least a year. Optimal control was defined as HbA1c < 7%, home arterial blood pressure (ABP) < 135/85 mmHg, or LDL-C < 100 mg/dL. Achieving all parameters indicated triple metabolic control. Results: HbA1c levels of patients (n = 5211) were 8.6 ± 1.9% (71 ± 22 mmol/mol) and 7.7 ± 1.7% (61 ± 19 mmol/mol), in Type 1 and Type 2 diabetes, respectively. Glycemic control was achieved in 15.3% and 40.2%, and triple metabolic control was achieved in 5.5% and 10.1%, respectively. Only 1.5% of patients met all the criteria of being non-obese, non-smoker, exercising, and under triple metabolic control. Low education level was a significant predictor of poor glycemic control in both groups. Conclusions: Few patients with Type 2, and even fewer with Type 1 diabetes have optimal metabolic control in Turkey. TEMD study will provide evidence-based information to policy makers to focus more on the quality and sustainability of diabetes care in order to reduce the national burden of the disease.
