Muftuoglu, Sevda Fatma; Bozdag, Gurkan; Mumusoglu, Sezcan; Portakal, Oytun; Coşkun, Neslihan; Sökmensüer, Lale Karakoç; Güneş, Ali Can; Zirh, Selim

Objective: This study aimed to evaluate whether administration of luteinizing hormone (LH) before the ovulation trigger could modulate elevated progesterone levels. Methods: Human granulosa-theca tissues were obtained from 15 women aged 18–38 years undergoing laparoscopic ovarian cystectomy at the Department of Gynecology and Obstetrics, Hacettepe University. The tissues were cultured in vitro and treated with 12.5 mIU/mL recombinant follicle-stimulating hormone (r-FSH; Group A), 18.75 mIU/mL r-FSH (Group B), or 18.75 mIU/mL r-FSH plus 6.25 mIU/mL recombinant LH (r- LH; Group C). Baseline and post-treatment estradiol (E2) and progesterone concentrations were measured at 0, 24, and 48 hours. Immunofluorescence staining for aromatase, cytochrome P450, and Ki67 was performed to assess steroidogenic activity and cellular proliferation. Results: A mild trend toward reduced progesterone and increased E2 levels was observed following LH supplementation at both time points; however, these changes did not reach statistically significant. No statistically significante differences were observed in progesterone or E2 levels among the treatment groups at 24 h (P=0.891 and P=0.805, respectively) or 48 h (P=0.878 and P=0.505, respectively). Expression levels of aromatase, cytochrome P450, and Ki67 did not differ significantly between groups at 24 h (P=0.665, P=0.932 and P=0.953, respectively) or 48 h (P=0.911, P=0.674 and P=0.202, respectively). Conclusion: These findings suggest that although r-LH addition to high-dose r-FSH does not significantly alter progesterone synthesis, it may exert a subtle regulatory effect on steroidogenesis. Larger studies with extended culture durations are needed to further elucidate the biological and clinical significance of these observations.

Gür, Volkan; Akyüz, Sümeyye; Yapici, Furkan; Gülhan, Barış; Doğan, İbrahim; Gökgöz, Mehmet Burak; Ozturk, Akin; Topaluğurlu, Nedim Batuhan; Dündar, Harun

Objective: Rapid identification of causative microorganisms is essential in suspected native septic arthritis and periprosthetic joint infection. This study evaluated the diagnostic performance of the BioFire® Joint Infection Panel and assessed its agreement with conventional culture and systemic inflammatory biomarkers. Methods: Hospital records from January 2025 to April 2026 were retrospectively reviewed. Of 115 screened patients with suspected joint infection, nine were excluded because BioFire® testing had not been performed and only culture results were available. The final analysis included 106 synovial fluid samples. All samples were tested using conventional culture and the BioFire® Joint Infection Panel during the same clinical episode. Demographic data, infection category, localization, C-reactive protein, erythrocyte sedimentation rate, procalcitonin, and white blood cell count were recorded. Agreement was assessed using Cohen’s kappa, discordance using the McNemar test, and diagnostic performance using culture as the reference method. Results: Sixty-five samples (61.3%) were classified as suspected native septic arthritis and 41 (38.7%) as periprosthetic joint infection. BioFire® was positive in 22 samples (20.8%) and culture was positive in 32 (30.2%). Overall agreement was 86.8%, with substantial agreement according to Cohen’s kappa (0.656). Discordance was significant, predominantly due to culture-positive/BioFire®-negative samples (P = 0.013). BioFire® showed 62.5% sensitivity [95% confidence interval (CI): 43.7-78.9), 97.3% specificity (95% CI: 90.6-99.7), 90.9% positive predictive value, 85.7% negative predictive value, and 86.8% accuracy. C-reactive protein, procalcitonin, and white blood cell counts were significantly higher in patients with suspected native septic arthritis. Conclusion: The BioFire® Joint Infection Panel provides rapid and highly specific microbiological support in suspected joint infection. However, its moderate sensitivity and inability to detect off-panel organisms indicate that it should complement rather than replace conventional culture.

Comprehensive MEFV Variant Spectrum in Pediatric Patients with Suspected Familial Mediterranean Fever: A Whole-Gene Sequencing Study

Gunay, Murat; Demir, Abdussamed Yasin; Yeter, Bahtinur

Objective: To determine the Mediterranean Fever (MEFV) variant spectrum using whole-gene next-generation sequencing (NGS) in pediatric cases with suspected Familial Mediterranean Fever (FMF). Methods: This retrospective cross-sectional study included 471 pediatric cases referred for MEFV analysis. All exons were analyzed using NGS, and variants were classified according to American College of Medical Genetics and Genomics criteria. Results: At least one MEFV variant was detected in 59.4% of cases. A total of 23 variants were identified, predominantly in exon 10 (43.5%) and exon 2 (34.8%). The most frequent variants were R202Q (43.3%), E148Q (16.3%), and M694V (13.7%). A low-frequency variants of uncertain significance, G150R, was detected in one case. Heterozygous genotypes were the most common (59.6%). Conclusion: Whole-gene sequencing enables the detection of both common and rare MEFV variants that may be missed by limited mutation panels. However, frequently detected variants, such as R202Q, require careful interpretation because of their high population frequency and controversial pathogenicity.

Kızılgöz, Volkan; Sunar, Mukadder; Gökgöz, Mehmet Burak; Kolak, Merve

Objective: To ascertain whether measurements of the quadratus femoris and ischiofemoral intervals obtained on pelvic magnetic resonance imaging (MRI) examinations are useful for diagnosing ischiofemoral impingement (IFI) syndrome. Methods: MRIs of 178 patients (104 females and 74 males) were reinterpreted bilaterally for signal changes on T2-weighted sequences in the ischiofemoral space (IFS) and quadratus femoris space (QFS), indicating IFI. Receiver operating characteristic (ROC) curves were plotted for each measurement parameter, and the corresponding area under the curve (AUC) values were reported. A P value of less than 0.05 was considered to indicate a significant difference, and the results were assessed at the 95% confidence level. Results: There was no significant difference in age between females and males (P = 0.064). Radiologically, 17.7% of all participants were diagnosed with IFI. Significant differences were observed between IFS and obturator femoris space values by gender, with both values being significantly lower in women. A higher percentage of patients testing positive for IFI was observed in women compared with men. In the ROC analysis conducted according to IFS, the AUC was 0.840. The highest value of sensitivity × specificity was 21.29 mm. The sensitivity at this value was 0.676. The specificity at this value was 0.857. In the ROC analysis using QFS, the AUC was 0.856. The highest value of (sensitivity × specificity) was 16.17 mm. The sensitivity at this value was 0.765. The specificity at this value was 0.794. Conclusion: The results of the current study indicated a significant relationship between the IFS and QFS measurements and IFI. These intervals showed a significant negative correlation with age. This study also identified important cut-off values for IFS and QFS intervals that indicate IFI, with relatively high sensitivity and specificity.

Retrospective analysis of multiplex panel tests in the diagnosis of respiratory tract infections

Ozturk, Ali; Koklu, Ramazan; Aydin, Merve; Celebi, Basar; Altun, Zehra Turkmen; Erdogan, Fatma; Evren, Kubra

Respiratory tract infections (RTIs) remain a major global health concern, particularly in children. The overlapping clinical manifestations of RTIs highlight the need for rapid and accurate diagnostic methods such as multiplex PCR (mPCR). This retrospective single-center study investigated the epidemiology of respiratory pathogens detected by mPCR, focusing on their distribution, age-related patterns, seasonal variation, and coinfections.Nasopharyngeal swab samples from 328 patients with suspected acute RTIs, collected between January 2022 and December 2023, were analyzed using the BioFire FilmArray Respiratory Panel 2.1. Demographic characteristics, clinical findings, pathogen distribution, seasonal trends, and coinfections were evaluated. At least one respiratory pathogen was detected in 270 of 328 specimens, yielding an overall positivity rate of 82.3%. Viral pathogens predominated; human rhinovirus/enterovirus HRV/HEV (28%), adenovirus (21.8%), respiratory syncytial virus (RSV) (14.2%), and influenza viruses (11.3%) were the most frequently identified agents. Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and human metapneumovirus (HMPV) were detected in 6.8% and 5.8% of cases, respectively. The vast majority of specimens (91.5%) were from pediatric patients (0-18 years). Coinfections were identified in 31.8% of positive cases, most commonly involving adenovirus and HRV/HEV. Seasonal analysis demonstrated a marked increase in positivity during winter and spring months, accounting for 74% of all positive cases. Bacterial pathogens were rarely detected.mPCR demonstrated high diagnostic value, with pathogens detected in 82.3% of samples, predominantly viral agents. These findings support the role of mPCR in improving diagnostic accuracy and providing valuable regional epidemiological data. Rapid identification of viral pathogens may also support antimicrobial stewardship by facilitating more informed clinical decision-making; however, antibiotic utilization was not evaluated in the present study.

Neuroendocrine effects of exogenous adropin administration on the hypothalamic pituitary testicular axis in male rats

Eraslan, Ersen; Tanyeli, Ayhan; Güler, Mustafa Can; Özbek Bilgin, Aslı; Ekinci Akdemir, Fazile Nur; Polat, Elif; Çomakli, Selim; Kurt, Nezahat; Bal Taştan, Tuğba

Background/aim: Obesity impairs male fertility through metabolic dysfunction, oxidative stress, and disruption of the hypothalamic– pituitary–testicular (HPT) axis. Adropin (ADR), a peptide hormone whose circulating levels are reduced in obesity, plays emerging roles in metabolic homeostasis; however, its involvement in reproductive endocrine regulation remains unclear. The present study was conducted in healthy, nonobese male rats and aimed to investigate the neuroendocrine and testicular effects of exogenous ADR administration, focusing on circulating reproductive hormones, hypothalamic regulatory peptides, and testicular antioxidant pathways. Materials and methods: Thirty-two male Wistar rats were randomized into control, sham, low-dose ADR (4 μg/kg/day), and high-dose ADR (40 μg/kg/day) groups and treated for 10 days. An enzyme-linked immunosorbent assay (ELISA) was used to measure circulating gonadotropins, testosterone, inhibin B, and activin A. Hypothalamic gonadotropin-releasing hormone (GnRH) and kisspeptin expression, and testicular superoxide dismutase 1 (SOD1) localization were assessed by immunohistochemistry. Brain and testis morphology were examined histologically. Results: High-dose ADR administration was associated with increased hypothalamic GnRH and kisspeptin expression, accompanied by reduced circulating LH levels, while FSH concentrations remained unchanged. Testosterone and inhibin B levels were higher, whereas activin A levels were lower, in the high-dose ADR group compared with controls. ADR administration was also associated with enhanced testicular SOD1 immunoreactivity and dose-dependent reductions in body weight. No overt histopathological alterations were observed in the cerebral cortex or testicular tissue. Conclusion: In healthy, nonobese male rats, exogenous ADR administration was associated with changes in central neuroendocrine markers and testicular antioxidant responses without overt histopathological alterations. These findings do not demonstrate improvements in fertility but suggest that ADR may be involved in pathways linking metabolic signals with reproductive endocrine regulation. The potential relevance of these observations to obesity-associated male reproductive dysfunction remains hypothesis-generating and requires confirmation in appropriate disease models and functional reproductive studies.

Determination of antimicrobial activity of Niaouli Essential Oil (Melaleuca quinquenervia) by two different methods; [Nioli Esansiyel Yağının (Melaleuca quinquenervia) antimikrobiyal aktivitesinin iki farklı yöntemle belirlenmesi]

    Akyüz, Sümeyye

Objective: This study aimed to investigate the antimicrobial activity of Niaouli Essential Oil (Melaleuca quinquenervia), a commercially available oil known for its antiseptic, antimicrobial, antioxidant, anti-inflammatory, and antiprotozoal properties due to its bioactive constituents, against eight different standard bacterial strains, two standard yeast strains, and one clinical Candida isolate. Methods: The antimicrobial activity of niaouli essential oil against the isolates was evaluated using the disk diffusion and broth microdilution methods in accordance with EUCAST and CLSI guidelines. Ten of the microbial strains were American Type Culture Collection (ATCC) strains, and one was a clinical Candida auris isolate. The isolates were obtained from the strain collection of our university. Results: In this study, the disk diffusion method revealed that niaouli essential oil (Melaleuca quinquenervia) was most effective against Staphylococcus aureus (20 ± 0.0 mm) and Streptococcus pneumoniae (17.3 ± 0.57 mm) among the bacterial strains, and against Candida albicans (20.6 ± 0.57 mm) among the fungal strains. The Candida auris isolate exhibited an inhibition zone of 16.0 ± 1.0 mm. The lowest MIC values determined by the broth microdilution method were 62.5 μg/mL for both S. aureus and C. albicans. Among the Gram-negative isolates, Klebsiella pneumoniae was the most susceptible (125 μg/mL), whereas Pseudomonas aeruginosa, Salmonella enteritidis, and Enterococcus faecalis were the least susceptible (>250 μg/mL). The MIC value for C. auris was determined to be 250 μg/mL, which is one dilution higher than the positive control (fluconazole: 125 μg/mL). Conclusion: Niaouli essential oil demonstrated antimicrobial activity against standard microbial strains, with varying levels of antibacterial and antifungal effects. According to both methods used, it was more effective against Gram-positive bacteria and fungi than Gram-negative bacteria. While it was particularly effective against Candida albicans among the yeasts, it also exhibited antimicrobial activity against the increasingly important Candida auris strain. Due to the increasing resistance to antibacterial and antifungal agents, further studies investigating alternative treatment options are needed.
