Coronavirus (COVID-19), advanced age, and malnutrition: A risky coexistence

    Kupeli, Ilke; Yüksel, Fatma

Malnutrition is a more common nutritional disorder in older patients that causes negative reactions to normal human functions and is associated with high risk of infection, prolonged mechanical ventilation, and increased cost. Clinical observations have revealed that many older patients with COVID-19 are at risk of malnutrition or suffer from malnutrition. The aim of this case series was to evaluate 20 older patients with coronavirus, who suffered from malnutrition, and to examine the parameters likely contributing to malnutrition. This series included a total of 20 patients over 65 years of age, who were hospitalized for COVID 19 and had malnutrition according to the Nutritional Risk Screening 2002 (NRS-2002) form applied during admission. The mean age of the patients was 75.5±8.7 (years) and the female/male ratio was 7/13. While only 6 patients had no comorbid disease, the others had coronary artery disease, diabetes mellitus, hypertension, cerebrovascular disease, Alzheimer's disease, and heart failure. Three patients were at risk of malnutrition (NRS <5); on the other hand, 17 patients were suffering from malnutrition (NRS ≥5). Advanced age, comorbidity, hypoalbuminemia, low lymphocyte count, and low hemoglobin and protein values were considered as risk factors for malnutrition in this case series performed in COVID 19 patients. The relationship of these findings with mortality could not be determined.

Insight into effects of Axillarin and Verbascoside isolated from Tanacetum alyssifolium and Plantago euphratica on probiotic properties of Lactobacillus acidophilus and Lactobacillus rhamnosus

Turan, Nasip Ümran; Çelebioğlu, Hasan Ufuk; Akşit, Hüseyin; Taş, Recep

Phenolic compounds are found in the seeds, flowers, leaves, stems, branches, and fruits of plants. They have an important place in human nutrition as many fruits and vegetables have phenolic compounds. Recently, many phenolic compounds have been used as functional foods and research of new phenolic compounds that are candidate to become functional foods is ongoing. Probiotics which are beneficial microorganisms, are living microbes that benefit the host when taken in sufficient amounts and they have an important role in human gastrointestinal microbiota. The present study aims to investigate effects of Axillarin and Verbascoside, two phenolic compounds isolated from Tanacetum alyssifolium and Plantago euphratica, respectively, on two very common probiotic bacteria Lactobacillus acidophilus and Lactobacillus rhamnosus. For this, growth kinetics, auto-aggregation, and microbial adhesion to solvents, which are some critical properties of probiotics, were investigated. Axillarin and verbascoside showed significant improvement on growths of Lactobacillus acidophilus LA-5 and Lactobacillus rhamnosus GG depending on the dose. Auto-aggregation properties were enhanced by these phenolic compounds, even though surface hydrophobicities were decreased. In conclusion, the present study indicates that the vital conditions of the probiotics could be modulated by Axillarin and/or Verbascoside, consequently their adhesion and colonization capabilities could be altered.

Does Adductor Tenotomy Need During Closed Reduction Have a Prognostic Value in the Treatment of Developmental Dysplasia of the Hip Between 6 and 12 Months of Age? “Adductor Tenotomy in the Treatment of Developmental Dysplasia’’

Ucpunar, Hanifi; Mert, Muhammed; Camurcu, Yalkin; Sofu, Hakan; Yildirim, Timur; Bayhan, Avni Ilhan

Background: Adductor tenotomy is needed for clinically complex hips with soft-tissue contractures. It may be proposed that the patients who need adductor tenotomy during closed reduction would have poorer prognosis than the ones without need for adductor tenotomy. The main purposes were to compare the normalization of acetabular index angle (AI), to determine the incidence of femoral head avascular necrosis (AVN), and to predict the need for any secondary surgical intervention between the patients who need adductor tenotomy and those who do not during closed reduction for developmental dysplasia of the hip. Materials and Methods: The study group consisted of 65 hips treated between 6 and 12 months of age. The mean age at the time of surgery was 8.1 ± 1.4 (6–12) months and the mean follow-up was 4.2 ± 1.5 years. Improvement of AI, rate and severity of AVN, and need for secondary surgery with its predictors were evaluated. Results: Adductor tenotomy was performed in 22 hips (Group 1), but not in 43 hips (Group 2). Normalization of the AI was − 14.8° ± 3.5° versus − 14.3° ± 3.2°. The overall incidence of AVN was 18.4%. The rate of secondary surgical intervention was higher in Group 1 (63.1% versus 36.9%) (p = 0.014). Conclusions: No significant difference was detected regarding the improvement of AI as well as the incidence of AVN between the groups. The need for adductor tenotomy during closed reduction was one of the main predictors of the possible secondary surgery.

Blocking VEGF by Bevacizumab Attenuates VEGF-Induced Vasospasm After Experimental Subarachnoid Hemorrhage in Rabbits

Akturk, Umut Dogu; Tuncer, Cengiz; Bozkurt, Huseyin; Sahin, Omer Selcuk; Bulut, Husamettin; Arikok, Ata; Dinc, Cem; Gurer, Bora; Turkoglu, Erhan

Objective: Vasospasm after subarachnoid hemorrhage (SAH) plays a vital role in the development of delayed cerebral ischemia. Anti- vascular endothelial growth factor (VEGF) antibodies, like bevacizumab (BEV), may attenuate VEGF-stimulated angiogenesis, reduced vascular cell proliferation, and improve vasospasm after SAH. Methods: Thirty-two adult male New Zealand white rabbits were randomly divided into 4 groups of 8 rabbits in each group: group 1 (control); group 2 (SAH); group 3 (SAH + vehicle); and group 4 (SAH + BEV). BEV (5 mg/kg, intraperitoneally) was administered 5 minutes after the intracisternal blood injection and continued for 72 hours once per day in the same dose for group 4. Animals were sacrificed 72 hours after SAH. Basilar artery cross-sectional areas, arterial wall thicknesses, and hippocampal degeneration scores were evaluated in all groups. Results: VEGF is associated with the narrowing of the basilar artery. Treatment with BEV statistically significantly increased the cross-sectional area of the basilar artery when compared with the SAH and the vehicle groups. Basilar artery wall thicknesses in the BEV group was statistically significant smaller than in the SAH and vehicle groups. The hippocampal degeneration scores for the BEV and control groups were similar and significantly lower than those for the SAH and vehicle groups. Conclusions: Cellular proliferation and subsequent vessel wall thickening is a reason to delay cerebral ischemia and deterioration of the neurocognitive function. Intraperitoneal administration of BEV was found to attenuate cerebral vasospasm and prevent delayed cerebral ischemia and improve neurocognitive function after SAH in rabbits.

An experimental study on the use of lycopene to prevent infertility due to acute oxidative ovarian damage caused by a single high dose of methotrexate

Turkler, Can; Onat, Taylan; Yildirim, Engin; Kaplan, Selcuk; Yazici, Gulce Naz; Mammadov, Renad; Sunar, Mukadder

Background. Methotrexate (MTX) is an antineoplastic agent, which increases the level of reactive oxygen species (ROS) and decreases the level of antioxidants. Lycopene, is a potent antioxidant, which is used because of its protective effect against tissue damage. Objectives. The aim of this study was to determine the effect of lycopene on ovarian MTX-induced injury in rats. Material and methods. Rats (n = 36) were randomly divided into 3 equal groups: a group with MTX only (MG, n = 12), a group with lycopene and MTX (LMG, n = 12), and a healthy control group (HCG, n = 12). Then, malondialdehyde (MDA), myeloperoxidase (MPO) and total glutathione (tGSH) levels and histopathological findings were examined in the ovaries of rats. Apart from the histopathological and biochemical evaluation, the reproductive performance of the experimental groups was also examined. Results. Our study demonstrated that, in ovarian tissues of rats administered MTX, there was a decrease in the levels of tGSH, while MDA and MPO were increased, but it is observed that these ratios are reversed in the LMG (p < 0.05). It also has been proven that a single, high-dose use of MTX causes infertility in female rats, prolongs the gestation period and reduces the number of offspring. Conclusions. Lycopene pretreatment ameliorates the MTX induced ovarian injury and infertility in rats through its antioxidative activities.

Ağgön, Eser; Agırbaş, Özturk; Alp, Hamit Hakan; Uçan, Izzet; Gürsoy, Recep; Hackney, Anthony C.

Background: The aim of this study was to determine hormonal responses to acute and chronic exposure to static and dynamic strength training programs using resistance bands in boxers. Material and methods: 19 male national boxers participated in the study. Boxers were instructed to perform strength exercises with resistance bands for 3 days a week for 8 weeks involving either dynamic (n=10) or static (n=9) resistance exercises. Blood samples were taken before exercise, immediately after the initial exercise session, and 8 weeks later following the last exercise session. Cortisol, growth hormone, adrenocorticotropic hormone adrenaline and noradrenaline levels were measured. Statistical analyses involved non-parametric analysis with an alpha level of .05. Results: Dynamic strength exercises were effective stimuli to growth hormone, adrenaline and noradrenaline, while static strength exercises provoked cortisol, growth hormone, adrenaline and noradrenaline responses both initially after exercise and after 8 weeks of chronic training. Neither dynamic nor static strength exercises were effective in prompting adrenocorticotropic changes after an exercise session or after 8 weeks of training. Conclusions: We showed that dynamic and static strength exercise protocols using resistance bands both could provoke acute and chronic hormonal responses in boxers similar to more traditional modes of such exercise. Fetal epicardial fat thickness: Can it serve as a sonographic screening marker for gestational diabetes mellitus?

    Aydin, Sonay; Fatihoglu, Erdem

Background: Gestational diabetes mellitus (GDM) is defined as the new onset of impairment in carbohydrate tolerance during pregnancy. The aim of the current study was to define fetal epicardial fat thickness (fEFT) changes that developed before 24 weeks of gestation, to evaluate the diagnostic effectiveness of fEFT in predicting GDM diagnosis, and to correlate fEFT values with hemoglobin A1C (HbA1C) values. Methods: The study included a total of 60 GDM patients and 60 control subjects. A record consisted of fEFT measurements, maternal body mass index, maternal subcutaneous fat thickness, and fetal subcutaneous fat thickness during sonographic screening performed at 18-22 gestational weeks. Fetal abdominal circumference (AC) values, estimated fetal weight (EFW), and fetal gender were also recorded. Results: The median fEFT measurement of the whole study population was 0.9 ± 0.21 mm; 1.05 ± 0.21 mm in the GDM patients, and 0.8 ± 0.15 mm in the control group. The median fEFT values of the GDM patients were significantly higher than those of the control group (P < 0.01). According to the correlation analysis results, a strong positive correlation was determined between the fEFT and HbA1C values (r = 0.71, P < 0.01), gestational week of the fetus (r = 0.76, P = P < 0.01), AC (r = 0.81, P < 0.01), and EFW (r = 0.71, P < 0.01). According to the receiver operating characteristic analysis results, a fEFT value of > 0.95 can predict GDM diagnosis with sensitivity of 65% and specificity of 88% (odds ratio = 13). Conclusion: fEFT values are increased in GDM cases, and the increase can be detected earlier than 24 weeks of gestation. fEFT values are positively correlated with HbA1C values and can serve as an early predictor for GDM diagnosis.

Protective effect of adenosine triphosphate against sunitinib-related skin damage in rats

    Yıldırım, N.; Karatas, A.; Cengiz, M.; Onalan, E.; Yazıcı, G.N.; Sunar, M.; Mammadov, R.; Coban, A.; Suleyman, H.

Cutaneous side effects associated with sunitinib use are a major problem in patients receiving cancer treatment. The aim of this study was to investigate the protective effect of adenosine triphosphate (ATP) against possible skin damage resulting from sunitinib use in rats. Thirty Albino Winstar rats were divided into the following three groups: healthy controls (HCs, n = 10), sunitinib (SUN, n = 10), and sunitinib + ATP (SAT, n = 10). ATP was injected intraperitoneally at a dose of 2 mg/kg. One hour subsequent to the administration of ATP and 0.9% NaCl, the SAT and SUN groups were orally administered a dose of 25 mg/kg sunitinib to the stomach. Macroscopic evaluation of the skin indicated lower levels of skin damage in the SAT group than in the SUN group. As an indicator of oxidative stress, malondialdehyde (MDA), total oxidant status (TOS), and oxidative stress index (OSI) levels were significantly higher in the SUN group than in the HC group, while total glutathione (tGSH) and total antioxidant status (TAS) levels were significantly lower. However, MDA, TOS, and OSI levels were significantly lower in the SAT group than in the SUN group, while tGSH and TAS levels were significantly higher. Histopathological examination revealed keratin plugs with edema, vasopathology, and inflammatory cell infiltration in the SUN group. The SAT group showed less necrotic epithelium, keratin plugs, edema, and vasopathology than the SUN group. ATP can be effective in preventing skin damage caused by sunitinib use by reducing oxidative stress.

Gore Karaali, Muge; Erdil, Duygu; Erdemir, Vefa Aslı; Gurel, Mehmet Salih; Koku Aksu, Ayse Esra; Leblebici, Cem

Acquired perforating dermatosis (APD) is a group of a rare dermatological disorder characterized by elimination of dermal connective tissue through epidermis. We aimed to evaluate the characteristics of patients diagnosed with APD and to determine the differences in comorbidities according to subtypes of APD. A retrospective, observational, cross-sectional study was designed. Patients diagnosed with APD between January 2008 and January 2019 were reviewed. Eighty patients were included in the study. 61.2% (n = 49) of the patients were female and 38.8% (n = 31) were male with a mean age of 58.4 ± 12.5 years. 82.5% (n = 66) of the patients were diagnosed with reactive perforating collagenosis (RPC) and 17.5% (n = 14) of perforating folliculitis (PF). The most common concomitant disease was diabetes mellitus (82.5%). 5.0% of the patients had malignancy. The comorbidity rate in RPC group was higher than PF (P <.05). Topical steroid was the most frequently (90.0%) used treatment. Complete response was obtained 55.0% of patients. Exitus was observed in 23.8% (n = 19) of patients in a mean 17.6 ± 25.7 months follow-up period. APD may be associated with many diseases. Comorbidities are more frequent in RPC group. This situation warns us to evaluate patients with RPC in more detail for underlying diseases. High mortality rate related to the underlying systemic diseases suggests being careful in terms of mortality in patients diagnosed with APD.

Objective: Chronic obstructive pulmonary disease (COPD) is a chronic inflammatory disease characterized by obstructed airflow in the lungs. Pulmonary hypertension (PH) is a common complication of COPD and it is associated with pulmonary vascular remodelling. Monocyte chemoattractant protein-1 (MCP-1/CCL2) is one of the key chemokines associated with migration of monocytes and macrophages. Both MCP-1 and its receptor CCR2 have been declared to be involved in various diseases. We aimed to research a possible association of MCP-1 level, MCP-1 -2518 A/G and CCR2 190 A/G polymorphisms with COPD and pulmonary hypertension in this study. Material and methods: Eighty patients and eighty controls were included in the study. Serum MCP-1 levels were measured by ELISA method. Restriction fragment length polymorphism (RFLP) procedure was used to detect the genotypes of patients and control group. Results: MCP-1 levels were found to be significantly higher in COPD patients than in healthy controls (P=0.001) and patients with COPD + PH had higher serum MCP-1 levels than COPD patients (P=0.005). No association was found between MCP-1/CCR2 gene polymorphisms and patient groups (COPD and COPD + PH). Conclusion: MCP-1 level seems to be associated with both COPD and pulmonary hypertension. Increased MCP-1 expression may most likely to be involved in the pathogenesis of these diseases.

    Systemic chemotherapy in colorectal cancer

Unal, Olcun Umit; Keser, Murat; Akagündüz, Baran

Colorectal cancer is the third most common type of cancer in the world. Relapse and death due to relapse develop in approximately half of the patients after curative resection in colon cancer. Adjuvant therapy is recommended to reduce relapses. Adjuvant treatment has yielded significant survival advantage in colon cancer. On the other hand, surgical resection rates were increased with neoadjuvant chemoradiotherapy in rectal cancer. The combination of 5-fluorouracil + oxaliplatin has become the standard treatment for stage III patients with colon cancer. In stage II patients, adjuvant therapy is recommended only for patients with a high risk of relapse (e.g., lymphovascular invasion, etc.), and 5-fluorouracil-based therapy is the standard treatment. Also, FOLFOX and FOLFIRI have standard primary chemotherapy options for metastatic colorectal cancer treatment. Addition of biological agents to FOLFOX or FOLFIRI chemotherapy increased survival over 2 years. In this section, all systemic therapies in colorectal cancer are examined.

Utilization of statins and LDL-cholesterol target attainment in Turkish patients with type 2 diabetes - a nationwide cross-sectional study (TEMD dyslipidemia study)

Bayram, Fahri; Sonmez, Alper; Haymana, Cem; Sabuncu, Tevfik; Dizdar, Oguzhan Sitki; Gurkan, Eren; Carlioglu, Ayse Kargili; Agbaht, Kemal; Ozdemir, Didem; Demirci, Ibrahim

Background: Attaining acceptable levels of LDL Cholesterol (LDL-C) significantly improves cardiovascular (CV) outcomes in patients with type 2 diabetes mellitus (T2DM). The LDL-C target attainment and the characteristics of patients attaining these targets were investigated in this study. Furthermore, the reasons for not choosing statins and the physicians’ attitudes on the treatment of diabetic dyslipidemia were also examined. Methods: A nationwide, cross-sectional survey was conducted in tertiary centers for diabetes management. Adult patients with T2DM, who were under follow-up for at least a year in outpatient clinics, were consecutively enrolled for the study. LDL-C goals were defined as below 70 mg/dL for patients with macrovascular complications or diabetic nephropathy, and below 100 mg/dL for other patients. Data about lipid-lowering medications were self-reported. Results: A total of 4504 patients (female: 58.6%) were enrolled for the study. The mean HbA1c and diabetes duration was 7.73 ± 1.74% and 10.9 ± 7.5 years, respectively. The need for statin treatment was 94.9% (n = 4262); however, only 42.4% (n = 1807) of these patients were under treatment, and only 24.8% (n = 448) of these patients achieved LDL-C targets. The main reason for statin discontinuation was negative media coverage (87.5%), while only a minority of patients (12.5%) mentioned side effects. Physicians initiated lipid-lowering therapy in only 20.3% of patients with high LDL-C levels. It was observed that the female gender was a significant independent predictor of not attaining LDL-C goals (OR: 0.70, 95% CI: 0.59–0.83). Conclusions: Less than 50 % of patients with T2DM who need statins were under treatment, and only a quarter of them attained their LDL-C targets. There exists a significant gap between the guideline recommendations and the real-world evidence in the treatment of dyslipidemia in T2DM.

Background: Boron nitride is a biocompatible and an osteo-inductive material for orthopedic applications. The aim of this study was to evaluate the effects of two different allotrope boron nitride coated implants, cubic boron nitride and hexagonal boron nitride, on fracture healing. Methods: In this experimental study, a total of 24 rats were divided into three groups. Group A was the control group with Kirschner wire without coating, while the wires were coated dominantly by cubic boron nitride in Group B and hexagonal boron nitride in Group C. Then a mid-third femoral fracture was created. The fracture healing was examined in terms of new bone formation with micro-CT analysis and histopathological examination, quantitative measurement of bone turnover metabolites and scintigraphic examination of osteoblastic activity on 28th day post fracture. Results: Micro-CT measurement results revealed a statistically significant increase in bone volume/tissue volume ratio and bone surface values in group B compared to group A. Cortex diameter and osteoblast counts were statistically higher in group B compared to group A. Inflammatory response was increased in group C compared to groups A and B. Biochemical test results showed significantly increased alkaline phosphatase levels and decreased osteocalcin levels in group B compared to group A. The increase in serum phosphorus and decrease in serum calcium levels was statistically significant in group C compared to Group A. Conclusion: Both types of boron nitride coating had superior fracture healing features compared to control group. Therefore, c-BN coating can accelerate the fracture healing and could lead to shorten of union time.

    Paediatric rhinitis and rhinosinusitis

Salcan, Ismail; Bayar Muluk, Nuray; Kopacheva-Barsova, Gabriela

Rhinitis may be defined as an inflammatory condition affecting the upper airways in which rhinorrhoea, nasal congestion or sneezing (or any combination thereof) has been present a minimum of 2 days in a row and lasts for at least 1 h on the majority of those days. The Allergic Rhinitis and Its Impact on Asthma (ARIA) guideline distinguishes two categories of rhinitis: allergic rhinitis (AR) and non-allergic rhinitis (NAR). The differentiating feature between these conditions is whether allergic sensitisation has taken place (i.e. AR) or not (NAR). The pathophysiology of AR depends on IgE, and it is usual for ocular pruritus, indicating conjunctivitis, to be present as an additional presenting feature. Rhinitis frequently occurs in children and adolescents. It is commonplace to dismiss the significant morbidity associated with the condition by viewing rhinitis as a self-limited coryzal illness. However, sternutation, pruritus, watery nasal discharge and nasal congestion are distressing for patients. The presentation in childhood and adolescence may also be atypical, with coughing or snoring. Rhinitis has an adverse effect on biopsychosocial well-being. In this chapter, paediatric rhinitis and rhinosinusitis are reviewed in all aspects.

Relationship between zinc content and carbonic anhydrase activity in blood of anemic pregnant women in Turkey

Göktuğ Kadıoğlu, Berrin; Nalçakan, Aysel; Dilek, Esra

Aims: Carbonic anhydrase (CA) in pregnancy plays an important part in gaseous exchange across the placenta. The aim of our study was to determine the relationship between zinc content and the CA activity in blood of anemic and normal pregnant women in their third trimester, in Turkey. Methods: The patients with hemoglobin values below 11 g/dL were accepted as anemic. CA enzyme activity and zinc levels in erythrocyte of anemic and non-anemic pregnant women were carried out in 27 non-anemic pregnant women and 31 anemic pregnant women. Mean corpuscular volume (MCV) levels of the participants were also tested. Results: Zinc levels were lower in the anemic group compared to the non-anemic group (P = 0.049). Total CA enzyme activity was also lower in the anemic group (P = 0.044). MCV levels were found to be lower in the anemic group (P = 0.007). The decrease in these values was statistically significant (P < 0.05). Conclusion: This is the first study to evaluate the zinc content in red blood cells of anemic pregnant women in the third trimester and its relationship with carbonic anhydrase activity. CA isoenzymes to work healthily, providing iron and zinc supports is important for fetal development.

Genotoxic analysis of nickel–iron oxide in Drosophila

    Nas, Burhan; Çolak, Deniz Altun

It is known that nickel–iron oxide nanocomposite (NiFe2O4NP) is used in many important areas such as modern industry, biomedical applications, magnetic resonance imaging, construction of sensors, targeted drug treatment, and photoelectric devices in our life. In this study, we have carried out a genotoxic evaluation of NiFe2O4NP (30 nm) in Drosophila melanogaster by using the wing somatic mutation and recombination assay. For this purpose, third instar larvae carrying the recessive genes (flr3) and multiple wing hairs (mwh) in their third chromosomes were used. The larvae were fed at concentrations ranging from 25 µg/mL to 200 µg/mL. The genotoxic effects of NiFe2O4NPs were evaluated according to mutant trichomes resulting from genetic changes (mitotic recombination, deletion, point mutation, nondisjunction) on development of the wing imaginal discs. Mutant clone evaluations were performed based on small single spots, large single spots, and twin spots classifications. The results showed that significant increases were observed in the frequency of all spots, indicating that the highest concentration of nanoparticles was able to induce genotoxic activity in the wing spot assay of D. melanogaster.

Objective: Delayed admission of myocardial infarction (MI) patients is an important prognostic factor. In the present nationwide registry (TURKMI-2), we evaluated the treatment delays and outcomes of patients with acute MI during the Covid-19 pandemic and compaired with a recent pre-pandemic registry (TURKMI-1). Methods: The pandemic and pre-pandemic studies were conducted prospectively as 15-day snapshot registries in the same 48 centers. The inclusion criteria for both registries were aged ≥18 years and a final diagnosis of acute MI (AMI) with positive troponin levels. The only difference between the 2 registries was that the pre-pandemic (TURKMI-1) registry (n=1872) included only patients presenting within the first 48 hours after symptom-onset. TURKMI-2 enrolled all consecutive patients (n=1113) presenting with AMI during the pandemic period. Results: A comparison of the patients with acute MI presenting within the 48-hour of symptom-onset in the pre-pandemic and pandemic registries revealed an overall 47.1% decrease in acute MI admissions during the pandemic. Median time from symptom-onset to hospital-arrival increased from 150 min to 185 min in patients with ST elevation MI (STEMI) and 295 min to 419 min in patients presenting with non-STEMI (NSTEMI) (p-values <0.001). Door-to-balloon time was similar in the two periods (37 vs. 40 min, p=0.448). In the pandemic period, percutaneous coronary intervention (PCI) decreased, especially in the NSTEMI group (60.3% vs. 47.4% in NSTEMI, p<0.001; 94.8% vs. 91.1% in STEMI, p=0.013) but the decrease was not significant in STEMI patients admitted within 12 hours of symptom-onset (94.9% vs. 92.1%; p=0.075). In-hospital major adverse cardiac events (MACE) were significantly increased during the pandemic period [4.8% vs. 8.9%; p<0.001; age- and sex-adjusted Odds ratio (95% CI) 1.96 (1.20-3.22) for NSTEMI, p=0.007; and 2.08 (1.38-3.13) for STEMI, p<0.001]. Conclusion: The present comparison of 2 nationwide registries showed a significant delay in treatment of patients presenting with acute MI during the COVID-19 pandemic. Although PCI was performed in a timely fashion, an increase in treatment delay might be responsible for the increased risk of MACE. Public education and establishing COVID-free hospitals are necessary to overcome patients' fear of using healthcare services and mitigate the potential complications of AMI during the pandemic.

Ethnopharmacological studies demonstrated that thymol (Thym) and oleuropein (Ole) have therapeutic potential for gastric ulcers. The molecular mechanism underlying the gastroprotective effects of these compounds have not been elucidated yet especially for their individual and combination use at high dose. Therefore, this study was conducted to explore their gastroprotective mechanisms on indomethacin (Indo)-induced gastric ulcer model. Ole (50,100, 250, and 500 mg/kg) and Thym (50,100, 200, and 500 mg/kg) were orally administered to the rats 10 min before the induction of ulcer with Indo. The combination of 500 mg/kg doses of Ole and Thym were applied. The gastric mucosa was evaluated histopathologically. Moreover, TAC/TOS, tumor necrosis factor-alpha (TNF-α), prostaglandin E2 (PGE2), endothelial nitric oxide synthase (eNOS), and caspase-3 levels were assessed by ELISA and the caspase-3 and TNF-α expressions were quantified by qRT-PCR. Indo-induced histopathological changes while Ole and Thym pretreatment prevented these effects. Unlike the 500 mg/kg dose of Ole treatment, the 500 mg/kg dose of Thym administration enhanced these damages. The decreased TAC, PGE2 levels and increased TOS, eNOS, TNF-α, caspase-3 levels were obtained in Indo group. However, these changes were reversed by Ole and Thym groups except the 500 mg/kg dose of Thym and the combination treatment groups. Similar trends were observed in the caspase-3 and TNF-α expression levels. These results demonstrated that enhanced inflammation, oxidant/antioxidant imbalance, and apoptotic activities were occurred in Indo, 500 mg/kg dose of Thym and the combination treatment groups while not in the other groups. The findings demonstrated the gastroprotective ability of Ole and low doses of Thym in gastric ulcer models.
