Davarcı, İsmail; Samasti, Mustafa; Kuşkucu, Mert Ahmet; Senbayrak, Seniha; Kocoglu, M. Esra; Aksaray, Sebahat

Aim: Carbapenem-resistant Klebsiella pneumoniae infection has become an important clinical problem with reduced therapeutic options. This study aimed to investigate the carbapenem resistancerates and responsible resistance genes in K. pneumoniae isolates derived from clinical samples collected in Istanbul.Materials and Methods: This prospective study included a total of 1452 K. pneumoniae isolates frompatients admitted to our hospital between July 2013 and July 2014. VITEK-2 (bioMérieux, MarcyI’Ѐtoile, France) was used for microbial identification and antimicrobial susceptibility testing. Thecarbapenem-resistant isolates identified by VITEK-2 were also found to be resistant to ertapenemby the ertapenem gradient test. Resistance mechanisms of the carbapenem-resistant isolates wereinvestigated using real time-polymerase chain reaction. Results: Of the 1452 K. pneumoniae isolates, 45 (3.1%) were carbapenem-resistant. Of these, 32(71.1%) were blaOXA-48-positive, 9 (20%) blaNDM-positive, and 1 (2.2%) blaVIM-1-positive. None had thegenes blaKPC and blaIMP-1. The greatest susceptibility by the isolated carbapenemase-producing K.pneumoniae was shown to the antimicrobials amikacin and gentamicin.Discussion and Conclusion: In our hospital, there are several mechanisms causing carbapenem resistance, and the blaOXA-48 positivity rate of 71.1% is significant. This resistance may spread rapidly and, through enzymatic resistance gene transfer, lead to hospital epidemics difficult to manage. For thisreason, accurate and rapid laboratory diagnosis is important in infection control. For faster results, molecular methods, as well as phenotypic methods, must be included in the hospital infrastructure.

Yildirim, Serhat; Özçelik, Merve Berivan; Kandur, Yaşar

Paraoxonase (PON1; E.C.3.1.8.1) is synthesized in the liver and hydrolyzes its specific substrate, paraoxone. It is an enzyme from the group of A-esterases bound to high density lipoprotein (HDL). The aim of this study was to investigate the in vitro effects of sultamicillin, cefdinir, vancomycin hydrochloride and colistimethate sodium active substances on the PON1 enzyme activity in human serum. The PON1 enzyme from the human serum was purified using ammonium sulfate precipitation (in the range of 60-80%), ion exchange chromatography, and gel filtration chromatography techniques approximately 320,7 fold with 44,6% yield. The paraoxonase activity of the enzyme at 412 nm was investigated to investigate the inhibition effects of the active substances. Paraoxone was used as the substrate. Sultamycillin and vancomycin hydrochloride showed noncompetetive inhibition for the PON1 enzyme, whereas cefdinir showed a uncompetitive inhibition and colistimethate sodium showed a competitive inhibition effect. Ki values are determined for sultamycillin as 38.3 mM; for vancomycin hydrochloride as 5.08 mM; for cefdinir as 0.86 mM; and for colistimethate sodium as 17.01 µM. According to the results, it was determined that the most effective inhibitor colistimethate sodium (IC50: 13.59 µM) was found on the PON1 enzyme with 50% reduction in activity at very low concentration.

The Diagnosis of Neuromyelitis Optica by Biopsy in a Patient with Long Spinal Cord Lesion: A Case Report

Öcal, Ruhsen; Tanoglu, Ceyda; Toprak, Munire Kilinc

Neuromyelitis Optica Spectrum Diseases (NMOSD) is an inflammatory demyelinating disease, which is characterized by recurrent optic neuritis, longitudinal long-segment transverse myelitis attacks and Aquaporin-4 antibody positivity. Except NMOSD, long-segment longitudinal transverse myelitis can be also detected in systemic autoimmune diseases, spinal cord tumors, spinal vascular pathologies, Early detection and treatment of NMOSD is very important as the disease may cause severe disability in young population. A 23-year-old female patient was admitted to our clinic with acute paraplegia and urine incontinence. Spinal Magnetic Resonance Imaging showed a densly edematose, contrast enhanced lesion longitudinally extending between C7 and T7 in the spinal cord. As the lesion was reported to be an astrocytoma, a biopsy was performed. However, pathological stainings revelaed no tumor. NMOSD was verified later with Aquaporin-4 positivity. Although longitudinally extending spinal cord lesions are not rare in NMOSD, our patient’s lesion was very edematose and longer than expected that deserved reporting.

Relationship Between C-Reactive Protein to Albumin Ratio and Thrombus Burden in Patients With Acute Coronary Syndrome

Duman, Hakan; Çinier, Goksel; Bakirci, Eftal Murat; Duman, Handan; Simsek, Ziya; Hamur, Hikmet; Degirmenci, Huesnue; Emlek, Nadir

Increased coronary thrombus burden is known to be a strong predictor of adverse cardiovascular (CV) outcomes. C-reactive protein to albumin ratio (CAR) can be used as a surrogate marker of pro-inflammation which is closely related to prothrombotic state. We aimed to evaluate the association between CAR and coronary thrombus burden in patients who presented with acute coronary syndrome (ACS). Patients who presented with ACS and treated with primary percutaneous coronary intervention were included in the study. Patients were divided into 2 groups as high thrombus burden and low thrombus burden. The study population included 347 patients with non-ST-segment elevation myocardial infarction (169 [48.7%]) and ST-segment elevation myocardial infarction (178 [51.3%]). The CAR was significantly higher in patients with higher thrombus burden (24.4 [1.2-30.2] vs 31.9 [2.2-31.3], P < .001). Independent predictors for increased thrombus burden were higher CRP level (odds ratio [OR]: 0.047; 95% confidence interval [CI]: 0.004-0.486; P = .010), lower serum albumin level (OR: 0.057; 95% CI: 0.033-0.990; P = .049), higher CAR (OR: 1.13; 95% CI: 1.03-1.23; P = .008), higher neutrophil-lymphocyte ratio (OR: 1.18; 95% CI: 1.05-1.31; P = .004), and baseline troponin I level (OR: 1.06; 95% CI: 1.01-1.13; P = .017). Novel CAR can be used as a reliable marker for increased coronary thrombus burden that is associated with adverse CV outcomes.

No previous studies have examined the effect of Hippophae rhamnoides fruit extract (HRE), which contains both water and fat soluble vitamins, on cisplatin-induced ototoxicity. Therefore, the aim of the study was to investigate the effects of FIRE on cisplatin-induced ototoxicity in Guinea pigs and to evaluate the biochemical, gene expression and histopathological changes associated with cisplatin use. Experimental animals were divided into three groups: healthy (HG), HRE+cisplatin (HRC) and cisplatin control (CCG) groups. Biochemical, gene expression and histopathological examinations were carried on the removed inner ear tissues. In the HRC group, the oxidant parameter was lower and the anti-oxidant parameter was higher than in the CCG. These results are supported by gene expression levels and histopathological results. The use of FIRE against cisplatin-induced oxidative ototoxicity may be easier, more cost effective and more beneficial than the use of vitamins alone or in combination with each other.

Can, Halil; Dolgun, Muge; Özgen, Utku; Gömleksiz, Cengiz; Diren, Furkan; Aydoseli, Aydin; Aras, Yavuz; Sencer, Altay

Objective: Nowadays due to developing imaging techniques, vestibular schwannoma cases are monitored more frequently. In this study, the results of the patients who underwent surgery in the Department of Neurosurgery, Istanbul University Istanbul School of Medicine were retrospectively evaluated. Methods: Radiological and clinical findings of 18 patients with vestibular schwannoma diagnosed in our clinic from 2012 to 2017 were evaluated. After describing the possible treatment methods of the disease in detail, oral and written informed consent was obtained from all of them. Results: In our study, 18 patients, consisting of 13 women and 5 males, were examined. Eleven patients were found with a mass diameter below 3 cm, and seven patients were found with mass diameter above 3 cm. All patients underwent mass resection with retrosigmoid craniotomy. Total resection was performed in 11 patients, and subtotal resection was done in 7 patients. In seven patients with total resection (63.6%), a newly developed facial paresis was observed. The newly developed glossopharyngeal nerve paresis was observed in three patients (27.2%), and also abducens nerve paresis in three patients (27.2%). The facial nerve was preserved at a rate of 42% in tumors larger than 3 cm in size. The CSF fistula developed in one patient (5.5%); hydrocephalus developed in one patient (5.5%). Conclusion: In cases of vestibular schwannoma, microsurgery, radiosurgery, and wait/follow approaches are monitored. Although gold is standard microsurgery in tumors sized above 3 cm, the treatment plan for tumors under 3 cm in size is controversial. In this study, postoperative facial paresis was developed in all patients with total resection, which were below the mass size of 3 cm. The fact that the abducens and glossopharyngeal nerve paresis in patients with total resection have been seen less frequently in patients with subtotal resection is a factor that can affect the decision of total-subtotal resection. Therefore, the treatment plan should be taken into consideration (total resection/subtotal resection + radiosurgery) considering the existing neuromonitoring facilities in tumors larger than 3 cm in size.

Materials and Methods: Patients who underwent esophagogastroduodenoscopy and taking antrum biopsy for dyspeptic complaints were included in the study between January 2015 and May 2017 at Ordu University Medical Faculty and Ordu State Hospital Endoscopy Units. Biopsy results of the patients were retrospectively analyzed. Results: The number of the patients was 526. 47.7% (n = 251) of the patients were female and 52.3% (n = 275) were male. The mean age of the cases was 53.65 +/- 16.44 years (min 17 -max 93). 68.1% (n: 358) of the patients were Hp positive and 31.9% (n = 168) were Hp negative. In patients; the results of activation (+) in 75.9% (n: 399), intestinal metaplasia in 15.2% (n = 80) (+), glandular atrophy (+) in 8.6% (n = 45) were observed. All biopsies had inflammation. The severity of Inflammation in cases was 47.53% (n = 250) mild, 41.45% (n = 218) moderate, and 11.02% (n = 58) severe. When we evaluate the relationship between inflammation, activation, intestinal metaplasia and glandular atrophy with Hp, it was observed that all the criteria were significantly higher in patients with Hp (+) than Hp (-). There were severe inflammation in 3 (1.78%) of 168 patients with Hp (-) and 55 (15.4%) of 358 patients with Hp (+) (p:0,000). 320 (89.4%) of the 358 patients with Hp (+) and 79 (47%) of the 168 patients with Hp (-) had activation (p:0,000). Intestinal metaplasia was observed in 63 (17.6%) of the 358 patients with Hp (+) and 17 (10.1%) of 168 patients with Hp(-)(p:0,015). Glandular atrophy was observed in 41 (11.45%) of 358 patients with Hp(+) and in 4 (2.38%) of the 168 patients with Hp (-) (p: 0,000). Conclusion: The prevalence of Hp in the community is still high. In patients with Hp (+), activation, inflammation severity, intestinal metaplasia and glandular atrophy rates are higher than those with Hp (-). Intestinal metaplasia and glandular atrophy are precancerous lesions. Early diagnosis and eradication of Hp may prevent the formation of precancerous lesions. Hp eradication can prevent not only stomach cancer but also many stomach diseases at the same time. Early diagnosis and eradication of Hp is important.

Kardas, Ferhat; Cam, Zekeriya; Eskisu, Mustafa; Gelibolu, Sedat

Purpose: The mainstream psychology has mostly focused on the negative factors that threaten human mental health. With the emergence of positive psychology approach, the human strengths have started to be studied and the number of studies on the issue of happiness and well-being has increased. The aim of this research is to reveal the level of prediction for psychological well-being by the variables of gratitude, hope, optimism and life satisfaction. Method: Research sample consisted of 510 students from various faculties and departments of four different universities. The study group consisted of 359 female (70.4%) and 151 male (29.6%) university students. The age range of participants varied between 17 and 30. Findings: All the variables discussed in the study were positively related to each other, and the variables of gratitude, optimism, hope, and life satisfaction all together accounted for about 51% of the variance of psychological well-being. Gratitude was determined as the most predictive variable for well-being and it was followed by the variables; hope, optimism, and life satisfaction, respectively. Accordingly; gratitude predicted 35.4% of the variance in well-being alone; gratitude and hope predicted 45.4% together; gratitude, hope, and optimism predicted 48.8% together; while gratitude, hope, optimism, and life satisfaction predicted 50.7% percent altogether. Implications for Research and Practice: These findings are thought to be important in terms of revealing variables that predict well-being with a high level of variance. It is thought that these findings can be used in the field of mental health especially by the researchers and practitioners using intervention studies. Especially, since the gratitude level accounts for 35.4% of the variance in well-being alone, gratitude interventions can effectively be used to improve well-being level. (C) 2019 Ani Publishing Ltd. Objective: Invasive breast cancer is a heterogeneous disease group that has different biological and pathological features, and characterized with different clinical behavior, treatment results, and consequences. The rarity of most specific neoplasies does not allow large or randomized studies to determine optimal treatment. With this study, we aimed to evaluate the clinical and pathological features and treatment options and results of rare breast carcinoma patients that followed and treated in our clinic. Methods: Files of 1280 breast cancer patients admitted to our center between years 2000 and 2016 were evaluated. From these patients, who have rare breast carcinoma subtypes were included and patients with pathological diagnosis of invasive ductal carcinoma and invasive lobular carcinoma were excluded. Seventy-three patients that included in the study were analyzed retrospectively in terms of age, menopausal status, family history, tumor-lymph node-metastasis stage, histological grade, hormone receptor level, human epidermal growth factor receptor2 expression, operation type and overall survival (OS). Results: Eight histological subtypes of breast tumors were evaluated. Significant difference wasn't found between groups in terms of mean age of the patients. In terms of histological subtypes, the most eldest patients were in papillary group, the youngest patients were in tubular group. The mean tumor diameter was also significantly different between groups (p=0.02). The mean age of the patients diagnosed with tubular and mucinous type breast carcinomas were younger than the one reported in literature and the mean tumor diameter was also larger in these tumors. Ratio of patients diagnosed at advanced stages was high (43%). While mucinous and tubular carcinomas were the histological subtypes with the best prognosis, the shortest OS was in primary breast sarcoma and neuroendocrine carcinoma group. Conclusion: Rare breast cancers are a heterogeneous malignity group with different behavior and prognosis. Our results, except some cases, were generally concordant with the data obtained from other studies. These exceptions may be related to race, regional factors, different pathological evaluation, and environmental factors. Nevertheless, comprehensive clinical studies are required because of the rare occurrence these tumors.
