Kalayci, Mehmet; Kocdor, Mehmet Ali; Kuloglu, Tuncay; Sahin, İbrahim; Sarac, Mehmet; Aksoy, Aziz; Yardim, Meltem; Dalkilic, Semih; Gursu, Onur; Aydin, Suna

Aims The purpose of our investigative work has been to determine whether there can be therapeutic roles in the administration of sildenafil citrate, heparin and several neuropeptides on an animal model where gastric ulcers were induced with acetic acid, and to compare their efficacy. Materials and methods The animals were divided into 13 groups, with 4 animals in each. Gastric ulcers was induced in the animals of 12 groups with one untreated group being left as the control (Group I - control; given normal saline (NS)). The other groups were: Group II (ulcer + NS); Group III (5 mg/kg sildenafil citrate, low dose); Group IV (10 mg/kg sildenafil citrate, high dose); Group V (0.6 mg/kg heparin, low dose); Group VI (6 mg/kg heparin, high dose); Group VII (20 nmol/kg des-acyl ghrelin); Group VIII (40 nmol/kg des-acyl ghrelin); Group IX (4 nmol/kg acyl ghrelin); Group X (8 nmol/kg acly ghrelin); Group XI (20 pmol/kg Nesfatin-1); Group XII (15 nmol/kg Obestatin) and Group XIII (5 nmol/kg Neuropeptide Y). Gastric neuropeptide expression was measured using an immunohistochemical method, and the amount in circulation was detected using ELISA. To compare with no treatment, the controls and other treatment groups, we recorded loss of the surface epithelium of the stomach, erosion, bleeding and inflammatory cell infiltration in the upper halves of the gastric glands. Key findings The muscularis and the layers beneath it were, however, apparently normal. The gastric mucosa healed with little or no inflammation when sildenafil citrate, low dose heparin, ghrelin, NUCB2/Nesfatin-1, obestatin, Neuropeptide Y were administered. Significance Overall the data indicate that low dose heparin, and especially sildenafil citrate and neuropeptides, can be used clinically as an alternative approach in the treatment of the gastric ulcer.

Buyuklu, Mutlu; Kandemir, Fatih Mehmet; Set, Turan; Bakırcı, Eftal Murat; Degirmenci, Husnu; Hamur, Hikmet; Topal, Ergun; Kucukler, Sefa; Turkmen, Kultigin

The aim of study was to determine the effects of ozone therapy on the oxidative stress, cardiac functions and clinical findings in patients with heart failure reduced ejection fraction (HFrEF). A total of 40 patients with New York Heart Association 2 and 3 HF with left ventricular ejection fraction (LVEF) <35%, and 40 subjects without HF as control group were included in the study. Patients with HFrEF were given additional ozone therapy of major and minor administrations along with conventional HF treatment for 5 weeks. Before and after ozone therapy, left ventricular end-systolic and end-diastolic volumes (LVESV, LVEDV) and the 6 minute walk distance (6MWD) and blood levels of the superoxide dismutase (SOD), catalase (CAT), glutathione (GSH), glutathione peroxidase (GSHPx), malondialdehyde (MDA), nitric oxide (NO) and N-terminal pro-brain natriuretic peptide (NT-proBNP) were measured. Ozone therapy significantly reduced the serum levels of NO and MDA (p < 0.001, respectively) and significantly increased the levels of SOD, CAT, GSH and GSHPx (p < 0.001, respectively). LVEDV and LVESV were found to be significantly reduced; however, LVEF was not found to be significantly increased (p = 0.567). As the biochemical improvement marker of HF, NT-proBNP was significantly reduced (p < 0.001). The clinical HF improvement marker of 6 minute walk distance was also modestly increased (p < 0.001). Ozone therapy might be beneficial in terms of activating antioxidant system and merit further therapeutic potential to conventional HF treatment in patients with HFrEF.

Activation of two different drugs used in alzheimer’s disease treatment on human carbonic anhydrase isozymes I and II activity: An in vitro study; [Alzheimer hastalığının tedavisinde kullanılan iki farklı ilacın insan karbonik anhidraz I ve II izoenzim aktiviteleri üzerindeki aktivasyonu: İn vitro çalışma]

    Dilek, Esra

Objectives: Human carbonic anhydrase I and II (hCAI, II) isoenzymes were purified from human erythrocyte. Kinetic interactions between the enzymes and memantine and donepezil, two different drugs used in Alzheimer’s disease (AD) treatment, were investigated. Materials and Methods: The purification procedure was composed of preparation of homogenate (or hemolysate) and affinity chromatography on Sepharose 4B-L-tyrosine-sulfanilamide. Results: Both drug exhibited in vitro activator effects on hCAI and II enzymes activity. Strong activations were found for these compounds: The CA values of memantine and donepezil against hCAI were 0.013 M and 1.8 µM, respectively. The KA values of memantine and donepezil against hCAII were 0.045 M and 3.7 µM, respectively. Conclusion: Since the levels of CA isoenzymes are low in patients with AD or in the older population, increasing activities of these isoenzymes are important for these patients. The effect of these drugs used in AD treatment was thought to be caused by positive changes in the levels of carbonic anhydrase isoenzymes.

Role of circulating tumor cells and cell-free tumor deoxyribonucleic acid fragments as liquid biopsy materials in breast oncology

    Telkoparan Akillilar, Pelin; Yildiz, Gökhan

Breast cancer is the most commonly diagnosed type of cancer among females worldwide. It is also the leading cause of female cancer-related death in developing countries. Though novel biomarkers and new strategies for diagnosis, monitoring course of disease, and treatment of breast cancer have been found, these methods are invasive, costly, labor-intensive, and inadequate to fully gauge treatment response and disease recurrence. Therefore, novel biomarkers with greater sensitivity and specificity, and which are easy to perform are needed in breast cancer oncology. There is growing interest in the potential use of circulating tumor cells and circulating tumor deoxyribonucleic acid fragments in liquid biopsy as non-invasive biopsy materials for early detection of breast cancer, monitoring disease progression, and understanding reasons for treatment resistance. This review is a discussion of current status of utilization of these liquid biopsy materials in breast oncology.

The conversion of carbon dioxide (CO2) and bicarbonate (HCO3 −) to each other is very important for living metabolism. Carbonic anhydrase (CA, E.C.4.2.1.1), a metalloenzyme familly, catalyzes the interconversion of these ions (CO2 and HCO3 −) and are very common in living organisms. In this study, a series of novel 2-amino-3-cyanopyridines supported with some functional groups was synthesized and tested as potential inhibition effects against both cytosolic human CA I and II isoenzymes (hCA I and II) using by Sepharose-4B-l-tyrosine-sulfanilamide affinity chromatography. The structural elucidations of novel 2-amino-3-cyanopyridines were achieved by NMR, IR, and elemental analyses. Ki values of the novel synthesized compounds were found in range of 2.84–112.44 μM against hCA I and 2.56–31.17 μM against hCA II isoenzyme. While compound 7d showed the best inhibition activity against hCA I (Ki: 2.84 μM), the compound 7b demonstrated the best inhibition profile against hCA II isoenzyme (Ki: 2.56 μM).

The current systemic treatments of the various solid tumors involve Cisplatin (CIS)-based chemotherapy. Due to its cytotoxicity, this approach is limited. Moreover, the safety of CIS is only discussed especially in breast and stomach cancers. Therefore, we, for the first time, explored the restorative efficacy of oleuropein (OLE), in stomach and lung injuries induced by CIS. Sprague-Dawley rats were divided into eight groups: control CIS, OLE and CIS + OLE. Single dose of (7 mg/kg) CIS was administered intraperitoneally to CIS and CIS + OLE groups. After 24 h, 50, 100 and 200 mg/kg OLE was given for three consecutive days to OLE and CIS + OLE groups. The 8-OH-dG, total oxidative/antioxidant status (TOS/TAS) and malondialdehyde (MDA) levels were evaluated and histopathological analyses were performed on the studied tissues. The results indicated that CIS significantly increased 8-OH-dG, MDA and TOS levels and caused severe tissue damages. However, high dose of OLE induced a significant decrease in the 8-OH-dG, MDA levels, an increase in TAS levels and it restores CIS-induced tissue damages. We hope that the results of this study will provide an impetus for future studies on novel therapeutic strategies including the protective use of oleuropein in gastric and lung cancers due to chemotherapy.

Oleuropein and rutin protect against 6-OHDA-induced neurotoxicity in PC12 cells through modulation of mitochondrial function and unfolded protein response

Elmazoglu, Zubeyir; Ergin, Volkan; Sahin, Ergin; Kayhan, Handan; Karasu, Cimen

Parkinson's disease (PD) is a highly prevalent neurodegenerative disorder, often associated with oxidative stress-induced transcriptional changes in dopaminergic neurons. Phenolic antioxidants, oleuropein (OLE) and rutin (RUT) have attracted a great interest due to their potential to counteract oxidative protein aggregation and toxicity. This study aimed at examining the effects of OLE and RUT against 6-OHDA-induced stress response in rat pheochromocytoma cells. When differentiated PC12 cells were exposed to oxidative stress composer 6-OHDA (100 μM, 8 h), a decreased mitochondrial membrane potential (ΔΨm) was observed along with a significant loss of cell viability and apoptotic nuclear changes. Exposure to 6-OHDA resulted in unfolded protein response (UPR) in differentiated PC12 cells as evidenced by an increased level of endoplasmic reticulum (ER)-localized transmembrane signal transducer IRE1α, adaptive response proteins ATF-4 and proapoptotic transcription factor CHOP. OLE or RUT pretreatment (24 h) at low doses (1-50 μM) protected the differentiated PC12 cells from 6-OHDA-induced cytotoxicity as assessed by increased viability, improved ΔΨm and inhibited apoptosis, whereas relatively high doses of OLE or RUT (>50 μM) inhibited cell growth and proliferation, indicating a typical hormetic effect. In hormetic doses, OLE and RUT up-regulated 6-OHDA-induced increase in IRE1α, ATF-4 and inhibited CHOP, PERK, BIP and PDI. 6-OHDA-activated XBP1 splicing was also inhibited by OLE or RUT. The presented results suggest that neuroprotection against 6-OHDA-induced oxidative toxicity may be attributable to neurohormetic effects of OLE or RUT at low doses through regulating mitochondrial functions, controlling persistent protein misfolding, activating and/or amplificating the adaptive response-related signaling pathways, leading to UPR prosurvival output.

Noninvasive models to predict liver fibrosis in patients with chronic hepatitis B: A study from Turkey

Korkmaz, Pinar; Demirturk, Nese; Batırel, Ayse; Cem Yardimci, Ahmet; Cagir, Unal; Nemli, Salih Atakan; Korkmaz, Fatime; Akcam, Fusun Zeynep; Barut, Huseyin Sener; Bayrak, Burcu

Background: Manynoninvasive methods, including aspartateaminotransaminase (AST)/alanineaminotransaminase (ALT) ratio (AAR), AST-to-platelet ratio index (APRI), Bonacini cirrhosis discriminant score (CDS), fibrosis-4 (FIB4) index, and age-platelet index (API), have been described to determine the stage of hepatic fibrosis. However, thesemethodsare developed for patients with chronic hepatitisC(CHC)andproduce conflicting results in the prediction of liver fibrosis in patients with chronic hepatitis B (CHB). Objectives: The aim of this study was to evaluate the relationship between 7 noninvasive models, including AAR, APRI, CDS, API, FIB-4, neutrophil-to-lymphocyte ratio (NLR), and red cell distribution width (RDW)-to-platelet ratio (RPR) in patients with CHB. Methods: The study population included all patients with CHB, undergoing liver biopsy to determine HBsAg and HBV DNA positivity in more than 6 months. Results: A total of 2520 treatment-naive CHB patients from 40 different centers were included in the study. In total, 62.6% of the patients were male, and the mean age was 40.60 ± 12.34 years (minimum, 18 years; maximum, 77 years). The Ishak fibrosis score was ≥ 3 in 29.8% of the patients, indicating significant fibrosis. The mean API, APRI, CDS, NLR, FIB4, and RPR scores in the noninvasive models were significantly different between the groups with significant and low fibrosis (P < 0.05). All the noninvasive models (API, APRI, AAR, CDS, NLR, RPR, and FIB4) were found to be significant in the discrimination of cirrhosis (P < 0.05). In the multiple logistic regression analysis, CDS, albumin, alkaline phosphatase (ALP), total bilirubin, neutrophil count, NLR, mean platelet volume (MPV), and FIB4 were independent indices for cirrhosis. Conclusions: In the present study, the role of noninvasive tests in the prediction of liver fibrosis stage and cirrhosis was evaluated in a large cohort of CHB patients. Overall, noninvasive models are gradually becoming more promising. Accordingly, the need for liver biopsy can be reduced with a combination of noninvasive methods in the future.

Karakeçili, Faruk; Çikman, Aytekin; Karagöz, Alper

Objective: We aimed to present clinical and laboratory findings of patients and the molecular analysis of strains isolated in a Salmonella typhimurium outbreak associated with food served at a wedding reception. Methods: A total of 108 patients admitted to the hospital after eating food served at a wedding reception were included in the study. The patients whose symptoms continued even after supportive care were hospitalized for further treatment. Two sets of blood and stool cultures were obtained from all the hospitalized patients, and samples were taken from the implicated food source as well. The VITEK (R) 2 Compact (bioMerieux, Marcy l'Etoile, France) automated microbiology system was used together with conventional methods to identify the isolates and determine their antibiotic susceptibility. Serotyping was performed according to the Kauffmann-White scheme using the slide agglutination method. The molecular epidemiology of the isolates was determined by pulsed-field gel electrophoresis using the XbaI enzyme. Results: All the patients presented with common complaints such as fatigue, abdominal pain, fever, diarrhea, nausea, and vomiting that had started 8 to 16 hours after the consumption of the food served. 45 patients were treated in the hospital whereas 63 patients were followed in an ambulatory setting after 4-6 hours of supportive care. Their hospitalization period varied between 1 and 5 days, and all patients were discharged with full recovery. S. typhimurium was isolated from stool cultures of 14 patients, and two of them were also blood culture-positive. All the isolates revealed a single clonal cluster in molecular analysis. All of them were susceptible to ampicillin, ceftriaxone, and ciprofloxacin, but resistant to amikacin. Conclusions: Despite the developments in the food industry in Turkey and across the world, food-borne disease outbreaks still occur. Hygiene rules should be strictly followed and appropriate inspections should be undertaken in the food sector to prevent food-borne outbreaks.

    Meral, Dursun; Bahar, Hüseyın Hüsnü

The purpose of the study was to determine whether school type and gender affected Problematic Internet Use (PIU) or not, and to specify whether Psychological Well-Being (PWB) and loneliness were significant predictors of PIU. In the study group, there were 242 secondary education students studying at Anatolian High School and Imam Hatip High School. As the data collection tool, Problematic Internet Use Scale-Adolescent (PIUS-A), Psychological Well-Being Scale-Adolescent (PWBS-A) and UCLA Loneliness Scale were used. PIU scores were determined to significantly differ according to gender and school type. Grade- school type common effect and gender-grade-school type common effect were found to be efficient upon PIU scores. Psychological Well-Being (PWB) and loneliness scores were specified to be significant predictors of PIU scores. Whereas PWB scores positively predicted PIU scores, loneliness scores negatively predicted PIU scores.

Lung Cancer Metastasis to the Spine

Aydin, Ahmet Levent; Emel, Erhan; Sasani, Mehdi; Gomleksiz, Cengiz; Oktenoglu, Tunc; Ozer, Ali Fahir

In patients affected by malignant tumors, spinal column metastases are frequent. The incidence varies between 30% and 70%. Lung cancer is the second most frequent cancer invading the spine in women, following breast cancer. All patients are under risk of symptomatic spinal cord compression. The prognosis is poor. Indications for surgery are progressive neurologic deficit, intractable pain, and impending vertebral fracture. Treatment is to preserve neurological functions, promote pain relief and provide functional improvement. Five patients, with symptomatic metastatic spinal cord compression at the thoracic level secondary to lung cancer were operated on. Invasion of the spinal column was from neighbouring lung tissue by direct extension or through segmental arteries. The patients were evaluated regarding neurological status, pain and disability scores, tumor type, level, and surgical procedure. Lung cancer is an aggressive tumor. Patients with symptomatic spinal cord compression must be treated aggressively. With current spinal surgical techniques and a coordinated effort, the life expectancy and quality of these patients are extended.

Diagnostic Value of Adenosine Deaminase Level for the Differential Diagnosis of Tuberculosis and Malignancy in Exudative Lymphocytic Pleurisy

Altinsoy, Bulent; Yalçinsoy, Murat; Ünver, Edhem; Güngör, Sinem; Mihmanli, Aydanur; Akkaya, Esen

Objective: To evaluate the diagnostic performance of adenosine deaminase (ADA) levels in patients with exudative lymphocytic pleurisy for the differential diagnosis of tuberculous pleurisy (TBP) and malignant pleural effusion (MPE). Methods: Data on patients with exudative lymphocytic pleurisy were retrospectively analyzed. The study population comprised 54 patients. Thirty-seven were diagnosed with TBP and 17 were diagnosed with MPE. Results: Significant differences were determined in terms of age and ADA, total protein, albumin, and LDH levels between the TBP and MPE groups. The optimal cut-off value of ADA levels was 35.1 U/L for diagnosing TBP. Sensitivity and specificity were 92% and 100%, respectively. Logistic regression analysis was performed to assess independent variables associated with TBP. Independent predictive factors in the model were ADA (OR: 1.21, 95% CI: 1.06-1.39, p=0.006)], and (OR: 0.92, 95% CI: 0.84-1.00, p=0.052)]. The AUC value by the regression equation was 0.979 (p<0.001). When patients were categorized according to age (<50 ve >= 50), two different cut-off values (> 13.51 and > 35.1) for each age range were found in all, but one, TBP patients. Conclusion: ADA levels are useful for the diagnosis of TBP in cases where pleural biopsy cannot be performed or that are inconclusive in making a diagnosis of TBP. In this group, reducing the conventional cut-off value and/or performing an age-based approach seems to improve the diagnostic performance of ADA levels.
