Hirik, Erkan; Bozkurt, Aliseydi; Karabakan, Mehmet; Onuk, Özkan; Balci, Mustafa Bahadir Can; Aydin, Memduh; Çakan, Murat; Nuhoglu, Baris

Purpose: To evaluate the effect of postoperatively administering a low daily dose of tadalafil on the erectile function of patients who underwent a nerve-sparing radical prostatectomy(NSRP) due to localized prostate cancer (PCa). Materials and Methods: Of 138 patients, who underwent NSRP due to PCa between 2012 and 2014, 55 patients who had not had pre-operative erectile dysfunction (ED) were included in the study. The mean age of the patients was 64 (54-72). On the 15th day after surgery, after ultrasound evaluation, all 55 patients started on a daily dose of 5 mg tadalafil that was continued for 2.5 months. The erectile function of patients was evaluated pre-operatively, post-operatively, and at the 3rd and 6th month after surgery using the International Index of Erectile Function (IIEF-5) test. None of the patients was treated with hormonal therapy or radiotherapy before or after surgery. Results: Three patients were excluded from the study due to the adverse effects of tadalafil and two patients elected to discontinue the treatment. Of the remaining 50 patients whose pre-operative erectile function had been found normal, at 3 months after surgery, 36 (72%) had normal erectile function; of the remaining patients in the study six (12%) presented with mild, two (4%) with moderate, and six (12%) with severe ED. Six months after surgery, 35 patients (70%) had normal erectile function while seven (14%) had mild, three (6%) moderate and five (10%) severe ED. There was no statistically significant difference between the results obtained at the 3rd and 6thmonth follow-up (p > 0.05). Three patients reported adverse effects with tadalafil including flushes in 2 (3.6%) and a headache in 1 (1.8%). Conclusions: The administration of a 5 mg post-operative dose of tadalafil to patients that had undergone a bilateral NSRP was found to have a positive effect on the recovery and maintenance of erectile function. However, there is still a need to investigate a larger series of cases.

The effects of low-dose radiotherapy on fresh osteochondral allografts: An experimental study in rabbits

Gönç, Uğur; çetinkaya, Mehmet; Atabek, Mesut

Objective: The aim of this study was to investigate the effects of low-dose fractionated radiotherapy on cartilage degeneration after distal femoral fresh massive osteochondral allograft transplantation. Methods: Twenty-four New Zealand White rabbits were divided into three groups of 8 rabbits each. All rabbits underwent distal femoral medial condyle fresh massive osteochondral allograft transplantation from California rabbits. The group 1 underwent transplantation without any preliminary process. The group 2 underwent fractionated local radiotherapy of 100 cGy for five days starting on the transplantation day. The group 3 included the rabbits to which the grafts transplanted after radiating in vitro by a single dose radiation of 1500 cGy. The hosts were sacrificed twelve weeks later. Anteroposterior and lateral radiographs were taken. Synovial tissue, cartilaginous tissue, and subchondral bone were assessed histopathologically. Results: Nonunion was present in three cases of group 2 and one of group 3 in which cartilage degeneration was more severe. Synovial hypertrophy and pannus formation were more obvious in nonradiated rabbits. Hypocellularity and necrosis of the subchondral bone were rare in group 2. More cartilage tissue impairment was present in group 3 compared to group 1. Conclusion: In osteochondral massive allograft transplantations, the immune reaction of the host could be precluded with radiotherapy, and the side-effects can be prevented by low-dose fractionated regimen. The total dose of fractionated radiotherapy for an immune suppression should be adjusted not to damage the cartilage tissue, but to avoid articular degeneration in the long term.

Kulhan, Mehmet; Kulhan, Gozde; Nayki, Umit; Nayki, Cenk; Ulug, Pasa; Sipahi, Mehmet; Yildirim, Yusuf

Objectives: We aimed to evaluate and compare the clinical and pathological features, diagnosis, treatment, and prognosis of uterine papillary serous carcinoma (UPSC) and clear-cell carcinoma (CC). Material and methods: Thirty-four patients who were operated on for UPSC and CC carcinoma at the Izmir Tepecik Research and Teaching Hospital, between January 1983 and December 2014, were included. Patients were evaluated for the following factors: Age, gravidity, parity, preoperative CA-125, tumor size, myometrial and lymphovascular invasion, lymph node tumor metastasis, presence of atypical cells in peritoneal cytology, pathology results, operation types, whether or not they received postoperative adjuvant therapy, prognosis, and death rate. Results: The stage of the disease was the main factor affecting disease-free and the overall survival. Cisplatin-based chemotherapy was partially effective in patients with postoperative recurrence, while postoperative adjuvant radiotherapy proved to be more effective in preventing relapse. Conclusion: The studied parameters generally were concordant with the literature but, due to the relatively small sample size, more comprehensive and multicenter studies are needed to generate valid results.

Baygeldi, Serdar; Karakose, Oktay; Özcelik, Kazim Caglar; Pülat, Hüseyin; Damar, Sedat; Eken, Hüseyin; Zihni, Ismail; Çalta, Alpaslan Fedai; Baç, Bilsel

Background and Aim. The aim of this study was to investigate the effects of demographic characteristics, biochemical parameters, amount of blood transfusion, and trauma scores on morbidity in patients with solid organ injury following trauma. Material and Method. One hundred nine patients with solid organ injury due to abdominal trauma during January 2005 and October 2015 were examined retrospectively in the General Surgery Department of Dicle University Medical Faculty. Patients' age, gender, trauma interval time, vital status (heart rate, arterial tension, and respiratory rate), hematocrit (HCT) value, serum area aminotransferase (ALT) and aspartate aminotransferase (AST) values, presence of free abdominal fluid in USG, trauma mechanism, extra-abdominal system injuries, injured solid organs and their number, degree of injury in abdominal CT, number of blood transfusions, duration of hospital stay, time of operation (for those undergoing operation), trauma scores (ISS, RTS, Glasgow coma scale, and TRISS), and causes of morbidity and mortality were examined. In posttraumatic follow-up period, intra-abdominal hematoma infection, emboli, catheter infection, and deep vein thrombosis were monitored as factors of morbidity. Results. One hundred nine patients were followed up and treated due to isolated solid organ injury following abdominal trauma. There were 81 males (74.3%) and 28 females (25.7%), and the mean age was 37.6 ± 18.28 (15-78) years. When examining the mechanism of abdominal trauma in patients, the following results were obtained: 58 (53.3%) traffic accidents (22 out-vehicle and 36 in-vehicle), 27 (24.7%) falling from a height, 14 (12.9%) assaults, 5 (4.5%) sharp object injuries, and 5 (4.5%) gunshot injuries. When evaluating 69 liver injuries scaled by CT the following was detected: 14 (20.3%) of grade I, 32 (46.4%) of grade II, 22 (31.8%) of grade III, and 1 (1.5%) of grade IV. In 63 spleen injuries scaled by CT the following was present: grade I in 21 (33.3%), grade II in 27 (42.9%), grade III in 11 (17.5%), and grade IV in 4 (6.3%). The mean length of hospital stay after trauma was 6.46 days in the medically followed patients. This ratio was 8.13 days in 22 patients with morbidity and 5.98 days in 78 patients without morbidity. There was a morbidity in 22 (22%) patients medically followed after trauma. In this study, nonoperative treatment was observed to be performed safely in solid organ injuries after trauma in case of absence of hemodynamic stability and peritoneal irritation. It has been emphasized that injury of both liver and spleen (p < 0.01), high respiratory rate (p < 0.01), trauma scores (GKS, ISS, RTS) (p < 0.0001), and elevation of ALT AST values (p < 0.01) are stimulants for morbidity that may occur during follow-up. Conclusion. Medical follow-up can be considered in patients with high grade injuries similar to patients with low-grade solid organ injury after trauma. The injury of both liver and spleen, high respiratory rate, high GCS and ISS, low RTS, and elevation of ALT AST values were found to increase morbidity again in the follow-up of these patients.

Suna, Kabil Kucur; Ilay, Gozukara; Aysenur, Aksoy; Han, Gozukara Kerem; Ulku, Uludag Eda; Pasa, Ulug; Fatma, Cengiz

Sexual dysfunction is common in women with infertility and interferes with the marital relationship. The study aims to compare sexual function among women with different infertility causes. The authors used a cross-sectional study design with 142 infertile women. Sexual functions and depression prevalence of infertile women were assessed using the Female Sexual Function Index and the Beck Depression Inventory. The prevalence of female sexual dysfunction was 43.3% (n = 13) in the female infertility group (Group A), 54.8% (n = 17) in the male infertility group (Group B), and 51.9% (n = 42) in the unexplained infertility group (Group C). There was no statistically significant difference in the prevalence of sexual dysfunction between the groups (p =.635). There was no significant difference in mean Beck Depression Inventory scores between the groups (p =.832). However, Beck Depression Inventory scores and depression prevalence were significantly higher in women with sexual dysfunction (p <.001). The total Female Sexual Function Index score and all subgroup scores were inversely correlated with the Beck Depression Inventory score (p <.01). Infertile women with sexual dysfunction were more likely to have depressive symptoms. Psychiatric assessment should be introduced in the management of infertility.

Apoptosis, autophagy & endoplasmic reticulum stress in diabetes mellitus

Demirtas, Levent; Guclu, Aydin; Erdur, Fatih Mehmet; Akbas, Emin Murat; Ozcicek, Adalet; Onk, Didem; Turkmen, Kultigin

The prevalence of diabetes mellitus (DM) is increasing secondary to increased consumption of food and decreased physical activity worldwide. Hyperglycaemia, insulin resistance and hypertrophy of pancreatic beta cells occur in the early phase of diabetes. However, with the progression of diabetes, dysfunction and loss of beta cells occur in both types 1 and 2 DM. Programmed cell death also named apoptosis is found to be associated with diabetes, and apoptosis of beta cells might be the main mechanism of relative insulin deficiency in DM. Autophagic cell death and apoptosis are not entirely distinct programmed cell death mechanisms and share many of the regulator proteins. These processes can occur in both physiologic and pathologic conditions including DM. Besides these two important pathways, endoplasmic reticulum (ER) also acts as a cell sensor to monitor and maintain cellular homeostasis. ER stress has been found to be associated with autophagy and apoptosis. This review was aimed to describe the interactions between apoptosis, autophagy and ER stress pathways in DM.

Xanthine oxidase, adenosine deaminase and vitamin e levels in patients with schizophrenia; [Şizofreni hastalarında ksantin oksidaz, adenozin deaminaz ve E vitamini düzeyleri]

Kiliçgün, Hasan; Erşan, Etem Erdal; Bakir, Sevtap; Erşan, Serpil; Koç, Derya; Demirpençe, Özlem; Doğan, Kübra

Objective: Neuronal damage caused by free radicals is believed to be effective in pathogenesis of several psychi-atric disorders. This belief is due to the toxic effects of free radicals that play a role in oxidative stress. Considering that the brain is one of the most sensitive organs to the oxidative damage, the importance of oxidative stress in psychiatric disorders will become more apparent. Additionally, high oxygen use in the brain, and its structure rich in lipid, which is one of the most sensitive molecules to the free radical damage, and its having the average antioxi-dant system yield support oxidative stress theory in the pathogenesis of psychiatric disorders. This study aimed to determine xanthine oxidase (XO), adenosine deaminase (ADA) and vitamins E levels in patients with schizophrenia and control groups, and to investigate the relationship between schizophrenia and the parameters by comparing the measured parameters with each other. Methods: Our study sample included 30 patients diagnosed with schizo-phrenia. The control group consisted of 30 healthy volunteers matched by sex with similar age and smoking habits. In the patient group and the control group, adenosine deaminase, xanthine oxidase, and vitamin E were measured manually using spectrophotometric methods. Results: Serum xanthine oxidase levels in the schizophrenic group were significantly higher than the control group levels. Serum vitamin E and adenosine deaminase levels in the schizophrenia group were significantly lower than the levels of the control group. Discussion and Conclusion: The fact that mechanism of schizophrenia pathogenesis which has a wide variety of clinical symptoms and a dis-ease process is yet to be elucidated reveals the importance of this kind of studies. In this study, low levels of antioxidant vitamin E and adenosine deaminase, and high levels of xanthine oxidase suggest that oxidative stress-mediated neuronal damage may play a role in the pathogenesis of schizophrenia. Therefore, we believe that further research with larger sample groups should be conducted.

Melatonin Attenuates Contrast-Induced Nephropathy in Diabetic Rats: The Role of Interleukin-33 and Oxidative Stress

Onk, Didem; Onk, Oruc Alper; Turkmen, Kultigin; Erol, Huseyin Serkan; Ayazoglu, Tulin Akarsu; Keles, Osman Nuri; Halici, Mesut; Topal, Ergun

Background. Inflammation and oxidative stress (OxS) contribute to the pathogenesis of diabetic kidney disease (DKD) and contrast-induced nephropathy (CIN). Patients with DKD were found to be more prone to CIN. Interleukin-33 (IL-33) is a proinflammatory cytokine, but its role in DKD and CIN is unknown. Methods. Thirty male Sprague-Dawley rats were enrolled. The first group was comprised of healthy rats (HRs), whereas the other four groups were made up of diabetic rats (DRs), diabetic rats with contrast-induced nephropathy (CIN + DRs), melatonin-treated diabetic rats (MTDRs), and melatonin-treated CIN + DRs (MTCIN + DRs). All groups except the HRs received 50 mg/kg/day streptozotocin (STZ). CIN + DRs were constituted by administrating 1.5 mg/kg of intravenous radiocontrast dye on the 35th day. MTDRs and MTCIN + DRs were given 20 mg/kg/day of intraperitoneal injection of melatonin (MT) from the 28th day for the constitutive seven days. Results. We observed increased IL-33 in the kidney tissue following induction of CIN in DRs. To determine whether MT is effective in preventing CIN, we administered MT in CIN + DRs and demonstrated that kidney tissue levels of OxS markers, inflammatory cytokines, and IL-33 were significantly diminished in MTCIN + DRs compared with other groups without MT treatment (p<0.05). Conclusion. Inhibition of IL-33 with MT provides therapeutic potential in DKD with CIN.

Ozcicek, Adalet; Cetin, Nihal; Keskin Cimen, Ferda; Tumkaya, Levent; Malkoc, Ismail; Gulaboglu, Mine; Yarali, Oguzhan; Suleyman, Bahadir

Objective: The aim of this study was to assess the impact of resveratrol (RST) on oxidative stress induced by methotrexate in rat ileum tissue. Materials and Methods: Twenty-four rats were divided into 4 groups with 6 in each group. Each rat was orally administered the following every day for 30 days: group 1 (MTXG), methotrexate (MTX; 5 mg/kg); group 2 (RMTXG), MTX (5 mg/kg) plus RST (25 mg/kg/day); group 3 (RSTG), RST alone (25 mg/kg/day), and group 4 (controls), distilled water. After the rats had been sacrified, the ilea were removed for the assessment of malondialdehyde (MDA), total glutathione (tGSH) and glutathione peroxidase (GSH-Px). Gene expression analyses for interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α) and myeloperoxidase (MPO) were also performed. Hematoxylin and eosin-stained paraffin-embedded sections of the ileum were analyzed under a light microscope and the findings were recorded. Statistical analyses of the data were performed using one-way ANOVA. Results: The administration of MTX in group 1 yielded a higher level of MDA (8.33 ± 2.5 μmol/g protein, p < 0.001) and lower levels of tGSH (0.97 ± 0.29 nmol/g protein) and GSH-Px (5.22 ± 0.35 U/g protein, p < 0.001) compared to the other groups. MTX also increased IL-1β (40.33 ± 5.43 gene expression levels), TNF-α (6.08 ± 0.59) and MPO gene expression (9 ± 1.41) in group 1 compared to the controls (11.33 ± 2.07, 2.15 ± 0.33 and 3.43 ± 0.48, respectively, p < 0.001). The impact of RST on IL-1β, TNF-α and MPO gene expression induced by MTX was observed as a reversal of these findings (p < 0.05). Severe inflammation, damage to the villus epithelium and crypt necrosis was observed histopathologically in the MTXG group, whereas only mild inflammation was seen in the RMTXG group. Conclusion: In this study, ileal damage caused by MTX was inhibited by RST.

A rare case of isolated cystic hydatid of thyroid gland

Eken, Huseyin; Isik, Arda; Balci, Gulhan; Firat, Deniz; Cimen, Orhan; Soyturk, Mehmet

Hydatid cysts are most frequently localized within the liver and lungs, although they can also be found in highly vascularized tissues such as the brain, muscle, heart, pancreas, adrenal, and thyroid glands. A 65-year-old female patient was admitted to our clinic with complaints of a progressively growing mass that was compressing the surrounding tissues and causing respiratory distress. The pathological result was obtained as cytic hydatid. In patients with diagnosed hydatid cysts in the liver, systemic evaluation is necessary to rule out involvement of other organs. Among patients presenting with growths located in the neck, primary hydatid cyst of the thyroid gland must be considered in endemic regions.

Immediate implant placement in fresh sockets versus implant placement in healed bone for full-arch fixed prostheses with conventional loading

    Altintas, N.Y.; Taskesen, F.; Bagis, B.; Baltacioglu, E.; Cezairli, B.; Senel, F.C.

This retrospective study assessed the success of immediate and non-immediate implants installed in patients undergoing planned extraction of all remaining teeth and rehabilitation with implant-supported full fixed prostheses. Patients in need of dental implants for full fixed prostheses to replace teeth extracted in the maxilla and mandible were included in this study. Dental implants were installed in the same surgical procedure, immediately at the extraction site, or in healed bone. Implant success, complications, and failures were recorded during follow-up. Forty-one patients with 512 implants were included in the study. Healing progressed uneventfully for 501 installed implants, but nine implants were lost in the non-immediate group and two were lost in the immediate group, during a mean follow-up of 44.9 months. All failures in both groups were observed in the maxilla. The success rate was the same in both groups, at 97.8%. This retrospective analysis showed that with thorough patient evaluation, the extraction of all residual teeth and implant installation in a single surgical procedure is a safe and predictable treatment modality for the successful rehabilitation of the edentulous patient with a fixed prosthesis.

Background: We sought to investigate the effect of morphine and fentanyl on shivering when used adjunctively with bupivacaine during spinal anesthesia in patients undergoing varicose vein surgery on an outpatient basis. Material/Methods: The study included a total of 90 patients, aged 25-45 years, ASA I-II, scheduled to undergo endovenous laser ablation under spinal anesthesia for lower extremity venous insufficiency/varicose vein disease. Patients were randomly allocated into 3 groups: Group M (morphine group) received 5 mg 0.5% hyperbaric bupivacaine + 0.1 mg morphine, Group F (fentanyl group) received 5 mg 0.5% hyperbaric bupivacaine + 25 μg fentanyl, and Group C (control group) received 5 mg 0.5% hyperbaric bupivacaine + physiologic saline. The level of sensory blockade was assessed with pin-prick test and the level of motor blockade was assessed with Bromage scale at 5-min intervals. Shivering grade and time to first postoperative analgesic requirement was recorded. Results: Level and time of sensory block showed a slight but insignificant increase in the Morphine Group and Fentanyl Group. Time of postoperative analgesic requirement was significantly longer in patients who received morphine (p<0.05). Shivering was significantly less common in patients who received morphine and fentanyl than in patients who are in the Control Group (p<0.02). Conclusions: Morphine or fentanyl may be used as adjunctives to spinal anesthesia to prevent shivering in patients undergoing venous surgery.

Shouldice herniorrhaphy - Still an effective technique: A retrospective cohort study

    Karakose, Oktay; Eken, Huseyin

Aim Only 4.8% of general surgeons in our country prefer Shouldice herniorrhaphy in primary inguinal hernia repair. In the evaluation of postoperative complications, this technique, which has the closest results to Lichtenstein herniorrhaphy, could be the preferred method in selected cases of primary inguinal hernia repair. It does not only provide an alternative to the surgeon, but it can also be the first choice in cases where mesh repair cannot be applied. In this study, we aimed to evaluate the efficacy of Shouldice herniorrhaphy in patients who got the primary inguinal hernia diagnosis, comparing Lichtenstein herniorrhaphy. Patients and method Our study was done two-centred. We followed 48 patients who were operated with Lichtenstein technique for a diagnosis of primary inguinal hernia at the General Surgery Clinic of Erzincan Mengücek Gazi Training and Research Hospital between February 2011 and February 2014 and 42 patients who were applied Shouldice technique at Bulancak State Hospital General Surgery Department between January 2010 and February 2013. The patients were evaluated in respect of age, gender, hernia area, hernia type and early or late complications. Findings In respect of early complications, urine retention developed in only 2 (4.7%) patients of the Shouldice group. In the Lichtenstein group, complications developed in a total of 3 (6.3%) patients, as superficial infection was observed in 1 patient, seroma in 1 and urinary retention in 1. When we searched late-stage complications of the patients, paraesthesia developed in the inguinal area of 1 (2.3%) patient in the Shouldice group, and neuralgia developed in 2 (4.1%) patients of the Lichtenstein group. No recurrence was determined in any of the patients. Conclusion In the repair of inguinal hernia, surgeons should not be tied to just one method. Shouldice herniorrhaphy is a good technique that could be the first choice when the use of mesh is not possible and could be an alternative to Lichtenstein herniorrhaphy when done appropriately. In surgical training, it should be considered after the Lichtenstein method in primary inguinal hernia repair.

The prevalence of femoroacetabular impingement as an aetiologic factor for end-stage degenerative osteoarthritis of the hip joint: Analysis of 1,000 cases

Oner, Ali; Koksal, Alper; Sofu, Hakan; Aykut, Umit S.; Yıldırım, Timur; Kaygusuz, Mehmet A.

Objective: The main purpose of the present study was to determine the prevalence of FAI among patients who underwent total hip arthroplasty (THA) with the diagnosis of end-stage osteoarthritis (OA) of the hip joint in a tertiary referral centre. Design: A retrospective search of the institutional archive database for patients who had undergone THA between the years 2005 and 2010 was conducted with the analysis of the recorded radiographic images. A total of 1,004 patients including 690 (68.7%) females and 314 (31.3%) males were identified. All radiographs were reviewed by 3 of the authors. Intra- and interobserver consistencies were calculated. The prevalence of femeroacetabular impingement (FAI) as a predisposing diagnosis resulting in end-stage degenerative joint disease treated with THA was determined. Results: The most common diagnosis was developmental dysplasia of the hip (DDH). The predisposing aetiologic factor leading to end-stage degenerative hip disease treated with THA was undetermined for 26 patients (2.6%). The number of patients who were diagnosed as cam-type FAI by all of the observers was 83, whereas it was 16 for pincer-type FAI. Interobserver agreement was very high in the evaluation of the stated ethiologies of hip osteoarthritis. The contingency coefficient for the diagnosis of FAI was 0.71 between observers. Conclusions: Our study may be a reference for further investigations to develop a better understanding of the underlying pathological factors in patients undergoing THA due to end-stage OA of the hip joint.

ApaL1 urokinase and Taq1 vitamin D receptor gene polymorphisms in first-stone formers, recurrent stone formers, and controls in a Caucasian population

Aykan, Serdar; Tuken, Murat; Gunes, Sezgin; Akin, Yigit; Ozturk, Murat; Seyhan, Serkan; Yuruk, Emrah; Temiz, Mustafa Zafer; Yılmaz, Ali Faik; Nguyen, Daniel P.

The purpose of this study was to determine differences in genotype distribution and allele frequency of urokinase and vitamin D receptor (VDR) single nucleotide polymorphisms (SNPs) between first-stone formers, recurrent stone formers, and controls in a Caucasian population. A total of 86 first-stone formers, 78 recurrent stone formers, and 167 controls were included. Urokinase and VDR SNPs were tested by gene amplification followed by ApaL1 and Taq1 endonuclease digestion, respectively. Baseline variables, genotype, and allele frequencies were compared between the three groups, using descriptive statistics. Adjusted odds ratios were calculated to estimate the risk for recurrent urolithiasis associated with genotypes. We found that differences in the distribution of ApaL1 SNP and Taq1 SNP genotypes were statistically different between recurrent stone formers and first-stone formers, and between recurrent stone formers and controls. Allele frequency analysis showed that the T allele for ApaL1 SNP and the C allele for Taq1 SNP were significantly associated with recurrent urolithiasis. For Taq1 SNP, logistic regression analysis showed that the C/C genotype was associated with a more than threefold higher risk for recurrent urolithiasis. We conclude that ApaL1 and Taq1 SNPs of the urokinase and VDR genes are associated with recurrent urolithiasis in a Caucasian population.

Assessment of smoking for low bone mineral density in postmenopausal Turkish women

Ugurlu, Ugur; Nayki, Umit; Nayki, Cenk; Ulug, Pasa; Kulhan, Mehmet; Yildirim, Yusuf

Objective: To investigate the effect of cigarette smoking on bone mineral density (BMD) in postmenopausal Turkish women. Study design: A total of 175 postmenopausal Turkish women admitted to Tepecik Training and Research Hospital for a routine menopausal checkup were included in this study. All participants completed a questionnaire regarding their age, educational status, parity, number of abortus, time since menopause, caffeine intake, tobacco use, alcohol consumption, history of a previous fracture related to trauma, and taking any medication for menopause and osteoporosis. Of all subjects, 23.3 % (n = 39) were smokers and 77.7 % (n = 136) were nonsmokers. T-scores and Z-scores of vertebra and femur neck were assessed using dual energy X-ray absorptiometry (DXA). For analyzing the results, Student t-test, one-way ANOVA, Mann–Whitney U test, Pearson correlation, and Kruskal–Wallis test were performed. Results: Femur T-scores (− 0.78 ± 1.07 vs. − 0.32 ± 1.56) and vertebra T-scores (− 2.26 ± 1.23 vs. − 1.82 ± 1.04) were significantly lower in smoking women than nonsmoking women (p < 0.05). However, there were no significant difference between duration of smoking, number of cigarettes consumed per day, and BMD (p > 0.05) Conclusion: Cigarette smoking is one of the modifiable risk factors influencing bone density in postmenopausal Turkish women. “Cessation of cigarette” should be recommended for lifestyle modifications to prevent postmenopausal osteoporosis.

Background In this study, the effects of levosimendan used in the treatment of acute congestive heart failure upon pulmonary fibrosis in rats induced with bleomycin (BL) were analyzed. Methods A total of 33 male Sprague-Dawley type rats were categorized into five groups randomly. About 2.5 U/kg BL was intratracheally administered to the rats in the BL, BL + L1, BL + L2, and BL + L3 groups, and 0.9% saline was intratracheally administered at the same rate to the control group. 0.3, 1, and 3 mg/kg levosimendan was intraperitoneally administered to the BL + L1, BL + L2, and BL + L3 groups, respectively. Blood and tissue samples were taken from the rats euthanized to determine the changes in erythrocyte enzyme activities and to conduct histopathological evaluations after 14 days. With values between 0 and 3, histopathological scoring damage was assessed by the presence of inflammation and fibrosis in a semiquantitative manner. Results Compared with those in the C group, glutathione reductase (GR) and Catalase (CAT) enzymes decreased in the BL group; compared with that in the BL group, GR increased in the BL + L1 and BL + L3 groups, 6-phosphogluconate dehydrogenase (6PGD) increased in the BL + L3 group, and CAT increased in the BL + L2 and BL + L3 groups (p < 0.05). In the histopathological evaluation, fibrosis occurred in all rats in the BL group, and tissue damage was noticed to be generally less in the BL + L1, BL + L2, and BL + L3 groups (p < 0.001). Conclusions The results obtained from biochemical and histopathological evaluations indicate that levosimendan had an anti-fibrotic effect without a dose-dependent response on pulmonary fibrosis.

Background: The prognostic value of blood culture testing in the diagnosis of bacteremia is limited by contamination. Objectives: In this multicenter study, the aim was to evaluate the contamination rates of blood cultures as well as the parameters that affect the culture results. Materials and Methods: Sample collection practices and culture data obtained from 16 university/research hospitals were retrospectively evaluated. A total of 214,340 blood samples from 43,254 patients admitted to the centers in 2013 were included in this study. The blood culture results were evaluated based on the three phases of laboratory testing: the pre-analytic, the analytic, and the post-analytic phase. Results: Blood samples were obtained from the patients through either the peripheral venous route (64%) or an intravascular catheter (36%). Povidone-iodine (60%) or alcohol (40%) was applied to disinfect the skin. Of the 16 centers, 62.5% have no dedicated phlebotomy team, 68.7% employed a blood culture system, 86.7% conducted additional studies with pediatric bottles, and 43.7% with anaerobic bottles. One center maintained a blood culture quality control study. The average growth rate in the bottles of blood cultures during the defined period (1259 -26,400/year) was 32.3%. Of the growing microorganisms, 67% were causative agents, while 33% were contaminants. The contamination rates of the centers ranged from 1% to 17%. The average growth time for the causative bacteria was 21.4 hours, while it was 36.3 hours for the contaminant bacteria. The most commonly isolated pathogens were Escherichia coli (22.45%) and coagulase-negative staphylococci (CoNS) (20.11%). Further, the most frequently identified contaminant bacteria were CoNS (44.04%). Conclusions: The high contamination rates were remarkable in this study. We suggest that the hospitals’ staff should be better trained in blood sample collection and processing. Sterile glove usage, alcohol usage for disinfection, the presence of a phlebotomy team, and quality control studies may all contribute to decreasing the contamination rates. Health policy makers should therefore provide the necessary financial support to obtain the required materials and equipment.

Comparison of open reduction alone and open reduction plus Dega osteotomy in developmental dysplasia of the hip

In the treatment of developmental dysplasia of the hip, concentric reduction of the femoral head into the acetabular cavity plays a key role in the natural development of the acetabulum. However, there is still debate on the need for additional acetabular osteotomies and their timing. In this study, we compared open reduction (OR) alone with OR plus Dega osteotomy for acetabular index (AI) development. Twenty patients, 10 in each group, who underwent either OR alone or OR plus Dega osteotomy were studied retrospectively. All patients were diagnosed with developmental dysplasia of the hip and none of them received treatment previously. Preoperative, early postoperative, and follow-up radiographs were gathered, radiological grading was performed according to Tönnis, and AI angles were measured. The OR group had a mean age of 24.5 months, whereas the Dega group had a mean age of 24.8 months. The mean follow-up period of the OR group and Dega group was 57.8 and 66.6 months, respectively. In the OR group, the initial mean AI was 37.5° whereas in the Dega group it was 46°. After the follow-up, despite the acetabular development in the OR group being twice as fast, the final mean AI was 25.5, whereas it was 15.9 in the Dega group. The difference was statistically significant (P<0.05). Using Tönnis' definition, the Dega group ended up with 70% normal hips, 20% mild dysplasia, and 10% severe dysplasia, whereas the OR group had 20% normal hips, 30% mild dysplasia, and 50% severe dysplasia at the final visit. There was no correlation between the initial radiological grading of dislocation and the final result. OR plus Dega osteotomy is a good option to regain acetabular coverage over the femoral head. It provides better radiographic results after a 5-year follow-up period in patients with a mean age of 25 months. OR alone should not be performed unless the child had mildly dysplastic acetabulum according to Tönnis' definition. Level of evidence: Level III, retrospective comparative study.
