Torun, Mumtaz Taner; Seçkin, Ender; Tuncel, Umit; Kiliç, Caner; Özkan, Ozalkan
Background. Cysts in the larynx are rare and generally asymptomatic. However, large cysts in adults can be symptomatic. If they are symptomatic, they typically present with respiratory and feeding difficulties. They are usually benign in terms of pathology. Several surgical techniques may be used for treatment. Case Report. A 56-year-old man presented to our clinic with hoarseness. Routine laryngeal examination revealed a giantmass and the larynx could not be visualized. At magnetic resonance imaging (MRI), a cystic mass originating from the vallecula was detected. There was no pathology at the glottic level. We planned tracheotomy for the airway and endoscopic surgery for excision. The mass was excised using CO2 laser and was reported as benign. Conclusion. An asymptomatic vallecular cyst may cause difficult intubation in any operation. It may also cause respiratory or other complications. Airway management should be led by an ear, nose, and throat surgeon, since tracheotomy may be required. Endoscopic excision with CO2 laser is a good choice for treatment in elective cases. In this report, we discuss the diagnosis and treatment of a patient with an asymptomatic giant vallecular cyst.

Objective: Invasive surgical interventions creating acute stress may give rise to immunological and hormonal disturbances in patients undergoing surgery. Previous studies reported elevations in serum prostate-specific antigen (PSA) level following coronary bypass and myocardial infarction. The current study aims to investigate the effect of surgical stress on serum PSA level. Methods: Thirty-three patients who underwent open renal stone surgery, without any known infectious prostatic disorder or prostate cancer that may elevate serum PSA level, were included in the study. In all patients, serum levels of total and free PSA (tPSA and fPSA), follicle-stimulating hormone (FSH), luteinizing hormone (LH), total and free testosterone, C-reactive protein (CRP), fibrinogen, albumin, and ferritin and erythrocyte sedimentation rate (ESR) were measured both at baseline and post-operative day 3. Wilcoxon signed-rank test was used for the statistical analysis. Results: The mean age of patients was 53.9 +/- 8 (40-66). The mean baseline and post-operative levels of the measured serum parameters are given in Table 1. All of them differed significantly between baseline and post-operative day 3, except for LH, tPSA, and fPSA. CRP, fibrinogen, ferritin, and ESR are known as acute phase reactants of the post-operative endocrine response. However, serum total and free testosterone levels, albumin, and FSH decreased on the contrary acute phase reactants in the present study. Conclusion: According to our results, serum PSA level was not affected by acute surgical stress.
- Review ArticleAçık Erişim2015Extracorporeal Life Support for Patients with Acute Respiratory Distress Syndrome: Review
Ayazoglu, Tulin Akarsu; Onk, Didem
Patients with severe acute respiratory distress syndrome (ARDS) is an acute diffuse, inflammatory lung injury, leading to increased pulmonary vascular permeability with hypoxemia and bilateral radiographic opacities, associated with decreased lung compliance. Extracorporeal membrane oxygenation (ECMO) has been used to support primary or secondary diseases causing respiratory or cardiac failures in newborns, children and adults. Patients with severe ARDS are candidates for ECMO therapy. ECMO is a support modality, not a treatment; it is only beneficial in patients whose primary disease is reversible. ECMO complications-which can lead to mortality, morbidity, long-term disability and reduced quality of life-include surgical and organ bleeding, renal and multiorgan failure and central nervous system problems. The aim of this article was to provide a general overview of ECMO use and outcomes patients with severe acute respiratory distress syndrom.

Akbas, Emin Murat; Güngör, Adem; Arslan, Volkan
Metformin-associated lactic acidosis in a diabetic patient is a rare and severe complication. The severity of the condition is associated with the underlying disease. The diagnosis may be delayed due to its rarity and the fact that its symptoms and signs are frequently confused with other pathologies. In such cases, generally, the underlying renal disease helps to establish the diagnosis. In this paper, we present the case of lactic acidosis in a patient without a serious underlying disease who was using metformin, ramipril and spironolacton for diabetes and hypertension. The level of blood creatinine was found to be 1.4 mg/dL. It should be kept in mind that metformin can cause life-threatening lactic acidosis, especially in patients using renin-angiotensin-aldosterone system blockers, even without a serious deterioration in renal functions.

Karakeçili, Faruk; Akin, Hicran; Pekgöz, Murat; Mistik, Resit; Oral, Haluk Barboros; Akalin, Halis
Chronic granulomatous disease (CGD) is a rare, genetically heterogeneous disease characterized by severe infections that can be life threatening. The disease develops due to defects in nicotinamide adenine dinucleotide phosphate (NADPH) oxidase system. It is characterized by granuloma formation depending on recurrent infections and increased inflammatory response. Lung, skin, lymph nodes, and liver are the most involved organs. Nitroblue tetrazolium test (NBT) is the most frequently used test in screening. In this article, a chronic granulomatous disease case diagnosed in adulthood and having recurrent liver and lung abscesses is reported in light of the literature. A 22-year-old male patient was admitted to our polyclinic with high fever for two weeks. Patient's family history was received in terms of CGD and NBT was carried out. Patient's NBT result was negative (0%) twice. The patient was administered empiric ceftriaxone and metronidazole treatment. Abscess drainage was planned for the patient diagnosed as CGD as a result of the analyses. The patient who developed fever response and whose laboratory values improved was discharged proposing polyclinic control with trimethoprim/ sulfamethoxazole and itraconazole prophylaxis. This case indicates that, although rarely, CGD might be diagnosed in adulthood in our country where consanguineous marriage and hereditary diseases are common.

Purpose: To identify clinical characteristics, treatment modalities, and course of spontaneous rectus sheath hematoma (SRSH). In the literature, there is no prospective clinical trial that is intended for treatment in clinical research. Methods: Seventeen SRSH patients diagnosed and treated between March 2012 and March 2014 at the general Surgery Department of Erzincan University Training and Research Hospital were included. Age, sex, weight, height, predisposing factors, comorbid diseases, Charlson index, current medical treatment, physical examination signs, imaging methods, lowest hemoglobin value, blood type, SRSH type, INR/Factor V Leiden mutation/Factor VII/Factor VIII/Factor X/Protein S/Protein C values, treatment method, transfusions hospitalization duration, and outcome were studied. Results: Thirteen patients were female. The mean age was 63.3 +/- 18.7 years (range, 22-87 years), and the mean BMI was 27.8 +/- 3.5 (range, 20.9-33.7). Six patients had Grade I SRSH, 5 had Grade II SRSH, and 6 had Grade III SRSH. Thirteen were hospitalized for a mean duration of 9.3 +/- 8.1 days (range, 1-30 days). The death rate attributable to SRSH was 5.8%. Conclusion: Early diagnosis of SRSH by ultrasonography and/or computerized tomography is important for a low mortality rate. Even though medical treatments are important, arterial embolization by interventional radiology, or more radical interventions such as surgery, may be required. (C) 2014 AEC. Objective: This study investigated the minimum inhibitory concentration (MIC) values and in vitro activity of colistin in combination with tigecycline against carbapenem-resistant Acinetobacter baumannii strains isolated from patients with ventilator-associated pneumonia (VAP) using the E-test method. Methods: A total of 40 A. baumannii strains, identified using the Phoenix Automated Microbiology System (Becton, Dickinson and Co., Franklin Lakes, NJ, USA) by conventional methods, were included in this study. Pulsed-field gel electrophoresis was performed to examine the clonal relationships between isolates. The carbapenem resistance of the strains to colistin and tigecycline was assessed using the E-test method (Liofilchem, Roseto Degli Abruzzi, Italy). The in vitro activity of colistin in combination with tigecycline was evaluated using the fractional inhibitor concentration (FIC) index. Results: While only 1 of 40 A. baumannii strains was determined to be colistin resistant, 6 were tigecycline resistant. The MIC50, MIC90, and MIC intervals of the A. baumannii strains were 0.19, 1.5, and 0.064‒4 μg/ml for colistin and 1, 8, and 0.094‒256 μg/ml for tigecycline, respectively. No synergistic effect was observed using the FIC index; 8 strains exhibited an indifferent effect and 32 exhibited an antagonist effect. Three of the six strains that were resistant to tigecycline were indifferent; the remaining three were antagonistic. The colistin-resistant strain also exhibited an antagonist effect. Conclusion: In contrast to their synergistic effect against carbapenem-resistant A. baumannii isolates, colistin and tigecycline were highly antagonistic to carbapenem-resistant A. baumannii strains isolated from patients with VAP when the drugs were administered together. Therefore, alternative treatment options should be used during the treatment of VAP attributed to A. baumannii.

Objectives: This study aims to investigate ultrasound examination findings of cartilage, tendon and nerve structures in patients with acromegaly and assess whether imaging findings have any association with the clinical and laboratory features of the patients. Patients and methods: The study included 38 diagnosed acromegaly patients (18 males, 20 females; mean age 45.6±10.4 years; range 28 to 63 years) and 21 control subjects (8 males, 13 females; mean age; 43.1±7.4 years; range 29 to 58 years) similar for age, sex and body mass index. Patients’ clinical characteristics and laboratory parameters were recorded. The cross sectional area of median nerve, flexor pollicis longus and Achilles tendon, and humeral cartilage thickness were bilaterally assessed by ultrasound. Results: Arthralgia was present in 13 patients (34.9%) and 11 (28.9%) had paresthesia in the hands. Thickness of humeral cartilage and the cross sectional area of median nerve, flexor pollicis longus tendon and Achilles tendon was significantly higher in acromegaly patients compared with controls (p<0.001). While measurements performed with ultrasound were correlated among themselves, no significant correlation was present between ultrasound measurements and growth hormone and insulin-like growth factor-1. When acromegaly patients were analyzed as active (n=22) and inactive (n=16), no significant difference was determined in the parameters with the exception of median nerve measurements. Conclusion: Ultrasound examination revealed significantly different cartilage, tendon and nerve parameters in acromegaly patients compared to the control group. Median nerve cross sectional area was significantly greater in active patients compared to inactive patients.

OBJECTIVES: The aim of our study was to evaluate the total atrial conduction time and its relationship to subclinical atherosclerosis, inflammation and echocardiographic parameters in patients with type 2 diabetes mellitus. METHODS: A total of 132 patients with type 2 diabetes mellitus (mean age 54.5¡9.6 years; 57.6% male) and 80 age-and gender-matched controls were evaluated. The total atrial conduction time was measured by tissue-Doppler imaging and the carotid intima-media thickness was measured by B-mode ultrasonography. RESULTS: The total atrial conduction time was significantly longer in the patients with type 2 diabetes mellitus than in the control group (131.7¡23.6 vs. 113.1¡21.3, p,0.001). The patients with type 2 diabetes mellitus had significantly increased carotid intima-media thicknesses, neutrophil to lymphocyte ratios and high-sensitivity Creactive protein levels than those of the controls. The total atrial conduction time was positively correlated with the high-sensitivity C-reactive protein level, neutrophil to lymphocyte ratio, carotid intima-media thickness and left atrial volume index and negatively correlated with the early diastolic velocity (Em), Em/late diastolic velocity (Am) ratio and global peak left atrial longitudinal strain. A multiple logistic regression analysis demonstrated that the neutrophil to lymphocyte ratio, carotid intima-media thickness and global peak left atrial longitudinal strain were independent predictors of the total atrial conduction time. CONCLUSIONS: We suggest that subclinical atherosclerosis and inflammation may represent a mechanism related to prolonged total atrial conduction time and that prolonged total atrial conduction time and impaired left atrial myocardial deformation may be represent early subclinical cardiac involvement in patients with type 2 diabetes mellitus.

BACKGROUND: Low back pain (LBP) is a public health problem commonly seen in all societies. OBJECTIVE: The aim of this study was to determine the prevalence and specific risk factors of low back pain (LBP) in the central and outlying districts of the province of Trabzon, a Black Sea region of Turkey. METHOD: A random sample of 7897 (4006 men and 3789 women) adults was collected by using sampling techniques of stratification. In this study questionnaires were completed at face-to-face interviews with participants selected on the basis of place of residence, gender and age group. The used variables in this study were: use of cigarettes, status of marriage, level of education, and presence of chronic disease, the prevalence of lifetime LBP and of LBP in the preceding year. Chronic LBP was determined as being present for more than 6 weeks. RESULTS: The lifetime prevalence of LBP in the general population was determined at 62.1%. Prevalence in the preceding year was 46.1%, and that of LBP lasting more than six week was 18.1%. Lifetime prevalence of LBP, prevalence of LBP in the preceding year and prevalence of pain lasting more than six week were all statistically significantly higher in women (p < 0.001). Use of cigarettes, female gender, marriage, a low level of education and presence of chronic disease were identified as independent risk factors for LBP (p < 0.001). CONCLUSION: Lower back pain is a common public health problem. Recommendations were made for local health services to prevent LBP, including health education through combating chronic diseases, reducing cigarette consumption, improvement of working environments and life styles.

Colak, Mergul
The present study is to reveal physical activity levels and type 2 diabetes risk scores of university students in accordance with gender differences and determine the relation between physical activities and type 2 diabetes scores. One thousand ninety three students studying at Erzincan University (540 female and 553 male) participated in the study. A short form of the International Physical Activity Questionnaire and Finnish Diabetes Risk Score Questionnaire were used to determine physical activity scores and type 2 diabetes risk scores of the students. For analysis of the data, an independent t-test, one-way analysis of variance test and Pearson correlation analysis were used. Diabetes risk scores of women were found to be higher than men (p<0.01). Diabetes risk scores of men have a negative relation with all physical activity sub-categories, but for women it was found only in terms of vigor and total activity levels (p<0.01). It can be concluded that the association between diabetes risk scores and physical activity levels of male students was stronger than that of female students. Thus, increasing physical activity may reduce the diabetes risk score in university students.

Objectives: The objective of the present study was to evaluate anti-inflammatory effects of hydroxyfasudil in a protamine sulfate (PS) induced cystitis rat model. Additionally, we investigated prevention of bladder overactivity (BO), and tissue damage in these experiments. Methods: Animals were divided into four groups. In Groups 1 and 2, chemical induced cystitis model was created by administrating intravesical PS with PE50 catheter by the transurethral route. In Group 1, Rho-kinase inhibitor hydroxyfasudil was administered intaperitoneally, and in Group 2, subjects were administered a corresponding volume of saline in the same way. In Group 3, vehicle was administered intravesically and hydroxyfasudil was administrated intraperitoneally. Group 4 was a control Group, and the vehicle was administered intravesically and intraperitoneally. Micturition frequencies were recorded. Biochemical analyses were performed for oxidative stress, and pathological evaluations were investigated. In vitro contractions of bladder tissue strips were measured in tissue-bath. Results: There were significantly lower Lipid peroxidase levels and higher levels of Glutathione in Group 1 than Group 2 (P=0.016, P=0.001, respectively). There was generally more inflammation in Group 2 than the other groups as determined by microscopy. There were significantly higher frequencies of micturition, lower volume, and mean voided maximum urine output after PS administration in Groups 1 and 2. In vitro contraction responses of bladder strips to potassium chloride and acetylcholine were statistically higher in Group 2 than Groups 1 and 3. Conclusions: Significant reduction of inflammation by affecting the anti-oxidant defense systems was provided by hydroxyfasudil. Decreased in vitro responses to contractions of bladder smooth muscle strips were obtained. Hydroxyfasudil may be a potential new therapeutic option for inflammation and BO, in rat bladder.

Oner, Gokalp; Ulug, Pasa; Elmali, Ferhan
Introduction: The aim of this retrospective case control study was to identify predictors of ovarian response and pregnancy outcomes in intrauterine insemination (IUI). Material and methods: One hundred women undergoing IUI cycles with clomiphene citrate were enrolled. The number of antral follicles and the total ovarian volume by ultrasound, and the basal levels of follicle-stimulating hormone (FSH), estradiol, and inhibin B on cycle day 3 were measured in groups that were divided according to ovarian response. The tests were also evaluated according to ovarian response and pregnancy outcomes. All analyses were performed using the Statistical Package for the Social Sciences, version 15.0 (SPSS, Chicago, IL, USA). Results: The antral follicle count (AFC) was the best single predictor for ovarian response and pregnancy outcomes. The sensitivity and specificity for prediction of ovarian response were 81% and 78% for AFC at an optimum cutoff value of ≤ 13.1. Age was negatively correlated with ovarian volume (r =-0.280, p = 0.021) and AFC (r =-0.358, p = 0.003). Increasing FSH was associated with a reduction in AFC (r =-0.273, p = 0.025). The AFC was significantly correlated with ovarian volume (r = 0.660, p < 0.0001) and FSH (r =-0.273, p = 0.03). Conclusions: Our data demonstrate that the AFC provides better prognostic information on the occurrence of ovarian response during clomiphene citrate stimulation for IUI.

Objective: The inflammatory process plays an important role in the development of cardiovascular complications in patients with obstructive sleep apnea syndrome (OSAS). YKL-40/chitinase 3-like protein 1 is a novel biomarker of systemic inflammation. This study aimed to investigate whether carotid intima-media thickness (CIMT), a useful marker for early atherosclerosis, is associated with serum YKL-40/chitinase 3-like protein 1 levels in patients with normotensive and nondiabetic OSAS. Methods: The study included 40 OSAS patients and 40 age-sex- and body mass index-matched healthy controls. Serum YKL-40 levels were detected by enzyme-linked immunosorbent assay. CIMT was measured by B-mode ultrasound. Results: The patients with OSAS had significantly increased CIMT and higher YKL-40 and high sensitivity C-reactive protein (hsCRP) levels than those of the controls. CIMT was strongly correlated with serum YKL-40 levels (r=0.694, p<0.001), hsCRP (r=0.622, p<0.001), age (r=0.525, p=0.001), and weakly correlated with apnea-hypopnea index (AHI) (r=0.365, p=0.021) and the percentage of recording time spent (PRTS) of oxygen saturation <90% (r=0.488, p=0.001). Moreover, it was detected that serum YKL-40 levels were strongly correlated with AHI (r=0.617, p<0.001), and weakly correlated with SaO2 <90% of PRTS (r=0.394, p=0.012) and hsCRP (r=0.486, p=0.001). In multiple regression analyses, age and serum levels of YKL-40 and hsCRP were found to be independent predictors of CIMT. Conclusion: In patients with OSAS, CIMT was increased. This increase was associated with serum YKL-40 level. Increased serum level of YKL-40 may be an early predictor of atherosclerosis development in patients with OSAS.

Background: Acute pancreatitis (AP) is a serious disease characterized by inflammation of the pancreas. Material/Methods: We prospectively investigated a total of 42 patients treated at Karadeniz Technical University Faculty of Medicine General Surgery Department with the preliminary diagnosis of AP from January 2010 to June 2011, and who underwent biochemical tests and magnetic resonance cholangiopancreatography (MRCP). Patient age, sex, amylase levels, and other biochemical parameters, as well as treatment type, complications, etiologic factors, morbidity rates, hospital length of stay, and Ranson scores, were included in the evaluation. Results: Our study population consisted of 24 women and 18 men. The serum amylase levels ranged between 146 and 3086 U/L (mean 1271.07 U/L). When patients were scored according to the Ranson scoring system, 3 (7.1%) patients had a Ranson score of 0, 10 (23.8%) patients had a score of 1, 15 (35.7%) patients had a score of 2, 11 (26.2%) patients had a score of 3, 2 (4.8%) patients had a score of 4, and only 1 (2.4%) patient had a score of 5. We found abnormal pancreatic duct morphology in 10.3% of the patients who had mild pancreatitis attacks and 33.3% of the patients who had moderate pancreatitis attacks. Although we observed a correlation between the Ranson score and pancreatic duct morphology, this relationship was not significant (P>0.05). Conclusions: Based on the results of our study, we believe that MRCP and MR are as effective as CT in the follow-up and treatment of acute pancreatitis.
