Coexisting primary cancers of endometrium and ovary: Experience of single institution; [Endometrium ve overin Eşzamanlı Primer Kanserleri: Tek Merkezin sonuçları]

Narin, Mehmet A.; Karalok, Alper; Basaran, Derman; Ureyen, Isin; Turkmen, Osman; Turan, Taner; Tulunay, Gokhan

Synchronous primary cancers of the endometrium and ovary (SEOC) occur in approximately 10% of all women with ovarian cancer and 5% of all women with endometrial cancer. Diagnosis of these tumors as independent primaries or metastases is necessary for appropriate staging and treatment. We aimed to demonstrate the clinic-pathological characteristics and prognosis of patients diagnosed as SEOC. Clinico-pathological data of cases with SEOC were retrieved from the computerized database of Etlik Zubeyde Hanim Women’s Health and Research Hospital. Twenty-six patients with SEOC who underwent comprehensive surgical staging between 1998 and 2014 were included in study. Mean age at diagnosis of women with primary SEOC in our study was 50.9. Median follow up was 86.5 months and the 10 year overall survival (OS) rates were 86.0% for patients with SEOC. Among the variables only omental involvement independently affected OS (p= 0.008). Most of the tumors were well-differentiated, early stage and of endometrioid cell type. In this study we showed that women with SEOC was often young and mostly have an excellent prognosis.

How do surgeons approach breast cancer surgery in Turkey? A national survey

Emiroğlu, Mustafa; Inal, Abdullah; Sert, İsmail; İlhan, Enver; Peker, Kemal; Gürçelik, Mehmet Ali; Güngör, Hilmi; Salimoğlu, Semra; Can, Didem; Ellidokuz, Hülya

Background: The aim of this study was to investigate the experience, practice and approaches of general surgeons in relation to the treatment of breast cancer in Turkey. Methods: A survey was conducted between November 2012 and February 2013 with 453 general surgeons who claimed to perform breast surgery. Initial and most preferred approaches for breast cancer surgery and demographic features of participants were questioned. Initial approaches of surgeons for a suspected breast mass were assessed with a clinical scenario. Results: A total of 12.6 % of practicing general surgeons in Turkey responded to the survey. A multidisciplinary assessment was employed by 57.2 % of participants. The most frequently used diagnostic tool was needle biopsies (64.9 %) and the most frequently performed surgery for early stage cancers was breast-conserving surgery (72.2 %). The initial approach for locally advanced breast cancer was neoadjuvant chemotherapy (59.8 %) and mastectomy for metastatic cancer (22.7 %). Sentinel lymph node biopsies were utilized by 59.2 % of participants by different methods in appropriate cases. Oncoplastic breast surgery was performed by 9.0 % of participants, frequently or constantly. The surgeons’ initial approaches for the clinical scenario were imaging (56.7 %) and biopsy (40.6 %). Conclusions: Although there are efforts to improve up-to-date approaches towards breast cancer surgery by surgeons, currently there are significant inadequacies for evidence-based medicine practices.

Analysis of genotoxic activity of ketamine and rocuronium bromide using the somatic mutation and recombination test in Drosophila melanogaster

    Koksal, Pakize Muge; Gürbüzel, Mehmet

The present study evaluated the mutagenic and recombinogenic effects of two commonly used anesthetic agents, ketamine and rocuronium bromide, in medicine using the wing somatic mutation and recombination test (SMART) in Drosophila. The standard (ST) cross and the high-bioactivation (HB) cross with high sensitivity to procarcinogens and promutagens were used. The SMART test is based on the loss of heterozygosity, which occurs via various mechanisms, such as chromosome loss and deletion, half-translocation, mitotic recombination, mutation, and non-disjunction. Genetic alterations occurring in the somatic cells of the wing's imaginal discs result in mutant clones in the wing blade. Three-day-old trans-heterozygous larvae with two recessive markers, multiple wing hairs (mwh) and flare (flr3), were treated with ketamine and rocuronium bromide. Analysis of the ST cross indicated that ketamine exhibited genotoxicity activity and that this activity was particularly dependent on homologous mitotic recombination at concentrations of 250μg/ml and above. Rocuronium bromide did not exert mutagenic and/or recombinogenic effects. In the HB cross, ketamine at a concentration of 1000μg/ml and rocuronium bromide at all concentrations, with the exception of 250μg/ml (inconclusive), exerted genotoxic effects, which could also be associated with the increase in mitotic recombination.

Accelerated catheter-directed thrombolytic treatment in deep venous thrombosis: Mid-term results

Polat, Adil; Ketenciler, Serkan; Yücel, Cihan; Boyacioğlu, Kamil; Akdemir, Ilknur; Kük, Zeynep Gülben; Yildizhan, Ibrahim; Erentuğ, Vedat

Background: This study aims to retrospectively analyze the early and mid-term results of ultrasound accelerated catheter-directed thrombolytic treatment (EKOS) performed in our clinic in patients with iliofemoral deep venous thrombosis (DVT). Methods: In our clinic, EKOS was performed in 16 patients (12 males, 4 females; mean age 50.2±18.2 years; range 20 to 86 years). Deep venous thrombosis was present on the left lower extremity in nine and right lower extremity in seven patients. Mean duration of treatment from the onset of symptoms was 8.2±5.2 days. Additional to standard EKOS, six patients were administered additional thrombolytic via an angiocath placed in dorsal vein of ipsilateral foot in 1/3 dosage. Patients' fibrinogen levels were monitored periodically and thrombolytic treatment was stopped under critical levels with continuation of heparin infusion. Mean duration of thrombolytic infusion was 18.4±9.8 hours (range, 6-48 hours) and mean dose of thrombolytic was 38.2±18.1 mg (range, 13-90 mg). Results: Complete patency was obtained in seven patients (43.8%). One patient (6.3%) developed post-procedural retroperitoneal hematoma. No mortality occurred during follow-up. Mean and total durations of follow-up were 9.3±8.3 months (range, 0.9-24.3 months) and 148.7 patient/months, respectively. In the final controls, 10 patients (62.5%) were asymptomatic or mildly symptomatic, and two patients (12.5%) had post-thrombotic syndrome. Recurrence of DVT was observed in three patients (18.8%). Recurrence-free average and total durations of follow-up were 7.6±7.3 months and 122.0 patient/months, respectively. No patients had pulmonary emboli during follow-up. Conclusion: Incidence of post-thrombotic syndrome after DVT with EKOS treatment was lower than the reported 25%-50% incidence in the literature compared to only anticoagulant usage. However, rate of asymptomatic or mildly symptomatic patients was not as high as expected. Larger scale studies and comparisons with other pharmacomechanical treatments are required to evaluate treatment efficiency.

Is the preoperative administration of amiodarone or metoprolol more effective in reducing atrial fibrillation: After coronary bypass surgery?

    Onk, Oruc Alper; Erkut, Bilgehan

This study examined the influence of preoperative administration of amiodarone and metoprolol in preventing postoperative atrial fibrillation (AF) after coronary artery bypass grafting (CABG) surgery. The study comprised 251 patients who underwent CABG surgery at our hospital between January 2012 and May 2014. The patients were randomly divided into 2 groups: amiodarone therapy group (n=122 patients) and metoprolol therapy group (n=129 patients). In the amiodarone group, the patients received amiodarone tablet orally 1 week before coronary bypass surgery and during the postoperative period. In the metoprolol group, the patients received metoprolol tablet orally 1 week before surgery and during the postoperative period. The AF development rate was retrospectively evaluated between the first 3 days and 4 weeks after surgery. AF developed in 14 patients in the amiodarone group and 16 patients in the metoprolol group 4 weeks after the operation (P=0.612). No significant difference was observed between the groups in terms of intensive care unit and hospital stay. Furthermore, hospital charges were similar in both groups (P=0.741). The results of the logistic regression analysis showed age, left ventricular ejection fraction, left atrial diameter, and aortic crossclamping time to be predictors for postoperative AF. This study demonstrates that amiodarone and metoprolol have similar effects in prevention of AF after cardiac surgery. However, larger-scale studies need to be conducted to substantiate these findings.

Investigation of antibiotic resistance patterns and reduced vancomycin susceptibilities of methicillin-resistant Staphylococcus aureus isolates: A multi-center study; [Metisiline Dirençli Staphylococcus aureus Izolatlarinin Antibiyotik Direnci ve Azalmiş Vankomisin Duyarliliʇinin Araştirilmasi: Çok Merkezli Bir Çalişma]

Çikman, Aytekin; Aydin, Merve; Cülhan, Bariş; Parlak, Mehmet; Gültepe, Bilge; Kalayci, Yildiz; Bayindir Bilmen, Fulya; Solmaz, Sinem; Özekinci, Tuncer

The aims of this study were to determine the minimum inhibitory concentration (MIC) values of vancomycin, teicoplanin, daptomycin, quinupristin/dalfopristin, linezolid, tigecycline, chloramphenicol, rifampicin, ofloxacin and tetracycline and to investigate the reduced vancomycin susceptibility among methiciliin-resistant Staphylococcus aureus (MRSA) strains isolated in hospitals located in different geographical regions of Turkey. A total of 100 MRSA strains isolated from patients (of which 50% were from intensive care units) hospitalized in seven centers in Turkey [Istanbul (n= 15), Ankara (n= 15), Izmir (n= 15), Adana (n= 15), Diyarbakir (n=15), Erzincan (n= 15), Van (n= 10) J, between August 2013-August 2014, were included in the study. Fourty-three strains were isolated from blood, whereas 21 were from lower respiratory tract, 17 from wounds, eight from catheters, six from urine, four from nasal swab and one from cerebrospinal fluid samples. Methicillin resistance of the isolates was determined by using cefoxitin (30 ug) disk with standard disk diffusion method, while the MIC values of other antibiotics were determined with E-test in accordance with the recommendations of Clinical and Laboratory Standards Institute (CLSI). MIC results obtained for quinupristin-dalfopristin (Q/D) were evaluated according to the CLSI criteria used for methicillin-susceptible S.aureus and for tigecycline according to the criteria recommended by the Food and Drug Administration for MRSA. Primarily, agar screening method (ASM) was used for determination of vancomycin-intermediate S.aureus (VISA) and heterogeneous VISA (hVISA) strains. Brain heart infusion agar containing 6 pg/ml vancomycin was used in ASM, and the strains with suspicion of VISA/hVISA were screened by standard E-test and macro E-test methods. All MRSA strains were susceptible to vancomycin, teicoplanin, daptomycin, Q/D and linezolid by E-test method; and their rates of susceptibility for tigecycline, chloramphenicol, rifampicin, ofloxacin and tetracycline were detected as 89%, 97%, 40%, 39% and 32%, respectively. MIC50/MIC90 values were 1.5/2 μg/ml for vancomycin, 2/4 μg/ml for teicoplanin, 0.19/0.38 μg/ml for daptomycin, 0.19/0.38 μg/ml for Q/D, 0.75/1 μg/ml for linezoiid, 0.19/0.75 μg/ml for tigecycline, 3/6 μg/ml for chloramphenicol, 32/32 μg/ml for rifampicin, 32/32 μg/ml for ofloxacin and 32/64 μg/ml for tetracycline, respectively. For the evaluation of reduced vancomycin susceptibility, 2% (2/100) of MRSA strains were defined as VISA and 5% (5/100) as hVISA with ASM. One of those seven isolates identified as VISA/hVISA with ASM was evaluated as suspected hVISA by using both standard E-test and macro E-test methods. In conclusion, no MRSA resistant strain to vancomycin, teicoplanin, daptomycin, Q/D and linezoiid was determined in our study. However tigecycline resistance (11 %) was found higher than expected. As the glycopeptide resistance is increasing in the world and because of the intense use of these drugs in Turkey, the rates of vancomycin resistance among MRSA strains should be investigated periodically.

Surgical Treatment of Hip Instability in Patients With Lower Lumbar Level Myelomeningocele: Is Muscle Transfer Required?

Yildirim, Timur; Gursu, Sarper; Bayhan, İlhan Avni; Sofu, Hakan; Bursali, Aysegul

Background: Treatment of hip instability in patients with lower lumbar level myelomeningocele is clinically challenging. Muscle transfer procedures, release of contractures, and intertrochanteric varus-rotation osteotomies have been described to restore weak or absent abductor strength as well as relocation of the hip. However, controlled trials evaluating hip instability in lower lumbar myelomeningocele are limited in the current literature. Questions/purposes: The purposes of this study were to compare the (1) radiographic evidence for joint stability; (2) clinical outcomes (including abductor strength, ambulatory ability, and residual use of orthoses); and (3) complications between patients undergoing combined periarticular contracture releases and bony procedures with and without external oblique abdominal muscle transfers. Methods: Between 2004 and 2013, 14 pediatric patients (16 hips) were treated for hip instability secondary to myelomeningocele using releases with or without muscle transfer. From those, 13 patients (15 hips) with a mean age of 6 years who had L3 to L5 level involvement were evaluated retrospectively. The patients were separated into two groups. Nine hips (in eight patients) were treated by performing a combination of external oblique abdominal muscle transfer to the greater trochanter, periarticular release of contractures, and bony procedures. These were compared with six hips (five patients) treated by performing a combination of periarticular release of contractures and bony procedures without external oblique abdominal muscle transfer. This study compared the results between two surgeons, one of whom always performed these muscle transfers in this setting and the other who never performed muscle transfer during the study period. The patients were clinically followed up at a mean of 41 months (range, 14–122 months); none of the patients was lost to followup. Radiographic evaluation criteria included Reimer’s migration index, acetabular index, femoral neck-shaft angle, pelvic obliquity, and the presence of scoliosis. Clinical evaluation included muscle strength examination, periarticular contractures, necessity for using orthoses as walking aids, and Hoffer criteria of mobility. All complications were also noted from a chart review. Results: There were no differences between the two groups regarding postoperative femoral head localization, reflecting the presence or absence of residual subluxation, according to Reimer’s index (Reimer’s index = 32%; range, 10%–40%, in the muscle transfer group compared with 27%; range, 15%–43%, in the no-transfer group at latest followup; p = 0.723). Postoperatively, abductor strength improved in the group treated with external oblique transfer compared with the group treated without muscle transfer (median improvement of 2 versus 0, p = 0.02), but this improvement neither resulted in a clinically important difference in Hoffer criteria of mobility (no change was detected in either group) nor decreased the need for use of an orthosis (no change was detected in either group). With the numbers available, there was no difference with respect to complications between the two groups (two complications versus one). Conclusions: External oblique abdominal muscle transfer did not provide a clinically important improvement in functional recovery when compared with patients with L3 to L5 myelomeningocele and hip instability who were treated without it. We therefore do not recommend a routine muscle transfer procedure during the operative management of hip instability in patients with lower lumbar level myelomeningocele. Level of Evidence: Level III, therapeutic study.

Proliferation of trophoblasts and Ki67 expression in preeclampsia

Kaya, Barış; Nayki, Umit; Nayki, Cenk; Ulug, Pasa; Oner, Gokalp; Gultekin, Emre; Yildirim, Yusuf

Background/aims: Preeclampsia is a pregnancy-specific disease with the increased risk of maternal morbidity and mortality. It is characterised by placental vascular dysfunction. Despite the numerous studies on preeclampsia, studies evaluating proliferation of villous trophoblasts in preeclamptic placentas are limited. Ki67 is a proliferation marker that expresses in the nuclei of proliferating cells. In this study, we examined the proliferation of villous trophoblasts in placentas of preeclamptic patients by using Ki67 and compared it with placentas of normal pregnant patients.Material and methods: The current study is a prospective one, including 15 placentas from preeclamptic patients and 14 placentas from normal pregnancies as controls. For detection of proliferation in villous trophoblasts, Ki67 was used. Results: The Ki67 index was 11.48±1.67% in normal patients and 15.53±2.28% in preeclamptic patients. There was a difference in Ki67 index between the two groups (p < 0.001).Conclusion: Our results support the opinion that trophoblasts undergo regeneration hyperplasia as a result of injuries arising on the villous surface in preeclampsia. Proliferation of trophoblasts may contribute the development of preeclampsia.

Aydin, Bahattin Kerem; Sofu, Hakan; Senaran, Hakan; Erkocak, Omer Faruk; Acar, Mehmet Ali; Kirac, Yunus

Randomized controlled clinical trial. The main purpose of the present study was to comparatively analyze the effectiveness, advantages, and the complications of using semirigid synthetic softcast with respect to plaster of Paris (POP) during the treatment of clubfoot deformity. The study group consisted of 196 babies (249 feet). A total of 133 feet treated by an orthopedic referral center using semirigid synthetic softcast were included in group A whereas the other 116 feet treated by another orthopedic clinic using POP cast were included in group B. The Pirani scores, number of cast applications, time period until Achilles tenotomy, any skin problems due to the cast itself, and/or cast removal were recorded. A final parent satisfaction score was also obtained. The mean Pirani sores were significantly improved from the first administration to the time before Achilles tenotomy in both groups. There was no significant difference according to the number of casts applied until tenotomy. The slippage of the cast and skin lesions was significantly more common in group B. Higher parent satisfaction levels were detected in group A. Semirigid softcast has been found as superior to POP in the aspects of parent satisfaction and cast-related complication rates.

Assessment of genotoxic potential of two mycotoxins in the wing spot test of Drosophila melanogaster

Gürbüzel, Mehmet; Uysal, Handan; Kızılet, Halit

Mycotoxins, the toxic products of molds, exposure causes serious adverse health problems in human, animals, and crops. Determining the potential genotoxic effects of these substances is, therefore, of great importance. We have evaluated the genotoxic toxicity of two trichothecenes – diacetoxyscirpenol (DAS) and T-2 toxin – using the wing somatic mutation and recombination test (SMART) in Drosophila melanogaster. The SMART is based on the principle that the loss of heterozygosis of recessive markers located on the left arm of chromosome 3 – multiple wing hairs (mwh) at the map position 0.3 and flare-3 (flr 3) at the map position 38.8 – may occur through various mechanisms such as mitotic recombination, mutation, deletion, half-translocation, chromosome loss, and nondisjunction. Both the mycotoxins were administered to third instar larvae (72 ± 4 h old) at concentrations ranging from 5 to 40 μM. Based on our results, DAS and T-2 toxins does not exert genotoxic effects up to a concentration of 40 μM.

The importance of vitamins for soccer players

    Eskici, Günay

Soccer is one of the most widely played and complex sports in the world, where success depends on technical, tactical and physical skills of the players. Studies to improve performance in soccer have often focused on technique and tactics. However, nutrition is one of the most important factors infl uencing athletic performance of the players. The duration of matches is long and the training is intense. This leads to increased requirements for energy and nutrients, as well as increased reactive oxygen radicals and hence increased muscle damage. Vitamins are micronutrients that a living organism requires in trace quantities for health. As these assume crucial functions in the body, the performance of the player is negatively affected particularly during long-term defi ciency. Beta-carotene, C and E vitamins are antioxidants that protect against oxygen radicals. In case of their defi ciency, oxidative stress and muscle fatigue increases. Vitamin D is involved in maintaining mineral balance, and it increases absorption of dietary calcium and phosphorus. In case of vitamin D defi ciency, injuries resulting from the musculoskeletal system might increase. B Vitamins (B1, B2, niacin, B6, B12, biotin, folic acid and pantothenic acid) perform duties such as energy production, absorption and transport of iron and blood cell production. Athletes who follow an energy-restricted and imbalanced diet might develop vitamin defi ciency. In such a case, supplements can be used as recommended by the doctor/dietician. It is further reported that supplement use by athletes who have an adequate and balanced nutrition does not increase performance.

The effectiveness of Crataegus orientalis M Bieber. (Hawthorn) extract administration in preventing alveolar bone loss in rats with experimental periodontitis

Hatipoʇlu, Mükerrem; Saʇlam, Mehmet; Köseoʇlu, Serhat; Köksal, Ekrem; Keleş, Ali; Esen, Haci Hasan

The purpose of this animal study was to evaluate the effects of hawthorn (Crataeus orientalis M Bieber.) extract on serum oxidative status and alveolar bone loss in experimental periodontitis. Twenty-seven Wistar rats were assigned to one of the following groups: nonligated+ placebo (saline) (NL, n = 9), ligature only+placebo (saline) (LO, n = 9), and ligature and treated with hawthorn extract in saline (H, n = 9) (100 mg/kg orogastrically, once a day for 11 days). Periodontitis was induced by submerging a 4/0 silk ligature in the sulcus of the mandibular right first molars of rats, and the animals were sacrificed after 11 days. Micro-CT examinations were performed for linear and volumetric parameter assessment of alveolar bone. Periodontal tissues were histopathologically examined to assess the differences among the study groups. Levels of serum total antioxidant status (TAS)/total oxidant status (TOS), and oxidative stress index (OSI) were also analyzed. Alveolar bone loss was significantly reduced by hawthorn administration compared to LO group (p<0.05). The number of inflammatory cells and osteoclasts in the LO group was significantly higher than that of the NL and H groups (p< 0.05). The number of osteoblasts in the LO and H groups was significantly higher than that of the NL group (p<0.05). TOS and OSI levels were significantly reduced in H group compared to LO group (P <0.05) and TAS levels were similar in H and NL group (p< 0.05). Hawthorn extract showed inhibitory effect on periodontal inflammation and alveolar bone loss by regulating TAS, TOS and OSI levels in periodontal disease in rats when administered systemically.

Duodenal Obstruction Caused by Acute Appendicitis with Intestinal Malrotation in a Child

    Biçer, Şenol; Çelik, Ali

Objective: Rare disease Background: In patients with intestinal malrotation, the diagnosis of acute appendicitis can be difficult due to atypical presentation. Duodenal obstruction caused by acute appendicitis with the presence of malrotation has rarely been reported in children. Case Report: We report the case of a 14-year-old male patient with bilious vomiting and abdominal distension. A diagnosis could not be made by computed tomography, ultrasonography, or endoscopy. We observed a dilated stomach and malrotation in laparotomy. The caecum was in the right upper quadrant, and an inflamed appendix was located in the subhepatic region. After the appendectomy, the cecum was mobilized and fixed in the right lower quadrant. Conclusions: In children with intestinal malrotation, acute appendicitis can present as duodenal obstruction without abdominal pain, and standard imaging methods can miss the correct diagnosis.

Pituitary Insufficiency and Hyperprolactinemia Associated with Giant Intra-and Suprasellar Carotid Artery Aneurysm

    Gungor, A.; Gokkaya, N.; Bilen, A.; Bilen, H.; Akbas, E.M.; Karadeniz, Y.; Eren, S.

Pituitary insufficiency secondary to internal carotid artery (ICA) aneurysm is a very rare condition. Its prevalence is reported as 0.17% (Heshmati et al., 2001). We present a case of pituitary insufficiency and hyperprolactinemia secondary to suprasellar giant intracranial aneurysm. A 71-year-old man was admitted to our clinic with symptoms of hypopituitarism, hyperprolactinemia, and visual field defect. His pituitary MRI and cerebral angiography revealed a giant saccular aneurysm filling suprasellar cistern arising from the ophthalmic segment of the right ICA. Endovascular treatment was performed on the patient to decrease the mass effect of aneurysm and improve the hypophysis dysfunction. After treatment, his one-year follow-up showed the persistence of hypophysis insufficiency, decrease of prolactin (PRL) level, and normal visual field. An intracranial aneurysm can mimic the appearance and behavior of a pituitary adenoma. Intracranial aneurysms should be taken into consideration in the situation of hypopituitarism and hyperprolactinemia. It is important to distinguish them because their treatment approach is different from the others.

Effect of N-acetylcysteine and allopurinol combination to protect spinal cord ischemia/reperfusion injury induced by aortic cross-clamping in rat model

    Erkut, Bilgehan; Onk, Oruc Alper

Purpose: The aim of this experimental study was to determine whether combination of N-acetylcysteine and allopurinol can reduce the ischemia/reperfusion injury of spinal cord in a rat model. Methods: Twenty-seven Spraque Dawley rats, all male, weighing between 220 to 370 (mean 325) gr were used in the study. 27 rats were divided into three groups: sham group, control group and experimental group. Abdominal aortic occlusion between the renal arteries and iliac bifurcations was carried out for 60 min with proximal and distal clip in control and experimental groups. Hindlimb motor functions were evaluated at 24, and 48 h using the Tarlov Scale. Besides, spinal cord samples were taken for determination of superoxide dismutase, and catalase activities as antioxidant enzymes, and malondialdehyde as an indicator of lipid peroxidation and xanthine oxidase levels as source hydroxyl radical for biochemical studies. Also, histopathological evaluation was made from cord tissue samples. Results: The experimental group subjects had better neurological functions than control group subjects. In experimental group; superoxide dismutase and catalase levels increased, while malondialdehyde and xantine oxidase levels decreased as compared with control group. Histopathological examination showed that experimental group had less cell degeneration, hemorrhage, edema and inflammation loss than control group. Conclusions: This study offers that combined use of N-acetylcysteine and allopurinol might help protect the spinal cord against ischemia/reperfusion injury.

IKDC score improved to A in 11 patients (58 %) from Group 1, and 18 patients (72 %) from Group 2. Mean anterior translation according to KT-2000 arthrometer measurements was 5.2 ± 1.3 mm in Group 1, and 4.6 ± 1.3 mm in Group 2. Post-operative time to return to sports activity was 8.5 ± 3.0 months in Group 1, and 6.5 ± 2.2 months in Group 2.; Conclusion: Partial meniscectomy for irreparable medial meniscal tears, applied during the same surgery with anterior cruciate ligament reconstruction, negatively affects the clinical outcomes in the short-term follow-up. This study may be a reference for long-term clinical trials and also future investigations of new methods in the treatment of similar cases.

Background: Atrial electromechanical delay (AEMD) times were considered independent predictors of cardiovascular morbidity among the general population. We aimed at evaluating AEMD times and other risk factors associated with 2-year combined cardiovascular (CV) events in HD patients. Material and Methods: Sixty hemodialysis (HD) and 44 healthy individuals were enrolled in this prospective study. Echocardiography was performed before the mid-week dialysis session for HD patients. Data were expressed as mean ± SD. Spearman test was used to assess linear associations. Survival was examined with the Kaplan-Meier method. Multivariate Cox regression analysis was used to determine the predictors of combined CV events in this cohort. Results: At the beginning of the study, left intra-atrial-AEMD times were significantly longer in HD patients compared to the left intra-atrial-AEMD times in healthy individuals. After 24 months, 41 patients were still on HD treatment and 19 (31.6%) had died. Serum triglyceride, total cholesterol and albumin were found to be higher and C-reactive protein (CRP) levels, left intra-atrial EMD time (LIAT) and interatrial EMD times were found to be lower in survived HD patients. With the cut-off median values of 3.5 g/dl for albumin, 0.87 mg/dl for CRP, 157 mg/dl for total cholesterol and 151 mg/ dl for triglyceride, the Kaplan-Meier curves demonstrated significant differences in terms of all-cause mortality. We also demonstrated the Kaplan-Meier survival curves of HD patients according to tertile values of LIAT. Cox regression analysis revealed that increased CRP and higher LIAT were found to be independent predictors of combined CV events. Conclusions: Increased LIAT and inflammation were found to be closely associated with 2 years combined CV events and all-cause mortality in HD patients.

    Karakurt, Papatya; Kasikci, Magfiret

The purpose of this study was to evaluate the validity and reliability of the Turkish version of the Diabetes Self-Care Scale. Self-care in diabetes is influenced by various factors, and it is important that the diabetes healthcare providers evaluate the efficacy of self-care activities. Accurate and careful measurements of diabetes self-care activities/levels provide important information both for healthcare providers and researchers to enable deeper understanding of appropriate management of and attitude towards self-care in patients with diabetes. Language properties of a Turkish adaptation of the Diabetes Self-Care Scale were ensured with the translation-backtranslation method, content validity by soliciting specialist opinions. Reliability, consistency over time (test-retest reliability), item-total item correlation analysis, and internal consistency were also analyzed. Data were collected between May and July 2007. The Cronbach’s alpha reliability coefficient was 0.81. The Diabetes Self-care Scale is a valid and reliable scale to evaluate self-care activities of diabetics.
