Serifoglu, Berkay; Evginer, Sercan; Akdag, Mehmet; Bostanci, Asli; Turhan, Murat
Purpose: To evaluate the feasibility, morbidity, and efficacy of transoral robotic surgery (TORS) tongue base resection (TBR) combined with upper airway airflow-facilitating surgeries in patients with obstructive sleep apnoea (OSA) characterized by tongue base hypertrophy and collapse, and to identify anatomical and nonanatomical predictors of surgical success. Methods: This single-center, prospective observational cohort study included 110 adult patients with moderate to severe OSA (AHI ≥ 15) or positional OSA, who failed or refused conservative therapies. Retrolingual/hypopharyngeal obstruction and tongue base collapse were confirmed via drug-induced sleep endoscopy or flexible nasopharyngoscopy. Patients with contraindications for surgery, inadequate mouth opening, absence of postoperative polysomnography, or under 18 years of age were excluded. Demographic, polysomnographic, and perioperative data were collected and statistically analyzed. Results: The mean patient age was 47.4 ± 10.8 years, and mean BMI was 30.2 ± 4.0 kg/m²; 93.6% were male. All patients underwent TORS-TBR without the need for tracheostomy. The mean hospitalization period was 8.5 ± 4.3 days. Statistically significant differences between surgical success and failure groups included AHIREM, obstructive apnoea counts in REM and NREM, mean obstructive apnoea duration, total apnoea duration, lowest oxygen saturation, mean desaturation percentages, and sleep time with oxygen saturation below 90% (ST90). Postoperative polysomnography demonstrated significant improvements across almost all parameters. Based on surgical success criteria, 73.6% of patients achieved a successful outcome. Conclusion: Transoral robotic tongue base surgery, combined with individualized multilevel airway interventions, provides an effective, feasible early treatment option for OSA. This approach supports personalized management strategies and significantly improves polysomnographic outcomes, contributing to the reduction of OSA-related morbidity and mortality.

Tooth color selection is a crucial step in prosthetic dental treatments. However, the process often suffers from subjectivity, environmental light variability, and the high cost or lack of standardization in instrumental methods. This study aims to develop a consistent and reliable tooth color prediction model using machine learning (ML) techniques, even under different clinical lights. In-vivo intraoral images of anterior teeth were collected from volunteer patients under five different clinical light sources. The teeth were annotated, segmented, and matched with corresponding color labels. LAB color space was used, and the CLAHE (Contrast Limited Adaptive Histogram Equalization) technique was applied to the L-channel to reduce light-induced glare. To address class imbalance, data augmentation was performed, resulting in a balanced dataset of 16,640 images. The dataset was split into 80% training and 20% testing subsets. Five ML algorithms—Random Forest (RF), K-Nearest Neighbors (KNN), Logistic Regression, Decision Tree, and XGBoost—were evaluated. As a result of experimental studies, RF and XGBoost obtained the highest performance, both achieving an accuracy of 97% in predicting tooth color across different lighting scenarios. These results demonstrate the robustness of the approach under variable lighting conditions. This study demonstrates that ML algorithms combined with image enhancement techniques such as CLAHE can provide accurate and light-independent tooth color predictions. The proposed method offers a practical and low-cost tool to support clinical decision-making in prosthetic dentistry, potentially enhancing standardization and efficiency in color matching processes.
- MakaleYalnız künye2025Development of Medical Device-Related Pressure Injury Risk Assessment Scale
Aydin Kahraman, Handan; Ipek Çoban, Gulay
OBJECTIVE: This study aimed to develop a valid and reliable measurement tool to assess the risk of medical device-related pressure injuries (MDRPIs) in patients. A methodological design was employed to evaluate the validity and reliability of the Medical Device-Related Pressure Injury Risk Assessment Scale (MDRPI Scale). METHODS: The study was conducted with 200 patients who were undergoing care and treatment involving the use of medical devices. Data were collected using the Patient Descriptive Characteristics Form, the MDRPI Scale, and the Braden Risk Assessment Scale. Analyses focused on evaluating the MDRPI Scale's validity and reliability. RESULTS: The content validity index of the MDRPI Scale was 0.87, and its Cronbach α coefficient was 0.73. The scale's items, which explained 47.063% of the total variance, were grouped under 2 factors: Factor 1-General Condition of the Patient, and Factor 2-Effect of the Medical Device on the Contact Area. The model fit indices obtained from the Confirmatory Factor Analysis (CFA) supported the 2-factor structure (χ2/df=2.091). No problematic items were identified within the final 8-item version of the scale. CONCLUSIONS: This study presents the first comprehensive and systematic scale specifically developed to identify MDRPI risk and to evaluate the integrity of skin and mucosa in patients exposed to medical devices. The findings demonstrate that the MDRPI Scale, consisting of 8 items and 2 factors, is a valid and reliable tool for assessing the risk of MPRPIs in clinical settings.

Lichen secondary metabolites with many remarkable biological activities are used in cancer treatments due to their low side effects and high anticancer potential. In particular, these metabolites constitute an interesting research area in cancer treatments due to their potential to induce apoptosis and suppress metastasis by inhibiting cancer-related signaling pathways. The Wnt/β-catenin signaling pathway plays a role in important biological processes such as oncogenesis, cell cycle regulation, cell proliferation, metastasis, differentiation, apoptosis, and drug resistance. Therefore, inhibition of this pathway is a potential target in cancer therapies. There is no detailed study explaining the potential anticancer molecular mechanism of the lichen secondary metabolite lobaric acid (LA) on breast cancer. Here, it is aimed to investigate the effect of LA on viability, apoptosis, and migration in MCF-7 cells and to elucidate the relationship between the potential anticancer effect and the Wnt/β-catenin signaling pathway. The dose- and time-dependent viability of LA-treated MCF-7 cells was evaluated by XTT assay, and the IC50 value was determined as 44.21 μg/mL at 48 h. LA increased the apoptotic cell population, as shown by flow cytometry analysis, qPCR, and Western blot results. LA inhibited β-catenin by inducing GSK3-β protein expression, thereby suppressing Wnt/β-catenin target genes. LA might be a natural active compound candidate for breast cancer treatment.

Aygörmez, Serpil; Makav, Mustafa; Kuru, Mushap; Eliş Yıldız, Sevda; Atila Uslu, Gözde; Küçükler, Sefa; Maraşlı, Şaban
The objective of this study was to conduct a biochemical and immunohistochemical assessment of the potential effects of zingerone (ZNG) on colistin (COL)-induced pulmonary injury in rats. To achieve this, a total of 35 female rats were utilized, of which 28 underwent ovariectomy (OV). The experimental groups were as follows: Control group received only saline intramuscularly without OV; OV group underwent OV and received saline intramuscularly; OV + ZNG group received 50 mg/kg ZNG via oral gavage; OV + COL group received a total of 73 mg/kg COL intramuscularly; OV + COL + ZNG group received a COL intramuscularly administration. Subsequently, ZNG was administered by oral gavage for a duration of 7 days. The results obtained at the conclusion of the study indicated that COL administration led to a suppression of antioxidant markers, including glutathione (GSH), catalase (CAT), superoxide dismutase (SOD), and glutathione peroxidase (GPx), while it elevated malondialdehyde (MDA) levels in the lung tissue of rats. Furthermore, COL administration induced inflammation by increasing the levels of interleukin 1β (IL-1β), nuclear factor kappa B (NF-κB), and tumor necrosis factor-alpha (TNF-α). Immunohistochemical analysis of Beclin-1, and Caspase-3 were found to be more pronounced in the OV + COL group compared to the other groups. ZNG treatment resulted in an increase in both nonenzymatic and enzymatic antioxidant levels and a reduction in MDA levels in rats. Furthermore, ZNG mitigated COL-induced inflammation, oxidative stress, autophagy, and apoptosis in lung tissue. No significant differences were observed in oxidative stress, inflammation, apoptosis, and autophagy parameters among the control, OV, and OV + ZNG groups. In conclusion, it was determined that COL administration in rats led to lung tissue damage and compromised tissue integrity, while ZNG treatment ameliorated the damage induced by COL.

The increasing global burden of oxidative stress-related conditions, diabetes, and neurodegenerative disorders underscores the urgent need for multi-targeted, plant-based therapeutic agents. In this context, the present study aimed to evaluate the antidiabetic, antioxidant, and neuroprotective properties of various Satureja hortensis and Satureja montana extracts and essential oils. In vitro biological activities were assessed through α-glucosidase, acetylcholinesterase (AChE), and butyrylcholinesterase (BChE) inhibition assays, along with DPPH• and ABTS• + radical scavenging assays. At a concentration of 100 µg/mL, S. hortensis root methanol extract (ShRME) and herb water extract (ShHWE) exhibited strong α-glucosidase inhibitory activity (69.88% and 71.23%, respectively). Conversely, S. montana herb water extract (SmHWE) and methanol extract (SmHME) showed the highest DPPH• radical scavenging activity (36.28% and 24.37%, respectively). S. montana essential oil (SmEO) demonstrated notable inhibition of AChE (32.58%) and BChE (41.62%) at 1000 µg/mL, while S. hortensis essential oil (ShEO) inhibited BChE by 44.39%. GC-MS analysis revealed that SmEO primarily contained carvacrol (43.72%), γ-terpinene (17.24%), and p-cymene (14.56%), while ShEO was rich in thymol (39.84%), γ-terpinene (20.16%), and p-cymene (13.72%). These phenolic monoterpenes are likely contributors to the observed antioxidant and cholinesterase inhibitory activities. Principal component analysis explained 74.8% of the total variance and clearly separated the samples based on their activity profiles. Extracts were closely associated with glucose-regulating effects, while essential oils clustered with enzyme-inhibiting neuroprotective activities. These findings highlight the multifunctional therapeutic potential of Satureja species and support their further investigation as candidates for the development of plant-derived agents against oxidative stress, diabetes, and neurodegenerative diseases.

Background: Isosexual precocious puberty (PP) is defined as the appearance of secondary sexual characteristics before the age of 8 years in girls and before the age of 9 years in boys. The aim of this study was to evaluate the association of ovarian volume and bone age measurements with the number of symptoms in female patients diagnosed with PP. Methods: The dataset includes pelvic ultrasound and bone age tests from 96 girls diagnosed with PP who presented to a tertiary hospital’s pediatric outpatient clinic with signs and symptoms of early puberty between January 1, 2020, and January 1, 2021. The study included 96 healthy ladies of the same age as a control group. Results: A total volume >2.75 was shown to predict the existence of pubertal pre with 79.2% sensitivity and 90.6% specificity (area under the curve [AUC]: 0.903; P = 0.001). A total volume of >3.25 predicted the existence of 2 or more symptoms in patients with PP, with a sensitivity of 72.1% and a specificity of 98.1% (AUC: 0.917; P = 0.001). Conclusion: To the best of our knowledge, no study has been published that investigates the relationship between the number of symptoms at the time of presentation and ovarian volume and bone age in girls with PP. Our investigation, which involved 192 female participants (96 patients and 96 controls), revealed a statistically significant correlation between the frequency of symptoms indicating early puberty and a rise in both ovarian capacity and bone age.

Acute lung injury (ALI) is a severe respiratory disorder associated with high morbidity and mortality. Lipopolysaccharide (LPS) is widely used to induce ALI in animal models. D-carvone, a natural monoterpene, has been reported to possess anti-inflammatory and antioxidant properties. This study aimed to investigate the protective effects of D-carvone on LPS-induced ALI in rats. Thirty-six male rats were randomly divided into six groups (n = 6): control, D-carvone (10 mg/kg and 20 mg/kg p.o.), LPS (10 mg/kg E. coli lipopolysaccharide i.p.), and LPS + D-carvone (LPS with either 10 or 20 mg/kg D-carvone). D-carvone was administered orally once daily for 10 days. On day 10, sepsis was induced with LPS administration, and samples were collected after 6 h under deep anesthesia. LPS administration caused significant lung injury, as evidenced by increased histopathological scores, upregulation of pro-inflammatory markers (TLR4, IL-1β, TNF-α), and oxidative stress (increased MDA, decreased GSH and SOD). Treatment with D-carvone at both doses significantly attenuated these changes. D-carvone downregulated pro-inflammatory markers, upregulated anti-inflammatory (NRF2) and anti-apoptotic (Bcl-2) proteins, and reduced the levels of pro-inflammatory cytokines (IL-1β, TNF-α, IL-8) in lung tissues. In conclusion, D-carvone protects against LPS-induced ALI in rats, possibly through its anti-inflammatory and antioxidant properties. These findings suggest that D-carvone could be a potential therapeutic candidate for preventing and treating ALI.
- MakaleAçık Erişim2025Assessing the short-term hematological and pulmonary effects of air pollution: a cross-sectional study in a Turkish urban setting
Kılınçer, Mehmet; Gürsoy, Ersan
Background: Air pollution has become a significant global public health concern, with evidence linking it to various adverse health outcomes, including respiratory and cardiovascular diseases. While numerous studies have investigated the effects of these particulate and gaseous pollutants on both healthy individuals and patients, further research is needed to clarify the short-term hematological and pulmonary responses in individuals without underlying health conditions. This study aims to explore the relationship between air quality, hematological parameters, and pulmonary function in a healthy population in Turkey. Methods: This cross-sectional study included 326 healthy, non-smoking adults aged 18–65 years. Air Quality Index (AQI) data for the examination day and the preceding 5 days were collected. Hematological parameters and pulmonary function tests were analyzed. Spearman and Pearson correlation tests were used to compare numerical variables. Group comparisons were conducted using the independent samples t-test and Mann-Whitney U test. Results: The mean AQI on the day of the medical visit was 68.20, indicating moderate air quality. Significant negative correlations were observed between AQI and hematological parameters, including leukocyte (r = -0.111, p = 0.046), lymphocyte (r = -0.134, p = 0.016), and platelet counts (r = -0.141, p = 0.011). Similar negative correlations were found for the 5-day average AQI. For pulmonary parameters, AQI was negatively correlated with FEF50% (r = -0.172, p = 0.002), FEF25% (r = -0.140, p = 0.012), FEV1/FVC% (r = -0.125, p = 0.024), and FEF75% (r = -0.124, p = 0.025). Conclusion: Short-term exposure to moderate air pollution significantly impacts hematological parameters and specific pulmonary function indices, even in healthy individuals. These findings emphasize the importance of continuous air quality monitoring and public health interventions to mitigate the health risks of air pollution.

In this study, a new series of 2-substituted-5-methylsulfonylbenzoxazole derivatives (C1-C16) were successfully synthesized. The structures of the compounds were confirmed by FT-IR, 1H NMR, 13C NMR, and mass spectroscopy, and the results for acetylcholinesterase (AChE) and butyrylcholinesterase (BChE) enzyme inhibitory activity were reported as IC₅₀ and Ki values. The compounds' Ki values against AChE ranged from 7.55 ± 2.88 to 87.39 ± 11.67 nM, and their Ki values against BChE ranged from 3.23 ± 0.64 to 107.86 ± 13.38 nM. Accordingly, all compounds except C1, C2, C11, and C12 were found to be more effective than donepezil against AChE, and all compounds except C1, C5, and C12 were found to be more effective than donepezil against BChE. Molecular docking studies were performed for all compounds against the AChE and BChE enzymes. Conventional hydrogen bonds and π interactions (π-π stacked, π-π T-shaped, π-alkyl, π-sigma, π-cation) were observed in the interactions of the compounds with the target enzymes. Molecular dynamics (MD) simulation of complexes formed by compounds with the highest inhibition activity on the two enzymes affirmed formation of stable complexes. Additionally, the molecular electrostatic potential (MEP) map of all compounds and the extent to which the HOMO-LUMO energy gaps affect the reactivity, stability, and inhibitor profiles of these compounds were investigated using density functional theory (DFT) studies, a computational analysis method. The study proves that 2-substituted-5-methylsulfonylbenzoxazole derivatives exhibit potent and selective cholinesterase inhibition.
- MakaleAçık Erişim2025The Protective Effect of Thiamine and Thiamine Pyrophosphate Against Linezolid-Induced Oxidative Liver Damage and Lactic Acidosis in Rats
Isik, Bahar; Ates, Irem; Yucel, Nurinisa; Suleyman, Bahadir; Mendil, Ali Sefa; Sezgin, Esra Tuba; Suleyman, Halis
Linezolid, an antimicrobial agent, has been linked to lactic acidosis, oxidative stress, and liver damage. Oxidative stress is considered to play a key role in this damage. Thiamine pyrophosphate (TPP), the active form of thiamine, may prevent lactate accumulation and enhance aerobic capacity. Therefore, this study aimed to evaluate the protective effect of TPP against possible linezolid-induced liver damage and lactic acidosis in rats. Twenty-four male Wistar albino rats were randomly assigned to four groups (n = 6): healthy control (HG), linezolid (LZD), thiamine plus linezolid (TLZD), and TPP plus linezolid (TPLZD). Thiamine and TPP (20 mg/kg, intraperitoneal (i.p.)) were administered once daily, while linezolid (125 mg/kg, per os (p.o.)) was given twice daily (250 mg/kg/day) for 28 days. Animals were euthanized under high-dose anesthesia (with 50 mg/kg, i.p. thiopental sodium). Liver tissues were analyzed for MDA, tGSH, SOD, and CAT, and examined histopathologically. Blood samples were collected prior to euthanasia to assess lactate, LDH, ALT, AST, and TPP levels. In the LZD group, MDA, lactate, ALT, AST, and LDH levels significantly increased, while tGSH, SOD, CAT, and TPP decreased (p < 0.001). Histopathology showed hydropic degeneration, necrosis, and mononuclear cell infiltration (p < 0.05). Thiamine did not prevent these alterations (p > 0.05), whereas TPP significantly prevented both biochemical and histopathological changes (p < 0.05), indicating its protective efficacy. TPP may offer significant protection against linezolid-induced hepatotoxicity and lactic acidosis.

- MakaleAçık Erişim2025Physiological and biochemical responses of wheat (Triticum aestivum L.) and barley (Hordeum vulgare L.) to combined exposure of Pharmaceuticals, personal care products (PPCPs), and artificial sweeteners (ASs)
Elveren, Müjgan
In this study, pharmaceutical active ingredients (gemfibrozil, acetaminophen and ciprofloxacin) and artificial sweeteners (aspartame and sucralose) were combined and applied to wheat and barley plants. After 15 days of cultivation, a series of biochemical and physiological parameters were evaluated. These included electrolyte leakage, antioxidant enzyme activities such as superoxide dismutase (SOD), catalase (CAT), peroxidase (POX), chlorophyll levels, total phenolic content (TPC), Ferric Reducing Antioxidant Power (FRAP), total flavonoid content (TFC) and concentrations. The relationship between the control samples and the samples treated with pharmaceutical active ingredients and artificial sweeteners was statistically evaluated at the 95% confidence interval, and significant differences were found. The treatments markedly influenced plant physiology and antioxidant defense mechanisms. Electrolyte leakage increased to 795.71 ± 50.14 µS cm-1 in wheat, while catalase (CAT), peroxidase (POX), and superoxide dismutase (SOD) activities showed significant elevations, reaching 5797.5 ± 115.14, 95176.00 ± 3675.42, and 1766.11 ± 13.97 EU g−1, respectively. In contrast, chlorophyll a + b, carotenoids, total phenolic content (TPC), total flavonoid content (TFC), and FRAP values decreased substantially compared with controls. These findings demonstrate that mixtures of pharmaceuticals and artificial sweeteners can disrupt photosynthetic pigment stability, and impair antioxidant defense in wheat and barley, highlighting potential ecological and agricultural risks associated with emerging contaminant mixtures.

Background: Calcifications in the carotid and vertebral arteries may be present on cranial and temporal bone CT imaging of pediatric patients. Few studies have investigated the frequency, location, and patterns of carotid artery calcifications in this age group. However, these studies are outdated and do not include data on the vertebral artery. The aim of this study was to determine the frequency, location, and pattern of incidental carotid and vertebral artery calcifications on cranial CT and temporal bone CT images in children under 15 years of age. We also aimed to investigate possible associations between these calcifications and various diseases. Methods: A total of 300 CT images of the cranial and temporal bone of 300 pediatric patients were retrospectively evaluated for the presence of calcification in the carotid and vertebral arteries. The evaluation included determining the presence of calcification in the artery, the pattern of calcification, the degree of calcification, and its anatomical location. Results: In the current study, 300 CT images were analyzed, and calcifications were found in the vertebral artery in 17 patients (5.6%) and the carotid artery in 82 patients (27.3%). The supraclinoid segment and the carotid siphon regions are the most common locations of carotid artery calcifications, with 62 patients (20.7%). The V4 segment is also the most common location for vertebral artery calcifications, with 15 patients (5%). Focal punctate calcification is the most common pattern (65 patients, 21.7%). Incidental carotid and vertebral artery calcifications did not correlate with other diseases. Conclusions: Carotid and vertebral artery calcifications are common incidental findings in pediatric patients. In our study, no association was found between other diseases and incidental carotid and vertebral artery calcifications.

Background/Objectives: The aim of this study was to evaluate computed tomography (CT) findings in penetrating globe injuries and their relationship with ocular trauma scores (OTSs). Methods: Patients with penetrating globe injuries who had orbital CT images were included in this study. Demographics, injury zone, and ophthalmologic exam data were collected retrospectively. OTSs and categories were determined. Orbital CT images were evaluated by a radiologist to determine decreased globe volume, globe wall irregularity, chorioretinal layer thickening, lens dislocation, asymmetric anterior chamber depth (ACD), abnormal vitreous density, and intraocular air presence. Results: This study included 30 eyes of 30 patients. The majority of patients (n = 20, 66.7%) had Zone I injuries. The most common CT findings were globe wall irregularity (53.3%) and asymmetric ACD (53.3%). No CT findings were observed in 10 patients (33.3%). Globe wall irregularity was the most frequent CT finding in the OTS II category, while asymmetric ACD was most frequent in the OTS IV category. All patients with decreased globe volume, lens dislocation, abnormal vitreous density, and ≥3 CT findings were in the OTS II category. A significant negative correlation was found between the number of CT findings and OTS (r = −0.644, p < 0.001), and a significant positive correlation was found between the number of CT findings and clinically measured wound size (r = 0.600, p < 0.001). Conclusions: CT findings help assess ocular trauma severity, but clinical examination remains essential for accurate diagnosis.

Considering the limited treatment options for acute lung injury (ALI) and pulmonary fibrosis (PF), ozone treatment may be promising as a new immunological agent with its ability to modulate cytokines and interferons. We aimed to investigate the effects of inhaled ozone therapy on both ALI and PF in rat models. A total of 48 albino Wistar male rats were included in the study. Lipopolysaccharide (LPS) was used to induce the ALI model, and bleomycin was used for the PF model. The effects of inhaled ozone (O3) were investigated using the ELISA method. Hematoxylin&eosin staining, Masson’s trichrome staining, and immunohistochemical methods were used for histopathological evaluation. The Interleukin-1 beta (IL-1β), Interleukin-6 (IL-6), Tumor Necrosis Factor-alpha (TNF-α), and Nuclear Factor kappa B subunit p65 (NF-κB p65) levels in the ALI + 0.08 ppm O3, ALI + 0.12 ppm O3, PF + 0.08 ppm O3, and PF + 0.12 ppm O3 groups statistically decreased to the same extent and approached the levels of control animals. It was observed that IL-1β, IL-6, TNF-α, and NF-κB p65 levels in lung tissues were significantly and dose-dependently decreased compared to the untreated PF and ALI groups, respectively. While fibrosis was severe in the PF + 0.08 ppm O3 group, it decreased to more moderate levels in the PF + 0.12 ppm O3 group. The cytokine levels confirmed that inhaled ozone protected the lungs from both ALI and the development of PF.

Objective: This study investigated the relationship between successful aging and life satisfaction in older adults presented to primary care. Methods: This descriptive and correlational study was conducted between January 2019 and May 2019. The sample consisted of 379 older adults (aged 65 and over) admitted to three family health centers (FHMs) in a provincial center in eastern Türkiye. Data were collected using a descriptive questionnaire, the Successful Aging Scale (SAS), and the Life Satisfaction Index (LSI). Results: Participants had a mean age of 70.87±5.61 years. They had mean SAS and LSI scores of 53.54±8.43 and 10.85±4.14, respectively. More than a quarter of the participants (38%) had high life satisfaction. There was a weak positive correlation between the SAS total and subscale scores and the mean LSI score (P<.001). Conclusion: Older adults had a good perception of successful aging. However, they had a moderate perception of life satisfaction. It was determined that as perceptions of life satisfaction increased, perceptions of successful aging also increased. Therefore, FHM nurses should counsel and support older adults to ensure successful aging and life satisfaction.

Objective: Gentamicin is an aminoglycoside antibiotic that impairs testicular and reproductive function. Apilarnil is an antioxidant-rich substance derived from drone larvae. In this study, the effects of apilarnil on gentamicin-induced testicular toxicity were investigated. Materials and Methods: The study used 35 rats, with 7 male rats randomly selected from each group. The rats were divided into five groups: control (Group 1), apilarnil (Group 2), gentamicin (Group 3), gentamicin + apilarnil 0.4 g/kg/day (Group 4), and gentamicin + apilarnil 0.8 g/kg/day (Group 5). All administrations were performed for 10 days. LC-HRMS assessment, oxidative stress, reproductive parameter analysis, and histological examinations were all conducted. Bacterial growth was evaluated by microbiological analysis. Results: The research results showed that gentamicin administration reduced total motility, increased the percentage of dead, abnormal, and DNA damaged spermatozoa, and decreased GSH, CAT, GPx, and SOD activity. Apilarnil treatment significantly increased SOD activity. MDA levels were significantly decreased in the apilarnil and low-apilarnil groups compared to the gentamicin group. Testicular histology was normal in both the control and apilarnil groups. Empty tubule lumens, vascular congestion, necrotic spermatids, and hyalinized interstitial tissues were seen in the gentamicin group, while the appearance improved in the high apilarnil group. Gentamicin administration increased 8-OHdG and caspase-3 immunopositivity. Coagulase negative Staphylococcus spp. Citrobacter spp., Escherichia coli, Enterococcus faecalis, Klebsiella pneumoniae, Pseudomonas aeruginosa, Staphylococcus aureus and Proteus spp. grew in the gentamicin group. Apilarnil administration decreased microorganism growth. Conclusion: Apilarnil administration increased sperm quality in gentamicin-induced testicular damage and had a positive impact on testicular histology, bacterial growth, and oxidative stress parameters.
