- MakaleAçık Erişim2024An Efficient Deep Learning Model for Prostate Cancer Diagnosis
Alici-Karaca, Demet; Akay, Bahriye
Prostate cancer is one of the most widespread and mortal cancers among men worldwide. Pathological evaluation is essential for the definitive diagnosis of prostate cancer, the symptoms of which are not evident during the early disease. During the pathological evaluation to determine the severity of cancer, Gleason grading, which is difficult to assign, is time-consuming, and the result may vary depending on the pathologist's experience. In order to make a fast and objective evaluation, deep learning systems have been recently used to automate prostate cancer diagnosis and grading. The inherent difficulties of detecting Gleason grades can be overcome with deep learning architectures, which present fast and precise classification rates. The performance of the methods proposed for Gleason grading in the literature is low, especially in multi-class problems, due to their model structures. Some mechanisms for better learning of critical representations are needed to determine the Gleason grading accurately. Therefore, in this study, a novel deep learning model called Eff4-Attn is proposed for detecting and grading prostate cancer. In this model, combining the Efficient Channel Attention (ECA) mechanism with EfficientNet-B4 distinguishes the tissues in the prostate cancer pathology images much better. In addition, the effect of data distribution in classes in the public DiagSet dataset on performance was investigated. Experimental studies have been carried out to perform three classification tasks for various magnification ratios: 8-class classification, 4-class classification, and 2-class classification for diagnosing prostate cancer and determining cancer severity, and promising results have been obtained. At 40x magnification ratio, the best result was achieved with 96.18% test accuracy in the 2-class classification task, that is, in detecting the presence of cancer. In determining the severity of cancer, at a 40x magnification ratio, a test accuracy of 94.86% was obtained. The proposed deep and light model has considerable potential to assist pathologists in detecting the characteristics of Gleason grades.

Pulmonology is one of the branches that frequently receive consultation requests from the emergency department. Pulmonology consultation (PC) is requested from almost all clinical branches due to the diagnosis and treatment of any respiratory condition, preoperative evaluation, or postoperative pulmonary problems. The aim of our study was to describe the profile of the pulmonology consultations received from emergency departments in Turkiye. A total of 32 centers from Turkiye (the PuPCEST Study Group) were included to the study. The demographic, clinical, laboratory and radiological data of the consulted cases were examined. The final result of the consultation and the justification of the consultation by the consulting pulmonologist were recorded. We identified 1712 patients, 64% of which applied to the emergency department by themselves and 41.4% were women. Eighty-five percent of the patients had a previously diagnosed disease. Dyspnea was the reason for consultation in 34.7% of the cases. The leading radiological finding was consolidation (13%). Exacerbation of preexisting lung disease was present in 39% of patients. The most commonly established diagnoses by pulmonologists were chronic obstructive pulmonary disease (19%) and pneumonia (12%). While 35% of the patients were discharged, 35% were interned into the chest diseases ward. The majority of patients were hospitalized and treated conservatively. It may be suggested that most of the applications would be evaluated in the pulmonology outpatient clinic which may result in a decrease in emergency department visits/consultations. Thus, improvements in the reorganization of the pulmonology outpatient clinics and follow-up visits may positively contribute emergency admission rates.
- MakaleAçık Erişim2024Novel beta-lactam substituted benzenesulfonamides: in vitro enzyme inhibition, cytotoxic activity and in silico interactions
Güleç, Özcan; Türkeş, Cüneyt; Arslan, Mustafa; Demir, Yeliz; Dincer, Busra; Ece, Abdulilah; Beydemir, Şükrü
In this study, a library of twelve beta-lactam-substituted benzenesulfonamides (5a–l) was synthesized using the tail-approach method. The compounds were characterized using IR, 1H NMR, 13C NMR and elemental analysis techniques. These newly synthesized compounds were tested for their ability to inhibit the activity of two carbonic anhydrases (hCA) isoforms, I and II, and acetylcholinesterase (AChE) in vitro. The results showed that the synthesized compounds were potent inhibitors of hCA I, with K Is in the low nanomolar range (66.60–278.40 nM) than the reference drug acetazolamide (AAZ), which had a K I of 439.17 nM. The hCA II was potently inhibited by compounds 5a, 5d–g and 5l, with K Is of 69.56, 39.64, 79.63, 74.76, 78.93 and 74.94 nM, respectively (AAZ, K I of 98.28 nM). Notably, compound 5a selectively inhibited hCA II with a selectivity of > 4-fold over hCA I. In terms of inhibition of AChE, the synthesized compounds had K Is ranging from 30.95 to 154.50 nM, compared to the reference drug tacrine, which had a K I of 159.61 nM. Compounds 5f, 5h and 5l were also evaluated for their ability to inhibit the MCF-7 cancer cell line proliferation and were found to have promising anticancer activity, more potent than 5-fluorouracil and cisplatin. Molecular docking studies suggested that the sulfonamide moiety of these compounds fits snugly into the active sites of hCAs and interacts with the Zn2+ ion. Furthermore, molecular dynamics simulations were performed for 200 ns to assess the stability and dynamics of each enzyme-ligand complex. The acceptability of the compounds based on Lipinski’s and Jorgensen’s rules was also estimated from the ADME/T results. These results indicate that the synthesized molecules have the potential to be developed into effective and safe inhibitors of hCAs and AChE and could be lead agents. Communicated by Ramaswamy H. Sarma.

Huyut, Zübeyir; Uçar, Bünyamin; Yıldızhan, Kenan; Altındağ, Fikret; Huyut, Mehmet Tahir
This study investigated whether abemaciclib (ABE) administration had any adverse effects on ovarian and sex hormones in female rats, and the protective effect of curcumin. Forty female rats were equally divided into the sham control, DMSO, curcumin (CMN), ABE, and ABE+CMN groups. Pharmaceuticals were administered by gavage daily for 28 days. Serum sex hormones were measured in an autoanalyzer operating with a microparticle immunoassay method. In addition, histopathological examination and 8-OHdG expression were performed on the ovarian tissue. Progesterone and testosterone levels were significantly decreased, while estradiol levels were significantly increased, in the ABE group compared to the sham and DMSO groups. In addition, there were significant differences in sex hormone levels in the CMN and/or CMN+ABE groups compared to the ABE group. There was decreased expression of 8-OHdG in the ABE+CMN group compared to the ABE or CMN only groups. This study exhibited that ABE administration can adversely affect functions and histology of the ovarian tissue, but CMN therapy may be protective against the adverse effects on ovarian in ABE-induced rats.

- MakaleYalnız künye2024The non-steroidal anti-inflammatory drugs based on Zinc(II) picoline complexes: Synthesis, characterization, and evaluation of antiproliferative effects against human breast adenocarcinoma MDA-MB-453 cells by inducing ROS-mediated apoptosis and down-regulating PI3K/AKT signaling pathway
Caglar, Sema; Polat, Tuğba; Altay, Ahmet; Yeniçeri, Esma; Caglar, Bülent
This work aims to synthesize zinc(II) non-steroidal anti-inflammatory complexes and to evaluate their anticancer abilities on different cancer cell lines. In this context, four new zinc(II) complexes including the NSAIDs mefenamic acid or niflumic acid and picoline derivatives {[Zn2(μ-mef)3(3-pic)2] 1, [Zn3(μ-mef)6(2-pic)2] 2, [Zn3(μ-mef)6(4-pic)2] 3 and [Zn(nif)2(4-pic)2]} 4 were synthesized. The molecular structures were identified by elemental analysis, FT-IR, 1H NMR (complexes 2 and 3), UV–Vis, thermal analysis, and X-ray diffraction (complexes 1 and 4), indicating that one complex was mononuclear, one complex was binuclear, and two complexes were trinuclear. The deprotonated mefenamato ligands bind to the zinc(II) ion via carboxylate oxygen atoms and chelate in a bridging bidentate asymmetric form. The deprotonated niflumato ligand acts as a monodentate ligand. UV–Vis spectroscopy was used to assess the complexes' stability. The stoichiometry of the synthesized complexes was determined by 1H NMR and thermal studies. XTT assays showed that all complexes exhibited dose-dependent antiproliferative effects against breast cancer MCF-7 and MDA-MB-453 cells, colon cancer HT-29 cells, and lung cancer A549 cells. The highest cytotoxicity was exhibited by complex 3 against MDA-MB-453 cells with an IC50 value of 8.28 µM. The mechanistic studies performed by flow cytometry revealed that complex 3 exerted its in vitro anticancer activity against MDA-MB-43 cells by inducing apoptosis, increasing ROS production, and inhibiting the PI3K/AKT signaling pathway.

- MakaleAçık Erişim2024Predictors of anastomotic leak and conduit necrosis after oesophagectomy: Results from the oesophago-gastric anastomosis audit (OGAA)
Griffiths, Ewen A.; Halle-Smith, J.M.; Kamarajah, S.K.; Evans, R.P.T.; Nepogodiev, D.; Hodson, J.; Bundred, J.R.; Gockel, I.; Gossage, J.A.; Isik, A.
Background: Both anastomotic leak (AL) and conduit necrosis (CN) after oesophagectomy are associated with high morbidity and mortality. Therefore, the identification of preoperative, modifiable risk factors is desirable. The aim of this study was to generate a risk scoring model for AL and CN after oesophagectomy. Methods: Patients undergoing curative resection for oesophageal cancer were identified from the international Oesophagogastric Anastomosis Audit (OGAA) from April 2018–December 2018. Definitions for AL and CN were those set out by the Oesophageal Complications Consensus Group. Univariate and multivariate analyses were performed to identify risk factors for both AL and CN. A risk score was then produced for both AL and CN using the derivation set, then internally validated using the validation set. Results: This study included 2247 oesophagectomies across 137 hospitals in 41 countries. The AL rate was 14.2% and CN rate was 2.7%. Preoperative factors that were independent predictors of AL were cardiovascular comorbidity and chronic obstructive pulmonary disease. The risk scoring model showed insufficient predictive ability in internal validation (area under the receiver-operating-characteristic curve [AUROC] = 0.618). Preoperative factors that were independent predictors of CN were: body mass index, Eastern Cooperative Oncology Group performance status, previous myocardial infarction and smoking history. These were converted into a risk-scoring model and internally validated using the validation set with an AUROC of 0.775. Conclusion: Despite a large dataset, AL proves difficult to predict using preoperative factors. The risk-scoring model for CN provides an internally validated tool to estimate a patient's risk preoperatively.

Gözcü, Sefa; Akşit, Zeynep; Şimşek, Samed; Kandemir, Ali; Aydin, Ali; Yilmaz, Mustafa Abdullah; Akşit, Hüseyin
The study aimed to comprehensively identify the phenolic fingerprint of Morina persica L. (Caprifoliaceae) and evaluate its various biological activities. LC-MS/MS analysis of 70% Morina persica methanol extract revealed the presence of 27 phytochemicals, with quinic acid, chlorogenic acid, and rutin being the major phenolics. The antioxidant, antibacterial, antiproliferative, and cytotoxic activities were evaluated for biological screening. The results showed that the methanolic extract of M. persica has a moderate DPPH radical scavenging and ferric-reducing capacity, indicating antioxidant activity. M. persica was observed to have a sufficient antiproliferative effect against cancer cells and low cytotoxicity against normal cells. Moreover, M. persica demonstrated good antibacterial activity against Clostridium perfringens, Enterococcus faecalis, and Escherichia coli. These data suggest that the methanolic extract of M. persica could be considered both an industrial source of quinic acid and a potential biologically active ingredient for developing drug formulations.

Aim: We aimed to investigate the preventive effects of carvacrol against ketamine-induced cardiotoxicity biochemically and histopathologically in an experimental model. Material and method: The rats were divided into three groups; healthy control (HC), ketamine alone (KG), and ketamine + carvacrol (KCG) groups. Serum Creatine Kinase Myocardial Band (CK-MB) and Troponin I (TP I) levels were determined. Malondialdehyde (MDA), Glutathione (GSH), Superoxide Dismutase (SOD), Tumor Necrosis Factor α (TNF-α), Interleukin 1 beta (IL-1beta), and Interleukin 6 (IL-6) levels were measured in the heart tissues of the rats. Heart tissues were also evaluated histopathologically. Results: In the ketamine-treated group, tissue MDA, TNF-α, IL-1beta, and IL-6 levels increased while tissue GSH and SOD levels decreased significantly compared with the control group. However, in the ketamine plus carvacrol applied group, all those alterations were significantly less pronounced, close to the healthy controls. Severe mononuclear cell infiltrations, degenerated myocytes and hemorrhage were determined in the ketamine alone administered group, and these alterations were at a mild level in the carvacrol + ketamine administered group. Conclusion: Prolonged exposure to ketamine resulted in induced oxidative stress in rat heart tissue; concomitant carvacrol application could counteract the negative effects of ketamine by protecting tissues from lipid peroxidation and decreasing the inflammatory response.

The mean total scores of the mothers were 26.71 ± 8.80 for the Zarit Caregiving Burden Scale and 27.00 ± 11.35 for the Perceived Stress Scale. There is a difference between the mothers' age, education level, income level and the type of disability of their child and the Zarit Caregiving Burden scale score average; It was determined that there was a significant difference between the place of residence and the number of disabled children they had and the average score of the Perceived Stress Scale. A positive correlation was found between Zarit Caregiving Burden Scale scores and Perceived Stress Scale scores of the mothers (p < 0.05). Application to practice: The study revealed that mothers with children with special needs experienced higher levels of stress as their care burden increased. Providing economic assistance and informing and training mothers about the care of children with special needs were recommended to reduce the care burden of mothers with children with special needs.
- MakaleAçık Erişim2024Effects of Nettle (Urtica dioica) Extract on Versus Pathogenic Microorganisms in Yogurt Production; [Yoğurt Üretiminde Isırgan Otu (Urtica dioica) Ekstraktının Patojenik Mikroorganizmalara Karşı Etkileri]
Gülhan, Barış; Yangilar, Filiz
Contamination with pathogenic bacteria is the most common cause of foodborne illness and represents a public health problem worldwide. These pathogens can be controlled by adding extracts to fermented milk. In the present study, the effects of nettle extract on selected pathogenic bacteria in yogurt production were investigated. For this purpose, the antibacterial effectiveness of the extract additives in the presence of bacteria was examined and compared. Yoghurt samples, 24 different samples contaminated with 7 standard pathogen strains, were analyzed for chemical properties (pH and acidity) and antimicrobial activity after 1, 7, 14 and 21 days of storage at 4 °C. In particular, it was found that the decrease in pH and increase in acidity in the nettle extract samples after 21 days were significantly greater than in natural yogurt and yogurt samples containing nettle extract (0.5% and 1%, respectively) (p<0.05). Pathogens were reduced more significantly in samples treated with nettle extracts, which have antibacterial activity against pathogens. In particular, compared to natural yogurt, these samples showed significant antibacterial effects on the standard strains Streptococcus pneumoniae ATCC 45615, Klebsiella pneumoniae ATCC 70063 and Pseudomonas aeruginosa ATCC 27853 (p<0.01). Furthermore, in the samples containing nettle extract, the 1% concentration had synergistic effects on Streptococcus pneumoniae ATCC 45615, while the 0.5% concentration had synergistic effects on Klebsiella pneumoniae ATCC 70063 and Pseudomonas aeruginosa ATCC 27853. Overall, these results suggest that the nettle extract could be used as a natural preservative to improve the safety of yogurt and other dairy products.

Objectives: This study aimed to evaluate the diagnostic tests and treatments applied in patients with multisystem inflammatory syndrome in children (MIS-C) and to determine the effect of the disease on health costs. Patients and methods: This retrospective cohort study included 59 MIS-C patients (40 males, 19 females; mean age: 7.7±4.2 years; range, 4 months to 16.5 years) who were admitted and treated between April 1, 2020, and November 1, 2021. Demographic and clinical features with hospital costs and length of stay were retrospectively reviewed from the medical files and computerized system of the hospital. Direct medical care costs of items were calculated with the hospital perspective using a combination of microcosting technique (resource-based accounting method) and hospital list data. Cases were classified as mild, moderate, or severe, and the patients were divided into two groups: the mild group and the moderate-severe group. Classification was determined by the vasoactive inotropic score (VIS), degree of respiratory support, and evidence of organ damage. Results: The mean age of the cases in the mild group was 6.5±3.7 years, and the mean age of the cases in the moderate-severe group was 9.2±4.3 years. Of 59 patients, 19 (32.2%) were followed up in the pediatric intensive care unit. The median duration of hospitalization in the hospital was 8 (interquartile range: 7-12) days. The total cost of the patients hospitalized with the diagnosis of MIS-C during the study period was 849,242.93$, and the mean cost per patient was 14,393.94±9,631.92$. In the distribution of the total cost of hospitalization according to expenses, the highest rate was pharmacy and blood products (51.99%) and IVIG costs (43.99%). While the mean total cost per person was 13,682.87±8,799.63$ in mild cases, it was 16,433.82±9,440.02$ in moderate-severe cases, and no statistically significant relationship was found between the two groups (p>0.05). There was no difference in the mean cost per patient between the cases with and without heart, lung, kidney, or neurologic involvement and advanced respiratory support (p>0.05). There was a strong positive correlation between the total costs and age (r=0.883, n=59, p<0.0001), with increased amount of costs with increased age. Conclusion: In the study, no statistically significant correlation was found between the total cost of per person in the mild group and the moderate-severe group (p>0.05). This finding may be due to the wide use of IVIG in MIS-C treatment, in addition to low transfer rates to pediatric intensive care units due to high-flow nasal cannula usage.

The present study was designed to evaluate whether AuNPs (gold nanoparticles) synthesized with the Cynara scolymus (CS) leaf exert protective and/or alleviative effects on arsenic (As)-induced hippocampal neurotoxicity in mice. Neurotoxicity in mice was developed by orally treating 10 mg/kg/day sodium arsenite (NaAsO2) for 21 days. 10 µg/g AuNPs, 1.6 g/kg CS, and 10 µg/g CS-AuNPs were administered orally simultaneously with 10 mg/kg As. CS and CS-AuNPs treatments showed down-regulation of TNF-α and IL-1β levels. CS and CS-AuNPs also ameliorated apoptosis and reduced the alterations in the expression levels of D1 and D2 dopamine receptors induced by As. Simultaneous treatment with CS and CS-AuNPs improved As-induced learning, memory deficits, and motor coordination in mice assessed by water maze and locomotor tests, respectively. The results of this study provide evidence that CS-AuNPs demonstrated neuroprotective roles with antioxidant, anti-inflammatory, and anti-apoptotic effects, as well as improving D1 and D2 signaling, and eventually reversed neurobehavioral impairments.
- MakaleAçık Erişim2024A detailed look in radioanatomical aspects of ligamentum arteriosum calcification in pediatric population
Aydemir, Huseyin; Fatihoglu, Erdem; Kizilgoz, Volkan; Kahraman, Sevket; Yazar, Mesut F.; Aydin, Sonay; Kantarci, Mecit
After birth, ductus arteriosus forms ligamentum arteriosum and sometimes can be calcified. Ligamentum arteriosum calcification can be seen as subtle-punctate or coarse-linear between pulmonary artery and the aorta in the non-contrast computed tomography (CT) images, and between the pulmonary conus and the aortic knob in the chest radiography. In this study, we aim to estimate the prevalence of ligamentum arteriosum calcification on non-contrast CT and chest radiography in a pediatric population. Patients aged 0-18 years, who underwent non- contrast CT and chest radiography at Department of Radiology between March 1, 2020 and October 31, 2021 were evaluated retrospectively. It was examined whether there was ligamentum arteriosum calcification in non-contrast CT and chest radiography. If there is calcification, it was recommended as subtle-punctate or coarse-linear, and information was documented by grouping the patients’ age. If ligamentum arteriosum calcification was present, it was divided morphologically into two groups (subtle-punctate and coarse-linear) and classified according to the age groups. The research population consisted of 1003 patients. Non-contrast CT revealed calcification in 25.5% of patients (n=256). 11.3% of patients had subtle-punctate calcification, and 14.3% had coarse-linear calcification. Radiography had a 1.7% (n:17) detection rate for calcification. Ligamentum arteriosum calcification is a common finding in all age groups in the pediatric population. When reporting pediatric CT studies, radiologists should be able to distinguish ligamentum arteriosum calcification from other pathological and anatomical conditions and accept it as a normal finding in order to avoid unnecessary further investigations.

Background: Hemodialysis is one of the treatment methods for chronic kidney disease, which is a common disease around the world. The problems that occur during the hemodialysis process may cause discomfort in patients. Therefore, it is important to regularly evaluate the concept of comfort in hemodialysis patients. Aim: To determine the comfort level of patients undergoing hemodialysis and the associated factors. Methods: This study was a descriptive cross-sectional study conducted among 95 patients who had been undergoing hemodialysis for at least 6 months. Data were collected using the sociodemographic characteristics form and the Hemodialysis Comfort Scale (HDCS). Results: The mean age of the participants was 58.37 ± 16.62 years. The median duration of hemodialysis was 5 (1–25) years. A total of 51% of the patients were male, 54.7% were married, 34.7% had completed primary school, and 85.3% had a comorbid chronic disease. The mean hemodialysis comfort score was 23.85 ± 6.93. The mean score was significantly higher in male patients (P = 0.041) and those without comorbid chronic disease (P = 0.013). There was a significant negative correlation between the age of hemodialysis patients and the mean hemodialysis comfort score (r = -0.260, P = 0.011). Conclusion: The comfort level was significantly better in hemodialysis patients who were male, those without comorbid disease, and those who were younger. There is a need to periodically assess the comfort level of hemodialysis patients and intervene when necessary in order to improve their quality of life.

Polygonum cognatum Meisn. (Polygonaceae) is used both as food and as a folk medicine to treat diabetes. This study aimed to evaluate the effect of the extracts, along with isolated compounds, from P. cognatum aerial parts on diabetes. In vitro studies were conducted using an α-glucosidase inhibitory assay, while in vivo antidiabetic studies were carried out on streptozotocin-induced diabetic rats. Effective extracts were subjected to isolation studies, and structures of the compounds were elucidated by spectroscopic methods. The ethyl acetate and n-butanol extracts had the highest effect in both in vitro and in vivo experiments. They also decreased aspartate transaminase, alanine transaminase and malondialdehyde levels, while increasing glutathione and superoxide dismutase activity in rats. From the active extracts, 11 phenolic compounds were isolated and characterized. Among the isolated compounds, quercetin was found to be the most active according to α-glucosidase inhibitory activity studies. This study provided scientific evidence for the traditional use of P. cognatum as a folk medicine for treating diabetes. The findings suggest that the ethyl acetate and n-butanol extracts, as well as quercetin, have the potential for development as antidiabetic agents.
