- MakaleAçık Erişim2024Association between the most superior point of patella-entrance of femoral trochlea distance ratio (SP-ET index) and chondromalacia patella: an investigation via magnetic resonance imaging
Kızılgöz, Volkan; Aydın, Sonay; Kantarcı, Mecit; Durmaz, Önder
Purpose: In this study, the effect of the most superior point of patella-entrance of femoral trochlea distance ratio (SP-ET index) on chondromalacia patella (CP) was investigated with 2 reviewers. Material and methods: A total of 348 knees of 308 patients were analysed retrospectively with magnetic resonance imaging (MRI). Patients with or without CP constituted the study and the control groups in this cross-sectional investigation. Two reviewers interpreted the dataset regarding the SP-ET index. This ratio was calculated as the distance between the most superior point of patella and the entrance of femoral trochlea (β) divided by the patellar articular surface length (α). The relationship between the SP-ET index and CP was presented using independent samples T-tests, and the intraclass correlation coefficient (ICC) was calculated to reveal the interobserver differences. Results: There was excellent agreement between the reviewers regarding α, β, and SP-ET values (ICC was 0.971, 0.964, and 0.943, respectively). Higher SP-ET values were obtained for patients with CP, in comparison with patients without any chondral lesion (p < 0.001). A significant, positive, and moderate level of correlation was revealed between SP-ET measurements and CP grades for the total study population. Conclusions: SP-ET index showed high interobserver agreement and indicated a significant difference between patients with and without CP. Both reviewers’ results indicated positive and significant correlation between the measured SP-ET values and different grades of CP for females, males, and the total study population.

AIM: To investigate the effect of lacidipine, thiamine pyrophosphate (TPP) and the combination of lacidipine and TPP against oxidative and inflammatory eye damage induced by bilateral common carotid artery ligation in rats.. METHODS: Male albino Wistar rats were categorized as those who underwent sham surgery (SG), right and left common carotid cross-clamping and unclamping procedure (CCU), lacidipine+CCU (LCCU), TPP+CCU (TCCU), and combination of lacidipine and TPP (LTC)+CCU (LTCCU). One hour before anesthesia, the LCCU (n=6) received lacidipine (4 mg/kg, orally) and the TCCU (n=6) received TPP (20 mg/kg, intraperitoneally). The SG (n=6) and CCU (n=6) received the same volume of distilled water from the same route. After anesthesia (60 mg/kg ketamine, intraperitoneally), the necks of the rats were opened in the midline. Ischemia was created for 10min by placing clips on the right and left common carotid arteries. Rats in the SG only underwent subcutaneous incision. After 10min, the clips were removed and reperfusion was achieved for six days. Then, the animals were euthanized (120 mg/kg ketamine, intraperitoneally) and the levels of oxidant, antioxidant and proinflammatory cytokines in the eye tissues were determined. The retinal tissue of the eye was also examined histopathologically.. RESULTS: Lacidipine, TPP, and LTC significantly prevent the increase in malondialdehyde, tumor necrosis factor-alpha, interleukin-1β (IL-1β), and IL-6 levels, decrease in total glutathione levels, superoxide dismutase and catalase activities and histopathological retinal damage in eye tissue induced by bilateral common carotid artery ligation in rats. The impact of these drugs on protection is determined to be LTC>lacidipine>TPP.. CONCLUSION: As a result of the study, it is concluded that LTC may be more effective than lacidipine and TPP alone in treating ocular ischemic syndrome.

The development of multifunctional nanoformulations with enhanced anticancer activity and drug delivery potential is of increasing scientific importance. In this study, we synthesized and characterized copper oxide nanoparticles mediated by oolong tea extract (OE-CuO/Cu2O NPs) using a variety of techniques, including TEM, XRD, FTIR, and UV–Vis spectroscopy, to assess their physical and chemical properties. Remarkably, the results revealed that OE/CuO NPs exhibited significantly enhanced anticancer activity compared to OE alone. The effects of OE and OE-CuO/Cu2O NPs were on cytotoxicity, apoptosis, and cell cycle progression. Furthermore, we assessed the impact of these nanostructures on CD44/24 antibodies, known as aggressive cell markers, following treatment. The green OE-CuO/Cu2O NPs displayed remarkable inhibitory effects on colon cancer HCT116 and breast cancer cells MCF-7 cells. It was shown that OE-CuO/Cu2O NPs reduced the production of the CD44 antibody, provided cell cycle arrest, and encouraged apoptosis. More specifically, it was discovered that these NPs caused MCF-7 cells to undergo apoptosis and G2/M phase cell cycle arrest. Additionally, drug delivery investigations were conducted to evaluate the potential of OE-CuO/Cu2O NPs as drug nanocarriers, further underscoring their promise in developing novel anticancer agents with significant cytotoxic, apoptotic, and drug delivery properties.

Haspolat, Namık Kemal; Ağırkan, Murat; Eşkisu, Mustafa
Psychological problems are prevalent among adolescents, and previous studies have robustly demonstrated that individual and family factors play a role in the formation and maintenance of these problems. On the other hand, not all adolescents show the same characteristics in terms of these factors and can be categorized into different groups. Identifying this heterogeneity is also important in intervention for psychological problems. Thus, this study aimed to identify patterns of parental attitudes, basic psychological needs, self-regulation, and alexithymia, using latent class analysis (LCA), and investigated how different patterns of these individual and family factors are associated with psychological problems. Participants included a sample of 331 volunteer Turkish adolescents (140 male [42.3%], 191 female [57.7%]). LCA indicated a two-class solution fit the current data well and 65% of the participants were classified into the first latent class which was labeled as “positive individual and family context” (PIFC). The PIFC were found to have higher levels of parental sensitivity and autonomy support, need satisfaction and self-regulation, but lower levels of mother's psychological and behavioral control and father's behavioral control, need frustration, and alexithymia. The second latent class, labeled as “negative individual and family context” (NIFC), included 35% of the adolescents. Adolescents in NIFC had low levels of parental sensitivity, autonomy support, satisfaction of basic psychological needs, and self-regulation, but high levels of mother's psychological and behavioral control, father's behavioral control, frustration of basic psychological needs, and alexithymia. Moreover, after controlling gender and age, adolescents classified in the NIFC displayed significantly higher levels of psychological problems than those classified in the PIFC. These findings support the idea that individual and familial factors interact to affect adolescents' mental health and suggest that clinicians and helping professionals would benefit from including these factors in intervention processes.

Muca Yiğit, Belkıs; Gözcü, Sefa
Objective: This study was conducted to systematically document the use of plants, plant parts and preparation methods used by people in eight districts and sixty villages in Ağrı province. Material and Method: The medicinal plant species employed by the indigenous population for therapeutic purposes were systematically gathered and identified. Comprehensive data on traditionally utilized information were compiled, and herbarium materials were prepared. These materials have been deposited at the Iğdır National Wild Life Museum (INWM), affiliated with Iğdır University. Result and Discussion: This research identified a total of 58 taxa of medicinal plants belonging to 31 families. Among these, 50 species were found to grow naturally, while 8 species were cultivated. The predominant plant families included Asteraceae (8), Apiaceae (4), Lamiaceae (4), and Rosaceae (4). Infusion emerged as the most widely employed preparation method. The practice of traditional medicine remained prevalent among the population in Ağrı. Nevertheless, with the increasing availability of health services in the region, herbal medicine appeared to be more closely associated with healthcare and illness prevention than with curative purposes. The influx of new immigrants also contributed to the erosion of traditional knowledge. Notably, there is a discernible decline in traditional knowledge regarding the use of medicinal plants, both among younger generations and due to migration. Furthermore, this research serves as a foundational resource for prospective scientific inquiries aimed at the development of novel commercial drugs derived from plant sources.

Fourteen novel Chalcone-Cu complexes were effectively synthesized in this work. The newly synthesized Chalcone-Cu complexes were assessed for their effects on human carbonic anhydrase isoenzymes I and II, acetylcholinesterase enzymes, and antioxidant activity. The intricate compounds exhibited Ki values ranging from 41.65–190.42 nM against hCA I, 15.79–259.07 nM against hCA II, and 14.36–175.73 nM against AChE enzymes. These complexes demonstrated potent inhibitory profiles against the specified metabolic enzymes, surpassing the inhibitory effects of acetazolamide (for hCA I and II) and tacrine (for AChE). The antioxidant properties of the compounds were assessed using DPPH and ABTS radical scavenging assays, revealing that the complexes had moderate to high efficacy in neutralizing free radicals. All complexes underwent molecular docking experiments. Compounds 14, 22, and 23 yielded the highest docking scores. Compound 14 demonstrated a docking score of −6.414 kcal/mol against hCAI, whereas compound 23 attained a docking score of −6.697 kcal/mol against hCA II. Compound 22 exhibited the most favorable docking score of −9.645 kcal/mol against AChE. The acquired results have the potential to help towards the development of new drugs containing Cu complex structures for the treatment of prevalent ailments such as glaucoma and Alzheimer's diseases.

Oksuz, Mehmet Kursat; Satoglu, Sule Itir
Purpose: Disaster management and humanitarian logistics (HT) play crucial roles in large-scale events such as earthquakes, floods, hurricanes and tsunamis. Well-organized disaster response is crucial for effectively managing medical centres, staff allocation and casualty distribution during emergencies. To address this issue, this study aims to introduce a multi-objective stochastic programming model to enhance disaster preparedness and response, focusing on the critical first 72 h after earthquakes. The purpose is to optimize the allocation of resources, temporary medical centres and medical staff to save lives effectively. Design/methodology/approach: This study uses stochastic programming-based dynamic modelling and a discrete-time Markov Chain to address uncertainty. The model considers potential road and hospital damage and distance limits and introduces an a-reliability level for untreated casualties. It divides the initial 72 h into four periods to capture earthquake dynamics. Findings: Using a real case study in Istanbul’s Kartal district, the model’s effectiveness is demonstrated for earthquake scenarios. Key insights include optimal medical centre locations, required capacities, necessary medical staff and casualty allocation strategies, all vital for efficient disaster response within the critical first 72 h. Originality/value: This study innovates by integrating stochastic programming and dynamic modelling to tackle post-disaster medical response. The use of a Markov Chain for uncertain health conditions and focus on the immediate aftermath of earthquakes offer practical value. By optimizing resource allocation amid uncertainties, the study contributes significantly to disaster management and HT research.
- Derleme MakaleAçık Erişim2024Radon, Concrete, Buildings and Human Health—A Review Study
Bulut, H. Alperen; Şahin, Remzi
A comprehensive evaluation of the results obtained according to the measurement of radon gas in buildings and concrete, which is the most consumed material in the world after water, in accessible studies carried out in the last 40 years is the main objective of this study. The paper additionally aims to address the gap in the literature by comparatively determining which parameters affect radon–concrete and radon–building relationships. The scientific knowledge compiled within the scope of this article was presented under the main headings of radon and radon gas measurements in concrete and buildings. Radon gas, also known as the “invisible killer”, is considered the second most important cause of lung cancer after smoking (the gas is responsible for 3–14% of lung cancer cases in the world). The results determined that radon concentration limits have been applied in the range of 100–400 Bqm−3 in houses and 100–3700 Bqm−3 in workplaces. Studies conducted on the exhalation rate of radon showed that the radon exhalation rate of concrete may be in the range of 0.23–510 Bqm−2 h−1. The results of indoor radon concentration measurements revealed that values between 4.6 Bqm−3 and 583 Bqm−3 were obtained. Despite the existing literature, some researchers state that there is an urgent need for an improved and widely accepted protocol based on reliable measurement techniques to standardize measurements of the radon exhalation rate of construction materials and the indoor radon concentration of buildings.

Objective: Based on the idea that there may be a link between smartphone addiction and adult separation anxiety disorder (ASAD), our aim was to examine the impacts of ASAD symptoms on nomophobia and whether they predict nomophobia among ASAD patients. Method: We randomly recruited 50 patients diagnosed with ASAD and 50 control subjects satisfying the inclusion criteria. We collected the data using a sociodemographic information form, the Adult Separation Anxiety Questionnaire (ASA-27), the Nomophobia Questionnaire (NMP-Q), the Beck Depression Inventory (BDI), and the Beck Anxiety Inventory (BAI). Results: Fifty-four percent of the patients had moderate, and 46% showed mild nomophobia symptoms. The results revealed that, compared to healthy controls, the patients had significantly higher scores on the ASA-27, the BDI, the BAI, the NMP-Q (total), the NMP-Q not being able to access information, the NMP-Q giving up convenience, the NMP-Q not being able to communicate, and the NMP-Q losing connectedness (p=0.006 for the NMP-Q giving up convenience; p<0.001 for others). Moreover, the results yielded significant positive relationships between ASAD and the participants' nomophobia total and subscale scores (except for losing connectedness) (p<0.05). Finally, ASAD scores significantly predicted nomophobia, not being able to access information, giving up convenience, and not being able to communicate. Discussion: To the best of our knowledge, our study is the first to report nomophobia levels among patients diagnosed with ASAD. The increased severity of separation anxiety symptoms contributed to the severity of nomophobia in the patients, which, in turn, significantly boosted the severity of their depression and anxiety.

Aydoğdu, Fatih; Gürsoy, Figen; Aral, Neriman
Problems related to social, emotional and psychological well-being in children at an early age may cause behavioural problems. Previous studies show that the development of emotion regulation strengthens social-emotional well-being, and psychological resilience and reduces behavioural problems in children. This study aims to test the possible mediating role of emotion regulation and to gain a better perspective on the association between social and emotional well-being, resilience, and behavioural problems. 250 children aged 4-6 years (122 boys, 48.8%; 128 girls, 51.2%; mean age = 4.93) studying in kindergartens in Turkey were included in the study. Data on children's behavioural problems and emotion regulation skills were obtained from mothers, and data on social-emotional wellbeing, and psychological resilience levels were obtained from teachers. Social and emotional wellbeing, and psychological resilience indirectly significantly predicted behavioural problems through emotion regulation. Emotion regulation has a partial mediator role in the relationship between social and emotional well-being, psychological resilience and behavioural problems. In line with the results, the importance of supporting the development of emotion regulation skills in the preschool period was emphasized.

In this experimental study we aimed to investigate the biochemical and histopathological effects of concomitantly administered taxifolin on tramadol-induced liver damage in rats. The rats were divided into three groups; control group (CG), tramadol alone (TRG), and taxifolin + tramadol given (TTRG) groups. Malondialdehyde (MDA), total glutathione (tGSH), total oxidant status (TOS), total antioxidant status (TAS), nuclear factor-kappa beta (NF-kB), tumor necrosis factor-α (TNF-α), and interleukin-1β (IL-1β) levels were measured in liver tissues. Liver tissues were also examined histopathologically. Alanine aminotransferase (ALT) and aspartate aminotransferase (AST) activities were determined in blood samples. In tissue analyses, determinants of oxidative stress and inflammation, all were significantly higher in the TRG group compared with the control and TTRG groups. In the TTRG group, all oxidative stress and inflammation markers were significantly lower than in the TRG group. In addition, there was not any significant difference between the control and TTRG groups regarding the TOS and TAS status. Serum liver enzymes were also significantly higher in the TRG group than in the other two groups. In histopathological examinations, the control group had a normal histological appearance. Degenerative-necrotic hepatocytes and hemorrhage, which were seen at a severe level in the TRG group, were found to be moderate in the treated TTRG group. In addition, mononuclear cell infiltrations were found to be severe in the TRG group and mild in the treated TTRG group. Finally it was concluded that Taxifolin alleviated the toxic effects of tramadol on the liver including the histopathological and biochemical changes as well as the oxidative damage.

Kacar, Mihrican; Alim, Nural Erzurum
Objective: the aim of this study is to evaluate the relationship between serum vitamin D and B12 levels, nutritional levels, depression, and anxiety in adult cancer patients before and after chemotherapy. Methods: a case-controlled study was carried out on 44 patients who were diagnosed with cancer and applied to the Chemotherapy Unit for treatment (patient group, PG) and 44 volunteer individuals (control group, CG) with similar characteristics to the age and gender-matched patient group but with no diagnosis of cancer. Results: the average age of individuals in PG is 52.50 ± 12.21 years and for those in CG is 52.84 ± 10.98 years. Serum D and B12 levels in the first cure in individuals in PG are higher than in the last treatment (p > 0.05). It was determined that vitamin C taken with a daily diet reduces the risk of cancer (OR: 0.920, 95 % CI: 0.899-0.942, p = 0.042). No correlation was found between depression and anxiety scores of both groups and serum vitamin D and B12 levels (p > 0.05). It was determined that the Beck Anxiety Inventory (BAI) score increased with decrease in body mass index (BMI) (β = 0.311, p = 0.040) and serum vitamin B12 level (β =-0.406, p = 0.006). In addition, it was found that the increase in the Patient-Generated Subjective Global Assessment (PG-SGA) score, which reflects the nutritional status of cancer patients, worsened the level of anxiety (β = 0.389, p = 0.009). Conclusions: as stated in the findings of the study, chemotherapy treatment mediated the development of anxiety in cancer patients by changing the vitamin B12 levels and anthropometric characteristics with its negative effect on nutritional status. It should be ensured that cancer patients treated with chemotherapy follow a healthy and balanced diet plan that is suitable for their needs and has adequate vitamin and mineral content.

Ozdemir, Gokhan; Eren, Alper; Aygul, Recep; Eren, Fettah; Kizildag, Nazım; Kocaturk, Idris; Mammadi, Azer; Ersoy, Ayse Nur; Korez, Muslu Kazım
Introduction: Endovascular treatment (ET) is a beneficial treatment for M1–2 occlusions of the middle cerebral artery. Mortality and disability rates are high if large vessel occlusions are not treated. While these rates are lower in M3 occlusions, important branch blockages can lead to disability. Endovascular treatment of small vessel occlusions is difficult, and there are no studies on the effectiveness of endovascular treatment for M3 occlusions. Accordingly, in this study, our aim was to assess the feasibility, safety, and preliminary efficacy of endovascular therapy for M3 occlusions. Methods: This study involved a retrospective analysis of a prospectively collected from two centres for acute ischemic stroke of the anterior system between July 2015 and April 2020. Demographic, radiologic, procedural and outcome variables were collected for patients who underwent endovascular therapy for acute ischemic stroke of the anterior system. Results: Complete or near complete reperfusion (mTICI 2b-3) of the M3 occlusion was achieved in 15 cases (38.5%). Complete (mTICI 3) reperfusion was achieved in 24 cases (61.5%). Twenty-six patients were treated for primary M3 occlusion, while 13 patients with M3 occlusion were treated as a rescue strategy after successful treatment of a proximal greater vascular occlusion. Complete or near complete reperfusion (mTICI 2b-3) of primary occlusion was achieved in eight cases (30.8%), and complete (mTICI 3) reperfusion was achieved in 18 cases (69.2%). In addition, complete or near complete reperfusion (mTICI 2b-3) of rescue M3 occlusion was achieved in seven cases (%53.8), while complete (mTICI 3) reperfusion was achieved in six cases (46.2%). Only one patient with primary M3 occlusion had ICH due to extravasation. The patient's neurological examination one month later was normal. Conclusions: This retrospective study demonstrates that endovascular treatment of M3 occlusions is safe, effective and reliable.
- MakaleYalnız künye2024The effect of doping different amounts of boron on the corrosion resistance and biocompatibility of TiO2 nanotubes synthesized on SLM Ti6Al4V samples
Acar, M.T.; Çomaklı, O.; Yazıcı, M.; Arslan, M.E.; Yetim, A.F.; Çelik, A.
This comprehensive study investigates the corrosion and biocompatibility performances of boron-doped TiO2 nanotubes (TNT) synthesized on Ti6Al4V alloy produced by the selective laser melting (SLM) process. Structural analysis reveals a consistent reduction in nanotube diameter across varying boron doping levels (0.05 - 2 M boric acid doping), with B4-TNT exhibiting the smallest diameter at 190 nm. Boron incorporation induces phase transformation and decreases in diameter of nanotubes, influencing the material's structural properties. Corrosion resistance assessments indicate a significant enhancement, with B4-TNT demonstrating the highest polarization resistance (Rt = 2849.03 kΩ.cm2). The observed improvements are attributed to the combination of reduced nanotube diameter, altered phase structure, and increased polarization resistance, collectively contributing to enhanced corrosion resistance. Biocompatibility experiments conducted with human fibroblast cells (HDFa) reveal that B-doped TNTs foster increased cell viability compared to undoped TNT and uncoated alloy surfaces. These findings underscore the potential of boron doping as a strategic approach to improve corrosion resistance and enhance the biocompatibility of Ti6Al4V implants. This study provides valuable insights for advancing materials in biomedical applications with improved structural, corrosion-resistant, and biocompatible properties.

Isoniazid and rifampicin (IRC) have been shown to cause hepatotoxicity in both clinical and preclinical studies. Oxidative stress and inflammation have been held responsible for the pathogenesis of IRC-induced hepatotoxicity. Antioxidative and anti-inflammatory effects of thiamine pyrophosphate (TPP) and cinnamon extract (CE) have been shown in previous studies. Therefore, our study investigated the protective effects of TPP and CE on possible liver damage caused by IRC treatment in rats. Twenty-four albino Wistar rats were categorized into four groups: a healthy group (HG), an IRC group (IRG), a TPP+IRC group (TIRG), and a CE+IRC group (CIRG). TPP (25 mg/kg) was administered intraperitoneally to TIRG, while CE (100 mg/kg) was administered orally to CIRG. In IRG, TIRG, and CIRG, isoniazid (50 mg/ kg) and rifampicin (50 mg/kg) were administered orally one hour after these treatments. For seven days, this procedure was repeated once a day. After this period, blood samples were taken from the tail veins, and the rats were sacrificed. The removed liver tissues were analyzed for oxidant, antioxidant, and proinflammatory cytokines and subjected to histopathological evaluation. Serum alanine aminotransferase and aspartate aminotransferase activities were also measured. An increase in malondialdehyde, nuclear factor kappa B, tumor necrosis factor-alpha, interleukin 1 beta, and interleukin-6 levels, a decrease in total glutathione levels, superoxide dismutase and catalase activities, and an increase in alanine aminotransferase and aspartate aminotransferase activities were found with IRC treatment (p<0.001). The histopathological analysis of the IRG suggested hepatotoxicity (p<0.001). TPP and CE administered with IRC inhibited the biochemical changes (p<0.001). In the TIRG, this inhibition was higher than in the CIRG (p<0.05). Histological damage was inhibited by TPP (p<0.001). CE prevented biochemical changes but not histological changes except inflammatory cell infiltration. Therefore, TPP may be better than CE in preventing IRC-induced hepatotoxicity.

Cankaya, Muhammed Safa; Pala, Omer Osman
Background: Various treatment modalities have been employed for mechanical low back pain (MLBP), but evidence of their efficacy varies greatly. Objectıve: This randomized controlled trial aimed to assess the outcomes of Mulligan concept applications, including sustained natural apophyseal glides (SNAGS) and natural apophyseal glides (NAGS), in obese patients with MLBP. Methods: The study, conducted between January 2021 and June 2022 at a tertiary hospital, involved randomizing patients into two groups. Both groups underwent six sessions of stretching and strengthening exercises every other day. The Mulligan group received additional intervention with SNAG and NAGS techniques. Measurements were made regarding the Visual Analog Scale (VAS) score, Oswestry Disability Index (ODI) score and range of motion (ROM) for the patients’ MLBP level. Results: Post-interventions, both groups exhibited positive changes in flexion ROM, extension ROM, right and left rotation ROM, right and left lateral flexion ROM, VAS score, and ODI score compared to pre-intervention (p < 0.001 for both groups and variables). The Mulligan group showed a higher increase in ROM and a more significant decrease in VAS and ODI scores. Conclusıons: Mulligan mobilization techniques prove significantly beneficial for enhancing ROM in all directions, reducing pain levels, and alleviating disability in obese individuals with MLBP.

Gursoy, Sule; Ersan, Ronak Haj
In this study, a series of 1,2-disubstituted benzimidazoles, designed to mimic nucleotides and interact favorably with amino acids in protein active sites, were synthesized and characterized. These compounds were characterized using IR, NMR spectral data, and elemental analyses, leveraging the recognition of substituted benzimidazoles as pharmacophores in the literature due to their documented anticancer activities. The compounds involved chloro and substituted phenyl rings, known for their anticancer activities, as key pharmacophores. Cytotoxicity assessments using the MTT assay were performed against MCF7, MDA-MB-231, and normal mouse fibroblasts (L929) for all synthesized compounds (11–19). Among them, compound 13 exhibited the most significant activity, inducing cell cycle arrest at the G0/G1 phase in MCF7 cells, suggesting its potential as an antitumor agent through apoptosis. To understand the underlying mechanisms of apoptotic cell death induced by compound 13, we investigated the involvement of apoptotic proteins such as Bax, Bcl-2, and p53. Our findings highlight Compound 13 as a promising scaffold for further optimization in the development of effective anticancer drugs.
