- Kitap BölümüYalnız künye2019Utilization of SVM, LSSVM and GP for predicting the medical waste generation
Jagan, J.; Dalkiliç, Yildirim; Samui, Pijush
The prediction of wastes generated in the hospital will help their management for several activities like storage, transport and disposing. This chapter adopts Support Vector Machine (SVM), Least Square Support Vector Machine (LSSVM) and Genetic Programming (GP) in order to estimate the rate of medical waste generation. In the event of predicting the rate, type of hospital, capacity and bed occupancy has been used as inputs of SVM, LSSVM and GP. SVM is based on statistical learning theory, which provides an elegant tool for nonlinear system modeling. LSSVM is the re-formulation to the general SVM. GP, a best part of evolutionary algorithm and also the specification of Genetic Algorithm (GA). These SVM, LSSVM and GP have been used as the regression techniques. The results show the performance of the developed SVM, LSSVM and GP models were elegant and outstanding.
- MakaleYalnız künye2019Identification of medically important yeasts by sequence analysis of the internal transcribed spacer and D1/D2 region of the large ribosomal subunit; [Identificación de levaduras de interés médico mediante análisis de secuencia del espaciador interno transcrito y de la región D1/D2 de la subunidad ribosómica grande]
Aydin, Merve; Kustimur, Semra; Kalkanci, Ayse; Duran, Tugce
Background: The prevalence of opportunistic yeast infections has increased in recent decades as the result of an increasing immunocompromised patient population. Aims: To evaluate ribosomal RNA (rRNA) gene sequence to identify medically important yeast species, to investigate the performance of both the rRNA gene internal transcribed spacer (ITS) and D1/D2 region in identifying clinically relevant yeasts, and to compare these results with those of a standard phenotypic method. Methods: Both regions from 50 yeast strains, comprising 45 clinical isolates and 5 reference strains, were amplified using PCR and then sequenced. The sequences were compared to reference data available from the GenBank database of the National Center for Biotechnology Information using the BLASTn tool. Results: Using ID32C, 88% (44/50) of all strains were identified accurately at the species level, although 6% were misidentified; two Candida eremophila isolates were identified as Candida glabrata and Candida tropicalis, and one Saprochaete clavata isolate was identified as Saprochaete capitata. Two of the four isolates identified by phenotypic methods as Trichosporon asahii were defined so by analyzing the ITS region, but the remaining two were not distinguishable from closely related species. Based on the D1/D2 region, these four isolates had 100% sequence identity with T. asahii, Trichosporon japonicum, and Trichosporon asteroides. The isolate identified as Trichosporon inkin using ID32C could not be distinguished from Trichosporon ovoides by analyzing the ITS and D1/D2 regions. Conclusions: Identifying medically important yeasts by sequencing the ITS and D1/D2 region is a rapid and reliable alternative to conventional identification methods. For a diagnostic algorithm, we suggest a two-step procedure integrating conventional methods (e.g. microscopic morphology on corn meal agar with Tween® 80 and API ID32C®) and sequence analysis of the ITS and D1/D2 region.

Aydin, Sonay; Fatihoglu, Erdem; Kacar, Mahmut
Objectives: The prevalence of thyroid nodules in pediatric population is 0.2–2%, which is lower than adults. However, the probability of the nodule to be malignant is higher than adults (20–73%). Differential diagnosis of thyroid lesions in children includes intrathyroidal ectopic thymus tissue (ITT). ITT can present as a thyroid nodule, and be confused with malignancy with its hyperechoic pattern; this might cause unnecessary fine-needle aspiration biopsies and/or surgical interventions. In the current study, we mainly aim to define both US and color Doppler ultrasonography (CDUS) characteristics of ITT. We also aim to describe the most sensitive and most specific diagnostic parameters of ITT. Methods: We have evaluated US examination reports of 56 children for whom differential diagnosis included ITT between February 2015 and August 2018. We have recorded sonographic characteristics of the lesions, CDUS data, and thyroid hormone levels. Results: Study population consists of 56 patients (22 ITT, 34 other diagnoses). Median age of the population is 10 years. Age, sex, laboratory results, and follow-up change in lesion diameters do not show any significant difference between ITT and other diagnosis groups. Typical US appearance, fusiform lesion shape, and isovascular CDUS characteristics are higher in ITT group. The median value of the lesion’s highest diameter is smaller in ITT group. The most valuable criteria to predict ITT presence were the fusiform shape and the longest diameter of the lesion. Conclusions: Fusiform shape and a maximum diameter of ≤ 9 mm are the most selective criteria to predict ITT diagnosis.

Background: Arterial hypertension is one of the leading causes of mortality and morbidity in general population. Sirtuin 1 (SIRT1) has diverse anti-inflammation, anti-oxidant, and anti-apopytosis effects on endothelium and is associated with endothelial aging and dysfunction. The objective of this study was to evaluate the relation of serum SIRT1 level with left ventricular hypertrophy (LVH) in newly diagnosed hypertensive patients. Methods: One hundered and twenty-five consecutive, newly diagnosed hypertensive patients were divided into two groups with regard to presence of LVH and compared to 40 healthy control subjects. LVH was determined by transthoracic echocardiography using the cube formula. Serum SIRT1 level was analyzed with enzyme-linked immunosorbent assay. Results: Serum SIRT1 level was significantly higher in patients with LVH compared to those without LVH (14.3 ± 3.9 ng/ml vs. 7.9 ± 3.6 ng/ml, P < 0.001) and healthy control subjects (14.3 ± 3.9 ng/ml vs 6.6 ± 2.0 ng/ml, P < 0.001). Multivariate logistic regression analysis revealed higher serum SIRT1 level independently predicted LVH in hypertensive patients (OR 1.50; 95% CI, 1.30–1.73; P < 0.001). Receiver-operating characteristic curve analysis demonstrated a cutoff value of 9.4 had a sensitivity of 90% and specificity of 74% for the prediction of LVH (AUC 0.885; 95% CI, 0.815–0.935; P < 0.0001). Conclusion: SIRT1 was a powerful biomarker for predicting LVH in hypertensive patients.

Objective: To investigate Blastocystis' etiologic role and association with gastrointestinal symptomatology in acute and chronic urticaria patients and to identify Blastocystis subtypes responsible for urticaria. Methods: The study included urticaria patients and healthy individuals that presented to our polyclinic between June 2015 and May 2017. The participants were assigned into Group I (137 patients), subdivided into acute (72) and chronic urticaria patients (65), and Group ? (129 control individuals). Blastocystis presence was investigated by native-Lugol examination, trichrome staining, PCR using sequence tagged site primers, and DNA sequencing analysis. The phylogenetic tree was constructed. Results: The native-Lugol and trichrome staining methods revealed that 16 patients (16/133, 12.0%) had Blastocystis-positive stool samples, of which seven samples (7/133, 5.3%) belonged acute and nine (9/133, 6.8%) to chronic urticaria patients. Concerning Blastocystis subtypes, of the acute urticaria patients, three had subtype 1 (ST1), one had ST2, and three had ST3. Of the chronic urticaria patients, one had ST1 and eight had ST3. Blastocystis positivity was detected in two control individuals (2/123, 1.6%), both being ST3. All subtypes identified by PCR were confirmed by the sequencing analysis. The acute and chronic urticaria groups showed no statistically significant differences for Blastocystis positivity (P=0.60) and subtype distribution (P=0.15). A statistically significant difference was found between the urticaria patients and the controls for Blastocystis positivity (P<0.01), but not for subtype distribution (P=0.67) or for Blastocystis presence and gastrointestinal complaints. Conclusions: This study on Blastocystis subtype distribution among Turkish urticaria patients showed results consistent with the literature. It was concluded that Blastocystis should be kept in mind in patients with urticaria.
- MakaleYalnız künye2019A potential risk factor for paraoxonase 1: in silico and in-vitro analysis of the biological activity of proton-pump inhibitors
Türkeş, Cüneyt
Objectives: Proton-pump inhibitors (PPIs) are drugs commonly utilized by about 7% of adults in the world. Recent researches have shown that there are countless and severe side effects of these drugs. This situation has raised concern among clinicians and patients alike. The purpose of this study is to contribute the novel drug discovery and development technology and toxicology field by researching interactions of PPIs on paraoxonase 1. Methods: In this study, the paraoxonase 1 enzyme was purified from human serum by using rapid and straightforward chromatographic techniques. Subsequently, the inhibition effects of pantoprazole, omeprazole, and esomeprazole, PPIs, were investigated on paraoxonase 1. Besides, molecular docking studies were performed to unravel the binding mechanism between the enzyme and drugs. Key findings: All drugs showed potent inhibitory activities. IC50 of the drugs values were 54.780 ± 0.524, 86.470 ± 0.818 and 93.390 ± 0.885 mm and Ki constants were found as 39.895 ± 0.005 mm, 70.112 ± 0.010 mm and 78.868 ± 0.008 mm, respectively. The binding scores observed in silico studies were found to agree with the obtained from in-vitro experimental results. Conclusions: We observed that the drugs decreased PON1 activity at low concentrations. The results show that adjusting the dosages of these medications is a crucial case for each patient. The physicians should more carefully interpret whether there is an essential indication before prescribing PPIs and, if there is, to approve the proper dosing for the situation.
- MakaleAçık Erişim2019Patient Characteristics and General Practitioners’ Advice to Stop Statins in Oldest-Old Patients: a Survey Study Across 30 Countries
van der Ploeg, Milly A.; Streit, Sven; Achterberg, Wilco P.; Beers, Erna; Bohnen, Arthur M.; Burman, Robert A.; Collins, Claire; Franco, Fabio G.; Gerasimovska-Kitanovska, Biljana; Gintere, Sandra
Background: Statins are widely used to prevent cardiovascular disease (CVD). With advancing age, the risks of statins might outweigh the potential benefits. It is unclear which factors influence general practitioners’ (GPs) advice to stop statins in oldest-old patients. Objective: To investigate the influence of a history of CVD, statin-related side effects, frailty and short life expectancy, on GPs’ advice to stop statins in oldest-old patients. Design: We invited GPs to participate in this case-based survey. GPs were presented with 8 case vignettes describing patients > 80 years using a statin, and asked whether they would advise stopping statin treatment. Main Measures: Cases varied in history of CVD, statin-related side effects and frailty, with and without shortened life expectancy (< 1 year) in the context of metastatic, non-curable cancer. Odds ratios adjusted for GP characteristics (ORadj) were calculated for GPs’ advice to stop. Key Results: Two thousand two hundred fifty GPs from 30 countries participated (median response rate 36%). Overall, GPs advised stopping statin treatment in 46% (95%CI 45–47) of the case vignettes; with shortened life expectancy, this proportion increased to 90% (95CI% 89–90). Advice to stop was more frequent in case vignettes without CVD compared to those with CVD (ORadj 13.8, 95%CI 12.6–15.1), with side effects compared to without ORadj 1.62 (95%CI 1.5–1.7) and with frailty (ORadj 4.1, 95%CI 3.8–4.4) compared to without. Shortened life expectancy increased advice to stop (ORadj 50.7, 95%CI 45.5–56.4) and was the strongest predictor for GP advice to stop, ranging across countries from 30% (95%CI 19–42) to 98% (95% CI 96–99). Conclusions: The absence of CVD, the presence of statin-related side effects, and frailty were all independently associated with GPs’ advice to stop statins in patients aged > 80 years. Overall, and within all countries, cancer-related short life expectancy was the strongest independent predictor of GPs’ advice to stop statins.

Pazarci, Ozhan; Kilinc, Seyran; Camurcu, Yalkin; Bulut, Okay
Background: Gunshot injury of the hip joint was reported to constitute 2–17% of all extremity firearm injuries. However, there are few studies in the literature related to gunshot injuries of the hip joint. The aim of the current study was to present the results of 10 cases treated with arthroplasty following a gunshot injury to the hip joint together with the recommended treatment algorithm. Methods: Patients with a previous medical history of hip joint region gunshot injury who underwent total hip arthroplasty were retrospectively evaluated. Those with incomplete medical records or who were lost to follow-up were excluded. Patients were classified according to the severity of the previous gunshot injury to the hip joint region. Harris hip score (HHS) and Short Form-12 quality of life score were the main outcome measurements. Postoperative complications encountered during follow-up were recorded. Results: The mean age of the patients at the time of surgery was 29.9 years. The mean preoperative HHS was 25.2 points and it was 65.8 at the final follow-up. Patients with bullet fragments in the hip joint, classified as group 1, had better HHS, whereas those with contaminated hip joint with intestinal flora, classified as group 3, had worst HHS. Conclusion: Hip arthroplasty after hip joint gunshot injury is a good treatment choice in young patients to reduce pain and regain functions. However, very high infection rates can be seen in patients with accompanying intestinal injury.

Wound infections are important health problems especially in underdeveloped and developing countries. In this study, it was aimed to identify the microorganisms causing wound infection and their antimicrobial susceptibility patterns in order to assist in the selection of effective treatment. We retrospectively evaluated the microorganisms isolated from wound swab specimens sent to the microbiology laboratory of patients followed between 2014-2017. The identification of the microorganisms thought to cause infection was done with MALDITOF MS (bioMérieux, France) and the antimicrobial susceptibility with VITEK-2 (bioMérieux, France) automated system. A total of 4492 wound cultures were evaluated. Microorganisms were isolated from 55 % of the specimens; 42 % in 2014, 54.1 % in 2015 and 65.1 % in 2016. The most common bacteria in wound cultures were Escherichia coli, followed by Staphylococcus aureus and Pseudomonas spp. Pseudomonas spp. (16.5 %) was most frequently isolated from intensive care unit. S.aureus was the most frequently isolated bacteria from pediatric and internal medicine units (31.1 % and 16.1 % respectively), and E.coli was most frequently isolated from surgical sciences (21.4 %). Colistin and carbapenems were detected as the most effective antimicrobials against E.coli isolates. Of the S.aureus isolates, 18.4 % were methicillin resistant, and no glycopeptide or linezolid resistance was detected. Isolation of different microorganisms from each clinic has shown that regular surveillance studies and antimicrobial susceptibility testingare a necessity for rational antimicrobial use in this direction.

- MakaleAçık Erişim2018HPLC Analysis of Phenolic Compounds from Gypsophila aucheri Boiss. and Investigation of Antioxidant and Cytotoxic Activity of Gypsophila aucheri Boiss. exracts
Altay, Ahmet
This study was designed to investigate the antioxidant and antiproliferative activities of Gypsophila aucheriBoiss. extracts as well as phenolic content by RP-HPLC technique. Antioxidant activity potentials of theextracts were evaluated by four different methods namely, 1,1-diphenyl-2-picrylhydrazyl (DPPH) and 2,2′-azinobis-(3-ethylbenzothiazoline-6-sulfonic acid) (ABTS) radical scavenging capacity tests, cupric ionreducing antioxidant capacity (CUPRAC) method, and metal chelating assay. Antiproliferative activities of theextracts were tested against breast carcinoma (MCF-7), colorectal carcinoma (HT-29), and hepatocellularcarcinoma (HepG2) cells. RP-HPLC analysis revealed the presence of eight phenolic compounds in eighteenphenolics scanned. Antioxidant activity results showed that methanol extract of Gypsophila aucheri displayedmore pronounced antioxidant activity than water extract according to DPPH and ABTS tests, whereas waterextract was superior than methanol extract for CUPRAC and metal chelating activity tests. Cytotoxicity testsdemostrated the promising antiproliferative activity of methanol extract against all the tested cancer cell lineswith its lower IC50 values. However, both extracts were observed to display moderate biological activitycompared to positive controls. The obtained data suggest that Gypsophila aucheri could be evaluated as apromising source for food and nutraceutical industries due to its striking antioxidant and moderateantiproliferative potentials together with high phytochemical profile.
