- MakaleAçık Erişim2018Evaluation of the Lifespan of Fruit Fly Drosophila melanogaster Exposed to Dioxins
Altun Çolaki, Deniz; Uysal, Handan
The polychlorinated dibenzo-p-dioxins (PCDDs) formed during combustion processes and as by-products of industrial processes are persistent organic pollutants. In the present study, the PCDDs of 2,3,7,8-TCDD, 1,2,3,7,8-PeCDD, 1,2,3,7,8,9-HXCDD and 1,2,3,4,6,7,8,9-OCDD (1, 2.5, 5 and 10 mu g/mL medium) were evaluated for their possible toxicity on the survival rate of Drosophila melanogaster, in vivo. The effects of different concentrations of dioxins were separately administered to female and male populations of D. melanogaster. In all application groups, both the survival rate and each population's longevity decreased, depending on the concentration of dioxins (p<0.05). In conclusion, the toxic effect for the survival rate and longevity was observed in the following order: 2,3,7,8-TCDD> 1,2,3,7,8-PeCDD> 1,2,3,7,8,9-HXCDD> 1,2,3,4,6,7,8,9-OCDD.

Objective: In this study, we aimed to assess sharps injuries occuring in a training and research hospital retrospectively and to clarify the current situation in order to reach conclusions and suggestions about the subject. Methods: The data was obtained from the notification forms with 18 questions filled by 124 health workers who were exposed to sharps injuries and applied to the infection control committee unit at the relevant dates. The data was statistically analysed with a p value of <0.05 being considered as significant. Results: At the end of the study, it was found out that 63.7% of the workers exposed to injuries were <30 years old and the average age was 27.9. A statistically significant correlation was found between the relevant age groups and injury prevalence. Injuries in the 16-30 age group were more frequent in clinical services, emergency services and blood sampling unit whereas injuries in >30 age group were mostly observed in clinic services (internal and surgical units) and operating rooms. It was found that hand injuries were the most common and needle sticks were the most frequent (91.1%) cause of injury. Conclusions: The primary way to prevent the sharps injuries is to improve safety and to develop effective employee safety policies. Additionally, taking universal precautions, regular trainings, reducing staff workload and provision of safe and useful materials can be among the suggestions that can prevent injuries.
- MakaleAçık Erişim2018Comparison of Carbapenem Inactivation Test and Modified Hodge Test in Carbapenemase-Producing Klebsiella pneumoniae Strains
Davarci, Ismail; Koçoglu, Mucahide Esra; Zengin, Ferhat
Objective: Carbapenem-resistant Enterobacteriaceae are reported increasingly everywhere throughout the world. Rapid and simple detection of carbapenemase-producing strains has become imperative because of the use of carbapenems in the treatment of extended spectrum beta-lactamase (ESBL)-producing Enterobacteriaceae infections. In this study, we aimed to compare the diagnostic value of modified Hodge test and carbapenem inactivation test for Klebsiella pneumoniae isolates producing carbapenemase, verified by molecular method, and to evaluate their use in routine laboratory practice. Methods: 64 K. pneumoniae strains which have shown resistance gene causing carbapenem resistance by the polymerase chain reaction, and 40 ESBL-negative and carbapenem-susceptible K. pneumoniae strains have been included in this study. Meropenem, imipenem and ertapenem were studied separately in modified Hodge test and carbapenem inactivation test was performed for all strains. Carbapenem inactivation test was evaluated at the 6th hour for preliminary decision and at the 24th hour for the final decision. Results: The sensitivity of the modified Hodge test using meropenem, imipenem and ertapenem for detecting presence of carbapenemase was 60.9%, 62.5% and 68.8%, respectively. In the carbapenem inactivation test with meropenem, imipenem and ertapenem, sensitivity to determine carbapenemase production was 32.8%, 75% and 96.9% after 6 hours, and 64.1%, 84.4% and 96.9% after 24 hours, respectively. There were no positive results in modified Hodge and carbapenem inactivation tests with K. pneumoniae strains that were ESBL-negative and carbapenem-susceptible. Conclusions: As a result, molecular tests are essential to detect carbapenem encoding genes. However, we believe that the carbapenemase inactivation test is a better alternative to the modified Hodge test to detect carbapenemase production in laboratories without adequate infrastructure.
- MakaleAçık Erişim2018Alteration of Fatty Infiltration Grades of the Liver in Sonography as an Outcome of Cholecystectomy
Koc, Ural; Eken, Huseyin
Introduction: Recent studies have not only reported that non-alcoholic fatty liver disease may be associated with cholecystectomy but also emphasized that cholecystectomy may be a risk factor for the accumulation of fat in the liver. In light of this information in the literature, in the present study, we compared the sonographic fatty infiltration grades of patients who had undergone cholecystectomy using laboratory findings obtained before and after the operation. Objective: Fifty-nine cases whose almost complete laboratory and sonographic data were available were included in the study. Patients were divided into three groups according to the preoperative period and postoperative follow-up periods as 3 months and 6-12 months. Results: In both groups, there was a statistically significant difference between the preoperative and postoperative steatosis and ultrasound (US) fatty infiltration grades (p<0.001). However, there were no statistically significant differences between the preoperative and two postoperative follow-up periods of US fatty liver grades alteration (p=0.650). The hepatic steatosis index had a significant correlation with steatosis and US fatty infiltration grades (Spearman's correlation rho=0.319 and rho=0.361, respectively, p<0.001). Conclusion: Cholecystectomy may lead to an increase in the level of fatty infiltration of the liver in the follow-up sonography, which may occur over a certain period of time through adaptive processes. Early postoperative US imaging for the purpose of hepatosteatosis follow-up is not necessary.
- MakaleYalnız künye2018An investigation of healthcare supply chain management and patient responsiveness: An application on public hospitals
Polater, Abdussamet; Demirdogen, Osman
Purpose This study aims to focus on the impact of supply chain (SC) integration, demand forecasting and supplier performance on patient responsiveness at public hospitals through the mediating role of SC flexibility. Design/methodology/approach To measure the above stated correlation, a Likert scale with five points and 23 items was used and structural equation modeling was applied. The scale was applied to public hospitals. Statistical software programs (SPSS 18 and LISREL 8.8) were used to analyze the data. The analysis of reported statistics is based on a sample of 129 logistics and SC specialists at public hospitals in the cities representing different regions of Turkey. Findings The research hypotheses are supported as a result of the analysis. The research reveals that SC flexibility has a mediation effect between SC integration, demand forecasting, supplier performance and patient responsiveness. Practical implications The increasing number of population, geopolitical position, migration waves, man-made and natural disasters lead Turkish health-care industry to have effective SC plans to satisfy the patients' needs successfully and reduce the effects of these fatal events. In this sense, SC flexibility is an important factor for health-care industry in responding changing patient demands. At this juncture, the main point is to bring required resources together in the right place and at the right time. Otherwise, health-care institutions cannot serve the affected people because of the non-availability of supply. To achieve it, public health-care institutions should give more importance to the SC principles. Originality/value Success in SC flexibility in health care can directly affect patient welfare. Thus, focusing on the patient responsiveness is an important aim of the health-care industry. However, it was determined that less attention has been given to understanding patient satisfaction as a result of SC operations. The results indicate that patient responsiveness improvement should be included into strategic plans, and SC efforts should be used as a means of satisfying patient needs quickly. To the best of our knowledge, this is the first study investigating the mentioned relationships at public hospitals. Findings of this paper will have a significant contribution for researchers and health-care professionals in understanding the impact of SC to patient responsiveness.

The study datawere collected using a descriptive questionnaire, the Family Environment Scale (FES), the Internalized Stigma of MentalIllness Scale and the Abbreviated Turkish Version of the World Health Organization Quality of Life Questionnaire. Results: This study found no significant difference between the mean scores of the patients and their relatives’ interpersonal relationships (43.94±8.20, 27.10±5.59) and control (46.29±6.72, 26.57±4.30) in the family environment (p>0.05).The patients’ mean score on the Internalized Stigma of Mental Illness Scale was 76.12±17.15. On its subscales, theywere: alienation (15.63±4.41), stereotype endorsement (18.24±4.20), perceived discrimination (13.67±4.09), stigma resistance (12.35±3.6) and social withdrawal (16.24±5.21). and the patients’ perceptions of quality of life were: physical(12.00±2.75), mental (11.11±3.02), social (9.15±3.72), and environmental (11.56±2.61).Conclusion: This study found no difference between the patients and their relatives’ perceptions of interpersonal relationships and control. The patients perceived internalized stigma above the moderate level. Their quality of life evaluations were at a moderate level. As their perceived internalized stigma level went up, their quality of life areas significantlydecreased, and their perception of interpersonal relationships and control in the family environment increased significantly in the positive direction. It is important that psychiatric nurses assess the factors that affect patients’ relapses andtreatment such as family environment and internalized stigma. They should also develop and implement programs.
- MakaleAçık Erişim2018Protective effect of Berberis crataegina DC. (Ranunculales: Berberidaceae) extract on Bleomycin-induced toxicity in fruit flies (Diptera: Drosophilidae)
Atici, Tugba; Çolak, Altun Deniz; Ersöz, Cagla
Antineoplastic agents due to their cytotoxic effects increase the amount of free radical in cells causing cell death. The aims of this study were to assess the possible toxic effects of Bleomycin (BLM) on the number of offspring and survival rate of fruit flies as well as the protective role of Berberis crataegina DC. fruit extracts (BCE) on these effects. Solutions prepared in different concentrations (0.125 mg/mL for BLM; 10 and 20 mg/mL for BCE) were applied to the adult fruit fly Drosophila melanogaster's wild-type individuals and 72 +/- 4 h larvae to determine the number of offspring and survival rate, respectively. As a result, the higher number of offspring and survival rate in the BCE + BLM applied groups than the only BLM applied groups shows that BCE is an antioxidant and has a protective effect (p<0.05). This protective effect can be explained as the prevention or removal of the free radicals formation from the environment.

Karavas, Erdal; Karakeçili, Faruk
Objective: The aim of this study was to perform common carotid, internal carotid and vertebral artery Doppler ultrasound imaging of patients with Crimean-Congo haemorrhagic fever (CCHF), to compare the results with those of healthy individuals, and to determine the value of radiological findings in the prediction of disease severity in the early period. Methods: Bilateral common carotid, internal carotid and vertebral artery Doppler ultrasound (US) imaging was performed in CCHF patients and a healthy control group between March 2016 and September 2017. Patients were grouped as mild-moderate or severe cases according to the scoring system of Bakir et al. to determine disease severity. The peak systolic and diastolic rates together with the resistive and pulsatility indexes were examined in the common carotid, internal carotid, and vertebral arteries bilaterally. The flow volumes in the internal carotid and vertebral arteries were also measured bilaterally. Results: Total 35 CCHF patients comprised 14 males and 21 females with a mean age of 52 years. 7 patients were evaluated as severe and 28 as mild-moderate. On Doppler US, the peak systolic and diastolic end flow rates in vertebral arteries bilaterally were found to be statistically significantly higher in the patient group compared to the control group (p<0.05). The internal carotid artery and vertebral artery flow volumes bilaterally were determined to be statistically significantly higher in the patient group (p=0.032). When the relationship between flow volume and disease course was examined, volume was 1.11 L/min in mild-moderate cases and 2.23 L/min in severe cases. Conclusions: Early diagnosis of CCHF is extremely important for the initiation of supportive treatment, prediction of disease severity, and precautions to be taken. The noninvasive radiological findings determined in this study can be considered to contribute to the prediction of the clinical course and severity of CCHF in the early stages.

Taşlı, Nurdan Gamze; Uçak, Turgay; Karakurt, Yücel; Keskin Çimen, Ferda; Özbek Bilgin, Aslı; Kurt, Nezahat; Süleyman, Halis
Aim: To determine the role of rutin in prevention of cisplatin induced retinal and optic nerve injury in an experimental study. Materials and methods: Totally 18 albino Wistar male rats were assigned into three groups, as follows: healthy controls (HC group), only cisplatin administered group for 14 days (CIS group), and rutin + cisplatin administered group for 14 days (RC group). Blood samples were obtained from animals just before the scarification. Serum malondialdehyde (MDA), myeloperoxidase (MPO), total glutathione (tGSH), superoxide dismutase (SOD), interleukin 1 beta (IL-1β) and tumour necrosis factor alpha (TNF-α) levels were investigated. The eyes were enucleated for histopathological evaluations of retina and optic nerve. Results: MDA, MPO, IL-1β and TNF-α levels were statistically significantly higher (p < 0.001) in CIS group compared with other two groups while tGSH and SOD levels were statistically significantly lower (p < 0.001). Regarding these parameters, in CIS group MDA, MPO, IL1β and TNF-α levels were statistically significantly increased with cisplatin administration and giving rutin concomitantly with cisplatin prevented this increase. On the other hand, tGSH and SOD levels were statistically significantly decreased with cisplatin administration and giving rutin concomitantly with cisplatin prevented this decrease. In qualitative analyses of histopathological findings of retina and optic nerve; the results of RC group were similar with the results of healthy controls; but there was statistically significant differences between CIS group and other two groups (p < 0.001). Conclusions: Concomitant rutin administration may prevent the detrimental effects of cisplatin on lipid peroxidation, oxidative stress and inflammation markers and may also avert the histopathological damage on retina and optic nerve. Further studies are warranted to determine the effects of cisplatin and rutin on eye.

Purpose: To investigate the effect of metyrosine against I/R induced gastric damage in rats. Methods: Eighteen albino Wistar male rats were divided into groups; gastric I/R (GIR), 50 mg/kg metyrosine+gastric I/R (MGIR), and sham (SG) groups. 50 mg/kg metyrosine was given to the MGIR group, and distilled water was given to the GIR and SG groups by the oral gavage. After 30 minutes, 25 mg/kg thiopental sodium was injected intraperitoneally. Ischemia was achieved for 1 hour by clamping the celiac artery of the MGIR and GIR groups, then reperfusion was achieved for 3 hours. After that, animals were killed with 50 mg/kg thiopental. Biochemical and histopathological examinations performed on the gastric tissues. Results: Metyrosine decreased the MDA and MPO and the increased the tGSH and SOD. In addition, it reduced inflammation by suppressing the decrease of COX-1 and the increase of COX-2. Histopathologically, metyrosine decreased symptoms caused by I/R such as mucosal necrosis, hemorrhage, edema, PMNL infiltration, and dilated congested blood vessels. Conclusions: Metyrosine prevented the I/R induced oxidative stress in the gastric tissue. Metyrosine may be beneficial for gastric I/R injury.
- MakaleAçık Erişim2018Effect of Ganoderma lucidum polysaccharides on oxidative damage in liver of STZ-diabetic rats
Eroğlu, Hüseyin Avni; Beytut, Ebru
In this study, the effects of exogenic application of Ganoderma lucidum polysaccharide on oxidative stress and hyperlipidemia was determined in diabetics rats. The study included 60 Wistar albino rats aged 4-5 months. Rats were divided into 6 equal groups. The groups were control, Ganoderma lucidum polysaccharide, diabetic, diabetic+60 mg/kg polysaccharides, diabetic+120 mg/kg polysaccharides and diabetic+10 mg/kg polysaccharide. Diabetes was induced by intraperitoneal injection of streptozocine at the dose rate of 50 mg/kg. The rats were fed ad-libitum throughout the experiment. At the end of the study, we took some samples out of total oxidant and total antioxidant levels and measured them with the spectrophotometer; thereafter we used the auto-analyser to measure the levels of total cholesterol, triglycerides, HDL, LDL, VLDL cholesterol and pancreatic amylase. We examined the blood of diabetic fasting rats and the effect was that their glucose level was increased significantly. (p<0.05), nonetheless the dose of 60, 120 and 180 mg/kg of Ganoderma lucidum polysaccharides did not reduce the blood glucose level (p>0.05). With the diabetic rats, the dose of 60, 120 and 180 mg/kg of Ganoderma lucidum polysaccharides rides effected in the increase of serum levels of total cholesterol, triglycerides, VLDL, and HDL cholesterol levels, although it was not a significant change (p>0.05), instead, we observed a statistically significant decrease of LDL cholesterol level (p<0.05). According to this study we concluded that polysaccharides effects in the decrease of hyperlipidaemia with regard to diabetes. During the study we applied Ganoderma lucidum polysaccharides exogenously (60, 120, 180 mg/kg) and it did not affect the level of total antioxidant in diabetes significantly (p>0.05), however with a dose of 180 mg/kg the total oxidant level in pancreas and liver tissues was reduced significantly and has been considered as crucial fact. The collected data supported our view on histopathologic examination. Based on all the information, we used one type of fungus Ganoderma lucidum polysaccharides which did not influence the blood glucose level in diabetes, whereas with regard to LDL and total oxidants it played a significant role in reducing its levels.
- MakaleAçık Erişim2018Chronic subdural hematoma associated with arachnoid cyst of the middle fossa: Surgical treatment and mid-term results in fifteen patients
Musluman, Ahmet Murat; Ozoner, Baris; Kircelli, Atilla; Can, Songul Meltem; Yilmaz, Adem; Kaldirimoglu, Ayca; Sahin, Balkan
AIM: To report neurological and radiological features, surgical management, and mid-term outcomes of patients with chronic subdural hematoma (CSDH) associated with ipsilateral arachnoid cyst (AC) of the middle fossa. MATERIAL and METHODS: A total of 453 patients with CSDH were treated at our clinic between August 2004 and August 2012. Of these patients, 15 had ipsilateral AC in the middle fossa. A single burr hole craniostomy was performed to drain the hematoma. The AC was left intact in 14 patients, and one patient had no surgical intervention. The follow-up period ranged from 13 to 88 months (mean 43.07 ± 23.23 months). RESULTS: The patients with AC associated CSDH were found to be younger than the patients with CSDH alone, and the mean age of 58 patients was 13.15 ± 13.17 years, while it was 11 ± 14.22 years in the other patients. Eleven patients had experienced head trauma at 21-50 days before admission. Hematoma evacuation through a single burr hole with closed-system subdural drainage performed at 2-4 days after surgery improved the symptoms in all patients. Two patients developed subdural fluid collection, which was treated by subduroperitoneal shunt placement. CONCLUSION: Greater prevalence of ACs in patients with CSDHs has been reported in the literature. We recommend the drainage of the hematoma via a single craniostomy and to leave the AC intact as the first choice of treatment if the associated AC is a Galassi type I or II. Additional subduroperitoneal shunting may be performed in patients with Galassi type III cyst.

Taydas, Onur; Kantarci, Mecit; Bayraktutan, Ummugulsum; Ogul, Hayri
Purpose: The purpose of this study was to evaluate extremely rare cases of supradiaphragmatic origin of the renal artery, with magnetic resonance (MR) imaging findings. Methods: The study included 9 patients diagnosed with supra-diaphragmatic originating renal artery between 2010 and 2017. The patients were 7 females and 2 males with a mean age of 58.25 years (range, 41–71 years). MR imaging was applied to all patients. Results: In 2 patients, lumen loss was of a moderate degree, and mild in 3 patients. Renal artery stenosis was not observed in the remaining 4 patients. None of the patients had advanced luminal stenosis. The distance of the renal arteries to the diaphragmatic crus was 24 mm at the longest and 8 mm at the shortest. Congenital anomaly was found in 6 patients. Of the 9 patients, 7 had concomitant hypertension, and 2 had no history of hypertension. Conclusions: The anomaly of supradiaphragmatic origin of the renal artery is rare but may be associated with renal artery stenosis, which may then result in hypertension. The clinician should investigate renal artery origin anomalies and renal artery compression syndrome in hypertensive cases where the cause cannot be explained.

Objectives: We aimed to assess the effect of Hippophae rhamnoides extract (HR) on oxidative stress induced by cisplatin (Cis) in rat small intestine tissue by evaluating the biochemical and gene expression levels and histopathological changes. Materials and methods: The control group and Cis group received distilled water, while the HR25+cisplatin (HR25+Cis) group and the HR50+cisplatin (HR50+Cis) group were given HR 25 and 50 mg/kg, respectively, orally for seven days. HR25+Cis, HR50+Cis, Cis groups were injected with a single dose of intraperitoneal cisplatin on the first day. After sacrifice, the jejunum of each rat was removed for the assessment of oxidants and antioxidants. Analyses of the gene expression (for IL-1ß and TNF-α) and histopathological changes evaluated. Results: HR significantly inhibited the increase of oxidants and the decrease of antioxidants caused by cisplatin in the jejunal tissue. IL-1β and TNF-α gene expression levels were almost the same in both the HR50+Cis and the control groups. HR better prevented increase of the serum levels of IL-1β and TNF-α in animals at 50 mg/kg dose compared to 25 mg/kg dose. We confirmed that HR prevented the histopathological changes caused by cisplatin. Conclusions: It was concluded that oxidative stress caused by cisplatin may be preventable by co-administered HR.

These studies are linguistic equivalence study, structure and concurrent validity study, predictive validity study, and reliability study, respectively. Nine hundred and thirty-two university students (61% female), who were included in the four steps, participated in the study. Results: Applying a confirmatory factor analysis [X2 (51,430)=145.16, p<0.001; CFI=0.96; GFI=0.95; IFI=0.96; SRMR=0.047; RMSEA= 0.066, 90% CI (0.053-0.078)] a structure was determined made up of three factors which corroborated the structure of the original scale. The result of the predictive validity showed that physical aggression positively predicted Machiavellianism, psychopathy and narcissism; verbal aggression positively predicted psychopathy and narcis-sism; anger positively predicted psychopathy and narcissism; hostility positively predicted psychopathy and narcis-sism. The Cronbach’s alpha coefficients have been calculated as 0.79 for Machiavellianism, 0.71 for psychopathy and 0.87 for narcissism. Discussion: The findings of the study have shown that the Turkish form of the Dirty Dozen Scale is a valid and reliable scale to measure the Machiavellianism, psychopathy and narcissism personality traits. Future studies will cast light on the understanding of the structure of the Dark Triad Personality Traits in Turkish culture.

Background: Impairment of thiol/disulfide homeostasis, as well as vitamin D deficiency, are responsible for the pathophysiology of many acute and chronic diseases. This study examined the relationship between thiol/disulfide homeostasis and vitamin D level and supplementation. Methods: A total of 203 healthy children were included in the study. The participants were divided into four groups according to 25-hydroxyvitamin D (25(OH)D) level: group 1, severe deficiency (<10 ng/mL); group 2, deficiency (10–20 ng/mL); group 3, insufficiency (20–30 ng/mL); and group 4, sufficiency (≥30 ng/mL). Furthermore, group 5 was defined as being on vitamin D supplementation. Result: Native thiol was lower in group 5 than in groups 2–4 (P = 0.003). Disulfide was higher in groups 1, 4 and 5 than groups 2 and 3 (P < 0.001). Total thiol was lower in group 5 than in group 4 (P = 0.032). The ratio of native thiol/total thiol was lower in groups 1 and 5 compared with groups 2 and 3, and in group 4 compared with group 3 (P < 0.001). The ratios of disulfide/total thiol and disulfide/native thiol were higher in groups 1 and 5 than in groups 2 and 3 whereas only the disulfide/total thiol ratio was higher in group 4 than in group 3 (P < 0.001). Conclusions: In healthy children, severe deficiency of vitamin D causes impairment of thiol/disulfide homeostasis and increases protein oxidation, which cannot be reversed by external vitamin D supplementation.
