The Effect of Family-to-Family Support Programs Provided for Families of Schizophrenic Patients on Information about Illness, Family Burden and Self-Efficacy

Yildirim, Arzu; Buzlu, Sevim; Hacihasanoglu Asilar, Rabia; Camcioglu, Tuba Hale; Erdiman, Sezgin; Ekinci, Mine

Objective: This study was conducted to determine the effects of family-to-family support programs provided for the families of schizophrenic patients on the need for information about the illness, family burden and self-efficacy Method: The study was carried out with 34 relatives who have key rolesin the lives of 34 outpatients who were diagnosed with schizophrenia under DSM-IV-TR. A volunteering family member was given training about the purpose, contents and planning of the program. Thefamily member who had been trainedthen extended this training to other family members with the assistance of the psychiatric nurse. The training was done once a week, for atotal of 12 sessions. The data were collected using a Disease Information Form (DIF), the Perceived Family Burden Scale (PFBS) and the Self-Efficacy Scale (SES). Results: There was a significant decrease in the PFBS objective scores (from 11.76 +/- 3.89 to 9.82 +/- 4.03, p=0.000), subjective scores (from 37.85 +/- 14.57 to 32.74 +/- 4.39, p=0.030) and total scores (from 49.62 +/- 18.25 to 42.56 +/- 7.56, p=0.005) and a significant increase in the SESS scores (from 82.65 +/- 12.01 to 97.79 +/- 7.17, p=0.000). Before training, 58.8% of family members correctly answered that schizophrenia is a brain disease, while 94.1% correctly stated that,it must be treated with medicadon;however, 55.9% of caregivers incorrectly believed in magical attributions relating to the disease. After training, all caregivers were able to answer correctly. Conclusion: A family-to-family support program was found to be effective in reducing caregivers' need for information, reducing their burden, and increasing their self-efficacy in our country. Key Words: Schizophrenia, family-to-family support program, families, knowledge of the disease, family burden, self-efficacy

Preventive Effect of Intrathecal Paracetamol on Spinal Cord Injury in Rats

Sahin, Murat; Sayar, Ilyas; Peker, Kemal; Gullu, Huriye; Yildiz, Huseyin

Background: Ischemic injury of the spinal cord during the surgical repair of thoracoabdominal aortic aneurysms might lead to paraplegia. Although a number of different mechanisms have been proposed, the exact cause of paraplegia has remained unknown, hampering the development of effective pharmacologic or other strategies for prevention of this condition. A number of studies suggested that cyclooxygenases (COX) contribute to neural breakdown; thus, COX inhibitors might reduce injury. Objectives: We aimed to assess the preventive effect of intrathecal (IT) pretreatment with paracetamol on spinal cord injury in a rat model. Materials and Methods: This experimental study was performed in Ataturk University Animal Research Laboratory Center, Erzurum, Turkey. Adult male Wistar rats were randomly allocated to three experimental groups (n = 6) to receive IT physiologic saline (controls), 50 mu g of paracetamol, or 100 mu g paracetamol one hour before induction of spinal cord ischemia. Six other rats were considered as the sham group. For the assessment of ischemic injury, motor functions of the hind limbs and histopathologic changes of the lumbar spinal cord were evaluated. Additional 20 rats were divided into two equal groups for the second part of the study where the survival rates were recorded in controls and in animals receiving 100 mu g of paracetamol during the 28-day observation period. Results: Pretreatment with 100 mu g of paracetamol resulted in a significant improvement in motor functions and histopathologic findings (P < 0.05). Despite a higher rate of survival in 100 mu g of paracetamol group (70%) at day 28, the difference was not statistically significant in comparison with controls. Conclusions: Our results suggest a protective effect of pretreatment with IT paracetamol on ischemic spinal cord injury during thoracolumbar aortic aneurysm surgery.

Diagnosis and Laparoscopic Repair of Obturator Hernia with Intestinal Obstruction

Somuncu, Erkan; Gökcal, Fahri; Çarkman, Sinan; Peker, Kemal

Obturator hernia is a very rare abdominal hernia that is located in the anterior pelvis. Patients apply to a hospital with a clinical intestinal obstruction table in this type of hernia. Prognosis in obturator hernia remains quite poor because of delayed diagnosis and surgical management. Nowadays, radiological techniques, especially computed tomography, are very advanced. Nevertheless, obturator hernia is a serious problem with its increased mortality, because it is detected quite hardly. We tried to offer an 82-year-old woman with acute abdomen that was considered on mechanic intestinal obstruction table for us in this article. Computed tomography is useful on these patients, just as it is useful in this article. For this reason, we scanned computed tomography and recognized a strangulated obturator hernia on the left. Afterwards, we planned a surgical operation and repaired the obturator hernia laparoscopically. The patient was transferred to the intensive care unit because of respiratory failure after the operation was finished. The patient, who was followed in the intensive care unit, died five days later because of acute respiratory failure. Obturator hernia is a rare cause of acute intestinal obstruction. Therefore, increased mortality rates should be remembered in elderly patients because of delayed diagnosis and accompanying chronic diseases.

Pectoralis major myofascial flap in salvage laryngectomy

    Cömert, E.; Tunçel, U.; Torun, M. Taner; Kiliç, C.; Cengiz, A. Bugra; Sencan, Z.; Kaya, M.

Objective: The main purpose of this study was to evaluate the effect of the pectoralis major myofascial flap on pharyngocutaneous fistula formation and time to oral feeding. Methods: This retrospective study reviewed 155 total laryngectomies. Patients were divided into two main groups. Group 1 included 110 patients who were treated primarily by total laryngectomy and group 2 comprised 45 patients who were treated by salvage laryngectomy with or without a pectoralis major myofascial flap. Results: The use of a pectoralis major myofascial flap did not have a significant effect on pharyngocutaneous fistula formation in the salvage group (p = 0.376). When comparing the oral feeding day of patients with pharyngocutaneous fistula, a significant difference was observed between the salvage group with pectoralis major myofascial flap reinforcement and the salvage group without pectoralis major myofascial flap reinforcement (p = 0.004). Discussion: Our study demonstrated that pectoralis major myofascial flap reinforcement did not decrease the rate of pharyngocutaneous fistula formation. Instead, it prevented the formation of large fistulas that would require surgical management, and showed a similar time to oral feeding and length of hospital stay to primary laryngectomy.

Recurrent anterior shoulder instability: Review of the literature and current concepts

Sofu, Hakan; Gürsu, Sarper; Kockara, Nizamettin; Öner, Ali; Issin, Ahmet; Çamurcu, Yalkin

The purpose of this review article is to discuss the clinical spectrum of recurrent traumatic anterior shoulder instability with the current concepts and controversies at the scientific level. Because of increasing participation of people from any age group of the population in sports activities, health care professionals dealing with the care of trauma patients must have a thorough understanding of the anatomy, patho-physiology, risk factors, and management of anterior shoulder instability. The risk factors for recurrent shoulder dislocation are young age, participation in high demand contact sports activities, presence of Hill-Sachs or osseous Bankart lesion, previous history of ipsilateral traumatic dislocation, ipsilateral rotator cuff or deltoid muscle insufficiency, and underlying ligamentous laxity. Achieving the best result for any particular patient depends on the procedure that allows observation of the joint surfaces, provides the anatomical repair, maintains range of motion, and also can be applied with low rates of complications and recurrence. Although various surgical techniques have been described, a consensus does not exist and thus, orthopedic surgeons should follow and try to improve the current evidence-based treatment modalities for the patients. Yilmaz, Yunus Keser; Çelikbilek, Mehmet; Sarikaya, Savas; Okur, Aylin; Çelikbilek, Asuman; Atalay, Tugay; Akyol, Lutfi; Gencer, Nagehan Sozen; Çelik, Serkan; Ekim, Hasan

Arteriovenous fistula presents rarely with ascites. Diagnosis, with an elusive clinical presentation, is often incidental or delayed. A 35-year-old woman presented with ascites and cardiac decompensation. Contrast enhanced computed tomography revealed arteriovenous fistula between the left common iliac artery aneurysm and the left common iliac vein. The patient underwent endovascular treatment with arterial access was performed, with implantation of a stent graft in the iliac artery to cover the fistulous communication. At follow-up 1 month later, she was asymptomatic without ascites. Arteriovenous fistula should be considered in the differential diagnosis of patients with ascites and cardiac decompensation. The endovascular treatment of the arteriovenous fistula should be considered as a first line option.

Aktas, Binhan Kagan; Bulut, Suleyman; Erbay, Guven; Karabakan, Mehmet; Gökkaya, Cevdet Serkan; Memis, Ali

Objective: In this study, we aimed to evaluate the role of various comorbidities that lead to removal of a penile prosthesis due to infection. Methods: We reviewed the records of 50 patients who underwent malleable penile prosthesis implantation between January 2010 and May 2014. The patients were classified into 2 groups: Group 1 (n=11) contained patients whose prosthesis was removed due to infections that developed in the post-operative period; Group 2 (n=39) patients did not have any infectious complications. The 2 groups were compared regarding the presence of diabetes mellitus (DM), hypertension (HT), smoking habit, atherosclerotic heart disease (ASHD), Peyronie disease, and the level of mean preoperative glycosylated hemoglobin (HbA1c). Results: There were no differences between the 2 groups regarding the presence of DM, HT, smoking habit, ASHD, and Peyronie disease (p>0.05). However, there was a significant difference (p=0.006) between patients whose prostheses were removed due to infection and patients who did not have any infectious complications regarding the mean preoperative HbA1c level (10.7 +/- 1.6 and 7.4 +/- 1.19, respectively). Conclusion: Having a normal HbA1c level is more important than being diabetic or not, regarding penile prosthesis removal due to postoperative infection.

Sirtuins as novel players in the pathogenesis of diabetes mellitus

Turkmen, Kultigin; Karagoz, Ali; Kucuk, Adem

Diabetes mellitus (DM) is a systemic and complex disease with micro and macrovascular complications that result from impaired metabolic pathways and genetic susceptibilities. DM has been accepted as an epidemic worldwide during the last two decades. A substantial gap in our knowledge exists regarding the pathophysiology of this metabolic disorder despite the improved diagnostic tools and therapeutic approaches. Sirtuins are a group of NAD+ dependent enzymes that are involved in cellular homeostasis due to their deacetylating activity. In the present review, we aimed to discuss the role of associated sirtuins in the pathogenesis and treatment of diabetes mellitus. (C) 2014 Baishideng Publishing Group Inc. All rights reserved.

    Cardiac Trauma

Ozkaynak, Berk; Gümüs, Funda; Polat, Adil; Kayalar, Nihan; Erentug, Vedat

Advances in cardiac surgery over the past decades have enabled more efficient, complex and successful repairs of cardiac injuries. Cardiac injuries may be caused by blunt, penetrating or iatrogenic trauma. 25% of the trauma related deaths are caused by chest trauma. The lethal nature of this injury is due to the high concentration of major vascular and visceral structures within the mediastinum. The most common cause of blunt cardiac injury is high impact motor vehicle accidents. Other sources of blunt cardiac trauma are falls from heights, crush injuries and athletic injuries. Only 5% of these patients would receive medical intervention. Blunt trauma causes pericardial and epicardial hemorrhage and myocardial contusion. Myocardial contusion may range in severity from minor subepicardial hematoma to larger extravasations including the full thickness of the myocardium. Larger contusions lead to necrosis which may heal by scarring or may lead to lethal rupture of the myocardium. Rupture in the ventricles, atria, atrioventricular septum and pulmonary veins can be seen. Rupture in papillary muscles may lead to acute mitral or tricuspid failure. Fistulization, laceration and thrombosis in the coronary arteries may lead to further myocardial injury necessitating coronary artery bypass surgery. Patients with blunt cardiac trauma, either symptomatic or asymptomatic, should be monitored closely for possible complications of myocardial contusion. Penetrating cardiac injuries may be caused by gunshot wounds or stab wounds. Iatrogenic injuries may be encountered most commonly during catheterisation procedures and reoperations due to excessive adhesions caused by previous operations. Major factor affecting survival of these patients is the duration of transport from the scene of trauma to the medical center for interventions. Patients with prominent signs of cardiac tamponade should be taken into the operating room promptly and treated without necessitating further diagnostic tools. Emergency thoracotomy or sternotomy may be instituted to decompress the heart and control possible hemorrhage. In severe hypovolemic shock, emergency sternotomy may be performed before placement of intravenous lines and volume replacement with crystalloids, colloids and blood can be administered directly into the right atrium through large bore needles. Simple injuries may be repaired primarily with careful monitoring and stabilization of hemodynamics. In cases of larger defects or complex injuries such as valvular disruption and acute failure, injury of coronary arteries necessitating coronary artery bypass surgery and atrioventricular septal defects, cardiopulmonary bypass may be used to avoid further myocardial injuries. Close monitoring of asymptomatic cardiac trauma patients for at least 48 to 72 hours after admission for possible complications and aggressive and rapid intervention in patients with manifest symptoms are vital for survival and effective treatment of cardiac injuries.

How safe are orthoroentgenograms in determining the amount of correction for varus deformities?

Gürsu, Sarper; Yildirim, Timur; Issin, Ahmet; Sofu, Hakan; Sahin, Vedat

Objectives: In this study, we evaluated the effects of the distance of the legs from the midline on alignment and angles of the lower extremities in orthoroentgenograms. Patients and methods: Between March 2012 and April 2013, 95 limbs of 56 patients with varus deformity who underwent orthoroentgenogram to identify the amount of joint laxity in two positions were included in this study. The initial X-ray was performed with the feet in contact, while the other was performed as the legs were abducted to be in line with the shoulders. For each orthoroentgenogram, the mean mechanical axis angle, anatomical axis, and joint line orientation angles were measured retrospectively. These measurements were repeated for 43 limbs with varus deformity > 10 degrees. Results: In the orthoroentgenograms with the feet in contact, the mean mechanical axis angle was 9.58 degrees +/- 5.7 degrees, (0.20 degrees; 26.0 degrees), the mean anatomical axis angle 3.65 degrees +/- 6.14 degrees, (-9.0 degrees; 21.0 degrees), and the mean joint line orientation angle -3.41 degrees +/- 2.52 degrees, (-12.0 degrees; 1.60 degrees). In the orthoroentgenograms with the legs abducted, the mean mechanical axis angle was 7.73 degrees +/- 5.58 degrees, (-3.0 degrees; 23.0 degrees), the mean anatomical axis angle 2.62 degrees +/- 5.87 degrees, (-11.0 degrees; 18.30 degrees), and mean joint line orientation angle was -2.44 degrees +/- 2.41 degrees, (-13.0 degrees; 3.0 degrees). The differences in the angles between the two positions were statistically significant (p < 0.005). Conclusion: Our study results showed that the mean values of mechanical axis angle, anatomical axis and the joint line orientation angle were higher in orthoroentgenograms with the feet in contact than the orthoroentgenograms with the legs abducted in patients with yarns gonarthrosis. We suggest that this may lead to mistakes in the preoperative planning. Ideal positions should be standardized to minimize possible problems.

Mastitis, which is infection of breast tissue, is a rare infection apart from breast feeding period; it is a relatively frequent case at neonatal period. Breast tissue reserves inclination to infections due to early infancy immaturity. Conventionally, as breast tissue, which is hypertrophic and affected by maternal hormones at pregnancy, is squeezed, massaged, or a coin is fastened on it in our country, its being infected due to skin breakdown plays a significant role at neonatal mastitis. In our study, we would like to present a 14-day-old case where mastitis abscess developed after massage on right breast.

Background/aim: Anal fissures are one of the most common proctologic disorders. This study aimed to investigate alterations in anal sphincteric resting pressures after lateral internal sphincterotomy, which was performed for chronic anal fissure treatment. Materials and methods: Fifty-six (68.3%) male and 26 (31.7%) female patients were included in this study. Patients complicated with fistula-in-ano, abscess, cancer, inflammatory bowel disease, dermatitis, poor general condition, allergy to any of the standard medications, previous history of anal trauma, previous anal surgery, or diagnosis of a neurological disease and patients who did not give consent to the trial were excluded from the study. Two groups were formed: a control group with 41 healthy volunteers and a study group with 41 chronic anal fissure patients. Preoperative and postoperative manometric anal measurements were compared. Results: Preoperative mean resting anal sphincter pressure was 51.29 mmHg for the control group and was 59.99 mmHg for the patient group. Although resting anal sphincteric pressures of the patient group remained within the physiological range, when compared with the control group there was a statistically significant difference. Postoperative mean resting anal sphincteric pressures of the patient group was 32.43 mmHg. Conclusion: Lateral internal sphincterotomy decreases resting anal sphincter pressures effectively in chronic anal fissure patients.

The relationship between neutrophil-to-lymphocyte ratio and vascular calcification in end-stage renal disease patients

Turkmen, Kultigin; Ozcicek, Fatih; Ozcicek, Adalet; Akbas, Emin Murat; Erdur, Fatih Mehmet; Tonbul, Halil Zeki

Chronic inflammation was found to be correlated with coronary (CAC) and thoracic peri-aortic calcification (TAC) in end-stage renal disease (ESRD) patients. Neutrophil-to-lymphocyte ratio (NLR) was introduced as a potential marker to determine inflammation in cardiac and noncardiac disorders. Data regarding NLR and its association with TAC and CAC are lacking. We aimed to determine the relationship between NLR and vascular calcification in ESRD patients. This was a cross-sectional study involving 56 ESRD patients (22 females, 34 males; mean age, 49.9±14.2 years) receiving peritoneal dialysis or hemodialysis for ≥6 months in the Dialysis Unit of Necmettin Erbakan University. TAC and CAC scores were measured by using an electrocardiogram-gated 64-multidetector computed tomography. NLR was calculated as the ratio of the neutrophils and lymphocytes. There was a statistically significant correlation between NLR, TACS and CACS in ESRD patients (r=0.43, P=0.001 and r=0.30, P=0.02, respectively). The stepwise linear regression analysis revealed that age, as well as NLR were independent predictors of TACS. However, increased age was the only independent predictor of CACS according to linear regression analysis. Simple calculation of NLR can predict vascular calcification in ESRD patients.

Regulatory neuropeptides (ghrelin, obestatin and nesfatin-1) levels in serum and reproductive tissues of female and male rats with fructose-induced metabolic syndrome

Catak, Zekiye; Aydin, Suleyman; Sahin, Ibrahim; Kuloglu, Tuncay; Aksoy, Aziz; Dagli, Adile Ferda

Although, the exact mechanisms underlying the development of the metabolic syndrome (MetS) are not still completely understood, obesity, circulated peptide hormone levels and their interaction with genetic factors are considered largely responsible. The purpose of this study is to explore how the levels of ghrelin, obestatin (OBS) and NUCB2/nesfatin-1 (NES)/NUCB2 change in serum and the reproductive tissues of female and male rats with fructose-induced metabolic syndrome, and whether the levels of each hormone is correlated with the hormones involved with fertility. Experiments were conducted on 5-week-old Sprague-Dawley male and female rats assigned to either a control group or a MetS group. Controls were fed standard rat food and water ad libitum, while the MetS group was fed standard food with 10% (v/v) fructose solution added to their drinking water for 12. weeks with a 12/12. h photoperiod circle. Then, all animals were sacrificed after a one night fast. Peptides levels in the serum and reproductive tissues of rats were studied using the ELISA method while the immunoreactivity of reproductive system peptide hormones were shown by immunohistochemical staining method. Furthermore, the other biochemical parameters were measured using Konelab-60 equipment and infertility hormones were measured with Immulite2000. Fasting serum insulin, glucose, triglyceride, alanine aminotransferase (ALT), gamma glutamyl transpeptidase (GGT), low-density lipoprotein cholesterol (LDL-C), and total cholesterol (TC) levels were statistically significantly higher, and the amount of high density lipoprotein cholesterol (HDL-C) was significantly lower, in the MetS groups. Serum and tissue supernatant NES levels were significantly higher in the rats with MetS than the control group. Ghrelin, OBS and NES were expressed in the cytoplasm, concentrated around the apical parts of the epithelial cells in the reproductive tissues of the rats. The amounts of ghrelin were lower in the reproductive tissues of the animals with MetS, while NES levels in the same tissues increased. Obestatin also decreased, though not in the seminal glands.

Objective: Impact of systemic lupus erythematosus (SLE) on fertility may be negative, and ovarian function can be also reduced by autoimmune oophoritis. In this article, we evaluated the ovarian reserve of pre-menopausal women firstly diagnosed with systemic lupus erythematosus (SLE). Methods: This was a prospective controlled study which included twenty women with SLE and twenty healthy women as controls in the reproductive age. Basal levels of FSH, estradiol (E2), and LH on cycle day 3 were measured. All participants underwent transvaginal ultrasonographic examination on the third day of their menstrual periods for the determination of ovarian volume (OV) and total antral follicle count (AFC). Results: A significant difference in FSH, LH, and E2 levels was observed between women with SLE and healthy controls. There was a statistically significant reduction in total AFC and OV in SLE group. Age was associated negatively with AFC, whereas positively with FSH and LH. Menstrual irregularity was significantly higher in SLE patients than control. AFC was the most reliable test to show the menstrual irregularity and negatively correlated each other in women with SLE. Conclusion: In this preliminary study, the first in SLE patients, we illustrated that women with SLE had lower ovarian reserves and higher menstrual irregularity compared with healthy controls according to hormonal and ultrasonographical evaluation.

Genotoxic evaluation of two oral antidiabetic agents in the Drosophila wing spot test

Gürbüzel, Mehmet; Çapoğlu, İlyas; Kızılet, Halit; Halıcı, Zekai; Özçiçek, Fatih; Demirtaş, Levent

In this study, two sulfonylureas—glimepiride and glipizide—commonly used in type 2 diabetes mellitus were investigated for genotoxicity in the Drosophila wing spot test. For this purpose, three-day-old transheterozygous larvae were treated with three mutagenic compounds, and the results obtained were compared with the control group. Mutational or recombinogenic changes were recorded in two recessive genes—multiple wing hairs (mwh) and flare (flr 3). Two recessive markers were located on the left arm of chromosome 3, mwh in map position 0.3, and flare-3 (flr3) at 38.8, while the centromere was located in position 47.7. Wing spot tests are targeted on the loss of heterozygosity, which may be grounded in different genetic mechanisms such as mutation, mitotic recombination, deletion, half-translocation, chromosome loss, or nondisjunction. Genetic changes formatting in somatic cells of the imaginal discs cause nascence different mutant cloning in different body parts of adult flies. Our in vivo experiments demonstrated that glimepiride and glipizide show the genotoxicity, which is especially dependent on homologous somatic recombination.
