Akin, Yiǧit; Ateş, Mutlu; Yücel, Selçuk; Başara, Işil; Çelik, Orçun; Bozkurt, Aliseydi; Nuhoǧlu, Bariş

Aim: To evaluate the effects of different intracorporeal legation techniques with titanium clips, Plasma Trisector (Gyrus, USA) (PTG), and surgical silk on bilateral laparoscopic varicocelectomy (LPVx). Materials and methods: Between May 2009 and August 2012, 100 patients who underwent bilateral LPVx were evaluated. Te demographic parameters of patients, preoperative radiological findings, semen analysis, operative data, and follow-up were recorded. All of the patients were divided into 3 groups, randomized prospectively. Te patients whose veins were legated by 5-mm titanium clips were included in group I, those whose veins were legated by PTG were included in group II, and those whose veins were legated by surgical silk were included in group III. Te recorded data of the groups were analyzed. Results: Mean follow-up time was 18.8 ± 1.1 months. According to the demographics of age, body mass index, spermiogram, and diameter of veins before surgery, there were no statistical differences between any of the groups (P > 0.05). However, operation time was longer in group III (P < 0.0001), while total numbers of legated veins did not differ among the groups (P > 0.05). Additionally, hospital stay, oral intake, and complications were not different among the groups (P > 0.05). In follow-up the sperm count analysis was higher than the preoperative count analysis for all of the groups (P < 0.05). Conclusion: PTG may provide a shorter operation time than using titanium clips and/or surgical silk in LPVx. Additionally, PTG may increase sperm count with fewer complications than the other legation techniques in LPVx. Terefore, PTG may be the new candidate electrosurgical standard device for LPVx in the nearby future.

Aim: In this study, 31 patients with a diagnosis of Type II odontoid fractures were reported. All patients were treated with anterior transodontoid screw fixation and clinical outcomes were reported. Mater Ial and Meth ods: In this study, the retrospective clinical analysis of 31 patients with traumatic type II odontoid fractures who were treated through anterior transodontoid fixation in Neurosurgery Department at VKV American Hospital between 1998 and 2012 was performed. The age, sex, cause of injury, diagnosis time, neurological examination before and after surgery, follow-up period were evaluated. The neurological status of patients was classified according to the Frankel scale. Result s: In 4 patients, 2 transodontoid screws were inserted. The mean hospital stay was 3.35 days. Posterior occipito-cervical fusion was done in 1 patient due to the lack of fusion in the first operation. No vascular injury, screw malposition, infection, neurologic deterioration, or complications were observed during the peroperative and postoperative stage. The mean postoperative follow-up period was 36 months after surgery. Radiological imaging of patients were performed at the early and late postoperative stage. Conclusion: We found satisfying fusion rates and better patient comfort during the postoperative period. We think that stabilization and fusion through a transodontoid screw is a minimal invasive method.

Frequency, awareness, treatment and control of hypertension in the over 40 population of Erzincan; [Erzincan’da Kırk yaş ve üzeri bireylerde hipertansiyon sıklığı, farkındalığı, tedavisi ve kontrolü]

Aşılar, Rabia Hacıhasanoğlu; İnandı, Tacettin; Yıldırım, Arzu; Karakurt, Papatya; Sağlam, Rabia

RESULTS: The overall hypertension frequency was 67.0% (69.6% in women and 63% in men). In the study sample, 77.4% of individuals were aware of their hypertension, all were receiving anti-hypertensive drugs, and 33.9% of patients had their blood pressure under control. Among all hypertensive individuals, the percentage of controlled hypertension (HT) was 26.2%. The frequency of hypertension was associated with age, body mass index (BMI), smoking, level of education, the use of salt, and diabetes.; AIM: The aim of this study is to determine the frequency, awareness, treatment and control of hypertension in population over 40 years of age who came to the 5 primary health care centers in Erzincan for any reason.; METHODS: This cross-sectional study was completed with 1570 people aged 40 and older, who received care at health care centers between January and May 2009. The data were collected through face-to-face interviews, and blood pressures were measured twice. Participants were considered as hypertensive if their average systolic blood pressure was ≥140 mmHg, or their average diastolic blood pressure was ≥90 mmHg, or if they were taking antihypertensive medication. The awareness, treatment and control status of the individuals were also assessed.; CONCLUSION: These results indicate that the frequency of HT was high, and it was inadequately managed in individuals who came to the primary healthcare centers for any reason and received treatment. Many of the individuals were not aware of their illness. Therefore, it becomes very important and necessary to ensure early diagnosis and control of hypertension and to develop strategies to increase awareness.

Uludaǧ, Eda Ülkü; Gözükar, Ilay Öztürk; Kucur, Suna Kabil; Uluǧ, Paşa; Özdeǧirmenci, Özlem; Erkaya, Salim

Objective: To compare the initial serum magnesium levels between preterm labor (PL) and control groups and to evaluate MgSO4 treatment response in preterm labor group according to their initial serum magnesium levels. Methods: Hundred women diagnosed as preterm labor between 28 and 33 weeks of gestation and 100 non-complicated pregnant women were enrolled in this prospective study. Total basal serum magnesium levels were measured in both the groups. After a 6 g intravenous bolus of MgSO4, a dose of 2 g/h was given as an infusion in the preterm labor group. Results: Serum magnesium levels were significantly lower in preterm labor group (p50.001). The active contractions stopped in 69 (73,4%) preterm patients. The basal Mg level was significantly lower in this preterm group (1.6 versus 1.9, respectively, p50.001). Predictive value of basal magnesium level measurement for magnesium tocolysis response was calculated by receiver operating characteristic analyses with 95% confidence interval. Positive predictive and negative predictive values were found as 64.5% and 92.5%, respectively, with 83% accuracy, when cut-off magnesium value was taken as a 51.75 mg/dl (sensitivity=80%, specificity=84,1%). Conclusions: Basal magnesium levels in preterm labor had a predictive value in evaluating MgSO4 tocolysis response. It may help to select patients who are appropriate for MgSO4 tocolysis.

Very long-term angiographic results of off-pump coronary artery bypass graft surgery

Özkaynak, Berk; Yildirim, özgür; Aksüt, Mehmet; Onk, Oruç Alper; Kayalar, Nihan; Ömeroglu, Suat Nail; Erentug, Vedat; Yakut, Cevat

Background: This study aims to evaluate the long-term results of angiography in patients undergoing off-pump coronary artery bypass grafting (CABG). Methods: Between September 1993 and December 1995, a total of 162 patients who underwent off-pump CABG by the same two surgeons at a single center, followed by angiography were retrospectively analyzed. Results: The mean time from surgery to angiography was 9.8±1.2 years (range, 7.1 to 12.6 years). The overall patency rate of the left internal mammary artery graft was 95.57% and 46.03% in saphenous vein grafts (SVG). During follow-up, 27 patients (16.67%) underwent percutaneous coronary intervention. The procedure was performed due to severe stenosis in the SVG in eight of them and due to severe lesion formation in native coronary artery in the others. Five patients underwent repeated CABG. There was no significant association between pre-, intra-, and postoperative variables and graft patency. Conclusion: Off-pump CABG technique, in contrast to the conventional bypass, has well-known advantages such as lower transfusion of blood and blood products, shortened length of intensive care unit and hospital stay, reduced cost, and lower morbidity and mortality. Our very long-term angiographic follow-up study showed that off-pump CABG is as reliable as conventional bypass procedure with excellent outcome and patency particularly in the left anterior descending coronary artery. Therefore, off-pump CABG technique should be the primary procedure of choice in selected patients.

A 9-kg ovarian mucinous cystadenoma in a 14-year-old premenarchal girl

Biçer, Şenol; Erkul, Zehra; Demiryilmaz, Ismail; Peker, Necla

Objective: Rare disease Background: Although ovarian tumors are most commonly observed in adults, they relatively rarely occur in children. The majority of ovarian masses encountered in the premenarchal or childhood stages are non-neoplastic lesions such as benign functional cysts. Epithelial tumors account for 8-10% of all ovarian tumors and are histologically classified as mucinous or serous. The most common benign epithelial ovarian tumor is cystadenoma. Case Report: We report the case of a 14-year-old premenarchal girl with chronic abdominal pain, constipation, and abdominal enlargement. A computed tomography detected a huge left ovarian cystic tumor. A 9-kg ovarian tumor was removed surgically. Pathology showed a benign mucinous cystadenoma (MCA). Conclusions: Ovarian neoplasms in children present a diagnostic quandary, and very often the diagnoses are missed or delayed. When the diagnosis is made, a prompt and fertility-preserving surgical treatment must be performed and followed to prevent recurrence.

Predictors of epicardial adipose tissue in patients with type 2 diabetes mellitus

Akbas, Emin M.; Hamur, Hikmet; Demirtas, Levent; Bakirci, Eftal M.; Ozcicek, Adalet; Ozcicek, Fatih; Kuyrukluyildiz, Ufuk; Turkmen, Kultigin

Background: Epicardial adipose tissue (EAT), visceral fat depot of the heart, was found to be associated with coronary artery disease in cardiac and non-cardiac patients. Platelet-to-lymphocyte ratio (PLR) and neutrophil-to-lymphocyte ratio (NLR) were introduced as potential markers to determine inflammation in various disorders. Recently, atherogenic index of plasma (AIP) was found to be closely associated with atherosclerosis in general population. Waist circumference is commonly used to assess the risk factors in various metabolic disorders. There has been a well known relation between inflammation and peripheral adipose tissue in diabetes mellitus. However, the data regarding EAT and inflammation is scant in this population. Hence, we aimed to determine the relationship between PLR, NLR, AIP, waist circumference and EAT in diabetic patients. Methods. This was a cross-sectional study involving 156 patients with type 2 diabetes mellitus (87 females, 69 males; mean age, 53.62 ± 9.33 years) and 50 control subjects (35 females, 15 males; mean age, 51.06 ± 8.74 years). EAT was measured by using a trans-thoracic echocardiogram. Atherogenic index of plasma was calculated as the logarithmically transformed ratio of the serum triglyceride to high density lipoprotein (HDL)cholesterol. NLR and PLR were calculated as the ratio of the neutrophils and platelets to lymphocytes, respectively. Results: Waist circumference, PLR, NLR, AIP and EAT measurements were significantly higher in diabetic patients when compared to control subjects. When diabetic patients were separated into two groups according to their median value of EAT (Group 1, EAT < 4.53 (n = 78) and group 2, EAT ≥4.53 (n = 78)), group 2 patients had significantly higher Body mass index (BMI), waist circumference, AIP, NLR and PLR levels. In the bivariate correlation analysis, EAT was positively correlated with PLR, NLR, AIP, BMI and waist circumference (r = 0.197, p = 0.014; r = 0.229, p = 0.004; r = 0.161, p = 0.044; r = 0.248, p = 0.002; r = 0.306, p < 0.001, respectively). Waist circumference was found to be independent variables of EAT. Conclusions: Simple calculation of PLR and measurement of waist circumference were found to be associated with increased EAT in diabetic patients.

Evaluation of the temperament and character properties of patients with ankylosing spondylitis; [Bestimmung der Temperaments- und Charaktereigenschaften von Patienten mit ankylosierender Spondylitis]

    Gokmen, F.; Altinbas, K.; Akbal, A.; Celik, M.; Savas, Y.; Gökmen, E.; Reşorlu, H.; Karaca, A.

Purpose: The aim of this study was to evaluate temperament and character of ankylosing spondylitis (AS) patients and to examine the association between these specific temperament and character properties and clinical variables.; Patients and methods: This study involved 73 AS patients. Temperament properties of patients were evaluated using Cloninger’s Temperament and Character Inventory (TCI). Association between clinical variables and specific temperament features were evaluated using correlation and regression analyses.; Results: Forty eight (65.8 %) of the study participants were men and the mean age was 42 ± 11.4 years. There was slight negative correlations between self directedness (S) and Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) scores (p = 0.01, r = − 0.30), and between the Visual Analog Scale (VAS) and reward dependence (RD) scores (p = 0.03, r = − 0.26). Regression analysis showed that correlations between BASDAI and S, and between VAS and RD scores were statistically significant.; Conclusion: Our study showed that the dimensions temperament and character are related to disease activation, and disease course is more severe in patients who have low scores in these TCI dimensions. Therefore, we suggest that evaluating temperament and character properties of AS patients will help clinicians to predict treatment compliance and motivation of patients during disease course.

Kisaoglu, Abdullah; Ozogul, Bunyami; Turan, Mehmet Ibrahim; Yilmaz, Ismayil; Demiryilmaz, Ismail; Atamanalp, Sabri Selcuk; Bakan, Ebubekir; Suleyman, Halis

Background: This study investigated the effect of thiamine pyrophosphate (TPP) on oxidative liver damage induced in rats with high-dose paracetamol. Methods: Rats for this experiment were divided into the following groups: healthy control, paracetamol control, thiamine+paracetamol, TPP+paracetamol, and N-acetylcysteine+paracetamol. Oxidant and antioxidant parameters and liver function test levels were compared between the groups. Results: The results show that TPP and N-acetylcysteine with paracetamol equally prevented a rise in oxidants such as malondialdehyde and nitric oxide. They also prevented a decrease in enzymatic and nonenzymatic antioxidants such as glutathione, glutathione peroxidase, glutaredoxin, glutathione S-transferase, superoxide dismutase, and catalase in the rat liver. Conclusion: Thiamine pyrophosphate and N-acetylcysteine had a similar positive effect on oxidative damage caused by paracetamol hepatotoxicity. These findings show that TPP may be beneficial in paracetamol hepatotoxicity.

Peker, Kemal; Sayar, Ilyas; Gelincik, Ibrahim; Bulut, Gülay; Ünal, Tuba Dilay Kökenek; Şenol, Serkan; Gökçe, Aysun; Isik, Arda

Background: The importance of the matrix metalloproteinase-7 (MMP-7) and nestin immunomarkers, C-kit proto-oncogene (CD117), and the efficiency of the Ki-67 proliferation index for gastrointestinal stromal tumors were evaluated. Material/Methods: This study was conducted by examining the microscope slides of 72 patients with gastrointestinal stromal tumors that were sent to the pathology laboratory between 2007 and 2012. Immunohistochemical staining for CD117, MMP-7, nestin, and marker of proliferation Ki-67 was performed. The correlations between the positive results for Ki-67, CD117, MMP-7, and nestin were evaluated relative to the tumor characteristics of size, localization, grade, cellular type, cellularity, cytology type, growth pattern, ulceration, necrosis, hemorrhage, invasion depth, and lymph node metastasis. Results: The tumor was localized in the stomach in 42 of the patients, the intestines in 19, the colon in 7, and the rectum in 4. Comparisons among the groups showed that MMP-7 was correlated with the tumor grade (p<0.001), cellularity (p<0.009), cytologic atypia (p<0.001), ulceration (p=0.002), necrosis (p<0.001), and tumor size (p=0.001). Nestin was correlated with the tumor grade (p=0.013), and tumor size (p=0.024). Correlations among CD117, MMP-7, nestin, and Ki-67 were examined. Nestin and Ki-67 were both significantly correlated with CD117 and MMP-7 [(r=0.279, p=0.018), (r=0.322, p=0.006), (r=0.386, p=0.001), (r=0.386, p=0.002)], respectively. Conclusions: MMP-7 and nestin may be beneficial as markers, given their sensitivity to gastrointestinal stromal tumors.
