Türkmen, Kultigin; Erdur, Fatih Mehmet; Dalmaz, Mahmut; Yeksan, Mehdi; Akkus, Idris; Türk, Suleyman

OBJECTIVE: Recombinant human erythropoietin (r-HuEPO) was found to be associated with increased blood pressure in hemodialysis (HD) patients. Plasma endothelin-1(ET-1) was reported to be increased during treatment with r-huEPO in HD patients. Prostacyclin (PGI2) was found to be associated with the reduction in vascular resistance. We aimed to determine the role of ET-1 and PGI2 on r-huEPO-induced hypertension in HD patients. MATERIAL and METHODS: 43 HD patients (19 females, 24 males) were randomly assigned to groups. Group 1 (n= 15) consisted of patients receiving r-HuEPO-a subcutaneously after each dialysis session for 6 to 42 months. Group 2 (n= 15, control group) consisted of patients who did not receive any r-huEPO and group 3 (n= 13) consisted of patients who received r-huEPO-a subcutaneously after each dialysis session for 12 weeks. Plasma ET-1,2 and 6-keto-PGF1-a were measured. RESULTS : There were no differences in terms of ET-1,2 and 6-keto-PGF1-a, systolic and diastolic BP and hematocrit levels in group 1 and group 2 patients. However, in group 3 patients, hematocrit, plasma ET1,2 and 6-keto PGF1-a levels were significantly elevated after 12 weeks of rHuEPO treatment. CONCLUSION: We showed that ET-1.2 and 6-keto-PGF1-a levels were found to be significantly increased with 12 weeks of rHu-EPO therapy in HD patients.

Surgical Management of Inferior Vena Cava Tumor Thrombus in Patients with Renal Cell Carcinoma

Kayalar, Nihan; Özkaynak, Berk; Gümüs, Funda; Kuplay, Huseyin; Nuhoglu, Baris; Erentug, Vedat

Renal cell carcinoma (RCC) is associated with a biological propensity for vascular invasion with extension of the tumor to the renal vein or inferior vena cava and total resection of the renal tumor and IVC tumor thrombus is considered the optimal therapy when no distant metastases are present. The surgical approach to a specific patient with RCC and IVC tumor thrombus should be selected according to the level of the tumor thrombus and the characteristics of the primary renal tumor such as size, location, regional lymphadenopathy and aberrant vascular anatomy. The surgical management of RCC and IVC tumour thrombus requires the commitment of a multidisciplinary surgical team particularly for a higher (level III and IV) tumor thrombus. Preoperative imaging is very important for the surgical planning and management. An extended chevron incision with or without a sternotomy provides excellent surgical exposure. Level 0, level I and most level II tumors can be removed using straightforward occluding maneuvers, while maintaining control of the caval thrombus and the contralateral renal vein. Vascular bypass can frequently be required for the safe and complete resection of level III tumor thrombus. The resection of level IV tumors is generally performed with the use of cardiopulmonary bypass and circulatory arrest. In this review, these surgical aspects and treatment of IVC tumor extension in patients with RCC will be summarized, hoping to provide a clear and comprehensive guideline for both urology and cardiovascular specialists.

Investigation of Some Vitamin Type Inhibition on Human Cord Blood Carbonic Anhydrase I and II

Çetin, Ahmet; Çoban, Taha Abdulkadir; Çankaya, Murat; Nayki, Usmit; Kuzucu, Mehmet

Objective: Human cord blood, a specialized blood tissue, contains many substances including several enzymes which act by different biochemical reactions. Carbonic anhydrase (CA) isoenzymes are one of the most important enzyme family which are crucial for living organisms to survive. However, there is limited data regarding the effect of vitamins on CA isoenzymes. The aim of this study is to test the inhibitory effect of some widely used vitamins on human cord blood CA I and II (hcbCA I and hcbCA II). Methods: We purified hcbCA I and hcbCA II from human cord blood erythrocytes by Sepharose-4B-l-tyrosine- sulfanilamide affinity gel chromatography. The inhibitory effects of vitamin A (retinol), vitamin B12 (cyanocobalamin) and vitamin K (menadione sodium bisulfate, k3) on two isoenzyme were checked using IC50 values. Result: IC50 values for vitamin A, vitamin B12 and vitamin K were found to be 44, 90, and 31 mu M for hcbCA I and of 51, 62, and 21 mu M for hcbCA II; respectively. All these substances were found to be non-competitive inhibitors. Conclusion: Vitamins are vitally important for continuation of life and activity of some of enzymes. While the physicans recommend pregnant women to take vitamins during pregnancy, we suggest that the findings obtained in this study be taken into consideration.

    Çelik, Necla; Arikan, Duygu

Objective: This study was semi-experimentally conducted in order to determine knowledge levels of child development students about foreign body aspiration and to explore the effect of the training given to these students about foreign body aspiration upon their knowledge levels. Methods: The research was conducted with a total of 231 students who studied at the department of child development of two high schools and one vocational higher school in Erzincan Province located in the east of Turkiye between May 2011 and February 2012. The data were gathered using a questionnaire form of 38 questions. The training on foreign body aspiration (Power point presentation, video presentation and practice on a dummy) was provided in three phases. For the data evaluation; percentages, chi-square test and Mc Nemar test were used. Results: It was found out in the study that 91.8% of the students did not take any training/course about foreign body aspiration and 55% confronted situations of foreign body aspirations (ear, nose, airway). After the training about foreign body aspiration; it was noted that the number of the students who knew risky behaviors for foreign body aspiration increased and the increase in the number of the students was at the highest level among high schools (p<0.05). It was seen that the training was most effective among the university students in terms of knowing first-aid intervention in case a foreign body would obstruct airway among the children aged. <1 year (p<0.05). Conclusion: It was noted that the training which was applied on the dummy by using visual aids about foreign body aspiration increased the knowledge level of the students. Based on this finding, it may be recommended that nurses should perform their counseling and educator roles more actively for the families and those who are responsible for the baby care about foreign body aspiration by playing a more active role in the health care team.

Rare malformation of glans penis: Arteriovenous malformation

    Akin, Y.; Sarac, M.; Yucel, S.

Pediatric glans penis malformations, especially arteriovenous malformations (AVM), are very rare. Herein, we report two rare cases. A 14-year-old boy attended our outpatient clinic with chief complaints of purple swelling and rapidly growing lesion on the glans penis. The lesion was excised surgically after physical and radiological evaluations. Pathology reported AVM and the patient is being followed up. The second case is a 2-year-old boy who was admitted with a big lesion involving glans penis and genital area that has been present since birth. In physical and radiological evaluations, lesion on the glans penis was pulsatile. Parents of the patient did not want any surgery and patient has been in follow-up. Diagnosis of the vascular lesions on glans penis is very easy by physical and radiological examinations today. Long-term follow-up is very important for AVM. Clinicians must make a careful effort to document new glans lesions in the pediatric population and decrease anxiety in the parents of affected children.

Secondary/tertiary benzenesulfonamides with inhibitory action against the cytosolic human carbonic anhydrase isoforms i and II

Alp, Cemalettin; Maresca, Alfonso; Alp, Nurdan Alcan; Gültekin, Mehmet Serdar; Ekinci, Deniz; Scozzafava, Andrea; Supuran, Claudiu T.

Carbonic anhydrase inhibitors of primary sulfonamide type, RSO2NH2, have clinical applications as diuretics, antiglaucoma, antiepileptic, antiobesity and antitumor drugs. Here we investigated inhibition of two human cytosolic isozymes, hCA I and II, with a series of secondary/tertiary sulfonamides, incorporating tosyl moieties (CH3C6H4SO2NR1R2). Most compounds inhibited both isoforms in low micromolar range, with inhibition constants between 0.181-6.01 μM against hCA I, and 0.209-0.779 μM against hCA II, respectively. These findings point out that substituted benzenesulfonamides may be used as leads for generating interesting CAIs probably possessing a distinct mechanism of action compared to primary sulfonamides. Indeed, classical RSO2NH2 inhibitors bind in deprotonated form to the Zn(II) ion from the CA active site and participate in many other favorable interactions with amino acid residues lining the cavity. The secondary/tertiary sulfonamides cannot bind to the zinc due to steric hindrance and probably are accommodated at the entrance of the active site, in coumarin binding-site.

Mapping corneal thickness using dual-scheimpflug imaging at different stages of keratoconus

Demir, Selim; Ortak, Hüseyin; Yeter, Volkan; Alim, Sait; Sayn, Osman; Taş, Ufuk; Sönmez, Barş

Purpose: To map the thickness of the entire cornea using dual-Scheimpflug imaging and to evaluate the changes in the corneal thickness over the entire cornea at different stages of keratoconus. Methods: Corneal pachymetry was performed using the Galilei dual-Scheimpflug analyzer. The thinnest (TCT), central (CCT), paracentral (PaCT), and peripheral corneal thicknesses (PeCT) were also analyzed. The study examined 150 eyes of 150 patients who had myopia or myopic astigmatism and 107 eyes of 75 patients with keratoconus. Of these 107 eyes, 48 were evaluated at stage I keratoconus, 32 at stage II, 12 at stage III, and 15 at stage IV keratoconus. The level of severity of the keratoconus was based on the Amsler-Krumeich classification. Results: There were significant decreases in the thickness values of the entire corneas at all the different stages of progression defined in the Amsler-Krumeich classification. Analysis of the receiver operating characteristic curve showed that the TCT provided a better parameter than did the CCT, PaCT, and PeCT for distinguishing between keratoconus at its different stages and myopic eyes. Although the TCT and CCT parameters provided an effective distinction of eyes with stage II, III, and IV keratoconus from normal eyes, they were not effective for discriminating eyes with stage I keratoconus from eyes with myopia. But, PaCT and PeCT parameters enabled the effective discrimination between eyes with stage IV keratoconus and those with myopia only. Conclusions: The data obtained by dual-Scheimpflug imaging for the corneal thicknesses of the entire cornea provide useful information for grading the severity of keratoconus.

The effect of first dose of tamsulosin on flow rate and its predictive ability on the improvement of LUTS in men with BPH in the mid-term

Akin, Yigit; Gulmez, Hakan; Ucar, Murat; Yucel, Selcuk

Purpose To investigate if effects of the first single dose of tamsulosin 0.4 mg on uroflowmetry parameters would predict treatment response at the third month. Methods Men over 40 years old with complaints of lower urinary tract symptoms associated with benign prostatic hyperplasia were studied with physical examination, urine and blood analysis, uroflowmetry (UFM), post-voiding residual urine volume (PVR), international prostate symptom score (IPSS), qualityof- life (QoL) index and transrectal prostate ultrasonography (TRUS), and patients with prostate cancer suspicion, past urinary surgery, urinary tract infection and neurologic diseases were excluded. UFM, PVR, IPSS, QoL were repeated at 6th hour of the first day, first month and third month of oral tamsulosin 0.4 mg treatment. All parameters were recorded as baseline, and changes in the UFM parameters, PVR, IPSS and QoL were evaluated in clinical visits. Results As a total, 48men (mean 60.17 ± 1.18 years) were recruited. There was a significant increase in maximumurine flow rate (Q max) and average urine flow rate (Qave) and decrease in PVR from baseline with the first dose of tamsulosin as well as first and third month of treatment (p<0.05). IPSS andQoL scores significantly improved at the first month in correlation with UFM parameters. Tamsulosin treatment was effective in 33 (68.7 %) patients at the first administration and 35 (72.9 %) at the third month. Positive predictive value and negative predictive value ofQmax change at first dose for the third month response rate were 90.9 and 66.6 %, respectively. Conclusions The mid-term effectiveness of tamsulosin may be predicted by changes in UFM parameters achieved with its first dose.

Comparison of vessel sealing systems with conventional

Peker, Kemal; Inal, Abdullah; Güllü, Huriye; Gül, Düriye; Şahin, Murat; Ozcan, Ayca Dumanli; Kiliç, Kemal

Background: Haemorrhoids are cushions of submucosal vascular tissue located in the anal canal starting just distal to the dentate line. Haemorrhoidal disease is a common anorectal disorder which has symptoms of bleeding, prolapse, pain, thrombosis, mucus discharge, and pruritus. Haemorrhoidectomy is one of most frequently performed anorectal operation worldwide. Objectives: The aim of this study was to compare the effectiveness of the LigaSure tissue sealing device, Harmonic Scalpel and conventional MM open haemorrhoidectomy. Materials and Methods: Sixty-nine patients with newly diagnosed symptomatic grade three or grade four haemorrhoidal disease, from July 2011 to December 2011 were recruited for the study. Patients were prospectively randomized to LigaSure, Harmonic Scalpel and conventional haemorrhoidectomy. Patients were evaluated on the basis of the mean operative time, postoperative pain, day of discharge, early and late complications. Results: Each group has twenty-three patients. Ten (14.5 %) were female and fifty-nine (85.5 %) were male. Mean age were 44.5 ± 10.8 for LigaSure group, 39.5±14.4 for Harmonic Scalpel group and 39.8 ± 13.6 for conventional haemorrhoidectomy group. Mean operative time was 12.6 ± 2.9 for LigaSure group, 12.6 ± 2.5 for Harmonic Scalpel group and 22.3 ± 4.5 for conventional haemorrhoidectomy group. Postoperative pain and required analgesic dose were significantly lower for conventional haemorrhoidectomy. Wound healing was also more rapid in conventional haemorrhoidectomy than both LigaSure and Harmonic Scalpel. Conclusions: Lateral heat dissipation of energy based cautery such as Harmonel Scalpel and LigaSure is considerably high when compared with conventional methods. More thermal damage which is generated on tissue seems to be the reason for increased degree of postoperative pain and delay in wound healing.

Aydin, Suleyman; Aydin, Suna; Kuloglu, Tuncay; Yilmaz, Musa; Kalayci, Mehmet; Sahin, Ibrahim; Cicek, Demet

The purpose of this study was to ascertain (1) whether human saliva contains irisin and whether its level correlates with serum irisin concentration, (2) whether salivary glands, eccrine glands and sebaceous glands in human skin produce irisin, (3) how the changes in saliva and serum irisin concentrations after the Turkish bath at 47 ± 3 C compare with the changes caused by moderate exercise in obese and normal weight subjects. Seven obese male subjects and seven normal weight subjects were enrolled for Turkish bath. Seven obese male subjects and seven normal weight subjects were also enrolled for moderate outdoor exercise, and thirteen male normal weight subjects neither exercised nor showered at the Turkish bath. From each participant, 1.5 ml of saliva and 5 ml blood were collected simultaneously before and after the moderate exercise and Turkish bath. Salivary glands and eccrine and sebaceous glands in the skin were screened immunohistochemically for irisin while serum and saliva irisin were measured with an ELISA. Submandibular glands, eccrine glands and sebaceous glands in the human skin showed strong irisin immunoreactivity. Human saliva contained irisin and its level was significantly higher than the serum levels in both obese and normal weight subjects. However, irisin concentrations were more markedly increased in both saliva and serum samples from subjects who had showered at a Turkish bath than in obese subjects who had exercised or in normal weight subjects. Human submandibular glands, eccrine sweat glands and sebaceous glands synthesize irisin.
