Objective: Rare disease Background: Phytobezoars are a common type of bezoar known to accumulate in the gastrointestinal system. Case Report: A 30-year-old mentally retarded woman was operated on due to small bowel obstruction. A piece of tree root was extracted from the ileum. The patient was discharged from the hospital on postoperative day 7. Conclusions: Due to difculties in determining the nature of the bezoar preoperatively, small bowel obstruction due to a phytobezoar is not a common diagnosis. For bezoars, prevention is considered preferable to treatment.

Akin, Yigit; Yucel, Selcuk; Baykara, Mehmet
Paraneoplastic syndromes (PNSs) are rare disorders which are triggered by an altered immune system response to a neoplasm. Although prostate cancer (PCa) is the second most common urological malignancy associated with PNSs, literature is lacking in defining the cases representing different PNSs in PCa. Herein, we present a 50-year-old man with fulminant distal phalangeal necrosis in the lower extremities after a diagnosis of PCa. Additionally, we review the literature in light of this case.

Background: In patients presenting with non-ST elevation myocardial infarction, we investigated the relationship of left atrial deformational parameters evaluated by 2-dimensional speckle tracking imaging (2D-STI) with conventional echocardiographic diastolic dysfunction parameters and brain natriuretic peptide level.; Material/Methods: We enrolled 74 non-ST segment elevation myocardial infarction patients who were treated with percutaneous coronary intervention and 58 healthy control subjects. Non-ST segment elevation myocardial infarction patients had echocardiographic examination 48 h after the percutaneous coronary intervention procedure and venous blood samples were drawn simultaneously. In addition to conventional echocardiographic parameters, left atrial strain curves were obtained for each patient. Average peak left atrial strain values during left ventricular systole were measured.; Results: BNP values were higher in non-ST segment elevation myocardial infarction patients compared to controls. Mean left atrium peak systolic global longitudinal strain in Group 2 (the control group) was higher than in the nonST segment elevation myocardial infarction group. Left atrium peak systolic global longitudinal strain was significantly correlated with left ventricular ejection fraction. There was a significant inverse correlation between left atrium peak systolic global longitudinal strain and brain natriuretic peptide level, left atrium volume maximum, and left atrium volume minimum.; Conclusions: Our study shows that Left atrium peak systolic global longitudinal strain values decreased consistently with deteriorating systolic and diastolic function in non-ST segment elevation myocardial infarction patients treated with percutaneous coronary intervention. Left atrium peak systolic global longitudinal strain measurements may be helpful as a complimentary method to evaluate diastolic function in this patient population.

Akin, Y.; Gulmez, H.; Bozkurt, A.; Nuhoglu, B.; Usta, M.F.
Erectile dysfunction (ED) is the most common male sexual problem worldwide. The association between ED and components of metabolic syndrome (MtS) is well established. This study examined neck circumference (NC) as a possible indicator of MtS and also of ED. Ninety-two patients were included and divided into two groups. Group 1 consisted of 47 patients with ED and Group 2 consisted of 45 healthy volunteers. Questionnaires, differences in anthropometric and laboratory measurements between patients with ED and the control group, and a cut-off value for NC were investigated. The mean NC in ED patients was higher in Group 1 than in Group 2 (P = 0.001), and Group 1 also demonstrated more MtS criteria than Group 2 (P < 0.001). The cut-off point of NC was defined as 34.75 cm for ED and MtS. The cut-off values of waist circumference for ED and MtS were 105.5 and 102.5 cm respectively. In the light of these findings, NC may be a new component of MtS in ED patients. Additionally, NC may be a novel indicator of central obesity and ED. We suggest that NC values of 35 cm and over may predict ED in patients with MtS.

Purpose: To examine perioperative, pathologic, and early continence outcomes of laparoscopic radical prostatectomy (RP) aided by a new-generation three-dimensional (3D) display system and compare them with those from the same operation aided by a conventional, two-dimensional (2D) display system. Patients and Methods: A total of 95 consecutive patients underwent laparoscopic RP for clinically localized prostate cancer (PC) by an experienced single surgeon from October 2009 to December 2012. Baseline characteristics, perioperative and pathologic variables, and continence data at 3 months after surgery were retrospectively reviewed from a prospectively maintained database. Categoric and continuous variables were compared using chi-square, Student t, and Wilcoxon rank-sum tests, as appropriate. Results: A total of 29 patients underwent laparoscopic RP using a 3D display system and 66 patients underwent laparoscopic RP using a 2D display system. The two groups were comparable for all clinical and pathologic variables. Mean total operative time for the 3D group was 131 minutes (standard deviation [SD]±18) compared with 190 (SD±31) for the 2D group (P<0.001). Mean time to perform the urethrovesical anastomosis was 28 minutes (SD±6) for the 3D group compared with 87 minutes (SD±17) for the 2D group (P<0.001). Blood loss was lower in the 3D group, and the difference was statistically significant (P<0.001). A statistically significant higher number of patients in the 3D group had early recovery of continence compared with patients in the 2D group (14/28 (50%) patients in the 3D group vs 16/64 (25%) patients in the 2D group, P=0.02). Conclusions: Laparoscopic RP aided by a new-generation 3D display system is associated with shorter operative times, reduced blood loss, and higher early continence rates in comparison with that aided by a 2D display system. In particular when considering economic issues, 3D laparoscopic RP may represent an acceptable alternative to robot-assisted laparoscopic RP.

Pilonidal sinus disease is a common disease of young adults, which most frequently occurs in the sacrococcygeal region on the skin's midline. Various procedures, ranging from simple incision and curettage to complex flaps for natal cleft obliteration, have been described in the literature. Material/Methods: We aimed to present the dermographic characters, post-operative complications, length of stay in hospital, time of return to daily activities, and recurrence rates of the patients in which we applied sinus excision and fasciocutaneous V-Y advancement flap due to primary complicated or recurrent sacrococcygeal pilonidal sinus disease. Results: Patients with primary complicated and recurrent pilonidal sinus received a fasciocutaneous V-Y advancement flap in the general surgery service of our hospital. Eleven patients had recurrent disease. Thirty-seven patients received a unilateral V-Y flap and 8 patients received a bilateral V-Y flap. None of the patients had post-operative flap necrosis or wound opening. Two of the patients had a self-draining simple seroma and 3 of the patients had delayed wound healing in the perianal region of the incision, which was treated with dressing. The mean time required to return to daily activities was 7 days, and return to work took 17 days. In the mean 25-month follow-ups of the patients, no recurrences were detected. Conclusions: We think that fasciocutaneous V-Y advancement flap is an easily learned and practicable method that reduces the recurrences in the patients with primary complicated and recurrent pilonidal sinus, length of stay in hospital, and time to return to daily activities and work in the post-operative period.

Buyuklu, Mutlu; Bakirci, Eftal Murat; Topal, Ergun; Ceyhun, Gokhan
Warfarin is commonly used for prevention of embolic events. Bleeding is the main side effect of warfarin. Lingual and sublingual haematoma are rare. In the literature, nine cases have so far been reported. We report the case of a 70-year-old Caucasian woman who developed spontaneous lingual and sublingual haematomas while on warfarin therapy. Spontaneous lingual and sublingual haematoma are rare, but can be potentially life-threatening complications as they cause airway obstruction. To the best of our knowledge, this is the first reported case of earliest haematoma after warfarin use. Copyright 2014 BMJ Publishing Group. All rights reserved.

Background: We evaluated the effects of local fax seed oil and glycerol application for reducing adhesion formation after thyroidectomy. Material/Method: We randomly assigned 18 female Wistar albino rats (median weight: 275 g, median age: 4.5 mth) to 3 groups: 0.1 ml 0.9% NaCl, glycerol, and fax seed oil were sprayed in a perithyroidal area after thyroidectomy operation on all animals as anti-adhesive barriers. Rats were sacrificed on the postoperative 14th day and adhesions were evaluated macroscopically and microscopically. Results: The median macroscopic adhesion score was 3.0±0.0 in the 0.9% NaCl group, 1.33±0.52 in the glycerol group, and 1.67±0.53 in the fax seed oil group. The median histopathological fibrosis scores were 2.33±0.82, 0.67±0.52, and 0.83±0.75, respectively. Both glycerol and flaxseed oil group macroscopic and microscopic scores were significantly lower than the 0.9% NaCl group (p<0.05). There was no significant difference among the groups (p>0.05). Conclusions: Glycerol and fax seed oil both decrease the incidence of post-thyroidectomy adhesion in rats, but glycerol is more effective.

RDL Kalıcı Kaydıİncele - ArticleAçık Erişim2013A Novel Approach to the Surgical Treatment of Lumbar Disc Herniations: Indications of Simple Discectomy and Posterior Transpedicular Dynamic Stabilization Based on Carragee Classification
Ozer, A. F.; Keskin, F.; Oktenoglu, T.; Suzer, T.; Ataker, Y.; Gomleksiz, C.; Sasani, M.
Surgery of lumbar disc herniation is still a problem since Mixter and Barr. Main trouble is dissatisfaction after the operation. Today there is a debate on surgical or conservative treatment despite spending great effort to provide patients with satisfaction. The main problem is segmental instability, and the minimally invasive approach via microscope or endoscope is not necessarily appropriate solution for all cases. Microsurgery or endoscopy would be appropriate for the treatment of Carragee type I and type III herniations. On the other hand in Carragee type II and type IV herniations that are prone to develop recurrent disc herniation and segmental instability, the minimal invasive techniques might be insufficient to achieve satisfactory results. The posterior transpedicular dynamic stabilization method might be a good solution to prevent or diminish the recurrent disc herniation and development of segmental instability. In this study we present our experience in the surgical treatment of disc herniations.

Objective: We aimed to determine whether having a newly served psychiatric outpatient clinic has a significant effect in a district on has or not effect on suicides. Method: There had never been a psychiatrist in the district hospital before September 2009, at which date the outpatient psychiatry clinic opened and began accepting patients. We compared the frequency of suicides and related factors in the first year before and after opening of psychiatric outpatient clinic. Results: Of all suicides 115 (54%) and 98 (46%) were admitted within the first year before and after the opening of the psychiatry outpatient clinic, respectively which was statistically marginal significant (p=0.084). The age, gender, marital, educational and working status of the suicide attempters were similar before and after opening of psychiatric outpatient clinic. Among the people who attended to outpatient psychiatry clinic after the suicide, the repeated of suicide attempts were significantly higher than the single suicide attempts (p=0.036). 19.3% (n=41) of all suicide attempts attended to the outpatient psychiatry clinic both two years. 36% of these patients attended only once, while 64% has attended regularly. Of the 41 total suicide attempters who attended the psychiatry outpatient clinic for suicide, 4 (9.8%) have repeated suicide attempt within a year after opening of outpatient psychiatry clinic. Conclusions: The presence of a psychiatry oupatient clinic may a protective factor in terms of suicide attempts. A multidisciplinary study team to prevent suicide attempts is needed, as dictated by the multidimensional nature of the issue.

Objective: We aimed to compare renal injury and vasculary resistance between standard and tubeless percutaneous nephrolithotomy (PCNL), in patients who were operated for kidney stone, by using colored Doppler ultrasonography (CDU). Methods: All consecutive PCNLs were evaluated between 2009 and 2011. The patients with access to the kidney provided in the lower pole and who visited our outpatient clinic regularly were enrolled in the study. Patients were divided into 2 groups; patients who had standard PCNL were included in Group 1 (n=22) and patients who had tubeless PCNL were included in Group 2 (n=21). The demographic data were collected from files of patients and were recorded. CDU was performed in all patients for evaluating resistive index (RI), parenchymal thickness and parenchymal echogenity before operation, after operation early postoperative (after 7 days nephrostomy catheter was taken off in Group 1, 7 days from operation in Group 2) and midterm postoperative period (6 months after operation). Statistical significant p was accepted as p<0.05. Results: Mean age was 47.18 +/- 1.3 years. There were 8 female and 14 male patients in Group 1 (n=22), 4 female and 17 male patients in Group 2 (n=21). The mean follow-up was 10.71 +/- 1.2 months. There was no statistical difference for mean age, body mass index, stone burden, serum creatine, hemoglobin, delta hemoglobin, complications, urinary culture, stone free rate between groups. Hospital stay was longer in Group 1 than Group 2 (p=0.04). Mean operation time was shorter in Group 2 than Group 1 (p=0.002). In the comparison of before and early postoperative operation, there were differences in groups for CDU, including RI, parenchymal thickness. There was an increase for RI and decrease for parenchymal thickness in midterm follow-up period when comparing RI before operation in the lower pole of the operated kidney. There was no difference in renal echogenity in CDU in groups between before and mid-term follow-up period. Conclusion: Recently, PCNL has been employed as a minimally invasive and safe procedure for kidney stones bigger than 2 cm. Tubeless PCNL was successful as standard PCNL, provided shorter hospital stay than standard PCNL and caused less damage to the kidney in the early follow-up period.
