Can perfusion index be used as an objective tool for pain assessment in labor analgesia?

    Kupeli, Ilke; Kulhan, Nur Gozde

Objective: To establish a relationship between the Visual Analog Scale for pain (VAS) in the recovery time of epidural analgesia and the Perfusion Index (PI) values at that time and to test the possibility of using PI as an objective tool for pain assessment. Methods: Thirty women were included in the study. After inserting epidural catheter, the initial applicationtime of epidural analgesia was taken as 0th minute. Hemodinamics, VAS, and PIvalues were recorded at 5th, 10th, 30th, 60th minutes and every two hours until the birth and the 30th minute after the birth. Results: HR, SAP, DAP, PI, VAS values before the procedure were different than all follow-ups (p<0.001). A negative and significant correlation was found at 10th, 30th, 60th minutes and 2nd hour after drug administration from epidural catheter(rho:0.38; p:0.03, rho:0.47; p:0.009, rho:0.75; p<0.001, rho:0.46; p:0.009, respectively). As the pain decreased, the perfusion index increased. In 17 patients requiring additional doses, PI increased after the all medications, but a decrease was observed in the VAS values(p<0.05). Conclusions: In this study, it was determined that the pain decreased with epidural analgesia, perfusion index increased and the pain level increased significantly when the perfusion index started to decrease.

Idiopathic granulomatous gastritis: An entity clinically mimicking tumor

    Tayfur, Mahir; Balci, Mecdi G.

Idiopathic granulomatous gastritis is a rare disease. Its diagnosis must be made after excluding other causes of granulomatous disease like Crohn's disease, tuberculosis, sarcoidosis, mycosis, syphilis, foreign body reaction and other reasons. Although it is associated with Helicobacter pylori, it might see rarely without Helicobacter pylori. It sometimes might be confused with malignancy clinically. The definitive diagnosis is made only by histopathological examination. In our case, a 49-year-old Caucasian woman admitted to hospital with epigastric pain complaint. In esophagastroduodenoscopy, there was antral hyperemia and a 13-mm hypoechoic mass lesion in submucosal layer. A biopsy was taken with prediagnosis of malignancy. Histopathologic examination determined a non-caseating gastric granulomatous inflammation. There was no histopathological finding of malignancy and any specific granulomatous diseases. This case was reported as idiopathic granulomatous gastritis.

An ischaemia/reperfusion (I/R) injury in an organ can also cause damage to other organs. In this study, the effects of nizatidine were investigated on ovarian I/R injury and subsequent heart damage in rats. Rats were divided into groups; ovarian I/R (IRG), nizatidine 25 mg/kg+ ovarian I/R (NIR), and control (CG) groups. The NIR and IRG groups were given 2 hours of ischaemia followed by 2 hours of reperfusion by applying, and subsequently releasing, a vascular clip to the right ovarian artery. While oxidant levels in the IRG group were higher than NIR and CG groups, antioxidant levels were lower. Necrosis, congested blood vessels, haemorrhage, leukocyte infiltration and oedema were observed in the ovarian and heart tissues of the IRG group. These findings were reduced in the NIR group. Nizatidine suppressed oxidative stress in ovarian and heart tissues and may be useful in prevention of I/R–induced ovarian and cardiac injury.

Serum NT-pro CNP levels in epileptic seizure, psychogenic non-epileptic seizure, and healthy subjects

Ceylan, Mustafa; Yalcin, Ahmet; Bayraktutan, Omer Faruk; Laloglu, Esra

Purpose: Epileptic seizure is the result of uncontrollable neural excitation in the brain. The C-type natriuretic peptide is a member of natriuretic peptide hormone family and is synthesized by brain and blood vessels in CNS. NT-pro CNP is an amino-terminal fragment of C-type natriuretic peptide and is more stable compared to its predecessor. In this study, we aimed to evaluate the role of NT-pro CNP in psychogenic non-epileptic seizures, epileptic seizures, and normal subjects. Methods: Thirty-three patients with epilepsy and 43 patients with psychogenic non-epileptic seizures were enrolled in this study. The control group consisted of 28 healthy subjects. Post-ictal serum levels of NT-pro CNP were acquired from all participants. Statistically significant differences between patient groups and controls regarding serum levels of NT-pro CNP were sought. Results: NT-pro CNP levels were significantly lower in the epilepsy group than the psychogenic non-epileptic seizure group and control group with no significant difference between the psychogenic non-epileptic seizure and control group (p < 0.05). Conclusion: Post-ictal serum NT-pro CNP levels were lower in epileptic seizures compared to psychogenic non-epileptic seizures as well as healthy controls. We think that such a difference is associated with C-type natriuretic peptide-related neural mechanisms such as altered microcirculation, increased brain-blood barrier permeability, and synaptic stabilization.

Cystitis is defined as an inflammation of the bladder caused by a bacterial infection, and it can be dangerous and painful when it spreads through the internal organs. In this study, antioxidant effects of hydroxylfasudil (HF) at the enzymatic and molecular level on kidney and liver tissues in cystitis rat model, which is caused by inflammation of the rat bladder with a protamine sulphate (PS), was examined. Quantitative changes of reduced glutathione (GSH) and lipid peroxidation (LPO) levels, which are a marker for oxidative stress, were determined in rat kidney and liver tissues for each groups. And then molecular and biochemical impact of HF treatment on antioxidant enzymes including superoxide dismutase (SOD) and catalase (CAT) in cystitis model were studied. The results suggest that HF could be beneficial to the renal and hepatic antioxidant system. Thus, HF might be used as a novel therapeutics agent to eliminate interstitial cystitis.

Çakir, Tayfun; Çakir, Mürteza; Önder, Hilmiokay; Yolaş, Coşkun; Tanriverdi, Osman; Ömeroǧlu, Muhammed; Arslan, Yusuf Kemal

Background: Low back and leg pain due to lumbar discal hernia (LDH) is an important health issue. Current evidences support surgery in carefully selected patients who have failed conservative treatment and do not exhibit any psychosocial overlay. However, as known, sometimes it may be still very difficult to normalize the life qualities of patients for long times. Now different surgical methods for LDH are in use with new technological materials. One of them is lumbar disc prosthesis. In this study, the radiological and clinical effects of using lumbar disc prosthesis were evaluated with comparing patients underwent simple lumbar microdsicectomy. The purpose of this study is to reveal whether inserting the disc prosthesis into the intervertebral distance after lumbar microdiscectomy is beneficial or not both radiologically and clinically. Methods: A total of 114 patients were analyzed; the first cohort consisted of 57 patients who received a lumbar discectomy at a single level along with the implantation of a disc prosthesis and the second consisted of 57 patients only received a lumbar discectomy at a single level. These 2 groups were studied by comparing the disc space on the level of carried out operations with pain scales, foramen diameters of coming about related roots preoperatively and postoperatively at 3 years. Results: One of the significant results of the implementation of the disc prosthesis is fulfilment of a healthy disc height again after microdiscectomy due to LDH. We concluded that fulfilment of a healthy disc height with lumbar disc prosthesis was clinically beneficial for patients underwent microdiscectomy. Conclusions: Based on the results obtained in this study, it can be concluded that the implantation of a disc prosthesis in appropriate patients is more favorable regarding pain and spinal physiology when compared to simple microdiscectomy.

The Comparison of Angiographic Scoring Systems With the Predictors of Atherosclerosis

Kalkan, Kamuran; Hamur, Hikmet; Yildirim, Erkan; Ipek, Emrah; Ermis, Emrah; Ozturk, Mustafa; Karal, Huseyin; Korkmaz, Ali Fuat; Bayantemur, Murat; Demirelli, Selami

Syntax score (SS) and Gensini score (GS) are used to determine the complexity and severity of coronary artery disease. Although there are some studies indicating the individual relationship of these scoring systems with the carotid intima–media thickness (cIMT) and epicardial fat tissue (EFT), there was no previous study that compared the SS and GS in that respect. We aimed to assess the relationship of SS and GS with predictors of atherosclerosis. We enrolled 522 patients. There were positive correlations between GS and SS tertiles with cIMT and EFT. According to multivariate linear regression analysis for GS, EFT (β: 0.035, t: 2.63, and P =.49) and cIMT (β: 0.339, t: 2.97, and P =.053) were not independently associated. For SS, EFT (β: 0.009, t: 6.5, and P =.006) and cIMT (β: 1.2, t: 10.1, and P =.001) were independently and significantly associated. We showed that the SS is significantly associated more with surrogate markers of atherosclerosis such as EFT and cIMT than the GS.

Evaluation of Dermatophagoides pteronyssinus (Trouessart) and D. farinae Hughes (Acari: Pyroglyphidae) sensitivity in patients with allergic rhinitis: A comparative study

Eytun, Erhan; Doǧan, Salih; Ünver, Edhem; Özçiçek, Fatih

This study was conducted to determine the sensitivity to Dermatophagoides pteronyssinus (Trouessart) and D. farinae Hughes with skin prick tests (SPT) and serologic tests in patients with allergic rhinitis (AR), and to specifically search for those mites in homes of patients. A total of 51 participants, (23 patients and 28 controls) were utilized. Skin-prick tests with D. pteronyssinus and D. farinae allergens were performed on all participants, and serum levels of allergen-specific lgE and total IgE were also measured. Dust samples were collected from homes of all participants and examined under a stereo microscope. 977 D. pteronyssinus (mean 44.4/g) and 24 D. farinae (mean 4.0/g) were isolated from the homes of patients, whereas 35 D. pteronyssinus (mean 4.4/g), and four D. farinae (mean 2.0/g) were isolated from the homes of the controls. Patients with D. pteronyssinus in their homes had 95.5% sensitivity to the species according to SPT and 27.3 according to IgE. Patients with D. farinae in their homes had 83.3% sensitivity to the species according to SPT, and 50% according to IgE. Dermatophagoides pteronyssinus sensitivity in the controls was detected as 12.5% according to SPT; however, D. farinae sensitivity was not detected according to both SPT and mite-specific IgE. Differences between patients and controls utilizing SPT results was statistically significant, but not when using mite-specific IgE results. As a result, it was determined that patients with AR in Erzincan province were sensitized to D. pteronyssinus and D. farinae, and that their homes contained those species. It may be helpful to consider these findings in clinical assessment of patients with AR, and also in treatment utilizing immunotherapy techniques.

Obesity-related metabolic and endocrine disorders diagnosed during postoperative follow-up of slipped capital femoral epiphysis

Ucpunar, Hanifi; Camurcu, Ismet Yalkin; Duman, Serda; Ucpunar, Esra; Sofu, Hakan; Bayhan, Avni Ilhan

Background and purpose — Patients with slipped capital femoral epiphysis (SCFE) are phenotypically overweight or obese and may therefore require clinical follow-up of obesity-related disorders. We evaluated obesity-related disorders such as dyslipidemia, type 2 diabetes mellitus (DM), and vitamin-D deficiency during the postoperative period in patients with SCFE. Patients and methods — 51 patients who were operated and followed-up for SCFE and 62 healthy adolescents without SCFE (control group) were included in this retrospective study. Patients’ BMI, serum lipid profile (total cholesterol, LDL-C, HDL-C, triglyceride), fasting blood glucose, HbA1c, and serum vitamin D levels were evaluated. Results — At the time of surgery, 45 patients in the SCFE group were overweight or obese (BMI >25). At the latest follow-up, 42 patients in the SCFE group and 53 patients in the control group were overweight/obese. Abnormal serum lipid profile and ratio of total dyslipidemia were similar between the groups. 8 patients had abnormal HbA1c levels in the SCFE group and mean HbA1c levels were significantly higher in the SCFE group (p = 0.03). All patients and controls had low levels of vitamin D. Interpretation — Although serum lipid profile and vitamin D levels were detected as similar in SCFE and control groups, the potential risk of type 2 DM identified via abnormal HbA1c levels was significantly higher in patients with SCFE. We recommend that patients diagnosed with SCFE should be considered as potential candidates for type 2 DM; thus follow-up after surgical treatment should include not only orthopedic outcomes but also evaluation of future risk for DM.

Coexistence of eccrine porocarcinoma, squamous cell carcinoma of the ear, and synchronous basal cell carcinoma of the face

    Balci, Mecdi G.; Tayfur, Mahir

Eccrine porocarcinoma is an unusual malignant cutaneous tumor arising from the intraepidermal portion of eccrine sweat glands. It is commonly found in elderly persons and frequently occurs on the lower extremities. It is an aggressive and potentially fatal form of sweat gland carcinoma, due to its propensity to metastasize through lymph vessels. Basal cell carcinoma and squamous cell carcinoma are the most common type of skin cancers. There was a combined tumor including eccrine porocarcinoma and squamouus cell carcinoma on left ear and synchronous basal cell carcinoma on face left side. We are presenting a very rare case that there was no in the literature. (Cite this article as: Balci MG, Tayfur M. Long-Term Outcomes of a New Minimally Invasive Approach in Chiari Type 1 and 1.5 Malformations: Technical Note and Preliminary Results

Kotil, Kadir; Ozdogan, Selcuk; Kayaci, Selim; Duzkalir, Hanife Gulden

Background: The treatment options for patients with Chiari malformation type 1 (CM1) and Chiari malformation type 1.5 (CM1.5) have not yet been standardized. In these malformations, the main factors include obstruction at the level of the foramen magnum and dural and ligamentous thickening. Here we present our outcomes of surgery and decompression using a minimally invasive surgery (MIS) technique. Methods: Sixty-one patients admitted to our clinics between 2009 and 2016 due to CM1 or CM1.5 and who had undergone MIS were investigated retrospectively. All patients were followed up for a mean period of 55 months, both clinically and radiologically, and the outcomes were recorded. Results: All 61 patients underwent foramen magnum decompression through a 1.5-cm mini-open incision, C1 laminectomy and C2 medial inner side tour, posterior atlanto-occipital membrane removal, external dural delamination, and widening of the internal dura with longitudinal incisions. Fifty-six patients (91.8%) were satisfied with the outcome, 4 patients (6.5%) remained the same, and 1 patient (1.6%) reported a poor outcome. Forty-five percent of the patients with syringomyelia demonstrated resolution within 2 years, and 92% demonstrated resolution in 5 years. Scoliosis was seen in 5 patients (8.1%). The rate of benefit from the surgical procedure was statistically significant (P = 0.0045), and no patient required additional surgery because of poor decompression. Conclusions: MIS is effective for uncomplicated cases of CM1 and CM1.5 due to its minimal connective and muscular tissue damage, short surgical duration, short recovery time, early mobilization, effective posterior foramen magnum widening, lack of liquor fistula development, and better clinical and radiologic improvement during long-term follow-up.

Evaluation of epidemiological, clinical, and laboratory characteristics and mortality rate of patients with Crimean-Congo hemorrhagic fever in the northeast region of Turkey

Karakecili, Faruk; Cikman, Aytekin; Aydin, Merve; Binay, Umut Devrim; Kesik, Ozan Arif; Ozcicek, Fatih

Background & objectives : Crimean-Congo hemorrhagic fever (CCHF), an illness characterized by fever and hemorrhage, is caused by a CCHF virus (CCHFV). It is an important public health problem in Turkey. The objective of this study was to evaluate the demographic, clinical, and laboratory characteristics and mortality rates of CCHF patients in the northeast region of Turkey. Methods : A total of 206 patients, diagnosed with CCHF, from northeast region of Turkey were included and evaluated between 2011 and 2017. Real-Time reverse transcriptase polymerase chain reaction (RT-PCR) and immunofluorescence (IFA) methods were used for the diagnoses. Results : Of the patients included in the study, 77.2% were farmers/livestockers, while 22.8% had other occupations. The incidence of tick bite or tick contact with bare hands was 52.9%. About 94.2% of the patients were living in rural areas and 5.8% in city centers. However, all the patients living in city centers had a history of visit to rural areas. The disease was more common in May, June, and July months. The most common symptoms at the time of admission included fatigue, fever, and widespread body pain, while laboratory findings were thrombocytopenia, leukopenia, and anemia. Bleeding, tachycardia, and rash were the most common findings on physical examination. Of all the patients, 95.6% were identified by RT-PCR and 4.4% by IFA methods. Severe cases constituted 22.3% (46) of the included patients. Throughout the course of this study, 7 (3.4%) patients died, and the remaining 96.6% (199) patients were discharged with a full recovery. Disease severity was significantly correlated with mortality rate and duration of hospitalization (p <0.001 and p = 0.013). Interpretation & conclusion : In this study, the mortality rate observed was lower than that reported in the literature because of accessibility of early supportive therapy. It would be beneficial in CCHF treatment to recognize the disease at an early stage, begin supportive treatment quickly, and educate the people living in high-risk areas as well as health care personnel working in these areas.
