- MakaleAçık Erişim2025Comparison of Neutrophil-to-Albumin Ratio and Neutrophil-to-Lymphocyte Ratio in Acute Phase Positive and Negative Ankylosing Spondylitis Patients
Karatas, Beliz
Aim: Acute phase reactants may not always be positive in ankylosing spondylitis (AS). We herein aimed to investigate whether the neutrophil-to-albumin ratio (NAR) is useful in monitoring AS activity. Methods: One hundred nineteen patients who were followed up in the internal medicine rheumatology clinic between August 2024 and September 2024 and diagnosed with AS according to the axial spondyloarthropathy classification criteria of the Association for the Assessment of SpondyloArthritis international Society and the 1984 modified New York criteria were included in our study. The C-reactive protein (CRP), neutrophil-to-lymphocyte ratio (NLR), NAR and Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) status of the patients were recorded at two visits, before and 6 months following treatment. Results: A significant correlation was found between the initial NLR and the NLR values after 6 months, and between the initial and 6-month BASDAI values in both the CRP positive and CRP negative groups (p=0.000 and p=0.017). It was observed that the changes in individual NAR and NLR values and BASDAI values for each case were parallel (p=0.012). Conclusion: C-reactive protein values in AS are not always consistent with disease activity, but the NLR may be helpful in such cases. As for the NA ratio, we found that each patient should be evaluated individually.

The purpose of this trial was to assess the effects of methylphenidate on the kidney tissues and to investigate the protective effect of adenosine triphosphate (ATP) against possible methylphenidate nephrotoxicity in rats. The rats were separated into; healthy control (HG), methylphenidate (MPHG), ATP (ATPG), and ATP+ methylphenidate (AMPG). The ATPG and AMPG groups were administered ATP 4 mg/kg bw/d, and the HG and MPHG groups received distilled water intraperitoneally. One hour from, ATP and distilled water administration, methylphenidate 10 mg/kg bw/d was applied via oral gavage to the AMPG and MPHG groups once daily for 30 d (1 × 1). Animals were euthanized after 30 d and tissues were collected. The levels of certain oxidant/antioxidant parameters, pro-inflammatory cytokines, and Blood urea nitrogen (BUN) and creatinine levels were measured. Kidneys were also examined histopathologically. ATP inhibited the increase in oxidant and decrease antioxidant levels induced by methylphenidate. The amounts of pro-inflammatory cytokines were increased in methylphenidate-treated kidney tissue compared with the HG and AMPG groups. However, ATP increased oxidative damage markers and cytokines levels close to the healthy group. Serum BUN and creatinine levels increased with methylphenidate but ATP prevented BUN and creatinine from rising in the ATPG and MPHG groups. ATP also reduced the histopathological damage increased by methylphenidate. The potential efficacy of ATP in treating kidney damage induced by methylphenidate use.
- MakaleAçık Erişim2025The Effect Of Post-Divorce Adjustment Program On Life Satisfaction, Adjustment To Divorce And Psychological Symptoms Of Divorced Individuals
Çelık, Oğuzhan; Nazli, Serap
This study examined the effectiveness of the Post-Divorce Adjustment Program (PDAP) on individuals' adaptation to divorce, life satisfaction, levels of depression, anxiety, and stress. The research employed a pre-test, post-test, follow-up, and control group quasi-experimental design. The research group consisted of 20 divorced individuals aged between 25 and 45. These individuals were divided into two groups: 10 in the experimental group and 10 in the control group. In the experimental group, participants received the PDAP individually, consisting of six sessions held once a week. This study utilized a mixed research approach employing a parallel embedded design, where qualitative data complement quantitative data. Therefore, in addition to quantitative data collection tools, semi-structured interview forms developed by the researchers were used to collect qualitative data. To determine the effectiveness of the PDAP in the experimental group, statistical analyses including the Mann-Whitney U Test and Wilcoxon Signed-Rank Test were conducted. Furthermore, a clinical significance assessment was carried out to determine individual changes within the experimental group throughout the PDAP application. Qualitative data were summarized and interpreted using various analytical techniques, including structural, descriptive, and longitudinal analysis methods. Findings indicated that the PDAP significantly contributed to improving divorced individuals' adaptation to divorce and their overall life satisfaction, while concurrently reducing levels of depression, anxiety, and stress. Additionally, the qualitative analysis of data derived from semi-structured interviews revealed that the PDAP facilitated positive transformations in participants' lives. It was found that the individuals in the experimental group took the initial steps to reorganize their lives and began applying the skills acquired from the program to their daily lives. The findings were discussed within the context of relevant literature, and recommendations were presented.

- MakaleYalnız künye2025Association between ACE (rs4343 and rs1799752), AGTR1 (rs5186), and PAI-1 (rs2227631) polymorphisms in the host and the severity of Covid-19 infection
Polat, Seher; Şimşek, Zühal Özer
Objective: It is necessary to identify appropriate clinical, biochemical, epidemiological and genetic biomarkers to elucidate the underlying mechanisms of the coronavirus disease-2019 (COVID-19) disease. The study focused on not only the link between disease severity (non-intense unit care (non-ICU) versus intensive unit care (ICU) and genetic susceptibility in COVID-19 patients but also the connection between comorbidity and genetic susceptibility affecting the severity of COVID-19. Subject and methods: One hundred and sixty-two COVID-19 patients treated in the non-ICU and ICU in Kayseri City Hospital were included. All volunteers underwent a physical examination and biochemical evaluation. Angiotensin-converting enzyme (ACE p.T776T G > A(rs4343) and g.16471_16472delinsALU (also referred to as I/D polymorphism; rs1799752), angiotensin II receptor type-1 (AGTR1) c.*86A > C (also referred to as A1166C; rs5186), and plasminogen activator inhibitor-1 (PAI-1-844 G > A (rs2227631) polymorphisms were analysed as well. Results: To have ACE “ID” genotype did not change the severity of the disease (OR: 0.92, 95% CI: 0.41–2.1, p = 0.84), but decreased the mortality risk 2.9-fold (OR: 2.9, 95% CI: 1.1–7.0, p = 0.03). In PAI-1-844 G > A, having the “AA” genotype in the “A” recessive model increased the risk of the diabetes mellitus (DM) 2.3-fold (OR: 2.3 95%, CI: 1.16–4.66, p = 0.018). In the “G” recessive model, to have the GG genotype increased the risk of chronic kidney disease (CKD) 4.8-fold (OR:4.8, 95% CI: 1.5–15.5, p = 0.008). “GG” genotype in the DM group had a higher fibrinogen level compared to those with the “AG” genotype (AG:4847.2 mg/L (1704.3) versus GG:6444.67 mg/L (1861.62) p = 0.019) and “AA” genotype in the CKD group had lower platelet levels and those with “GG” had higher platelet levels (AA:149 µL (18–159) versus GG: 228 µL (146–357) p = 0.022). Conclusion: This study was shown that genetic predispositions that causes comorbidities were also likely to affect the prognosis of COVID-19.

In this study, machine learning (ML) algorithms were employed to predict analyte concentrations using sensing results and evaluate the anticancer effects of nanostructures. Multifunctional oolong tea extract-mediated silver nanoparticles (OTE-Ag NPs) were synthesized via a photo/ultrasound method and utilized in various applications, including a smartphone-based H2O2 sensor and electrochemical sensors for urea and fructose. Key features were extracted from electrochemical results, and feature importance analysis was used to select the most predictive features. The artificial neural network (ANN) model provided accurate predictions, particularly strong for urea (R2 = 0.8575, RMSE = 0.4266, MAE = 0.3380). The study revealed the selective toxicity of OTE-Ag NPs to MCF-7 breast cancer cells through analyses of cytotoxicity, apoptosis, cell cycle phases, and CD44 surface marker expression using Annexin V/PI dye and flow cytometry. Experimental results demonstrated that OTE-Ag NPs suppressed MCF-7 cell proliferation while exhibiting lower cytotoxicity in normal HUVEC cells (46% cell death). OTE-Ag NPs arrested MCF-7 cells in the G2/M phase, induced apoptosis, and reduced CD44 expression, suggesting metastasis suppression. The CD44+/CD24- ratio decreased from 84.79% in control MCF-7 cells to 47.7% in OTE-Ag NP-treated cells. Overall, OTE-Ag NPs significantly inhibited MCF-7 cell proliferation through the apoptotic pathway by regulating the cell cycle in the G2/M phase.
- MakaleAçık Erişim2025EXAMINING PRESCRIPTION DRUG USE AND HEALTH CARE ACCESS AMONG OLDER ADULTS IN RURAL AREAS: A CROSS-SECTIONAL STUDY BASED ON THE HEALTH BELIEF MODEL

Introduction: Medication adherence is critical for achieving desired health outcomes, particularly in older rural patients with limited healthcare access. This study, structured according to the Health Belief Model explored the correlation between health beliefs regarding medication intake and adherence in older rural patients with chronic disease who used prescription medication. Materials and Method: A cross-sectional survey was conducted with 206 community-dwelling adults aged ≥65 years in rural villages of the Gemerek district (Sivas, Turkey). Health beliefs and adherence were assessed using validated HBM and ARMS-7 scales. Results: The structural model fitted very well overall (χ²/df = 1.65; RMSEA = 0.041; SRMR = 0.036; CFI = 0.982; TLI = 0.976). Among the predictors, perceived barriers had a significant negative impact on medication adherence. (β = –0.31; p < 0.01). In contrast, perceived benefits (β = 0.39; p < 0.01) and self-efficacy (β = 0.36; p < 0.01) were positively and significantly related with regard to adherence. The other dimensions were not significant predictors. Conclusion: These findings highlight the key role of psychosocial determinants in medication use among older adults in rural communities. To enhance medication use among older rural patients, education and counselling by primary care physicians and pharmacists should focus on reducing perceived barriers and increasing patients’ self-efficacy. Implementing such targeted strategies may improve continuity of care and help older adults manage their treatments more confidently. Ultimately, strengthening these supports could contribute to better health outcomes and a higher quality of life in rural aging populations.
- MakaleAçık Erişim2025A new HLA susceptibility haplotype defined in three familial cases of frontal fibrosing alopecia; [Üç ailevi frontal fibrozan alopesi vakasında tanımlanan yeni bir HLA duyarlılık haplotipi]

Frontal fibrosing alopecia (FFA) is a scarring alopecia that primarily affects postmenopausal women. Although its etiology remains unknown, familial cases suggest a genetic basis. In this case report, we present the findings of three sisters with familial FFA, aged 55, 60, and 62 years, from Türkiye. Genetic analysis revealed that all three sisters shared the human leukocyte antigen (HLA)-A*11:01; B*35:01; C*04:01 haplotypes. Two sisters had HLA-DRB1*03:01 and HLA-DQB1*02:01, while the third had HLA-DRB1*01:01; HLA-DQB1*05:01. The shared HLA-A*11:01, B*35:01, and C*04:01 haplotype has not been previously associated with familial FFA. This finding marks the first familial FFA report from Türkiye and suggests a new genetic susceptibility haplotype for FFA in the Turkish population. The variation in HLA-DRB1 and HLADQB1 alleles among the sisters indicates complex genetic influences on the familial FFA. Further research is required to determine the role of these genetic variations in disease progression and to identify potential therapeutic approaches.

Introductıon: Considerable overlaps exist between asthma phenotypes and the clinical significance of these overlaps remains undetermined. The objective of this study is to analyze the characteristics of asthma overlap phenotypes using data from the Turkish Adult Asthma Registry (TAAR). Methods: This cross-sectional registry study included 2053 adult patients (74.8% female) with asthma. Results: Overall, 39.3% (n = 697) had allergic–eosinophilic (AE), 26.0% (n = 461) had allergic–non-eosinophilic (ANE), 21.3% (n = 377) had non-allergic–eosinophilic (NAE), and 13.4% (n = 237) had non-allergic–non-eosinophilic (NANE) asthma. Severe asthma exacerbations and emergency department (ED) visits were more frequent in the AE (28.3%, 31.2%, respectively) and NAE groups (36.0%, 34.0%, respectively) than in the ANE (14.3%, 20.6%, respectively) and NANE groups (12.6%, 16.7%, respectively) (p < 0.001). FEV1 values were significantly lower in the AE group than in the ANE and NANE groups (p < 0.001, p = 0.048, respectively) and in the NAE group than in the ANE group (p < 0.001). Risk factors for poor asthma control included living in rural areas, asthma-related ED visits, FEV1 < 60% in the NAE; being overweight, chronic rhinosinusitis, oral corticosteroids use, age < 40 years in the NANE; FEV1 < 80% in the AE; and severe asthma exacerbations, ED visits for AE and ANE groups. Conclusıon: The considerable overlap between allergic and eosinophilic asthma phenotypes has clinical implications as increased rates of asthma exacerbations and healthcare utilization. The clinical heterogeneity among asthma phenotypes based on a single biomarker highlights the importance of multidimensional asthma phenotyping.

Background: Recent studies indicate that the organo-axial subtype of a sigmoid volvulus is more prevalent than the conventional mesentero-axial subtype. Our study aimed to assess the clinical and radiological findings that differentiate between these two subtypes, as well as to ascertain treatment outcomes and prognostic characteristics. Methods: A retrospective review included 54 patients, during which abdominal plain radiographs and computed tomography images were analyzed by two radiologists, and data on recurrence, mortality, and treatment outcomes were documented. Results: The mesentero-axial subtype comprised 40 cases (74%). No distinct radiographic findings were observed to differentiate between the two groups. In computed tomography, the sole significant parameter for differentiation was the number of transition zones. The diameter of the segment exhibiting a volvulus was greater in instances of the mesentero-axial subtype. The endoscopic detorsion treatment proved ineffective in five patients within the mesentero-axial sigmoid volvulus cohort. Conclusions: Identifying these two types of SV on CT images is essential because of their distinct prognoses and therapeutic results.

Background: Asthma is one of the most common causes of chronic respiratory disease, and countries with low socioeconomic status have both a high prevalence of asthma and asthma-related death. Objective: In this study, we aimed to determine socioeconomic levels of asthmatic patients according to a national database and investigate the effects of social markers on disease control in our region. Methods: This is an analysis of data from 2053 adult asthma patients from a multicentre chart study in Turkey. Socioeconomic status (SES) data were collected from questionnaires and this form was sent to the patients via e-mail. Parameters related to social status and poor disease control were analyzed. Results: Illiteracy (OR:2.687 [95% CI: 1.235–5.848]; p = 0.013) and lower household income (OR:1,76 [95% CI: 1.002–3.09]; p = 0.049) were found as independent risk factors for hospitalization in the multivariate logistic regression analysis. Therewithal, being aged between 40 and 60 (OR: 1.435 [95% CI: 1.074–1.917]; p = 0.015), illiteracy (OR: 2.188 [95% CI: 1.262–3.795]; p = 0.005) and being employed (OR: 1.466 [95% CI: 1.085–1.847]; p = 0.011) were considered as independent risk factors for systemic corticosteroid use at least 3 days within last 1 year. Conclusion: As a result of our national database, education level, household income and working status briefly socioeconomic status have impacts on asthma control. Identification of social markers in asthma and better recognition of risk factors based on the population gives us clues to provide better asthma control in the future.

Various carbonic anhydrase (CA) enzyme isoforms are known today. In addition to the use of CA inhibitors as diuretics, antiepileptics and antiglaucoma agents, the inhibition of other specific isoforms of CA was reported to have clinical benefits in cancers. In this study, two groups of 1,3,4-thiadiazole derivatives were designed and synthesized to act as human CA I and II (hCA I and hCA II) inhibitors. The activities of these compounds were tested in vitro and evaluated in silico studies. The activity of the synthesized compounds was also tested against acetylcholinesterase (AChE) to evaluate the relation of the newly designed structures to the activity against AChE. The synthesized compounds were analyzed by 1H NMR,13C NMR and high-resolution mass spectroscopy (HRMS). The results displayed a better activity of all the synthesized compounds against hCA I than that of the commonly used standard drug, Acetazolamide (AAZ). The compounds also showed better activity against hCA II, except for compounds 5b and 6b. Only compounds 6a and 6c showed superior activity against AChE compared to the standard agent, tacrine (THA). In silico studies, including absorption, distribution, metabolism and excretion (ADME) and drug-likeness evaluation, molecular docking, molecular dynamic simulations (MDSs) and density functional theory (DFT) calculations, were compatible with the in vitro results and presented details regarding the structure–activity relationship.
