Association Between Acromiohumeral Interval and Isolated Supraspinatus Muscle Tears Using Magnetic Resonance Imaging

Kızılgöz, Volkan; Çetin, Türkhun; Kahraman, Şevket

Objective: The authors aimed to investigate the acromiohumeral interval (AHI) using Ellman’s classification in patients with isolated supraspinatus tendon tears. The AHI differences between patients and controls, in addition to controls, partial tears, or total supraspinatus ruptures, were compared concerning AHI differences. Methods: The imaging was performed using a 1.5 Tesla (T) magnetic resonance imaging (MRI) device and a shoulder coil. The AHI measurements were determined by taking the average of measurements in 3 different planes. For Ellman’s classification, the grading of the tear was graded based on how much the depth of the tear or the thickness of the tendon was affected. Results: A total of 304 shoulder MRIs meeting the criteria were included in our study. The study included 41.4% (126/304) without rotator cuff pathology, 30.9% (94/304) with grade 1 tear in the isolated supraspinatus tendon, 6.6% (20/304) individuals with grade 2 tear, 11.8% (36/304) with isolated supraspinatus tendon grade 3 tear, and 9.2% (28/304) with grade 4 tear in the isolated supraspinatus tendon. The AHI values in men were found to be statistically significantly higher than in women. The patient group and the control group showed a significant difference regarding the AHI values. The AHI measurements were significantly lower for the patients with total rupture. Conclusion: The AHI values in isolated supraspinatus tendon injuries, including all tear groups, were found to be significantly lower than those in the patients without rupture; however, there was also a significant difference between total and partial-tear groups.

Changes Caused by Smoking in Hemogram Parameters in Patients Presenting to the Emergency Department

    Salcan, Sara; Işık, Bahar

Objective: The aim of this 3-stage study is to reveal the smoking rate after the exclusion criteria was applied in patients who presented to the emergency department and to assess if smoking has any effect on their hemogram test results. And at last, the opinions of those included in the study are sought regarding whether establishing a smoking cessation center in the emergency department would be beneficial or not. Methods: After exclusion criteria were applied, a total of 2000 patients presenting to the emergency department were included in the study. The study was carried out in 3 stages. In the first stage, the smoking rate of 2000 patients was investigated. In the second stage, in addition to the exclusion criteria at the beginning of the study, in order to reduce the possible impact on blood parameters, those with any comorbidities (n = 747) were excluded, and hemogram parameters were compared in the remaining patients (n = 1263). Then hemogram levels were compared both in male and female in smokers and non-smokers since the normality limit of some of these values differs for both genders. In the third stage, establishing a smoking cessation center in the emergency department would be beneficial or not was statistically researched. Results: In total, 50% of the patients were male and 50% were female. The mean age was 51.57 ± 20.29 years. Of the patients, 28.26% (n = 566) were smokers. In total, 38.7% (n = 385) of men and 17.1% (n = 178) of women were smokers. The smoking rate was 30.7% in those without comorbidities, which was higher. The rate we obtained, although not directly comparable to the results of other studies due to differences in inclusion and exclusion criteria, was similar to the rate in the general population. There were significant differences in white blood cell (WBC), erythrocyte (RBC), and hemoglobin counts, as well as mean corpuscular volume (MCV) between the 2 groups. No significant difference was found in platelet count. White blood cell count was 12% higher, erythrocyte count was 3.13% higher, MCV was 1.5% higher, and eosinophil count was 24.1% higher in smokers. Hemoglobin level was 7.8% higher in smokers. When men and women without comorbidities were analyzed separately as smokers and non-smokers, hemoglobin, WBC, and eosinophil counts were higher in male smokers; hemoglobin, WBC, MCV, and eosinophil counts were higher in female smokers. Mean corpuscular volume and RBC count in men, and RBC count in women were higher in the smoker group, although it was not statistically significant. Only 33.7% of patients stated that it would be beneficial to establish a smoking cessation unit in the emergency department. Conclusion: Cigarette smoking has severe adverse effects on hematological parameters that might be associated with a greater risk of developing chronic diseases. The rate of smoking among the patients in the emergency department is similar to the general population. Making the necessary arrangements for emergency departments to become primary centers for smoking cessation and including this topic as part of residency training could be beneficial.

Appearance of Dehiscence at the Uterine Scar Site on Ultrasound: A Case Report

    Yılmaz, Betül Kalkan

With the increase in cesarean section rates, uterine rupture, one of the obstetric emergencies, has become an important clinical condition leading to maternal–fetal mortality and morbidity. According to risk factors for uterine rupture, it can be classified as rupture of a previous scar (cesarean section, myomectomy), traumatic rupture of the uterus (resulting from trauma), spontaneous rupture of the uterus due to underlying pathologies (uterine anomalies, multiparity), and spontaneous uterine rupture in apparently normal primigravida patients. Among these, uterine rupture due to a scarred uterus is the most commonly seen. Currently, optimal cutoff values for predicting uterine scar rupture have not been definitively established. A 35-year-old pregnant woman, gravida 3, parity 2, with a history of 2 previous cesarean sections, was seen during a routine outpatient clinic visit when she was 23 weeks and 2 days pregnant based on her last menstrual period. During the visit, approximately a 5 cm area of amniotic fluid protruding toward the bladder was observed along the old incision line, suggesting a disruption in the continuity of the myometrium at the scar site. Preoperative preparations were made, and the patient was taken for a cesarean section. Interestingly, there was no apparent evidence of dehiscence, which is commonly seen in many patients who have had previous cesarean sections. The cesarean section was successfully completed without complications.

Tilapia piscidin 4 (TP4), a cationic antimicrobial peptide, is recognized for its diverse biological roles, including antibacterial, wound-healing, and anticancer properties. Herein, the codon-optimized sequence of TP4 peptide was expressed using the pPICZαA expression vector containing the AOX1 promoter, a strong and inducible promoter, in the Pichia pastoris KM71H expression system. Recombinant TP4 peptide was purified by Ni-NTA affinity chromatography. After purification, the anticancer activity of TP4 was assessed in HUH-7 hepatocellular carcinoma cells, and the underlying mechanisms were determined. In the present study, it was demonstrated for the first time that recombinant TP4 displayed strong anticancer activity in the human HUH-7 cell line. The TP4 antimicrobial peptide can be used as a competitive candidate for the treatment of cancer cells due to its anticancer effects.

Objective: The aim of this study was to examine the normal values of the optic nerve in different segments of the nerve trace in the pediatric population using magnetic resonance imaging (MRI). Methods: Two radiologists retrospectively reviewed images obtained with a 1.5 tesla MR device of 426 youngsters (213 males/213 girls). Measurements were performed on the T2 sequence. Parts of the orbital region, prechiasmatic region, and optic tract were measured. Participants were divided into 5 different age groups. Results: The mean age of the population is 8.3 ± 1.5 years. The average diameter of the orbital segment of the entire population was 3.58 ± 0.53 mm, 4.74 ± 0.48 mm for the prechiasmatic segment, and 3.61 ± 0.48 mm for the optic tract section. Age correlates positively with the orbital plane (OP), prechiasmatic portion (PP), and optic tract (OT) values. All age groups have considerably varying mean OP, PP, and OP diameters. Good interobserver reliability was observed for all 3 diameter measurements (OP cap value = 0.62, PP cap value = 0.58, OT cap value = 0.67). Conclusion: Normal diameters of the optic nerve can be measured by MRI at above mentioned sections with a good interobserver reliability. These normal values can be used to detect obscure thickness changes/abnormalities in optic nerves of the children.

Objective: Carpal tunnel syndrome is a condition caused by the compression of the median nerve within the carpal tunnel, resulting in clinical symptoms in the area innervated by the nerve. The role of electromyography in surgical planning for carpal tunnel syndrome is controversial. The current study aimed to compare the clinical results of patients who underwent carpal tunnel release surgery based on their preoperative electromyography grading. Methods: We examined 102 patients who underwent carpal tunnel release surgery between 2015 and 2018. All patients were evaluated by electromyography preoperatively and 2-year postoperatively and completed the Boston Questionnaire. Based on the Bland classification for electromyography grades, the patients were divided into 2 groups: group 1 (grades 1-3) and group 2 (grades 4-6). Changes in scores were analyzed after carpal tunnel release surgery for each group. Results: When considering the results for all patients irrespective of group, both scores showed significant improvement after surgery. Group 2 exhibited greater improvement in the functional status scale score and electromyography grade compared to group 1, while similar changes were observed in the symptom severity scale scores. Conclusion: Both the symptom severity scale and functional status scale scores improved significantly after surgery for all patients. However, group 1 consistently had lower scores than group 2 across all evaluation periods. The evaluation of the improvement in the electromyography grade (ΔEMG) indicated that group 2 had better improvement than group 1. Level of Evidence: III

Pneumomediastinum Mistaken as Angioedema: Case Report

    Kantar, Yusuf

Pneumomediastinum and massive subcutaneous emphysema are clinical conditions that can occur during head and neck surgeries, bronchoscopy and tracheostomy, after thoracic trauma, rarely during dental procedures and sometimes spontaneously. Patients often present with symptoms such as swelling of the face, neck and sometimes the whole body, difficulty swallowing, shortness of breath, chest pain and skin crepitus. These findings usually develop suddenly and sometimes may occur within hours or days. A 48-year-old man is applies to the hospital with complaint chest pain due to a fall in the garden. If no pathology is thought to be present as a result of the X-ray, the patient is prescribed ibuprofen for pain relief and discharged. Patient who presents with what appears to be angioedema after taking ibuprofen is found to have pneumomediastinum. In the cases mentioned in the report and initially thought to be angioedema, seeing that the lips are not swollen and feeling subcutaneous crepitation, i.e. even just inspecting and palpating the patient, will distract the clinician from angioedema in a few seconds and direct him/her to the correct diagnosis and perform examinations in the direction of pneumomediastinum.

Psychometric Properties of the Turkish Version of the Belief About Losing Control Inventory: Testing the Measurement Invariance Across Gender

    Eskisu, Mustafa; Doğan, Uğur

Recent studies suggest that beliefs regarding losing control may have a pivotal role in the formulation of anxiety-related problems. The Beliefs About Losing Control Inventory (BALCI) is a self-report measure that is employed to assess thought control failure. The BALCI comprises 21 items and three subscales. This study aimed to investigate the psychometric properties and measurement invariance across gender groups of the Turkish version of the BALCI. The BALCI, Obsessive Belief Questionnaire-44 (OBQ-44), Vancouver Obsessional Compulsive Index (VOCI), Anxiety Sensitivity Index-3 (ASI-3), and State-Trait Anxiety Inventory–Trait Version (STAI-T) were administered to 531 volunteer undergraduates. Confirmatory factor analysis replicated the original 21-item three-factor structure of the BALCI. Internal consistency of the BALCI total and subscales were adequate. Retest reliability for the total scores of the BALCI was .76, for the subscales varying from 0.72 to 0.78. The BALCI scores significantly correlated with OBQ-44, ASI-3, and STAI-T. The BALCI and subscales also explained a significant amount of variance in OCD symptoms above and beyond obsessive beliefs. This study also provided evidence of the measurement invariance of the BALCI across gender groups. In conclusion, the Turkish version of the BALCI is a valid and reliable instrument to assess negative beliefs regarding losing control.
