- MakaleAçık Erişim2024The Impact of Complex Loadings on the Structure of the L2-L3 Intervertebral Disc in a Sheep Spine Cadaver Model: A Biomechanical and Histological Evaluation
Öztürk, Akin; Karakasli, Ahmet; Erbil, Gueven; Gökgöz, Mehmet Burak; Yapici, Furkan
Background The human vertebral column generates movements under versatile, dynamic loads. Understanding how the spine reacts to these movements and loads is crucial for developing new spine implants and surgical treatments for intervertebral disc injuries. Mechanically uni-axial compression models have been extensively studied. However, the spine's daily loading is not limited to compression, so it is crucial to measure its behavior in all movements (flexion-extension, rotation, and axial compression). Methods This study utilized L1-L5 segments from 19 healthy adult sheep spines. The L2-L3 disc of the first spine underwent only histological evaluation without biomechanical testing to define basic histological parameters. The remaining 18 were divided into three groups of six and subjected to biomechanical tests. Different mechanisms for three groups of spinal segments were prepared, and tests were performed on Shimadzu AG-IS 10 KN (Universal Drawing Press, Kyoto, Japan). An axial load (800 N) was applied to the first group, an axial load with 15 degrees of flexion to the second group, and an axial load with 10 degrees of rotation plus 15 degrees of flexion to the third group. A biomechanical evaluation of the maximum elongation amounts (MEAs) was performed and compared between the groups. Then, the L2-L3 discs were removed from the sheep spines, and a histological examination of the discs was conducted using Hematoxylin-Eosin (HE), Alcian Blue (AB), and Masson's Trichrome (MT) staining. Results The mean MEA +/- Standard Deviation (Range) was 1.39 +/- 0.38 (0.91-1.94) for Group 1, 2.02 +/- 0.75 (0.91-3.01) for Group 2, and 2.47 +/- 1.09 (0.64-3.9) for Group 3. Biomechanically, although MEAs increased from Group 1 to Group 3 (meaning that the mean MEAs increased as the number of types of applied force increased), there was no statistically significant difference between the groups regarding the MEAs (P = 0.092). Histologically, no significant differences were observed between all groups after HE staining. In all groups, hypercellularity, edema in the connective tissue, separation between tissue layers, delamination, and signs of swelling and necrosis in the cells were observed similarly. For the AB staining, there was a decrease in the glycosaminoglycan (GAG) structure in the tissue samples compared to the control tissue, but no significant differences were observed between the groups. However, it was observed that the stratification in Group 3 was slightly more deteriorated than in the other groups. For the MT staining, collagen structure deterioration was observed in all groups. It was observed that the amount of collagen was significantly reduced compared to the control tissue. Conclusion As a result, when the axial load is applied biomechanically, there is more displacement of the vertebral discs in Group 3 with multidimensional movements. Furthermore, histological studies revealed deterioration between tissue layers when exposed to complex movements, and the degradation of stratification in group 3 compared to other loading combinations in groups 2 and 3 may indicate the role of complex loads in the formation of disc herniation.

Objective: It is known that inflammation plays a role in the etiopathogenesis of schizophrenia. In this study, we examined high mobility group box 1 protein (HMGB1) and Beclin 1 levels and their relationship with clinical variables in patients with schizophrenia. Method: Forty-three patients with schizophrenia and 43 healthy controls were included in this study. The patients were administered sociodemographic data form, the Positive Negative Symptoms Assessment Scale (PANSS) and the Clinical Global Impressions (CGI) scale. After the scales were filled, venous blood samples were taken from both the patient and control groups to measure serum HMGB1 and Beclin 1 levels. Serum samples obtained at the end of centrifugation were measured by Enzyme -Linked ImmunoSorbent Assay (ELISA) method. Results: The mean serum HMGB1 levels were significantly increased and the mean serum Beclin 1 levels were significantly decreased in the schizophrenia group compared to the control group. In addition, a negative correlation was found between HMGB1 and Beclin 1 levels. Conclusion: In conclusion, current research shows that HMGB1 is increased and Beclin 1 is decreased in patients with schizophrenia, and these findings may contribute to the role of autophagy in the pathogenesis of schizophrenia.

Aim: This study aimed to tension-type headache (TTH) investigate the efficacy of greater occipital nerve (GON) blockade performed with dexketoprofen under ultrasonography in patients presenting to the emergency department. Materials and Methods: This prospective, randomized, controlled study was conducted with patients who presented to the emergency department with TTH. The treatment was planned as 50 mg of intravenous (IV) dexketoprofen over 5 min for Group 1 and IV dexketoprofen followed by ultrasonography-guided GON blockade with 0.5% bupivacaine for Group 2. The patients' demographic characteristics and pain levels according to the 10-cm Visual Analog Scale were recorded at the time of presentation. The pain scores of the patients were recorded at the 10th, 20th, 30th, 60th, and 120th minutes, and the difference between the 0th and 120th minutes was calculated as the delta value. Results: Of the 159 patients included in the study. There was a decrease in the pain scores at the 10th minute in both groups with treatment, and the greatest decrease occurred in Group 2 at the 20th minute. The delta visual analog scale score was found to be 4.71 in Group 1 and 7.11 in Group 2, and it was observed that GON blockade therapy together with IV dexketoprofen reduced the severity of pain more rapidly and effectively than IV dexketoprofen alone. Conclusion: When managing acute pain attacks in patients presenting to the emergency department with TTH, the combined use of IV nonsteroidal anti-inflammatory drugs with a GON block increases treatment outcomes, reduces treatment duration, and enhances the efficacy of analgesics compared with their use alone.

Background and Objective: Ischemia-reperfusion (IR) causes tissue damage by inducing oxidative stress and inflammation. In this study, the protective ability of leflunomide, which is reputed to have antioxidant and anti-inflammatory effects, against IR-induced ovarian damage in rats was researched. Materials and Methods: Twenty-four albino Wistar female rats were divided into 4 groups of six each: Healthy (HG), sham-operated (SOG),ovarian IR (OIR) and leflunomide+ovarian IR (LOIR).One hour beforethe IR procedure, the LOIRgroup received 10 mg/kg leflunomide orally. Ovaries of rats in OIR and LOIR groups were subjected to 3 hrs ischaemia and 6 hrs reperfusion. For six days, leflunomide were implemented once a day. On day seven, the animals were euthanized and their ovaries were collected. The tissues were examined biochemically and histopathologically. Results: Leflunomide treatment significantly suppressed IR-induced rise in malondialdehyde (MDA),Tumor Necrosis Factor-Alpha (TNF-) and Interleukin 6 (IL-6) and the decline in Total Glutathione (tGSH), superoxide dismutase (SOD) and catalase (CAT) (p<0.001). Severe stromal hemorrhage, edema and follicular hemorrhage were noted in the ovaries of the IR group. Leflunomide treatment significantly prevented IR-induced ovarian tissue damage (p<0.05). Conclusion: Leflunomide could protect the ovaries with antioxidant and anti-inflammatory activity in ovarian IR injury.

Background and Objective: Polymorphonuclear leukocytes (PMNLs) play an important role in the pathogenesis of oxidative ischemia-reperfusion (I/R) damage. There is information that sugammadex inhibits PMNL infiltration into organs and tissues undergoing I/R. Present study aimed to investigate the potential protective effect of sugammadex against I/R-induced ovarian damage in rats and to elucidate its mechanism of action. Materials and Methods: The 24 female albino Wistar-type rats were divided into four groups as follows: Sham operation (SG), ovarian I/R (IRG), sugammadex (4 mg/kg) +ovarian I/R (SIR-4) and sugammadex (8 mg/kg) +ovarian I/R (SIR-8). Sugammadex was administered intraperitoneally. The animals were subjected to 2 hrs of ischemia followed by 2 hrs of reperfusion. Results: Sugammadex significantly inhibited the increase in I/R-induced malondialdehyde and the decrease in total glutathione, superoxide dismutase and catalase levels in ovarian tissue at a dose of 8 mg/kg compared with the dose of 4 mg/kg. Moderate follicular degeneration, vascular congestion, necrosis, oedema and hemorrhage were observed in the sugammadex (4 mg/kg) group, which significantly reduced the increase in I/R-related PMNL infiltration in ovarian tissue. Only mild vascular congestion was observed in the ovarian tissue of the 8 mg/kg sugammadex group, which did not show PMNL infiltration. Conclusion: Histopathological findings show that sugammadex protects ovarian tissue from oxidative and inflammatory injury of the I/R by inhibiting PMNL infiltration. Results also suggest that an 8 mg/kg dose of sugammadex, which completely inhibits PMNL infiltration into ovarian tissue, may be more beneficial than the 4 mg/kg dose in the treatment of I/R-induced ovarian damage.

Background and Aim:Tongue base reduction surgery is the only minimally invasive technique that can be performed under local anesthesia as an outpatient procedure, especially to treat obstructive sleep apnea caused by hypopharyngeal obstruction. Studies reported that various devices could be used for tongue base reduction, but comparisons of these methods are limited in the literature. Our study aimed to compare the histological effects of tongue base reduction methods performed with the Celon radiofrequency, monopolar cautery, Coblator, and Sutter devices on the tissue. Methods:This study included 23 female rats (aged 4-8 months). Rats were divided into five groups. Tongue base reduction was performed with monopolar cautery in the first group (5 rats), then Celon radiofrequency in the second group (5 rats), the Coblator in the third group (5 rats), and the Sutter in the fourth group (5 rats). The fifth group was the control group, which comprised three rats. The rats' tongues were resected for histological examination 1 week after procedures. Results:While the highest amount of fibrosis was seen in the Sutter group, fibrosis was also relatively high in the Coblator group. A significant difference was observed in all groups when compared with the control group regarding tissue thickness. While the average tissue thicknesses of the monopolar, celon rf, and Coblator groups were almost the same, tissue reduction was more marked in the Sutter group. Conclusion:Although we observed that all the methods used were safe, the method that produced the most fibrosis and tissue reduction was the Sutter method.
- MakaleAçık Erişim2024Use of the vertebrae and iliac bone as references for localizing the appendix vermiformis in computed tomography
Ozturk, Muhsin Ozgun; Resorlu, Mustafa; Aydin, Sonay; Memis, Kemal Bugra
BACKGROUND The appendix vermiformis is a part of the gastrointestinal tract, situated in the lower right quadrant of the abdomen. Acute appendicitis, acute inflammation of the appendix vermiformis, is the most common cause of acute abdomen requiring surgical intervention. Although computed tomography (CT) offers high diagnostic efficacy in assessing the appendix across various anatomical positions, it also involves radiation exposure. Reducing exposure factors and narrowing the field of view (FOV) are ways to decrease the radiation dose to the patient. To narrow the FOV, appendix locations within the population must be defined using metric markers. AIM To determine the location of the appendix vermiformis on CT using the vertebrae and the right iliac bone as anatomical landmarks. METHODS This retrospective study examined 470 patients presenting with abdominal pain who underwent abdominal CT scans between January 01, 2015 and January 01, 2018. Forty-three patients were excluded due to various reasons. The most superior and inferior points and the origin of the appendix were measured separately in relation to the vertebrae and right iliac bone for localization. The population was divided into normal and acute appendicitis groups, and the relationship between appendix location and anthropometric parameters relationship was examined. P values below 0.05 were considered statistically significant. RESULTS The final analysis included 427 adult patients (206 females and 221 males) with a mean age of 42.1 +/- 19.5 years. An ascending appendix course was the most common (90.4%). The appendix ranged from the L2 vertebral body level to the coccygeal vertebral level relative to the vertebrae. The appendix ranged between (-) 140.5 mm and (+) 87.4 mm relative to the right iliac bone. A negative correlation was found between patient age, height, body mass index, and the highest and lowest points of the appendix in regard to the vertebrae. CONCLUSION The study's findings unveiled the locations of the appendix in the population in relation to the bony anatomical landmarks. These data can be used as the basis for future research aimed at reducing patient exposure to ionizing radiation.

Alveolar echinococcosis (AE) primarily manifests in the liver and exhibits characteristics resembling those of slow-growing malignant tumours. Untreated Echinococcus multilocularis infection can be lethal. By infiltrating the vascular systems, biliary tracts, and the hilum of the liver, it might lead to various problems. Due to its ability to infiltrate neighbouring tissues or metastasize to distant organs, AE can often be mistaken for malignancies. We present a concise overview of the epidemiological and pathophysiological characteristics of AE, as well as the clinical manifestations of the disease. This article primarily examines the imaging characteristics of AE using various imaging techniques such as ultrasonography, computed tomography (CT), magnetic resonance imaging, diffusion-weighted imaging, and virtual non-enhanced dual-energy CT. We additionally examined the contribution of radiography in the diagnosis, treatment, and monitoring of the condition.

Background and Objective: Everolimus is an anticancer agent that inhibits the Mammalian Target of Rapamycin (mTOR). It causes side effect such as pneumonia. This study aimed to evaluate rutin's effect on potential pulmonary injury in everolimus-administered rats biochemically and histopathologically. Materials and Methods: All rats were split into: Healthy (HG), everolimus-treated (EVR) (2 mg/kg) and rutin (50 mg/kg)+everolimus-treated (REV). Rutin was administered to the REV group and physiological saline was given orally tothe EVR and HG groups. As 1 hr later, everolimus 2 mg/kg was administered to REV and EVR groups. The process was repeated daily, 4 times weekly. Then, high dose anaesthesia was used to kill the rats and lung tissue samples were extracted. Oxidative stress and pro-inflammatory cytokine markers were examined and ANOVA test was applied. Results: Oxidative and inflammatory lung damage developed in the everolimus-treated rats. Rutin attenuated this oxidative and pro-inflammatory lung damage induced by everolimus. Rats administered with everolimus developed severe interstitial pneumonia, lymphoid hyperplasia and desquamation in lung tissues. This severe damage was reduced to mild level by administering rutin. Conclusion: The adding rutin to the treatment regimen for pulmonary damage associated with everolimus may have a preventive impact.

Background and Objective: Acrolein-induced tissue damage has been attributed to oxidative stress and inflammation. This study aimed to determine the effects of adenosine triphosphate (ATP) and agomelatine, two substances known to have antioxidant and antiinflammatory effects, separately and in combination, on possible acrolein-induced lung damage in rats. Materials and Methods: There were 30 albino Wistar rats divided into five groups of six: Healthy (HG), acrolein (AC), ATP+acrolein (ATAC), agomelatine+acrolein (AGAC) and ATP+agomelatine+acrolein (AAAC). The dose of ATP administered intraperitoneally was 4 mg/kg and the dose of agomelatine administered orally was 25 mg/kg. An oral dose of acrolein 3 mg/kg was given 1 hr following the administration of ATP and agomelatine. For 30 days, the treatment protocol was followed once a day. Afterwards, the rats were euthanized and their lungs were removed. Oxidants, antioxidants and pro-inflammatory cytokines were measured in lung tissues. Results: In lung tissues, acrolein increased malondialdehyde, nuclear factor kappa, tumor necrosis factor-alpha, interleukin-1beta and interleukin-6 levels and decreased total glutathione, superoxide dismutase and catalase levels (p<0.001). The ATP+agomelatine, ATP and agomelatine treatments were the most effective at preventing these biochemical changes, respectively (p<0.001). As a result, the biochemical data of the HG and AAAC groups were similar (p>0.05). Conclusion: It could be suggested that both ATP and agomelatine might be effective against the pneumotoxic effects of acrolein, though a combination therapy would be more effective.

Introduction: Understanding the variations of abdominal vascular structures is important for preventing complications of abdominal surgical procedures for gastrointestinal disease such as necrotizing enterocolitis or others that may arise in patients with congenital cardiac disease. We analysed the coeliac trunk and its branches in children with congenital heart disease to determine whether there is a greater prevalence of associated vascular abnormalities. Methods: We retrospectively analysed thoracic computed tomography (CT) angiograms performed in our hospital in paediatric patients with congenital heart disease. We documented the anatomical variations observed in abdominal sections in which the coeliac trunk and hepatic arteries were included in the field of view. We used the Uflacker classification to describe anatomical variants of the coeliac trunk, and the Michels classification and its modified version (Hiatt classification) to describe the anatomy of the hepatic artery system. Results: Our study included 178 patients with congenital heart disease. We identified coeliac trunk variants in 10.7% of the patients. Gastrosplenic trunk was to the most prevalent variant, amounting to 5.6% of total cases. We found hepatic artery variations in 19.1% of the patients. According to the Michels classification, the prevalence of accessory left hepatic artery arising from the left gastric artery as 4.5%, compared to 6.7% based on the Hiatt classification. Conclusion: The prevalence of coeliac trunk and hepatic artery variations in patients with congenital heart disease was not greater in our study compared to other series in the literature. Clinicians must be vigilant about the variations detected in multislice CT scans to avoid complications resulting from vascular abnormalities, especially in patients who undergo abdominal surgery. Results: The study was participated in by 197 individuals, with an average age determined to be 29.45 +/- 7.48. The proportion of those supporting the implementation of SMA screening was 84.8% (n=167), while the intention to undergo the test was identified at 72.6% (n=143). A significant relationship was found between the willingness to be tested and variables such as the level of education, the absence of disabled close relatives, not considering SMA to be genetically inherited, finding screenings appropriate, the perceived impact of test results on marriage, and the belief that screening could not be conducted after marriage. Conclusion: While most recognized SMA's severity, there's a notable deficit in understanding its preventability, suggesting the need for targeted educational interventions to enhance genetic screening uptake.

Objectives Pain, a common human experience, is also experienced by nursing students, and pain beliefs, thoughts, and behaviors toward pain play an important role in coping with pain. There is insufficient data about the relationship between pain beliefs and pain coping strategies. Thus, this study aims to reveal the relationship between pain beliefs and pain coping approaches of nursing students and affecting factors. Methods A descriptive, cross-sectional, and correlational design was used, and the data were collected with respondent characteristics form, the Numerical Rating Scale (NRS) and the Pain Beliefs Questionnaire (PBQ), by researchers from 380 nursing students in the nursing department. Results Nursing students who used non -pharmacological interventions to cope with pain had higher levels of psychological belief (PBQ-P) scores (4.97-0.86) than organic belief (PBQ-O) scores (3.90-0.71) and the difference was statistically significant (p<0.001). According to the multivariate linear regression analysis results, nursing students' gender, utilizing non -pharmacological interventions, and NRS scores affected PBQ-P scores by 87.1% (R 2 =0.871) and PBQ-O scores by 81.0% (R 2 =0.810). Conclusions As can be seen from the results of this study, the higher psychological beliefs of nursing students who use non -pharmacological interventions to cope with pain are an example of this situation. In light of the information in this study, it should be taken into consideration that both psychological and organic beliefs have a strong relationship with pain intensity and pain coping approaches. Nursing students, the nurses of tomorrow, should be aware of the impact of psychological and organic beliefs on individuals' pain experiences and coping approaches and should take this into account when planning nursing care.
