Malnutrition and Related Factors in Older Patients With Gastrointestinal Cancer Receiving Chemotherapy

Akagündüz, Digdem Dogan; Sahin, Hilal; Akagündüz, Baran

Objectives The incidence and mortality of gastrointestinal (GI) malignancies increase exponentially with age. Malnutrition is a documented poor prognostic factor in older patients with cancer. There is insufficient data about the prevalence of malnutrition and associated factors in older patients with GI cancer. Thus, we aimed to investigate the prevalence of malnutrition and related factors among older patients with GI cancer. Methods A total of 121 patients aged over 70 years diagnosed with various types of GI cancers applied to the medical oncology clinic included in this cross-sectional study. We evaluated the nutrition status with a mininutritional assessment (MNA) score. Results The prevalence of malnutrition was 76 (62.8%) in our study population. The mean age was 76.5 (range 70 to 90 years), and 71 (58.6%) were male. In the multivariate logistic regression model, lower BMI (OR: 3.379, 95% CI: 1.465-7.812, p = 0.005), having gastroesophageal cancer (OR: 5.797, 95% CI: 2.387-14.091, p <0.001), treating with palliative chemotherapy (OR: 4.597, 95% CI: 1.799-11.772, p = 0.002), and frailty according to G8 score (OR: 10.798, 95% CI: 4.495-25.924, p <0.001) were associated with malnutrition. Conclusions Our study revealed that palliative chemotherapy, low BMI, frailty, and gastroesophageal cancer are risk factors for malnutrition in older patients with GI cancer. Physicians need to be aware of patients who may be at risk for malnutrition. Patients at risk of malnutrition may benefit from interventions to enhance their nutrition. Further studies consisting of larger cohorts are needed to determine malnutrition and related factors in older patients with cancer.

Comparing Personal Risk Communication with Generic Information on Breast Cancer Screening Attendance: A Randomized Controlled Trial

    Ustun, Cihat; Gursoy, Ersan

Objective: This study sought to discern the differential impacts of communicating individualized breast cancer risk versus disseminating generic breast cancer information on women's proclivity towards breast cancer screenings. Method: A structured experimental design was used, including 300 female volunteers aged between 40-69, who had not previously been diagnosed with breast cancer, in Erzincan. Data collection entailed a face-to-face administration of a 22-item questionnaire complemented by the ASSISTS scale. Participants were randomized into two conditions: one receiving a standardized briefing on breast cancer screening modalities, and the other being apprised of their lifetime risk of breast cancer development based on the modified Gail model. Results: Post-intervention, 55.8% (n=72) of the risk-informed cohort and 40.5% (n=41) of the generic information cohort expressed a willingness to undergo screening (p=0.022). Notably, the group informed of their personalized risk exhibited aAmore pronounced uptick in requests for breast self-examination, clinical breast examination, and mammography screening compared to their counterparts. Conclusions: The communication of personalized breast cancer risk proves to be aAmore efficacious and time-efficient strategy than generic information dissemination in bolstering screening participation rates.

The Effect of Intra-abdominal Pressure on Heart Rate Variability and Hemodynamics During Laparoscopic Cholecystectomy: A Prospective Observational Study

Eren, Nurhan; Onk, Didem; Koç, Alparslan; Kuyrukluyildiz, Ufuk; Nalbant, Remziye Aysenur

Introduction This study aimed to evaluate hemodynamic changes using heart rate variability (HRV) measurements in diabetic and nondiabetic patients who will undergo laparoscopic cholecystectomy and to provide our preoperative measurements to guide us for better perioperative anesthesia management. Materials and methods The study included 143 patients aged 40 years and older who would undergo elective laparoscopic surgery, did not have any comorbidities other than diabetes mellitus (DM) type II, and were in the American Society of Anesthesiologists (ASA) class I -III risk group. Patients were divided into two groups: the control group (n = 77) and the DM group (n = 66). The preoperative glycated hemoglobin (HbA1C) level was measured. Peripheral oxygen saturation (SpO2) and hemodynamic parameters such as systolic arterial pressure (SAP), diastolic arterial pressure (DAP), mean arterial pressure (MAP), heart rate (HR), and HRV parameters were measured preoperatively, perioperatively, and postoperatively. Intra-abdominal pressure (IAP) was administered at 10-12 mmHg. Results Even though SAP, DAP, MAP, and HR decreased with induction, they increased with insufflation, and an overall decrease was seen at the postoperative 24th hour for all parameters. When the groups were evaluated, no difference was observed except that the DAP was significantly lower in the DM group (p = 0.029) at insufflation and the HR was higher in the DM group at induction, and the difference was significant (p = 0.001). Preoperative HRV parameters were significantly lower in the DM group. According to the HRV parameters, although a decrease was observed after induction and insufflation, conversely, an increase was observed postoperatively. When the postoperative and preoperative values were compared, the standard deviation of the NN (R -R) intervals (SDNN), SDNN index, high frequency (HF), low frequency (LF), and LF/HF parameters were found to be significantly lower in the DM group than in the control group. Conclusion Diabetic patients are more sensitive to increased intra-abdominal pressure (IAP) in laparoscopic surgery, and the effects on cardiac autonomic functions can be determined by HRV measurements without clinically reflecting on hemodynamic data. Additionally, in diabetic patients with preoperative LF and/or HF values less than 100, we believe that careful follow-up in terms of autonomic neuropathy complications and anesthesia management should be done more meticulously in these patients.

Bas, Murat; Ayaz, Moh. Qasim; Agin, Kaya

Purpose: The purpose of this study is to explain the mediating effect of knowledge sharing (KS) between leader-member exchange (LMX) and work integration (WI) on healthcare workers. The study also examines the direct effects of LMX on WI among selected healthcare workers in T & uuml;rkiye. Methodology: The study targeted a sample of N = 295 to provide relevant information. The data were coland WI (Schaufeli et al., 2002). A purposive stratified sampling method was used since the participants were considered based on predefined criteria. The study used a 5-point Likert scale ranging from 1= strongly disagree to 5= strongly agree. SPSS 22.0 and AMOS 17.0 packages were used for data analysis and interpretation of statistical outputs. Results: The study revealed a significant positive relationship between variables. Goodness of fit values of the scales were also checked. First, the mediating variable (knowledge sharing) between LMX and WI was investigated. Second, the effect of LMX on WI was explored. Based on the findings, a significant positive Conclusion: According to the research results, significant relationships were found between LMX and WI, between LMX and KS, and between KS and WI. In addition, it was determined that knowledge sharing had a mediating role in the effect of LMX on WI, which was the main purpose of the research. According to the research results, high-quality LMX relationships enable employees to be more open to KS and to integrate more into their work. In this case, KS acts as a bridge that strengthens the link between LMX and WI. Employees find greater meaning in their work by sharing their knowledge, which in turn increases their commitment to it. As a result, leaders creating a culture that supports knowledge sharing can be seen as an effective way to increase work integration and improve organizational performance.

The Relationship Between Fear of Missing Out and Depression Among Tourism Students: A Serial Mediating Roles of Social Media Addiction and Anxiety

    Günes, Erkan; Ekmekçi, Zeynep

There is relatively limited research examining the impact of fear of missing out (FOMO) on increasing social media addiction (SMA) in youth, increasing anxiety, and ultimately leading to depression. The study aimed to examine the cumulative mediating effect of SMA and anxiety between FOMO and depression among university students majoring in tourism. In this context, an online survey was conducted with 395 participants using the convenience sampling method. Descriptive statistics, correlation analyses, and serial mediation analyses were performed in the study. Data analyses were conducted using SPSS 24.0, PROCESS Macro for SPSS v4.0, and AMOS 24.0. The findings revealed the mediating role of SMA and anxiety on the relationship between FOMO and depression. The increased level of FOMO in youths leads to an increase in SMA and anxiety levels, which ultimately leads to an increase in depression. Therefore, the bidirectional relationships of SMA and anxiety should be taken into account in the effect of FOMO on depression in youth.

EBÛ BEKİR RAcirc;ZÎ ON SPIRITUAL HEALTH: IMPORTANCE OF THE MIND

    Çelikkol, Suat

According to Islamic philosophers, man is the composite of both the body and the spirit, the spirit being the most important aspect. They have given the effort to care for the spiritual health of humans as much as their physical health. Eb & ucirc; Bekir R & acirc;z & icirc; also stated in his work ' et-T & imath;bbu'r-r & ucirc;h & acirc;n & icirc;' that spiritual diseases disrupt spiritual health and should be treated. The objective of this essay is to investigate what mental health is according to R & acirc;z & icirc;, how it can be protected through following certain moral values, and what the role of the mind in the treatment of spiritual diseases that disrupt spiritual health. It also aims to explain what kind of practices are essential for a wise life and to clarify the main principles of a moral righteous life. The essence of R & acirc;z & icirc;'s perspective of ethics the importance of the mind and hedonism is studied; and hedonism which causes spiritual illness is criticized in this essay. This studied indicates that R & acirc;z & icirc; has aimed to point out the hegemony of the mind in spiritual medicine; and that the concept of spiritual health is composed of keeping clear of disgraceful acts and battling with urges. Battling with urges and abstaining from disgraceful acts are the criterion of spiritual health in this philosophy. It is emphasized that by being knowledgeable in the solutions put forward, one can cope with spiritual diseases such as self-conceit, envy, and sadness. It can be said that Raz & icirc; gave medicine that are still valid today for the protection and treatment of spiritual health. [The Extended Abstract is at the end of the article.]

Dabanlioǧlu, Bülent; Yilmaz, Aysun; Akyüz, Sümeyye; Gülhan, Bariş

Introduction: The measurement of hepatitis C virus (HCV) RNA is a test that requires high cost, advanced technique, and qualified personnel. Diagnosis and treatment of patients may be delayed due to the high rate of false-positive results. This study aims to predict true antibody positivity and viremia by determining the most appropriate anti-HCV signal-to-cutoff (S/Co) value reflecting HCV infection. Methodology: The presence of anti-HCV antibodies and HCV RNA levels were examined in 72341 people who applied to the Mengücek Gazi Training and Research Hospital between January 2018 and December 2020. The anti-HCV levels were determined by using the Abbot Architect i2000 SR device (Abbot Diagnostics, Chicago, IL, USA). The levels of HCV RNA were determined in the COBAS AmpliPrep/COBAS, TaqMan 48 (Roche, Diagnostics, Pleasanton, USA) devices using serum samples from patients. Our study is a retrospective and methodological study. Results: Of the 150 patients with anti-HCV antibodies, 50 (33.3%) were HCV RNA positive, and 100 (66.7%) were HCV RNA negative. Anti-HCV levels of HCV RNA-positive patients were statistically higher than HCV RNA-negative patients. The most appropriate anti-HCV S/Co value for diagnosing hepatitis C patients was 15.4. The sensitivity of this value was 72%, specificity 88%, positive predictive value (PPV) 73.5%, and negative predictive value (NPV) 86.1%. Receiver operating characteristic (ROC) curve was significantly higher than 0.5 (95% confidence interval 0.938-0.827). Conclusions: Correct approaches can be applied in the diagnosis of HCV infection using the anti-HCV S/Co value found in our study.

Assessment of Psoas Muscle Index in Middle-Aged Type 2 Diabetes Patients: Impact of Insulin Therapy on Sarcopenia

Taskent, Ismail; Ece, Bunyamin; Aydin, Sonay

Objective: Sarcopenia, characterized by progressive skeletal muscle loss, poses significant health risks, including physical impairment and mortality. The relationship between sarcopenia and insulin resistance suggests insulin therapy’s potential in preserving muscle mass, particularly in Type 2 diabetes mellitus (T2DM) patients. This study aims to evaluate the Psoas Muscle Index (PMI) via computed tomography (CT) in middle-aged T2DM patients on insulin therapy versus oral antidiabetic drugs (OAD) and controls. Methods: This retrospective study included 107 middle-aged T2DM patients undergoing non-contrast CT scans and 58 age-matched controls. CT images were analyzed to calculate PMI. Statistical analysis included Chi-square tests, independent samples t-tests, Mann–Whitney U tests, and correlation analyses. Results: Insulin-treated patients exhibited higher PMI than OAD users (p < 0.001), while OAD users had lower PMI than controls (p < 0.001). No significant difference was found between insulin-treated patients and controls (p = 0.616). Negative correlations were observed between T2DM duration/age and PMI across all groups, with a positive correlation between T2DM duration and BMI observed in the OAD group. Conclusions: Insulin therapy in T2DM patients, regardless of age or disease duration, positively impacts muscle mass, highlighting its potential in preserving muscular health and advocating for tailored treatment strategies in T2DM management.

Cetin, Turkhun; Tokur, Oguzhan; Bozkurt, Hayrunnisa Bekis; Aydin, Sonay; Memis, Kemal Bugra; Kantarci, Mecit

Shear wave elastography (SWE) has become popular in clinical practice for many diseases. However, there is not adequate research on spleen-related diseases. This study aimed to investigate the potential of quantitative values obtained through SWE in evaluating spleen pathologies in the pediatric population and to demonstrate its performance to differentiate splenomegaly-related diseases. The research group retrospectively included children with pathological diagnoses related to the spleen from November 2016 to April 2021, and they were categorized into three groups, including portal hypertension (PH), benign lymphoid hyperplasia (BLH), and malignant infiltration (MI). Spleen sizes and parenchymal stiffness were also calculated for each group. Subsequently, mean spleen stiffness in each group was compared with normal values within the same age group. In total, 2781 children (1379 children for the study group; 1402 children for the control group) were enrolled in the study. The highest stiffness was observed in the PH group, which is statistically higher than others (p < 0.05). Although the mean spleen stiffness in the group with BLH was higher than the control and MI group, the difference was not statistically significant (p = 0.08). The mean stiffness in the group with MI was significantly lower than both the control group (p = 0.005) and PH (p = 0.01). In conclusion, using SWE in the differential diagnosis of etiologies causing splenomegaly could make an important contribution.

The role of blood nitric oxide level in predicting return of spontaneous circulation: a prospective case-control study

Bayramoğlu, Atıf; Tekin, Erdal; Kurt, Engin; Kaşali, Kamber; Kurt, Nezahat

OBJECTIVE: The aim of this study was to investigate whether there is a difference in serum nitric oxide levels between patients who return spontaneously after cardiopulmonary resuscitation and those who do not. We also examined the potential of using serum nitric oxide levels as a marker to make an accurate decision about patient survival. METHODS: We included 100 consecutive patients who were brought to the emergency clinic due to cardiac arrest. Blood samples were taken from these patients at admission, 30 min after admission, and when resuscitation was terminated. RESULTS: We found that there was a significant difference in NO1 and NO3 values between the group of patients who did not return after cardiopulmonary resuscitation and the group in which spontaneous circulation returned. The NO1 value was significant in the receiver operating characteristic (ROC) analysis, while the NO3 value was not. A higher NO1 value provided a higher rate of survival. CONCLUSION: Our findings suggest that nitric oxide may be a useful parameter to support the decision about patient survival. A higher NO1 value is associated with a better prognosis and survival rate. Therefore, serum nitric oxide levels may be a suitable indicator to support the decision-making process regarding patient survival.

Ayran, Gülsün; Çevik Özdemir, Hamide Nur

Aim: This research was conducted to evaluate the mediation effect of anxiety and life satisfaction on the relationship between Care burden and Parent Child Relationship in Turkish parents during the COVID-19 epidemic. Methods: The research was carried out with parents (221 women and 219 men) who have children between the ages of 6–18 living in Turkey. Data were collected in June–July 2021 using the demographic data form, “Care Burden Scale,” “Pervasive Anxiety Disorder-7 Test,” “Satisfaction with Life Scale,” and “Parent Child Relationship Scale.” Path analysis was used to analyse the relationships. In this study, structural equation modelling (SEM) was used to examine the path analysis. Results: It was found that the burden of care and anxiety of the parents and the parent–child conflict relationship were positively correlated and negatively correlated with the life satisfaction and parent–child closeness relationship (p < 0.05). Path model analyses revealed that the burden of care had a direct effect on anxiety and life satisfaction, and an indirect effect on the parent–child relationship, respectively. Conclusions: It shows that parents' perceptions of how the COVID-19 pandemic is affecting their mental health have implications for parent and child well-being, with stronger relationships for low-income families. Anxiety and life satisfaction had a mediating effect between care burden and parent–child relationship. Given the demonstrable impact of COVID-19 on the parent–child relationship, this study may guide the planning of coping strategies and programmes focused on mental health.

The purpose of this study was to evaluate the effectiveness of mRNA (BNT162b2), and CoronaVac (Sinovac Biotech) vaccines as a reminder dose following two doses of CoronaVac against COVID-19 infection, serious illness, and mortality in the geriatric population aged 75 and older during the delta variant dominant period. Our study comprised 2730 individuals the age of 75 and older in total, of which 1082 (39.6%) were male and 1648 (60.4%) were female. The vaccine effectiveness (VE) of 2 doses of CoronaVac + 1 dose of BNT162b2 vaccine combination against COVID-19 was determined as 89.2% (95% Confidence interval (CI) 80.7–93.9%), while the VE of 3 doses of CoronaVac vaccine was determined as 80.4% (95% CI 60.5–90.2%). Geriatric patients who received three doses of CoronaVac vaccine did not need intensive care. No deaths were observed in the vaccinated groups. While the VE of vaccination with 2 doses of CoronaVac + 1 dose of BNT162b2 was 41.8% (95% CI 0–74.1%) against hospitalization, 64.4% (95% CI 0–94.7%) against intensive care unit admission, the VE of vaccination with three doses of the CoronaVac was 78.2% (95% CI 0–96.5%) against hospitalization. In conclusion, our research showed that, even with the emergence of viral variants, a third dose of the CoronaVac and BNT162b2 vaccines is highly effective against symptomatic SARS-CoV-2 infection. Third-dose vaccination regimens, including heterologous and homologous vaccines, can be an effective tool in controlling the COVID-19 pandemic and the emergence of new variants.
