Objective. To date, there is still no consensus on the treatment of spinal degenerative disease. Current surgical techniques to manage painful spinal disorders are imperfect. In this paper, we aimed to evaluate the prospective results of posterior transpedicular dynamic stabilization, a novel surgical approach that skips the segments that do not produce pain. This technique has been proven biomechanically and radiologically in spinal degenerative diseases. Methods. A prospective study of 18 patients averaging 54.94 years of age with distant spinal segment degenerative disease. Indications consisted of degenerative disc disease (57%), herniated nucleus pulposus (50%), spinal stenosis (14.28%), degenerative spondylolisthesis (14.28%), and foraminal stenosis (7.1%). The Oswestry Low-Back Pain Disability Questionnaire and visual analog scale (VAS) for pain were recorded preoperatively and at the third and twelfth postoperative months. Results. Both the Oswestry and VAS scores showed significant improvement postoperatively (P < 0.05). We observed complications in one patient who had spinal epidural hematoma. Conclusion. We recommend skipping posterior transpedicular dynamic stabilization for surgical treatment of distant segment spinal degenerative disease.

Purpose. In the study presented, preoperative examinations and surgical methods were discussed along with literature, regarding two cases who were operated with the prediagnosis of acute appendicitis and for whom caecum diverticulitis was determined. Case 1. 21 years old male patient who had applied to hospital with complaint of abdominal pain, underwent an operation with a prediagnosis of acute appendicitis. Right hemicolectomy was performed with mass perioperatively determined in caecum. Histopathological examination revealed necrosis and inflammation in diverticulum wall. Case 2. 36 years old female patient applied to emergency department with abdominal pain and underwent an operation with a prediagnosis of acute appendicitis. Appendectomy and diverticulectomy were performed for whom inflame diverticula in caecum was determined perioperatively. Histopathological examination was revealed acute inflammation in diverticulum wall. Conclusion. Although solitary caecum diverticulitis is a rarely encountered disease, it must be considered in the differential diagnosis of right lower abdomen pain.

Bayram, B.; Limon, O.; Limon, G.; Akay, S.
Ovarian hyperstimulation syndrome (OHSS) is a rare iatrogenic complication of ovarian stimulation. Thromboembolic events are the most feared complication of OHSS. Most of the thrombotic events (75%) are of the venous origin. Stroke represents a rare but life-threatening complication of severe OHSS. A 29-year old woman was admitted to our emergency room, with left sided weakness, difficulty in breathing and nausea. The patient was given the diagnosis of OHSS and right middle cerebral artery infarction. Here we emphasise that the development of ovulation induction is a newly recognised cause of cerebral stroke in an otherwise healthy woman. (Hong Kong j.emerg.med. 2012;19:408-410)

Gomleksiz, Cengiz; Sasani, Mehdi; Oktenoglu, Tunc; Ozer, Ali Fahir
Interspinous spacers were developed to treat local deformities such as degenerative spondylolisthesis. To treat patients with chronic instability, posterior pedicle fixation and rod-based dynamic stabilization systems were developed as alternatives to fusion surgeries. Dynamic stabilization is the future of spinal surgery, and in the near future, we will be able to see the development of new devices and surgical techniques to stabilize the spine. It is important to follow the development of these technologies and to gain experience using them. In this paper, we review the literature and discuss the dynamic systems, both past and present, used in the market to treat lumbar degeneration.

Akin, Yigit; Kol, Arif; Sahiner, Ilker Fatih; Usta, Mustafa Faruk
Objective: The aim of this study is to evaluate surgical results and postoperative results of men diagnosed with organic erectile dysfunction (ED) who underwent penile prosthesis (antibiotic-coated vs. non-coated) implantation in the last 13 years. Additionally, we compared the frequency of complications between the two groups. Materials and methods: In the last 13 years, 202 men diagnosed with organic erectile dysfunction (ED) had previously undergone penile prosthesis implantation. Patient data were evaluated retrospectively. Of the 202 patients, 175 were included to the study. The mean age of patients was 57.5 (34-83) years. Preoperatively, a physical examination, medical and sexual history, International Index of Erectile Function (IIEF) questionnaire, Combined Injection and Stimulation (CIS) test and, if necessary, Nocturnal Penile Tumescence (NPT) / Penile Doppler ultrasonography (US) were performed in all cases. Results: All patients were diagnosed with organic ED. We determined that 43 (21%) patients had diabetes mellitus, 53 (26%) had a history of radical pelvic surgery, 38 (19%) had cardiovascular pathology and 19 (9%) had other reasons for ED. Approximately 98% of patients and their partners were satisfied with the surgical results of the penile prosthesis operation. Conclusion: The infection rate results in the present study were not statistically significant, and the number of patients in our study was not sufficient to make reliable conclusions. Studies with a larger sample sizes are needed.

Lesions of the pediatric glans penis are uncommon but important for pediatric urological care. Pediatric glans penis lesions have been reported more frequently in the literature in recent years. The lesions are diagnosed at birth or with advancing age. All lesions are benign and diagnosed with a simple physical examination and radiological evaluation. Although, a minority of these lesions have been linked with malignant transformation, the malignant transformation has not been reported for the pediatric glans penis in the literature. After diagnosis and treatment, long-term follow-up is very important. To date, the literature describes 142 pediatric glans penis malformations. We prepared a comprehensive study by using data from 2 different clinics and added 3 new cases from our institutions. The common glans penis anomalies, which are hypospadiac-epispadiac anomalies, were excluded.

Yeter, Volkan; Aritürk, Nursen
Purpose: To investigate the relationships between the birth parameters such as birth weight, gestational age, and birth type and the parameters of macula, optic nerve and retinal nerve fiber layer (RNFL) in preschoolers. Material and Method: After a broad ophthalmologic examination was performed for every healthy child at age between 3 and 6, the children were scanned by optical coherence tomography (OCT) for the evaluation of macula, optic disc, and RNFL parameters. The relationships between the birth parameters and OCT parameters were analyzed by Spearman correlation test and Mann Whitney U test. Results: One hundred and seventy-two eyes of 111 healthy children were evaluated in the study. The mean age of the children was 57.48 +/- 11.87 months (range: 30-80 months). Forty-eight percent of the children were delivered vaginally and the rest of them-by cesarean section. The negative correlations between gestational age and thicknesses of superior inner and temporal inner macula, and temporal and inferior RNFL were statistically significant(r=-0.204, p=0.019; r=-0.203, p=0.020; r=-0.230, p=0.009; r=0.175, p=0.047; respectively). While the parameters that showed negative correlations with birth weight were superior outer macular thickness, temporal, inferior, and average RNFL thicknesses and disc area, the parameters that demonstrated positive correlations with birth weight were cup/disc-area and horizontal cup/disc ratios (r=-0.196, p=0.025; r=-0.317, p<0.001; r=-0.269, p=0.002; r=-0.228, p=0.009; r=-0.189, p=0.032; r=0.181, p=0.04; r=0.197, p=0.025, respectively). Optic disc area of the children who were delivered vaginally was larger than that of the children who were delivered by cesarean section and this difference between these two groups was statistically significant (p=0.034). Discussion: There significant effects of the birth parameters such as birth weight and gestational age on some parameters of macula and optic disc obtained by OCT. It may be thought that the impairment of intrauterine growth could possibly influence optic nerve head and macula.

Urinary incontinence after prostatectomy for malignant and/or benign prostatic diseases is one of the most serious complications. Urinary incontinence occurs rarely after prostatectomy for benign diseases. However, it may occur more commonly after radical prostatectomy. These complications affect the patient's quality of life. Treatment of incontinence due prostatectomy includes several methods which differ from conservative to invasive surgical procedures. Slings and artificial urethral sphincters (AUS) are used more often than other procedures. Infection ratios and erosion of urethra are less frequent in AUS than in slings. Although AUS is the standard surgical procedure for severe urinary incontinence, slings may be used for severe male incontinence after prostatectomy due to the current development in sling materials.. Herein, we evaluate post prostatectomy urinary incontinence and treatments in the light of a review of current literature.

Taskesen, Orhan; Kanat, Sevtap; Cigir, Ebru; Gor, Tuba Kinay; Ilhan, AC
Hopelessness can be defined as negative expectations for the future. In other words, it is the level of pessimism for future (Yenilmez, 2010: 307). One of the most important reasons for the failure at school (Yenilmez, 2010: 1), hopelessness causes many mental health problems as well. In particular, it is among the main reasons of suicide, which has been shown as a public health issue throughout the history. In our country, suicide's being dense especially between the ages of 15-24 and 25-34 (Ucan, 2005: 19) led the different disciplines to perform some research on the subject. The fact that participation in artistic activities is effective for socialization of the person, dismissing stress, expressing himself gave birth to the idea that studying the effect of need of receiving art education informally on the level of hopelessness is important for future findings. This study was conducted to determine the status of receiving informally art education needs of high school students, to determine whether there is a significant relationship between the levels of hopelessness and needs of receiving art education informally, and to detect whether their levels of hopelessness contain significant differences according to the needs of receiving art education informally. This study, conducted through relational scanning method, was carried out with high school students in a medium-sized city in eastern Turkey. The study population is composed of 6883 students attending general high school in the academic year 2011-2012. The study group is composed of 115 girls and 65 boys a total of 182 students attending general high school in the academic year 2011-2012. In order to measure the students' need of receiving art education informally, Need of Receiving Art Education Informally Scale, developed by the researcher Taskesen, was used and in order to measure the levels of hopelessness, Beck Hopelessness Scale developed by Beck, Lester, Trexter (1974) and adapted to Turkey by Seber (1991), Durak (1993). For The data analysis, to compare the scores of the dependent variable of the sample, one-way ANOVA was used, which is one of the parametric tests. Personn correlation coefficient was used to determine the relationships between two variables. At the end of the ANOVA test, there was no significant difference in highs school students' level of hopelessness and also despite the negative correlation between the scores of receiving art education informally and level of hopelessness, there was significant association. The occurrence of metastatic insulinoma in a patient with pre-existing diabetes is extremely rare. We report the case of a diabetic patient who had frequent hypoglycemic episodes, apparently related to metastatic insulinoma. A 47-year-old woman with type 2 diabetes mellitus who had been treated with a sulfonylurea for 10 years began experiencing frequent episodes of hypoglycemia even after complete withdrawal of the hypoglycemic agent. Endogenous hyperinsulinism was found using a prolonged fasting test. Computed tomography identified a pancreatic tumor with metastatic liver lesions and peripancreatic lymphadenopathies. Ultrasound-guided hepatic mass biopsy was performed and the pathology examination of the tumor demonstrated a neuroendocrine tumor. The patient refused surgery; octreotide (10 mg) was administered. A few days later, the patient had no further episodes of hypoglycemia. Although the coincidence of insulinoma and diabetes is extremely rare, after excluding common reasons for hypoglycemia in diabetic patients, insulinoma must be considered.

This study has been carried out to investigate the level of loneliness, determine the level of dependence in the ADL and influential factors in the elderly people. This descriptive, cross-sectional study was conducted in 5 Family Healthcare Centers (FHC) located in central Erzincan, Turkey between March and June 2010. The data of the research was collected using a questionnaire that determined the descriptive and UCLA Loneliness Scale (UCLA-LS). Mean score of the UCLA-LS was determined as 51.59 ± 4.44. It was determined that 2% of the elderly ADL were completely dependent, 14.5% were semi-dependent. Factors such as being old, a widow/divorced, having a lower level of education and/or income, living alone, having a chronic disease, poor self-perceived health, lack of visits by relatives or acquaintances, dissatisfaction with the place of living, and being fully dependent while performing daily activities were determined as factors which increased the level of loneliness. Furthermore, factors such as being old, a female, a widow/divorced, living together with a daughter/son, having a chronic disease and poor self-perceived health were found to be influential in dependency. Elderly people who are alone and dependent in fulfilling their ADL should be monitored more closely.
RDL Kalıcı Kaydıİncele - Conference ObjectYalnız künye2012Online estimation of lower and upper bounds for heart sound boundaries in chest sound using Convex-hull algorithm
Çaǧlar, F.; Ozbek, I.Y.
Heart sound localization in chest sound is an essential part for many heart sound cancellation algorithms. The main difficulty for heart sound localization methods is the precise determination of the onset and offset boundaries of the heart sound segment. This paper presents a novel method to estimate lower and upper bounds for the onset and offset of the heart sound segment, which can be used as anchor points for more precise estimation. For this purpose, first chest sound is divided into frames and then entropy and smoothed entropy features of these frames are extracted, and used in the Convex-hull algorithm to estimate the upper and lower bounds for heart sound boundaries. The Convex-hull algorithm constructs a special type of envelope function for entropy features and if the maximal difference between the envelope function and the entropy is larger than a certain threshold, this point is considered as a heart sound bound. The results of the proposed method are compared with a baseline method which is a modified version of a well-known heart sound localization method. The results show that the proposed method outperforms the baseline method in terms of accuracy and detection error rate. Also, the experimental results show that smoothing entropy features significantly improves the performance of both baseline and proposed methods.

Karakurt, Papatya; Kaşikçi, Maĝfiret
The aim of this study descriptive study was to evaluate concordance with medication and those factors that affect the use of medicine in patients with hypertension. Data were collected using a questionnaire completed by 750 patients with hypertension between December 25, 2003, and April 30, 2004, in an outpatient hypertension clinic in Erzincan, Turkey. It was found that 57.9% of the patients did not use their medicines as prescribed. Forgetfulness, aloneness, and negligence were ranked as the top three reasons for this non-concordance, accounting for almost half (49.3%) of all patients with hypertension studied; price (expensive medicines) accounted for another quarter (26.5%). A statistically significant relationship with non-concordance was found for age, education level and profession. Patients' lack of knowledge related to the complications of hypertension was also found to have a statistically significant relationship with not taking medicines as prescribed. Gender, location of residence and salary were not found to be statistically related to concordance. These results indicate the need to educate patients with hypertension on how to use their medicine regularly and indicate also the target populations for this.

Objective: Study purpose was to examine the validity of a 13-item short form of the Medication Adherence Self-Efficacy Scale-Short Form (MASES-SF) in an independent sample of 150 hypertensive Turkish patients. Methods: This is a methodological study and 150 adult patients, who are receiving medication for hypertension in the last one year, were included as the study sample. The sample of the study was chosen with the method of simple random sampling. The study was conducted between October 25, 2010 and December 31, 2010 at Family Health Center (FHC). In this study, exploratory and confirmatory factor analyses were used for psychometric evaluation. Cronbach's alpha coefficient was used to evaluate the reliability of the scale. Results: Confirmatory factor analysis showed that all goodness indexes were at acceptable quality: χ2=61.72, df=65, p>0.05, GFI=0.99, CFI=1.00, RMSA=0.00. Reliability coefficient of the Turkish adaptation of MASES-SF was found as 0.94 and item-total correlations ranged between 0.13 - 0.52. Patients with uncontrolled hypertension had lower self-efficacy scores compared to those with normal blood pressure. Conclusion: This result indicated that the preliminary criterion validity is adequate. Psychometric testing demonstrated satisfactory internal consistency and validity of the instrument for patients in the study group. It can be used confidently in determining and testing interventions to improve medication adherence self-efficacy perceptions and behaviors in hypertensive Turkish patients. The MASES-SF is brief, quick to administer, and capture useful data on medication adherence self-efficacy in hypertensive patients.

For analyzing the changes in immunoglobulins, HSP70, ghrelin levels in blood samples were collected from volunteers vaccinated against swine flu before the vaccinations and on days 3, and 15, and 1 and 2 months after the vaccination in the presence or absence of fever associated with the it. The study included 11 subjects having developed a fever, and 13 subjects not having a fever, and 20 control subjects. Immunoglobulins were measured by nephelometry, and HSP70 and ghrelins by appropriate ELISA tests. The level of ghrelin was reduced, while the level of HSP70 was significantly increased in subjects who developed fevers. When temperatures were normalized, both levels were found similar to the control group. These results indicate that the increase in serum immunoglobulins levels associated with vaccinations, along with, elevations in HSP70 and reduced ghrelin levels associated with fever, may be the important parameters in the clinical evaluation and follow-up of treatments with vaccines.
