Kars, Ayhan; Salcan, Ismail; Altıparmak, Serkan; Dikmen, Mustafa Cihan
Objective: The aim of this retrospective study is to describe the clinical features and management of patients diagnosed with deep neck infection (DNI) and hospitalized in our clinic, and to share our experience, together with a review of the current literature. Methods: Patients who were diagnosed with DNI and treated in our clinic between January 2020 and June 2025 were retrospectively analyzed. Inclusion criteria: age > 18 years, complete medical records, diagnosis confirmed by clinical, laboratory, and imaging findings. Exclusion criteria: immunosuppressed patients, missing data, and pediatric cases. Diagnosis was based on clinical examination, laboratory tests, and imaging. Intravenous antibiotics were administered to all patients: incision and drainage or repeated aspiration was performed when abscesses were detected. Patients were followed for at least 3 months after discharge. Results: Sixty-two patients, 36 male and 26 female, were included in the study. Analysis of the etiological factors causing DNI revealed that 25 (40%) cases were odontogenic in origin and 19 (30.6%) derived from tonsillitis. Surgical drainage was required in 50% of patients. The overall complication rate was 9.7%, and no mortality occurred. Conclusion: Odontogenic infections were the leading cause of DNIs, followed by tonsillitis. Complete recovery was achieved with antibiotics alone in 50% of patients, and surgical intervention was successful in all remaining cases, resulting in 100% overall cure without recurrence. Early diagnosis and prompt combined management are key to excellent outcomes.

Aydemir, Hüseyin; Kızılgöz, Berihat
Objective: The researchers aimed to verify the mean values and range of Cobb’s angle to measure the lumbar lordosis on magnetic resonance (MR) images and to reveal any possible differences between genders or among age groups regarding the lumbar lordosis. Methods: After exclusions, 527 patients, with a mean age of 48.73 ± 14.65 years, were measured for lumbar lordosis using the Cobb’s angle of L1-S1 levels of the spine on MR images. A Kolmogorov-Smirnov test was performed to verify the data distribution properties. The comparison of the female and the male group was considered by the Mann-Whitney U test and the correlation with age was revealed using Spearman’s Rho test. Results: Cobb’s angle measurements revealed a mean value of 51.45 ± 10.71 degrees. Regarding the angle measurement results, the female group had higher lordosis than the male group (P < 0.001). A low grade, positive, and significant correlation between age and the Cobb’s angle was observed for the total study population and the male group. A moderate, positive, and significant correlation between age and lordotic angle measurement results was reached for the female group. Conclusion: Females had higher lumbar lordosis than males and age is positively correlated with lumbar lordosis according to the results of this investigation. Many other scientific investigations are required to verify the outcome of this research to ensure the mean value and the normal ranges of Cobb’s angle.
- Review ArticleAçık Erişim2025The Role of Imaging Methods in the Diagnosis and Treatment of Idiopathic Granulomatous Mastitis
Aydin, Sonay; Kantarci, Mecit; Aydemir, Hüseyin
Idiopathic granulomatous mastitis (IGM) is an uncommon chronic inflammatory disease of the breast with an unclear etiology that mostly impacts young women. Its incidence is higher in the Middle East and Asian countries, particularly in Türkiye. Clinical signs such as mass, fistula formation, swelling, and radiographic abnormalities such as lymph node enlargement, abscess, calcifications, localized or widespread asymmetric density, and hypoechoic lesions may result in a misdiagnosis of malignancy or infection. There are no imaging findings that distinguish between IGM and breast cancer in any imaging method; in most cases, imaging tests like ultrasonography, magnetic resonance imaging, or mammography reveal non-specific characteristics, leading to biopsy. Histological assessment is used to reach a conclusive diagnosis of IGM. Common medical treatments include systemic steroid treatment (oral corticosteroid), local steroid treatment (local steroid injection in breast,), antibiotics, immunosuppressive drugs (methotrexate, etc.), and monoclonal antibodies. Abscess drainage and surgical excision are further alternatives. Although the conclusive diagnosis of IGM is made histologically, imaging methods have a significant impact in evaluating the number, location, their size, abscess development, and the likelihood of intervention, stability or change in lesions, and response to treatment in patients with confirmed diagnosis. In addition, radiology and interventional radiology units guide clinicians in the timely diagnosis of the disease and play an important role in local steroid injection, which is used effectively in its treatment.

Aydin, Sonay; Taş, Necip Gökhan; Taş, Hakan Gökalp
Aims: Magnetic resonance imaging (MRI) frequently shows unexpected results unrelated to the presumed pathology. The aim of this study is to look into the frequency and type of incidental brain MRI findings in ICU patients. Methods: The Anesthesiology and Reanimation Intensive Care Unit at Erzincan Mengücek Gazi Training and Research Hospital was the setting for the retrospective observational study. A total of 5,145 ICU patients admitted between 2021 and 2023 were screened using the hospital information management system, eliminating those without a brain MRI. Incidental findings in brain MRI data were documented and divided into five categories: vascular (aneurysms, cavernomas, and other vascular structures), neoplastic (meningioma, pituitary mass, glioma, and other tumors), Chiari malformation, pineal cyst, and arachnoid cyst. Results: 1317 (25.6%) participants who received brain MRI were included in the study. Incidental MRI findings were detected in 366 patients (27.8%). The most common incidental discoveries were vascular anomalies (9.79%, primarily aneurysms and cavernomas), neoplastic lesions (8.81%, primarily meningiomas and pituitary masses), arachnoid cysts (2.89%), nonspecific plaques (2.81%), pineal cysts (2.66%), and Chiari malformations (0.84%). A large majority of these patients had clinically important findings that demanded additional clinical follow-up or evaluation; nevertheless, none required emergency surgical intervention. Patients with incidental findings had a mean age of 70.92±18.42 years (range 18-98). Conclusion: In conclusion, incidental MRI results are common in critically ill patients and have significant clinical consequences. Enhanced clinician awareness and consistent management practices are critical for improving patient care, reducing unnecessary interventions, and optimizing results. Future prospective studies are required to create tailored guidelines for this population.

Gürsoy, Ersan; Gürsoy, Nurcan; Vatansever, Mehmet; Nas, Mehmet Akif
This study aimed to evaluate family physicians’ knowledge and practices in primary healthcare settings. It focused specifically on their approaches to eye screening, early diagnosis, and management of eye diseases. This descriptive, cross-sectional study was conducted among family physicians working in the provinces of Erzincan, Trabzon, and Erzurum. Data were collected between September and December 2024 using a 37-item questionnaire developed through a literature review and pilot testing. The questionnaire, delivered via Google Forms, included Likert-scale items covering demographic information, eye screening practices, and knowledge levels. Data were analyzed using descriptive statistics, the Kolmogorov–Smirnov test, Kruskal–Wallis test, Mann Whitney U, and Spearman correlation analysis. A total of 124 family physicians participated in the study. While 40.3% reported performing red reflex screening only for infants aged 0–3 months, 29.8% stated they did not perform it at all. Additionally, 21.8% indicated that a direct ophthalmoscope was either unavailable or its availability was unknown at their centers. The mean knowledge score was 58.15, with male physicians scoring significantly higher than female physicians (p=0.023). No significant associations were found between knowledge scores and age, total professional experience, or job title. However, a positive and significant correlation was identified between the duration of family medicine practice and knowledge level (r=0.240, p=0.007). The findings revealed notable deficiencies in the knowledge and practices of family physicians concerning eye health screening and early diagnosis. To enhance the quality of primary eye care, it is essential to implement regular training programs, ensure the availability of ophthalmic examination tools, and establish national screening protocols. This study contributes foundational insights to strengthen the role of family physicians in the early detection and management of eye conditions.

Objectives: The study was conducted to profoundly examine the COVID-19 perceptions of individuals over 65 years during the lockdown using the Health Belief Model. Materials and Methods: The study was conducted using the case study design, one of the qualitative research methods. A study group was formed by selecting participants who met specific criteria, with a focus on maximum diversity. The sample consisted of 13 elderly individuals, including eight men and five women, with an average age of 69. Most of the participants had lived in rural areas (n=7) and three had contracted COVID-19. Additionally, the most common chronic diseases among the participants were hypertension (n=8) and diabetes (n=7). During the interviews with the participants, a semi-structured interview form consisting of 12 questions was used to ensure content and conceptual coherence. The data collection process was conducted through face-to-face interviews in the participants' own homes, where they felt most comfortable. Results: They more often expressed the COVID-19 perceived susceptibility as virulent disease, getting the disease, contaminating the disease, and death. In perceived severity, they stated that they would get the disease and die. Staying at home and following hygiene rules during quarantine were considered beneficial by most participants. However, some participants stated that quarantine caused stress, loneliness, inactivity and harmful effects. They mainly associated perceived barriers with prevention of basic and healthcare needs and defined self-efficacy as being patient, successful, and obeying the rules. Conclusion: The factors influencing older people's COVID-19 perceptions during the quarantine process are believed to have been identified by the Health Belief Model and these factors are important for the development of preventive health behaviours.
- ArticleAçık Erişim2025Investigation of the Effects of Fingolimod on the LPS-Induced Inflammatory Response in the HUVEC Cell Line
Halici, Hamza; Yilmaz, Eda
Aim: The objective of this study was to examine the impact of fingolimod on tumour necrosis factor-alpha (TNF-α) production in the context of a lipopolysaccharide (LPS)-induced inflammatory response in a human umbilical vein endothelial cells (HUVEC) model. Material and method: A total of 5×10³ cells were seeded into each well of the 96-well plate. The cells were incubated for 24 hours to allow for adhesion and settlement at the bottom of the well. Subsequently, the cells were treated with LPS (L2880 Sigma-Aldrich Lipopolysaccharides from Escherichia coli O55:B5) at a dose of 10 µg/ml for 12 hours. Our experimental groups were determined as follows. Healthy group: Group without LPS and Fingolimod treatment LPS Group: The Group was treated with 10 μg/ml LPS for 12 hours, but not treated with Fingolimod LPS+ Fingolimod 1 µM (LPS+ Fin 1) group: Group treated with 10 μg/ml LPS for 12 hours and treated with 1 µM fingolimod.LPS+ Fingolimod 0.5 µM (LPS+ Fin 0.5) group: Group treated with 10 μg/ml LPS for 12 hours and treated with 0,5 µM fingolimod. LPS+ Fingolimod 0.1 µM (LPS+ Fin 0.1) group: Group treated with 10 μg/ml LPS for 12 hours and treated with 0,1 µM fingolimod. At 12 and 24 hours, the plates were measured at a 570 nm absorbance value with a microplate reader spectrophotometer (Epoch Microplate Spectrophotometer, BioTek, USA) using the MTT method. At 12 and 24 hours, the supernatants of all experimental groups in the plates were collected. The levels of TNFα were quantified using ELISA kits in an Epoch Spectrophotometer System and a Take3 Plate device. Result: When the MTT results of our study were analysed, it was observed that the percentage of cell viability increased significantly in the fingolimod treatment groups compared to the LPS group. In addition, when the TNF-α results were analysed, it was observed that TNF-α levels were significantly decreased in the fingolimod-treated groups compared to the LPS group. Conclusion: The findings demonstrate that the production of TNF-α was markedly elevated in LPS-stimulated HUVEC cells. However, the administration of fingolimod exhibited a notable inhibitory effect on this increase, exhibiting a dose-dependent response. These observations suggest that fingolimod may possess the potential to regulate the inflammatory response.

- ArticleAçık Erişim2025The relationship between renewable energy consumption and health expenditures: The case of G-7 countries
Akın, Fatih
This study examines the impact of renewable energy consumption, economic growth, and death rates on healthcare expenditures in G-7 countries over the 1992-2023 period using panel data analysis. Preliminary analyses indicate cross-sectional dependence and heterogeneity across countries. The Fourier LM unit root test reveals that the variables are non-stationary, while the LM Bootstrap cointegration test supports a long-run relationship. Panel-wide AMG estimation results indicate that renewable energy consumption reduces healthcare expenditures, while economic growth and death rates increase healthcare expenditures. Country-based results indicate that renewable energy consumption reduces healthcare expenditures in Germany, Japan, and the United Kingdom. While economic growth increases healthcare expenditures in France, Germany, Italy, and the United Kingdom, death rates increase healthcare expenditures in France, Germany, and the United States, and decrease them in Japan. Furthermore, according to the Dumitrescu & Hurlin causality test, there is a unidirectional causality relationship running from renewable energy consumption and economic growth to healthcare expenditures. The findings show that energy and health policies should be integrated and that renewable energy investments can have positive effects on health systems.

Objective: Coronavirus 2 (SARS-CoV-2), the etiological agent of the COVID-19 pandemic that is rapidly affecting the entire world both in terms of health and economy, belongs to the beta coronavirus family. Although oxidative stress imbalance occurs in COVID-19 patients, how changing nitric oxide (NO) levels affect COVID-19 pathogenesis is not fully understood. For this purpose, NO levels were evaluated in the serum of COVID-19 patients. Material and Methods: A total of 50 adult COVID-19 patients and 32 healthy control individuals were included in the study. COVID-19 patients were classified as Mild-COVID-19, Moderate-COVID-19, and Intensive Care-COVID-19 according to national guidelines. NO concentrations were measured spectrophotometrically from serum samples of all patients. Biochemical data were also collected. Results: NO levels were lower in the patient group compared to the control group, but this difference was not statistically significant (p>0.05). Conclusion: The variability in NO levels suggests that it may be a result of immune inflammation and vascular dysfunction in COVID-19 patients.
