Investigation of Bromelain, Inulin and Saccharomyces cerevisiae Effects Against Escherichia coli Infection Developed in Fibroblast Wound Model; [Bromelain, İnulin ve Saccharomyces cerevisiae’nin Fibroblast Yara Modelinde Gelişen Escherichia coli Enfeksiyonuna Karşı Etkilerinin Araştırılması]

Başer, Sümeyye; Çelebi, Demet; Çelebi, Özgür; Genç, Sıdıka Tülay; Taghizadehghalehjoughi, Ali

Aim: Wound infections, especially in today’s world where antibiotic resistance is increasing, pose a serious health problem. Escherichia coli complicates treatment by forming a biofilm. In this study, the individual and combined effects of bromelain, inulin, and Saccharomyces cerevisiae on an Escherichia coli infection model created in fibroblast cells were investigated. Methods: The antibacterial activity of bromelain, inulin, and Saccharomyces cerevisiae was evaluated using the disk diffusion method; minimum inhibitory concentration (MIC), minimum bactericidal concentration (MBC), and fractional inhibitory concentration (FIC) analyses were conducted. Biofilm inhibitory activity was evaluated using the crystal violet staining method. In vitro wound model has been created using human fibroblast cells. Bromelain, inulin, and Saccharomyces cerevisiae agents were applied to the cells infected with Escherichia coli. Cell viability was determined by the MTT test and analyzed through measurements of total oxidant status (TOS) and total antioxidant capacity (TAC). To evaluate the inflammatory response, IL-1β and IL-10 levels were quantitatively measured using the ELISA method. Results: According to disk diffusion, MIC/MBC, FIC, and antibiofilm analyses, it has been determined that the triple combination (Bromelain + Inulin + Saccharomyces cerevisiae) provides the highest antibacterial and synergistic effect. In the antibiofilm tests, bromelain showed the strongest inhibitory effect (58.3%), but the effect of the triple combination was more limited. In the MTT analysis, cell viability increased in the treatment groups, reaching 112% especially in the triple combination. While TOS levels increased with infection, they significantly decreased after treatment, whereas TAC levels increased. In ELISA analyses, the IL-1β level significantly increased with infection, but decreased with treatment. IL-10 levels, on the other hand, significantly increased, especially in the triple combination, demonstrating an anti-inflammatory response. Conclusions: The combination of bromelain, inulin, and Saccharomyces cerevisiae has shown antimicrobial, antioxidant, and anti-inflammatory activity against Escherichia coli infection, thereby supporting wound healing.

Background: The evaluation of axillary lymph nodes (ALNs) using ultrasound (US) plays a crucial role in the diagnosis and management of breast cancer. This study aims to assess the diagnostic performance of both quantitative and qualitative features of suspicious ALNs using superb microvascular imaging (SMI). To the best of our knowledge, this is the first prospective study in the literature to evaluate the largest number of suspicious ALNs using SMI and to analyze the combined diagnostic value of these findings. Methods: Our prospective study involved 102 patients with suspicious lymph nodes in the axilla. SMI method was employed individually to assess distribution, appearance, and number of vascular structures. After histopathological or follow-up confimation, images were re-evaluated. We also examined the diagnostic value of various combinations of these vascular features. Lastly, two radiologists, each with 22 and 4 years of experience, independently reviewed the images, blinded to clinical and pathological data, to assess interobserver agreement. Results: A total of 108 lymph nodes were evaluated in 102 patients, including 67 benign and 41 malignant cases. The diagnostic performance of various combinations in receiver operating characteristic (ROC) analysis revealed that the highest accuracy (78.70%) and area under the curve (AUC) value in ROC analysis were achieved by distribution-appearance combinations (AUC: 0.776, p < 0.001). Interobserver agreement was significant for both the SMI and PDUS methods. In patients diagnosed with breast cancer, the diagnostic accuracy for suspicious axillary lymph nodes reached 92.76%, with an AUC value of 0.855. Conclusion: In conclusion, our findings suggest that SMI facilitates more accurate identification of suspicious axillary lymph nodes prior to surgical intervention.

Surgical trauma triggers oxidative and inflammatory responses that contribute to postoperative complications. Although the antioxidant and anti-inflammatory effects of ketamine have been reported, the impact of anesthesia duration on these mechanisms remains unclear. Forty-two male Wistar rats were randomized into healthy control (HG), ketamine only (KET; 60 mg/kg, i.p.), or laparotomy plus ketamine with 0–4 additional ketamine doses at 20 min intervals (KET + L, KET + L1–L4). At 24 h, levels of MDA, tGSH, SOD, CAT, IL-1β, IL-6, TNF-α, adrenaline and noradrenaline were measured in tail-vein blood. One-way ANOVA with Tukey’s post hoc test was used. Laparotomy under single-dose ketamine increased MDA and pro-inflammatory cytokines and decreased tGSH, SOD, CAT, ADR, and NDR versus HG and KET (all p < 0.001). After laparotomy, repeated ketamine dosing produced graded decreases in MDA and cytokines and increases in tGSH, SOD, CAT, ADR, and NDR toward control levels; effects were most pronounced in KET + L4 (all p < 0.001). Ketamine alone did not differ significantly from HG. In rats, ketamine modulates postoperative biological stress in a duration-dependent manner; prolonging anesthesia reduces oxidative–inflammatory load and restores catecholaminergic tone. These findings strongly support revisiting dose–duration protocols and underscore the need for mechanistic and clinical studies.

ML-based tooth shade assessment to prevent metamerism in different clinic lights

Karcioglu, Abdullah Ammar; Efitli, Esra; Simsek, Emrah; Ozdogan, Alper; Karatas, Furkan; Senocak, Tuba

The aesthetic understanding has found its place in dental clinics and prosthetic dental treatment. Determining the appropriate prosthetic tooth color between the clinician, patient and technician is a difficult process due to metamerism. Metamerism, known as the different perception of the color of an object under different light sources, is caused by the lighting differences between the laboratory and the dental clinic. The traditional trial-error color determination method, coupled with the high cost of instrumental color value determination, has prompted the need for alternative technologies. The integration of AI technologies into dental practices aims to minimize errors in tooth shade assessment, reduce equipment usage, eliminate the impact of clinic lighting on color detection, and decrease costs for patients, dentists, and laboratories. In this study, a machine learning (ML) based approach that can correctly detect tooth shade even under different clinical lights has been developed. A dataset consisting of 580 dental images taken under four different clinical lights and with five repetitions was created using the Vita color shade guide. Experimental studies were performed using the HSV color space, 6 different ML algorithms and color histograms. As a result, 97.93% accuracy rate was achieved by using cross-validation (cv = 5) in the classification of 29 color values independent of clinical lights. It has been shown that the tooth colors can be determined with high accuracy using ML algorithms and metamerism can be prevented.

Yilmaz, Ayşe Daştan; Çalik, Kıymet Yeşilçiçek; Budak, Mehtap

Backgroud and objective: Maternal obesity and gestational weight gain (GWG) are important determinants of maternal and neonatal health outcomes. This study aimed to investigate the effects of prepregnancy body mass index (PPBMI) and GWG on maternal and neonatal outcomes in Türkiye. Methods: This prospective cross-sectional study was conducted in a public hospital in Turkey between December 2021 and December 2022. A total of 1137 women between the ages of 19 and 45 who had singleton and live births and had complete medical records were included in the study using simple random sampling. Data were collected using medical records and a structured interview form. PPBMI and GWG were calculated from self-reported weight and height data. Kruskal-Wallis, Mann-Whitney U, and binary logistic regression tests were used to analyze the data. Results: Among the participants, 4.7% were underweight, 61% had a normal weight, 27.6% were overweight, and 9.9% were obese on the basis of PPBMI. According to the Institute of Medicine (IOM) GWG guidelines, 17.2% had insufficient GWG, 55.1% had adequate GWG, and 27.8% had excessive GWG. Overweight women had significantly increased odds of gestational diabetes mellitus (OR = 0.479, p = 0.024), genital tract infections (OR = 2.15, p = 0.000), urinary tract infections (OR = 2.42; p = 0.011), elective cesarean delivery (OR = 8.62, p = 0.035), macrosomia (OR = 9.15, p = 0.031), and low APGAR (Appearance, Pulse, Grimace, Activity, Respiration) scores (< 7 at 5 min) (OR = 0.20, p = 0.000). Obese women also showed higher odds of elective cesarean (OR = 9.56; p = 0.030) and macrosomia (OR = 8.27, p = 0.044). Underweight women had higher odds of neonatal hospital stay > 5 days. Insufficient GWG was associated with increased risks of low birth weight (OR = 0.46, p = 0.013), third-trimester bleeding (OR = 0.45, p = 0.016), and neonatal hospitalization > 5 days (OR = 0.44, p = 0.003), while macrosomia risk was lower (OR = 2.06, p = 0.001). Conclusion: PPBMI and GWG are significant predictors of maternal and neonatal health outcomes. Elevated PPBMI and inappropriate GWG are associated with increased risks of gestational diabetes, macrosomia, cesarean delivery, low APGAR scores, low birth weight, and prolonged neonatal hospitalization. These findings highlight the critical role of routine assessment and management of maternal weight status and weight gain during prenatal care in reducing preventable complications for both mothers and their newborns.

Yakan, Hasan; Muğlu, Halit; Erdoğan, Musa; Türkeş, Cüneyt; Demir, Yeliz

New naphthyl-thiosemicarbazone derivatives (1–9) were obtained from 1-naphthaldehyde and numerous thiosemicarbazides. New naphthyl-thio/carbohydrazones (10–12) were prepared from 1-naphthaldehyde and various thio/carbohydrazides. FT-IR, 1H NMR, 13C NMR, and elemental analysis were used to elucidate the structures of the newly obtained compounds. The inhibitory effects of these compounds against human carbonic anhydrase isoforms I and II (hCA I and hCA II) and acetylcholinesterase (AChE) were systematically evaluated. Several compounds exhibited potent inhibition, particularly derivatives bearing halogenated and electron-donating aromatic substituents. Among them, compound 11 demonstrated the strongest inhibition for all three enzymes, with KI values of 52.42 nM (hCA I), 59.23 nM (hCA II), and 40.16 nM (AChE), surpassing the reference standards acetazolamide and tacrine. Structure–activity relationship (SAR) analysis highlighted the critical influence of substituent type and position on enzymatic activity. In addition, molecular docking simulations were conducted to elucidate the binding interactions of the most potent compound with hCA I, hCA II, and AChE enzymes. The docking results supported the in vitro findings, revealing favorable binding energies and key interactions such as π–π stacking and hydrogen bonding, especially for compound 11.

Protective effect of adenosine triphosphate against hydroxychloroquine ototoxicity in rats

Erhan, Ertugrul; Suleyman, Zeynep; Altuner, Durdu; Demir, Ozlem; Gulaboglu, Mine; Suleyman, Halis

One of the side effects of hydroxychloroquine is ototoxicity. This study examined the effect of adenosine triphosphate (ATP) on ototoxicity in rats treated with hydroxychloroquine (HCQ). In this study, 24 rats (male, albino Wistar) were used. The animals were divided into four groups: healthy control group (HG), ATP alone group (ATPG), HCQ alone group (HCQG), and ATP + HCQ (AHQG) group. The ATPG and AHQG groups were injected with ATP intraperitoneally (IP) at a dose of 4 mg/kg/day. The HG and HCQG groups were treated with the same volume of saline as the solvent by IP route. The AHQG group was administered HCQ orally at a dose of 120 mg/kg/day 1 h after the ATP and saline administration. This procedure was repeated daily for 10 days. The rats were euthanized (50 mg/kg thiopental sodium, IP) and after, vestibulocochlear nerve tissues were removed and the tissue oxidant and antioxidant parameters were studied. The tissues were also subjected histopathological examination. ATP significantly (p < 0.001) suppressed the increase in malondialdehyde associated with HCQ in vestibulocochlear nerve tissues as well as the decrease in total glutathione, superoxide dismutase, and catalase levels. ATP also reduced the severity of oedema, vascular congestion, dilatation, and collagen deposition in degenerated myelinated nerve fibers associated with HCQ. ATP is effective against oxidative damage associated with HCQ in vestibulocochlear nerve tissue.

Kılınç, Seda; Karaoğlan, Mert; Kuzucu, Mehmet

Tilapia piscidin 4 (TP4) is a 25-amino-acid cationic antimicrobial peptide derived from Nile tilapia (Oreochromis niloticus). TP4 has demonstrated strong antimicrobial activity against a broad range of pathogens and exhibits additional biological functions, including wound healing and anticancer activity. In a previous study, recombinant TP4 peptide was produced in the Pichia pastoris KM71H expression system and purified by Ni–NTA affinity chromatography. The aim of this study was to evaluate and compare the cytotoxicity of recombinant TP4 against HT-29 colon cancer and HGF-1 healthy gingival fibroblast cells. The antimicrobial activity of TP4 was first assessed using minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) assays, confirming its efficacy. Subsequently, the cytotoxic effects of TP4 on HT-29 and HGF-1 cells were further analyzed using the MTT, lactate dehydrogenase (LDH), total antioxidant/oxidant status (TAS/TOS), and malondialdehyde (MDA) assays. TP4 disrupted the antioxidant balance in both cell types, with a significant increase in the oxidant levels and a marked decrease in the antioxidant levels in HT-29 cells. The IC50 values of TP4 were determined to be 15 μg/mL for HT-29 cells and 25 μg/mL for HGF-1 cells. Apoptosis assays, including Annexin V & Dead Cell, RT-qPCR, and ELISA, revealed that TP4 induced apoptosis predominantly in HT-29 cells, as evidenced by the upregulation of BAX and CASP3 and the downregulation of the BCL-2. In conclusion, TP4 exhibited selective and potent apoptotic effects on HT-29 colon cancer cells, suggesting its potential as a novel therapeutic agent for colon cancer treatment.

Yılmaz, Ahmet; Aydın, Hakan; Akkaş, Önder; Aydın, Yener; Eren, Suat; Uslu, Hakan

Background: We aimed to investigate the genetic diversity and population structure of Echinococcus granulosus using various cyst materials from the parasite obtained from humans in eastern Türkiye. Methods: We used cyst samples from 61 patients diagnosed with hydatid cysts by the Department of Thoracic Surgery and Department of Radiology at Atatürk University Faculty of Medicine between 2019 and 2023. Microscopic examination of protoscolex in cyst samples was performed, and, if available, the patients' ELISA IgG results were obtained from the hospital automation system. Two primer pairs revealed DNA regions specific to E. granulosus (12S rRNA and mt-COX1). Sequence analysis was performed on samples found positive for mt-COX1 PCR, and a phylogenetic tree was constructed from the resulting data. Results: The rate of scolex in microscopic examination was 31/61 (50.8%), and the ELISA IgG positivity rate was 36/51 (70.6%). As a result of PCR analysis of 12S rRNA and mt-COX1 gene regions of 61 cyst samples, positivity was determined in 48 (78.7%) samples. As a result of PCR product sequence analysis, it was found that all samples were of the E. granulosus sensu stricto (G1, G3) genotype. Lung cyst alone was detected in 25 (40.9%) cases, while only liver cyst was detected in 15 (24.6%) cases. In 13 (21.3) cases, cysts were present in both the lungs and the liver. Conclusion: The genotype of E. granulosus isolates obtained from cyst material from patients diagnosed with hydatid cysts in Erzurum and surrounding provinces was a domestic sheep strain (G1, G3).

Ates, Irem; Isik, Bahar; Gozen, Fusun; Yazici, Gulce Naz; Gulaboglu, Mine; Mammadov, Renad; Huseynova, Gulbeniz; Altuner, Durdu; Suleyman, Halis

Background: Tramadol binds to opioid receptors and inhibits norepinephrine and serotonin reuptake, causing serotonin syndrome. Tianeptine stimulates serotonin reuptake and reduces serotonin levels. The aim of this study was to investigate whether tianeptine is effective against serotonin syndrome that may occur with serotoninergic drugs such as tramadol and citalopram. Methods: Rats were divided into eight groups (n = 6) that received tramadol (50 mg/kg), citalopram (10 mg/kg), or tianeptine (5 mg/kg) alone or a combination of tramadol + citalopram, tramadol + tianeptine, citalopram + tianeptine or tramadol + citalopram + tianeptine at the same doses administered to the stomach by oral gavage for 3 weeks. The healthy control group was given saline. Malondialdehyde, total glutathione, superoxide dismutase, and catalase levels were measured in removed brain tissues. The tissues were also examined histopathologically. Results: In the tramadol, tramadol + citalopram, and tramadol + citalopram + tianeptine groups, malondialdehyde levels were found to be higher compared to the control group, while glutathione, superoxide dismutase, and catalase levels were found to be lower. In other groups, values close to the control group were measured. Morphological degeneration was observed in neurons in the tramadol + citalopram group. The swelling of astrocytes and pericellular edema in oligodendrocytes were also observed. A significant population increase was noted in microglial cells. Blood vessels belonging to the tissue were observed to be severely dilated and congested. Histopathological damage was partially resolved in the group given tramadol + citalopram + tianeptine. Conclusions: The tramadol + citalopram combination caused severe oxidative stress in brain tissue. Tramadol alone caused mild damage in brain tissue, whereas tianeptine prevented the brain damage caused by tramadol.

The phenolic composition, antioxidant capacity, and enzyme inhibition activities of Mangifera indica L. leaf (MLs) ethanol extract were comprehensively evaluated to explore its therapeutic and industrial applications. Quantitative profiling of 21 phenolic compounds was performed using the LC–MS/MS method, with vanillic acid (1242.47 μg/L), gallic acid (283.58 μg/L), and quercetin (102.40 μg/L) identified as the most abundant constituents. Antioxidant activities were assessed through DPPH, ABTS, FRAP, and CUPRAC assays, revealing moderate radical scavenging (DPPH: 26.87% ± 2.25%, ABTS: 14.65% ± 1.83%) and metal reduction capacities (FRAP: 0.118 ± 0.07, CUPRAC: 0.172 ± 0.03). In addition, MLs extract demonstrated dose-dependent inhibitory effects on acetylcholinesterase (AChE, IC50: 18.73 μg/mL), butyrylcholinesterase (BChE, IC50: 8.56 μg/mL), and α-glucosidase (IC50: 10.83 μg/mL), highlighting its potential in the management of neurodegenerative diseases such as Alzheimer's and metabolic disorders including diabetes. The findings emphasize the bioactive potential of M. indica leaves, positioning them as a promising resource for sustainable valorization. By showcasing the applicability of this agricultural by-product, the study provides a foundation for innovations in the food, nutraceutical, and pharmaceutical sectors. Nevertheless, further in vivo and clinical investigations are essential to fully validate their safety and therapeutic efficacy.

Enhanced therapeutic efficacy of platinum-doxorubicin nanoparticles on colon and breast cancer cell lines

Kalındemirtaş, Ferdane Danışman; Cilasun, Gökçe Erdemir; Kariper, Afşin

In this study, platinum nanoparticles (PtNPs) were synthesized and their potential to improve the efficacy of doxorubicin (DOX) in cancer treatment was investigated. H2PtCl6, LiAlH4, and trisodium citrate were used during the synthesis of PtNPs. They were characterized using dynamic light scattering (DLS), Fourier transform infrared spectroscopy (FTIR), and scanning transmission electron microscopy (STEM). The diameter of the PtNPs was measured to be 21.72 nm without DOX loading and approximately 212 nm after DOX loading (DOX-PtNPs). FTIR confirmed the binding of DOX to PtNPs. In addition, MTT assays showed that DOX-PtNPs have a stronger effect on MCF-7 and HCT116 cancer cells than free DOX, even at low doses. The IC50 value for MCF-7 cells treated with DOX was determined to be 4.81 µg/ml, while it was significantly lower for the DOX-PtNP group at 0.64 µg/ml. A similar trend was observed in HCT116 cells, where the IC50 value for DOX was 5.03 µg/ml, while for DOX-PtNPs it was 0.62 µg/ml. In summary, the activity of DOX in these cells was increased approximately eightfold by PtNPs. Moreover, DOX-PtNPs showed no significant cytotoxic effects on normal HUVEC cells at low doses. Moreover, DOX-PtNPs enhanced apoptotic activity in HCT116 cells without inducing drug resistance as demonstrated by Rho123 staining and annexin/PI analyses. The significance of this study lies in the pioneering use of DOX-PtNPs in colon cancer, the synthesis of smaller PtNPs, the eightfold increase in the efficacy of DOX, and the demonstration that DOX-PtNPs do not significantly increase drug resistance.

A Mixture Modeling of the Behavioral Activation System and Problematic Internet Use in Adolescents: The Role of Metacognition, Online Dissociation, and Insomnia

Çelik, Oğuzhan; Yalçın, Ragıp Ümit; Eşkisu, Mustafa

Due to the rapidly developing technological advancements, the overuse of the Internet has led to the emergence of problematic Internet use, which has become a part of our daily lives. This study aims to investigate the relationships between problematic Internet use, Behavioral Activation System (BAS), metacognition, online dissociative experiences, and insomnia. Participants were 341 Turkish adolescents aged between 15 and 19 years (Mean = 15.49, SD = 1.03), of whom 63% were female. Participants responded to the Behavioral Inhibition/Behavioral Activation System Scale, Metacognition Questionnaire, Van Online Dissociative Experiences Schedule, Insomnia Severity Index, and Chen Internet Addiction Scale. The mixture structural equation modeling approach was used to analyze the data. We found a positive relationship between BAS and metacognition, as well as insomnia, and a negative relationship with online dissociation. Furthermore, BAS was indirectly related to problematic Internet use via metacognition, online dissociation, and insomnia. The mixture analysis grouped participants into two latent classes. The first class (76%) had low levels of metacognition, online dissociation, insomnia, and problematic Internet use and high levels of behavioral activation while the second class (24%) had low levels of behavioral activation and high levels of metacognition, online dissociation, insomnia, and problematic Internet use. These findings indicate that individuals with high impulsivity, a tendency toward fun-seeking, and reward sensitivity are predisposed to excessively use the Internet and that metacognition, online dissociative experiences, and insomnia play a determining role in this behavior.

Intestinal endometriosis: A rare cause of acute care surgery; [Bağırsak endometriozisi: Acil cerrahinin nadir bir nedeni]

Kutluk, Fadime; Ergün, Sefa; Uludag, Server Sezgin

BACKGROUND: Intestinal endometriosis is a rare condition that can cause gastrointestinal symptoms such as abdominal pain, constipation, and diarrhea. It occurs in approximately 5%–15% of women with endometriosis. Although it rarely leads to obstruction or perforation, there is no clear consensus on its optimal management. Hormonal therapy is considered the first-line treatment for endometriosis; however, the best approach for intestinal involvement remains controversial. This study aims to contribute to the literature by evaluating patients with intestinal endometriosis who underwent surgery for acute abdomen. METHODS: Sixteen patients who underwent emergency surgery for acute abdomen at the Emergency General Surgery Clinic of İstanbul University-Cerrahpaşa, Cerrahpaşa Medical Faculty of Medicine, between February 2016 and April 2023 were identified. Four patients voluntarily withdrew from the study, and 12 were included in the analysis. Demographic data, laboratory findings, surgical records, length of hospital stay, pathology reports, postoperative complications within the first 30 days, and their management were reviewed. Patients’ current health status was assessed through telephone interviews. RESULTS: The mean age of the patients was 39.3±9.2 years. Nine patients underwent surgery with a preoperative diagnosis of intestinal obstruction, while three were operated on for suspected acute appendicitis. Among the patients with intestinal obstruction, three underwent laparoscopic ileocecal resection, and three had segmental small bowel resection. One patient underwent anterior resection with appendectomy, and another underwent both anterior resection and ileocecal resection with ileocolostomy. One patient underwent a left hemicolectomy. The mean hospital stay was 7.9±5.9 days. Postoperative complications occurred in three patients (25%). The mean age of patients who experienced complications was significantly higher than that of those who did not (p<0.007). Histopathological examination revealed benign full-thickness endometriosis in all cases. The mean follow-up period was 50.6 months, with no recurrences observed. CONCLUSION: Acute abdomen due to intestinal endometriosis-related obstruction is extremely rare. In reproductive-aged women presenting with acute abdomen, intestinal endometriosis should be considered in the differential diagnosis. In cases of clinical suspicion, intraoperative consultation with a gynecologist is recommended, and a multidisciplinary approach should be adopted to optimize treatment planning.
