Dilitatlı, Sümeyye; Demirbanka, Fatma Gül Özçelik; Türkmenoğlu, Burçin; Köse, Muhammet
A new series of pyridine-based 1,2,3-triazole derivatives (SD5–7) was synthesized through Cu(I)-catalyzed azide–alkyne cycloaddition and fully characterized by FT-IR, NMR, HRMS, and elemental analysis. The molecular structures of SD6 and SD7 were further confirmed by single-crystal X-ray diffraction, which revealed stabilizing π–π stacking and weak intermolecular interactions. Additionally, the properties of compounds SD5, SD6, and SD7 were investigated by density functional theory using B3LYP/6-311G*. Cytotoxic properties were investigated against OUMS (cancerous) and HUVEC (healthy) cell lines using MTS assays. SD5 exhibited the most potent effect on OUMS cells (IC50 = 98.05 µg/mL), exceeding the activity of cisplatin, while SD6 displayed statistically significant selectivity between cancer and normal cells. Antibacterial screening against Gram-positive (S. aureus, B. cereus) and Gram-negative (E. coli, K. pneumoniae) strains showed that SD5 and SD6 had moderate inhibitory effects (MIC = 5 mg/mL), whereas SD7 was inactive. These results suggest that pyridine-substituted 1,2,3-triazoles represent valuable scaffolds with promising dual anticancer and antibacterial potential for further development. Compounds SD5–7 exhibited cytotoxic effects against the OUMS cell line within the concentration range of 98.05–876.0 µg/mL, while in antibacterial tests, the ligands showed lower activity compared to streptomycin. SD5 and SD6 compounds, whose activity was determined in vitro studies, were analyzed to elucidate the interaction mechanism with the targets identified in molecular docking studies. Compounds SD5 and SD6 were analyzed in molecular docking studies by interacting with the crystal structures of 5I96 and 6U4J.

Arslanlı, Sevilay Ergün; Aygüler, Edip; Kasımoğlu, Necla; Çelik, İsa
Aim: This study aimed to investigate fathers' newborn care-related experiences, identify the factors affecting their participation in the care process and explore their support needs. Methods: This qualitative study was conducted using a phenomenological design. Data were collected through in-depth face-to-face interviews conducted based on a semi-structured interview guide. The data were analysed using Colaizzi's phenomenological data analysis method. Results: Fathers were willing to participate in newborn care and were particularly involved in practices such as feeding and basic hygiene care. Lack of experience, fear of harming the baby and lack of self-confidence regarding caregiving skills emerged as significant difficulties experienced by the fathers in newborn care. The fathers pointed out that their wives, healthcare professionals and family members were important sources of support, while demanding work schedules, limited paternity leave and traditional gender roles were among the leading barriers limiting their participation in caregiving. Conclusion: The fathers were willing to participate in newborn care, but their participation in the care process was limited owing to individual, social and structural barriers. It is believed that strengthening their caregiving skills and creating supportive environmental conditions will increase their participation in newborn care. Implications for Practice: Healthcare professionals, particularly pediatric and neonatal nurses, should develop father-focused training programmes and provide guidance on care practices to encourage fathers to participate in newborn care actively. Developing policies that support paternity leave and family-centred care practices can also increase fathers' involvement in early infant care.

Karakurt, Papatya; Ünsal, Ayla; Bilgin, Sonay; Özdemir, Selen
Introduction: Hypertension is a leading global public health problem requiring effective long-term self-management. This study aimed to determine the knowledge and self-efficacy levels of patients with hypertension and examine the relationship between these variables. Methods: This cross-sectional study was conducted with 200 hypertensive patients attending cardiology and internal medicine outpatient clinics of a training and research hospital between March and July 2024. Data were collected using the Patient Information Form, Hypertension Knowledge Level Scale (HK-LS), and Hypertension Self-Efficacy Scale (HSES). Descriptive statistics, independent t-test, Mann-Whitney U test, Kruskal–Wallis test, and Spearman correlation analysis were used. Results: The mean HK-LS score was 17.23 ± 3.9 (range: 4-22), and the mean HSES score was 55.16 ± 6.4 (range: 41-72). Of the participants, 57% (n=114) were female, and 43% (n=86) were male. Female participants, individuals with higher education, better perceived income, regular exercise habits, and absence of comorbid disease had significantly higher knowledge and self-efficacy scores (p <0.01). A moderate positive correlation was found between knowledge and self-efficacy (p <0.01). Discussion: The findings indicate that increased hypertension-related knowledge is associated with stronger self-efficacy in disease management. Sociodemographic and lifestyle factors influence both variables. Conclusion: Improving hypertension knowledge may enhance self-efficacy and disease management. Tailored educational interventions targeting vulnerable groups are recommended. 2026, Bentham Science Publishers

Antibiotic resistance is one of the most important problems threatening global public health by complicating the treatment of infections worldwide. The increase in resistant microorganisms creates a serious economic and social burden on healthcare systems and increasingly limits treatment options. Conscious use of antibiotics, infection control measures and the development of alternative treatment strategies are vital to ensure sustainability in health. Klebsiella pneumoniae is the most common gram-negative bacterium among urinary tract infections. Treatment of infections has become difficult due to the resistance to beta-lactam antibiotics. Bee venom (BV) and nanovesicle fractions isolated from BV are bioactive compounds with antimicrobial and antibiofilm activity. The aim of this study was to determine the antimicrobial and antibiofilm effects of BVand bee venom-derived nanovesicle fractions against the nosocomial infection agent K. pneumoniae, and to evaluate CTX-M PCR band detection under the tested conditions. Minimum inhibitory concentration (MIC), antibiofilm activity, fractional inhibition concentrations (FIC), CTX-M PCR band detection, and viability rates in L929 cells were evaluated for both BVand the bee venom-derived nanovesicle fraction against K. pneumoniae. MIC value of nanovesicle fractions isolated from BVwas determined as 1.95 mg/L. In combination with piperacillin and tazobactam, a synergistic effect was detected with a value of 0.5. Antibiofilm activity was measured with the highest absorbance value of 0.163 and 0.094 for BV nanovesicle fractions. A detectable CTX-M PCR band was observed in the BVgroup at 0.5× MIC, whereas no detectable band was observed at both concentrations by day 4. In the bee venom-derived nanovesicle fraction group, no detectable CTX-M PCR band was observed at 2× MIC and 0.5× MIC under the tested conditions. In contrast, detectable CTX-M PCR bands were observed in the piperacillin-tazobactam group. It was reported that the combination groups decreased the viability rate in L929 cell lines. The bee venom-derived nanovesicle fraction showed antimicrobial, antibiofilm, and PCR-based detection findings under the tested conditions, supporting the need for further investigation in broader in vitro and in vivo models.
RDL Kalıcı Kaydıİncele - ArticleAçık Erişim2026Physicians’ perspectives on human papillomavirus vaccination in Türkiye through YouTube content analysis
Yanmaz, Gadime; Gursoy, Ersan
Background: Human papillomavirus (HPV) is a significant public health problem that is closely associated with various types of cancer, especially cervical cancer. Despite the proven effectiveness of HPV vaccines, misinformation and anti-vaccine rhetoric are widespread in the public, especially on social media platforms such as YouTube. In Türkiye, the HPV vaccine has not yet been included in the national immunization program, limiting its accessibility. Objective: The aim of this study is to analyze the opinions of physicians about the HPV vaccine in the most watched videos on YouTube in Türkiye and to reveal how expert opinions contribute to public awareness, correction of misinformation and guidance. Method: The 26 most watched HPV vaccine videos on YouTube produced by physicians in Türkiye were analyzed using thematic analysis. Videos were selected through searches using specific keywords (and their Turkish equivalents) such as “HPV vaccine”, “Should HPV vaccine be given?“, “Side effects of HPV vaccine”, “Is HPV vaccine harmful?” and “Cervical cancer vaccine” between February 2025 and April 2025. Only videos shorter than 20 min with physicians as the primary speakers and at least 1000 views were included. The verbal content of the physicians’ opinions in the videos were transcribed, and the analyzed videos were categorized under five main themes and six subthemes. Results: Physicians generally emphasized that the HPV vaccine is safe, effective, and necessary for both women and men. The types of vaccines, age of administration, side effects, and misconceptions among the public were discussed in detail. While there was a general consensus that the vaccine provides long-term protection, different views were expressed on whether it is effective for a lifetime. Physicians tried to correct misinformation in the videos and emphasize the importance of the vaccine in cancer prevention. Another important finding was that the fact that the vaccine was not included in the national vaccination program constituted a serious obstacle to access. Conclusion: Physicians play a critical role in delivering scientifically based health information to the public via social media. Expert content on digital platforms such as YouTube increases access to accurate information about the HPV vaccine and corrects misconceptions in the public. Findings indicate a pressing need for policy action to incorporate the HPV vaccine into the national immunization program and enhance expert visibility in combating misinformation on digital platforms.

Bingol, Koray; Celik, Elif
Purpose: The common hepatic artery (CHA) demonstrates considerable anatomical variability with important implications for hepatobiliary, pancreatic, transplant, and endovascular procedures. This study aimed to evaluate the prevalence, morphometric characteristics, and three-dimensional topographic relationships of CHA variations using computed tomography angiography (CTA) in a large cohort. Methods: This retrospective study included 1347 consecutive patients (604 women, 743 men; mean age 55.1 ± 14.4 years) who underwent abdominal CTA. Hepatic arterial anatomy was classified according to the Hiatt and Song classifications. Morphometric parameters of the celiac trunk, CHA, gastroduodenal artery, and proper hepatic artery, and the three-dimensional relationships of the CHA to adjacent structures, were evaluated using multiplanar reformation, maximum-intensity projection, and volume-rendering techniques. Comparisons were performed according to sex, age group, and variation status. Results: Conventional hepatic arterial anatomy (Hiatt type I) was identified in 64.7% of patients, whereas 35.3% had variant anatomy, most commonly an isolated left hepatic artery variation (13.4%). The CHA originated predominantly from the celiac trunk (97.0%), with hepatomesenteric (2.3%) and direct aortic (0.7%) origins being uncommon. Male patients had significantly larger arterial calibres, and most morphometric parameters increased with age. Variant anatomy was associated with longer CHA, greater celiac trunk-to-CHA distance, wider departure angle, and more frequent retropancreatic, intrapancreatic, and portal/superior mesenteric venous confluence-related courses (all p < 0.001). Conclusion: Hepatic arterial variation is associated with distinct morphometric and topographic characteristics. These population-specific reference data may improve preoperative assessment and support safer hepatobiliary, pancreatic, transplant, and endovascular interventions by improving procedural planning and reducing vascular injury.

Işık, Nurten Arslan; Ayran, Gülsün
This study examined acculturative stress and state anxiety among Afghan refugee minors forced to migrate due to war, comparing unaccompanied minors with those who migrated with their parents. The sample comprised 136 participants aged 10–18 years: 77 accompanied and 59 unaccompanied refugee minors. Acculturative stress was measured via the Acculturative Stress Scale, and state anxiety was assessed via the State-Trait Anxiety Inventory for Children (STAIC). Data were collected through face-to-face interviews. The results indicated that unaccompanied refugee minors experienced significantly higher levels of both acculturative stress and state anxiety than their accompanied peers did. These findings underscore the substantial psychological burden associated with migrating without parental support and highlight the importance of implementing targeted, trauma-informed mental health interventions to foster resilience and emotional well-being among unaccompanied refugee minors.

Purpose: To evaluate the impact of the “My Hospital Friend YARENCİK” initiative on hospital adaptation among children aged one to three, and to identify factors influencing this adaptation. Method: The study was conducted using a pre-test–post-test controlled quasi-experimental design and took place between March 2021 and October 2021 at the pediatrics clinic of a teaching and research hospital with children aged 1–3 years. Participants were sequentially assigned first to the control group (n = 38) and then to the experimental group (n = 34). Data were obtained using the Personal Information Form (containing the children’s and mothers’ personal information) and the Children’s Hospital Adaptation Evaluation Scale. Ethical principles were followed. Results: A significant difference was found in favor of the experimental group in the post-test between the mean scores of The Children’s Hospital Adaptation Evaluation Scale (p<.001). In addition, the hospital compliance of three-year-old children whose mothers were working was found to be significantly higher (p<.05). Conclusion: A group of one-to-three-year-olds admitted to the hospital found it easier to adhere to the nursing initiative when using My Hospital Friend Yarencik in their playtime. Hospitalized children who are one and two years old and whose mothers do not work may need more attention.
RDL Kalıcı Kaydıİncele - ArticleAçık Erişim2026Safety and equity in scaling minimally invasive surgery worldwide in 109 countries using cholecystectomy as a tracer procedure: a prospective cohort study
Summary Background: Minimally invasive surgery is rapidly expanding globally, yet there is insufficient knowledge of how to scale this technology safely and equitably across diverse health systems. We aimed to identify health-system factors associated with safe implementation of minimally invasive surgery globally, using minimally invasive cholecystectomy as a tracer procedure. Methods: We conducted a multicentre, prospective cohort study of consecutive adults undergoing cholecystectomy between July 31 and Nov 19, 2023, in 1218 hospitals across 109 countries. Data were collected by more than 10 000 health-care workers using a core measurement set mapped to the WHO Health System Building Blocks and the Global Patient Safety Action Plan. The primary outcome was 30-day procedure-specific complications, with multilevel logistic regression used to examine associations between health-system features and patient outcomes. This study is registered on ClinicalTrials.gov (NCT06223061). Findings: Among 52 187 included patients, the adjusted procedure-specific complication rate varied 40-fold between hospitals, from 0·3% in the lowest risk quintile to 12·1% in the highest risk quintile. Despite large structural differences across income groups in access to minimally invasive surgery, diagnostics, and emergency services, country income level was not independently associated with complication rates (adjusted odds ratio [OR] 0·81 [95% CI 0·59–1·10] for upper-middle income vs high income and 0·99 [0·70–1·39] for lower-middle income or low income vs high income). Three modifiable hospital-level factors were strongly associated with safer outcomes: establishment of local simulation-based training facilities (adjusted OR 0·78 [0·71–0·86]; p<0·0001), adoption of intraoperative safety and communication strategies (0·87 [0·79–0·96]; p=0·0046), and on-site CT diagnostics (0·79 [0·65–0·97]; p=0·0220). Training facilities showed the greatest benefit in hospitals with limited infrastructure and an inexperienced workforce: the number needed to treat to prevent a procedure-specific complication was 21 (95% CI 14–35; p<0·0001). Interpretation: Safe implementation of minimally invasive surgery varies widely worldwide but is not defined by national income level; differences in outcomes reflect the ability of health systems to adopt and safely deploy new surgical techniques. We identified for the first time that the presence of local simulation-based training facilities is independently associated with improved patient outcomes. Simulation appears to be fundamental to the safe delivery of minimally invasive surgery, particularly in resource-constrained settings. Together with safety systems and diagnostic capacity, these findings offer actionable targets for health systems seeking to equitably scale up essential surgical technologies. Funding: NIHR Global Health Research Unit and Wellcome Leap SAVE Programme.

Background The most common cause of end-stage kidney disease is diabetes mellitus (DM). The most commonly used renal replacement therapy in Turkey and in many countries around the world is haemodialysis (HD). Glycaemia control is important in these populations. In this study we aimed to screen for glycaemic control and complications in a large population of diabetic HD patients in Turkey. Methods A total of 16 043 patients were screened in 253 dialysis centres in Turkey and 5038 diabetic HD patients were included in the study. At participating centres, patients' diabetes history, complications, medications, haemoglobin A1c (HbA1c) and other laboratory data were reviewed and recorded by nephrologists. Results The average age of the patients was 64.0 ± 11.2 years and 56% were male. The mean HbA1c was 7.4 ± 1.5%. Patients were divided into three groups according to the HbA1c level (<6.5%, 6.5-8% and >8%). As the HbA1c levels increased, the mean systolic blood pressure and diastolic blood pressure increased significantly. In addition, as the HbA1c levels increased, the number of patients with coronary artery disease, patients undergoing coronary artery bypass graft surgery and the rate of patients with diabetic retinopathy and vision loss increased. Diabetic foot disease and amputation rates were also higher in the group with poor glycaemic control. The number of patients using intensive or mixed insulin was also higher in the group with high HbA1c levels. In ordinal logistic regression analysis, age significantly decreased and higher body mass index slightly increased the risk of a higher HbA1c. Also, the need for a diabetic diet was greater in those with high HbA1c levels. Conclusion Our study highlights that the target values for diabetic HD patients in Turkey are partially compatible with the 2022 Kidney Disease: Improving Global Outcomes guidelines for diabetes management. Nevertheless, more effort and teamwork are needed to improve patient outcomes.
RDL Kalıcı Kaydıİncele - Review ArticleAçık Erişim2026Who Is the Surgeon Now: Human Hands or Machine Minds? Artificial Intelligence in Orthopedics from Diagnosis to Follow-Up—A Structured Narrative Review
Yapıcı, Furkan
Background: Artificial intelligence (AI) is transitioning from proof-of-concept prototypes to clinically utilized tools in orthopedics. The key translational question is whether AI will replace surgeons or, more realistically, augment human expertise. Methods: A structured narrative review was conducted using PubMed/MEDLINE, Web of Science, and Google Scholar (completed 31 January 2026). Peer-reviewed English-language studies that utilized AI for orthopedic clinical problems were eligible. To synthesize the 73 included papers without forced quantitative pooling, evidence was qualitatively charted and organized using a four-axis framework: clinical task, data modality, validation maturity, and intended user/setting. Results: The evidence base was dominated by retrospective, imaging-centered AI studies (predominantly LOE III). Radiograph-based fracture detection and automated measurements were frequently reported to achieve high discrimination, though performance degraded in complex or “edge” cases. Predictive models for arthroplasty and spine outcomes demonstrated variable actionability and inconsistent reporting of calibration. Common translational barriers across subspecialties included limited external validation, dataset shift, and a scarcity of prospective impact studies. Conclusions: Current evidence supports an augmentation paradigm rather than a replacement paradigm. AI acts as a “co-surgeon,” improving triage and standardizing quantification. However, safe clinical translation requires representative external validation, rigorous failure analysis, and human-in-the-loop workflows where surgeons retain ultimate accountability.

Bakir, Ebru; Ergün Arslanlı, Sevilay; Tümen, Tuğba
Aim To explore nursing students' clinical experiences in pediatric pain management and identify their educational needs. Background Hospitalized children worldwide continue to experience unrelieved pain despite treatment advances. Undergraduate nursing education is essential for pain management competence; however, research has focused primarily on theoretical knowledge, offering limited insight into real clinical experiences. Design Descriptive qualitative study. Methods Twenty-four third- and fourth-year nursing students participated in individual semi-structured interviews. Data were analyzed using thematic analysis. Results Five themes emerged: (1) Interpreting Pediatric Pain: Gaps in Clinical Recognition and Assessment; (2) Clinical Decision-Making: Difficulties Integrating Assessment and Intervention; (3) Communication as a Relational Strategy; (4) Parental Influence: Balancing Support and Clinical Decision-Making; and (5) Clinical Performance Under Emotional Strain. Students demonstrated gaps in pediatric pain assessment, frequently neglecting its subjective nature and underusing standardized tools. Pharmacological decision-making was largely unrecognized; non-pharmacological strategies were emphasized instead. Although students applied appropriate communication techniques, integrating these into clinical decisions proved difficult. Emotional challenges were prevalent, underscoring the need for self-regulation and reflective practice. Conclusions Nursing students faced considerable challenges meeting the complex demands of pediatric pain management. Deficits in pain assessment and pharmacological competence, combined with overreliance on non-pharmacological approaches, may compromise effective pain care during transition to professional practice. Implications for practice Curriculum developers should design pediatric pain education bridging theory and clinical practice through concept-based, student-centered strategies — including simulation, case-based discussions, and reflective practice — alongside structured, supervised clinical experiences targeting pain assessment, pharmacological decision-making, and family communication.

Introduction – The present study aimed to examine the effect of sport mindfulness on athlete burnout among Olympic combat sport athletes and to investigate the mediating role of athlete self-compassion and the moderating role of emotion regulation in this relationship. Methods – This study employed a cross-sectional correlational research design and included 379 athletes (279 men and 100 women) actively competing in Olympic combat sports. Data were collected using the Mindfulness Inventory for Sport, the Athlete Burnout Questionnaire, the Self-Compassion Scale–Athlete Version-Short Form and the Athlete Emotion Regulation Scale. The construct validity of the measurement instruments was evaluated using confirmatory factor analysis. Relationships among the study variables were examined using Pearson correlation analysis. The proposed hypotheses were tested using the PROCESS macro, specifically Models 4, 1, and 14. Results – The findings indicated that sport mindfulness positively and significantly predicted athlete self-compassion (B = 0.155, p < 0.001). Athlete self-compassion, in turn, was found to have a significant negative effect on athlete burnout (B = −0.798, p < 0.001). Furthermore, the interaction between athlete self-compassion and emotion regulation significantly predicted athlete burnout (B = −0.060, p < 0.001). The moderated mediation analysis demonstrated that emotion regulation significantly moderated the indirect association between sport mindfulness and athlete burnout (Index = −0.009). Examination of the conditional indirect effects revealed that the burnout-reducing effect of sport mindfulness through athlete self-compassion was stronger among athletes with higher levels of emotion regulation. Conclusion – The findings suggest that the association between sport mindfulness and athlete burnout cannot be explained solely by a direct relationship. Rather, this relationship operates through athlete self-compassion and varies according to athletes' levels of emotion regulation. These findings underscore the importance of integrating mindfulness-based skills, self-compassion development, and emotion regulation capacity into interventions designed to enhance psychological resilience and prevent burnout among combat sport athletes.

Background/Objectives: N-terminal pro-B-type natriuretic peptide (NT-proBNP) is an established heart failure biomarker, but its associations with cardiac magnetic resonance (CMR) parameters in patients with reduced ejection fraction remain unclear. This study examined the associations between serum NT-proBNP concentrations and CMR-derived functional, structural, and tissue characterization parameters in patients with HFrEF and evaluated which imaging parameters remained associated with NT-proBNP after multivariable adjustment. Methods: This retrospective single-center study included 92 consecutive patients with HFrEF who underwent CMR and NT-proBNP measurement within a 24 h window, with a median absolute interval of 6 h (range, 1–15 h) between the two assessments. CMR analysis included left ventricular ejection fraction (LVEF), left ventricular and left atrial (LA) cavity diameters, myocardial mass, extracellular volume fraction (ECV), the late gadolinium enhancement (LGE) burden, and the T2 signal intensity (T2-SI) ratio. Spearman correlation, univariate linear regression, and multivariable linear regression analyses were performed using log-transformed NT-proBNP values. Results: Log NT-proBNP demonstrated significant correlations with LVEF (rs = −0.359, p < 0.001), T2-SI ratio (rs = −0.332, p = 0.001), ECV (rs = 0.383, p < 0.001), and LA diameter (rs = 0.347, p < 0.001). No statistically significant correlations were observed between log-transformed NT-proBNP and LGE burden, LV cavity diameter, myocardial mass, LV wall thickness, age, or the recorded comorbidities. In multivariable analysis, LVEF, T2-SI ratio, and LA diameter remained associated with NT-proBNP after adjustment for the available covariates. Conclusions: Lower LVEF, a lower T2-SI ratio, and a larger LA diameter were associated with higher NT-proBNP levels in patients with HFrEF. The T2-SI ratio and LA diameter should be considered exploratory imaging markers requiring prospective validation.

Curcumin (CUR) exhibits potent anti-cancer effects; however, its poor water solubility limits its clinical use. This study explores gelatin nanoparticles (GelNPs) as nanocarriers to improve curcumin delivery. We aimed to enhance the release and efficacy of photosensitive curcumin-loaded GelNPs (Cur-GelNPs) using infrared (IR) light-induced localized hyperthermia in a 3D co-culture cancer model. Cur-GelNPs were synthesized and characterized via DLS, FTIR, STEM, Raman, HPLC, and DSC. Cytotoxicity, migration, invasion, apoptosis, and drug resistance were assessed using MTT, Transwell, Hoechst/PI staining, and Rho123 assays, respectively. Optimal results were achieved by IR treatment at 38 °C for 30 s. Cur-GelNPs localized in the cytoplasm due to curcumin’s natural fluorescence. At 25 µg/ml, Cur-GelNPs significantly reduced the viability, invasion, and migration of colon cancer cells, while promoting apoptosis and mitochondrial damage—more effectively when combined with IR. Importantly, minimal toxicity was observed in healthy cells, suggesting selective cancer targeting. This method enables spontaneous drug-macromolecule binding, reducing time, cost, and labor. Overall, IR-activated Cur-GelNPs represent a promising approach for targeted colon cancer therapy and may offer broader contributions to nanomedicine and drug delivery systems.
RDL Kalıcı Kaydıİncele - ArticleAçık Erişim2026Effects of Subconjunctival Bevacizumab and Ranibizumab on Corneal and Systemic Oxidative Stress Biomarkers in an Alkali Injury Model
Yarbağ, Abdulhekim; Bardaş Özkan, Ebru; Ulaş, Mustafa; Arslan, Yusuf Kemal
The study tests the efficacy of subconjunctival ranibizumab and bevacizumab treatments in determining the oxidant and antioxidant levels during alkali-induced corneal neovascularisation. The researchers assigned 24 New Zealand White rabbits into four different groups, which included a healthy control, an alkali-injured control, a BV-treated group, and an RN-treated group, with six rabbits per group. All animals received alkali injury treatment except for the HC group. The AC group received six doses of subconjunctival saline 24 h after their injury, while the BV group and RN group received one dose of 0.5 mg of their respective treatment. The researchers conducted 14-day assays on normal tissue, blood plasma, and erythrocytes to determine total antioxidant status and total oxidant status. The researchers performed biochemical assays for total antioxidant status TAS and total oxidant status TOS on samples that were collected after the 14-day observation period. The corneal TOS levels increased in all injured groups when matched against the HC group. The RN group showed the highest corneal TOS levels, but the difference between the RN and AC and BV groups did not reach statistical significance. The injured groups showed higher corneal TAS levels than HC (p = 0.002), but the AC, BV, and RN groups showed no differences. The injured groups showed a significant decrease in erythrocyte TAS compared with HC (p < 0.001), but the injured groups showed lower plasma TOS levels than HC (p < 0.001). The researchers found no other systemic differences between the groups that received anti-VEGF treatment. The study results show that alkali injury leads to both local and systemic changes in redox status. The two anti-VEGF agents caused numerical changes in corneal oxidative parameters, but these changes did not achieve statistical significance. The research requires further investigation to understand potential agent-related effects on corneal redox balance, which should take place in larger, more detailed studies.

Erlotinib, an epidermal growth factor receptor tyrosine kinase inhibitor (EGFR-TKI), is widely used in cancer therapy; however, hepatotoxicity limits its clinical use. This study investigated the protective effects of Liv-52, a polyherbal hepatoprotective formulation, against erlotinib-induced hepatotoxicity in rats and compared its efficacy with melatonin. The animals (n = 24, Wistar albino rats) were randomly categorized into four groups: healthy (HG), erlotinib (ERG), Liv-52 + erlotinib (LEG), and melatonin + erlotinib (MEG). Liv-52 (50 mg/kg/day, orally) and melatonin (10 mg/kg/day, orally) were administered once daily for two weeks. Erlotinib (10 mg/kg, orally) was given every other day to ERG, LEG, and MEG groups for two weeks. Serum alanine aminotransferase (ALT), aspartate aminotransferase (AST), and lactate dehydrogenase (LDH) were measured. Hepatic malondialdehyde (MDA), total glutathione (tGSH), superoxide dismutase (SOD), and catalase (CAT) levels were analyzed. Additionally, double immunofluorescence staining was performed to evaluate apoptotic (poly[ADP-ribose] polymerase-1 [PARP-1], apoptosis-inducing factor [AIF]), inflammatory (cyclooxygenase-2 [COX-2]), and anti-inflammatory (interleukin-10 [IL-10]) biomarkers in liver tissues. Histopathological examination was also conducted to assess structural alterations. Erlotinib significantly increased MDA, ALT, AST, and LDH while decreasing tGSH, SOD, and CAT (p < 0.001). Strong immunoreactivity for PARP-1, AIF, IL-10, and COX-2, as well as severe hydropic degeneration and necrosis, was observed in ERG (p < 0.05). Both Liv-52 and melatonin significantly ameliorated biochemical, histopathological, apoptotic, and inflammatory alterations (p < 0.05). Notably, Liv-52 demonstrated superior hepatoprotective efficacy compared to melatonin. These findings indicate that Liv-52 effectively attenuates erlotinib-induced hepatotoxicity by modulating oxidative stress, inflammatory responses, and apoptotic pathways, thereby preserving liver function and structural integrity.

Background: Pembrolizumab, a programmed cell death protein 1 (PD-1) inhibitor, is widely employed in oncological practice; however, its propensity to induce nephrotoxicity through immune-mediated oxidative and inflammatory mechanisms remains an insufficiently characterized clinical concern. The present study comparatively investigated the renoprotective effects of flunarizine, a voltage-dependent calcium channel antagonist, and carvacrol, a monoterpene, against pembrolizumab-induced renal injury in rats. Methods: Twenty-four male Wistar albino rats were assigned to four groups (n = 6): healthy control (HG), pembrolizumab alone (PZB), flunarizine+pembrolizumab (FLPZ), and carvacrol+pembrolizumab (CCPZ). Pembrolizumab was administered intraperitoneally at 5 mg/kg; flunarizine orally at 5 mg/kg and carvacrol intraperitoneally at 50 mg/kg, once daily for seven consecutive days. Renal oxidative status was assessed by measuring malondialdehyde (MDA) and total glutathione (tGSH) levels. Histopathological evaluation was performed using hematoxylin and eosin staining. Two double immunofluorescence panels were employed to assess 3,3′-dityrosine/Hepatitis A virus cellular receptor 1 (HAVCR1) and cyclooxygenase-1 (COX-1)/cyclooxygenase-2 (COX-2) expression, respectively. Results: Pembrolizumab caused pronounced oxidative stress and inflammatory responses in renal tissue, leading to a significant increase in renal MDA levels and a marked decrease in tGSH levels. These biochemical alterations were accompanied by severe tubular degeneration and increased expression of 3,3′-dityrosine, which is associated with oxidative damage, as well as HAVCR1, a marker of cellular injury, and COX-1 and COX-2, which reflect inflammatory activity. These findings indicate that pembrolizumab disrupts the renal redox balance and activates both oxidative and inflammatory pathways in kidney tissue. Flunarizine and carvacrol significantly reduced these pathological changes. Both agents attenuated oxidative stress markers and supported antioxidant defenses, thereby alleviating tissue damage. However, flunarizine demonstrated a more pronounced renoprotective effect across all evaluated parameters, restoring MDA and tGSH levels closer to physiological values and reducing tubular injury to a minimal level. Carvacrol showed a more limited but still statistically significant protective effect. Conclusions: Both agents confer significant renoprotection against pembrolizumab-induced oxidative injury; however, flunarizine exhibits a more robust protective profile, likely attributable to its capacity to attenuate calcium-mediated mitochondrial dysfunction and preserve cellular bioenergetic homeostasis.

Introduction – Patients’ psychological well-being, social functioning, and treatment adherence are all adversely impacted by the stigma associated with dialysis. For the purpose of recognizing stigma and directing nursing interventions, accurate and culturally relevant measurement instruments are crucial. The purpose of this study was to assess the validity and reliability of the Dialysis-Related Stigma Scale (DRSS) in Turkish. Methods – This methodological study was conducted at a private dialysis facility in Sivas, Turkiye, between July and September 2025. Data were collected from 92 participants using the “Patient Information Form” and the “DRSS.” Language validity, content validity, item analyses, internal consistency tests, and test–retest reliability analyses were carried out using SPSS 23.0 and AMOS 23.0 software during the scale’s validity and reliability stage. Content validity was evaluated using the Content Validity Index (CVI), construct validity was assessed using exploratory and confirmatory factor analyses, and reliability was examined using internal consistency and test–retest analyse analyses. Results – With a Cronbach’s alpha coefficient of 0.89 (McDonald’s omega = 0.894), the scale demonstrated excellent internal consistency. The scale’s content validity index of the Turkish version was 0.91. Test–retest analysis revealed a strong positive correlation (r = 0.967, p < 0.05). Discussion – Finally, the Turkish version of the DRSS was found to be a valid and reliable instrument for measuring stigma in dialysis patients.

Tas, Hakan Gokalp; Akyol, Fethi; Somunkiran, Mustafa
Background: – The goal of this prospective randomized controlled experiment was comparing the analgesic efficacy, opioid needs, and adverse effect profiles of paravertebral (PVB) block, thoracolumbar interfascial plane (TLIP) block, and retrolaminar (RLB) block in patients undergoing lumbar disc herniation surgery. Methods: – One hundred eighty-five adults (American Society of Anesthesiologists I–III) undergoing elective lumbar disc herniation surgery made up this single-center study. Among the exclusion criteria were coagulation problems, injection site infection, allergy to local anesthetics, and incapacity to provide informed consent. Patients were randomly assigned into 4 groups: control (systemic analgesia only), PVB, TLIP, and RLB (45 patients per group). An expert anesthesiologist supervised the ultrasonography during each block. The visual analog scale was used to measure postoperative pain at 0, 1, 2, 6, 12, and 24 hours after surgery. This was the primary outcome. Motor block incidence, rescue morphine consumption, and quality of recovery-40 scores at 24 hours were secondary objectives. Unfavorable incidents were noted. Analysis of variance and Kruskal–Wallis tests (P < .05) were used to examine the data. Results: – At every time point, the TLIP and RLB groups’ visual analog scale scores were significantly lower than those of the PVB and control groups (P < .001). The TLIP and RLB groups consumed considerably less rescue morphine (P < .001). The TLIP and RLB groups had the highest quality of recovery-40 scores (P < .001). The PVB group had the highest incidence of motor block (26.7%), whereas the RLB group had the lowest rate (8.9%). There were no significant adverse effects noted. Conclusion: – Compared to PVB and control group, TLIP and RLB provide better and longer postoperative analgesia, smaller opioid needs, and higher-quality recovery. Their importance in improved recovery regimens following lumbar disc herniation surgery is supported by their good safety profile.
