Ayran, Gulsun; Firat, Meryem; Küçükakça, Gulden; Cüneydioglu, Beyazit; Tahta, Kubra; Avci, Esra
Objective: The current study was semi-experimentally designed in order to identify the effect of peer education upon breast self-examination (BSE) behaviors and self-esteem among university students. Materials and methods: The study was undertaken with 100 female students who studied at Erzincan University. Peer educators were recruited from the 4th year students. The data were collected with a questionnaire form, BSE skill form and Rosenberg Self-esteem Scale in two phases one month before and after the BSE peer education. For the data assessment; percentage distributions, frequency, mean, standard deviation and Simple T test were employed. Results: The mean age of the participant students was 20.45 +/- 1.67 year and all of them were single. It was found that during the first data collection phase, only 16 % of the students performed BSE while during the final data collection phase, the rate of the students performing BSE rose to 77 %. During the first data collection phase, students received a mean score of 2.36 +/- 4.13 from BSE skill form while during the last data collection phase they had a mean score of 10.70 +/- 3.40 from BSE skill form. When the scores obtained from Rosenberg Self-esteem Scale by the students were examined, it was seen that they received a mean score of 1.20 +/- 1.34 during the first data collection phase while they had a mean score of 0.84 +/- 1.07 during the final data collection phase. Conclusion: Although short-term feedback was obtained, it was noted that students' BSE knowledge and skills increased considerably.

Objective: Gastric cancer is the fifth most commen neoplasm in terms of incidence in men and women in Europa. Male to female ratio was determined as 1.6:1. Gastric cancer is the second most commen neoplasm in men and third most commen neoplasm in female in deaths due to cancer in our country. Prognosis of gastric cancer is generally reserved. The low survival rate is due to the delay in diagnosis, most cases being diagnosed in an advanced stage. Material and Methods: Between 1995 and 2009, 204 gastric cancer patients were retrospectively evaluated who was treated surgically in Faculty of Medicine, Suleyman Demirel University. It was observed that tumors of the 131 patients were curatively resected. The great portion (80%) of these patients were in locally advanced stages (stage IIIa, IIIb and IV). Age, gender, symptoms, tumor localisation, T, N, M, stage, tumor differantiation, histologic type, occurence of vascular and perineural invasion, surgery type, resection type, dissection type, count of dissected lymph nodes, metastatic lymph nodes, ratio of metastatic lymph nodes, complications, morbidity and mortality, adjuvant chemotherapy schedules, adjuvant radiotherapy and other adjuvant therapies, and surviaval of these patients were analyzed. For detecting the effects of these factors upon survival, logictic regression analysis was performed. Survival times were analyzed with Kaplan-Meier method. Results: It was seen that surgical morbidity was 15.2%, and mortality was 7.6% in curatively resected 131 patients. Stage and the ratio of metastatic lymph nodes were evaluated as significantly important in survival (p< 0.05). Survival ratio for 5 years were estimated a s 79.1%, 78.3%, 61.8%, 46%, 24.8% and 25.8% according to the stages Ia, Ib, II, IIIa, IIIb and IV respectively. Conclusion: In conclusion, it could be considered that although the great portion of gastric cancer patients were locally advanced cases, the survival rates were increased with radical resections.

Objective: Diabetes is a serious health problem worldwide that leads to high morbidity and mortality rates. The current study was conducted to determine the knowledge levels and attitudes of diabetic patients about their disease. Methods: The population of this cross-sectional study comprised type 1 and type 2 diabetic patients who presented to the outpatient internal disease polyclinic of a public hospital. No sampling was done, and 335 diabetic patients who volunteered to participate and were able to communicate were included. Data were gathered using a personal information form that contained questions on the descriptive characteristics of the patients and disease features, using the Diabetes Attitude Scale (DAS), and using the face-to-face interview technique. To analyze data, percentages and arithmetic means were calculated and Mann-Whitney U and Kruskal-Wallis tests were performed. Results: When the knowledge levels of the patients about the disease were investigated, 70.4% of the patients did not know the organ that caused the disease and 37% did not know how often they should wash and control their foot. The mean DAS score of the patients was 3.58 +/- 0.30, and the patients developed a positive attitude about their disease. Further, there were significant differences between regular medicine use and the need for special training to provide diabetes care, blood glucose control and complications, patient autonomy, team care, and the DAS total score. Conclusion: The knowledge levels of the patients about the disease were not satisfactory, but the patients developed a positive attitude about the disease.

The prevalence of congenital inferior vena cava anomalies is difficultto pinpoint and is estimated to occur in 0.05-8.7% of the population. These abnormalities are often asymptomatic and are incidentally identified in patients undergoing imaging investigations for other reasons. Inferior vena cava agenesis maybe a risk factor for deep venous thrombosis due to inadequate venous drainage. We describe thecase of an 84-year-old female who was hospitalized due to unprovoked deep venous thrombosis and inferior vena cava agenesiswas foundin a further investigation.

Aim of the study: The present study aims to estimate the prevalence and distribution of HPV genotypes and identify related risk factors among Turkish women. Material and methods: 11 624 Turkish women attending our gynaecological clinic and expressing a desire for access to cervical cancer screening were assessed during the years 2014-2016. Cervical specimens were collected and transported using the HC2 HPV DNA Collection Device (consisting of a cervical brush and digene Specimen Transport Medium). Results: Among these 11 624 individuals, positive HPV test results were obtained for 325 (2.79%), and negative results were observed for 11 299 (97.2%). The vast majority of patients were between the 3rd and 5th decades and the mean age of the patients was 44 ±9.12 (range 27-66). Among the HPV-positive women, 205 were positive for a single HPV type (205/325 = 63.1% of HPV infections; 205/11624 = 1.76% of all samples) and 120 were positive for multiple types (120/325 = 36.9% of HPV infections; 120/11624 = 1.03% of all samples). The four most prevalent high-risk types were HPV 16, 31, 51 and 52, with frequencies of 11.25%, 7.83%, 6.06% and 3.16%, respectively. Conclusions: There appears to be geographic variation in the distribution of HPV genotypes. In this study, the four most prevalent high-risk types were HPV 16, 31, 51 and 52, with frequencies of 11.25%, 7.83%, 6.06% and 3.16%, respectively.

Camurcu, Yalkin; Cobden, Adem; Sofu, Hakan; Saklavci, Nuh; Kis, Mehmet
Background Primary hemiarthroplasty is proposed for the treatment of unstable intertrochanteric fractures in elderly patients with the advantages of early mobilization, acceptable functional results, and lower failure rates. The 1-year mortality rates demonstrated high variance in the literature, whereas, the factors related to 1-year mortality were not widely investigated. The main purpose of the present study was to determine predictive factors related to 1-year mortality after primary cemented calcar-replacement bipolar hemiarthroplasty performed for unstable intertrochanteric fracture. Methods One hundred six patients with the mean age of 80.7 years were included in this retrospective study. Age, gender, body mass index, comorbid diseases, American Society of Anesthesiologists score, total hospitalization time, time from injury to surgery, operation time, estimated blood loss, postoperative mobilization time, and decrease in Koval ambulatory categories were evaluated. Univariate and multivariate analyses were performed to determine major predictors of 1-year mortality. The Kaplan-Meier survival analysis was used to construct the cumulative survival rate. Results Three or more American Society of Anesthesiologists scores, presence of ≥3 comorbid diseases, and postoperative mobilization time of ≥2 days were significantly correlated with 1-year mortality. Presence of ≥3 comorbid systemic diseases was identified as the major predictive factor for 1-year mortality. The overall 5-year cumulative survival rate was 5.6%. Conclusion Having three or more comorbid systemic diseases has been detected as the major determinant of 1-year mortality after primary cemented calcar-replacement bipolar hemiarthroplasty performed for unstable intertrochanteric fracture in elderly patients.

Objectives: The aim of this study was to assess the possible role of HPV in the development of prostate cancer (PCa) and investigate the distribution of the p53 codon 72 polymorphism in PCa in a Turkish population. Materials and methods: A total of 96 tissues, which had been obtained using a radical surgery method, formalin-fixed and parafin-embedded, were used in this study. The study group consisted of 60 PCa tissues (open radical prostatectomy) and the control group contained 36 benign prostatic hyperplasia tissues (BPH) (transvesical open prostatectomy). The presence of HPV and the p53 codon 72 polymorphism was investigated in both groups using real-time PCR and pyrosequencing. Results: The results of the real-time PCR showed no HPV DNA in any of the 36 BPH tissue samples. HPV-DNA was positive in only 1 of the 60 PCa samples (1.7%). The HPV type of this sample was identified as HPV-57. The distribution of the three genotypes, Arg/Arg, Arg/Pro and Pro/Pro was found to be 45.6, 45.6, and 8.8% in the PCa group and 57.1%, 34.3% and 8.6% in the control group, respectively. Compared with the control group, patients with PCa had a higher frequency of the Arg/Pro genotype and Proline allele (odds ratio (OR)=1.67, 95% confidence interval (CI)=0.68-4.09, p=0.044; OR=1.13, 95% CI=0.76-1.68, p=0.021, respectively). Conclusions: The results of the study do not support the hyphothesis that prostate cancer is associated with HPV infection but indicated that Proline allele can be a risk factor in the development of PCa in the Turkish population.

Continuing our work on the sources of natural bioactive compounds, we evaluated the antimicrobial and antioxidant activities of Nepeta trachonitica as well as its major phenolic content using the high-performance liquid chromatography-mass spectrometry/mass spectrometry (HPLC-MS/MS) technique. For antioxidant activity, ferric reducing antioxidant power (FRAP) and cupric ion reducing antioxidant capacity (CUPRAC) methods were performed to measure the reducing power and 1,1-diphenyl-2-picrylhydrazyl (DPPH) assay was employed to evaluate the radical scavenging activity of the sample. For antimicrobial activity, three Gram-positive and four Gram-negative microbial species as well as three fungi species were tested. N. trachonitica appeared to have reasonable antioxidant activity and decent antimicrobial activity as indicated by the inhibition of the organisms’ growth. The most susceptible species were Bacillus subtilis ATCC 6633 and Escherichia coli ATCC 11229 among the organisms tested. Ethanol extract of the plant has the highest effect on Saccharomyces cerevisiae but no effect on Yarrowia lipolytica. The HPLC-MS/MS analysis showed that at least 11 major phenolic compounds of N. trachonitica exist, the major ones being rosmarinic acid, chlorogenic acid and quinic acid. The obtained results suggest that N. trachonitica could be a promising source for food and nutraceutical industries because of its antimicrobial and antioxidant properties and phenolic compounds.

Study Design: Retrospective study (level of evidence: level 3). Purpose: The purpose of this study was to evaluate the clinical and radiological results of the posterior-only approach with pedicle screws for the treatment of Scheuermann's kyphosis (SK). Overview of Literature: The correction of SK with instrumentation can be performed using posterior-only or combined anteriorposterior procedures. With the use of all-pedicle screw constructs in spine surgery, the posterior-only approach has become a popular option for the definitive treatment of SK. In a nationwide study involving 2,796 patients, a trend toward posterior-only fusion with lower complication rates was reported. Methods: We retrospectively reviewed the data of patients who underwent posterior-only correction for SK between January 2005 and May 2013. Patients with a definite diagnosis of SK who fulfilled the minimum follow-up criterion of 24 months were included. The thoracic kyphosis (T5-T12), lumbar lordosis (L1-S1), and thoracolumbar junction (T10-L2) angles were measured from preoperative, postoperative, and last control radiographs. Sagittal balance, thoracic length, thoracic diameter, Voutsinas index and the sacral slope, pelvic tilt, proximal junction kyphosis, and distal junction kyphosis angles were also measured. Results: Forty-five patients underwent surgery for the treatment of SK between 2005 and 2013. After applying the exclusion criteria, 20 patients (18 males and 2 females) with a mean age of 19 years were included. The mean thoracic kyphosis angle was 79.8 degrees preoperatively, 44.6 degrees postoperatively, and 44.9 degrees at the last control. There were statistically significant differences between preoperative and postoperative values in the thoracic kyphosis and lumbar lordosis angles, thoracic length, thoracic diameter, and Voutsinas index (p < 0.05). Conclusions: The clinical and radiological results of the current study suggest that posterior-only fusion is an efficient technique for the treatment of SK.

Introduction: Chronic inflammation is a major risk factor in the pathogenesis of cardiovascular disease in end-stage renal disease (ESRD) patients. Epicardial adipose tissue (EAT) is the true visceral fat depot of the heart. The relationship between coronary artery disease and EAT was shown in healthy subjects and ESRD patients. In the present study we aimed to investigate the relationship between EAT and inflammation parameters including neutrophil-to-lymphocyte ratio (NLR) in hemodialysis (HD) patients. Material and methods: Forty-three HD patients (25 females, 18 males; mean age: 64.1 ± 11.9 years) receiving HD and 30 healthy subjects (15 females, 15 males; mean age: 59.1 ±10.8 years) were enrolled in the study. Epicardial adipose tissue measurements were performed by echocardiography. Results: Neutrophil-to-lymphocyte ratio levels were significantly higher in HD patients than in the healthy control group. Hemodialysis patients were separated into two groups according to their median value of NLR (group 1, NLR < 3.07 (n = 21) and group 2, NLR . 3.07 (n = 22)). Group 2 patients had significantly higher EAT, C-reactive protein and ferritin levels, while albumin levels were significantly lower in this group. In the bivariate correlation analysis, EAT was positively correlated with NLR (r = 0.600, p < 0.001) and ferritin (r = 0.485, p = 0.001) levels. Conclusions: Neutrophil-to-lymphocyte ratio was found to be an independent predictor of EAT in HD patients (odds ratio = 3.178; p = 0.008). We concluded that this relationship might be attributed to increased inflammation in uremic patients.

Background: In this study, it was aimed to investigate the relationship between diabetes and breast cancer and the detection of enzymes and ornithine levels in polyamine synthesis pathway in diabetes, breast cancer and diabetic breast cancer patients. Methods: Ornithine, arginine decarboxylase, ornithine decarboxylase and agmatinase levels have been measured in serum of all groups. Ornithine levels were measured spectrophotometrically. Arginine decarboxylase, ornithine decarboxylase and agmatinase levels were determined by ELISA kits. Results: Except for the diabetic group, the levels of enzymes in the polyamine synthesis pathway were increased in all and statistically significant (P < 0.05). The increase in the levels of agmatinase was very important among the enzymes (P < 0.001). Conclusions: Decreased levels of polyamine synthase enzymes in diabetes mellitus were found to be increased patients with breast cancer. Whether and how diabetes-based breast cancer development relates to increase activity of enzymes responsible for polyamine synthesis requires further mechanistic and prospective monitoring studies in larger patient cohorts.

Background. Preconditioning is one of the most powerful mechanisms preventing the myocardial ischemic damage that occurs during coronary artery bypass grafting. Objectives. We aimed to investigate the effects of different propofol and/or desflurane administration protocols in terms of the prevention of ischaemia-reperfusion damage. Material and methods. Ninety patients, aged > 18 years, American Society of Anesthesiologists (ASA) category III, scheduled to undergo primary elective coronary artery bypass grafting (CABG), were included in the study. During maintenance, the patients in group 1 (n = 30) received a propofol infusion (5-6 mg/kg/h) combined with a fentanyl infusion (3-5 mcg/kg/h); the patients in group 2 (n = 30) also received a propofol infusion (5-6 mg/kg/h) combined with a fentanyl infusion (3-5 mcg/kg/h), but they were also given 6% desflurane inhalation for 15 min both before cross-clamping of the aorta and after removal of the clamp; the patients in group 3 (n = 30) received a propofol infusion (2-3 mg/kg/h) combined with a fentanyl infusion (3-5 mcg/kg/h) and received the continuous 6% desflurane inhalation. Blood samples were drawn in the preoperative period (S1), during cardiopulmonary bypass, before cross-clamping the aorta (S2), after removal of the cross-clamp (S3) and 24 h after the operation (S4). Results. All groups were similar in terms of age and BMI (p > 0.05). TNF-á levels were higher at S3 compared to S1, S2 and S4 (p > 0.001). The TNF-á levels at S4 were lower in group 3 than those in group 1 and group 2 (p < 0.05). In all groups, h-FABP levels showed an increase in S3 but were significantly lower at S4 (p < 0.05). In group 3, h-FABP levels at S2 and S3 were significantly lower than those in group 1 (p < 0.05). There was a moderate correlation between h-FABP and TNF-á levels (Spearman's rho = 0.472, p < 0.001). Conclusions. On the basis of the measurement of h-FABP and TNF-á, low-dose propofol and continuous desflurane inhalation provide more effective preconditioning than propofol alone or a short course of desflurane in patients undergoing CABG.

This prospective study investigated the effects of dexamethasone and low-dose propofol infusion on postoperative nausea and vomiting in 90 patients undergoing tympanoplasty. The dexamethasone group (Group D) received 8 mg dexamethasone and the control group (Group C) 2 ml saline 15 min before the end of surgery, while the propofol group (Group P) received ıntrave-nous propofol infusion at a dose of 20 mcg/kg/min throughout surgery following induction of anaesthesia. Incidence rates and severity of nausea-vomiting and antiemetic requirements were recorded throughout the first postoperative 24 hours. Between hours 0 and 2, any incidence of postoperative nausea and vomiting was not observed in 23 (76.7%) patients in Group P, 17 (56.7%) in Group D and 8 (26.7%) in Group C. Differences were observed between the groups in hours 0-2 and 2-8 in terms of verbal descriptive scale values for postoperative nausea and vomiting (p<0.01 and p=0.004, respectively). Total incidence rates of postoperative nausea and vomiting at hours 2-8 h were 10.0% (n=3) in Group P, 36.7% (n=11) in Group D and 53.3% (n=16) in Group C, the difference being statistically significant in favour of Group P (p=0.032). At 0-24. hrs, the number of patients vomiting, despite treatment, were lower in Groups P and D (n=4, 13.3% and n=5, 16.7%, respectively) compared to Group C (n=14, 46.7%) (p=0.005). Antiemetic use was higher in Group C than in Groups D and P (p=0.001). Intraoperative low-dose propofol infusion is as effective as dexamethasone in reducing the incidence and severity of postoperative nausea and vomiting, as well as reducing post-tympanoplasty antiemetic requirements.

Objective: The incidence of varicocele is approximately 15% in adolecent men. Early treatment with varicocele is aimed to resolve testicular hypotrophy and ensure catch-up growth. The aim of this study was to evaluate ipsilateral catch-up growth rates relative to contralateral testicular growth in adolescents with varicocele undergoing microsurgical subinguinal varicocelectomy. Material and methods: Fifty adolescents with unilateral grade 2-3 varicoceles were included in the study. All patients underwent microsurgical subinguinal left varicocelectomies performed by the same experienced surgeon. All patients were evaluated clinically and using orchidometric measurements to define the grade of varicocele and testicular volume at presentation and follow-up. The number of internal and external spermatic veins, testicular arteries and lymphatic vessels preserved during the subinguinal microsurgical varicocelectomy were recorded. The mean follow-up period was 26 months (range 6-48 months). Results: At presentation, mean patient age was 12.9±2.1 years. Mean testicular preoperative volumes were 7.1±4.3 mL for the right and 5.4±3.4 mL for the left testis. There were significant differences between mean volumes of the right and left testis (p=0.002). At the final postoperative follow-up visit, mean testicular volumes were 10.8±5.1 mL (range 3-25) for the right and 9.9±4.3 mL (range 2-20) for the left, and the difference between the right and left testicular volumes was insignificant (p=0.47). In our series, catch-up growth was observed in 70% (35/50) of our patients. Conclusion: Adolescent varicocelectomy is associated with a higher percentage of patients showing testicular catch-up growth. In our study, similarly to the available literature the catch-up growth rate was found as 70% and observed to have positive effects of adolescent varicocelectomy on testicular growth.

Photobiomodulation (PBM) and photodynamic therapy (PDT) are two major methods, which use light in medicine and dentistry. PBM uses low-level laser light to induce cell proliferation and activity. In contrast, PDT use laser light combined with a photosensitizer (PS) to cause cell death. Due to similar, not fully understood mechanisms and biphasic response of light, unexpected and complex outcomes may be observed. In the present study, the effect of 635 nm laser light, with power density 50 mW/cm2, at three different energy densities (0.5, 1, and 2 J/cm2 which last 10, 20, and 40 s, respectively) mediated by methylene blue (MB) on the human osteoblast cell line (ATCC-CRL-11372, Rockville, MD, USA) was investigated. Cell viability (MTT assay and acridine orange/propidium iodide staining) and proliferation (Alamar Blue assay) were assessed at 24, 48, and 72 h post irradiation. Alkaline phosphatase (ALP) activity, mineralization (Alizarin Red staining) and gene expressions (RT-PCR analysis) were analyzed at 7th and 14th days after treatment. Five groups were formed as the control group (no MB, no irradiation), MB (only 0.05 μM MB), MB + 0.5 J/cm2, MB + 1 J/cm2, and MB + 2 J/cm2. Cell viability was decreased at 72 h (ANOVA; p < 0.05) for MB + 0.5 J/cm2, MB + 1 J/cm2, and MB + 2 J/cm2 groups. Although proliferation does not seem to be effected by MB-mediated laser application, osteo-anabolic activity is altered. ALP activity was significantly increased at day 7 (ANOVA; p < 0.05) for MB-combined laser groups; on the other hand, mineralization was significantly decreased (ANOVA; p < 0.05) in all treatment groups. Alkaline phosphatase and collagen-I expressions were upregulated in MB + 2 J/cm2 group at 7th and 14th days, respectively. These results may contribute to the low-dose PDT researches and understanding PBM effects on osteoblast behavior but further studies are needed since inappropriate conditions may lead to undesirable results for both therapies.
